Blended Vision is a LASIK technique you might want to consider if you wear reading glasses or are near the age when reading glasses may become necessary. When this happens, most people need reading glasses. Inpatient medical coding: This refers to coding the records of patients who are required to stay in a hospital or any other healthcare unit for more than 24 hours, hence the name inpatient coding. Since the medical records of patients who are admitted to a hospital for treatment tend to be a lot more complex, this naturally makes the job of inpatient medical coders that much harder.

Due to advances in medicine, a lot of procedures that earlier required a hospital stay can now be performed on a same-day basis. What this means is that outpatient coding is a lot more prevalent than inpatient coding.

Another factor to consider when choosing between inpatient and outpatient coding is that employment opportunities may be more for outpatient medical coders as they can find jobs in a variety of healthcare settings that include hospitals, physician offices, diagnostic labs, outpatient care centers, nursing care facilities, etc.

Aspirants can also choose a medical billing and coding training program that provides them the skills for both inpatient and outpatient coding. However, it’s important to remember that most medical coding professionals start their careers as outpatient coders and then move up the ranks to inpatient coding.

How to Know When You Just Can’t Get Over Infidelity

I often get emails from folks who tell me that they doubt that they will ever be able to get over their spouse’s infidelity in a way that is healthy enough to ensure that they can move on. They really want to be to get over the feelings of hurt, frustration, and betrayal, but there is often always memories or doubts or issues that seem to get in the way of this. No one wants a life sentence like this, but people often fear that this is exactly what is going to happen. They often just can’t envision a future that doesn’t include the way that they feel right now.

I understand these fears. I felt them myself, but in the end, I was able to get over it. And, I exhibited many of the “signs” that people assume mean that you aren’t going to be able to move on. I’m often asked things like “how do I know that I’m at the point where I just can’t get over this? Part of me is hoping that with time, I will heal. But another part of me just knows that this is a deal breaker that I will never recover from. How do I know which one of these assumptions is true?” I will attempt to answer this question in the following article.

Anger And Frustration Do Not Always Mean That You Can’t Move Past The Infidelity: I often hear from people who tell me that they hate the spouse who cheated with a passion. They tell me that they think that these strong emotions mean that they will never be able to go forward. I understand this assumption but it often is not true. Almost every one is angry for quite a while. You need time to process things and it will take some time for your spouse to prove to you with their actions that you and your marriage can recover if that’s what you want.

And, strong negative emotions are actually not indicative of an inability to move on. The lack of emotions are actually more troubling. Strong emotions like anger, hurt, and fear are indicative of the fact that you still care very much about the person and the relationship – even if you wish you didn’t right now.

A Lack Of Trust After Infidelity Is Not Something That Can’t Be Overcome: On the other side of the coin, I sometimes hear from people who tell me that with time, their anger and hurt has started to fade some. They are starting to see their partner in a more positive light, but they just can not seem to restore the trust. They are always doubting and suspecting their spouse. They are always suspicious and this thwarts their ability to move on.

Again, restoring the trust is something that takes time. But more than that, if you aren’t getting what you need, you may have to ask for it. You might have to spell out for your partner that you want and need for them to check in, include you in any lengthy times away, and allow you access to their cell phone or computer if you feel the need to check up in order to make yourself feel secure.

A spouse who is serious about restoring your trust should not have a problem with this. Sure, it may be frustrating for both of you and you may feel like the parent while they feel like the child. But if this is part of what you need to restore the trust, then you should both work through it and hang in there.

Indifference And A Wish To Stop Trying Is More Indicative Of A Person Who Can’t Get Over Infidelity: As I’ve alluded to, a spouse who is hurt and angry and who is not willing to trust again (at least right now) doesn’t always mean that the marriage won’t survive or that you can’t get over this infidelity. It usually just means that there is work to do and that more productive time (and rehabilitation) is needed. These things can usually be overcome if both people are willing to continue this work.

