For clarity purposes the term “ES” or “Estim” is a generic declaration of an external device in most cases, that produces a flow of electrons from one electrode to the other of the same channel. The flow of the electrons produces an ionic reaction in the body in the areas between the electrodes, in most cases motor nerves and muscle tissue.

The most common use of a specific form of ES with multiple sclerosis patients is the term “Functional Electrical Stimulation”, or FES. The tens pain machine can be worn continuously, even sleeping with it on to help the patient sleep. A Tens machine is not addictive, nor like drugs, does it affect ones mental abilities to think and reason. Often if the Tens machine is turned off the pain comes back. There is no residual pain relief generally speaking when one uses a Tens unit. Fro many patients the tens pain machine may be worn continuously.

What is Interferential?

Interferential machines go off and on from 8,001 to 8,150 times per second. This is also referred to as frequency, rate, PPS and there is a relationship between frequency and depth of penetration. The interferential treatments go deeper into the body and affect more nerves. When the electricity is able to penetrate deeper then the better the treatment outcome is for the pain patient.

Is there a difference between treatment times between tens and interferential?

An interferential treatment generally lasts in the clinic for 20 minutes. During the interferential treatment the pain the patient is feeling goes away and, after the treatment, the pain is gone for some time period. The time from the treatment until the pain returns is called “carryover pain relief.”

What is the major difference between the two therapies?

The biggest difference between a Tens treatment and an interferential treatment is the carryover pain relief period. With TENS the pain often comes back shortly after the pain machine is turned off. With interferential therapy the pain is gone and stays away for longer periods of time. As Interferential therapy treatments are increased then the carryover pain relief is extended..

Eventually will the interferential unit not be needed?

What is most likely is when a patient starts using an interferential unit they may need to do 3 – 5 treatments the first 2 days. After the first week or few days then the patient finds they only need to do a treatment every 2-3 days. It is not unusual for a patient who has been in chronic pain for years to do an interferential treatment upon waking in the morning and/or upon going to bed at night.

Then is it best to only use the unit twice or what?

With tens or interferential one can use the unit anytime there is pain present. If using tens or interferential and its difficult to sleep, due to pain, then wear the unit all night long if it helps the pain go away and one can sleep. If using interferential therapy then use the AC adaptor and plug into the wall since the unit will be on for extended time period.

What happens if one falls asleep with an interferential unit on?

Nothing other than the patient is able to sleep. First, that will not be a problem. One will not be harmed in any way. On most interferential units, and on some TENS units, there is also a timer. Most timers can be set for 15, 30, 45 minutes of stimulation, or set on Continuous mode. If 15,30 or 45 minutes of treatment occurs and the unit goes off, but the pain comes back, then switch to Continuous and wear until the pain is gone. The explanation for this is the purpose of tens or interferential is to get rid of the pain signal. Neither a tens or interferential unit is meant for the patient to “feel the unit all the time”. The purpose is for the patient to not be in pain and to not feel pain..

So if the pain machine, whether tens or interferential is not being felt, then does that mean it is not working?

No again the purpose is to not feel pain. Many times after the interferential machine goes off the patient will not feel any stimulation but that is fine and expected. What the treatment goal is for the patient is to not feel pain, not to feel the stimulation.

Second, the main goal with interferential is to extend the time periods when the patient is pain free, which is referred to as carryover or residual pain relief.

Why is that?

Unlike pain medications the goal with interferential is to not have to use the unit, to extend the carryover pain period and not be in pain. With medications it can be the opposite and the patient is increasing the frequency of taking pills, or asking for more or stronger drugs..

So its successful to not need to use the interferential unit?

Yes. The best outcome is when the use of the interferential unit has resulted in fewer treatments because there is less pain and it is not needed..

Third and last, but not necessarily least, is due to some chemical changes that occur in the body, as well as a patient being able to become more active, the real possibility that permanent change is occurring in the body and the unnatural pain impulse is being modified or simply goes away for good may be occurring.

The explanation for that is we know for sure that positive and negative charges affect our cell and tissue structures which includes nerves. Now that a patient can self treat as needed the ability to do such is opening up new possibilities of permanent structural change and the pain impulse is thwarted.

O.K. so how might we tell if this is being successful?

The big difference now is the treatments can be done by the patient at home, work or where needed, and not have to go to a clinic or hospital for treatments.

How does one try tens and interferential to see which works best?

Either a tens unit can be tried or an interferential unit and one can see which helps most. Often the combination of using tens sometimes and interferential at other times is best for the patient. Also seek a pain machine that combines tens and interferential together in one unit so both tens and interferential can be tried using the same machine. Switch the treatment between tens and interferential as needed. Tens and interferential units use the same electrode pads, cables etc. so two sets of supplies are not needed, although interferential treatments because of more electrical energy, work best when larger electrodes are used than what is customarily used with only a tens unit.

