Inflammatory Breast Cancer (IBC): When a Red Breast Means Cancer

Jesse’s Story

A 44-year old professional horse trainer, Jesse had been extremely active all of her life. She had no medical problems other than a broken leg as a young girl. One day, after several hours of riding, she noticed in the shower that her right breast was quite red. Jesse didn’t remember falling or hitting her breast. Possible Reasons for a Red Breast

There are several reasons why a woman or man can develop acute (fairly sudden onset of) breast redness. Infection and inflammation are the two most common causes. Both of these conditions need to be treated in a timely fashion in order to improve health & minimize pain. They usually resolve over a week or two. However, there is one cause of breast redness that is always life-threatening: inflammatory breast cancer (IBC).

Warning Signs

IBC is a particularly aggressive form of breast cancer. Among every 100 patients diagnosed with breast cancer, about 2 – 5 will have IBC. It’s more commonly diagnosed in younger women and the redness can come on literally over a few days. While most people who develop a red breast will NOT have IBC, both patients and their primary doctors need to be aware of the possibility. This diagnosis should be particularly suspected in patients who still have a red breast after treatment with antibiotics or anti-inflammatories for a presumed infection or inflammation. The classic description of the appearance of IBC is peau d’orange (orange peel) skin.

Diagnosis & Staging

The diagnosis is confirmed by biopsy of the breast skin, usually by a breast surgeon or radiologist. Pathologists, the specially trained doctors who look at the biopsy tissue under the microscope, will usually describe tumor cells in the lymphatic channels of the breast tissue and breast skin. The blockage of these channels is usually the cause of the breast redness in IBC. Often there is no specific mass or lump in the breast, only diffuse red, thickened breast skin. Evaluation of the breast itself should include mammogram, often ultrasound (if a mass is felt), and sometimes MRI. All patients with IBC are considered to have aggressive disease. In that regard, unless the patient is in very poor condition and would not tolerate treatment, staging studies should be performed to assess whether the cancer has visibly spread elsewhere. These scans would include PET/CT or CT of chest and abdomen and bone scan. Patients with symptoms such as severe headache, nausea, and vomiting should also have MRI of the brain.

Treatment & Outcome

As with all breast cancers, the treatment for IBC can be broken down into two categories: locoregional (breast and lymph nodes) and systemic (throughout the body). A common mistake that a surgeon can make is to recommend a mastectomy (removal of the breast) first. Patients who are in reasonable medical condition should almost always have chemotherapy first, then mastectomy, then radiation therapy (RT) to the chest wall and regional lymph nodes. All three treatments, chemotherapy, surgery, and RT, are required in order to provide the best chance for cure. Hormonal and targeted therapies (such as Herceptin) may be recommended also, depending on the specific tumor biology. Among patients who have no evidence of metastasis (disease spread to distant areas in the body) at the time of diagnosis and are potentially curable, 40-50% are alive 5 years later. Awareness about this particularly aggressive form of breast cancer and its treatment is critical to ensure the best chance for cure.

Is Prevalence of ADD/ADHD the Same in the U.S., Europe, and Japan?

Attention deficit disorder (ADD) and attention deficit hyperactivity disorder (ADHD) are increasingly common diagnoses among school-aged children. This poses the question is our society more aware of the condition or are children being over diagnosed or misdiagnosed? The diagnosis of ADD/ADHD also results in the prescription of ADHD medications to better manage the condition. Is ADD/ADHD being confused with other psychological issues that children experience such as dealing with a parent’s divorce or the death of a loved one? Is ADD/ADHD a label being issued to children who are struggling with academic issues or learning disabilities?

There are numerous scenarios that must be differentiated from a true diagnosis of ADD/ADHD. Regardless of the issue, children who appear distracted, cannot follow instructions, do not pay attention, seem overwhelmed, frequently interrupts conversations, or cannot sit still are likely to have a red flag held up over them seeking answers. The diagnosis of ADD/ADHD is not always straight forward and may vary significantly from one child to another, thus further complicating the diagnosis. Most cases of documented ADD/ADHD diagnoses include a physician reviewing a checklist of common signs and symptoms, feedback from school teachers, and parent observations and feedback.

