When I was in my pediatric rotation, I remember seeing a young child for a well child visit, and this was actually my first rotation as a physician assistant student. When the physician came in, the child clearly responded adversely to the pediatrician who was a bit older with a beard, and the mom said that he looked kind of like his father. The child was really kind of frightful of the physician. When he looked over the child, the child had a number of bruises especially on his lower legs. I believed this child was being abused by his father. The doctor sent him off for blood work, and I was quite upset with the physician which I talked about wondering how he could not see how apparent abuse was. Well, when the blood work came back, this child had idiopathic thrombocytopenia, and it was not child abuse.

Sepsis, in the setting of a low platelet count, is a red flag for a patient requiring intensive care. If the platelet count is less then 50K, they have an increased risk of post traumatic bleeding. If you have platelet counts of less than 20,000, you can have increased spontaneous bleeding. It was common to transfuse platelets in a patient when their count falls below 10K.

The bleeding time is a test of platelet function. The bleeding time is a small abrasion in the skin, and the time it takes to stop is a measure known as bleeding time. I may have only ordered this test once in my life. It is pretty rare. The most common reason why platelets would not clot well is aspirin, someone who takes aspirin, their blood is a little bit thin. Or uremia. Hypothermia can do this as well.

The lifespan of a platelet is 7 days. Aspirin deactivated the platelet for that length of time. We will exclude genetic coagulopathies here. The question was simple and direct, asking 'Why can not an ADHD child work alone?'

In my experience and the experience of many child psychiatrist as well child psychiatrist, an ADHD child can very much work by themselves. Of course it takes a lot of effort and energy to cultivate the self restraint habit but nonetheless it is not only possible but doable.

A child with ADHD is gifted with many talents. One of them is hyperfocus. Hyperfocus is a state of focus wherey they are so absorbed into the things that they are engaged it that it literally shuts down their perception of all else happening around them. This blessing like any other is a double edged sword. Let me give you an example.

Suppose you are looking after an ADHD child who is playing soccer with his / her friends. The match is an intense one and you can see that he / she is really getting absorbed into it. One of the other kids make a mistake and the ball goes hurtling towards the road. The ADHD child's eyes are totally focused on the ball. All his / her energy forces him / her to maneuver and pursue the soccer ball regardless whatever obstacles may be in their way. As the ball builds toward the busy street, he / she runs with all his / her might with the sole purpose of getting the ball. They enter the hyperfocus state and literally shut out all external perceptions. In their mind, there is only them and the ball, and they are going to get it.

The above scenario happens not just with ADHD children but also with normal children. The only differentiating factor is that the ADHD child will have more hyperfocus events. Imagine what would happen if the ADHD child could harness their hyperfocus and apply it to studying. What would happen if they applied it to learning music? Or perplex physics, like Einstein?

So let me get back to the question, 'Why can not an ADHD child work alone?' There are a few very important implied meanings that we as parents, guardians and teachers must be aware of. The question assumes that an ADHD child can not work alone. That is of course not true. It is simply in the early stages, they need to learn to develop the habit of doing things themselves. Notice that I said they need to develop the habit of doing things themselves, this involves dressing themselves, eating themselves, cleaning themselves etc. All the basic chores that we as humans are required to do must be mastered by the ADHD child first.

Often times the problem starts not because the ADHD child is not able to work alone, it is that they have never had to work alone before. Think about how their lives are organized. Usually the parents are the ones who have to get them out of bed, sometimes coerce them into getting dressed. Then beg them to eat and finally persuade them to get on the car to go to school. All their whims and wishes are tended to immediately, there is no chance for them to learn to become independent.

So can we do to help the ADHD child?

The answer is very simple. Learn to communicate in their language and help them love the process of learning. Many ADHD children have short attentions spans so we are told. The truth is that they are easily distracted. This means that in any given situation you have to create the ideal environment that minimizes distractions. One where they are comfortable enough to be in but not so comfortable that they feel bored.

Next match their learning preferences by interacting with them. This usually takes the form of playing. Play to the ADHD child's preferred sense. If they are kinesthetic move about more, hug them, kiss them, use their arms, legs, head etc. If they are of auditory preference then talk to them, use language such as listen, hear, talk etc. The same goes for the ADHD child that loves to use their visual sense. Get them to use their eyes, make pictures in their heads, use words like see, picture, imagine, pretent etc.

