Dry Eye Symptoms and CPAP Therapy – An Eye Doctor’s Perspective

The causes of red eyes can often be a mystery. Patients have reported dry eye problems induced by CPAP machines. Obstructive Sleep Apnea Disorder causes interruption of breathing at night in short repeated intervals. The resulting low levels of blood oxygen saturation cause daytime sleepiness and fatigue. It is often accompanied by very loud snoring and the affected person may awake with “air starvation” several times in a night. Sleep Apnea can result in serious health problems and mortality if untreated over time. Continuous Positive Airway Pressure, or CPAP, is used as the standard of care treatment. A CPAP keeps the upper airway open by providing a constant flow of air delivered through a face mask or “nasal pillows worn while sleeping. Compliance is frequently reported to be low with CPAP machine due to various problems. Resolving these issues is critical to your well being and continued use of a CPAP machine.

Eye problems reported with CPAP therapy include dry eyes, red eyes, swollen eyes (skin around the eyes), and redness of the eyelids and face around the eyes. – Worn nasal pillows could start to blow air towards the eyes.

– It is postulated the air pressure could induce dry eye by forcing air though the tear puncta openings. Punctal plug occlusion by an Eye Doctor can be tried with temporary collagen punctual plugs to rule out this possibility.

-Sleep apnea patients also have an increased frequency of floppy eyelid syndrome. Floppy eyelid syndrome is a lax upper eyelid that will evert (flip inside out) easily. Patients with untreated sleep apnea often sleep on their side face down on a pillow in an unconscious effort to keep their airway open. Due to a loose eyelid the lid flips up exposing the conjunctival tissue lining. This constantly rubs on the pillow case causing mechanical irritation and exposes the tissue to any allergens that are on the pillowcase. The conjunctival tissue can become chronically inflamed with papillary conjunctivitis and a red eye upon awakening. Treatment of the sleep apnea will help resolve the problem since there will be a reduction in rolling face down. There is no longer hypoxia (oxygen deprivation) and the mask itself provides behavioral changes in sleep posture since it protrudes from the face. Treatment of any remaining dry eye symptoms is still essential.

– Occasionally an individual will have dry eyes already or sleep with partially opened eyes which will make leaky masks more symptomatic. Treating the primary Dry Eye Syndrome is essential in this case and should be done in consultation with your Eye Doctor.

– Some people may be more sensitive to the pressure of the mask under the eyelids on a repetitive basis and could benefit by using different shaped masks on subsequent nights. The fit of a mask is so important this may not be an option and your Doctor may need to use prescription ointments (usually after removing the mask).

– Supplemental treatment by your Eye Doctor may be needed for the Eye dryness, artificial tears such as Thera Tears, some prescription eye drops.

– Your Optometrist may also prescribe eye treatment for the mask induced red eye conjunctivitis with eye medications like Patanol.

Hemroids, Hemorrhoids, Haemorrhoids or Piles – One And The Same Symptoms

Before moving on to other symptoms of hemroids, we need to know the differing medical words that define a line demarcating the boundary of the rectum and the start of the anal canal. Known variously as the pectinate line or dentate line or anal verge or anorectal junction, this line separates internal (in the rectum) and external (on the anus) hemroids. The differentiation that is more important to the hemroid sufferer is that there are somewhat fewer pain nerve endings above the line, verge or junction and a great number outside the boundary. In comparison, therefore, external hemroids may result in excruciating pain whilst internal hemroids may be painless. This pain element affects the decision-making when selecting between mainstream or alternative treatment methods.

Symptoms Of Internal Hemroids

The most typical symptom of internal hemroids is rectal bleeding, also known as bowel movement bleeding or blood in stool. Toilet tissue may be stained red, stools be interspersed with blood droplets and the toilet bowl be coloured red.

Another distinct symptom is the bulging movement of the internal hemroid during bowel motion. Medical researchers classify bulging internal hemroids according to 4 stages of prolapse or protrusion:

– a first-degree hemroid bulges into the anal canal;

In the extreme, they become strangulated hemroids (deprived of blood supply) that require urgent medical treatment as the adjoining tissues may be imperilled.

The agonizing urge to return to the toilet bowl right after stools have just been purged, anal lumps, rectal lumps and skin itches are other common symptoms of internal hemroids. The feeling of unfinished defecation is enhanced in cases where the hemroid is extremely large and in the proximity of the anal opening.

Symptoms Of Hemroids – External

One clear differentiation between external and internal hemroids is that anal pain is more acute in the former as they arise outside the dentate line on the anal region where pain nerve ends abound.

