The treatment of pneumonia can be done on outpatient bases in mild cases. In the severe case, in the very young and the very old, hospital admission is required. The use of appropriate antibiotics, analgesic and other supportive therapy like intravenous infusion and oxygen may be necessary.

6. Complication.

i. Pneumonia may result in pleura effusion (accumulation of fluid in the chest cavity).

ii. Lung accesses can also result especially when pneumonia complicates aspiration.

7. Prognosis.

The outcome of treatment is usually very good. Patient on outpatient treatment should show appreciable improvement over 24-72 hours. Patients on admission always show signs of rapid improvement if there is no morbidity.

Death may occur from pneumonia itself, progress to septicemia or from co-morbidity. Aspiration is a major risk factor for death, especially in the older age group.

b. When Jacob lost his son Joseph, he was depressed and refused anyone to comfort him. He said in Genesis 37:35, “…. In mourning will I go down to the grave to my son.”

c. The burden of leading the people of Israel was too much on Moses that he told God to put him to death straight away. (Numbers 11:15)

3 Important Things You Should Do As a Christian When Depression Attacks You

Don’t handle depression lightly. It is dangerous, and can attack any of us.

Here are some of the things you should do when depression strikes you:

1. Remember God and Think About Who He is.

The first thing you should do when you are depressed is to remember God.

Go to His Word immediately, study it, and think deeply about His divine nature.

Turn over in your mind what the Bible says about Him.

Remember that He is an all-knowing God and perfectly understands what is happening to you.

Remember also that He is closely watching you and watching over you.

Psalm 147:5 says that our God is great and mighty in power and His understanding has no limit.

God can use the depression you are facing to work out something good for you.

That’s why the Bible says in Romans 8:28 that in all things, God works for the good of those who love Him and who have been called according to His purpose.

Remember what Jeremiah said in Jeremiah 32:17 concerning God.

He said, “Ah, Sovereign LORD, you have made the heavens and the earth by your great power and outstretched arm. Nothing is too hard for you”

God has the power to pull you out of where you have sunk into in your depressed condition.

David said in Psalm 40:1, 2:

“I waited patiently for the Lord; he turned to me and heard my cry.

2. He lifted me out of the slimy pit, out of the mud and mire; he set my feet on a rock and gave me a firm place to stand.”

2. Think About What God Has Done

Decide to think deeply about the wonderful things you have read in the Bible that God did for His people.

Be persistent in thinking about them, and your faith in God will start building up. This is a positive sign that shows you are on your way out of depression.

Your distressed and gloomy condition will start turning to a happy and cheerful one.

3. Trust In God

Put all our trust in God.

Listen to this advice in Isaiah 50:10:

“Who among you fears the LORD and obeys his servant? Let him who walks in the dark, who has no light, trust in the name of the LORD and rely on his God”

God loves you with an everlasting love and has drawn you to Himself with loving-kindness. (Isaiah 31:3).

He will never abandon you. Continue to trust Him.

He has spoken these words to help you trust and cling to Him firmly. He says,

“When you pass through the waters, I will be with you; and when you pass through the rivers, they will not sweep over you. When you walk through the fire, you will not be burned; the flames will not set you ablaze.” (Isaiah 43:2)

He also encourages you with this…

15. “Can a mother forget the baby at her breast and have no compassion on the child she has borne? Though she may forget, I will not forget you.

16. See, I have engraved you on the palms of my hands; your walls are ever before me.” (Isaiah 49:15, 16)

And He said in Zechariah 2:8, 9 that whoever touches you touches the apple of His eye, and He would punish the person who does so.

Stand firm child of God, and trust in God and His mighty power. You will see how He will deliver you from your depressed condition.

You may not understand the depth of His ways as He leads you. However, rest with the assurance that at the end of the day, you will sing victory songs.

Finally, child of God, remember that depression can attack any of us without considering our spiritual, physical or material attainment.

When such a situation confronts you, the first and most important thing you should do is to seek God and be encouraged by what He tells you in His Word.

