Astigmatism is a condition in which the uneven curvature of the cornea blurs and distorts both distant and near objects. A normal cornea is round, with even curves from side to side and top to bottom. With astigmatism, the cornea is shaped more like the back of a spoon, curved more in one direction than in another. This type of curving causes light rays to have more than one focal point and focus on two separate areas of the retina, distorting the visual image. Two-thirds of Americans with myopia or hyperopia also have astigmatism.

Presbyopia commonly affects people as they enter their 40s, and it occurs as the lens inside the eye becomes less flexible and the muscles controlling the lens weaken. People with presbyopia may experience blurred vision when performing everyday "close-up" tasks, such as reading, sewing, or working at the computer. This is why some people with presbyopia need reading glasses or need to hold objects farther away to see them.

Correcting the curvature of the cornea is different for each condition previously mentioned. Glasses and contact lenses are designed to compensate for the refractive errors caused by myopia, hyperopia, and astigmatism. Many people, however, want to reduce their dependence on glasses and contacts. The reasons people find eye glasses or contact lenses are diverse.

There are a number of highly advanced surgical procedures available that effectively reduce the need for glasses or contact lenses. However, not everyone is an ideal candidate for these procedures. I’m sure I can speak for most people, that we want to sustain a healthy life so that we have the ability to live up to a ripe old age. One such sore subject we never like dealing with is weight loss. The one negative, green tea is not the most tastiest of teas, it kind of has that hint of grassy seaweed taste, but you get used to it.

In addition, from what I understand, green tea has antioxidants to prevent sickness, make our skin feel smooth, and protect our memory. But by seeing the short term effects, I can say that I will make green tea a regular staple in my diet. I would not recommend green tea pills as they have caffeine in there, don’t want to be up into the wee hours of the morning! All in all, my thirty day test has proven that green tea has great positive properties that just can’t be ignored. Let’s see what other effects it has six months from now!

Importance Of Pediatric Advanced Life Support

Emergencies occur anytime, anywhere, which is why being prepared is something that most people should exemplify. The American Heart Association took this belief by heart, which had served as the basis of their program called Pediatric Advanced Life Support or PALS. AHA originally developed PALS classes in 1983 as response for the needs of resuscitation guidelines and trainings that would aid health care professionals in providing care for patients especially for children. Since then, PALS provide health care professionals the skills and knowledge to properly respond and treat critically ill patients suffering from cardiac arrest or other cardiopulmonary emergencies, specifically infants and children, whether in or out of the hospital.

PALS is a program that follows simulated clinical scenarios and learning stations to actively encourage students to learn and practice life-saving skills individually, as a part of a team and as team leaders. PALS give emphasis on the importance of basic life support CPR for the survival of patients, integration of advanced pediatric life support intervention as well as the importance of team communication, interaction and cooperation during resuscitation. The realistic simulation involved in PALS course reinforce major health care concepts that include basic life support care proficiency, cardiac arrest management, recognition and early management initiation for peri-conditions, algorithms, ischemic chest pain and coronary syndrome identification and treatment, effective team interaction, and recognition of other life-threatening conditions in order to administer initial care.

To get PALS certification, individuals are required to take PALS course. This could be done in two different ways either through AHA approved online PALS providers or through attending PALS classes in medical schools or hospitals. If you are looking to complete your PALS certification immediately, you could go online and complete your PALS course from your computer at your own pace. If you are more adept to learning in a classroom setting, it would be more preferable to attend in medical schools or hospitals that offer PALS course.

PALs classes have a standardized curriculum wherein students attend a classroom-based education led by an instructor with video aid and skills conducted throughout and are provided with materials for continuing education that is strongly based from AHA’s “assess-categorize-decide-act.” PALS course lasts for 14 hours long and most providers offer it for two intensive days. They are also expected to complete and pass a written examination before they receive their certification.

Accordingly, the development of PALS has been shown to consistently increase the survival rate of patients suffering from cardiac arrest or other cardiopulmonary emergencies, who were attended by PALS certified health care practitioners in pre-hospital settings. Types of Underpads Used in Hospitals and Homes

The two primary types of underpad used in hospitals are disposable underpads and washable or reusable underpads. In this article, we will discuss the features of these two types of underpads in more detail. Here, it must be mentioned that although the biggest consumers of these products are hospitals and nursing homes, individuals with bed-ridden patients, disabled or elderly individuals or even those with babies at home can and do use these pads.

