Everyone deserves to have the nose that they want. But shouldn’t it be the nose that their face wants too? Every face has certain proportions and dimensions that often make the nose stand out because it doesn’t quite fit – this is what usually drives one to seek a rhinoplasty. But exchanging one nose that doesn’t match nicely with the face for a “prettier” nose but which still doesn’t really match isn’t such a good idea. The best rhinoplasties result in a nose that looks nicer and fits better. A new nose which makes you still look like you – only better and in better overall proportion to your face. A nose that doesn’t scream out “I had surgery” because it is actually in better harmony with your face than the nose you had beforehand.

Generally, this will be a nose that has been sculpted to have improved aesthetics but doesn’t look like it is a nose which should belong to someone else. Do you really want to hear “Hey, nice nose job!” or would you prefer “You look nice – what did you do?”? Women will often realize during their consultation that the new characteristics they seek for their nose are really all about making their nose look more feminine. Unfortunately, it seems that many women have inherited a male-like nose (i.e. it looks more like Dad’s than Mom’s!). For a woman, the sculpting of the nose to look more feminine is another important strategy to bring it more into alignment with the rest of the face. For a man, the ideal reshaping of the nose will simultaneously maintain (or assert) its masculinity.

A “nose job” procedure sounds like it should hurt, but it really doesn’t. Really! Most patients believe that a rhinoplasty has got to be a painful procedure. They delay their consultation while dreading a recovery they actually know very little about. The truth is that a rhinoplasty is one of the least uncomfortable procedures in all of plastic surgery! Patients frequently seem surprised by this fact but quickly learn after their procedure that they truly have very little discomfort.

Stuffiness of the nose can be around for a while as the inside of the nose swells even more than what you might see on the outside but this is more “annoying” than it is painful. Speaking of which, due to advances in rhinoplasty techniques, the bruising and swelling is typically far less than what most people expect too. In under a week, most of the tell-tale signs that a procedure was performed will be gone – and it is likely that you will still have most of your pain pills left!

Do not watch any videos of a rhinoplasty or you will never have it done. Even though it doesn’t really hurt all that much afterward, it is probably wise for you to not watch any explicit videos of this procedure and wrongfully conclude otherwise. Watching things that you really weren’t meant to watch might dissuade you from following up on your dream…and for all the wrong reasons. The surgical techniques which are used, while sophisticated and artistic, do involve some maneuvers that might make you cringe a bit here and there.

For that matter, the same could probably be said for almost every surgical operation that there is, not just this one. Psychologically, patients who expect more pain and obsess about it (especially after watching things that are best left unwatched) will literally talk themselves into having more pain. Imagine – the reason it hurts more and your worst fears seem to be coming true are simply because you believed that they would. An old saying says that while your doctor cannot talk you out of your pain, you can certainly talk yourself into it!

“Packing” is essentially a thing of the past. This is another one of those still common but totally misplaced fears patients often express about a rhinoplasty. The idea of having the nose “packed” with about 8 yards of gauze per nostril that days later has to be removed with all the subtlety of starting a lawnmower – that would scare anybody. This all goes back to the old days, some 20 years ago or more, where packing of the nose after a rhinoplasty was routine.

The packing removal was literally the most painful part of the recovery. Modern plastic surgeons questioned the science behind this standard routine and for the most part it has been abandoned. For a cosmetic rhinoplasty, many plastic surgeons will use a small piece of a non-adherent type of packing that is removed shortly after your procedure, before you go home, not left in for days. More complex nasal procedures, reconstructions, or those involving the sinuses might require packing left in for longer.

Why does it seem like so many of the celebrities have their noses done and “re-done”? About 10% of patients having rhinoplasties will ultimately need or want at least some type of touch-up for their nose. This statistic has held true for many years. Why? Some patients will heal or scar (yes, there is scar tissue which can form under or in the skin in unkind ways) such that the result is not as good as it should have been and a revision, not a total overhaul, will be of benefit. Sometimes parts of the cartilage framework that support the nose will twist or bend in a wayward way as it recovers.