Remember also that God is with you at all times, and He is always ready to deliver you from any oppressive condition.

As human animals, we tend to make things harder than they have to be. Our big brains often get us in hot water when we take shortcuts and ignore what is most important to us. Take for example the recent rise in disabling mental illness in the US At last count, over 40 million American adults, or about 18 percent of the population, had an anxiety disorder.

With a lifetime dating of near one-third, some experts believe US society itself is to blame for the alarming increase in these illnesses. One thing is for certain, however, the pills are not working! According to recent prescription data, more than 20 percent of US adults (1 in 5) are on behavioral meds. This brings us back to our opening assertion, ie, we make things harder than they have to be!

Just Breathe!

Overworked, underpaid, stressed-out, and feeling the strain? There are plenty of ways to find calm in a frantic world that do not involve prescription pills. One of the most effective options is to do what you are doing right now. No, we do not mean reading. We are talking, of course, about breathing. The oldest relaxation technique known to man consist of simply taking a breath and expelling the air in a controlled manner.

The Benefits

When we are nervous, upset, or angry about anything, someone always instructions us to take a deep breath. Why is this? Because even though we take them every 3 to 5 seconds, shallow gasps from the upper chest do not deliver much air. 1. Abdominal Breathing

How to do it: Sit comfortably with your back straight. Place one hand on your belly and the other hand on your chest. Take a deep breath in through your nose. As your diaphragm descends, the hand on your stomach should rise, while the hand on your chest should move very little. Exhale slowly through your mouth, pushing out as much air as possible as you squeeze your abdominal muscles. Each inhalation and exhalation should be slow and steady, lasting an average of 10 seconds from start to finish. (You may want to time yourself at the outside to ensure that you take between 6 and 10 deep breaths per minute.) The goal should be to complete at least ten full minutes of deep abdominal breathing each day. If you have a hard time breathing in a confined position at the start, you can lie on the floor and practice the technique before sitting up.

When to do it: Abdominal breathing is arguably the most important deep breathing technique because it teaches us breathe in a different way. Those who have never attempted it are often surprised by just how hard it is to breath in and out from the abdomen with deep, slow breaths, compared to taking quick, shallow ones from the upper chest. Because it delivers a lot of oxygen in a short period of time, abdominal breathing gives us energy and helps improve concentration. As a result, it is an extremely popular relaxation technique for students to employ before an exam. Not only does it help them relax after an all-night cram session, it also helps revive their tired minds.

2. Progressive Muscle Relaxation

How to do it: One of the reasons Americans love massages is that we tend to hold a lot of our tension in our muscles. Back problems, muscle spasms, general aches and pains, and even serious injuries are all common symptoms of this unhealthy habit. We can address the issue with a technique that combines deep breathing with muscle tension and relaxation. Here's how to do it.

Dressed in loose, comfortable clothing, take a seat in a quiet place. Remove your shoes and begin to breath in and out with slow, deep breaths. Once relaxed, close your eyes and focus on tensing then relaxing a specific muscle for a few seconds at a time. Most beginners start with one of their hands or feet, then move on to other sore or overworked areas. As you progress, you will want to tense certain muscles a bit harder for at least ten seconds at a time. Although not the focus of the technique, your breathing should remain slow and deep through. For many newcomers, it is helpful to count breaths while tensing and relaxing each muscle. Experts recommend inhaling three to five times through the nose while the muscles are tensed, then exhaling three to five times through the mouth upon release.

When to do it: While it is true that more experienced practitioners can complete an effective progressive relaxation session just about anywhere – at a desk, at home, even on the road -most tyros need a quiet place where they can relax and focus on each muscle. It is also important to note that tensing muscles too hard and / or too long can result in dizziness, which in not the goal of the technique. How to do it: Like most yoga techniques, equal breathing is designed to calm the body and focus the mind. Practiced by both beginners and yogis alike, it begins in a comfortable, cross-legged positioned position such as sukhasana. You must then close your eyes and focus on your breathing. Do not change anything at first, but simply observe your natural breathing rhythm. Because most of us take quick, shallow breaths that are not conducive to relaxation during our waking hours, odds are you must slow your breathing down a bit. A good rule of thumb is to count to four as you inhale and four as you exhale. You may change this interval depending on your personal needs, just be sure to breath in and out for equal lengths of time.