Usually what is more troublesome is that the cheated on spouse becomes indifferent. They aren’t all that hurt anymore. They may not even be angry. They actually don’t care at all anymore because they are no longer invested in the relationship. In short, they know that they are “done.” They are no longer interested in trying to work things out or in moving forward. They aren’t emotional or angry about this. They just intellectually know that they are no longer interested in continuing down this road as they know that this isn’t the best choice for them at this time.

If you’re still angry, hurt or distrustful, it’s likely that you still care and are still invested. It also means that you have more healing to do. I can’t promise you that this healing will be easy. It is likely going to be work. But I’ve seen it lead to rehabilitation and peace many times so that most people who follow this path feel that it’s been worth it in the end. This is usually true whether the marriage survives or not, because at least you know that you did all that you could.

I became interested while talking to my Pharmacist that my son couldn’t take decongestant cold medications with his heart medication. Well, I knew about interactions between two drugs, but when the Pharmacist told me he couldn’t have grapefruit juice, I was stunned. Seems that grapefruit juice increases the blood concentrations of the drug. Basically that means that it greatly enhances the effects of the drug which could result in an overdose of the medication. This is everyday grapefruit juice! It got me wondering about other everyday foods and Herbs we take. Below are a few foods and Herbs that have an effect on your medications.

This list is a basic guide, and is by no means the meant to replace your Doctor or Pharmacist. Consult with them about any prescriptions you are taking and the foods and Herbs that will interact with them. Although I have taken this information from a handout from a local Hospital, Use this information at your own risk. Again, I am not a Doctor or Pharmacist, and I have had NO medical training, so please consult with yours for more information.

People usually ask if Herbal supplements change the way their prescription drugs work. The answer is YES! Some medications should never be taken with Herbal supplements. Some can cause unwanted side effects or cause the medication to stop working to their full effect.

Another misconception about supplements is that they are completely safe because they are natural. Yes, they may be from plants, but they are not natural to your body. Herbal supplements are not tested or inspected like prescription drugs are tested, so the supplements vary in quality and strength from pill to pill and bottle to bottle.

Just like supplements, foods can change the way your body responds to medications. One example is grapefruits and grapefruit juice. Many people aren’t aware that grapefruit juice can increase the drug levels of certain medications. An increase in the levels will also increase the side effects of the drug.

New Help For Alzheimer’s Patients and Their Caregivers

Alzheimer’s disease is one of society’s most devastating diseases, not only because of what it does to the Alzheimer’s sufferer but as much because of the toll it takes on the caregiver. For this reason Alzheimer’s care has become a growing priority in America.

While medical science searches for a cure and a preventative, the search is underway to develop therapeutic strategies for Alzheimer’s care that can slow the progress of the dementia or can bring relief to patients and Alzheimer’s caregivers alike. In recent years, memory stimulation has been discovered to slow the progress of dementia in Alzheimer’s patients as well as other forms of progressive dementia. Studies have found that by exercising the memory of the Alzheimer’s patient, one can successfully slow the progress of the dementia. This has led to the suggestion that family members create what is termed a “memory repository” which can be used to help stimulate and exercise the portions of the brain that deal with memory. Memory repositories, simply put, are external artifacts which can trigger the memory of a patient. These can be in the form of photo journals, scrapbooks or DVDs or any practical method for displaying these memory triggers.

In commenting on this area of study, the Mayo Clinic said, “Your life is like a tapestry, woven from your memories of people and events. Some threads are dark, while others are bright. Your individual tapestry shines vividly in your mind, reminding you of who you are, where you’ve been and what you’ve done.

“Alzheimer’s disease gradually robs people of the memories that make up their tapestries. You can help mend these holes by creating a tangible repository of memories – in a scrapbook, videotape or audiotape.

“Caregivers become the memory for their loved one with Alzheimer’s disease,” says Glenn Smith, Ph.D., a neuropsychologist at Mayo Clinic, Rochester, Minn. “By gathering memories, you can bring important events and experiences from your loved one’s past into the present. You’re the link to his or her life history.”*

The need to create more practical and supportive way to provide mental stimulation has resulted in a new market and the development of new products to fill that need. New table top and hand held puzzles and other toys have been created and existing toys that once entertained young children, like clear plastic containers filled with water and colored oil that flows through a partition like an hourglass have been updated to be more appropriate for this new market. Each one either provides stimulation for the mind or the hands.