Unexpected and unpredicted, but in need of some minor surgical modification to be righted. A surgical solution is not always needed though – a good number of imperfections can be corrected with special injectable treatments in quick fashion in the office. It is actually only a small percentage of patients who wanted their nose to look a certain way and just didn’t get it, or worse. Unfortunately, it is somewhat of a standard Hollywood happenstance that a result is deemed good today, but then needs to go in a whole new and more aggressive direction next year. This sort of thing is often seen with breast implants and face lifts as well.

When Opposites Work Together

The concept of “Yin and Yang” originated in ancient China. The concept describes two main opposing but complementary forces or principles that are found in objects and processes in the universe. Yin is the darker element, often related to the feminine aspect, and the night. Yang, on the other hand, is associated with the brighter element the masculine aspect, and also with the day (Wikipedia). Psychologist William Indick argues that in some stories, occasionally, “two characters are as different as day and night…yet [they] complement each other” (Indick, Psychology 117). In the story Charlotte’s Web by E.B. White, Charlotte and Templeton complement each other and work together to save Wilbur’s life.

Charlotte and Templeton differ from each other physically. Charlotte is a female spider or “yin,” while Templeton is a male rat or the “yang” counterpart. The spider describes herself as a trapper, and she loves drinking the blood of the animals she traps (White 39). In addition, Wilbur thinks that his new friend -Charlotte- is “fierce, brutal, scheming, and bloodthirsty” (41). In contrast, Templeton explains that he “prefers to spend his time eating [leftovers] gnawing, spying, and hiding…I am a glutton” (29). Charlotte believes that “a rat is a rat” (47). But these physical characteristics can not be completely related to the characters’ personalities.

In the story, Charlotte is not only an ordinary spider, but a mother figure to Wilbur. E.V. White employs the “Archetype of a mother” as part of Charlotte’s personality. Dr. Indick explains that archetypes are shared associations and images, ideas, or significant unconscious figures to which all people can relate (Indick, Psychology 113-114). Charlotte is a caring and loving “mother” to Wilbur. When Wilbur discovers that by the end of the year he is going to be killed, he starts screaming in desperation and asks for help. As a mother will do, Charlotte worries and promises Wilbur to develop a plan to save him from the ax (51). Also, Wilbur acts like a child and from time to time asks Charlotte to tell him a story. So the spider tells Wilbur the story of her beautiful cousin, who managed to spin a web across a small stream. At the end of Charlotte’s story Wilbur asks her to sing him a song, and she sings a song like a loving mother to her dearest son (101-104).

Despite Charlotte’s physical appearance, she is also a teacher mentor to Wilbur. Dr. Indick argues that the mentor archetype is a very common figure in stories, myths and movies. Also, a teacher mentor will be a wise and inspirational figure who has taught and guided students. Moreover, “the teacher sees his students as emerging heroes, overcoming their own weaknesses under his tutelage…and the victories of his students become victories of his own” (Indick, Movies 53-54). Charlotte fulfils the teacher mentor archetype because she teacher her student a variety of subjects for example, “One afternoon, Wilbur says to Charlotte that she has awfully hairy legs. The Charlotte goes on explaining him that her legs are hairy for a good reason, [spins webs] and that her legs are divided in seven sections -the coax, the trochanter, the femur, the patella, the tibia, the metatarsus and the tarsus” (55). The teacher also helps her student develop his vocabulary, Wilbur is always asking for the meaning of a word like “What does salutations mean? What does sedentary mean? Or what does gullible mean? (35,61,67). On the other hand, towards the end of the story, Wilbur wins a prize on the fair and asks Charlotte why she is so quiet. Charlotte responds that she is a little tired, but she feels peaceful, because Wilbur’s success in the ring, to a small degree, was her success (163). Therefore, after all her hard work and after Wilbur overcomes his weaknesses, the teacher feels the student’s success as her won success.

Like Yin and Yang, Templeton’s personality is the exact opposite of Charlotte’s personality.

To begin with, he does behave like an uninterested parent to a child. At the beginning of the story, Wilbur is sad and bored and asks Templeton to talk to him and to play with him. But the rat disappoints Wilbur because, unlike a loving father, Templeton does not want to be with him and says “I never do those things if I can avoid them” (29). Templeton’s behavior can be explained by one of Erickson’s psychosocial stages of identity crisis; the trust versus mistrust stage. Based on this stage, Dr. Indick explained that a dubious character (like Templeton) is reluctant to commit himself to a heroic cause, because he is unable to trust the good people who need his help (Indick, Psychology 80). In order to resolve the mistrust conflict, the character has to give a “leap of faith,” which means that he has to let his guard down and give himself over to someone else. The leap of faith is not a great transformation, is just a step where the character moves to a more proactive state to help someone (Indick, Psychology 81).

