The Many Styles of Midfacial Implants For Aesthetic Facial Augmentation

The use of synthetic implants for augmenting skeletal deficiencies of the chin, cheeks, and jaw angles are common plastic surgery procedures. While these are common locations for implants, there are many more implant styles that can be used for facial enhancement of the midfacial area.

The bones of the midface have complex convexities and concavities from the prominences of the cheek to the concave surface of the maxilla. As a result, there is no one implant that can be used for all of them. Today’s midfacial implants have nine (9) different styles to meet these complex anatomical needs.

Cheek implants are the most frequently used facial implant next to that of the chin.. There are four different styles to choose. The standard cheek implant, also known as a malar implant, fits on top of the cheek bone and has a broad surface area that covers most of the external bony prominence. The two styles of this standard cheek implant differ in that the more extended version has a portion that goes up higher to make a smooth transition into the lateral orbital bone. The choice between one style vs. the other is based on how much flattening the cheek has and whether it extends up into the orbital area.

The submalar cheek implant is not the same as the malar implant. It is indicated when the cheek soft tissues are a little saggy with age and the area under the cheek prominence is a little sunken in. This implant fits on the underside of the cheek bone which helps lift up sagging tissues and fills out the sunken area under the cheek. The submalar cheek implant comes in two styles which differ in how much fullness is added to the submalar space.

The tear trough implant is a specialized lower orbital rim implant. It fills out the depressed suborbital groove that some people have naturally or develops from aging and tissue atrophy. This groove runs like a gutter from the inside of the eye downward. Instead of fat injections for augmentation of this area, a carefully placed tear trough implant provides a permanent solution.

The pyriform aperture implant fits along the bone which forms the side rim of the nasal cavity. It is designed to build out the base of the nose underneath the nostril. I have most commonly used this implant in secondary cleft surgery to build out the deficient maxillary bone where the cleft went through. But it is also helpful as part of rhinoplasty surgery when one has a flatter face.

The premaxillary or peri-pyriform implant builds out the entire base of the nose from one side of the pyriform aperture to the other. This includes anterior nasal spine area as well. For very flat midfaces, this implant can really help provide augmentation to the nose and upper lip.

The nasolabial or melo-labial groove implant fits onto the bone to the side of the nose and helps soften or build-out the very deep nasolabial fold or groove. Rather than using temporary injectable fillers placed beneath the skin, this implant placed on the underlying bone serves to provide the same push.

The Lefort or maxillary implant, the least common of all midfacial implants, is designed to be used during or after a LeFort I osteotomy. This implant would fit above the osteotomy line so that the midface would not look more deficient as the maxillary teeth come forward.

All midfacial implants, while being very different in shape and indication, share two common features. First, they are all placed from an incision inside the mouth so no skin incision is ever needed. Secondly, they are all best secured into position with a screw as their position is critical in getting the desired esthetic result.

ADHD Herbal Remedies – Chinese Medicine

The Chinese have a very old medical tradition that makes use of herbs and homeopathic remedies to treat a number of disorders. Tiaoshen liquor

Tiaoshen liquor is made up of skullcap root, Oriental white ginseng extract, and Chinese thorowax root. The tincture was especially formulated to control hyperactive behavior in children, and its claims have been tested by Chinese scientists. 100 children with ADHD Hyperactive Type who participated in the studyreceived a dose of Tiaoshen liquor every day for several weeks, and the trial ended with positive results. Parent observations reported that 94% of the participants were less hyperactive, had improved attention spans, and performed better at school.

Yizhi syrup

Also known as wit-increasing syrup, yizhi syrup is made of ingredients similar to those in Tioashen liquor. When tested on 66 hyperactive children, 84% experienced reduced ADHD symptoms, improved behavior, and better academic performance. Through urine sample testing, researchers confirmed that yizhi syrup can increase the production of dopamine and norepinephrine.

