Disorders of the anterior pituitary gland and the posterior pituitary gland are usually associated with excess or deficient hormone synthesis and secretion. Tumors of the anterior pituitary gland are mutually common contracting a small percentage of all brain tumors. In autopsies done, when the cause of death is not limited to any group of diagnoses, almost one third of those examined have small pituitary tumors.

Predisposing factors are uncommon. Persons with untreated hypofunction of end organs such as the gonads, adrenals or thyroid may be at risk. They may develop hyperplasia initially and then autonomously functioning tissue that secretes the stimulating hormone for the hypofunctional gland. Long term malnutrition, particularly if it is severe, can cause excess growth hormone secretion. These events are logical exit of the negative feedback system, in which the pituitary gland attempts in vain to restore homeostasis by ever-increasing hormone production.

Malignant tumor symptoms include neurological findings like headache caused by pressure on the dura. These nonspecific headings are inconsistent in regard to location and degree of relief from analgesics. It is preferred approach for most anterior pituitary tumors because it eliminates manipulation of the brain substance that occurs with previously used frontal craniotomy approach and there is no visible scar.

The incision is made at the base of the upper lip where it joins the gum. After the floor of the sella tursica is exposed, the dura mater over the pituitary gland is opened and the tumor is removed. A small piece of fascia and muscle obtained from the thigh of the patient is used to secure a tight seal at the floor of the sella tursica and prevent cerebrospinal fluid.

The sphenoid sinus is not packed. The nostrils are packed with vaseline gauze to facilitate proper reapproximation of the nasal mucosa. Nasopharyngeal airways may be placed prior to packing to allow the patient to breathe through the nose during the immediate postoperative period. The nasal packing is usually removed 2 days after surgery.

Alterations in pituitary function after pituitary surgery will vary according to the preoperative hormone dysfunctions of the patient, amount of pituitary tissue damaged or removed, and extent of postoperative edema in the region of the hypothalamus and the pituitary. Cortisol may need to be replaced at levels associated with major stressors during surgery. Surgery is an effective pituitary tumor treatment. It is one of the methods that patients and the medical team decide to do.

Considerations for treatment

It is highly recommended that your physician provide a written recommendation for massage, and your therapist probably won’t agree to treat you until your doctor has given the green light.

In all likelihood, you will also be seeing a physical (physio) therapist. It’s a good idea to provide written permission for your massage therapist to collaborate with your other health care professionals to ensure the best possible care and treatment outcome.

Before your massage therapist begins to treat you, he or she will want to verify the following information:

  • your physician has approved massage therapy
  • are you on any type of medication? (anti-inflammatory, analgesic, anti-coagulant, antibiotics).
  • what is your general health – for example, consider age, fitness prior to injury, nutrition (your MT may refer you to a nutritionist, as proper nutrition impacts the healing process), lifestyle (i.e. do you smoke?)
  • are there any open wounds which must be addressed? (i.e. with a compound fracture)
  • Will treating the affected limb with massage therapy stress and disrupt the healing of bone, muscle, skin or nerve tissue?
  • is there any neurological or vascular impairment in the area which prevents the use of certain massage techniques or modalities?
  • do you have any plates, wires or pins? If so, hydrotherapy over the site will be contraindicated.
  • the most important consideration is to avoid stressing and interfering with healing tissue. Any pressure or traction applied to the affected bone is contraindicated.
  • have other types of injuries been sustained, such as sprains or contusions? (in all likelihood, there will be other injuries)
  • muscle atrophy from disuse will be present when the cast is removed. Provided that the injury is not an avulsion fracture, the development of atrophy can be reduced by the use of isometric contractions at the appropriate time with cast on. If an avulsion fracture is present, avoid isometric contractions, as this may disrupt the healing bone.
  • tissue under the cast will be fragile when the cast is removed, so extra care must be taken when massaging this tissue

As treatment progresses and the injury heals, issues such as tissue atrophy, altered biomechanics and altered proprioception must be taken into account. For this reason, there are different considerations for treatment with cast on and cast off.

Types of fractures

There are several basic types of fractures:

1. Simple – there is no broken skin, and the break is clean. Also known as a “closed” fracture.

2. Compound – the broken bone damages surrounding tissue and skin by piercing it. There is a greater risk of infection in this type of break due to breaking of the skin and protrusion of the injured bone.