Charlotte and Templeton are as different as day and night. Charlotte is caring, loving, trustworthy, and loyal, while Templeton is the opposite because he is not trusted, he ” has no morals, no conscience, no scruples, no consideration, no decency, no milk of rodent kindness, no compunctions, no higher feeling, no friendliness, no anything” (45-46). In addition, Templeton is selfish. Charlotte says to the oldest sheep that she is not sure Templeton will be willing to help them because “he is always looking out for himself, never thinking of the other fellow” (89).

Therefore, Templeton is neither a parent or teacher figure nor a friend to Wilbur or anyone.

Even though Templeton is selfish and untrustworthy, he takes his “leap of faith” when he cooperates with Charlotte and the rest to save Wilbur. Charlotte needs some new words to write them in her web, so the old sheep orders Templeton to bring a clipping from a magazine (90-91). Charlotte can not do the errand herself, because of her physical limitations. But Templeton is a rat; he has more physical possibilities to perform the job because he comes and goes as he pleases, unlike the rest of the animals. Although Templeton refuses to do the job in the first place, after a short explanation from the old sheep, he agrees to help and says, “I’m making a trip to the dump tomorrow afternoon. I’ll bring back a magazine clipping if I can find one.” Later, Templeton finds an advertisement with the words ‘With New Radiant Action’ and together, as Yin and Yang, (Templeton and Charlotte) study the soap ad (97). Even though Templeton does not like to be treated as a “messenger boy,” he looks around in the fair and brings Charlotte the last word she writes in her web: “Humble” (139-140). At the end of the story, without being asked, Templeton also bits Wilbur’s tail when he faints in front of all the people, and he is also easily persuaded to bring Wilbur Charlotte’s egg sac (168-169). So Templeton moves to a more active state, and helps Wilbur.

Regardless of the characters’ differences, they unite their abilities and capacities to work together and at the end, Wilbur is safe. Jane Garry and Hasan El-Shamy explain that:

A critical facet of the concept of opposites is the perceived inherent union between opposites constitutes a new affective entity that is greater than mere elaboration of either component… In most general terms, the union refers to the idea that opposites attract and combine to make up a whole greater than the sum of the opposing parts…(Garry and El-Shamy 482)

Charlotte and Templeton are like day and night. Their personalities differ substantially. The spider is a mother, a teacher, and a friend to Wilbur. On the other hand, the rat is a selfish, inconsiderate and insensitive character. However, these two personalities complement each other, they wok together to save Wilbur form death. Charlotte develops a plan and Templeton helps her and brings the words she needs. They unite their talents, and like Yin and Yang form one strong whole, which effectively accomplish an unselfish task: to save Wilbur’s life.

Cancer Of The Bladder

Bladder cancer is a tumor within the bladder, usually starting with the cells lining the bladder walls. These abnormal cells multiply without control. The tumor may or may not be malignant depending on the invasiveness of the type of cancer involved. The cause of bladder cancer is uncertain as with most types of cancer. Studies have shown that several risk factors may contribute to the development of bladder cancer.

About 25 percent of bladder cancer can be attributed to the exposure to cancer-causing chemicals or carcinogens in the workplace. The chemicals that belong to the Arylamines and Benzidine families are considered the most responsible. Arylamines exposure used to be very high in Dye, Rubber, Aluminum, Leather industry workers along with truck drivers and pesticide applicators but most arylamines usage has been reduced in the workplace because of government regulation.

Radiation therapy for women with cervical cancer have an increased risk of developing bladder cancer. Certain drugs are known to result in a high risk factor to developing bladder cancer such as chemotherapy agent cyclophosphamide and the analgesic phenacetin. Repeated or chronic bladder infections may also lead to the type of cancer called squamous cell bladder cancer. This type of bladder cancer is very slow growing and as with all cancers, early detection can lead to a higher cure rate.

