So how can you increase your ability to pay active attention while driving? Never chat on your cell phone – in some states like California, it is now illegal to drive and talk on the phone. Try not to daydream, and if someone else is in the car with you, do not look at them while talking – keep your eyes and attention on the road. Averting your eyes, for even two seconds, while talking to a passenger, could put both your lives in serious danger.

Also form good driving habits. For instance, if you’re making a trip to a new destination, get familiar with your map or directions before getting into your car. Make sure that you can recall most of them from memory so that you are not forced to finger through, or constantly look at your papers while driving. This is because if you maintain the proper distance, it’s almost impossible to hit the vehicle in front of you – because you’ve allowed yourself enough reaction time to stop.

So what is the proper following distance? A good rule of thumb is the “two second” rule. You want to always remain a two-second count behind the vehicle in front of you. However, the faster your speeds are, the more space you must allow. At freeway speeds, maintain a good 4 to 5 second leeway.

If counting seems difficult to measure while driving, consider car lengths…always have at least one car length between you and the car in front of you. At freeway speeds, lengthen that space to roughly two car lengths or more. If you find yourself having to brake every time you see the car in front of you brake, then you are following too closely.

If you love to drive, you hate the admonition to not speed. However, driving too fast for road conditions or traffic exponentially increases your chances of getting into an accident. Yes, inertia’s a witch. But it’s a law of physics and no pouting will change that. So work with the universe and watch how fast you’re going. The easiest way to manage your speeds is to keep pace with the traffic around you. And most importantly, don’t change lanes or weave in and out of traffic when at high speeds.

If you have any alcohol, you must wait two hours for every drink consumed before you drive. And always follow up alcohol with an equal amount of water. More than 2.6 million people in the United States have atrial fibrillation – the most common type of cardiac arrhythmia or irregular heart beat. While atrial fibrillation itself is a very serious condition, the symptoms of the condition can also be quite serious.
Symptoms of atrial fibrillation are related to the fact that your heart is beating faster than is normal. Your body may perceive this rapid or irregular heart rate as:
•    palpitations
•    intolerance for exercise
•    angina (chest pains caused by a lack of blood to the heart)
•    congestive symptoms like edema (the build-up of fluid) and shortness of breath
Sometimes people don’t experience any symptoms and the condition is identified during a routine physical examination.
The good news is that you can reduce your symptoms when the right diagnosis is made and you follow through with the recommended treatment.

Why worry
Atrial fibrillation is one of the most common rhythm disorders in U.S. adults over age 65. And those with the condition are four to five times more likely to have a stroke – a major stroke that leaves a person severely disabled or dead.
The American Heart Association and the American Stroke Association think that educating people on the dangers of atrial fibrillation will save lives. They have established a joint goal to improve the cardiovascular health of all Americans by 20 percent, while reducing the number of deaths from cardiovascular disease and stroke by 20 percent. A big part of their plan is to educate the public about atrial fibrillation.

Types of treatment
Once you have been diagnosed as having atrial fibrillation, the goal of treatment is to get your heart back into a normal rhythm through medication or a procedure such as electrical cardioversion, catheter ablation or surgical ablation. The second goal is more long term – to use anticoagulants to avoid developing blood clots or having a stroke.

The following are steps your doctor may recommend:
•    Atrioventricular node ablation. The doctor uses radiofrequency energy to destroy the electrical connection (AV node) between the upper and lower chambers of the heart. This blocks the heart’s electrical impulses. Once the AV node is destroyed, you will receive a pacemaker to establish a normal heart rhythm.
•    Cardioversion. The doctor gives your heart an electrical shock to help restore its normal rhythm by placing paddles or patches on your chest to electrically shock your heart.
•    Catheter radiofrequency ablation. The doctor inserts a thin, flexible catheter (tube) into a vein in your arm or groin and then threads it through your blood vessels to your heart. Heat is applied through the catheter to destroy the problematic heart tissue. This heat is actually radiofrequency energy.
•    Medications. Your doctor may prescribe medication to control your heart rate or heart rhythm, or to prevent blood clots. Beta blockers, especially cardioselective beta blockers such as metoprolol, atenolol and bisoprolol nebivolol can be given. Non-dihydropyridine calcium channel blockers, such as diltiazem or verapamil, can be considered. And in some cases, cardiac glycosides, such as digoxin, have limited use. Anticoagulants, like warfarin (brand name Coumadin) are given to decrease your body’s clotting proteins – and lessen the likelihood you will throw a clot.
•    Pacemakers. A pacemaker may be the best way to help regulate your heartbeat. If that is the case, the pacemaker will be surgically placed under your skin near your collarbone.
•    Surgery. A surgical option is maze heart surgery. A surgeon will make small cuts in the upper heart chamber to help disrupt the electrical signals that are causing the irregular heart beat. The doctor can sometimes access the heart through small incisions in the side of your chest – described as minimally invasive heart surgery. The benefits of minimally invasive surgery are that you have smaller, less noticeable scars, a lower risk of infection and less blood loss.

