Lower back pain is a common affliction, with millions each year visiting doctors for relief. Not only will they seek relief, they will want a diagnosis.

It is not always easy to diagnose lower back pain. Many body structures can cause it. There are muscles, ligaments, and tendons; spinal column bones; joints, discs and nerves. In addition to these structures, there may be under medical conditions your physician needs to evaluate.

Whether you initially diagnose lower back pain yourself, or leave that to your physician, the diagnosis will need to consider both the location and symptoms of your pain.

Step 1 – Location

The first step is to decide the location. "Where does it hurt?"

1. Axial lower back pain: This lower back pain hurts only in the low back. Pain does not travel into any other area.

2. Radicular lower back pain: This lower back pain hurts in the low back, and also radiates down the backs of the thighs into one or both legs.

3. Lower back pain with reported pain: Diagnose lower back pain with reported pain if it hurts in the low back area, and tends to radiate into the groin, buttocks, and upper thighs. The pain will rarely radiate below the knee, but may seem to move around.

Step 2 – Symptoms

Once you diagnose lower back pain as to location, you will consider symptoms. "How does it feel?"

1. Worsens with certain activities: If you play football, for example, the pain is worse.

2. Worsens in certain positions: Perhaps it gets worse if you stand for too long. Or it is more painful after you sit in a car.

3. Feels better after rest: Resting from the activity or position usually reduces the lower back pain.

4. Deep and steady: Not a sharp muscle catch, this pain is constant and deep within the affected areas.

5. Severe: The pain is excruciating, possibly more so in the calf than the lower back.

6. Numbness and tingling: There may be "pins and needles" within the area.

7. Fleeting pain: Pain may seem to come and go, leaving you unsure at times just how it feels.

8. Achy and dull: Like the flu, this pain is sore and dull, though sometimes intensifying.

9. Migratory: It hurts in one spot, then another.


AXIAL: If location is best described by number 1 above, and symptoms are a combination of 1, 2, and 3, you can probably diagnose lower back pain as being axial – the most common type. This is also called "mechanical" lower back pain. A variety of back structures can cause axial lower back pain, and it is difficult to identify which is the cause. Axial pain gets better on its own, and about 90% of patients recover within six weeks.

RADICULAR: If location is best described by number 2 above, and symptoms are a combination of 4, 5, and 6, you can probably lower lower back pain as being radicular – commonly called sciatica. This lower back pain is caused by compression of a lower spinal nerve, usually the sciatica nerve that runs from the spinal column, down the back of the thighs to the feet. Doctors usually recommend conservative treatment such as physical therapy exercises, medications, and possibly spinal injections, for six to eight weeks.

REFERRED: If location is best described by number 3 above, and symptoms are a combination of 7, 8, and 9, you can probably diagnose your pain as being lower back pain with referred pain – the least common type. This lower back pain is treated the same as axial back pain and frequently goes away as the problem resolves on its own.

How do you diagnose lower back pain?

Diagnose lower back pain with care. You need an accurate diagnosis, which your physician can best make, to be sure no underlying causes need attention. It is not enough to know you have sciatica. You need to know the undering cause of the sciatica to determine treatment options.

If it is mild; then we call it schizoid behavior and if it is extreme then schizophrenia.

As a result of inadequate control, coordination, regulation and synchronization and harmonization of the metabolic, hormonal, neuro-hormonal and neural activities by objective consciousness, we often deviate from reality and / or vacillate between reality and illusions or hallucinations arising from various subjective factors !

We come to terms with life as we have no idea about our problem; which could be schizoid behavior; and the solutions for it.

Often the pressure of peers, teachers, parents and other relatives; inhibits us from negotiating the two personalities or even more personalities living, conflicting and contradictory within us. This pressure operates at international, local and even intrapersonal (difficulty to accept, concede and negotiate with so called deficiencies, mistakes, crimes, or sins; of ours); levels.

Often we begin to assume ourselves to be; what we bought to be; rather than what we are; and continue to manifest contradictions in our policies, plans, implementation and activities!

This can create sever conflict and cause severe STRESS, which can ruin us and also the others; to a greater or lesser extent in accordance with our position in the society! For example; the global decision makers' decisions would devastously affect the global population.

In recent times the excessive electronic and print input; through mobile phones, TV, compact disc (CD) players, in various formats; including the talks, jokes, commercials, news, movies, serials, reality shows (compounded with pollution by flickering lights and noise); constituents a major stressor. Can it due to its excessively sensational, melodramatic, confusing, contradictory, and disruptive contents; cause borderline schizophrenia or borderline multiple personality disorder?

In view of the massive unproductive employment, massive profit margins and massive monetary gains involved in this field, a brave and honest finding after thorough research of these phenomena is a big thing to expect!