When to do it: You do not have to be in a yoga studio to practice pranayama, which is the art and science of controlled breathing in yoga. In fact, there are over 50 pranayama breathing techniques and forms, including sama vibrti. 4. Guided Visualization

How to do it: A classic song instructs us to "accentuate the positive and eliminate the negative …" What's the best way to do just that? Fraud in Nigerian Crude Oil Selling: Authenticating The Crude Allocation and Documents by Oil Buyers


This observation by the research report on the subject, done by the Africans in America News Watch, a New York based non-profit organization, in August 2010, sums up the issue:

“There are many genuine crude oil sellers in Nigeria but the problem is getting the real and verifiable ones. Crude Oil trade is a booming and thriving business and many people seem to want to go into it. Buyers from other countries contact sellers in Nigeria in order to buy Nigerian Crude Oil. [But the business is now full of]… scammers on the prowl.” It adds that “There are lots of crude oil sellers in Nigeria, but the challenge there is the ability to find genuine and verifiable sellers.”

In point of fact, as this author has amply documented elsewhere in another study, the assertion that in the arcane world of international crude oil buying and selling today the landscape is literally littered and crawling with fraudsters and scammers, is now a well-established, well-settled truth about which there can hardly be any serious argument or disputation in the contemporary international oil buying and selling industry.

In consequence, given that stark REALITY that “there are many genuine crude oil sellers in Nigeria but the problem is getting the real and verifiable ones,” the big million dollar question is this: AS A CRUDE BUYER, HOW THEN DO YOU GET THOSE SELLERS FROM AMONG THE WHOLE LOT WHO ARE THE “REAL AND VERIFIABLE” ONES?

THE KEY? Most experts, in the case of Nigeria, say that basically you (the crude buyer) would have to demand and insist on the purported Seller showing you AUTHENTIC documentation and other proofs of having genuine BLCO and FLCO allocation from the Nigerian NNPC, as well as proof that that product is still currently availability. (For Nigeria, the NNPC, which stands for the Nigeria National Petroleum Corporation, is a Federal government-owned company that administers the buying and selling of petroleum, including giving allocation to genuine sellers of the crude oil in Nigeria).


There are some key documents that are crucial in the purchasing of the Nigerian crude oil transactions. They will include the following documents, among others:

– Seller’s shipping documents, such as: Clean Ocean Bill of Lading; Seller’s Commercial Invoice

– Seller’s Proof of Product (will comprise the License to Export & the Approval to Export, issued by the country’s government, statement of Availability of the Product, Port Storage Agreement, etc)

– SGS/Sayboat Certificate of Quantity and Quality issued at the loading port

– Certificate of Origin issued by the NNPC

– Certificate of Authenticity issued by the NNPC

– Charter Party Agreement on the vessel, issued to the charterer of the vessel and presumably showing that the vessel is actually chartered in the designated Seller’s name

– the Q88 questionnaire, filled out by the managers of the vessel providing the relevant information and specs of the vessel;

– Etc.


Probably the most important document of all that a crude buyer may need to see from the seller, is the proper Proof of Product (POP). This document, which has to be one issued by the appropriate department of the NNPC in Abuja, Nigeria, serves as a clear indication to a crude buyer that the owner of the oil commodity has true possession of the product, and also serves as an indication that, at least as at the time of the transaction (but only at that time), the seller has the commodity available for sale.

IMPORTANT: It should not just be any form of POP, however. It needs to be one that is in a format that will enable satisfactory verification to ascertain that it is valid and authentic. Based on this writer’s research, to ensure the optimum likelihood that this goal can be attained, there are basically two types of POP documents that are acceptable, and none others, and only sellers whose POP documents meet those “preferred” POP documents standards, ought to be entertained or attended to.