Blood clots or thrombuses tends to occur with external hemroids resulting in the body’s blood circulatory system being retarded. This thrombosed hemroid can cause detrimental inflammation in the whole anal and rectal regions.

Unlike internal hemroids which can often go undetected due to the lack of overt symptoms, external hemroids can create persistent irritation. Cleaning the anal region is understandable but doing this excessively only inflames the hemroid and sets up a vicious continual cycle.

Juvenile Court in New Jersey

Juvenile cases in New Jersey differ greatly from cases involving adults. The goal of the juvenile justice system, the rights which juvenile defendants have, the procedures which police and courts must follow, the facilities in which juveniles are detained, the roles of the defense lawyer and the judge, and many other aspects of juvenile jurisprudence are all significantly different from the adult criminal system.

Even the Juvenile Court is separate. Juvenile cases are handled in the Family Division, not the Criminal Division, of Superior Court. In a growing number of counties, such as Essex, Family Court matters are heard in a separate building from the criminal courts.

The goal of Juvenile Court is to rehabilitate. By definition, the adult penal system contains an element of punishment. The juvenile system, on the other hand, is designed to rehabilitate the youth, rather than punish the criminal act. Thus, the case will not be called “State vs. Jane Doe”, but “The State of New Jersey in the Interest of Jane Doe, a juvenile.”

A juvenile case begins with a determination of probable cause. When a person under the age of 18 is accused of committing an offense, the matter is brought to a court’s attention. This is usually the municipal court, and the matter is brought usually, although not always, by the police. Then, a judge or court official such as the Court Administrator or Clerk must determine that there is probable cause to think that the juvenile has been delinquent, s/he can be taken into custody.

Juvenile charges are brought in the county where the juvenile resides, rather than where the offense occurred. In appropriate cases, a judge will grant the juvenile’s lawyer’s motion to transfer the case to the county of the offense. While the New Jersey’s twenty-one counties should strive for uniformity in the handling of juvenile cases, this is not always achieved.

Juveniles are not arrested; they are detained. They are, according to law, taken in into custody for their own protection. Parents or guardians must be notified without delay. Juveniles may not be detained in the same facility, or even the same police car, as adult suspects. They will be given a “detention hearing” by the morning following their detention to determine whether it will be safe to return the juvenile to the custody of the parent or guardian while the matter is pending.

While in custody, a juvenile is brought before a judge at least once every three weeks, to review the need for continued detention. Sometimes juveniles are released to home, but subject to home confinement, electronic monitoring, curfews, continued employment or school, or other conditions imposed by the court.

A form called a “5A Notice” is sent to the parent(s) or guardian early in the case. This is the Family Court’s summons for the parent(s) and juvenile to appear and also to file an application for a Public Defender. The form is a bit confusing, and the various counties treat the 5A hearings differently.

A juvenile must have an attorney, and a Public Defender will be appointed for a juvenile whose family cannot afford to retain a “private” lawyer. Public defenders are lawyers who are available to low-income families at little or no cost. They are usually experienced in juvenile law and are familiar with the courts. Many of them are excellent lawyers. In most NJ counties defendants and their parent(s) or guardian(s) must appear at the “5A Hearing,” even if they intend to hire a lawyer, as the state or the court may require “intake” information or procedures such as fingerprinting.

Juveniles have no right to a trial by jury; juvenile trials are heard by a judge without a jury. The rules of trial in juvenile court are different from adult court, and at sentencing, the judge has many options that are unavailable to adult defendants. Most juvenile cases are settled, however without a trial.

New Jersey’s juvenile justice system provides many diverse options for rehabilitating the youth. The system strives to understand each defendant and to treat each as an individual. In counties such as Essex and Union, where there are several judges sitting in the Juvenile part, repeat offenders are usually scheduled to appear before the same judge, often with the same prosecutor. In appropriate cases, there are programs and plea bargains that allow for dismissals and downgrades, intensive supervision, probation, job training, substance abuse remediation, pyromania counseling, anger management, and much more. An experienced juvenile attorney can often help fashion a resolution that makes sense.

Not all juveniles are tried in juvenile court. Some are “waived up” to adult court where they receive adult court treatment and are exposed to adult penalties. Among the factors a court will consider in determining whether to waive a juvenile up to adult court are the gravity of the crime, the juvenile’s age, history, gang affiliation, and the involvement of “adult” instrumentalities such as firearms, motor vehicles, and sexual activity. Offenders convicted as juveniles are not sent to prison, but to places with names like The Training School for Boys, and custodial juvenile sentences do not exceed five years. Cases that are waived up expose the youth to penalties ranging to twenty years in prison, and even more.

