In fact a year ago after I had had hip surgery I spent a week in the hospital because of overuse of Oxycontin and Hydrocodone

My advice is Use caution and common sense consult your doctor befor taking any of these drugs. Never take any of them or any other drugs without his or her knowledge, even those sold over the counter. Many of them are not necessarily dangerous when taken by themselves, but taken in conjunction with other medictions, can be lethal.

Here's an example of a drug conflict that occurred even when my physician was aware of all the medicines I was taking and in this case, no one was to blame. I doubt that this situation occurs frequently. Drug manufacturers are careful to list side effects and it's a good idea to read them and if you have any questions, consult your doctor or pharmacist.

I am a Diabetic and for seveal years I took a drug called Glucophage, a very benign medication. I develoed a case of Edema, swelling of the legs. To remedy Edema it is required that I take a Diuretic, or water pill. Every time I took a Diuretic I became violently ill. This became a crisis when I had a hip replacement, as I filled up with fluid and the only relief was a diuretic, which had near lethal effects.

I'm over eighty years old and take a number of drugs so it was not esy todetect this problem. When I was in rehab after hip surgery I was taken off all my oral medications and given Insulin for my Diabetes. Guess what? The Edema disappeared. Since I do not take Glucophage any more I am able to take diuretics daily with no ill effects.

So the word is CAUTION be alert, be aware, keeep your doctor informed of all the medications you take, even over the counter products. BE AWARE.

Boerhaave's Syndrome – Spontaneous Rupture Of The Esophagus

Case Presentation:

An 80 year old female with a past medical history of hypertension, renal cell carcinoma, and status-post nephrectomy, experienced an episode of choking, coughing and vomiting when she had difficulty swallowing her Centrum Silver tablet. Soon after vomiting, the patient developed sever chest pain with radiation to her back. The pain was significantly worse with deep inspiration.


Boerhaave's Syndrome is a spontaneous rupture of the esophagus, classically described as post -emic, but may also occur after lifting a heavy weight, with severe asthma, or prolonged coughing. The sudden rise in esophageal intraluminar pressure causes an esophageal tear which most commonly occurs in the lower esophagus above the left diaphragm.

Boerhaave's Syndrome was first described in 1724 by Hermann Boerhaave, a Dutch physician. His patient was a 50 year old admiral (Baron John van Waasenaer) who developed a sudden excruciating chest pain while training to vomit. The patient developed shock and died 18 hours later. An autopsy demonstrated rupture of the distal esophagus into the left chest.

Clinical Presentation:

Classicly, the patient is a middle-aged, white male with a history of overindulgence in food or drink. The patient often develops vomiting, lower chest pain, and mediastinal or subcutaneous emphysema (Meckler's triad – vomiting, pain and subcutaneous emphysema).

The physical exam typically reveals a critically ill patient, usually sitting up in bed with a forward-crouching position. Subcutaneous emphysema is frequently seen. Hamman's sign is seen in up to 20 percent of Boerhaave's patients. Hamman's sign is a crunching, rasping sound, synchronous with the heartbeat, heard over the precordium, and is often indicative of spontaneous mediastinal emphysema. Patients will present with varying degrees of epigastric tenderness, sometimes imitating an intra-abdominal catastrophe. When the rupture is confined to the mediastinum, the patient may not look particularly sick and vital signs may be deceptively normal.

Differential Diagnosis:

The differential diagnosis of Boerhaave's Syndrome is extensive and should include the following:

· Perforated or Bleeding Ulcer

Acute Pancreatitis

· Myocardial Infarction

· Pulmonary Embolus

Dissecting Aneurysm

· Spontaneous Pnuemothorax

· Mallory-Weiss Tear

· Acute Cholecystitis

Diagnostic Evaluation:

In addition to the patient history, radiography remains the cornerstone of the diagnostic evaluation for Boerhaave's Syndrome. Plain chest radiographs may show mediastinal or free peritoneal air (most common finding on initial films), a widened mediastinum, hyrdrothorax, hydropneumothorax, or mediastinal emphysema. Ten to fifteen percent of all patients presenting with Boerhaave's Syndrome may have a normal plain chest radiograph.

A swallow contrast radiographic study remains the diagnostic gold-standard. Either a thoracic CT scan or an esophagram is required to locate the exact site of perforation, and helps to determine the best surgical approach. A water-soluble contrast agent such as gastrografin is utilized. Most recommend avoidance of barium since its penetration into the thorcacic cavity can inductive an inflammatory reaction leading to granuloma formation.


