Brachytherapy Showing Promise For More Than Prostate Cancer

Brachytherapy has shown promise for a few different forms of cancer and even for arteries which were unplugged from angioplasty. Low-dose rate (LDR) prostate brachytherapy is generally well tolerated in patients with a history of inflammatory bowel disease. Episcleral plaque brachytherapy is a complex procedure and should only be undertaken in specialized medical centers with expertise in this sophisticated treatment program. It is not yet known whether interstitial (within the tissue) brachytherapy is more effective with or without external-beam radiation therapy in treating prostate cancer.

The most common uses of this form of treatment involves the planting of radioactive seeds through 18 gauge needles into the prostate via a perineal template with rectal ultrasound guidance. High dose rate (HDR) brachytherapy is one of the latest advances in this type of treatment method as it's very effective for treatment of cervical cancer as an alternative to surgical removal of the cervix and uterus. Additionally, HDR brachytherapy is used to reduce the risk of recurrence of soft tissue sarcoma following surgery.

Permanent prostate brachytherapy is a form of low dose chemical implants (up to 120) rice-sized radioactive seeds into the prostate with guidance from ultrasound and X-rays. Low-dose rate treatment is also used in the treatment of coronary artery disease to prevent restenosis after angioplasty. High-dose rate (HDR) is usually an outpatient procedure.

Breast brachytherapy is a more localized means of treatment than standard external beam radiotherapy, so there is a less dose to undamaged tissue of the breast, heart, lung, muscle, skin and ribs. LDR (low dose) brachytherapy is the most commonly used of the two. LDR and HDR Brachytherapy is performed under anesthesia, and radioactive "seeds" are each placed in needles that are inserted into the target through the perineum (the area between the legs, scrotum and anus).

LDR brachytherapy makes use of iodine-125 and palladium-103 stored in titanium cases usually referred to as the "seeds". A big advantage of HDR brachytherapy is that the final doses are known before any radiation treatment is given.

Currently, at Wake Forest Baptist in North Carolina, cancer of the prostate, breast, lung, cervix and nasopharynx are being successfully crowned with HDR brachytherapy. Endobronchial brachytherapy is an outpatient procedure again using radioactive sources enclosed within a bronchus (airway) to treat patients suffering from airway obstruction due to lung cancer and other tumors.

The utilization of brachytherapy is highly dependent on the experience of the physician and the available resources although, the flexibility of the HDR system allows it to be used for a wide range of malignancies.

HDR brachytherapy is most commonly used with cancers located near the outside of the body but with MRI assisted techniques, it's possible to adjust the isodose distribution by taking into account tumor size and features as well as the topography of the risked organs.

Although it's true that HDR brachytherapy is currently used for the treatment of cervix cancer in the UK, most of the techniques are based on standard triple applicator procedures. While brachytherapy is not commonly used for the treatment of vulval cancer in the UK, there are some options for the management of these rare patients with small vulval or vaginal situations which are not surgical candidates.

Before the development of this type of treatment, surgery was the only option but now prostate brachytherapy is an alternative to traditional external beam radiation or surgery for men who have early stage prostate cancer (cancer that has not spread outside the prostate gland). The most common hindrance prevention brachytherapy is when there is benign prostatic hypertrophy and the long term prognosis for retaining erectile function after brachytherapy is similar to or slightly better than with surgery or external beam radiation.

Proton brachytherapy is considered the method of choice as opposed to neutron brachytherapy for treating small tumors for the fact that protons do not relocatd as far as neutrons. HDR treatment is given over the course of several minutes, but the entire procedure typically takes a few hours. Once a course of HDR is started, the treatment plan and computer guided isodose calculations are finished within 24 to 36 hours.

As always it's wise to discuss any new treatment options with your trusted doctor and / or cardiologist with experience in radiation therapy to see if brachytherapy is just what the doctor ordered for your unique situation.

Children should be trained and disciplined to keep their rooms neat. Start them young to become responsible adults. These light tasks are appropriate for children at least 4 years of age. Of course, do not expect their work to be perfect, but with time, they will be able to get the hang of it.

Teaching them basics

Placing their dolls, cars, and tiny tea sets in the toy chest will be the easiest job they can do. At the start, they will just toss their toys into the chest. Later on, you can show them that it is smarter to arrange things inside the toy chest. Explain to them that being organized has its rewards.