The whole learning process is easy when they are having fun. So the goal to get an ADHD child to learn something and do it by themselves is to get them to like the subject or better yet love it and then own it.

Many parents I see can not bear watching their child struggle for a short time while learning and I fully understand. But here is the catch, if you as the parent, guardian, teacher do not have the patience to allow the ADHD child to learn at his / her own pace then I'm afraid the implicit assumption that an ADHD child can not work alone will become A self fulfilling prophecy.

Be strong, be kind, be fun and learn to communicate the joy of learning to your ADHD child. You will find that once they understand the value of learning and start to love it, everything else will become easy too.

Changing what you eat and how often you eat is the safest method devised for controlling blood sugar. And body weight. If you eat well and in moderation, a healthy eating style can survive an occasional mistake.

No Quick Fix – Journey of a Lifetime Begins With One Step

Select any magazine or text on obesity and you will be able to access many reasons why people are overweight today. Many magazines will explain the scientific reason for the eating of five small meals rather than two large ones, so I will not discuss that at length in this article. I will focus on how an average to a very overweight person can lose the excess weight and never regain it. Losing weight begins with a decision NOW. Take a look at yourself and say “enough”. Tell yourself that you are tired of huffing and puffing around. Tell yourself that you don’t want to be embarrassed when you’re on the airplane and you’re walking down the aisle towards a seat and everyone is hoping that you would not occupy the middle seat next to him/her. Tell yourself that from this moment on you will enjoy getting on the scale to check your weight. Get sick and tired of making excuses for the way you look. Get tired of all of the bulges in your clothing. Become angry that you are ultra-sensitive about the word “fat”. That is a perfectly normal word. Become tired of being the butt of “fat jokes”. Say to yourself “today is the first day of the rest of my life”.

After you have made your decision that you will change from one state to the other then you are ready to pack up and leave. No one leaves to reside in another country without equipment. You have to clean house after sorting and packing. So, let’s see what we need to sort and pack. Firstly, you have to sort. Go through the house and remove any food or snack that is not in its natural form. For example let’s look at rice. Rice as it came from nature is called brown rice. It is not white. It should be eaten as brown rice. Remove all forms of white rice from the house. Do the same with pasta. Pack the cans of meat, fish, deserts, puddings, and any food with a lot of sugar or salt. Packets of chips and salted nuts, candy and chocolate must go. Cakes, pies and ice-cream need to reside somewhere else, not in your home. Please don’t forget the sodas and cool aid drinks. Now that you have no food in the house you need to go to the supermarket. These are the things to purchase: Begin with breakfast items. Oatmeal, nuts (all kinds), raisins, prunes, dates, all kinds of fruits especially lemons, grapefruits and oranges. Splurge on the different kinds of fruits. Do not purchase them all at one time because they have a limited refrigerator life. Every week eat one or two fruits that you did not eat the previous week, but keep the staples : grapefruits and oranges. Next go on to lunch items. Chicken, fish, lean meat, tofu (ask me about tasty recipes), other meat analogs made with soy, an assortment of vegetables: squash, cabbage, green leafy vegetables, bell peppers, every vegetable that is available. Desert items can be home made from the fruits and whole grain products that are purchased. Oils such as olive oil, sunflower oil, flax oil should be used generously. The list of fruits, nuts and grains is a long list. The guideline is natural.

Use the food in its most natural state. For example when cooking mashed potatoes, leave out butter and add olive oil instead. Use spices like oregano, sage, thyme, celery, cayenne pepper etc. Learn to marinate meats before cooking so that the food is tasty when cooked. (I can tell you how). So, now you have changed states and it is time to learn how to prepare the foods that you have selected. Get a good cook book or go online and type in fat free cooking or just the food that you want to cook and ask for a recipe. There are many. I said earlier that the experts say to eat smaller, more frequent meals so that the body is engaged in digesting which keeps the metabolism or burning mechanism working and thus promoting more weight loss. The breakfast is fairly large and the lunch is fairly large also, but the other meals are small consisting of maybe some celery sticks with peanut butter or cottage cheese or other soft cheese or some yogurt. It may be a slice of whole wheat toast with cheese or a piece of chicken. It could also be fruit with some garbanzos. The idea is protein and carbohydrate. The most important key to eating healthfully is to eat before six p.m in the evening. Try to plan the day so that the last meal which should not be too heavy is finished by that time. After that, water and will power will have to be applied.