Chinese medicine versus Western medicine

One study made a direct comparison between the effects of Chinese medicine and Western medicine in treating ADHD. 80 children were given an herbal concoction made up of herbs known to reduce hyperactivity and invigorate the spleen: Scutellaria baicalensis, Ligustrum lucidum, Lophatherum gracile, Astragalus membranaceus, Codonopsis pilosula, Bupleurum Chinese, and an herb called “thread of ivory.” The rest of the children received 5-15 mg Ritalin tablets two times a day.

The children were monitored for three months and were subjected to EEG scans, IQ tests, and symptom scales. At the end of the trial, researchers observed that 29% in the Chinese medicine group gained 10 IQ points, had normal EEG levels, and no longer met the diagnostic criteria for ADHD. 58% gained some improvements, while 13% had no improvements at all. The Ritalin group had similar results: 30% no longer met the diagnostic criteria, 60% showed some symptoms, and 10% had no improvements.

What this study shows is that in terms of effectiveness, there is very little difference between Chinese medicine and Western medicine. In terms of side effects, however, Ritalin and other ADHD medications are known to cause permanent damage with long-term use. Aside from rare allergic reactions, people do not experience any serious side effects from using Chinese herbs. So if you are looking for natural remedies for ADHD, consult a certified practitioner of Chinese medicine.

The Ordeal of Vernon Oneal: The Story of President Kennedy’s First Casket

November 22, 1963. Even before President Kennedy’s near-lifeless body was removed from the presidential limousine at Parkland Hospital, United Press International’s initial reports that “three shots were fired at President John F. Kennedy’s motorcade today” was racing across teletype machines nationwide. Holding a transistor radio to his ear, one among the millions of Americans who monitored the breaking news was Vernon B. Oneal, a Dallas funeral director whose business also included an ambulance service.

At approximately 12:46 CST, only sixteen minutes after JFK was shot, and 47 minutes before Assistant White House Secretary Malcolm Kilduff officially announced the President’s death to the world, Oneal’s telephone rang. It was Secret Service agent Clint Hill calling from Parkland Hospital, advising Oneal to select his best casket and transport it to the hospital “as soon as possible.”

“Is it for the President?” Oneal asked.

“Yes,” Hill replied, “it is for the President of the United States.”

Grasping the urgency of Hill’s directive, Oneal bolted into his showroom and selected his signature model, an eight hundred pound solid bronze casket named the “Brittania”. Manufactured by the Elgin Casket Company, it was double-walled and could be hermetically sealed. Fit for a president or king, Oneal’s price was $3,995.

As his profession dictated, Vernon Oneal was prepared to discreetly comfort and assist the bereaved Kennedy family in every way possible. At Parkland, he whispered words of sympathy to Jacqueline Kennedy. He and two of his associates were then assisted by nurses in carefully wrapping JFK’s remains in several sheets and plastic, hoping the blood and brain matter still seeping from the massive wound in the President’s head would not stain the casket’s plush satin shirring.

Before departing Parkland Oneal dutifully stood by Mrs. Kennedy and the casket as the late President’s White House aides and Secret Service agents endured an ugly shouting match with a local official named Dr. Earl Rose, who insisted that JFK’s body must, by law, be held for autopsy in Dallas County. Oneal had every right to assume that his services would continue to be required all the way to President Kennedy’s burial site, and he wanted Mrs. Kennedy to be assured of his loyalty and respect; the full scope of his establishment’s services were at her solicitation, and the man was determined to go to any avail, whether the funeral be held in Washington or Massachusetts, to satisfy her every wish.

Then, suddenly, Oneal’s hopes were dashed. The Secret Service agents and JFK’s most loyal aides had no sooner brushed Dr. Rose aside when the casket containing the assassinated President was precipitously loaded into the rear of his Cadillac hearse, the same vehicle he and his personnel used to deliver the bronze coffin to Parkland Hospital. It had been Oneal’s intention to drive Kennedy’s body directly to his funeral home for embalming and the scheduling of funeral arrangements, but the Secret Service commandeered the hearse and an agent advised him to follow in another car, neglecting to tell Oneal that their true destination was Air Force One at Love Field airport.