3. Comminuted – a fracture that is in several pieces – common among a population with more brittle bones (i.e. elderly)

4. Compression – a fracture where the bone is crushed

5. Depression – a fracture where the bone is crushed and pressed inward

6. Impacted – bony surfaces are forced into each other (impacted)

7. Spiral – an break with ragged edges that result from twisting

8. Greenstick – an incomplete break that is common in children, as bones are more flexible

9. Stress fracture – a.k.a. hairline fracture, is a tiny crack in the bone and may not be visible with an x-ray

10. Avulsion – this occurs when a ligament or tendon pulls away a chunk of bone to which it is attached.

Fractures and breaks are repaired by “reduction” – when the bony ends are placed back together. A closed reduction is done manually by a physician, whereas an open reduction involves surgery, and possibly the placement of pins or wires to hold the bony surfaces together.

It goes without saying that an injury as serious as a fracture or break will require treatment from a therapist who really knows his or her stuff and understands the healing process. Personally, I would ask my physical therapist if he or she could refer me to a qualified and experienced massage therapist, and then ensure that I give them both permission to communicate with each other so that my treatment plan would address all of my needs and concerns.

Otitis Media And Ear Pain Relief

Inflammation of the inner ear (OM) is extremely common in children as well as affecting quite a few adults. Recurrent, protracted ear inflammation causes severe pain, restlessness and muffled hearing, which can hamper development and prejudice learning. Ear infection is one of the main reasons for visiting the doctor or the emergency room.
Blockage of the Eustachian tube (OT) which links the middle ear cavity to the oral cavity can be a main cause of inflammation or a secondary factor – caused by inflammation and swelling of the mucus membrane of the ear and blocking the Eustachian canal.

Opening the Eustachian tube is an important and even essential step towards healing the inflammation. Blockage of the canal prevails drainage of the fluids secreted by the mucosal membrane of the middle ear and predictions equalization of the atmospheric pressure on either side of the eardrum. This increases the pressure inside the ear cavity and pushes the eardrum outwards, leading to pain, muffled hearing, and occasionally problems with balance. Moreover, increased pressure in the middle ear cavity prevents the absorption of medication into the region via local osmosis (eardrops) or systemically (via the blood).

Prevailing medical treatment – until now, treatment has involved the use of steroids and pseudo-epinephrine to reduce edema, with antibiotic cover for primary bacterial infection or secondary complication. If medical treatment failures and inflammation recurs constantly extended antibiotic treatment, the usual practice is to perforate the tympanic membrane and ensure that it remains open by inserting tiny drainage tubes. This allows the contents of the middle ear cavity to drain out, equalizing pressure, improving hearing, stopping pain, and permitting medication to enter locally and heal.

The newly invented device is called the EarDoc (Figure 1). EarDoc is a small, low cost electrical device for treating and opening the Eustachian tube. It works by producing vibrations at a sub-sonic frequency. The instrument head is specifically designed so that when positioned behind and resting on the base of the ear, the bulge on the head of the device lies against the mastoid bone and the pointed front section rests against the base of the ear.

The front of the head of the device causes the cartilage of the ear base to vibrate along the outer ear canal to the tympanic membrane. The vibrating membrane produces tiny pressure waves affecting the air and pressurized fluids of the middle ear cavity. The previously constant pressure inside the middle ear cavity is converted into cyclical waves, which acting as shock waves exert pressure on the walls of the narrowed or blocked Eustachian tube. In addition, the vibrations from the head of the device via the mastoid bone to the wall of the middle ear cavity produce vibrations in the swollen mucus membrane of the middle ear canal and the Eustachian tube, reducing edema. This combined action tremendously improves the possibility of opening the canal and enabling trapped air and fluids from the middle ear cavity to drain into the mouth.

Adjusting the frequency helps to improve the efficiency of the device in several ways: the optimum operating frequency is different for each individual depending on the size of the middle ear cavity and the Eustachian tube, which naturally vary. Frequency adjustment also enhances the oscillation of the mucosal membrane. It is recommended that the patient should gradually adjust the frequency to cover the entire frequency range. EarDoc should be used for 3-5 minutes approximately six times a day when the canal is narrowed or blocked. This improves the likelihood of opening the canal and ensuring it positions open, and provides instant pain relief, improved absorption of medication, and healing of the inflammatory.