The disposable underpads as their name suggests, are meant for one time use. They are available in a range of construction types and sizes. They usually contain a waterproof plastic material for the bottom, a middle layer designed to absorb liquid and a top layer made from a soft and comfortable fabric. The middle layer usually houses multiple layers of tissue to absorb fluids. The topmost layer of these units come in contact with the patient’s body, therefore manufacturers use materials such as cotton that will make patients are comfortable while using their disposable underpads. You will also come across pieces in which the top sheets are made up of tissues or a tear-resistant spun-bond material that allows for movement without fear of ripping the pad.

There are some disposable underpads carrying top sheets attached to the plastic layer at the bottom by means of an adhesive. Then, there are others with two sides rolled and glued to ensure that the liquid comes out only from the side in case the pad overfills. Experts refer to this process as channeling. The pads adopting this process are known to be more absorbent compared to the other underpads available on the market. You can get all these disposable underpads in a range of sizes; so, purchasing pads that match your body, bed or chair size will be simple.

Now, we will be talking about the washable or reusable underpads. These pieces are classified based on factors such as absorbency, materials, and size. The term absorbency refers to the volume of urine or bodily fluid that an underpad is capable of holding. The material used for crafting the absorbent layer is a kind of absorbing polymer.

The quantity of polymer used and the material’s density are two factors that decide how much liquid the underpad is capable of absorbing; basically, the underpad’s urine holding capacity. Here, it must be noted that one should never confuse an underpad’s absorbency level with its size. A pad that’s large in length and width might not absorb the most urine.

Smaller sized pads are designed for use in chairs and make excellent absorbers in baby cribs or on diaper changing tables. Many hospitals use small pads in bassinets for easy clean up.

US Citizenship Options For Elderly Applicants – Medical & Exam Waivers

I frequently receive calls from the adult children of US permanent residents (i.e. green card holders) who want to know how their parent can become a US citizen if the parent cannot study for the English history test and / or does not speak English very well. I have created an article for you on the two most common scenarios that I answer weekly in my US Immigration Law Office of Lena Korial-Yonan, P.A.:

1. My mom is eligible for US citizenship because she has had the green card for 5 years, and she does not have extensive travel or any criminal issues. The problem is that she does not speak or read English and cannot study for the history test. Any solution?

USCIS has specific rules that relate to an applicant’s age and how long they have had the green card that can exempt them from having to take the US English History test and also exempt them from passing the interview portion of the N-400 as well as the reading and writing exam. Please see the next question for more details on this.

However, here the applicant has had the green card for only 5 years and so the only solution is to see whether the applicant is eligible for a medical disability waiver. A medical disability waiver is completed by a US licensed general medical doctor, licensed clinical psychologist or other specific doctor on USCIS form N-648, Medical Certification for Disability Exemptions. The form must be completed a specific way and with specific wording / answers before USCIS will approve the medical waiver. The doctor, in order to complete the medical disability form, must give your parent, the N-400 applicant, a test to determine whether he or she can learn or remember basic things. This test can include having to recite basic information just explained to your parent and may also include basic exams having your parent place a circle object with circle object, etc.

Applicants with dementia or Alzheimer’s, for example, are eligible to file form N-648, Medical Disability Waiver, which states that because of their mental limits, they cannot study for the US history test. A major limitation for eligibility under form N-648 is that the resulting mental disability cannot be a result of drug use.

Our immigration law office has successfully completed medical waivers prepared by licensed doctors of applicant’s own choosing for the mental conditions of dementia, Alzheimer’s and schizophrenia. Please note that these cases are being mentioned are examples and in no way guarantee that your case will have similar results for both the mental conditions listed and for the USCIS district that you will file citizenship with. Please contact an experienced immigration lawyer of your choosing for a detailed analysis of your own specific case.

Although our immigration law office does not have any doctor(s) that we recommend for completion of Form N-648, we will let you know if a particular type of doctor can complete the form on your behalf. We also review the Form N-648 for completeness, as USCIS is strict in the language that they require from the licensed medical doctor in order for the US citizenship applicant to be approved. If the Form N-648 is approved by the US Immigration Officer, then the US citizenship applicant does not have to take any test in order to have their form N-400 approved and be sworn-in.

Also, at least in the USCIS district office located in Jacksonville Fl, we usually file a copy of the Form N-648 with the N-400 application, and we bring the original N-648 with us to the N-400 interview. You may of course provide the original N-648 in your initial filing if you wish. Just be sure to keep a copy of the form N-648, as it is not uncommon for USCIS to require a few changes to the form N-648 before your case can be approved.

2. My mom is 65 years old and she has resided in the US as a green card holder for over 20 years. Does she still have to take the English test, etc.?