But our study of Total Stress Management suggests that borderline schizophrenia and multiple personality disorder; could be caused by excessive electronic and print input; through mobile phones, TV, compact disc (CD) players, in various formats; including the talks, jokes, commercials, news, movies, serials, reality shows (compounded with pollution by flickering lights and noise).

The conscientious scientists, psychiatrists, psychologists, philosophers, thinkers, lawyers, activists and NGOs all over the world; can investigate this dangerous possibility and the imminent threat to our healthy value system, and endorse our perspective.

Community health is a very important topic that gets overlooked by both citizens and politicians. We put our trust into government officials to find cures and solutions to our health issues, but fail to look at the source of these issues. If we want to make a change in community health we must look at the core of the problem and focus more on primary prevention rather than tertiary prevention techniques. With the majority of our community health issues being preventable diseases, in order to prevent them we must eradicate the cause.

Childhood obesity is constantly on the rise and we have failed to do anything about it. Of course there have been numerous attempts to reduce the obesity rate in children but these attempts are more focused on secondary and tertiary prevention. We are focusing too much on reversing the effects of obesity and not enough time on preventing obesity in the first place. If we inform our community of the health risks that they are entering when they are obese we can teach them how they can prevent it. We must target the parents and not the children. Parents are the source of food for children and at a young age they must be properly feed and active so that these preventable diseases do not occur later on in life. Physical education in school must be more active and less optional. We must actually teach physical education rather than allow students to roam freely among themselves and choose whether or not they will be active.

A child is unable to make life decisions surrounding their health and as adults we should be assuring the children in our communities are in their best health. Poor communities are just as capable of living healthy lifestyles are middle class but without the understanding of how to do so, they will reason their lack of health to their financial standings. Community leaders must provide options for parents to seek out in order to keep their children healthy. Community health groups, activities, or even a park would be helpful. If we can get our kids out of the house and into the yard we are making progress. By lowering childhood obesity, we are able to prevent the likelihood of strokes, high blood pressure, diabetes, cancers, and heart diseases. All of these major issues are linked to obesity and there has to be something done to lower our rates. Obesity has risen from 7% in 1980 to 18% in 2012. We fear telling others that they are obese because it sounds hurtful and parents despise being told that their child is obese. The truth of the matter is obesity is here and is rising. There is no way to sugarcoat the facts. We can continue to act as if it is not an important issue and the rates will continue to increase as the years go on. Children should not have high blood pressure, or any other disease associated with overweight. This issue will not change until we start to make it a priority. I believe this to be the most important issue in our community health because people fear the outcome of bringing childhood obesity up around the parents. It is also critical to point out that you don’t have to be rich or extremely wealthy to eat healthier foods and be active. Parents should be more involved in their children’s lives and encourage them to be more active on a day to day basis. Restrict TV time and increase time spent outside. Maker better food choices when feeding your kids. Teach parents the importance of nutrition and fitness and why it should matter to them.

These are ways in which we can help make a turnaround in our communities for the better. We must focus on the now and not the later. When flu season comes around we get flu shots weeks before the virus is actually being accounted for. We vaccinate because it prevents us from getting the flu. It does no good to get a flu shot once you are infected with the flu virus. Just as this is true so is it for obesity. We must take the necessary steps before hand to prevent the disease from affecting our children.

Provide them a one page overview of Inattentive ADHD, and ask that they implement 3 to 5 specific accommodations that you have researched that will be helpful to your child. These might include, but are not limited to … making sure your child is imprisoned in the front of the class … shortening your child's homework assignments … having your child working in small groups in class … or allowing your child more time to complete tests. As long as you have a medical diagnosis of ADHD the teacher is allowed, in fact encouraged, to help you with this. None of these requests should be a burdens on the teacher, draw undue attention to your child, or take too much time away from the rest of the class. Small but meaningful changes can make all the difference regarding your child's success at school. But none of it will work without the teacher's willing involvement and active cooperation.

This would seem to be a perfect fit for ADHD kids, both Hyperactive and Inattentive. But the big price tag puts this option out of reach for many people. And even though the program has its' fervent supporters, there are plenty who feel the gains did not justify the investment. My nephew, who has Inattentive ADHD, did the program. Create a Structured Environment at Home where Routines are Established and Repeated Every Day.

This is a no-brainer that pretty much everyone agreements provides significant benefit for ADHD kids, both Hyperactive and Inattentive. Left to his own devices, my son would watch Cartoon Network and play his PSP from the minute he gets home until midnight. Imposing a Routine, (come home, take a break, have a snack, finish homework, eat dinner, put on PJs, watch a little TV before going to bed at 9:30) is calming and reassuring, instills good habits, encourages independence , responsibility, and dramatically cuts down on nagging and procrastination. It also helps carve out some time that can be preserved for family life, which is important to maintain balance and a happy household. Honestly, there is no substitute for a parent putting in the time and attention, every day, to work with your child, drill them on the basics, discipline and correct them, encourage and motivate them, and generally help to focus and keep them on track. Each person's experience and results can vary broadly, so I think it's worth a try to see if it might work for your child.