Let’s look at a day in the life of a person who is going to live in another state. Rise early and drink 20 ounces of warm water with or without half of a squeezed lemon(depending on whether your stomach can tolerate lemons). Next, spend fifteen to thirty minutes reading a good motivational book and repeating affirmations: for example, I am sitting at my desk, it is one year from now and I am a hundred pounds lighter than I am at this moment. I feel great and I look great. I accept responsibility for my body. I will allow no weakness to pervade my mind. I can do ALL things. What I am, I am now becoming. I have begun with the end in mind. I am getting slimmer and healthier every day. Next is the exercise. Anyone who tries to sell you a diet plan that does not involve an exercise component is lying to you. The human body is designed for movement. Begin small. Walk around the bedroom. Walk to the living room. Walk to the kitchen. If you have a downstairs go down stairs for something. Move around for at least fifteen minutes. The best thing to do is to go outside in the fresh air, the earlier in the morning the better it will be. Walk for at least fifteen minutes. Breathe deeply of the fresh air. Gradually increase this to thirty minutes and then an hour and then you can count it in miles. Get some kind of weight training because you want to build muscle mass as you lose the fat . Get magazines and read up on that. Celebrate every victory.

The first day that you walk for fifteen minutes and you could not do that before, celebrate. Give yourself high fives and drink five ounces of water. Never celebrate with a food item other than water. (During the day try to drink at least sixty four ounces of water or more.)When the exercise routine is over then a good shower with warm water ending with cold water will invigorate the body. Breakfast will then be eaten.

Eat some form of oatmeal everyday. It will clean the arteries and is good for the body in general. Eat three or four servings of fruit. Eat some nuts, some lean protein (cheese, egg-white, meat analog etc). Between meals drink water. Remember to eat a huge salad with nuts, garbanzos, or red beans and olive oil and lemon juice and add salt to taste. Eat the salad first and then eat the carbohydrate with the three ounces of meat followed by a tiny desert (could be yogurt, smoothie, one oatmeal cookie etc.). Again the last meal should be before six p.m or at least four hours before bedtime. Drinking Juice to Reduce Cholesterol and Prevent Heart Disease

Heart disease effects many of us in the UK, more so than in many other countries, but there is a strong possibility that this could be changed simply by changing our attitude towards food and consuming more juice.

The term ‘heart disease’ includes several conditions such from Congenital heart defects seen at birth to atherosclerosis (hardened arteries) that develop later. It is a complex issue with several inter-related factors. Hardening of the arteries and angina are a result of the inner walls of the bodies arteries narrowing due to a build up of plaque (fat, LDL cholesterol and other substances). Plaque build up is increased in people who have high levels LDL (bad) cholesterol and low levels HDL (good) cholesterol. Oxidation of excess LDL cholesterol results in an increase of plaque. The HDL cholesterol helps to prevent the LDL cholesterol from increasing the plaque, moving it instead to the liver. Plaque build up alone may be enough to cause pain (angina) or heart attack. However, if the plaque is ruptured or torn it can block the artery causing a coronary thrombosis (heart attack). Atherosclerosis can also be caused by inappropriate platelet activation causing the platelets in the blood to clot.

Grape Juice inhibits Atherosclerosis

Many of us will have seen news items claiming red wine helps to keep our hearts healthy. This is due to a substance in the grapes that used to make the wine known as polyphenols. However, recent tests have shown that although less polyphenols are present in grape juice than red wine, grape juice is better at inhibiting atherosclerosis [1]. Purple Grape juice has also been found to help patients with coronary artery disease due to significantly improving the function of the cells (endothelial) lining the hardened arteries as well inhibiting inappropriate platelet activation [2]. Purple grape juice’s action as a potent platelet inhibitor is important as Platelets are involved in the development of Atherosclerosis [3]. Other research has shown grape seed proanthocyanidin extracts reducing Atherosclerosis by up to 50% in animals. [4] Although that study was for grape seed extract alone, other studies have shown that the combination of grape seed extract and grape skin, as you would find in the juice, is more effective [5].

