Dysthymia: Myth or Monster?

Chronic, low-level depression has reached such epidemic proportions that the medical community has given it a name – Dysthymia (pronounced dis-thigh-me-ah). It’s a big problem affecting nearly 8% of the U.S. population. Low-grade depression (dysthymia) is one of the most common ailments on the planet and one of the least likely to be diagnosed. Like its cousin, clinical depression, low-grade depression hits women roughly twice as often as men. Dysthymia is, by definition, chronic. A diagnosis requires the presence of symptoms on more days than not for a period of at least two years, which is what makes it so hard to pin down. Any given day might be okay, even happy. Yet in the general run of days, there are more gray ones than not, more unhappiness than joy. Most people afflicted with this kind of chronic malaise instinctively blame themselves: They would rather believe they can solve the problem-if they could just find the right job or the right man or lose weight-than admit they have a psychiatric disorder. Unfortunately, many people who are suffering do not seek help because they have been feeling this way for so long, the feelings of depression and malaise are their “normal”.

The presence of two or more of the following symptoms persisting for at least two years is indicative of dysthymia in adults:

  • Poor appetite or overeating
  • Insomnia or hypersomnia
  • Low energy or fatigue
  • Low self-esteem
  • Poor concentration
  • Difficulty making decisions
  • Feelings of hopelessness

So, what’s the problem? Why are so many women having such an issue with anxiety and/or depression? One reasons is that we are taught and conditioned from our first day on this planet to go after “the American Dream” (or the European dream, or the Australian Dream – or whatever – you get our point) – to have it all – the family, the relationship, the career, the house, car, money, etc. Over time, one of two things seems to happen – either you fall short in one or more areas of life and eventually give up on your dreams, or you actually get it all and find yourself in this place of comfort and routine with no new goals on the horizon. In either case, you find yourself in a dark place where you feel absolutely stuck and dissatisfied with your life and if left unchecked, that dissatisfaction has a nasty way of manifesting itself as chronic, low level depression.

The mental health experts have identified all kinds of underlying factors why women, in particular, are so susceptible to developing depression in their middle years. According to the National Institute of Mental Health, women develop depression at more than twice the rate of men. We’ve boiled these factors down into six basic areas:

Frazzlemania (Overload): The demands on women today are huge! Most middle-aged women have careers, families, households, aging parents and a myriad of other responsibilities vying for their time and attention. Striking a balance between all of them has a tendency to leave one feeling like a failure – spend too much time on the career and the children suffer – too much time on the kids and the marriage suffers – you can see the problem. plus, many women were born at a time when it was very common for women to be stay-at-home-moms and there is guilt in play as well when they feel they are not doing as good a job managing everything as their own mother did. The guilt, coupled with the sense of failure and stress, make a fertile ground for depression to take root.

2nd Class Setback (Societal Gender Inequity): As evolved as we would like to think we are, the fact remains that in much of the industrialized world, women are often still not valued at the same level as men. Women who do not work outside the home are often perceived as sitting around watching soap operas and eating bonbons during the day. Women who do take time away from their careers to raise children have a difficult, if not impossible, time making up lost ground in terms of pay and retirement savings. These types of “no-win” scenarios can lead women to feel like second-class citizens and cause stress and hopelessness.

Foggy Glasses Syndrome: Depression is very much a disease of perception. Everywhere we look we see images of perfection – the perfect body, the perfect car, the perfect house, the perfect relationship, the perfect children, etc. Perfection is an illusion, yet everyone is trying to attain it. These images are hard to live up to at any age, but become particularly glaring as we age. Constant exposure to these images of perfection works on us in subtle, subconscious ways making us feel “less than”.

The Sucker Punch (Traumatic Life Events): How we react to unexpected events in life certainly affects our mood and emotions. A stressful life event can plunge a person into depression, especially if a person is at risk for depression due to other factors. Stressful life events may include: prolonged medical illness; illness or death of a loved one; divorce; ending a close relationship; loss of a job; relocating; and financial or legal problems.

The Clone-Effect (Environmental Factors): The environment in which you were raised can be a major factor underlying your susceptibility to depression. If you were raised to think you were never good enough or that you were a “bad” kid or other negative ideas, these ideas are incorporated into your self-image over time. If you have a negative self-image, the twists and turns of life can make you much more susceptible to becoming depressed.