Air travel and diving – Blockage of the Eustachian tube is a commonplace consequence in air travel and underwater diving. Fluctuating differences in atmospheric air pressure on both sides of the tympanic membrane induced reduced pressure in the middle ear cavity, swelling of the mucus membrane cells in the inner ear, and increased fluid secretion. This produces a vicious circle in which the greater the difference in pressure, the narrower the Eustachian tube becomes and the less effect it can drain fluid and equalize air pressure. Impaired functioning of the tube causes insufficiency in the canal and blockage. Inflammation of the pharynx and upper respiratory tracts leading to early boiling of the mucosal membrane exacerbate the phenomenon and risk of blockage. Use of EarDoc during flights and following diving sessions can reduce pain and produce healing as described above.

Sinus inflammation – like the middle ear cavity, when the sinus drain is blocked due to swelling or inflammation of the sinus cavity mucus membrane, EarDoc can help by resting it against the bone adjacent to the sinus in order to open the closed opening and allow drainage and healing.

All About Sleep Apnea & How it Affects You

Sleep apnea can be a dangerous and scary problem. The scariest thing about it is that you are not conscious for most of it. Anybody should know about the causes, symptoms, and treatments for sleep apnea.

Sleep apnea is episodes of not breathing while you are sleeping. These episodes usually last up to 10 seconds and can happen over and over every hour of the night.

Causes

The Secret to Chicken Pox Relief

So, there I was with two children covered in chicken pox. I felt grateful that they were both getting them over with while they were young, and nervous about my ability to treat them effectively on my own, without over the counter remedies.

This was definitely the most serious illness I had yet faced as an herbalist parent. How I wished for a chicken pox treatment page like the one I'm about to write, a page from another parent saying here's what I did, and it worked.

Luckily, my herbalist husband was on hand to remind me to trust the plants and to use the plants we know.

Chicken pox, we discovered, is a herpes related virus, so we chose St. Louis. John's Wort, as our primary healer, since we know it to be effective against herpes.

We brewed up teas for their baths – concoctions of St. John's Wort (dried leaves, stem and flowers), dried burdock root, and dried licorice root.

To make the concoctions we would put one large handful of each into a gallon of boiling water and simmer for 20 minutes.

Then strain out the herbs and add the tea to the bath water.

I would also fill a thin cloth bag with oats, calendula flowers and baking soda and put that into the bath. The kids would squeeze the oat milk onto their skin and even rub the bags over the irritated areas. These baths did wonders to relate the itching and calm the children. What a great herbal chicken pox treatment!

We also had the children drink the St. Paul. John's Wort, burdock and licorice tea as much as possible while they were broken out.

Still, we had times when the itching and burning was very uncomfortable and made sleeping especially difficult.

At these times, St. Johns Wort oil offered some relief. We did not come up with the perfect anti-itch formula, but have since learned that tea tree oil can provide amazing relief, and that chickweed poultices are effective for cooling and calming the itch.

We did try a mixture of baking soda and water and found it not only ineffective, but irritating to the children. This is actually a popular chicken pox treatment for some reason.

Hailey had the easier time with the chicken pox. At one year old, she did not get nearly as many as her six year old brother, and for the first few days was happy and playful and still practicing her walking.

She did get quite a few pox on her butt and genital area, probably because of the heat from her diaper. Then, just about the time that Rowan was beginning to feel better, Hailey got a fever and we not one of the pox on her leg where her diaper wrap was rubbing was very red and swollen.

We took her diaper off and began using poultices of calendula flowers and plantain leaves (crushed in a mortar and pestle with a little bit of water) directly on the pox. The swelling and redness went away by the end of the day and her fever was gone by morning.

We then used these poultices (attached with gauze and tape) on any of the more angry looking pox until they were cured.

Another worrisome occurrence that happened for us, was that Rowan sat up in bed near the middle of the night with his eyes wide open, yelling nonsense. It was a very unsettling experience for me as a mother, but after doing some research on the web, I found that it is not uncommon during chicken pox. I simply calmed him and he went back to sleep and woke a few minutes later with no memory of the episode.

The worst of the virus had passed within a period of about four days, and they were intense days for me. My suggestion to you is that if you call on whatever support system you have during this illness.

The First Signs of Chicken Pox

The chicken pox is an infectious disease caused by the varicella zoster virus (VZV), which is related to the herpes family. It is extremely common among children, but can also occur in adults as well.

It usually takes about 10 to 21 days before the first signs of chicken pox to appear in someone after contact with another person that is suffering from this contagious virus (also known as the incubation period).

What are the first signs of chicken pox?

The first signs of chicken pox include symptoms such as tiredness, low-grade fever, and a loss of appetite. These are usually the earliest signs of the disease that occurs, which about 1-2 days later is next followed by a rash or red spots. However, the first signs of chicken pox do not always occur before the rash developments.