Along with a police motorcycle escort, three cars began pulling out of Parkland’s service road: Oneal’s hearse, a car full of Secret Service agents and JFK’s aides, and the last carrying Oneal and two of his employees. The funeral director sensed something serious was amiss when he observed his hearse turn left in the direction of the airport rather than right to his mortuary. Agents in the second vehicle radioed their counterparts at Love Field, instructing them to permit “the first two cars only” beyond the airport fence near Air Force One. Under no circumstances would Oneal and his employees, or any other vehicle, be allowed to enter the area near the presidential jet.

Sure enough, agents let the first two cars pass through a fence within view of the aircraft, but halted Oneal’s sedan. The undertaker was furious, and justifiably so. The martyred President was inside his casket and his coach, both supposedly bound for his funeral home. The agents ignored his protests, leaving Dallas police officers to assure the beleaguered businessman that the hearse would be returned to him the moment Air Force One took flight. Vernon Oneal felt like a spare tire. The United States government had used him to the extent their primary requirements were satisfied – later dumping him at the gate.

Perhaps the ultimate insult to Vernon Oneal involved payment of the coffin itself. He repeatedly sent a $3,900 bill to Jacqueline Kennedy for nearly a year but she never replied. Finally, fourteen months after JFK’s assassination, in January 1965, the federal government paid him a sum of $3,160. But by then Oneal was living a public relations nightmare; his attempts to collect payment from Mrs. Kennedy were widely published and his mortuary suffered an agonizing 50 percent drop in business.

Interestingly, Oneal’s casket was not used in the interment of John Fitzgerald Kennedy at Arlington National Cemetery. Its handles and finish were damaged at Love Field as the Secret Service agents hurriedly struggled to maneuver through Air Force One’s narrow doorway. In addition, blood from Kennedy’s head wound did ooze through the protective sheets and plastic, ruining the casket’s satin interior. The flaws were noted prior to the autopsy at Bethesda Naval Hospital, resulting in JFK’s closest aides picking out a new, more expensive casket at Gawler Funeral Home in Washington’s Georgetown district, being a more exquisite example made of five-hundred-year-old African mahogany. Gawler also prepared the late President’s body for burial following an autopsy at Bethesda.

For many years the whereabouts of the Oneal casket remained a closely-guarded secret. Understandably, the Kennedy family didn’t want the artifact to fall into the wrong hands and become a gruesome sideshow relic. It was eventually disclosed that, in 1966, the casket was purposely loaded aboard a C-130 Air Force transport plane and unceremoniously deposited into the depths of the Atlantic Ocean, never to be seen again.

Changing VO2max by Breathing Differently at Rest

VO2max is the key factor in athletic endurance. Currently it is the best predictor of performance in many sports. However, only a very few people know that VO2max is tightly linked with the breathing pattern and body oxygenation of the athlete at rest. Hence, if the athlete changes their unconscious breathing pattern at rest, his VO2max will also change.

Patients with heart disease, diabetes, chronic fatigue, and many other diseases have low body oxygen content 24/7. They suffer from tissue hypoxia (low oxygenation), excessive anaerobic metabolism, and high blood lactate already at rest. Hence, when they have even light exercise, anaerobic energy production sharply increases, lactate quickly rises, and experiences of pain and physical exhaustion are the outcomes.

Oxygen delivery cascade (from outer air to body cells) and tissue oxygenation depend on our breathing. How? Hundreds of physiological studies proved that the more we breathe at rest, the less oxygen is delivered to tissues. Consider the basics of oxygen transport.