They can find the transformer or the rag doll at once, if things are arranged inside the toy chest. You can show them how to put their books properly on the bookshelf and check on them if they are following your instructions to the letter. Teaching them these responsibilities will train them for life and they will thank you for it.

Lead by example

Show the kids how you keep your room neat and clean and how nice it is to come home to a clean house. You can demonstrate how you sort out your vanity table, books, and craft basket.

To make the training fun, you can let the kids help you around the house too. Give them the responsibility of watering the potted plants and feeding the dog. The older children should be given responsibilities they can handle. Never assign kids tasks their little bodies can not handle, and be patient with them. Nagging them all the time will not give the desired results. If little Mickey has done a good job, then praise him lavishly. Do not relax your discipline, however. Let the kids know you mean business.

Make their jobs fun

Come up with fun activities that they enjoy outdoors while they pick up the litter. Why do not you prepare their favorite sandwiches and cold drinks and surprise them with a hearty snack when their chores are done?

Providing the appropriate storage facilities will get the kids' cooperation. Organizers just as tall as them or a big toy chest can make their work manageable. Kid-sized furniture and storage bins in bright colors and playful designs will get the kids going. You can let the child choose his or her toy chest for starters. Teaching the kids house chores will also strengthen your bonding. That is why you should make work also fun.

The 4 Stages of the Counseling Process: What Every Youth Counselor Should Know

There is a natural progression that takes place within the context of the helping relationship. This process enables you and the person you are working with to build a relationship, assess the situation, set goals and come up with a plan to bring about your desired results. This progression is known as the counseling process. There are four stages of the counseling process. They are: developing a relationship, making an informed assessment, establishing mutually agreed upon goals and objectives and developing an implementation plan.

Phase 1. Developing A Relationship

In order to develop positive helping relationships with youth, you’ve got to be able to connect with them. This can only happen when youth are made to feel like you genuinely care about their well-being and that you understand where they are coming from. It’s about behaving in a way that demonstrates the core conditions of genuineness, respect and empathy.

To develop solid relationships with youth, you need to create a safe environment where young people will feel comfortable enough to open up to you and talk to you about anything that is on their minds. You also need to help youth see that despite their circumstances they have strengths. In short, you should start things off from a strengths-based perspective.

Questions to Consider When Trying to

Develop A Relationship

· In what ways can you build better relationships with the youth in your program?

· If there are youth who are not actively engaged, what can you do differently to engage them?

· If a youth is resistant, what steps can you take to reduce resistance?

· What worked in the past with resistant youth?

· How do you know when you’ve built a solid relationship with a youth? Could you use these indicators to strengthen your relationships with other youth?

Phase 2. Making An Informed Assessment

An informed assessment happens when both you and the youth gather information in order to figure out what’s “really” going on so that you can assess what needs to happen next in order to change the situation for the better or build up the youth’s coping skills to better deal with a problematic situation. The first step in making an assessment is to find out if change is necessary, and if it is what needs to happen for change to take place. If you have determined that change is necessary, then the next step is to figure out what needs to change. Is it a behavior? An attitude? A situation?

A good assessment can provide an opportunity for a young person to see how his/her behavior or attitude might be contributing to an undesirable or unhealthy situation. Assessment is an ongoing process. You need to regularly check in with your youth to see how things are going. Reassessments enable you to ensure that you and the youth are on the right track.

How do you gather information in order to make an informed assessment? You can gather information in a number of ways: talking with youth, observing the youth’s behavior and interactions, discussions with other people who are involved in the young person’s life, and reading any documented information on the young person. Keep in mind that when utilizing someone else’s verbal or written report as a source of background information, you run the risk of subjecting yourself to their biases and assumptions.

Points to Keep In Mind When Making An Assessment

· Be aware of your biases and how they impact on the assessments you make.

· Involve youth in the assessment process.

· Don’t rely on one single source to make an assessment, gather as much information as you can from a variety of sources.

· Don’t automatically label a behavior as dysfunctional because you don’t understand it, or it is not germane to your culture.

· Make sure to point out a young person’s strengths even when addressing problematic behavior.