No, your mom has to take a modified version of the civics test in her language of choice. She does not have to take the reading or writing test. The N-400 interview, which includes reviewing the details of the N-400 and asking whether your mom has ever been arrested, files US taxes, etc. is completed with the help of the translator that your mom brings with her to the N-400 interview. She must bring her own translator with her to the N-400 interview so that the translator can translate the civics questions in your mom’s best native language.

The age qualifications that exempt applicants from the English, reading and writing tests are as follows:

· 65 years and has resided in US as a green card holder for 20 years or more.

· 55 years old and has resided in the US as a green card holder for 15 years or more.

· 50 years old and has resided in the US as a green card holder for 20 years or more.

Again, Applicants that meet one of the above categories must only take the civics test in their language of choice.

My Spouse Says Our Marriage Will Fail Because He Doesn’t Deserve Me After The Affair

Many people understand that the time period after learning of your spouse’s affair is a volatile one. Often, it’s impossible to think clearly and objectively. And for these reasons, many people understand that, most of the time, it’s not in your best interest to make any important and lasting decisions until you are able to calm down and think rationally. But not every one has this luxury because sometimes, your spouse is the one who will make rash decisions.

A wife might say: “much to my great shock, I did not immediately leave or kick my husband out after his affair. I was kind of numb and I knew that I wasn’t thinking clearly. So while I asked for some time before we really talked this through, I did not do anything drastic. I truly don’t have any clue if we can save our marriage. But I would like the opportunity to explore that. However, I might not get that opportunity. Because my husband announced last night that he thinks our marriage might be over because he feels that he doesn’t deserve me and he thinks that he will never be able to get over these feelings of inadequacy. Frankly, this makes me angry. He is the one who cheated and now he gets to call the shots? What if I don’t care if he doesn’t deserve me?”

This isn’t that all uncommon. Sometimes, the cheating spouse is being absolutely truthful when they make this claim. They fully believe that their actions have made them unlovable and unredeemable. And they feel that you might ultimately be better off without someone as flawed as them.

But other spouses pull the pity card to try to get you to say something like: “you’re wrong. You do deserve me. You are a good person who made a mistake. Let’s start over.” What cheating spouse would not want this easy, fast way out? It helps to be aware that, if you allow them to take this quick out, then you may be cheating yourself out of rehabilitation and healing.

That doesn’t mean that you just have to accept what he is saying without having a conversation about this. You can certainly tell him your opinion by offering something like: “all of the research that I’m doing says that you shouldn’t just make drastic decisions about ending or saving your marriage until you’ve taken the time to evaluate things calmly. It is too soon for us to do that. So I think that it is too soon for us to declare that our marriage is over. We might need to explore counseling or having meaningful conversations before we are at a place where we can make a decision about our marriage. But I think it’s very premature to make a marriage-ending decision before we even attempt to sort this out. We may be able to heal and there may be a time in the future when we both feel deserving of the other. We won’t know that unless we take the first steps toward exploring how we feel and seeing what we can salvage. But to just declare that our marriage is probably over because of any initial feelings, that’s just rushing. Can we just wait to see what happens in the days to come before we make any huge, life-changing decisions?”

His answer may give you some clues as to whether he really believes in the whole “I don’t deserve you” claim or if he’s posturing just a little bit. Because you’re not telling him that he DOES deserve you right this second. You are just saying that you are willing to wait and see what tomorrow brings.

He may frankly believe what he’s telling you. But the most effective way for him to eventually believe that he is deserving in the future is to become rehabilitated, to do the work, and to eventually become the best husband that he can be.

He obviously can not do that if he bails at the first sign at trouble. And part of taking responsibility is hanging in there regardless of whether or not there are any guarantees. Sometimes, he is looking for reassurance from you. But no one can give him reassurance before the work has been done.

So I think that the best strategy is to try to convince him that this type of decision is premature. Then, you do the work. Honestly, if he is willing to do all of this, then it only makes sense that the both of you are much more likely to think that he is deserving in the future.

Multivitamin Comparison – From Empty Air to Massively Boosting Health

A staggering 50 out of the total 110 multivitamins did not even reach 10% and only the top 11 multivitamins were above 25% of what the human body needs on a daily basis.

The popular supermarket brands all featured below 10% possibly due to being forced to compete on price with other supermarkets. The downside for you, means the product you take every day is a hugely inferior supplement.

In other words, you get what you pay for. In some cases ignorance is bliss, in others it is a big wakeup call. My father has taken supplements for many years before and after he developed cancer and never once questioned the quality of what he was consuming. After reading this, he too, wondered why he ever thought not to research something so important to his health, when he did such a great job of researching anything else that he bought.