When we breathe more than the medical norm (which is 6 l of air per minute), our arterial blood is almost fully saturated with O2 (up to about 98%) and cannot get much more additional oxygen. However, during overbreathing we exhale (or lose) more CO2 and hypocapnia (CO2-deficiency) in all body cells causes 2 negative effects in oxygen transport:

1) Vasoconstriction: our blood vessels are CO2-sensitive and CO2 is a powerful vasodilator. (This effect explains, according to tens of medical publications, why we can easily pass out or faint due to heavy voluntary hyperventilation.)

2) The suppressed Bohr effect: the Bohr law governs oxygen release in tissues; O2 is mainly left in those parts and organs of the body which have high CO2 content (or metabolically active). When we hyperventilate, less O2 is unloaded in all tissues since CO2 is a chemical catalyser of this release.

Indeed, dozens of published studies found that ALL sick people with asthma, heart disease, diabetes, epilepsy, cystic fibrosis, etc., breathe at least 2-3 times more air than the norm and suffer, as a result of overbreathing, from tissue hypoxia (see my other articles for these references.) They usually breathe about 15 l/min instead of 6, while utilizing less than 10% of inhaled oxygen (over 90% is breathed out).

During maximum exercise humans can have up to 150 l/min for minute ventilation. Thus, if sick people start mild exercise and they require 10 times more energy than at rest, they will breathe about 10 times more or about 150 l/min, but this is near the human physiological limit.

People with normal minute ventilation at rest (6 l/min) during the same mild exercise (10 times energy expenditure increase) will breathe about 60 l/min. Such breathing is much lighter. It indicates their better fitness and can be done strictly through the nose.

It is not a surprise then that the amount of freely available oxygen in normal people is high or about 40-60 s (breath holding time after exhalation and without any pushing themselves for better numbers), while sick people can hold their breath for this special test (again, it involves usual exhalation and no stress at the end of the test) only for 10-15 s of oxygen. This completely confirms the main practical law of respiratory physiology: more breathing means worse tissue oxygenation.

However, if an athlete learns how to breathe much less than the norm 24/7 or only about 2 l/min at rest or during sleep for minute ventilation, their body oxygenation will be about 2-3 minutes. During the same mild exercise (with tenfold metabolic rate) their minute ventilation will be only about 20 l/min (or slightly more than the sick people breathe at rest). There are, indeed, two well known facts that (a) fittest athletes breathe less during exercise and (b) when an athlete is in his peak shape his breathing during races is much lighter.

Such breathing retraining can be achieved within 1-2 months. It will result in very light breathing at rest (only 3-4 breaths per minute even during sleep) and greatly increased VO2max values, up to 10-15%.

This short respiratory review in exercise physiology explains why numerous Soviet elite athletes, including many Olympic champions, used the Buteyko breathing method, as a secret weapon, for superior sport performance. Tens of Australian Olympians also learned the same breathing technique during recent years.

Breast Cancer, Tamoxifen and Hair Loss – "One Woman’s Solution!"

On what should have been a beautiful late spring day two years ago, soon turned into tragedy for two women (Helen and Denise) in their early 40’s when they both were diagnosed with invasive-

hormone-receptor-positive breast cancer.They were very much alike, these two women. Both were happily married. Both had three children and both held down full time jobs. They were two bright and ambitious women who without realizing it, entered into a bond on that numbing, spring afternoon.

Both underwent several lymph node removals and a lumpectomy. Both endured the effects of 6 weeks of radiation therapy. Neither had to endure the grueling effects of chemotherapy. Both were determined to beat this monster that had invaded their bodies. Both bonded as close friends and when the feelings of hopelessness began to creep into their thoughts they had each other as confidants to lift up their spirits. They stood together, without ever having to alarm their families of their inner fears that their cancer would return.

With the conclusion of their treatments they both resumed their everyday routines and tried to reclaim a sense of normalcy back into their lives. They both were prescribed Tamoxifen for the next five years. Tamoxifen is very effective in preventing recurrence in invasive hormone-receptor-positive breast cancer. Unknown to Denise, she was not yet finished with the challenges that this demon called cancer forced her to face.