The diagnosis of ADD/ADHD is subjective and evidence of this is relevant based upon reports from the Centers for Disease Control and Prevention (CDC). The CDC reports that the American Psychiatric Association states that 3-7% of United States school-age children have been diagnosed with ADD/ADHD. American parents report the contrary suggesting that as many as 22% or more school-age children in the United States have ADD/ADHD based upon survey results of parents. Physicians have reviewed this correlate and concluded that approximately 1 in 20 children have ADD/ADHD symptoms.

The incidence of ADD/ADHD diagnosis in Europe, Japan, and other non-American countries ranges around 5-7%. According to research performed in numerous national and international studies, the differences in diagnosis paradigms appears to be that the United States is more aware of this condition and recognizes it as a treatable disorder. In some cultures, labeling a child with ADD/ADHD is perceived as derogatory resulting in a negative psychological effect.

The actual diagnosis rules vary significantly between the United States and Europe. The primary differences in achieving statistical evidence of the prevalence of ADD/ADHD is a country related to the research methodologies. European researchers follow the ICD-10 which specifies that children must have issues in school, at home, be impulsive, lack attention, and be hyperactive. American researchers follow the DSM-IV. The DSM-IV is not as restrictive as the ICD-10 rules and possibly explains why Americans have such a higher reported diagnosis rate of ADD/ADHD. The DSM-IV essentially states that the child has several of the listed symptoms but not enforce the rule that all criteria must be met, as in the ICD-10 rules.

Japan reports a 7% of school-aged children have ADD/ADHD. Several reports explain that Japanese parents have been giving their school-aged children nutritional supplements to increase their attention capacity. The belief is that if children lack the essential fatty acids, the less than optimal outcome would be ADD/ADHD. I get so many requests asking me how to handle objections, that I thought I’d go ahead and give you the secret method that I reveal in my in-person trainings – the definite way you should handle and deal with all objections.

This is a Top 20% favorite and it works because you are not answering or dealing with objections, rather, you’re qualifying and isolating first, then, once you’ve found out if it’s a real objection or a smokescreen, you’re dealing with it in the most efficient way possible.

And you’re asking for the deal when done. It’s called the 5-Step Method of Handling Objections, and once you begin using it, you’ll be amazed by how successful it is. Here’s how it works:

The Five-Step Method

Step One: (Two Parts):

1. First, hear them out – completely. Don’t interrupt!

2. Put in a softening statement before you answer:

“I complete understand how you feel.”

“Some of my best clients felt that way also.”

“I completely understand how you feel, this is a big decision, and right now I’m sure it makes sense for you to think about this.”

Step Two: Question and Isolate the Objection B-4 Answering it.

As I’ve written about before, you can’t begin chasing and answering every objection they give you! First you must question and isolate the objection to make sure it’s the REAL objection.

Example: “The Price is Too High”

“I understand __________, and let me ask you a question: Assuming that the price on these hearing aids weren’t an issue (or fit within your budget, or if someone were suddenly going to buy them for you), but if price weren’t an issue here, is this the solution you feel is right for you today?” Or, “is this something you would go ahead and order today?”

Step Three: Answer the Objection (using a scripted response!)

No secret here that I’m recommending you use a scripted response once you understand what the objection is. Either use one of the scripts you already have, or write one. No matter what, though, USE A SCRIPT!

Step Four: Confirm Your Answer:

One of the biggest mistakes 80% of sales reps make when answering objections is they don’t confirm their answers – in other words, they just keep talking! And I mean talking past the close. The Top 20% understand the danger of talking past the close (like introducing new objections), and instead, after they have used their scripted response, they confirm their answer. Use any one of the following:

“Does that answer that for you?”

“Does that make sense?”

“Have I satisfied that for you?”

Step Five: Ask For the Deal!

This might sound obvious, but you’d be amazed by how many (like 80% or more) closers don’t ask for the deal. Scripts assure that you do! And after you do ask for the order,

Fixing Problems With Jaw Angle Implants

Jaw angle implants are one of the most infrequently performed types of facial implants used for aesthetic or reconstructive enhancement of the back part of the jaw. They are much less commonly done than cheek or chin implants. Like implants placed anywhere on the body, they have the potential for complications such as infection, shifting and asymmetry, and size issues.