Another new line of innovate products are found in therapeutic quilts. One such product, the “touch quilt” is created out of a variety of various cloths, each with a highly distinct texture. Textures such as like corduroy, flannel or terrycloth can engage the nervous hands of an Alzheimer’s sufferer and calm him/her. At the same time the quilt itself can provide warmth and comfort to the patient. Cold extremities are common among even the healthiest of seniors, the result of a natural reduction in circulation.

A recent development in therapeutic quilts is the “fidget quilt.” A fidget quilt is smaller than a “touch quilt”, usually around 20″ square, and is particularly useful for Alzheimer’s patients who exhibit greater nervous tension in their hands. The fidget quilt is adorned with a variety of buttons, zippers and other items designed to hold the attention of the Alzheimer’s patient and can provide hours of tactile entertainment while adding the natural comfort that comes from a good quality of flannel or other soft cloth. Many experienced Alzheimer’s caregivers swear by the effectiveness of the fidget quilt.

The therapeutic photo memory quilt is yet another recent development which is receiving raves reviews throughout the industry. Although there are no universal templates for a therapeutic photo memory quilt, the most effective therapeutically designed photo memory quilts have large photos for easy viewing and the photos are labeled to not only further trigger the memories in early stage Alzheimer’s patients, but also to facilitate interaction with professional caregivers who aren’t familiar with the patients family. It’s this conversation that significantly enhances the effectiveness of the photos which have been printed into the fabric.

The natural advantage of the therapeutic photo memory quilt over a scrapbook is that it can provide the opportunity of continuous stimulation while resting on the patients lap or on their bed and caregivers can utilize the quilt during casual contact with the patient whereas the scrapbook has to be brought to the patient and requires the active participation of the caregiver during its use. Nor does the scrapbook doesn’t offer the opportunity for casual use by caregivers during routine interactions.

The therapeutic photo memory quilt can be particularly helpful outside the home in an assisted care facility where the Alzheimer’s care givers don’t usually have the time to bring out a scrapbook, lack knowledge of the history of the patients but interact with the patient many times a day in the normal course of their duties. Each encounter becomes an opportunity to stimulate the memory by directing the patient’s attention to a photo, identifying the picture and asking the patient a question about it.

While the new toys and puzzles can be substituted with recycled children’s varieties, the adult versions are usually more ergonomically suited and can be purchased at various websites on the internet. Touch quilts and fidget quilts can be crafted at home and in some limited cases, volunteer groups might be found stitching them as a charity project. Some skilled quilt makers also make different forms of memory quilts for their loved ones which can be adapted for Alzheimer’s patients as well.

Pig Farming: What Is Biosecurity?

Pigs are particularly in need of a level of 'biosecurity' because they live in herds, often thousands of individuals in close proxies in closed buildings, and are susceptible to a wide range of different diseases which can either seriously interrupt growth and productivity, or at worst wipe out entire herds.

These diseases have various 'vectors' – means of spreading – ranging from vehicle tires and stockman's clothing to the odd rat, mouse or bird, and even the wind itself.

To protect our stock and our business, keeping everything performing to their economic best, every farm needs a measure of Biosecurity built into its plans.

Let's start at the beginning. There is a range of infectious agents out there: viruses, bacteria, fungi and parasites being the principal players. They can cause a wide variety of diseases from common colds and inflammations affecting a few individuals to population threatening epidemics. They access their hosts through five 'pathways', these being: people (for example stockmen, reps, vets and visitors); pigs (seems obvious, but we're thinking about individuals brought into a herd from another farm – replacement breeding stock for example); vehicles (bringing in and taking away people, pigs, feed and so on); the environment (wind / water borne diseases, extremes of temperature for example); and vermin (from the farm cat to the ubiquitous rat, and the night-time fox to the daytime starling, rook and chook).

All of these can bring disease of one form or another, and all diseases, by definition cause a negative change in the animal's physiology, which in turn leads to reduced productivity and therefore a reduced profit (at this point many pig farmers will be laughing, as they do not often or easily make a 'profit' in the UK).