Juvenile records, that is, records of the juvenile offense, “disappear” once the juvenile turns eighteen. That is not exactly true – the records remain available for certain purposes, but may not generally be disclosed. Subject to some very rare exceptions, no employers, schools or government officials may inquire about a juvenile record. Juvenile records may be expunged, later on, in most cases. Experienced New Jersey juvenile lawyers know that the juvenile justice system favors the youth who make efforts to improve, and who shows promise for a law-abiding future. Supportive families, success in school, part-time or full-time employment, involvement organized community, religious or athletic activities all suggest that the youth has a significant likelihood of rehabilitation. Juveniles with these advantages benefit most from the non-penal philosophy of the juvenile system.

Too Much Caffeine in Your Diet? Try Some L-Theanine

It’s time for the 3rd ingredient in the “Stress Less for Poker Success” formula. After this one, only two more ingredients to go before you have the perfect formula for a stress free life. Ah, you can almost feel the relief already.

Before I dive into the third ingredient, I just want to give you a quick recap on the first two ingredients.

Personally, I use Rhodiola rosea only during times of intense stress. I’ll typically begin using it two to three days before the expected stress and continue using it 3-5 days after it has ended. It probably has one of the largest amounts of clinical evidence supporting its benefits in terms of nutraceuticals. In my opinion, it’s an excellent, well-studied nutraceutical.

Bacopa is considered my long-term stress supplement. I use it 365 days a year. It helps to neutralize the stress of everyday life. On top of that, it also has some cognitive-enhancing abilities. What it lacks in clinical evidence, it makes up for in real-world feedback. You can’t argue with a supplement that’s been used for hundreds of years in Ayurvedic medicine.


I think it’s blindly apparent that we live in an energy-addicted world today. It seems that we can’t get enough energy. We have Red Bull which quickly became a billion dollar drink. Then there was 5 Hour Energy, for those that thirsted for energy yet wanted to avoid caffeine. Now there’s umpteen different energy drinks along with hundreds of different energy pills. We’re definitely a society that’s overdosed on energy. C’mon people, relax a little.

That’s exactly what the dietary supplement industry is predicting for the next couple of years: a complete reversal. Now longer will we search for energy, but we’ll NEED relaxation. Thus, they predict that supplements that affect GABA, a relaxing neurotransmitter, will be the next big craze. What caffeine is to energy, GABA is to relaxation. Ativan, Xanax, Restoril, etc all work via GABA receptors in the body. Heaven knows, those prescription drugs are already a hot item.

This is why you’ll probably hear a lot more about L-theanine in the next couple of years. In fact, don’t be surprised if a major energy drink begins adding l-theanine to its mix. Get used to seeing “enriched with l-theanine for a relaxed energy.”


L-theanine is an amino acid most commonly found in tea. In fact, the relaxing effect of tea is primarily a result of l-theanine.

Mechanism of action

It works through a variety of mechanisms. However, the 4 that are most important to us are:

Its ability to increase the levels of serotonin, dopamine, and GABA activity. These three neurotransmitters are extremely important for relaxation, mood, and energy.

Its ability to increase alpha brain wave activity. Alpha brain waves are thought to correlate with a relaxed, yet alert state in humans.

Its ability to counteract some of the negative stimulant effects of caffeine. In fact a couple studies have shown that a combo of l-theanine and caffeine is superior to caffeine alone with regards to mental performance.

Its ability to block the harmful effects of excitatory neurotransmitters that can destroy brain cells.


The typical dosage is 200mg taken anywhere from 1-3 times daily. I’ve found this to be almost exactly what I need. I may need a little larger dosage if I’ve taken a lot of stimulants during the day. However, always titrate to a dosage that works for you. It’s extremely safe so you won’t have to worry about overdosing.

Why I like it

It’s extremely safe and well-tolerated. That’s always of the utmost importance.

Just like Rhodiola rosea, I don’t use this every day. Typically, I’ll only use it during times of intense stress or when I’ve consumed a large amount of stimulants. I’ve found it to be one of the few supplements to counteract the negative effects of stimulants. It works.

In addition, even though there have only been one or two studies to show this, it supposedly has some cognitive-enhancing abilities. As we all can agree, this is a benefit we’re looking for.