Although, Tamoxifen does have some side effects it is relatively safe and thousands upon thousands of women take it daily without experiencing any side effects at all. Helen scarcely noticed any changes while taking the drug. She returned to work part-time at first, but quickly resumed her role as wife, mother and full time employee. One would scarcely believe that she had ever undergone cancer treatments and she glowed once again with signs of good health.

Denise on the other hand didn’t fair quite as well. She noticed immediately that her hair was starting to fall out. Sometimes averaging 200 strands a day. After two weeks of being on Tamoxifen and experiencing this rapid hair loss she was seriously considering discontinuing its use. The radiation therapy that was used to spare her life had left her tired and listless. Tamoxifen that was used to prevent the cancer from returning, was thinning her hair and most of all destroying her self esteem.

She researched the prescription medication Rogaine, but found the chemical name (minoxidil) had side effects as well. Rogaine was also very expensive and time consuming. She had contemplated ovary removal. Her reasoning being, she was positive that her family was complete. She quickly dismissed that thought not wanting to undergo anymore surgeries.

She then looked into Aromasin an FDA approved aromatase inhibitor. Aromasin was given to women with early stage disease, but who had already undergone 2 to 3 years of Tamoxifen. The next inhibitor that she looked into was Femara. Although, FDA approved for early stage breast cancer she needed to have completed 5 years of Tamoxifen. The third inhibitor was called Ariminex. This is given to patients with early stage disease and directly after surgery. After discussing this with her Doctor, she immediately switched from Tamoxifen to Ariminex.

Hair loss was the primary reason Denise switched from Tamoxifen to Ariminex. She had no other side effects with this drug, but as vanity would have it she just didn’t want to become bald. After taking Ariminex for just under four months she realized that she had traded one cosmetic side effect for another that was physical. Denise was still experiencing slight hair loss, but was now suffering from excruciating joint pain.

Her Doctor explained that by taking Tamoxifen, her continuing hair loss just might have been caused by the drug giving her a nudge into menopause. Women who enter menopause, because of the lower estrogen sometime experience hair loss. He also explained that everyone’s tolerance was different for these drugs and he could prescribe Femara for her instead of Ariminex. By this time Denise was taking extended release Tramadol for joint pain and felt exhausted and defeated. She was still not capable of returning to work, even though it had been almost a year since she had been first diagnosed with breast cancer.

Financially Denise and her family were barely making ends meet. Without her much needed paycheck, the household bills were accumulating. Her once vibrant marriage was suffering and the talk of their separation was only silenced by the tears in their children’s eyes. In one year everything she and her family had worked for appeared ready to come crashing down!

Denise was still contemplating whether she should begin taking Femara or stop taking all drugs altogether.

That morning she received a phone call that would change her life. It was Helen. Her confidant through the worst of times had called to find out how she was doing. It wasn’t long before Denise’s emotions came spilling out. Through tears and laughter Helen and Denise were once again reunited, and Denise was no longer fighting this battle, seemingly alone.

Helen who did and continues to work in the organic and natural food business, was saddened that Denise had not confided in her earlier on her struggles. She was delighted with the thought that she may be able to help her ailing friend.

Upon hearing Denise’s story it was clear to Helen that the only side effect that Helen’s brief taking of Tamoxifen had was hair loss. Because of those two simple four letter words (hair loss) the past year for Denise and her family had turned into a roller coaster of spiraling downward turns.

The Pros and Cons of E Ink Technology

A chorus of praise was heard amongst the avid book reader when E Ink technology emerged onto the market. Why? Well because we wouldn’t be reading our favorite novels on ereaders like the Kindle or Sony Reader without it. And now that the market has had pleasure of reading electronically in greyscale, the demand for color is there. Simply put, we want to see ebooks in color.

If you surf the internet for information on ereader technology, then you know that manufacturers are trying to discover a way to create a color version comparable to E Ink. Some of the disadvantages of this technology are:

• The user loses out on color publications. These include the popular graphic novels, photography books and art books.