After care
Most likely your doctor is going to want you to make some healthy lifestyle changes that will keep your blood pressure under control and improve your overall health. Common suggestions include:
•    Eat heart-healthy foods that are full of phytonutrients, which prevent and repair damaged cells. Examples include salmon, whole grain foods and leafy green vegetables.
•    Get some exercise, based on your doctor’s directions. The American Heart Association recommends 150 minutes of moderate exercise each week – or 75 minutes per week of vigorous exercise. You can break this down to 30 minutes a day, five times a week, or break it up into even smaller segments. Studies show you will also experience the benefits of exercise even if you divide your time into two or three segments of 10 to 15 minutes a day.
•    Use less salt. Again, the American Heart Association recommends no more than 1,500 milligrams of salt a day.

Is MSG A Trigger For Headaches?

Does Monosodium glutamate (MSG) cause headaches? It’s a simple enough question but there seems to be no clear-cut answer. Research studies have been inconclusive. Many say that consuming MSG is not worse than eating a high salt diet. Others campaign vigorously against its use.

As far as headache sufferers are concerned, the answer seems to be – if it affects you, avoid it. People seem to have varying tolerance levels for MSG. Some can eat it quite happily, others appear to react to a very tiny amount. Many migraine sufferers believe that MSG will trigger a migraine for them. Even non-sufferers have been known to develop a headache after eating food containing MSG.

If it’s known to cause headaches – why is it put into food?

It’s all down to money. MSG is a flavor enhancer. The food industry can use cheaper, lower quality ingredients and by adding MSG, the taste is improved. MSG is a cheap additive and so it adds up to more money for the food provider.

MSG makes so much money for them, that MSG manufacturers have got together to finance ‘medical research’ to prove the MSG is safe. Those who are against the use of MSG say that these research studies are biased as the board who advises the FDA includes many people who work in the food industry.

What sort of food has MSG in it?

Some foods are clearly labeled as containing MSG. Others are not. Anything that has the following in it always contains MSG:

  • Hydrolyzed protein
  • Sodium or calcium caseinate
  • Yeast extract or autolyszed yeast
  • Yeast nutrient
  • Textured protein
  • Glutamic acid

In addition, if you see these words on the packaging, the food often contains MSG:

  • Flavoring
  • Flavors
  • Natural flavors
  • Stock / bouillon
  • Broth (any kind)
  • Carrageenan
  • Whey protein concentrate
  • Whey protein
  • Whey protein isolate
  • Soy sauce
  • Soy protein isolate
  • Soy protein concentrate
  • Malted barley
  • Malt extract
  • Malt flavoring
  • Maltodextrin
  • Anything ‘protein fortified’

It’s quite a list, isn’t it? But is doesn’t stop there. Other potential sources of ‘hidden’ MSG are sports drinks, chewing gum, bubble gum, colas and candy. It’s becoming clear that if your headaches do seem to be triggered by MSG, you need to be vigilant when buying packaged or processed food.

How can MSG affect me? If you are susceptible to it, MSG can cause bring on a Migraine attack and cause other severe headaches. It can also affect all of these bodily systems:

  • Cardiac: drop in blood pressure, rapid pulse, angina, arrhythmias
  • Circulatory System: fluid retention
  • Neurological: light headedness, mood swings, depression, ADD
  • Digestive System: nausea, vomiting, diarrhea, cramps, bloating
  • Respiratory: shortness of breath, chest pain, runny nose, sneezing
  • Skin: rash, numbness, hot flashes, dry mouth
  • Urological: prostate swelling, nocturia (having to go to the bathroom in the night)
  • Visual: blurred vision or difficulty focusing

How can I find out whether it’s MSG that’s affecting me, or something else?

If you think you have a problem with MSG, you may wish to try an ‘exclusion’ diet. Go for 3 weeks with no (or little) MSG. At the end of the 3 weeks, eat something that you know has MSG and see if your symptoms come back.

I can read labels – but what about eating out?

Most airline food contains MSG, unless you choose a vegetarian option. But be careful, even salad dressing can contain it. The fast-food restaurants use of MSG is as follows:

  • Kentucky Fried Chicken: Everything contains MSG
  • Taco Bell: They claim that there is no MSG in any of their food. However, some people have reported symptoms after eating there.
  • Burger King: The Garden Salad and French Fries are clear. Everything else contains MSG.
  • McDonalds: Garden Salad with lite Italian Dressing is fine. Iced tea is fine. Apple pie is fine. Everything else contains MSG. McDonalds does have a complete list of their ingredients in their restaurants. MSG is usually found under the ‘hidden names’ in the list above.

