P.H.I. is any information about a resident’s condition, treatment and payment. P.H.I. includes: Medical Records – printed and on computer, Arm Bracelets, Pharmacy Orders, Conversations about a resident’s health, Dietary Cards, IV Bags and Meds, Payment and Insurance Records. Under HIPAA, you can share P.H.I. for three purposes:

The title of the article highlights the double jeopardy of this sector.

Cat and mouse: Government vs hospitals, where are the drugs?

You cry (or weep) for our beloved country because it would seem that many reports indicate that whereas the Government is constantly allocating money to hospitals for buying medication, when you get to the hospital the doctors tell you there are no drugs in stock and you will have to buy them from somewhere else.

Ugandan hospitals appear to have therefore been transitioned into diagnosis clinics identifying underlying health issues of patients and then sending them out to look for medication. The doctors claim there are no drugs in stock. It’s therefore common for patients in Uganda to go to government hospitals, receive diagnosis for their diseases and leave without even the basic medication of pain killers. The government claims they send the medicine to the hospitals but the hospitals claim they never receive the drugs so the big question is where do the drugs go?

The pharmacy sector: More weeping.

According to the Pharmaceutical society of Uganda, the body responsible for the sector, there are currently 465 qualified pharmacists, of whom 70 are abroad, leaving about 395 practicing within the country.

With a population of approximately 34 million people, this represents a pharmacist to population ratio of 1: 88,000 which is way below the recommended World Health Organisation (WHO) 1: 2,000 ratio.

The chronic shortage is being addressed by the pharmaceutical society in conjunction with Universities to among others increase the number of pharmacists being trained but in the meantime, illegal pharmacies continue coming up, the cat and mouse games with National Drug Authority (NDA), the regulator, continue and meanwhile the population suffers as a result of the imbalance.

This therefore presents an opportunity to invest, and hence reap (whilst weeping for the sad state of affairs).

When I further analysed the state of the pharmacy sector in Uganda, I noted a number of key aspects to consider:

1. Whilst many press report place the number of clinics in the country to over 10,000 there are only 414 registered pharmacies in Uganda (registered with the regulating body).

2. Of these 414 registered pharmacies, over 292 are located in Kampala, the capital and in the Central region. This is a huge imbalance considering Uganda’s population is almost equally distributed at 25% in all the four regions (Northern, Eastern, Western and Central).

3. The opportunity is therefore to invest in a franchise or network of pharmacies that target the up country towns.

Prior to investing, there are however a few key considerations:

1. Drug licence. You must have a licence and your approved pharmacist must be regulated by the pharmaceutical society.

2. Retail business considerations. As any retail outlet, you must consider retail business risks such as location and stock controls (as my mother alluded to). Many drugs come in tablet or pill form and as such the quantities can be hard to monitor. It is therefore important to first understand the drug classes (in Uganda, this is mostly class B and C) and then understand how to ensure proper controls are put in place.

3. Return on investment. From my estimates, the profitability from investing in this sector is a revenue of Shs. 187m and a Return on Investment (ROI) of only 0.87 months.

Final word

In a country characterized with limited availability of medicine in the government hospitals, more and more pharmacists and businessmen have seen an opportunity to open independent pharmacies. They do this because of the unsatisfied demand for medication by the patients who don’t get it in the hospitals.

Moisten this thoroughly and scatter the spores evenly over it, and after sprinkling cover with glass. Water only when they show they are dry. Keep covered until the seedlings have started. It will be best to prick out the young seedlings into flats, from which they should later be transferred to pots.

Pot them in soil one-half leaf mold and the reminder of loam and sand. For propagating on a large scale, a box covered with a glass sash, of suitable size, will answer. The seed bed can be prepared upon the bench itself. Ferns for dwellings should be grown at 55 to 60 degrees, as they will then be firm and well hardened, and will thrive far better than soft, spindling plants, in the dry atmosphere of the living room. For small fern pans, two or three plants will be enough. Fern pans can be filled to advantage by using some erect growing kind in the center, with fine Adiantums, Selaginellas or similar types, around it.

Many varieties are readily increased by dividing the crowns. To increase them quickly, they should be bedded out where they can be kept well moistened at a temperature of 60 to 65 degrees. In dividing and transferring to pots, it is hard desirably to make a very fine division, as, although more plants can be obtained, they will be slower in starting and less satisfactory. During the spring and early summer, the young plants should be kept in a cool house or in a frame, where they can be properly shaded and watered.