Phase 3. Establishing Mutually Agreed Upon Goals and Objectives

Why is it important to establish “mutually agreed” upon goals and objectives? Because if a young person is in agreement with the goals then he/she is more likely to follow through on them. When a youth is actively involved in the goal setting process and is in agreement with the goals, then he/she is more inclined to take ownership of the goals. What are goals? Goals are broad statements that identify what you want to accomplish. Think of goals as the end result that you are trying to achieve. While goals are broad statements that identify what you want to accomplish overall, objectives are the measurable steps that you take to achieve your goals. For example if you have a goal that states, “youth will be better able to manage her anger.” One of your objectives might be, “youth will recognize emotional triggers that lead to angry outbursts and use positive, self-talk to calm herself down.” Your objectives should always be concrete and measurable. They should also be derived from the overall goal.

Questions to Consider When Developing

· What do you and the young person want to achieve?

· How are you going to achieve it?

· When do you want to achieve your stated goal?

· What obstacles do you anticipate?

· How will you address these obstacles?

· How will you use to measure and monitor progress?

· Are your goals realistic?

Phase 4. Implementation Plan

The implementation plan is a plan that you and the youth work on together. It is designed to prevent, intervene, or address unhealthy behaviors and practices. The implementation plan identifies who will perform the activities, where the activities will occur, how frequently they will occur, how they will be carried out and when they will be carried out. Implementation activities are designed to help individuals re-think risky behavior, work through problematic issues, address unhealthy lifestyles practices, learn new skills and build strengths. Implementation activities can include: counseling, crisis intervention, training and education, supportive services, concrete services and constructive use of free time.

As you can see, each stage of the counseling process builds upon the former. As you move through each stage, you will come to realize that it takes patience and practice to counsel youth effectively, but if you are committed to the goal you’ll do just fine. You may not feel completely confident in your ability as a counselor, but as you expand your knowledge base, gain more experience and strengthen your helping skills, you will become a more effective counselor.

Diseases are among the most unfortunate realities in human life. Illnesses are dreaded as they can cause not only inconveniences but even disadvantages in a personís lifestyle. The pain and suffering one has to end because of some ailment is severely describable, add to this the distress and misery experienced by oneís family, friends, and loved ones. What could be more devastating than a dreaded disease other than the combination of two or more diseases? And this was just what some Australian doctors have discovered – the combination of diabetes and kidney diseases.

August 6, 2006, Google News – a recent research in Australia has just discovered that one out of two patients diagnosed with Type II Diabetes also suffers a type of chronic diseases in the kidneys, and the figures may be even higher than what has been observed , doctors feared. Blood samples from four thousand diabetic patients were tested by the Baker Heart Research Institute and the results showed that although the patients were seemingly fine except for their diabetes, they had under-child conditions that remained undetected for years. This has pushed Australian doctors and health institutions to perform mandatory tests to screen kidney diseases among Type II Diabetes patients, according to a subsequent report by Yahoo! News. The tests recommended include not only simple urine tests, but more extensive blood tests.

Type II Diabetes is a disorder in a personís metabolic function that manifests in the resistance to and the deficiency in insulin, it is also characterized by hyperglycemia. Currently, the disease has no cure. It has been observed to be increasing greatly in the developed countries, the rest of the world is also following suit in the coming years that its rapid growth has already been considered as a form of epidemic. Unlike in Type I diabetes, there is not much inclination to develop ketoacidosis (the accumulation of acid in the blood due to the breaking down of fat) in Type II diabetes. But the two types of diabetes could cause disability in several vital organisms, particularly in the cardiovascular system due to the multifaceted changes in metabolism occurring.

The kidneys are the organs that are responsible for filtering wastes in the blood. The bean-shaped organs come in pairs and perform the vital function of removing wastes from the body in the form of urine. Kidneys also keep the balance in many of the bodyís conditions including the concentration of acids and bases, the volume of blood plasma, blood pressure. Kidneys also secret certain important hormones for normal function. Great dangers arise when the kidneys grow awry. Without the kidney functioning well, body wastes may reach toxic levels, causing poisoning and great impairments and the bodyís many functions. Advanced onsets of kidney diseases may create the need for dialysis ñ filtering blood painstakingly outside the body, or the need for the kidney transplants.

Clearly, both conditions of diabetes and kidney disease are dangerous, if not fatal to oneís health. The combination of these two illnesses could be even more hazardous considering the grave symptoms that they cause. Curing both diseases is still rather difficult nowdays if not unfeasible. Such is why prevention is always seen as the best cure. But for those who already have one of the two conditions, early detection for the other is the next best thing. Perhaps developments for cures and treatments may be available in the near future, but for now, patients would have to deal with the diseases according to current accessible therapies.