Regarding Behavior Modification, from what I have read, treatments run the gamut from therapy with a psychologist … to off the shelf or online behavior modification programs … to basic common sense tactics employing consistency, boundaries, rewards and consequences. Speaking of looking online, there is a wealth of computerized learning aids available that you can try out for free. For example, my son needed help with his multiplication tables, and I found a game created to do just that. He liked it at first, then got bored with the repetition and refused to play it. But other kids seemed to like it and stick with it. Inattentive kids seem to respond to the hyper stimulation provided by formats like computer games, so if you can find one that targets a skill that needs improvement, try it out for free first. If it keeps your kid's interest, by all means, invest in it.

A Word on Medication.

For some families medication has produced good, in fact at times miraculous, improvement in their children's academic and social life. Others have had more mixed or even negative experiences. The most productive thing you can do is thoroughly examine every single option available, with an open mind. Talk to your child's doctor. Research online. Reach out to other parents and survey their experiences. Consider your child's unique personality and situation. Only then can you make an informed decision and be confident you have acted in the best interest of your child. Sometimes it will require a combination of options and strategies to overcome your child's Inattentive ADHD. These are tough challenges. If you have pain in your thumb, and it gets worse close to the tip, you might try massaging your thumb. You might even try massaging the pad of your thumb.

If none of those actions help, that means the cause of your pain is not there – it's somewhere else.

A "trigger point" is a hyper-irritable place in a muscle (or other soft tissue) which "triggers" or "fires" painful symptoms elsewhere. A symptom could feel like burning, aching, numbness or tingling. So trigger points cause pain away from themselves.

In the case of pain at the tip of your thumb, the trigger point that causes your pain is in your lower arm. The pain is called "referred" pain because it is referred to your thumb by a trigger point.

There are muscles that allow you to move your fingers and thumb. These muscles are NOT in your hand! The tendons (tough strong tissue at each end of your muscles) attach to bones. Your finger muscles all have a muscle "belly" in the middle and tendons at each end which attach the muscle to bones.

Finger and thumb tendons are very long. The bellies of the muscles that move your fingers are actually in your arm !

The muscle that causes pain in the tip of your thumb is deaf than (it is buried under) muscles that are close to the surface (where your skin is.)

What Should You Do?

Turn your hand so it is palm-side up. Warm the palm-side of your lower arm. You can do this with a hot pack or by massaging (rubbing.)

Massage lightly at first. As your soft tissues and muscles start to soften and relax, you can massage deeper and deeper. As you press more deeply, start seeking tender places in your lower arm.

The trigger point will feel tender when you find it. It will be approximately 4 to 6 fingers away from your hand and wrist. When you locate it and press on it, you may feel notified sensations in your thumb. That is how you will know you have found it!

Press into that tender area for 12 seconds, at a pressure of about 7 out of 10 (10 being the most intense discomfort.) Trigger points in muscles respond and relax Best with a pressure of about 7.

It will not be a tiny little dot. You may have to follow the tenderness and apply pressure in several spots. Take all the breaks you need. You can keep going back to tender areas and re-treating them. If you feel a little bruise-y afterward, that is just because your muscles were tight and they were not used to someone pressing on them.

It could take a day to notice the difference, but if you have less pain in the tip of your thumb, you have done it! You did your own therapeutic massage.

Back Pain – How It Starts

The “interruption” has its own variables, including the L4 vertebra and L5 vertebra, and the C5 through C7 vertebra. When the cervical vertebrae are considered, the symptoms are very different. Doctors look for rigidity in the neck, numbness in the arms, weakness, and possible tingling in the fingers and hands. If the pain radiates down the arms and into the hands, there is an evaluative focus on a possible slipped disk in the neck. Because the cervical vertebrae are so close to the origin of the spinal cord, there are other symptoms that may accompany the one’s previously mentioned. Weakness can develop in unlikely locations, such as higher portions of the neck and even at the base of the skull.


When physicians are considering a back-pain problem, they will perform a physical examination, and possibly a series of physical tests. The results of these tests may be compared to results from previous tests to determine if there are any structural or inflammatory changes. X-rays can be used to determine if there is any narrowing or expansion in the disk space. A Myelogram may be used to determine whether there is any compression of the spinal cord. This is typically performed in the case where a patient reports numbness of the extremities.

How Slipped Disks are Managed

At the first sign of back injury, doctors typically provide pain relief management. This allows the patient some comfort while the doctor isolates the reason for the pain. The pain management regimen will depend on the patient’s history and metabolic variations.