If to be used for cutting, the best results can be obtained if the plants are bedded out. This should be done early in August, in order that the plants may have time to develop and harden off. The beds should contain from four to five inches of compost, consisting of two parts pasture sods and one part each of sand and rotten manure. For most ferns a temperature of 60 to 65 degrees is desirable, and the stove ferns are beneficial if it is slightly higher, although some of the greenhouse species do well if it is significantly lower.

For the florist's use, in addition to the well-known Adiantum cuneatum and gracilimum, such others as A. eleyans, A. Capillus-Voneris, A. continuum and A. c. latum, A. St. Catherina and A. decorum will be useful. Adiantum Farleyense is among the best of the Adiantums for decorative purposes, but except for very elaborate cut-flower work, it will be less useful than some of the more delicate sections. The Pteris serrulata and P. s. cristata, and other forms that are readily grown, are also desirable for planting out, either on or under the benches, while Pteris tremula, and its variegated form, P. aryyrea, and P. cretica alba lineate should not be neglected.

When ferns are shifted, or planted out, they should be shadowed from the blight sun for several days, and the foliage should be frequently syringed. If to be used for cutting, after they have made their growth, the shading should be taken off, and abundant ventilation should be given, in order that the fronds may harden off, and then stand up longer when used. The florist will find specimen plants, and baskets and pans of ferns useful in decorating his house, as well as for outside work. If well grown and of well selected types, the visitor will be attracted to them, and they will have a ready sale.

What is FIE Fencing Equipment?

The FIE is the international governing body for the Olympic sport of fencing.  FIE is the “Federation Internationale d’Escrime” or International Fencing Federation and it governs the world rankings and competitions for modern sport fencing.

When someone is talking about “FIE” gear, they are talking about fencing uniforms, masks, and blades that are approved for use in these international FIE competitions.  Each item must meet specific requirements for use in an FIE competition. 

FIE Blades

In general, the FIE approved items will provide a greater level of protection and durability over other grades of protective gear and blades. 

There are specific tests for the FIE blades for foil and epee.  These blades are, for the most part, made of maraging steel and sample blades must pass a minimum number of bends before breaking to be FIE certified.  There are also blades that meet the durability requirement but are not made from maraging steel.  These are specifically marked “FIE-N” blades.

A common myth regarding the FIE blades is that when they break they will break flat, reducing the chance of a puncture wound.  While this may be a stated goal of the steel used, in practice maraging steel blades break with jagged edges around the break point.  The main benefit of an FIE blade is it’s increased durability, which increases safety since at any point in time the blade is less likely to break.

Sabre blades do not have to be made from maraging steel.  Instead they must meet a maximum flexibility requirement that is the “S2000” regulation.  This regulation extends all the way down to local events, so any reputable fencing equipment supplier that sells a competition grade sabre is going to use an S2000 blade.

FIE Fencing Masks

Standard fencing masks must have a bib that resists 350N of force and mask mesh that will stand up to the 12kg punch test.  FIE fencing masks have increased safety requirements, with a bib that resists 1600N of force and mask mesh that will stand up to a 24kg punch test.  In other words, the FIE masks offer more than twice the protection of a standard fencing mask.

The FIE also has specific requirements for foil, epee, and sabre based on the current rule set for the target areas of each weapon.

FIE Uniforms

The fencing uniform consists of the jacket, pants (often called knickers or breeches), and plastron (or underarm protector.)  The FIE requirement for the uniform is that each piece protect against a force of 800N, which is the CE Level 2 protection requirement in the European Union.  Many domestic events in Europe require a minimum of CE Level 1, or 350N protection.  As of 2009, the US Fencing Association mandates no minimum protective requirement, only that the uniform be of “robust” material – whatever that means.

The quality of uniforms from various equipment manufacturers varies.  While all FIE fencing uniforms must pass the CE Level 2 testing in order to become FIE certified, the quality difference is in the stitching, cut, and other aspects outside of the pure protective requirements.  

Recommended Upgrades

If you are an active fencer and are competing in tournaments, you may already have a full set of gear.  If you are considering the jump up to FIE gear, the top pieces to get would be the fencing mask and plastron.  The mask is one of the most important upgrades to make and will provide additional peace of mind.  The FIE plastron is the least expensive upgrade you can make that will provide the most protection for you.

Upgrading your blades (in foil and epee) to FIE class blades is more of a performance decision.  FIE blades tend to last longer but more importantly they are more consistent in how they act.  Once worked in, FIE blades hold their shape better than non-FIE blades, which provides a more consistent performance.