Remember MSG is not a problem for everyone, as far as headaches are concerned. But, it is good to be armed with knowledge, if you have determined you need to avoid it.

Knee Replacements And Bodybuilding, What Is Acceptable

If you are a bodybuilder or a fitness enthusiast that strength trains, you will have some concerns when it comes to making the decision to have a knee replacement. After all, there will be some physical restrictions placed upon you such as no high impact sports or running as these will only shorten the lifespan of the prosthesis.

When it come to bodybuilding, there really isn’t too many contradictions in regards to exercises that you can’t complete. Therefore, you should be able to continue your bodybuilding without jeopardizing your surgical knee if you use some restraint in the amount of weight you intend to lift.

I have used the following exercise for my legs to keep them strong and from also allowing muscle atrophy to develop which is something anyone involved in bodybuilding knows all to well about.

Leg Press: I use the leg press more often than the squat because the weight is balanced more appropriately and, I can concentrate on the quadriceps more effectively.

The intention when it comes to lifting weights with he affected leg is to stimulate the muscles involved not necessarily how much weight someone can lift.

The leg press allows you to keep control of your body so that you can concentrate entirely on the surgical leg. I like to use the leg press as it allows me to build more muscular endurance in the leg without sacrificing form and setting me up for injury.

The Squat: Yes, I have found you can still use the squat effectively however, common sense has to be used. The days before surgery where you may have placed 400-550 pound on a barbell is not recommended. You can still squat safely to parallel or even a little deeper however, the weight used should be something you can use safely for 12-15 repetitions. The amount a weight someone uses of course will differ but keeping the weight no heavier than 225 pounds can be acceptable.

Placing too much weight on a consistent basis with the squat can cause a breakdown around the prosthesis and lead to a loosening of the components. The weakest link with a knee replacement will be the cement used by the surgeon to hold the knee components in place. With excessive weight over long periods of time can cause loosening.

Hamstring exercises: Basically all the hamstring exercises are allowable and I find none of them have caused any harm with my knee replacement in over 14 years now. The same goes for the calf muscle routines as well. The important thing to remember is that keeping your weight selection modest is the key.

There really to my knowledge have not been any studies done about how much weight an individual can use when strength training on a knee replacement and, if you ask your surgeon unless they are active bodybuilders themselves, will not have any advice for you either other than don’t lift!

So do not fear that your weight lifting or bodybuilding days are over with just because you have had a knee replaced. Just be a little more creative in your weight selection and, find other ways to fatigue those quadriceps and hamstrings such as using shorter rest periods or compound sets in the future.

What Is the Difference Between LASIK and ILASIK?

What is LASIK Surgery?

Floaters in the Eyes – Their Causes and How to Get Them Removed

Eye floaters are small specks, spots and cobweb like particles that drift about in your field of vision. Ordinary eye floaters are fairly common and need not be a cause of alarm.

Your eye contains a clear gel like liquid called the vitreous. Unfortunately, the vitreous humour degenerates as we age. When we are young the vitreous has a gel like consistency but as we grow older it may become more watery, especially in the centre of the eyeball.

Parts of the vitreous become more watery and other parts become more solid than they should be. The more solidified gel particles may float around in the more watery parts of the eye.

When small pieces break loose from the back of the eye and float around your eye it is called a floater.

Floaters also have a tendency to sink to the bottom of the eyeball and can blur the vision until they move to a different area. Sometimes people may think they see something out of the corner of their eye but nothing is there.

Floaters are more noticeable in bright light or looking at a computer screen for example. You may not see the floaters but they may cast shadows on the retina at the back of the eye, which you do see.

The Sudden Appearance of Floaters

If floaters appear suddenly then seek expert advice immediately. It may mean serious damage to the eye. It could indicate serious problems with the vitreous gel or even the retina becoming detached.

Posterior Vitreous Detachments – PVDs

This occurs when the vitreous gel starts to become detached from the retina. This is serious and will need urgent attention. The gel may pull on the retina causing physical damage and even retinal detachment.

This may be noticed by the occurrence of light flashes because there is mechanical stimulation to the retina such as the vitreous gel pulling on it. This will stimulate the retina in a similar way when light falls on it.

Weiss Ring Floater

A large floater that is attached around the optic nerve area. Usually circular, doughnut or C shaped.

In more serious cases the retina may be torn and blood will leak into the vitreous, seen as small dots moving across the field of vision.

Vitreous Haemorrhage

This is a severe condition, which could mean a retinal tear or retinal detachment.

Retinal detachment requires urgent attention as it could lead to blindness.

Common Causes of Severe Eye Floater Conditions

> Cataract Surgery

> Laser eye surgery

> Diabetes – diabetic vitreopathy

> Eye infections – may cause the vitreous to liquefy

> Cytomegalovirus (CMV) retinitis is inflammation of the retina of the eye caused by a virus

> Being very nearsighted – high myopia. The elongated shape of the eye may cause tension on the retina. More likely to get floaters at a younger age.