Infection from any type of facial implant is possible but is really quite uncommon. The risk is probably 1% or less. Jaw angle implants are placed beneath the thick masseter muscles and the jaw bone, both of which have a great blood supply that completely surrounds the implant. Even if infection were to occur, I would reopen the implant pocket, clean off the implant thoroughly and reinserting it back into the same pocket. This is a sound approach that will solve the infection most of the time as long as they are smooth silicone rubber implants. Implants composed of porous materials, however, make it near impossible to rid the bacteria which are embedded in the irregular deep channels of the implants. Porous Jaw angle implants should be removed and replaced at a later date.

Jaw implants are unique from cheek and chin implants because they are positioned under the big jaw chewing muscles. The motion of the jaw and the masseter muscle makes their displacement possible, and very likely if they are not rigidly secured into place. This displacement or movement of the implant will be toward and through the incision in the mouth that was used to place them. Extrusion of the implants can be solved by their repositioning and screw fixation to the bone. Often jaw angle implants displace because the pocket into which they were placed was not large enough or extended to the edge of the jaw bone. Pocket expansion and screw fixation can solve this problem.

Asymmetry or differences between the two sides of the jaw after surgery is another potential complication. Even when the implants are the same size (which is most common), jaw asymmetry can be seen after surgery. This is most commonly due to subtle differences in the two jaw sides that existed prior to surgery, which have only become ‘unmasked’ not that the jaw angles are larger. A good photographic and computer analysis can spot this potential problem beforehand. Differences in implant position can also account for this problem. If the asymmetry is significant enough, changing an implant’s size or position on the bone needs to be done.

Size concerns (too big or too small) is usually not as likely a postoperative problem as that of cheek or chin implants. It is very uncommon to have jaw angle implants that are too big. That is because the off-the-shelf or stock implants that are available are not that big and jaw bones are big. That combination does not make for a result that is too large. The result is most often a moderate change on each side which, because it is done to both sides, creates a nice effect that is not overdone.

Revisions of problematic jaw angle implants with a satisfactory outcome can usually be done by changing the size and shape of the implant or repositioning it with more secure fixation to the bone.

Ranitidine and High Blood Pressure

Ranitidine, known to millions by its trade name of Zantac, is an acid reducer and histamine blocker. It works by blocking histamine receptors in the lining of the stomach. Doctors prescribe it mainly to treat peptic ulcers, gastritis, and stomach reflux. They also prescribe it to treat rare conditions where the stomach produces too much acid possibly due to enlargement of the pancreas (Zollinger-Ellison syndrome). Ranitidine, in addition to the name Zantac, is marketed by other companies under various other names as well.

Fifty million or more people in the United States suffer from hypertension and the number increases every day. Blood pressure is the force of blood against the walls of arteries. When the pressure is too high, it causes the heart to work too hard and can lead to heart attacks or stroke. But what, if any, is the relationship of ranitidine to hypertension?

Although ranitidine was introduced to the public in 1981 and has been the subject of many studies, there has been very little direct study and no trials on the effects of ranitidine on hypertension. One study tested the effect on hypertension in patients that already had high blood pressure. The testing showed no elevation in blood pressure on these test subjects. This same study did not, however, test the effects on ranitidine on patients with a normal level of blood pressure.

In a study testing the cardiovascular effects of ranitidine on children, twelve children with congenital heart disease were intravenously given dosages of ranitidine. Although their heart rate fell below baseline levels, the researchers came up with no conclusion of the drugs affect on high blood pressure, instead determining that more study was needed. This study also did not test children with normal blood pressure levels.

Many people with hypertension who are thinking of taking Zantac or another ranitidine derivative are rightly concerned with how the drug will affect their high blood pressure. One of the confirmed possible side effects of ranitidine is the slowing of the heart rate or bradycardia. If this happens the heartbeat may become too slow or too irregular to meet the body’s demand. The result could be dizziness or lightheadedness. Whether this is dangerous to your health can only be determined by a qualified physician.

Although, not generally considered a side effect, there have also been reports from some patients that taking ranitidine increased their heart rate. If this is true, it would follow that their blood pressure would have increased also. However, at this time, there is no conclusive evidence showing that there is any one-to-one correlation between ranitidine and high blood pressure.