A). It must have the Loaded vessel documents that are CURRENT (that is, must not be more than 2 days old, otherwise the document will immediately be rejected as it may mean that the vessel is no longer available), and should include the following documents:

i. A Current Authority to Board (ATB). Seller must provide, for the buyer’s inspection, the ATB that was specifically issued to the initial buyer (consignee) of the crude in whose name the vessel was issued. The name on the ATB must match exactly with the name on the POP and other documents named here. And, here again, the ATB MUST be CURRENT – that is, it must be no more than 2 days old. (An ATB that is more than 2 days old, should be automatically be viewed as representing a vessel which is no longer available and hence not acceptable).

ii. Certificate of quality.

iii. Certificate of origin

iv. Cargo manifest

v. Vessel ullage report

vi. Certificate of quantity.

vii. Bill of lading

viii. ATS (Authority to Sell) from the NNPC


Generally, the Buyer may require (and hence the Seller must be willing and ready to release them), vessel particulars such as the following: the name of the vessel, location of the vessel, the IMO name, call sign and other vessel details. The reason this is required is so the buyer can do the tracking of the said loaded vessel, and to ascertain its current availability.


As in the case of the POP which reputable buyers’ facilitating outfits like the Reliable Dealings International require from any AWR seller before they can begin to do business with them, the other things that may often be required from a seller, would include the following:

= the Lifting Lease/License from the NNPC, and

= the Letter of Authority to Sell (ATS) from the NNPC. The Letter of Authority to Sell, also called a Letter of Allocation, which should usually come from the NNPC’s Crude Oil Marketing Department, is basically the official document which shows the buyer that the seller actually has the authority from the official government agency for the crude product that he’s selling. (Must usually be in a paper format and on NNPC company letterhead; must contain the date of issue and expiration date, and be signed. Scanned copies of document are alright. All documents must be certified, valid, authentic and verifiable.)

= If, for example, the seller claims that the cargo has been cleared, then he should usually be able to provide the CPA (Charter Party Agreement), the ATL (Authority To Load), and Q88 vessel details.


OK, so let’s say you’ve assembled the proper Proof of Product and the other essential documents such as those that are outlined above. There’s one key, in deed critical, question that still remains for you – how do you assure that these documents you’ve gotten from the seller are even any good? That they are real, valid, AUTHENTIC and GENUINE to warrant your taking the Seller’s offer seriously?

This question is, perhaps, often the most critical for a buyer because, as a rule, most fraudsters and con artists who operate in the Nigerian crude oil industry, are simply masterful forgers and copiers of every bit of the legitimate industry documents that are used in selling and buying operations by refineries and government agencies, and who are highly skilled at the craft. Consequently, buyers are strictly wary never, ever to accept outright at face value or be ever fooled by, any document submitted by sellers or claims made by them, however seemingly convincing or real-looking!. And what it all means, is that one crucial facility that a crude oil buyer and his aides must quickly develop and have, are some good, fool-proof, tools or skills by which they can INDEPENDENTLY VERIFY the authenticity of at least the key, most significant pieces of documents from among the tons of documents that sellers and their agents will often present them in the course of hawking their products. And, above all, that they must have the skills and the knowledge and business sophistication to be able to detect which ones among such documents are genuine and legitimate, and which ones might be plain bogus.

To be sure, making such verification and confirmation may often be problematic for a buyer. However, it is not really that difficult a task, at least for the schooled and experienced eyes. You only need to know what and what to look for, the right questions to ask, and how to counter check and cross check facts and information. And, in any case, whenever in serious doubt about the authenticity of a document, you should always take the path of caution – ask for more proof, or even reject the offer, depending on the particular facts at issue in an offer.

Chemotherapy – A Prostate Cancer Therapy

As the term suggests, chemotherapy is done using chemicals to kill rapidly dividing cells. At some point, one of the disadvantages of chemotherapy is that it also kills normal cells that divide rapidly. Since most cancer cells are really fast in splitting and dividing compared with the normal cells, most chemicals used for this type of therapy attack the cancer cells first.