Fruit Juices Reduce Cholesterol

Pomegranate juice also offers wide protection against cardiovascular diseases6 and has been shown to reduce cholesterol build up in plaque [7] and reduce the development of atherosclerosis [8].

In laboratory tests orange juice has been found to lower cholesterol and significantly inhibit atherosclerosis [9]. A 750ml serving of orange juice daily has been shown to decreased the LDL-HDL cholesterol ratio by 16% in tests on people [10]. Tests on smokers has shown that carrot and orange juice combined significantly susceptibility of LDL to oxidation [11] (the process that increases plaque).
As little as 330 mL of tomato juice [12] has also been shown to significantly reduce LDL oxidation [13]

Adding garlic to vegetable juices takes some getting used to, but garlic has also been shown to decrease bad LDL cholesterol while increase HDL cholesterol [14] as well as reducing LDL oxidation [15].

Plant sterols and stanols found in high quantities in veg like broccoli, Brussels sprouts, cauliflower16 have been found to reduce LDL cholesterol [17]. One report says around 2g per day of plant sterol ester can decrease LDL cholesterol levels from 9% to 20% [18]

Ginger has also been shown to significantly reduce LDL cholesterol and abnormal cell tissues (Aortic atherosclerotic lesion) [19] in the arteries around the heart.

With so much evidence proving the benefits of fruit and vegetable juices why on earth would we not want to include them in our diet?

History of Christenings

Water itself has been used in religious rites for thousands of years. It represents a cleansing of the body, spirit or soul. Many cultures throughout history have used this symbolism for different occasions. The act of Christening, or Baptism, is but one of many such rites. The beginning history of Christenings can be traced through the Bible.

As far back as the 6th century BC, Ezekiel mentions "I will sprinkle clean water on you, and you will be clean …" (Ez. 36:25). The word baptism comes from the Greek "baptizo" meaning, "immerse, dip, submerge". Christenings and baptisms as we know them today, however, stem from the New Testament of the Bible, albeit each religion has its own way of performing the rites. Some prefer total submersion while others only sprinkle water on the elect. The Christening is performed to bring you into the Body of Christ, or the Church and is considered an act of grace and you join in the New Covenant with God.

The story of John the Baptist is the initial New Testament story that we hear that introduces us to the baptisms. He himself was baptized in the Jordan, and as the cousin of Jesus, he would carry the torch forward and baptize people from the surrounding areas himself. He would tell the people that we would baptize with water, and One greater than he would come forward. Therefore the Christenings are looked upon as a cleansing, as well as a union with the Holy Spirit. "… a baptism of repentance for the forgiveness of sins." (Mark 1: 4) and "He will baptize you with the Holy Spirit and with fire." (Matthew 3: 11-17).

In the Roman Catholic Church, for instance, there are full submersion and sprinklings in the Baptismal Rite. It is considered a Sacrament, a birthday in the Church as the infant, youth or adult is brought into communion with Christ. There are two oils used for anointing referring to salvation and forgiveness / repentance to wash the elect of the original sin (that which was performed by Adam and Eve), a white garment to signify purity and grace and the lighting of a candle to signify the Light of Christ. The baptisms are held during the church service or privately, dependent on how the local diocese handles the Rite. Other Christian churches use water and white garments to signify the cleansing and grace.

In the first four centuries it was first the adults, and later the children that would be Christened. Hippolytus mentions an Apostolic tradition to the Rite, while Cyprian of Carthage argued it was Christ that owned us in the Rite, not the priest. In 337 Constantine was baptized on his deathbed, forming the Holy Roman Empire. During the times of the Bubonic Plague, many would have their babies baptized before their deaths to secure entrance into heaven. Luther thought that people would not be saved unless they were baptized. In the 17th through 20th centuries John Smyth, the Westminster Confession, Pentecostals and Pope John Paul II argument on if infant Christenings are necessary, should people be sprinkled or immersed, and are they saved.

When it was said to the Apostles to go out and spread the Word of God to all nations, they instigated the first rash of Christenings, as they baptized people into the faith of Jesus, away from Roman and Judaic laws. Holy Water is used now; As well as each faith has Godparents to raise the children within the faith if anything were to happen to the parents. The elect are pledged to a life in Christ.