Stereotypical Signs of Aging

• You get dizzy when you stand up or bend over

• Your joints and muscles ache all the time

• You are often irritable, grouchy, depressed and generally unhappy

Physiologic Changes and Aging

Past research about aging has focused on patients suffering from illness and disability, observed in doctors’ offices, clinics or hospital settings. What we have believed about aging, it seems, has been a reflection of the effects of disease process and unhealthy lifestyle. • Aging is NOT Disease

Physiologic changes that occur with aging do not necessarily cause disability. Aging does not inevitably lead to declining levels of cardiac functioning, bone density, muscular strength, cognitive ability and memory, sexual desire and activity, physical and social functioning, nor does aging insure rising levels of blood pressure, cholesterol and anemia. But aging does decrease the body’s ability to withstand and respond to stress. As we age, we are less able to regulate pulse rate, blood pressure, oxygen consumption, blood glucose, serum sodium, and blood ph levels under stress. Aging leads to greater difficulty reacting to injury and the probability that the stress of injury will lead to acute or chronic illness over time.

• One Percent Rule

From age 30 onward, most organ systems lose roughly one percent of their functioning each year. The percent of loss does not increase as we age.

• Body Organs Age Differently

The physiologic state for any organ in our body is affected by the rate of change that organ has experienced multiplied by the number of years that change has occurred. As we age, changes in one organ does not predict changes in other organs.

Memory decline with age is common, but does not inevitably lead to dementia which is an illness. Dementia-type symptoms include hearing loss, confusion or disorientation, difficulty performing simple tasks and making every day decisions, as well as changes in mood and loss of interest in life activities.

Anita Hollander’s Journey From Cleveland Child Actor to LAMDA

Anita Hollander: I always wanted to be an actor – ever since I was four, and I started with a professional job at eight.

And it was funny because I was very realistic at the age of four. What I really wanted to do was to make my living at something I loved to do and that I could do well – right down to the brass tacks of living in New York, having an apartment and going to [work at] a studio or theatre. It was all in my mind from a very young age.

Susan Dansby: I think that it’s actually typical that people are drawn to certain professions, certain creative outlets from the time that they’re children and we just kind of drown it out of ourselves.

Anita Hollander: Yeah, it’s lucky when your parents support that in you even if nobody else recognizes it. I hope I’m that kind of parent to my daughter.

Susan Dansby: What kind of training did you do to become an actor?

Anita Hollander: My mom was wonderful to take me to dance classes, and youth theatre training, and the Cleveland Play House. Curtain Pullers (Cleveland Play House theater program) was a training ground for many great actors who ended up doing great things. So, I started young with that – dance classes, acting classes.

When I was eight, doing The Sound of Music, there were 250 children, and I got the understudy job for Gretel. And she was kind enough to get German measles and allowed me to go on for a couple of nights and she even said to me, “I’m so glad I got sick.” Because it didn’t hurt her at all to get German measles because she knew I had a chance to go on. She was very generous.

When I wasn’t doing professional theater, I would do community theatre. I would do whatever a kind of theatre I could get.

Transitioning from Child Actor to Young Adult

Anita Hollander: While I was in high school, I took a summer graduate workshop in theatre at Case Western Reserve – which was a real experience for a 15-year-old being with graduate students in theatre – a real eye opener for me.

But then, I went on to Carnegie Mellon University and got my Bachelor of Fine Arts there in acting and then, went to LAMDA, London Academy of Music and Dramatic Arts. Studied for one year in the overseas program there, and they asked me to tour with an original production about Elizabeth I with some of their graduate students who were Brits. So I ended up staying on a year, touring with them, which led to a job in Copenhagen, which led to all kinds of jobs.

Susan Dansby: In terms of getting acting jobs, it sounds as though a lot of your work has come out of the professional organizations that you were part of like the schools and different acting companies.

Anita Hollander: In my younger years, yeah, it did. But I did audition for everything. I had learned at a young age how to audition, so it wasn’t just that the training led to it.

However, I was lucky that in many cases my training helped me to get work. In fact, when I was in London just finishing up the British tour which we did in Holland and Belgium and Wales, came back to London and I got a phone call from Copenhagen, The Mermaid Theatre. This guy had seen my resume somewhere and knew that I had studied with Paul Draper, dancing, at Carnegie Mellon and on that alone he hired me for an acting job at his theatre in Copenhagen, on the spot. So, there was an occurrence where my training led me to a job.

Oncology nurses are especially trained to provide chemotherapy, radiation aftercare and work with cancer patients. OCN’s usually have set hours and a number of prescribed duties requiring less physical strain, but practice with more compassionate care.