My advice, choose a multivitamin that is backed by a great company with a good reputation and do not just buy any old vitamin because of the price. You would be better off spending the money on something else.

Stress Relief and the Smooth Flow of Energy

My first conscious brush with stagnant energy occurred during my first semester in school to study Acupuncture. I was returning back to school for a master’s in Oriental medicine after having been away from the academic world for years (okay, maybe decades). I can remember thinking at the time, “How hard could this be?” Much to my surprise, it was incredibly hard. I had classes during the day and studied every night and weekends. I was working part time and my husband traveled for his business. My kids were still in grade school and junior high, and they weren’t happy with my return to school.

About two months into the semester, I began to get this funny feeling in my throat. It felt like a lump stuck somewhere below my Adam’s apple and above my sternum that made it hard for me to swallow and very uncomfortable to eat. After lots of antacids and a trip to my doctor, I ended up in the Chinese medicine teaching clinic at my school. Within the framework of Chinese medicine, my symptoms were easily diagnosed. I had something called Plum Pit Qi, which sounded exactly like what I was feeling–a plum pit was caught I my throat. I learned that Plum Pit Qi is a kind of stagnation of Qi (pronounced chee), or energy, and occurs as the result of a situation that is figuratively too difficult to swallow.

While the acupuncture and herbal treatments at the clinic were very effective, I also had to acknowledge that I had taken on too much. I had to either quit school or find a way to balance all the commitments I had taken on. After retooling my life somewhat, quitting my job, and studying when my kids were occupied or in bed, I managed to get through that first semester and ultimately, three years later, complete my master’s degree.

Perhaps what was most startling to me from this whole episode was how powerful the physical symptoms were from stress that was purely emotional. In Chinese medicine this phenomenon is related to the smooth flow of Qi, or energy, throughout the body. Qi can be described as life energy or the energy necessary for transformation to occur.

Every cell in our bodies make energy. One of the foundations of Chinese medicine is that the energy in our bodies move along pathways, and good health is dependent on the unobstructed flow of that energy throughout the pathways of the body. The smooth flow of Qi in our body is most closely related to the Liver, the symbolic organ system (not the actual Liver) associated with the element of wood. This may relate the hard wood found in trees, but the color associated with the Liver is green or blue/green, suggesting younger plants and their slow, steady growth upward toward the sun. If some obstacle (or stressor) inhibits that growth, then the plant will grow crooked and become deformed.

While the Liver is responsible for the smooth flow of Qi, it also has a strong relationship to the smooth flow of emotions. When our emotional state is even, Qi will flow evenly throughout our body. However, during emotional upheaval or unrelenting stress, our Qi tends to stagnate. This stagnation is much like turning on the water to your garden hose and then bending it until the flow is cut off. This blockage of Qi can cause numerous physical symptoms, from my Plum Pit Qi, to insomnia, headaches, gynecological problems, and pain.

Ultimately, smooth emotions equal smooth Qi. Unfortunately, this is easier said than done. We have all, at one time or another, become caught up in the way we think things should be and have trouble accepting what really is. For many of us this feeling is constant, and when the feeling becomes overwhelming, our Qi becomes bound up and begins to stagnate. This feeling can take the form of too many things to do and not enough time, a job we don’t like, people who annoy us, or anything that makes us unhappy, uncomfortable, frustrated, or angry. The end result may be overwhelming stress, anxiety, depression, or panic attacks.

On a daily basis, we see people attempting to move Qi, some more successfully than others. On any given weekend morning during the summer, we see thousands of people outside running, biking, and walking. They usually feel better after having exercised, because physical activity moves Qi. However, practitioners of Chinese medicine would caution that excessive exercise can be depleting.

Drinking a cup of coffee speeds things up and also moves Qi, too. However, coffee is a diuretic, and excess can also be depleting. Like coffee, chemical use/abuse is an attempt to move stagnant Qi and feel better, but the depleting effects of such practices are obvious.

Perhaps the most successful ways of dealing with stagnant Qi, and ultimately the emotions, are some of the Eastern practices, such as taiqi, qigong, or yoga. These combine gentle exercise with breathing techniques that allow us to even out emotions, nourish our bodies, and smooth Qi. Through proper breathing and quieting the mind, meditation is also an excellent way to calm the emotions and allow the Qi to move freely.

When these practices aren’t enough, or when stagnant Qi begins to manifest as illness, acupuncture can help bring relief. Many people believe that acupuncture is best used for relieving pain. However, it’s also very effective in treating stress-related disorders such as irritable bowel, headaches, PMS, depression, and insomnia.