Those drugs used for chemotherapy treatment work by impairing and misaligning cell division. And the targets are the fast-dividing cells. Eventually, as chemical compounds work to attack fast-growing cells, tumor cells which are prone to cell division is being damaged faster than the regular cells, thus allowing the impacted part to have less constraints.

Are we aware that chemicals used for the chemotherapy process effectively reduce younger tumors than malignant one? It is said that it is easier to cure a cancer if the roots are just new, and it is not in an advance state of growth. With chemotherapy, it triggers the division of tumor cells while leaving them with less ability to reproduce and create harmful compounds.

At the same time, as the chemotherapy process progresses, it totally ceases the division of the cells, thus preventing the cancer cells to spread out. For a prostate patient scenario, it is easier to cure the disease if the patient is in T1 and T2 of the disease. Since during these stages, the roots are just within the prostate gland, and the cancer cells are still intact to it. It did not spread out yet.

With the chemotherapy process, it will allow the chemical components of the drugs to attack the cancer cells within the prostate gland alone to prevent division and secretion of harmful elements damaging the good cells. As a result, the spread of the cancer cells will be stopped, since the cell division will be halted using the chemical compounds present on those drugs.

However, there are certain limitations on the chemotherapeutic process. It is a fact that the center core of the tumor is insensitive of chemicals, thus chemotherapy processes might not work. Since the center is the core of every activity of the cell division, because of that cells complexity, chemicals during chemotherapy does not reach the core or the center of the tumor.

This scenario implies that the chemotherapeutic process is not effective in killing the cancer cells. During this situation, another therapy higher than that of the chemotherapeutic process is being performed.

We might think that chemotherapy is a simple process but it is not.

Medical experts should make sure that the chemicals will reach the core of the tumor or else chemotherapy is useless since it will not kill the root of the cancer cells. The worst part is that, the chemical treatment might trigger the cancer cells to even divide rapidly after it is being touched. At some point chemotherapy might be the reason for higher-level surgeries and therapies.

Chronic Low Back Pain & Exercise Therapy Strategies

As described in the companion paper in the same issue of this journal exercise therapy encompasses a heterogeneous group of interventions which are effective in the treatment of chronic low back pain. Questions remain however as the best method to deliver exercise to have the best effect.

As a follow-up to the companion paper, this systematic review was then performed to identify some particular exercise intervention characteristics that improve function and reduce pain in adults with non-specific low back pain. The research was completed within some time and the results were kind of an amazing.

43 trials consisting of 72 exercise treatment groups and 31 comparison groups were included in the review.

Improved pain levels and functional scores were most common in exercise programs that:
• are individually designed and implemented in a supervised format
• may include home exercise with therapist follow-up
• were administrated in groups or individually (with a healthcare provider)
• Were high-intensity or high-dose in nature
• were administered in conjunction with other conservative treatments such as advice to stay active, non-steroidal anti-inflammatory medications, or manual therapy
• consist of stretching or muscle strengthening exercise

Conclusions & Practical Application:
This systematic review provides healthcare providers with evidence-based recommendations for implementing exercise therapy to treat chronic low back pain in adults.

Adults with non-complicated low back pain should consult a qualified health professional to have their specific case assessed, and an appropriate regimen of care, including exercise, prescribed. As always, do not begin an exercise program without proper supervision of a regulated health care professional.

Researchers looked at the likelihood of those who were undergoing screening for cancer and their corresponding vitamin D status and found there does appear to be a positive correlation. The lower the vitamin D status, the higher the chances of cancer was detected.

This said, the study did not prove if you do not have a deficiency of this vitamin you will be immune from cancer, so it is more an issue of reducing your overall risk factor than it is offering robust protection.

3. Erectile Dysfunction. Another connection to know about is erectile dysfunction and vitamin D status. It appears as though popping that vitamin may do the trick to help you protect against this problem.