Comfortitis (Comfort vs. Achievement): This was my KEY underlying factor! I was a goal achiever from Day One – I always wanted to be doing it better, faster, smarter than anyone thought possible. Once I got to a point in my life where I had achieved the major goals I had set for myself, I became “comfortable” and in that “comfort”, found myself utterly miserable. There are many people who need to constantly be striving for the next goal – without it, they feel rudderless, without purpose, and depression can easily take hold.

Regardless of the underlying factor (or factors) that caused it, the first step in getting through the depression is to admit that there is a problem. How many conversations have you had with yourself saying that you have to find a way to be okay with your lives, to accept it – this was just the way it was! Well, that’s just not true! The life you are living right now is not the way it has to be! You are not a slave to your circumstances!

Sadly, everyone will at some time in their life be affected by depression, whether it be their own or someone else’s. Fifty-four percent of those people believe depression is a personal weakness and that there is no hope for them to get through it, which is why 80% of people suffering from chronic, low level depression are not currently having any treatment. And get this, A National Mental Health Association survey showed that more than 50% of women believe depression is a “normal part of aging” and that treatment for depression during menopause is not necessary!

According to the World Health Organization depression is the second largest killer of man, after heart disease, with studies showing that depression is a contributory factor to fatal coronary disease. and current projections show depression overtaking heart disease as the #1 killer worldwide by the year 2020. There have even been studies done showing links between depression and illness including osteoporosis, diabetes, heart disease, some forms of cancer, eye disease and back pain.

But you know, we just don’t pay as much attention to our mental health and well-being as we do our physical health. We read those statistics and most of us just shrug and say there is nothing I can do about it, that’s just life. Well, in part, that’s true. It is your life and that is because everything we do, every feeling we have and our belief system and the programming we received as a child are driving every result we are getting. That self-image, that belief system, is nothing more than a huge collection of habits – the programming. It’s happening in the background and without your permission. It’s like a computer virus. Those little bugs get inside your computer and wreak havoc, but they don’t keep you from using your computer until things get so bad that the system just crashes. They operate in the background, not caring what you try to do, they just carry out their program unbeknownst to you. So how do you keep from becoming a statistic? Is there a way to change your programming?

The cool thing is that it really is possible and actually quite simple to re-program your “hard drive” with clean, virus-free programs of your choosing! It is so simple (although not necessarily EASY), but it’s not something that we’re generally taught. There’s nothing new about it – it’s been around for thousands of years, but it’s stuff that isn’t taught in school – although it certainly should be!

That feeling that life is hopeless or out of control, being angry at yourself for not being able to do anything about it, changes in sleep or eating habits, loss of interest in engaging in activities that once gave you joy – we’ve convinced ourselves that all of these are normal parts of life. We convince ourselves further that as long as we are not under a doctor’s care or on prescription medications that everything is okay. But the fact is that these are all signs that something is wrong and the cause can be traced back to some “virus” in your mind. The emotional pain that you feel is the higher side of yourself screaming at you to live up to your infinite potential – to do more, be more, have more!

There is help out there! Traditional programs for stress and behavior modification get you all hyped up with motivation, but offer no follow through action plan. Think of the typical diet program – it teaches you how to modify your behavior to eat in a certain way, to exercise a certain way, but fails to address the root cause of the excess weight – the self-image – the programming! For lasting change to occur the cause of the problem has to be tackled, not just the symptoms.

It is your birthright to be happy and prosperous and your obligation to be all you can possibly be. Why on earth should we settle for the status quo when greatness and abundance beyond our wildest dreams is within our grasp?

The Pain Factor

Last year I told an acquaintance that at the age of 46, I had taken up running. The reply was, “Oh, that’s awfully hard on your knees.” At first I was taken aback. Yes, I knew if I didn’t stretch, warm-up or wear properly fitted shoes, I might experience pain or an injury. But the person telling me this was overweight, had borderline high blood pressure, and never worked out. I wondered if this friend realized by living a sedentary lifestyle, the odds of developing heart disease, osteoporosis, diabetes and some cancers had substantially increased. Not to mention that being overweight actually placed this person at a higher risk than me for developing knee pain from osteoarthritis and heel pain as a result of plantar fasciitis.

