Breast Cancer – Signs and Symptoms

The cancer found in the cells of the breast is Breast cancer. A mass of breast tissue that is growing in an abnormal and uncontrolled way is a cancerous tumor. A cancerous tumor may attack the surrounding tissues or shed cells into the blood stream or lymph system. After lung cancer, breast cancer is the second most common type of cancer found world wide and fifth most common cause of cancer death. This is the most common cause of cancer among women in incidence as well as death. It caused 502,000 deaths worldwide in 2005. The number of cancer cases increased from 1970 which should be blamed on the modern styles of the present day world. Breast cancer is the most feared disease by women than the much more common fatal heart attack among women. The death rates for this cancer are declining for the last few years.

This type of cancer also occurs in men as the breast in males and females consists of same type of tissues. But it is 100 times lesser than the women and it has the same survival rates as women. The tumor in the breast is very hard to feel and have and irregular shape and not smooth. During a breast self exam the cancerous tumor may not move but the tissues around the tumor may move.

The cancer begins as single or more cells which lost its control of differentiation and increase but remains restricted with in the basement membrane of the duct. These divides and they invade the basement membrane of the duct and ultimately spread to distant organs also.

Four different classification or groups based on different reason and have a different purpose describes breast cancer:

(a) Pathology – Each tumor based on its histological appearance and for other reasons is categorized by the pathologist. The categories are invasive ductal carcinoma, malignant cancer in the ducts of the breast, invasive lobular carcinoma and the malignant cancer in the breast lobules.

(b) The pathologist determines the histological grade of the tumor under a microscope. A low grade tumor resembles normal tissue. A high grade tumor does not look like a normal tissue and composed of disorganized cells. Intermediate grade is in between the high and low grade.

(c) All breast cancers should be tested for expression or detectable effect of the estrogen receptor(ER), progesterone receptor (PR) and HER2/neu proteins and are done by immunohistochemistry and will be shown in the pathologist’s report. To predict the prognosis, the profile of expression helps the oncologist to choose the proper treatment.

(d) Stage of a tumor-The TNM classification is the currently accepted staging scheme and it is as follows:

Tumor- Depends upon the presence or absence of invasive cancer, the dimensions of the invasive cancer and the presence or absence of invasion outside of the breast there are five tumor classification values (Tis, T1, T2, T3 and T4).

Lymph Node- Depends on number, size and location of breast cancer cell deposits in lymph nodes there are four lymph nodes classification values (N0, N1, N2 and N3).

Metastasis- Depends on the presence or absence of the breast cancer cells in locations other than breast and lymph nodes there are two metastatic classification values (M0 or M1).

Breast cancers are not always classified by its histological appearance. Breast tumor is dense mass of cells appear whiter than the tissue around it. Benign masses are round or oval in shape but a tumor may be partially round with any shapes according to its circumference. If the mass has a multi pointed star shaped outline, it is known as speculated.

If there is a dense mass appears as tumor in a mammogram, ultrasound should be taken for that breast mass. If the mass shows an irregular outline or having fuzzy edges and is pressing the tissue around it, the person needs a biopsy of the mass. The most accurate way to diagnose its actual nature is an analysis of the tissue sample.

The exact cause of the this cancer is not known. The healthy BRCA 1 and 2 genes act as a tumor suppressor for breast and ovarian tumors. The mutated BRCA genes due to radiation or chemicals can give rise to cancer. The risk of tumor increases as the age increases. About 17% of women were diagnosed with breast cancer were in their 40s and 78% were older than 50.

General terms for this cancer

(1) Carcinoma is the term to describe a cancer that begins in the epithelial cells of organs. Most of the breast cancers are carcinomas.

(2) Adenocarcinoma which starts in glandular tissues. Glandular tissues in the breast are ducts and lobules and because of this it is known as adenocarcinoma.

(3) The early stage of cancer, when it is limited to the layer of cells where it began is described by the term Carcinoma in Situ. The cancer cells remain restricted to ducts and lobules are in situ especially in breast cancer. These types have not invaded in to the deeper tissues in the breast or spread to other organs of the body and are referred as non-invasive breast cancer.

(4) The one that has already spread beyond the layer of cells where it started are invasive or infiltrating cancer. Most breast cancers are invasive. Breast cancers are either invasive ductal carcinoma or invasive lobular carcinoma.