If you have high blood pressure, and are taking ranitidine or Zantac, the best that you can do is to monitor your own blood pressure after taking the medication and notify your doctor of any significant change in the readings. You should also notify the doctor if you experience either a significant increase or decrease in your heart rate as either condition can adversely affect your health.

Chronic Renal Failure – Specialized Ayurvedic Treatment

Chronic kidney disease is defined as kidney damage or a decreased kidney glomerular filtration rate of less than 60, for 3 months or more, irrespective of the cause. The Ayurvedic treatment of chronic renal failure is based on three principles: (i) treating the damaged kidneys (ii) treating the body tissues (dhatus) which make up the kidneys and (iii) treating the known cause.

Medicines used are: Patol, Saariva, Patha (Cissampelos pareira), Musta (Cyperus rotundus), Kutki (Picrorrhiza kurroa), Chirayta (Swertia chirata), Guduchi (Tinospora cordifolia), Chandan (Santalum album) and Shunthi (Zinziber officinalis).

Lastly, the known cause of chronic renal failure is treated using medicines which also act upon the kidneys. Vascular (related to the blood vessels) diseases like renal artery stenosis and inflammation of the artery walls(vasculitis) can be treated using medicines like Arogya Vardhini, Tapyadi Loha, Mahamanjishthadi Qadha, Kamdudha Vati, Manjishtha (Rubia cordifolia), Bhrungraj (Eclipta alba), Saariva, Kutki and Sarpagandha (Rauwolfia serpentina). Primary glomerular diseases like membranous nephropathy and glomerlonephritis can be treated using Punarnava, Gokshur, Saariva and Manjishtha. Secondary glomerular disease resulting from diabetes, systemic lupus erythematosus, rheumatoid arthritis etc. can be treated accordingly, using the medicines appropriate for those diseases. Similarly, suitable Ayurvedic medicines can be given for other causes like polycystic kidneys, prostate enlargement and neurogenic bladder.

The advantage of using Ayurvedic medicines in chronic renal failure is that in most patients, the kidney damage can be either partly or fully reversed, the frequency of dialysis can be reduced, and the increased risk of death from cardiovascular diseases can be significantly reduced. Thus, Ayurvedic medicines have the potential for an important therapeutic contribution in all the stages of this condition.

For patients with chronic renal failure intending to take Ayurvedic treatment (or for that matter, any alternative treatment), the following points should be kept in mind: (i) all patients should be under the regular supervision and treatment of a qualified and experienced Urologist (ii) Ayurvedic medicines should be taken in the form of additional treatment, and should not replace other, regular treatment or dialysis and (iii) the attending Urologist should be informed of the decision to start Ayurvedic treatment.

When obesity in kids is on the rise we need to start from home by introducing to them healthy diet, kids get to know their diet from home and they will get used to healthy food if it is introduced to them early at home. As parents we must expose our kids to different type of food and everything healthy.

It is not easy changing kids eating habits or introducing healthy food to them and this is what I suggest for parents who are interested in improving their kids eating habits.

1- Talk to your kid about health and food. Food and health has to be important topic and a good time to talk about food is during meals and when buying groceries. Preventing our kids from eating unhealthy food without explaining to them the reason is not the way. My daughter likes sports and I always explain to her how healthy diet is good for keeping her fit for sports and it is working.

2- Try different recipes. My daughters do not like eating red meat but when I tried meat balls and burgers they liked it. Sometime it is about how the food is prepared and with extra efforts you will discover that your kid will like certain food if it is prepared differently. If your kid don’t like eating steamed broccoli try to bake them with some cheese, there is no limit to what you can do with food and you can find many recipes on the internet that you can use.

3- Try dry fruits and vegetables. If you are trying to introduce vegetables and fruits to your kid diet you can start with dry fruits and greens if fresh is not appealing to them. When you choose dry fruits make sure it is natural, without coloring agents, preservative and sugar.

4- Let them participate. Allow your kid in the kitchen and let them participate in preparing their meal, you can start with sandwiches.

We should not force our kids eating healthy food with patient they will understand what is healthy for them and it has to be a family issue to eat healthy.