The Attack Of The Bear

She talked about it; she dreamed about it, she never let it go. This was how a friend of mine regarded her fear of coming across a bear when we would go hiking in the wilderness areas of Idaho. Her character is strong in all other ways. She is kind, hard working, intelligent, prosperous and loving. But when the word “bear” crosses those ears of hers, she turns to Jell-O. In other words, her life is somewhat Un-Bear-Able!

We were hiking last spring in the Coeur d’Alene mountains going for a 10 mile excursion to visit one of the beautiful alpine lakes that are plentiful through out this part of Idaho. We packed a lunch, threw on our backpacks and set off for a glorious day of exercise and becoming one with nature. My friend, however, was nervous and carried a canister of bear spray (kind of like pepper spray only for bears) with her just in case. I assured her that bears were harmless in this area. Only black bears roamed the woods here…no grizzlies I told her. Yet she was set out to believe that a bear was a bear and in her mind, they were all very very bad.

After about an hour of vigorous hiking we sat down for a minute to take a drink of our water and take in the natural beauty that surrounded us. SNAP! went a tree branch just up the trail and there it was…everything she ever imagined in her worst nightmares was in front of her. It was, you guessed it, a big black 10 foot growling snarling bear! Actually, that is how she remembers it. It was actually a small female black bear just crossing the path about 50 yards in front of us. My good friend screamed, grabbed a hold of me and started to shake. I joined in with the screaming part because the first thing you do when you come across a bear is make a lot of noise to scare it off. So I figured, what the heck, I might as well scream too to make her feel better.

Well, the bear scurried off, my friend almost fainted and I laughed till I cried. I gave her a few minutes for her heart to slow down from it’s 160 beats a minute and then I calmly and gently said to her, “You see, bears are not going to hurt you”. She calmly said to me, “We got lucky this time buster”. Well, needless to say, this experience did not help her fear of bears but only increased the intensity of her phobia.

We still go on hikes together and now my friend is more at peace in the wilderness like never before. No, she did not get over her extreme terror of bears. No, her dreams have not stopped. No, she has not made peace with this precious animal. When we returned to our city dwellings after that last hike, my dear loveable sweet caring friend immediately called up a shooting gallery, enrolled in a class to learn how to shoot a gun, and now carries a pistol on her side whenever we roam the hills and valleys of bear country (just in case, she says). My friend feels less nervous and seems to enjoy our hikes like never before. Me, I feel sorry for the poor bear.

For decades forms of electricity have been applied to the body to make the body heal itself. Originally the use of electrotherapy was to correct a situation where the body was not healing itself. My hopefully educated opinion is the waveform, and the current are secondary in accelerating the healing processes of our body. The commonality of practically all these devices is the changing polarities. Most of these device alternate between positive and negative charges. In understanding what excites our cells to act and, do so quicker, it’s almost always the changing of tissue and space between cells of polarity. Like a magnet when you put north end to north end you repel, south to north and you attract. Bones have polarities on each end of a bone, and on the bone particles. This is the same state for cells within our body when damaged or diseased. When one end of a bone, or cells within, becomes more positive, and the end to reattach to is more negative, then they grow together or unionize. The waveform is not doing it but the physical attraction of positive and negative charges, are the healing agents.

The Story of Ted – Using Interferential Therapy for Peripheral Neuropathy

Ted is a patient who has suffered from peripheral neuropathy in his right calf and foot since 1987. Ted today is 77 years old and in good health but for the lack of sensation, and some pain, from the peripheral neuropathy. Let me tell you a little about Ted and his methods.

Ted was a pharmaceutical sales rep. over 35 years ago and always had a keen interest in the way our body works chemically. His pharma training helped him understand the intricacies of chemical actions in healing and disease. But Ted is a true “doubting Thomas”. Ted was originally diagnosed with peripheral neuropathy in 1987 by a neurologist after an extensive nerve conduction study. In 1986 Ted had visited Duke Medical Center as he had noticed a small tremor that was not visible to others, but he knew of it. Ted was a certified marksman in the U.S. Army and National Guard so he continued his marksmanship contests. It was the need to stay totally still and slowly squeeze the trigger that alerted Ted something was not right.

He competed at the national level so the progression of the peripheral neuropathy was something that limited his competitive abilities and he lost that competitive edge to win at that level of competition.

Ted came to us because he was losing sensation in his right calf and foot. This was causing an issue because Ted is a sole surviving spouse. He lost his wife to cancer about 20 years ago and is self dependent.