• E Ink can’t render full-color illustrations, photos, or other images that add to the reading experience.

• Textbooks are incomplete for students because hyperlinks aren’t clickable.

• It doesn’t highlight in color, so it can be harder for the user to detect the previously highlighted text.

• It doesn’t translate magazines or cookbooks well.

Although there are these cons, E Ink does have some great positives. These include:

• It can last for many days or even weeks on a single battery charge.

• In comparison to LCDs, it is easier on the eyes, and mimics the appearance of a physical book

In order to meet the demand, companies are trying to come up with color technology that is as efficient as E Ink. During the CES in Las Vegas held in January 2010, companies presented different color technology to the consumer. Color ereaders have already hit the Japanese and Taiwanese markets as well, so hopefully we will be seeing models in the US soon. These companies are:

Pixel Qi – There isn’t a prototype yet, but Pixel Qi created a LCD technology that can be compared to OLPC laptop displays. Experts say that Pixel Qi’s technology may be better for netbooks.

Qualcomm Mirasol – A beautiful discovery, Mirasol mimics the iridescent colors we see in nature like from butterfly wings, peacock feathers or pearls. Mirasol worked well at the CES on a small screen, but how it will look on a larger screen hasn’t been determined yet.

Kent Display – Already being utilized by the Fujitsu FLEPia out of Japan, the Kent Display is created out of film that uses cholestric crystals; however, the refresh rate is very slow and the display panel is uncomfortably thick.

Companies like Amazon may hesitate to change the Kindle display from E Ink technology to one of the emerging color technologies. Obviously a full color ebook reading experience would delight us all, but we will have to wait and see if one of these companies pulls a rainbow out of their hat.

Kidney Disease – The Hidden Cause of Chronic Resistant Hypertension (High Blood Pressure).

The kidneys are the key organs that regulate systemic blood pressure. The kidneys are the bean-shaped organs at the back of the abdominal cavity located below the lower margin of the rib cage and the upper limit of the pelvic cavity. The two kidneys regulate blood volume, excrete acidic wastes, metabolize drugs, balance electrolytes and produce hormones. Yes, the kidneys does a lot more than making urine. In Chinese medicine, it is even believed that the force of the kidneys control the bones as well as fertility. In this article, I will focus on how renal disease can cause or provoke intractable high blood pressure.

Hypertension accelerates renal insufficiency. The opposite is also true. In physiology, it is always thought that diseased kidneys need high blood pressure to maintain tissue perfusion. This simply means that for the rest of the body tissues to keep on receiving nutrients and oxygen through the blood, the kidneys has to raise blood pressure. This is a natural survival mechanism which is incompatible with optimum health.

The renal vasculature is a low pressure bed. In other words, the kidneys are very sensitive to changes in blood pressure and tend to react with renal artery spasm (sclerosis) and raising blood pressure through the renin-angiotensin-aldolsterone mechanism. Don’t let this throw you, I will explain it.

Renin is the hormone that is secreted by the juxtaglomerular cells in response to decreased blood volume, low potassium, low adrenaline, or decrease renal perfusion pressure. Renin then converts a 10-amino acid protein (packaged in the liver) called angiotensinogen to 8-amino acid polypeptide called angiotensin I. Angiotension I is still inactive but is activated in the lungs by angiotensin converting enzyme (ACE) to the active angiotension II. Angiotensin II is a potent vasoconstrictor. It is said in many yogic teachings that balancing sun and moon, or pingala and ida, allows the awakening of kundalini, and thus the awakening of the higher consciousness that is felt at sahasrara, the seventh chakra. In fact, certain yoga teachings say that as long as either ida or pingala is dominant, sushumna nadi remains dormant or asleep and the power of kundalini cannot be awakened and so you cannot sense the awareness of sahasrara.

If we look at these definitions in terms of science we observe that the left and right hemispheres of the brain are each responsible for different functions.