Any surgery or damage to the eye such as an accident or fall may aggravate a floater condition.

As you get older these problems become more likely as the vitreous gel becomes more watery in the centre and the gel attached to the retina starts to pull more on the retina and parts can become detached and float around in the watery centre of the eye.

Treatment of Floaters

Most eye floaters are merely annoying and irritating and will probably not cause any major problems. They become more likely as we grow older.


For severe cases – the vitreous is drained from the eye and is replaced with a saline liquid.

Laser Vitreolysis

The floaters are targeted with a laser and burnt away. This method is less invasive than the Vitrecomy procedure but does pose risks to what otherwise could be simply annoying floaters.

Trigger Finger – What Causes My Finger To Lock And Catch And Get Stuck?

Trigger finger is a common problem in the hand that causes locking and pain in the involved finger or thumb. The technical name used to describe trigger finger is stenosing tenosynovitis. Stenosing means a narrowing of a tunnel or tube-like structure (the sheath of the tendon). Tenosynovitis means inflammation of the tendon.

Your fingers flex (make a fist) and extend (straighten out). Two sets of tendons make this possible – flexor tendons (on the palm side) and extensor tendons (on the back side of the hand). Trigger finger involves the flexor tendons of the hand.

Flexor tendons are normally smooth, white bands of tissue that start as muscles in the forearm and connect to the bones in your fingers. As they go from the forearm into the wrist and fingers, they pass through tight tunnels (sheaths) that keep them close to the bones of your hand.

The sheath is lined with a lubricating tissue called synovium. This tissue sits between the tunnel wall and the tendon. Trigger finger is caused by inflammation of the synovium, enlargement of the tendon, and thickening or narrowing of the sheath itself, usually in the area of the palm closest to the fingers.

The name of the tendon sheath in this area is the “A1 pulley”.

These problems with the tendon’s lining, the tendon, and the sheath, all combine to make it hard for the tendon to glide smoothly through the tunnel as your finger bends and straightens.

As the patient tries to flex (bend into a fist) the affected finger, the enlarged tendon has trouble passing underneath the sheath’s tight opening. When the patient grips tight enough, the swollen tendon is suddenly pulled through the sheath with a painful snap, which may lock the finger in a bent position (it won’t straighten except with help from the other hand). The action of straightening the finger from its locked position creates another often painful snapping sensation as the enlarged tendon passes back through the tunnel.

What are the Causes?

The cause of trigger finger cannot be known for sure in all cases. Work or hobby activities that involve repetitive gripping or holding tools strongly for long periods of time may strain the tendons and cause swelling and irritation of the tendon synovium (lining) or the tendons themselves.

Some patients with other medical problems like diabetes, rheumatoid arthritis, or gout may develop trigger fingers more frequently than the average person.

What are the signs and symptoms?

Pain in the area of the inflamed tendon lining or sheath may be the first sign of trigger finger. Sometimes the pain may shoot or extend into the finger on the top side (back side of the hand), but is usually focused on the palm side at the base of the finger or thumb.

Painful locking or snapping of the finger is the next stage of symptoms that most patients experience. This is often worse in the morning but may be worse at the end of the work day.

When symptoms have been going on for several months, the locking may stop, but patients may be left with a stiff finger that does not bend or straighten like it used to. The pain may or may not subside as the locking stops.

How is it treated?

Non-surgical treatment is a good first step in treating trigger finger. Surgery may be a reasonable first step if the finger is locked in a painful, stiff position and cannot be “unstuck” by the patient without extreme pain. This is unusual.

The first step in treatment is to avoid or decrease the activity that seems to aggravate the locking and pain. Warm water soaks, anti-inflammatory medicines, and over-the-counter rubs and creams may help with the symptoms of trigger finger.

A steroid injection may also be recommended. The injection places a small amount of anti-inflammatory cortisone directly where the problem is – in the tendon sheath. This reduces swelling and inflammation in the tendon sheath and may cure the problem forever. Success rates with injections are close to 60% in most cases.

Surgery may be appropriate when these non-surgical treatments fail to cure the problem. I usually recommend no more than two steroid injections in the same finger, but this rule is not absolute.

Trigger finger surgery is outpatient surgery. It is done under local anesthetic, with or without sedation. An incision is made in the palm of the hand at the base of the finger or thumb.

The surgeon cuts the swollen, tight tendon sheath at this level, and this frees up the tendon to glide smoothly through the finger.

After surgery a small dressing is placed on the hand to protect the incision but allow flexibility and motion for the fingers. In most cases the dressing can be removed in five days and the patient may lightly use all the fingers the day after surgery. If there are sutures on the outside of the skin, they are removed in 10 to 14 days.