The initial emergency department management of Boerhaave's Syndrome includes strict NPO, broad spectrum antibiotic, fluid resuscitation, and continuous nasal gastric suction. A cardiothoracic surgeon should be consulted urgently, and if cardiovascular services are not available at your facility, the patient should be transferred to an appropriate facility.

Patient's are often placed on total parenteral nutrition, and early surgical repair remains the standard of care. Complications of Boerhaave's Syndrome include persistent esophageal leak, mediastinitis, polymicrobial sepsis, pneumonia and empyema.

Continued optimal management, the mortality of patient's with Boerhaave's Syndrome remains high. Mortality rates have been quoted as high as 72 percent and are most likely attributable to difficulty in making the diagnosis. In contrast to spontaneous rupture of the esophagus, iatrogenic esophageal rupture carries a mortality rate of only 20 percent, and traumatic perforation has a mortality of only 7 percent.

Case Conclusion:

The patient was treated in the emergency department with Aspirin, Morphine, Reglan, one liter of normal saline, Zosyn 3.375 grams intravenous, and a nasogastric tube was placed. A thoracic CT scan with oral administration of 20 ml Redicat demonstrated bilateral pleural effusions, with a tract of contrast and air noted within the anterior wall of the esophagus.

A semi erect single contrast esophagram with thin barrium solution demonstrated an esophageal tear adjunct to a short esophageal restriction in the mid to distal 1/3 of the esophagus.

The gastro-intestinal and cardio-thoracic surgery services were informed and the patient was transferred to the ICU

Types and Symptoms Of Heart Valve Disease

There are two types of heart valve disease – heart disease resulting from narrowed heart valves, and heart disease resulting from leaky heart valves. Each of them stems from several different causes, ranging from birth defects, to bacterial infection, to aging.

Congential Heart Valve Disease
Congenital heart valve is the most common, and can result in inflexible or narrowed, or floppy, valves, or irregular valve flaps. Heart valve disease can be very hard to diagnose because some forms of it do not produce symptoms.

Some people, however, acquire heart valve disease because of complications from another disease such as heart muscle disease, coronary artery disease and heart attack. A child who has suffered from a rheumatic heart disease because of a simple strep throat will likely have a valvular disease when he or she reaches adulthood.

Heart valve disease, however, can also develop as a complication from some other illness; children who have had rheumatic fever following an untreated case of strep throat have a greater than fifty percent chance of developing scarring on their heart valves. A heart with scarred valves has to work harder than one with smooth ones, and as the years mount, the extra strain on the heart can lead to rheumatic heart disease.

Another form of heart valve disease which can result from infection is endiocarditis. Endiocarditis develops when bacteria enter the bloodstream during surgery or dental procedures, causing inflammation of the heart and scarring both its valves and leaflets. If you take it to the extreme, I believe that rotating a hot and cold shower could have close to the same effect as High Intensity Interval Training, but either way will help with a workout recovery. If I have the time, here is what I do before I workout.

Cold Water (as cold as I can take it, you might want to scream) for 30 seconds.

1 minute of hot water

Repeat for 10 minutes.

Feed a Cold; Starve a Tumor

Do you ever feel like you are stuck in a rut so deep that you do not think you will ever get out? Reason # 1. Analysis Paralysis.

As human animals, we generally have a tendency to analyze everything that is put before us. After all, is not that why we are equipped with these amazing inner computers called brains? "If this is this, then that is that" kind of thinking will paralyze even the smallest of decisions or opportunities. The truth of the matter is that we do not know, nor can we control, the outcome of situations. The only thing we can control is the reaction that we attach to the outcomes. (And trust me, that is very good news for all of us!)

Overanalyzing also shuts down our intuitive sense, so we no longer have access to following our "gut" or our heart. There are certainly times where it is just best to take a deep breath and a leap of faith. This is not to say that the options are not weighed out beforehand, however. But at least we are not paralyzed. If the choice does not work out as we thought it should, it is but a blip in the radar screen of life. Perhaps it was not the right time or the right direction for us. There are no accidents, though oftentimes we are left scratching our heads for a while as we sit in the aftermath of our decisions.