Why for poker players

Compared to every nutraceutical I’ve talked about thus far, l-theanine probably will be most beneficial to poker players. Why? I’ve noticed poker players are energy junkies. Caffeine, Red Bull, Ritalin, etc are commonly used, especially by the on-line players. Energy. Energy. Energy. Take a capsule of l-theanine, and not only do you block the negative effects associated with stimulants, but you also improve their cognitive-enhancing abilities. That’s called synergy.

Poker players have to remember that being over-stimulated is just as bad as being mentally fatigued. Both can cause mental mistakes. Playing energized and overzealous is inferior to playing relaxed and focused. Relaxation is the missing ingredient for the endless amounts of energy products available today. Please remember, the energy drinks and products on the market today are designed for physical energy (working out or waking up after a long night) not for mental energy. It’s like trying to fit a square peg in a round hole.

In addition, the typical energy supplement further increases your stress response. The goal behind these products is to get you in that flight-or-fight mode, aka stress. Thus, you’re putting an additional amount of stress on your mind. Not good. Remember, stress kills brain cells.

Relax a little man. It may just be the answer you were looking for. Balancing relaxation with stimulation is the key to success at anything that requires mental performance.

Avascular Necrosis – Ayurvedic Herbal Treatment

Avascular necrosis is the term used for the death of bone tissue due to a lack of blood supply, leading to fragmentation of the bone and eventual collapse. The head of the hip bone is most commonly affected due to this condition, but other bones may be affected as well. The most common symptom is pain in the local area, which is aggravated by movement. Trauma, long term use of steroids, excessive alcohol intake, and chronic diseases like blood disorders and auto immune disorders are the causes for this condition. This disease is progressive and may require life long management.

The Ayurvedic treatment of avascular necrosis is aimed at treating the pain, increasing the blood supply to the affected bone and preventing bone destruction. Medicines like Yograj-Guggulu, Triphala-Guggulu, Maha-Rasnadi-Guggulu, Maha-Vat-Vidhvans-Ras, Vat-Gajankush-Ras, Maha-Rasnadi-Qadha, Dashmoolarishta, Nirgundi (Vitex negundo), Rasna (Pluchea lanceolata), Tagar (Valeriana wallichii), Guggulu (Commiphora mukul), Shallaki (Boswelia serrata) and Khurasani ova (Hyoscyamus niger) are used to reduce pain and stiffness in the affected bone and joint. Medicines like Tapyadi-Loh, Ekang-Veer-Ras, Kaishor-Guggulu, Trayodashang-Guggulu, Maha-Manjishthadi-Qadha, Saarivadi-Churna, Manjishtha (Rubia cordifolia), Saariva (Hemidesmus indicus), Patol (Tricosanthe dioica), Patha (Cissampelos pareira), Musta (Cyperus rotundus), Kutki (Picrorrhiza kurroa), Guduchi (Tinospora cordifolia) and Amalaki (Emblica officinalis) are used to increase blood supply to the affected bone. Medicines like Laxadi-Guggulu, Praval-Panchamrut-Ras, Shankh-Bhasma, Shrung-Bhasma, Laxa (Purified wax) and Asthishrunkhala (Cissus quadrangularis) are used to strengthen the bone and prevent collapse of the bone and joint.

Local application can be done on the affected area using medicated oils like Mahanarayan oil, Mahamash oil, Chandan-Bala-Laxadi oil and Vishgarbha oil. This is followed by a localized steam fomentation using medicines like Dashmool-Qadha and Nirgundi-Qadha. Repeated courses of a medicated enema known as ‘Tikta-Ksheer Basti’ are given to prevent damage to the affected bone. This enema is prepared by boiling milk with medicines like Guduchi, Patol, Nimba, Indrayav and Chirayta (Swertia chirata).

If left untreated, avascular necrosis can lead to severe pain and loss of movement within 2 to 5 years. Early diagnosis and prompt initiation of treatment are therefore very important. Ayurvedic treatment can help to reduce pain and speed up the recovery process in this condition. All people affected with this condition should be under the regular care and supervision of an Orthopedic Surgeon.

Schedule IV- This category of drugs have a very low abuse tendency and have a low chance of addiction. This category contains drugs like Valium, Xanax, Rohpnol (more often known as the “date rape” drug). Like the others, this drug is available with a prescription only and is not always available at pharmacies.

Schedule V- This category has the lowest abuse tendency of all of the schedules as well as the lowest addiction rate. Schedule V drugs are like cough suppressants with Codeine. These drugs are usually regulated by stores and don’t require a prescription of any kind.