'Biosecurity' encompasses a range of measures that can be taken to combat / prevent these pathogens from gaining entry to a very good food source (your pigs) and taking hold of your herd and business.

Three definitions with regards to pig farming would be useful here, I think:

HEALTH is a state of physical and psychological well-being that allows the pig to express its genetic potential for maximizing productivity, reproductive performance and lean meat production.

DISEASE means an unhealthy disorder of body and mind, sometimes with pain and unease, that is likely to prevent the pig from exploiting its genetic potential resulting in reduced productivity.

BIOSECURITY is a set of preventative measures designed to reduce the risk of transmission of infectious diseases, invasive alien species and living modified organisms.

So what are these measures? Remembering that you can not always see a disease, particularly at the early stages of its development; that good Biosecurity is as relevant everyday as it is when there's a major disease outbreak; and that personal hygiene is as important around livestock as it is at home – then the following are (some of) the things that bought to be considered.

People: only allow people onto the farm who have had no contact with any other pigs for at least 72 hours (some pathogens can hang around on human skin for a couple of days, however much you scrub – and those that stay up you nose are particularly devious: maybe your nose is not as complex and interesting as a pig's, but it feels familiar and a safe place to hide before you can find a nice pig to take hold of through a sneeze, cough or a contaminated breath); only allow people wearing your farm's Personal Protective Equipment (PPE) onto your unit – the soles of shoes and boots are good transport for all sorts of pathogens wanting to get out and about; use disinfectant footbaths at the entrance c = gate, and preferably between different parts of the farm as well; keep a record of who enters, close and lock gates and keep entrances to a minimum; the ideal is a shower-in / shower-out unit, where only clean personnel wearing pig farm clothes can enter.

Pigs: try to 'close' the herd, allowing no other pigs inside – breed your own replacement breeding stock, only use semen from a minimal disease unit; only allow clean, empty, disinfected, vehicles up to a loading ramp away from the main stock buildings / paddocks.

Vehicles: during an epidemic, such as the Foot & Mouth Disease outbreak in the UK n 2001, all stock movements are strictly monitored / controlled, and there are disinfectant soaked sponges and wheel washed at every farm gate. Tires are a great disease carrier!

Environment: do not establish a pig unit within five miles of another one, especially down wind; do not allow pigs to live in an environment that you've felt uncomfortable in – not too hot, cold, damp or dirty; keep the perimeters intact.

Vermin: depending on your part of the world the list will be a little different to mine, but no less extensive. There are common factors, rats and cats for example, and your local version of our fox and flocking birds like pigeons and starlings. All of these need controlling, which basically means keeping them out, as all can carry something or other from a host outside your herd. At the very least good rodent control is essential!

Can Gout Attack Somewhere Other Than The Joints?

Many people think that gout only attacks the joints, especially the big toe joints. As a matter of fact, gouty inflammation can also affect some other parts of the body such as tendon, ligament, and muscle tissue. It is all caused by the deposits of crystallized acidic compounds known as urate crystals.

When the urate crystals form within the synovial fluid of the joint, it causes a painful inflammation known as gout. If the crystals deposit below the skin surface, it forms some hard bumps (that can grow bigger and bigger) known as gouty tophi.

There are some cases of which the acid crystallization happens at the tendon that connects the bone to muscle. It might not cause the gouty inflammation that occurs in the typical gout attack, but the patient experienced some burning and tingling sensations. In some cases, the urate crystals deposit is so serious that it hardened the tendon and disabled the movement of the affected part.

Since cases like this are rather rare and is usually mistaken as some physical injury that causes the tearing of the tendon, it is important to have proper differential diagnosis for gout to identify the actual cause of the pain.

The first diagnostic method is to have a blood test to find out the uric acid level of the blood serum. If the reading is way beyond the healthy range, then it is confirmed that a person is having hyperuricemia, which support the deposition of urate crystals.

If the blood test result is normal and healthy, then other information is required to further diagnose the case. This is because there are many cases in which the patients are having normal blood uric acid reading, but the joint pain is actually gouty arthritis.