Also, with enough analysis we have a fool-proof excuse (as valid as it may seem at the time) NOT to make a decision or take action. It is with each decision that we take a step towards our ideal life. Too much analysis will make us freeze in our tracks or, worse yet, start moving backwards and taking refuge in our "comfort zone". Ironically, this is the behavior that should become the focus of our analysis!

Reason # 2. Overwhelm.

There is so much going on in life. How can we possibly stay on top of everything? How can we begin to tackle each item on that "to do" list? We do not have sufficient time. We do not have enough help. We do not have enough experience, drive, willpower, sanity. . . you fill in the blank. We just do not have enough! It feels as though there is no reprieve.

We first need to consider that it is impossible to eat an elephant in one bite. We can not tackle everything at once! We need to break it down into manageable steps in order to make the least bit of progress and build momentum. If it is not at the forefront, let it go for now. If you do not really want to do it, have the courage to say no. If you do not know how to do it, either learn (if you really want to) or delegate to someone who does know how. If you do not enjoy it, say so. If it can not be done "right" if you are not doing it yourself, consider lowering your standards a bit.

We are our own largest contributors to the overwhelm we have in our lives. If you are procrastinating with something, figure out why. Are you afraid? Would you rather say no? Do you feel obligated? Find comfort in only feeling "obliged" to yourself. You teach people how to treat you. If you set up boundaries, people will respond. The clock ticks the same amount of times for each one of us. We all turn the calendar on the same day. Figure out how to prioritize your time, spending it the way that YOU want to. It is not selfish, it's sanity!

Reason # 3. We are misinformed, with a faulty base of operation.

Because we are being constantly bombarded with it, we really start to believe that we need to be what we see portrayed in the media. We feel as though we have to compare ourselves to other people, because we do not really know their true stories ~ we are comparing only to what we think we know. And this becomes the basis for our decisions and thoughts about ourselves. I am here to tell you that this is faulty thinking!

Whatever it is comparing your house savvy to Martha Stewart, your success to Oprah Winfrey, your smile to Julia Roberts or your body to Halle Berry, you are not using the correct information. You can only compare yourself to you! For example, instead of Martha Stewart even entering into the equation. . . How is my homemaking now versus what it was five years ago? Have I improved? In what areas can I get better? Do I even want to? Is it really a priority of mine?

Same thing for success. . . We all know that Oprah rocks, but this is not exactly relevant to our level of success. Ask. . . How have I moved forward in the past year? Where do I want to go? Am I getting closer? If not, what can I do to make it more of a priority? Is it a priority I want for myself or is it something that is expected of me by others? You see the point.

Work on building a solid base of authenticity from which to operate, and you will not only make better decisions, but you will ask better questions too.

Reason # 4. We lack the safety and support that we need.

We all feel vulnerable at times (some more than others). What we are missing is a "safe place to fall" where we can drop the "front" and acknowledge these feelings. It is in our vulnerability that we lose the fear to share our true selves and to reach out for help ~ or at least we feel the fear and do it anyway! We are able to get in touch with ourselves on a much deeper and genuine level when we know it is safe, when we know it will not get thrown in our faces, and we know that it will stay confidential (if we wanted others to know, we'd tell them ourselves, right?). Without this safe haven, we are forced into living a life that lacks authenticity.

It is up to us to surround ourselves with people that we trust and people who will support us no matter what the situation. Although it is a difficult task, I recommend starting to evaluate the relationships that are currently in your life to see if you are surrounding yourself with safety and support or potential poison. I encourage you to really look deep, because things are not always as they appear to be on the surface. Without the obstacle of analysis paralysis, follow your heart and your instinct on this one. And most importantly, BE the person that you would love to surround yourself with. Not only can you benefit great from being your own best friend, you will also attract the kind of people into your life that you need for unconventional love and support.

Reason # 5. We fail to take action.

I believe that fear is the number one reason that we do not take action. (And do not kid yourself here … fear is at the root of procrastination!) Fear of failure, fear of success, fear of looking "stupid", fear of not knowing the right answer, fear of being wrong, fear of whatever! If you find yourself not taking action on something, consider fear to be the number one culprit. Then ask yourself the worst possible thing that could happen if you do take action.

Let's play a game called "What If". Here's how it works:

Say I have to give a speech tomorrow. I am starting to panic, thinking of all kinds of ways to get out of it. . . I'm starting to feel feverish. . . I will have to go to the doctor. . . I'm losing my voice. . . ANYTHING but admitting your fear.