The Facts About Yellow Fever

Yellow fever also called yellow jack, black vomit or sometimes American Plague is an acute viral disease. As of 2001, the World Health Organization (WHO) estimates that yellow fever causes 200,000 illnesses and 30,000 deaths every year in unvaccinated populations.

Yellow fever begins suddenly after an incubation period of three to five days. In mild cases only fever and headache may be present. The severe form of the disease commences with fever, chills, bleeding into the skin, rapid heartbeat, headache, back pains, and extreme prostration. Nausea, vomiting, and constipation are common.

Jaundice usually appears on the second or third day. After the third day the symptoms recede, only to return with increased severity in the final stage, during which there is a marked tendency to hemorrhage internally; the characteristic coffee ground vomits contains blood. The patient then lapses into delirium and coma, often followed by death. During epidemics the fatality rate was often as high as 85%. Although the disease still occurs, it is usually confined to sporadic outbreaks.

There is no true cure for yellow fever, therefore vaccination is important. Treatment is symptomatic and supportive only. Fluid replacement, fighting hypotension and transfusion of blood derivates is generally needed only in severe cases. In cases that result in acute renal failure, dialysis may be necessary. A fever victim needs to get a lot of rest, fresh air, and drink plenty of fluids.

In 2007, the World Community Grid launched a project where by computer modeling of the yellow fever virus (and related viruses) thousands of small molecules are screened for their potential anti-viral properties in fighting yellow fever. This is the first project to utilize computer simulations in seeking out medicines to directly attack the virus once a person is infected.

Yellow fever is a significant cause of hemorrhagic illness in many African and South American countries despite existence of an effective vaccine. The yellow refers to the jaundice symptoms that affect some patients.

Yellow fever has been a source of several devastating epidemics. More than half of the army perished from the disease when the French soldiers were attacked by yellow fever during the 1802 Haitian Revolution.

Outbreaks followed by thousands of deaths occurred periodically in other Western Hemisphere locations until research, which included human volunteers (some of whom died), led to an understanding of the method of transmission to humans (primarily by mosquitoes and development of a vaccine and other preventative efforts in the early 20th century.

Human infection begins after deposition of viral particles through the skin in infected arthropod saliva. The mosquitoes involved are Aedes simpsaloni, A. africanus, and A. aegypti in Africa, the Haemagogus genus in South America, and the Sabethes genus in France.

In 1937, Max Theiler, working at the Rockefeller Foundation, developed a safe and highly efficacious vaccine for yellow fever that gives a ten-year or more immunity from the virus. The vaccine consists of a live, but attenuated, virus called 17D. The 17D vaccine has been used commercially since the 1950s.

The mechanisms of attenuation and immunogenicity for the 17D strain are not known. However, this vaccine is very safe, with few adverse reactions having been reported and millions of doses administered and highly effective with over 90% of vaccinees developing a measurable immune response after the first dose.

Although the vaccine is considered safe, there are risks involved. The majority of adverse reactions to the 17D vaccine result from allergic reaction to the eggs in which the vaccine is grown. Persons with a known egg allergy should discuss this with their physician prior to vaccination.

In addition, there is a small risk of neurologic disease and encephalitis, particularly in individuals with compromised immune systems and very young children. The 17D vaccine is contraindicated in infants, pregnant women, and anyone with a diminished immune capacity, including those taking immunosuppressant drugs.

In current research ribavirin treatment during the first five days after virus infection improved survival rates, reduced tissue damage in target organs (liver and spleen), prevented hepatocellular steatosis, and normalized alanine aminotransferase a liver damage marker levels.

The results of this study suggest that ribavirin may be effective in the early treatment of yellow fever, and that its mechanism of action in reducing liver pathology in yellow fever virus infection may be similar to that observed with ribavirin in the treatment of hepatitis C, a virus related to yellow fever.

Because ribavirin had failed to improve survival in a virulent primate (rhesus) model of yellow fever infection, it had been previously discounted as a possible therapy.

This immune response comes from our body’s ability to fight a plethora of toxins that we ingest over our lifetimes.

One of these is antibiotics. Sure, they save lives. But rampant and often unnecessary antibiotic use is the biggest issue of healthy intestinal flora.