Different individual has different rate of uric acid metabolism. For some, the uric acid in the blood seeps faster into the intercellular and synovial fluid and that prevent the rise of blood uric acid level. For some other, the excess acid tends to stay within the bloodstream, resulting in hyperuricemia but do not show any signs of gout attack. The most common gout cases are those of which a patient is having high uric acid content both inside and outside of the bloodstream.

Gouty inflammation is usually triggered by the food a person eats. This is because when someone eats some high purine or high protein meals, it will cause a surge of uric acid in the in the body which will eventually result in the formation of new urate crystals that trigger another round of gout attack.

The conclusion is that gout can exist in many different forms and different location. Once the urate crystals formed, it is very hard to remove them. The only effective way to get rid of these sickening crystals is to dissolve them. Try to soak your feet in hot warm water for 10 to 15 minutes everyday. That can help to improve the blood flow and increase the fluid exchange rate. Higher temperature increases the solubility uric acid which helps to melt away the urate crystals.

What Is an ‘Accompanied Shop Visit’ In Qualitative Research?

If you receive paid market research participant invitations, you may have heard the term ‘accompanied shopping’ to describe a specific kind of research activity you can take part in. In case you were wondering what it meant, there is no great mystery.

Some people say the British high street is in decline, but shops are still where most retail businesses make their biggest sales and profits, even today. In any typical high street or shopping mall you will see similar clusters of shops, such as boutiques, supermarkets or pharmacy-cosmetics outlets… often there are lots of shops in one area selling pretty similar categories of goods to one another.

So, what makes the difference to you as a consumer, when you decide between them whereabouts to go and spend your money? Of course there are a range of factors including price, convenience, and knowing you can get what you want – but with crunched credit and squeezed margins, advertisers and marketers have never been more keen to understand exactly what it is that can motivate you to go in and spend in one shop, whilst passing by the one next door.

Accompanied shopping trips are when a researcher tags along beside you, whilst you try to behave and respond as ‘normally’ as possible on a typical retail experience. Of course the experience is inevitably constructed to some degree, because you will be asked to think about and explain certain actions and activities as you do them – things you probably don’t even think about normally, especially in typical everyday shopping for inexpensive consumer goods.

Because these things are sold in such high volumes however, understanding exactly what makes you decide between one brand and another is of priceless value to marketers. Parkinson’s Disease – Ayurvedic Herbal Treatment

Parkinson’s disease is a movement disorder usually seen in the elderly population. This disease is characterized by tremor, stiffness, slowness of movement and impaired balance and coordination. Usually the symptoms persist and worsen over time. Although there is currently no cure, there are many treatment options such as medication and surgery to manage the symptoms.

Parkinson’s disease is known as “Kampa Vata” in Ayurveda. The comprehensive treatment which is advised in the treatment of disturbed Vata dosha is recommended in this disease too.

Levodopa is considered the standard of treatment in Parkinson’s disease, and the herbal medicine Kapikachhu (Mucuna pruriens) contains levodopa in the natural form. This is used in powder form or tablet form of aqueous extract. High doses are given, which are very effective and do not usually exhibit the side effects seen with the modern chemical molecule.

Other Ayurvedic formulations which are used in Parkinson’s disease are: Visha Tinduk Vati, Agni Tundi Vati, Maha Yograj Guggulu, Abhrak Bhasma, Shrunga Bhasma and Brihat Vat Chintamani. Herbal medicines which are useful are: Dashmool (Ten Roots), Ashwagandha (Withania somnifera), Brahmi (Bacopa monnieri) and Sarpagandha (Rauwolfia serpentina). Most of these medicines are believed to stop or slow down the degeneration of brain cells and probably help in impulse transmission in the intact cells.

The above mentioned medicines can be safely added to the current modern medicines for Parkinson’s disease. The herbal medicines can be given long term and significantly reduce tremor and stiffness, and improve body coordination. Most of the Ayurvedic formulations mentioned as “Rasayanas” (rejuvenatives) may probably be quite useful in Parkinson’s disease, and indeed, all diseases resulting from ageing and degeneration, however, not much research has been done in this area.