What if you give the speech tomorrow?

People will not like it.

What if people do not like it?

I will look stupid and people will see me as incompetent.

What if people see you as stupid and incompetent?

Then I will lose my job.

What if you lose your job?

Then I will never be able to get another one. I will not be able to pay my bills and I will be homeless.

What if you are homeless?

I will have nowhere to go. I will lose my family and friends. I will be alone. I will die.

Wow. Now put that through your reality filters. Will giving a speech really lead to death? Probably not, but it will definitely point out the ellogical nature of most of our fears. If you do the "what if" exercise each and every time you find yourself resistant to taking action, you will no longer fall victim to your fears because they will no longer hold power over you. Sometimes you just have to take the leap, and build your wings on the way down.

Comparison Between Egyptian and Mesopotamian Religions and Beliefs!

The religious beliefs of the ancient Egyptians were the dominating influence in the development of their culture. The Egyptian faith was based on a collection of ancient myths, nature worship, and innumerable deities. Sumerian lives were spent serving the gods in the form of man-made statues. There was no organized set of gods; each city-state had its own patrons, temples, and priest-kings. The Sumerians were probably the first to write down their beliefs, which were the inspiration for much of later Mesopotamian mythology, religion, and astrology. Sumerians believed that the universe consisted of a flat disk enclosed by a tin dome. While the Mesopotamian’s didn’t have anything quit to scale with the pyramids, they did use and build ziggurats for religious purposes.

Both civilizations were centered on religion. Egypt believed in many gods. The gods Mesopotamia believed in tended to be absolute rulers to whom the people owed total devotion. In both civilizations religious leaders were given very high status and held in high regard. Mesopotamia and Ancient Egypt are two religions that believed in monotheism. Both Egypt and Mesopotamia were polytheistic, that is, they believed their worlds were ruled by more than one god. Both civilizations believed that the gods created them. Both cultures also believed that they themselves were created for the purpose of serving their gods. Both worshipers took their names from the numerous gods and the cults that honored the deities, and priests in both religions were no special clothes, and made daily offering in the temples and held annual festivals open to public.

Mesopotamian religion saw humans as the servants of the gods, who had to be appeased for protection. Egyptians believed that the gods created all humans but were also controlled by the principle of maat, or order. Unlike followers of Mesopotamian religion, the Egyptians had a strong belief in the afterlife, which they expressed by building elaborate tombs such as the pyramids. The Sumerian afterlife involved a descent into a gloomy netherworld to spend eternity in a wretched existence as a Gidim (ghost). Egyptians believed that their gods had created Egypt as a sort of refuge of good and order in a world filled with chaos and disorder. The major god for much of Mesopotamia was the sky god Enlil; later th e worship of Enlil was replaced by the worship of the Babylonian god Marduk. For Egyptians, Amen-Ra was the most powerful deity, chief of the pantheon. Statues of winged bulls were a protective symbol related to the god Sin Mesopotamia, while the ankh, a kind of cross with a loop at the top, was a prominent representation of life in ancient Egypt. The Enuma Elish tells the Mesopotamian story of creation and explains how Marduk became the chief of the gods. The Egyptian Book of the Dead was a guide for the dead, setting out magic spells and charms to be used to pass judgment in the afterlife. Ancient Nippur was the site of the chief temple to Enlil, while Babylon was the location of Marduk’s sanctuary. Thebes and the temple complex of Karnak were home to the worship of Amen- Ra. In the modern world the remains of these early religions can be seen in Egypt’s pyramids, tombs for the pharaohs, and in Mesopotamia’s ziggurats, temples to the gods. The New Year’s Festival was a major event in Mesopotamian religion, while Egypt’s most important festival was Opet. Because Egypt was the “gift of the Nile” and generally prosperous and harmonious, Egyptian gods tended to reflect a positive religion with an emphasis on a positive afterlife. In contrast, Mesopotamian religion was bleak and gloomy. Ancient Mesopotamian prayers demonstrate the lack of relationships with gods and goddesses who viewed humans with suspicion and frequently sent calamities to remind everyone of their humanity. Such was the message found in the Gilgamesh Epic.

Although the religions of both civilizations shared many similarities, the differences were vast. The most notable ones are the importance and belief of afterlife and the relationship between Gods. Because of these differences, we believe, the civilizations were different because in early times, civilizations revolved around their beliefs and values but unfortunately, there was an end to these great civilizations.