According to the TNM (tumor, lymph node, and metastases) classification system the cancer stage is classified by the location, size and aggressiveness . Recently the TNM staging system has become very popular with the medical industry to describe all types of cancer. The TNM staging system is divided into 5 main stages along with several sub stages using the following scale.

Stage 0 – Tumors limited to the bladder lining but do not extend through the bladder wall lining.

Stage 1 – Tumors involve the layer beyond the lining of the bladder and beyond but do not involve the muscles of the bladder.

Stage 2 – Tumors involve the muscles in the bladder.

Stage 3 – Tumors involve the fatty tissue surrounding the bladder, the prostate gland, the vagina or uterus.

Stage 4 – Tumors have spread into the lymph nodes or other areas of the body such as the liver, lungs and bones.

The following symptoms of bladder cancer can also be associated with non-cancerous conditions. Nerveless any symptom of a suspicious nature should be always evaluated by a trained urologist. Early detection is critical in the successful outcome of all cancer treatments.

Do Middle Aged Men Conceive More Girl Babies?

I sometimes hear from people who want help in gauging their chances or odds of conceiving the gender of their choice. Sometimes, they think they have special circumstances which might make their odds better or worse for what they are trying to achieve. One example of these special circumstances is the age of the father-to-be.

I heard from a woman who said “I just married a man who is middle aged. He already has grown children from a previous marriage. We would like to conceive and we are hoping for a girl. His previous children are boys. This is discouraging, but I have heard that the older a man is, the more likely he is to have a girl. Is this true, why or why not?”

This is not the first time I’ve been asked this question. This seems to be one of those old wives tales that has just enough scientific merit to make it seem believable. The thinking behind this goes something like this: since a man’s sperm count declines as he ages and there is another old wives tale which theorizes that a higher sperm count favors boy babies, then the lowered sperm count due to age must make a girl baby more likely by default.

So let’s break all these theories and old wives tales down. First, let’s look at the theory that a man’s sperm count declines with age. This one is considered true. However, such declines are usually very slight. In fact, I’m sure we all know older men that father children without any problems at all. It’s not at all rare. In fact, while there comes a point in a women’s older life when she can no longer have children due to her age, the same is not true of men.

But, accepting the fact that some men have a declining sperm counts with age, there is no definitive scientific basis to say for sure that a high sperm count favors boy babies. The thinking behind this is that because boy sperm don’t live for very long but are faster moving than girl sperm, you are going to have a better chance of conceiving a boy with a man who has a higher sperm count because the faster moving boy sperm will make it to the egg more quickly. But here’s what this theory doesn’t take into account. If a higher sperm count exists, there will be just as many girl sperm fighting for the egg. Sure, they are slower than the boys, but the ratio of X to Y or boy to girl is the same for men with high or low sperm. And the end result is usually a 50 / 50 chance of either gender. And, even if the high sperm count theory was true (which I’ve just argued that it isn’t,) this is not guarantee that the opposite would also be true.

Because according to this thinking, a lower sperm count means less X and Y sperm available. But as with the above example, the ratio would remain the same the same. And so the result (50 / 50 odds) should also remain the same. So my opinion is that no, an older man would still have roughly the same 50 / 50 chance of conceiving a girl or a boy as a younger man. This is true whether he has a high or low sperm count. Because nothing has changed with the ratio of those sperm. As long as he has roughly the same ratio of girl to boy producing sperm (which studies have shown is the norm in men of all ages) then (unless you make other changes) you still have roughly equal odds.

So what do I mean when I say “other changes?” Believe it or not, I mean changes to the woman or mother. The woman who wrote me would likely have more success trying to change her own PH than worrying about her husband, who could not change his ratios. Whether you get a girl or boy depends on whether an Y (boy producing) or X (girl producing) sperm chromosome fertilizes the egg. You can make an X or girl more likely by the woman having an acidic PH, the couple conceiving earlier in her cycle, and using shallow intercourse positions. These things would have more impact on conceiving a girl than the father’s age. If they wanted a boy, then the opposite would be true, (late intercourse, an alkaline PH, etc.) but the man already had boys and wanted a girl this time.

Where did we go so wrong? I grew up in Kenya, went to school there, college and even worked there for about four years. I am not sure where people got the idea that happiness is something that happens to you. The truth is that nobody owes you happiness.