Herpes Symptoms in Women – How to Identify a Genital Herpes Outbreak

Hamster Diabetes – What to Look For!

Diabetes is a chronic condition in which an animal either does not produce insulin or the insulin produced is not effective. This disease has been documented in dwarf hamsters – especially Campbell's Russian hamsters, but in other types as well. It is a condition rarely seen in Syrian hamsters. It is possible for hamsters to develop diabetes as they age due to hormonal changes. Diabetes in hamsters can cause serious complications, just as the condition can in humans. Special care and attention must be given to hamsters that have been diagnosed with diabetes.

Diagnosis can be difficult, since there are other conditions that can mimic this disease, such as kidney and bladder infections. Symptoms include excessive thirst and copious urination. Excessive defecation can sometimes be a symptom, as well. If you suspect diabetes, consult your veterinarian for a diagnosis.

At this time, treatment options are limited to changes in diet and encouraging more exercise. There is no cure for the condition. Do not feed your diabetic pet fresh fruit or vegetables. Avoid feeding commercial trees, as these often contain honey or other sweeteners. Remove peas and corn from their feed.

Age is a consideration with diabetes. If you have an older hamster that is displaying symptoms, diabetes may be a likely cause. (In younger hamsters, a kidney or bladder infection might be more likely.)

Species is a factor. Diabetes is rarely seen in Syrian hamsters and is usually found in dwarf hamsters. If you have a Syrian hamster, a kidney or bladder infection is more likely.

Weight is something else to consider. Obesity in hamsters (just like in humans) can contribute to diabetes. Avoid feeding your hamster "people junk food" such as potato chips, French fries, cookies, cakes and candy. Although hamsters love these foods and it is fun to watch them eat these things, eating these types of foods can cause hamsters to have significant weight gain and does not give the nutrition that hamsters need. Weight gain and obesity can also make hamsters unwilling or unable to get the proper exercise, which also leads to diabetes. It is also wise to avoid feeding even healthy hamsters too much fresh produce and commercial trips. Treats are best reserved for special occasions, not for everyday feeding.

Complications include heart failure, blindness, cataracts, and kidney failure. Blindness and cataracts are not life-threatening for your pet. Hamsters are very near-sighted any way, so being completely blind is not too serious for them (however, some hamsters are more timid if totally blind) and they can still live productive lives. Heart failure and kidney failure are fatal, however, so great care needs to be given to avoid these complications.

If the complications of this disease become too debilitating or painful for your pet, you may want to discuss the option of euthanasia with your veterinarian. At times, loving your pet can mean ending unnecessary pain and suffering. Your veterinarian can give you your options. If you do choose euthanasia for your pet, be sure to only have a veterinarian perform the procedure.

It can also produce the appearance of warts around the nail bed, and what can be dangerous: passing bacteria and fingers to the mouth. Why is it dangerous? As you know children tend to play everywhere: on the ground, the street, and other places that are full of diverse bacteria.

What can you do about it?

You have to talk to your child in a positive way. Ensure that he or she does not feel punished; instead talk to your kid about what could trigger such behavior. This is the first and most important step towards getting rid of child nail biting. Then take as a parent preventive action:

– Teach your kid to wash their hands after playing in dirty places.

– Keep your kid's nails trimmed.

– Have a toy or another thing at hand to distract him when he starts biting his nails.

– Build confidence and a strong relationship with your child.

Digital Signatures and Suppress-Replay Attacks

Digital signatures are seen as the most important development in public-key cryptography. Sun Developer Network states, “A digital signature is a string of bits that is computed from some data (the data being “signed”) and the private key of an entity. The signature can be used to verify that the data came from the entity and was not modified in transit” (The Java Tutorial, n.d.). Digital signatures should have the properties of author verification, verification of the date and time of the signature, authenticate the contents at the time of the signature, as well as be verifiable by a third party in order to resolve disputes. Based on these properties, there are several requirements for a digital signature. The first of these requirements is that the signature must be a bit pattern that depends on the message being signed. The next requirement is declared in order to prevent forgery and denial. It states that the signature must use some information that is unique to the sender. The third requirement is that it must be fairly easy to generate the digital signature. Being relatively easy to recognize and verify the digital signature is another requirement. The fifth requirement states that it must be computationally infeasible to forge a digital signature, either by constructing a new message for an existing digital signature or by constructing a fraudulent digital signature for a given message. The last requirement is that it must be practical to store a copy of the digital signature. Many approaches for the implementation of digital signatures have been proposed, and they fall into the direct and arbitrated digital signature approaches (Stallings, 2003).