I’m no spring chicken and sometimes my running resembles a dawdling old hen. But I do know it’s necessary to take precautions at any age to guard against injury when participating in physical activity such as running. Because the truth is, sometimes pain happens. When it does, you can either use pain as an excuse or you can use it as a diagnostic tool to help improve and go forward with your performance.

There are three classifications of pain. In simple terms, these can be described as the following:

Nociceptive Pain: felt after an injury to body tissues such as cuts, sprains, broken bones, bruising, surgery, and sometimes cancer. Most pain is of this type.

Neuropathic Pain: resulting from an injury to nerves, the spinal cord or the brain, examples being Phantom Limb Pain and shingles – which affects nerve tissue.

Psychogenic Pain: is related to a psychological disorder where the type, intensity or proportion of pain experienced is greater than the injury. Some chronic ailments may be related to this type of pain.

Pain can also be defined as acute (an immediate response to an injury) or chronic (a pain lasting more than six weeks). The majority of injuries from physical activity fall into the category of acute nociceptive pain. Although some overuse injuries such as Plantar Fasciitis or Runner’s Knee can become chronic if not properly treated or allowed enough time to heal. Most injuries to body tissues are minor and can be treated with nonsteroidal antiinflammatory drugs (NSAID), such as ibuprofen, and ice therapy or R.I.C.E. (rest, ice, compression and elevation) to decrease pain.

Cuts, bruises, strains, sprains, swelling and inflammation can generally be treated in this way. Severe acute injuries, such as fractured bones and ruptured tendons, should always be treated by a medical professional, as is the case with injuries resulting in chronic, neuropathic and psychogenic pain. If you experience minor pain or inflammation during an activity, this is a good time to evaluate what your body is saying and respond in a positive, strengthening manner. Try asking the following questions:

  1. What particular part of my body is affected?
  2. Does the pain happen only during a certain activity or is it constant?
  3. Am I experiencing pain when running or walking on a certain type of terrain?
  4. Is this a new pain or one that has happened before?
  5. What measures can I take to correct or strengthen the affected body area?

I used this list of questions to understand a reoccurring pain on the outside of my legs. After a little research and a trip to my local running specialty store, I learned the pain was IT Band Syndrome. The IT Band is a long fibrous muscle, located on the outside of the leg. When it becomes inflamed, pain is felt at a point near the knee joint. I knew the pain usually happened when I was hiking downhill for long periods of time and it went away after a few days of rest. This signified several areas of my body that needed work: weak leg muscles, a tight IT Band, and not enough arch support to stabilize my knee during an activity. I have incorporated the following positive measures to help strengthen my body’s weak points:

  1. Wearing well-fitted running and hiking shoes with strong arch support.
  2. Adding leg strengthening exercises to my daily routine.
  3. Wearing neoprene knee braces on a long hike with a lot of downhill climbing.
  4. Increasing my daily stretching routine, with particular stretching for the IT Band.
  5. Doing stretching and warm-up exercises before a strenuous hike or run.
  6. Cross-training: running, walking, hiking, biking, weight lifting and using the elliptical trainer to provide a variety of exercises to all leg muscles.
  7. Using ice therapy immediately after a hike if I feel pain.
  8. Working up to a strenuous hike by doing shorter hikes on hilly terrain weeks before the big day.
  9. Maintaining a normal weight so as not to place added stress on my legs.

Don’t let pain, or your fear of it, be a factor in whether or not you are an active person. Not exercising will result in far worse consequences. Use pain as a guide to become a stronger, more aware and healthier person.

Many people think that most bullies suffer from a low self-image and lack a sense of self-respect or self-worth. The truth is, most bullies are popular kids and come across very confident. In many cases, bullies are not the insecure kid who is trying to compensate for any problems that they may have like many people assume. There are some who believe that bullying may be a way that one child tries to dominate other children and assert their power and that children who bully frequently act with the approval of their friends and see it as a way to fit in and impress their friends.