Like the “Plum Pit” caught in my throat, many physical symptoms that make us uncomfortable are actually a barometer to our emotional health. Often, when we have ignored the obvious signs that we are doing too much or need to make an emotional change, this discomfort finally gets us to stop and take our lifestyle into account.

High Blood Pressure and Heart Disease

There are many things which contribute to the development of heart disease and problems such as a heart attack. Some factors that have been implicated include:

  • cholesterol and other blood fats (although this appears to have been over emphasized),
  • coronary inflammation (homocysteine and C-reactive protein),
  • blood clotting,
  • weight,
  • blood pressure,
  • diabetes,
  • syndrome X (believed to be a product of insulin resistance),
  • aerobic capacity,
  • smoking and
  • personality.

As well as these there are genetic factors. This is a variable over which we have no control. If you do have a family history of heart disease this does not necessarily mean that you will develop heart disease yourself. There are many factors, besides genetics, that go into the development of heart disease. It is these factors that can be changed and thereby reduce your risk of developing heart disease. One area where you are able to bring about change is your blood pressure.

Blood pressure

Blood pressure (hypertension) is the force that blood exerts on your arterial walls. It is needed to pump the blood through your arteries and veins. All vessels, including the arteries provide resistance to the pressure of the blood. If the arteries are narrow or thickened or have lost their elasticity then they provide more resistance to the force of the blood. Resistance creates pressure – causing the heart to work harder to keep the blood moving. A high blood pressure can lead to many problems including: strokes, heart attacks, it can affect the arteries lining the lungs and rupture the arteries (an aneurysm).

It is estimated that high blood pressure affects about 63 million Americans. One study done by Boston University estimated that 9 out of 10 middle aged Americans will, at some point, develop high blood pressure.

The following are symptoms that are associated with the early stages of high blood pressure:

  • dizziness,
  • chest pains,
  • swelling in ankles and feet,
  • headaches,
  • changes in vision,
  • leg cramps and
  • loss of concentration.

Blood pressure is expressed as two numbers measured in millimeters of mercury (mm Hg). The top measure is the systolic pressure which is taken when the heart contracts – when the blood is pumped out of the heart – and placing the greatest force on the walls of the arteries. The bottom number is the diastolic pressure and is reflects the pressure when the heart is at rest – between the beats of the heart. This is when the pressure of the blood on the arteries is at its lowest.

In simple terms, the commonly accepted normal systolic pressure is 120 mm Hg and the normal diastolic is 80 mm Hg (120/80 mm Hg).

Blood pressure varies naturally with weight, age, activity, body position, time of day and emotions being felt (including being anxious about having your blood pressure taken).

The following are regarded as the standard for assessing blood pressure.

systolic less than 120 and diastolic less than 80 – optimal

systolic less than 130 and diastolic less than 85 – normal

systolic 130 to 139 and diastolic 85 to 89 – high normal

systolic 140 to 159 and diastolic 90 to 99 – stage 1 hypertension

systolic 160 to 179 and diastolic 100 to 109 – stage 2 hypertension

systolic 180 and above and diastolic 110 and above – stage 3 hypertension

It is important to remember that high blood pressure and heart disease don’t just suddenly occur – the risk of developing heart disease rises along a continuum as blood pressure increases.

What can you do to reduce your blood pressure?

  • High blood pressure may be caused by a number of nutritional deficiencies including omega fatty acids, calcium, and magnesium. You need to supplement with high quality non-contaminated vitamin and mineral supplement.
  • You need to eat plenty of fresh fruit and vegetables and avoid fatty foods, dairy products and reduce your intake of animal protein. Reduce the amount of refined carbohydrates and sugars that you consume and avoid tea, coffee and alcohol.
  • Coenzyme Q10 has now been shown to reduce blood pressure and has a beneficial effect on the walls of the blood vessels.
  • You need to cleanse your body, including your arteries, of toxic material so that everything, including your circulatory system, is able to work effectively.
  • If you smoke one of the best things that you can do is to quit. Join a quit smoking program for the support and assist you need and go through the process of becoming smoke free.
  • Get plenty of exercise and if you are overweight take steps to lose it. Excessive body weight is associated with high blood pressure.

There are several herbs that will dilate the blood vessels, thereby increasing the total volume of the system. Similarly there are several herbs that will help the kidneys pass more water thus reducing the amount of fluid in the system. The most important herbal remedies are:

  • buckwheat,
  • cramp bark,
  • hawthorn berries,
  • lime (linden) blossom,
  • mistletoe and
  • yarrow.

If you are at risk of developing heart disease because of high blood pressure (and any other factors) don’t leave it until it is too late – take action now and enjoy an improvement in your overall health as well as the health of your heart and circulatory system.