The direct digital signature involves only communication between the source and destination parties, and the arbitrated digital signature schemes include the use of an arbitrator. The direct digital signature is created by encrypting the entire message or a hash code of the message with the sender’s private key. Further confidentiality can be provided by encrypting the message in its entirety and adding signature using either the receiver’s public key or a secret key shared between the sender and receiver. One weakness in the direct signature scheme is that a sender can later deny having sent a message. Another weakness is the threat of a private key being stole and sending a message using the signature. Both weaknesses are the primary reason for the arbitrated digital signature scheme. In arbitrated scheme, a sender’s message must first go through an arbiter that runs a series of tests to check the origin and content before it is sent to the receiver. Because the arbiter plays such a crucial role, the sender and receiver must have a significant amount of trust in this arbitrator. This trust in the arbiter ensures the sender that no one can forge his signature and assures the receiver that the sender cannot disown his signature (Stallings, 2003).

The issue of replay attacks is a main concern when dealing with mutual authentication when both parties are confirming the other’s identity and exchanging session keys. The primary issues with mutual authentication lies in the key exchange: confidentiality and timelines. Timelines are susceptible to replay attacks that disrupt operations by presenting parties with messages that appear genuine but are not. One type of replay attack is suppress-reply attack that can occur in the Denning protocol. The Denning protocol uses a timestamps to increase security. The issue here revolves around the reliance on clocks that are synchronized throughout the network. It is stated, “…that the distributed clocks can become unsynchronized as a result of sabotage on or faults in the clocks or the synchronization mechanism” (Stallings, 2003 p. 387). Li Gong states, “…the recipient remains vulnerable to accepting the message as a current one, even after the sender has detected its clock error and resynchronized the clock, unless the postdated message has meanwhile been somehow invalidated,” which is unlikely. If the clock of the sender is ahead of the receivers and the message is intercepted, the opponent can replay the message when the timestamp becomes current. This type of attack is known as suppress-replay attack.

In order to address the concern of suppress-replay attack, an improved protocol was presented. Here are the detailed steps.

1. “A initiates the authentication exchange by generating a nonce, Na, and sending that plus its identifier to B in plaintext. This nonce will be returned to A in an encrypted message that includes the session key, assuring A of its timelines.

2. B alerts the KDC that a session key is needed. Its message to the KDC includes its identifier and a nonce, Nb. This nonce will be returned to B in an encrypted message that includes the session key, assuring B of its timeliness. B’s message to the KDC also includes a block encrypted with the secret key shared by B and the KDC. This block is used to instruct the KDC to issue credentials to A; the block specifies the intended recipient of the credentials, a suggested expiration time for the credentials, and the nonce received from A.

3. The KDC passes on to A B’s nonce and a block encrypted with the secret key by A for subsequent authentications, as will be seen. The KDC also sends A a block encrypted with the secret key shared by A and the KDC. This block verifies that B has received A’s initial message (IDB) and that this is a timely message and not a replay (Na), and it provides A with a session key (KS) and the time limit on its use (Tb).

4. A transmits the ticket to B, together with the B’s nonce, the latter encrypted with the session key. The ticket provides B with the secret key that is used to decrypt EKS[Nb] to recover the nonce. The fact that B’s nonce is encrypted with the session key authenticates that the message came from A and is not a replay” (Stallings, 2003 pgs. 387-388).

This protocol is not vulnerable to suppress-replay attacks due to the fact that the nonces the recipient will choose in the future are unpredictable to the sender (Gong, n.d.).

In conclusion, digital signatures are seen as the most important development in public-key cryptography and include direct and arbitrated digital signature approaches. The direct digital signature involves only communication between the source and destination parties, and the arbitrated digital signature schemes include the use of an arbitrator. Suppress-replay attacks can occur if the clock of the sender is ahead of the receivers and the message is intercepted. This allows the opponent to replay the message when the timestamp becomes current. This issue is overcome by the implementation of a protocol that uses timestamps that do not require synchronized clocks because the receiver B checks only self-generated timestamps (Stallings, 2003).