When the rash appears, it most often appears on the face, scalp and trunk first. Then it is usually followed by about 50-500 blisters that consume the entire body. Although many of these factors are determined by age (for instance, adults get it much worse than children), as well as how your immune system is functioning.

Eye myopia, or near-sightedness, as it is often called, is one of the most common and annoying eye ailments.

It is due to muscular imbalance. The myopic eyeball is elongated by pressure of the two oblique muscles which bind the eyeball about the middle. Because of the tightness of these muscles, the parallel rays of light are brought to a focus at a point in front of the retina. The result is blurred, distorted vision when trying to see a distant object. Myopia, in other words, involves a strain to see at the far point and it is our task to stretch the vision.

When the eyes are fitted with lenses, the light rays focus on the retina as in normal vision. But-since the shape of the eyeball remains unchanged-the cause of the difficulty is not remedied. On the contrary, the eyeball becomes more elongated as time goes on, and lenses must be made stronger and stronger to counteract the increasing myopia of the eye.

Myopia usually appears in school children between the ages of eight and fifteen. The reason appears to be that during puberty the child's physical and nervous system is undergoing such drastic changes that the anxieties and fears often accomplicing the confinement and competition of the school room create an entirely new series of strains.

These strains, in turn, are potent factors in the development of myopia and other visual defects. Too often the well-intentioned parents hustle the child off to the ophthalmologist who fits him with glasses. It is always an unpleasant sight to observe a child's face behind spectacles. Automatically, they cut him off from many games and forms of exercise which the growing body needs. But away from that, the vision generally gets worse and the glasses are increased in strength until, some ten or fifteen years later, the patient's eyes have lost much of their power of accommodation. Smithfield Foods, the nation’s largest hog producer and pork processor is a Corporate Hog Baron on steroids and being the dominate hand of the pork industry has the clout of the sow pork lobby to suckle the pork barrel politics of Washington DC, they are able to do just about whatever they want.

There is no doubt that the boar’s share of the pork meat produced in La Gloria Mexico is bound for markets in the USA, and Mexico is a runt Third World piglet of corporate lactating greed. Developing a LEAN culture with a LEAN Management system developed in parallel with the LEAN process system ensures that the roles, processes, standards, and procedures are documented and followed. Although we are not discussing physics, the law of entropy applies. It states "Organized systems tend to move toward disorganization". LEAN involves the adoption of new processes, work habits, and the use of new tools. Everyone must learn new habits and, as psychologists say, extinguish the old habits. It is so easy to fall back into old habits. They lurk in the back of our mind waiting for the opportunity to resurface. This propensity of old habits to re-occurs is the reason why a management system must be developed and implemented in parallel with the LEAN process system.

A LEAN management system will provide the tools and processes to maintain and support the LEAN process system. It will sustain the integrity of your system and ensure you realize the benefits of LEAN. A management system will close the loop on processes and allow employees to make continuous improvements. A LEAN production system and a LEAN management system are interrelated.

For example, a company instituted LEAN and standardized processes in a major area. They developed a flow for the work and cross trained employees and immediate supervisors so that flow could be maintained. The implementation rejected in less wait time for customers, reduction in time and costs, and an increase in quality. But then, employees in the LEAN area were transferred to new areas, management changed their focus to the latest new technology, and new employees were not cross trained. LEAN tools to document abnormalities stacked up and were not reviewed and no one acted on them. The result was a return to old habits and business as usual, which affected customer relations and profits. When management tried to reinstate LEAN, there was resistance and many felt it was a failure because it did not last, so why try again?

Actually, LEAN had worked but it was not sustained because there was not a parallel management system developed. Another contributing factor was the lack of discipline in completing, reviewing, and acting upon LEAN tools. LEAN is not a one-time fix but a journey to continuous improvement.

The tools in LEAN make sense and are easy to use. The challenge is to actually employ the discipline to complete the charts, interpret data, and make the improvements. Let's take a small scale example of what can happen when the tools are not used and the processes in not followed. In a manufacturing plant, an employee on the line had a call for the production of a number of units of a product not produced every day. The employee was considered very knowledgeable about the product and had a standard of work document. His team leader also had a standard of work document for the inspection of the process. The employee took a quick look at the steps in the standard of work and began producing the product. Unfortunately, he skipped an important step that ensured quality and eliminated customer complaints. This would have been noticed on the first unit or at least in the first hour of production if the team leader had inspected as required in her standard of work.