Many parents have the misconception that bullying happens only in low-income neighborhoods and schools. There are just as many bullying cases in upper-class neighborhoods and schools; and in many cases, the bullying (cyber-bullying) is more severe in the upper class schools that result in fatalities. One of the reasons that upper-class schools have bullying problems is because oftentimes, the kids that attend these schools feel as though they are superior than other kids and want to show their friends how much power they have over the kids that they feel are weaker than they are.

One thing that you must remember when talking about bullying; when boys bully, most of the time boys are physical but girls tend to use emotional bullying which often looks like:

  • They will say mean things and spread nasty rumors to humiliate other girls.
  • Girls will also threaten the girls that they bully online by disparaging them for amusement.
  • The fact is; girls are more likely to be cyber bullies than boys and it is important to know that cyber bullying can happen anywhere as long as your child has a computers or a cell phone.

The truth of the matter is that bullying often has disturbing effects on those who are bullied and most kids who are bullied become very reluctant to talk about being bullied making it even harder to help them.

Your Child and Bullies-Things to Look For

Let’s talk about a few things to look for that may give an indication that your child might be the target of a bully. Does your child have few or little friends, do they appear to be withdrawing from the friends they were close to? It is important to be aware of one of the signs of being bullied, is the sudden decreases in friends. This could very well show that your child is possibly being bullied; when kids are bullied they will often withdraw from the very things that they used cared about due to the stress of their situation.

If you begin to see that your child repeatedly comes home with damaged things; torn clothes, or any missing property (books, phones, things of value to them), this may also show signs that they are being bullied. When kids take or damage things of value that belong to other kids, it is often a way that kids bully others as well as physically harming them. Take notice also if you begin to see your child coming home with bruises, scrapes, and other injuries that they can’t explain. And one last thing; pay attention when your child’s grades start to slip and take a rapid plunge for no apparent reason or when their teachers mention that they have notice a sudden change in your child’s behavior.

As we mentioned earlier, many kids who are being bullied find it difficult to talk about and become very reluctant to talk to you about being bullied. This does not mean that we don’t talk to our kids, it simply means that we can’t become frustrated with our children when they become quiet and struggle to talk about their situation.

If in fact your child is being bullied, it can be a stressful, debilitating experience. Most of the time children don’t know what to do when they are being bullied and you want make sure that they know that you are there from them. Many children make the wrong decision when it comes to dealing with bullies, or they take no action at all.

Most children are scared that telling someone will only make matters worse. Reassure your child that telling an adult is one of the best ways to make the bullying stop. It is normal for a child who is bullied to be unwilling to talk about it, so be sure you reassure them that it can only help. When kids don’t want to talk to their parents or their teachers because they fear that those adults will only make matters worse, have them find an adult that they trust and can confide in them. It could be an aunt, an uncle, maybe a coach or even a trusted clergy person. The bottom line is that you want to help them and want them to know that help is available to them, that it is not a hopeless situation.

A tube placed down the esophagus to the duodenum (the first portion of the small intestine) will calibrate the diameter of the portion of the stomach that will remain. The stomach is cut in a vertical direction, removing the part of the stomach that “grows” when an individual eats. Only a thin portion of the stomach (in the form of a cylinder or a sleeve) remains. The surgeon will use sutures to reinforce staples that close the ends of the remaining stomach portion together, reducing the risk of leaks or bleeding. The small intestine is left completely intact.

Unlike gastric bypass surgery, the gastric sleeve procedure does not require a reattachment of the stomach portion to the small intestine. Consequently, there is less risk of complications. Also, since the amount of time the patient is required to be under anesthesia is lessened in comparison to a gastric bypass procedure, the patient is at less risk, and benefits from a faster reincorporation, reducing pulmonary complications.

The gastric sleeve operation by conventional open surgery is reserved for patients for whom laparoscopic surgery is contraindicated, or for patients with a history of prior multiple abdominal surgeries or for patients with a gastric band, which have recuperated part of their lost weight. Open gastric sleeve surgery is begun with an incision from the sternum to the belly button. The same instruments used in the laparoscopic surgery are used for the open surgery, which also requires general anesthesia. However, this surgical method may be completed in about an hour.

Regardless of which technique was used for the gastric sleeve, patients generally are allowed, and able, to get out of bed within 24 hours. However, patients are advised not to drink water until 48 hours post-op. All medications and liquids are administered intravenously.

You will stay under observation for 24 additional hours in order for your doctor to be assured that you have no problems drinking water and then the patient is released to his or her hotel room with a recommended nutritional diet. Patients are generally able to engage in normal activities in about 6 to 8 days after the operation.

Polyps: polyps (unusual glands) increase the chance of having cancer. These polyps are due to excess growth of cells in the colon region or due to the family pedigree.

: This activity is the major risk factor in causing cancer. Smoking leads to lung cancer and also causes several side effects like digestion problems, burning sensation, headache and site blindness. So avoid smoking to prevent cancer from occurrence.

Colon cancer symptoms:

Sleep disorder: Due to severe illness, stress, emotional pain, usage of drugs and loneliness are the driving factors for causing sleep disorder. People suffering with colon cancer will generally face this disorder .

The nature of this head and neck cancer cells were brought to the normal condition by controlling the divisibility of the cells.

• In tablet or pill form(head and neck)
• By taking injection(Intravenous or intramuscular)
• Injection taking in the (Intrathecal)

Palm Wine and Its Importance In Traditional Igbo Society

Palm wine in Igbo land comes in two sources: nkwu (palm tree) – the source of the palm wine known as “Mmanya Nkwu”, and ngwo (rafia palm tree) – the source of the palm wine, known as “Mmanya Ngwo”. Each of the palm wine categories are regarded with different respects. Each has different functions in different occasions, depending on of course, how that particular area or village regards it.

In production, they are tapped in much the same way, by climbing to the required height or to the neck of the palm tree and cutting ducts, under which are placed local mugs (calabashes) or plastic gallons. In some areas, the “Mmanya nkwu” is tapped also from a fallen palm tree. However, it is worth mentioning that the quantity of “Mmanya ngwo” obtainable from the palm tree is usually about thrice that of “Mmanya nkwu” within the same time limit.

In taste, they are also different while “Mmanya ngwo” tastes very sweet (sugary) but goes sour (fermentation) within a shorter duration. The “Mmanya nkwu” has a unique sweet, but pleasant taste which it maintains for a longer duration. Some areas in Igboland regards “Mmanya nkwu” better than “‘Mmanya ngwo”. This preference sometimes depends on the availability of one over the other in that area. Hence in some areas, traditional marriage and bride price ceremonies are not honoured with “mmanya ngwo”, some other areas can accept “mmanya ngwo” where “mmanya nkwu” is not available. In some areas, the preferred kind of wine is employed in settling land disputes, in traditional gatherings, marriages, burials, festivities, land leasing occasions, among a host of other activities and ceremonies too numerous to mention.

When being served in any occasion, the associated rules are obeyed. In any gathering, it is the youngest man or male that serves the wine, which is usually drunk traditionally in either elephant tusks or cow horns for titled men. There is also another drinking cup called “Okuku”, a small type of cup made from a calabash. The young man serving will hold the wine container (calabash or gallon) on his left lap (thigh) and supports it with his left hand, while he holds the tusk or horn (“mpi” or “Okuku” – calabash cup) in his right hand. The first one he serves is handed to the host to drink. The second goes to himself, while the third goes to the eldest man in the gathering.

After all the formalities, the rest of the men are equal, and are served except for titled men who are served first before the others. But if the young man is to serve the wine standing, he holds the base of the container with his right hand and the neck with his left hand, while the partakers holds out their cups for him to pour out the wine. This is not the case for a woman. If she is to be served the wine, the young man pours it himself and hands the cup of wine to her, who in turn receives it with both hands as mark of respect and honour. She must not drink it while standing in the gathering; she squats down or sits somewhere before drinking it. If at any point the young man decides to shake the container to make for even concentration, he must first drop the container on the ground before he continues serving and the first person to tap him will receive a cup of wine.

The last cup of palm wine which contains the dregs (Ugwu mmanya) is usually given as a mark of honour to the eldest or the host. The Igbo man, you may infer, is actually rich in culture and tradition.