(5) Cancers that starts from connective tissues such as fat tissue or blood vessels are sarcomas. In the breast sarcomas are rare.

The different types of breast cancers are ductal carcinoma in situ, lobular carcinoma in situ, invasive ductal carcinoma and invasive lobular carcinoma. Less common types of breast cancers are inflammatory breast cancer, mixed tumors, medullary cancer, metaplastic carcinoma, mucinous carcinoma, Paget disease of the nipple, tubular carcinoma, papillary carcinoma, adenoid cystic carcinoma, phyllodes tumor and angiosarcoma.

Signs and Symptoms

The primary sign of this cancer is a lump that feels different that the surrounding tissue. Lump in the breast of 80% cases are discovered by the women herself. Lumps found in lymph nodes located in the armpits or collarbone can also indicate breast cancer. Other indications are change in breast size or shape, skin dimpling, nipple inversion or spontaneous nipple discharge. Testing

The most common methods for the test of breast cancer are self and clinical exams, x-ray mammography and breast magnetic resonance imaging (MRI).

Diagnosis

By examining the surgically remove breast tissue, the breast cancer can be diagnosed. The breast tissues can be taken by fine needle aspiration, nipple aspirates, and ductal lavage, core needle biopsy and local surgical excision. The accurate diagnosis of this cancer includes radiographic imaging along with these diagnostic steps. To detect metastasis imaging tests are sometimes used and it include chest X-ray, bone scan, CAT scan, MRI and PET scanning. Microscopic evaluation of a biopsy specimen can give a cancer diagnosis.

Treatment

The main treatment is the surgery of the tumor with the hormonal therapy, chemotherapy and radiotherapy. In individuals with high risk hormonal therapy has been used for chemoprevention. The three macronutrients are protein, fat, and carbohydrates. Ketogenic diets restrict your carbohydrate intake to 20 grams or less per day. Each carbohydrate gram has four calories, each protein gram has four calories, and each fat gram has nine calories. This results in the vast majority of your caloric intake coming from proteins and fat. Ketone bodies are made from fat, which is used by your brain for fuel instead of glucose from carbohydrates. And all complex carbohydrates must be avoided like grains, pastas, and breads. Just one slice of bread can have more than 20 grams of carbs. This can make staying on the diet a difficult task given the restrictive amount of carbohydrates allowed, which does limit food choices.

Summary

While scientific evidence has lagged in showing empirical findings that a ketonic diet works well for bipolar patients, there has been enough evidence that it is gaining attention, like at the Standford Medical School. And scientific evidence does show that it does similar things to the neurotransmitters in the brain, like lithium does, which is a common bipolar patient medication. While further research is certainly warranted, several bipolar patients have already begun prescribing to this diet and proclaiming that it does help with their symptoms. So, if you are a bipolar patient, utilizing a ketogenic diet might be worth trying to see if it helps alleviate the symptoms of your disorder.

Step One: The First Steps

There are several reasons why you may need to see a new doctor:

  • Maybe you are dissatisfied with your current doctor

  • Changes in your insurance coverage.

Maybe your new doctor is a specialist and your symptoms are beyond your primary care doctors expertise. If this is the case then you will want to make sure you understand the reason for the specialist. Be sure to write down the specific reasons for seeing the specialist.

Be sure to request the specialists name and specialty in writing so you can prepare for the first visit. This way you will be able to do some research so you will feel confident that the specialist has the right training and credentials and that he or she will be a good fit for you.

Before seeing a specialist you should check with your insurance company to make sure you follow their rules for specialist referral. It will depend on the insurance company and the specialist you are seeing, but the insurance company may require your primary care doctor to submit a record of the referral to them. Without you medical records, including any test results, it is much harder for the new doctor to get to the root of your problems. You could find yourself repeating test you already have the results to and they are both time consuming and costly.

With the changes in information technology, providing your doctors are in the same health care system, your records can be brought up through the electronic medical record on the computer. If, however, your records are not on the electronic medical record system your new doctor will have to have the hand written charts from your primary care doctor. In order for your new doctor to get your records you will have to sign a "Release of Medical Records" request form that allows your primary care doctor to send your medical records to the new doctor. It could take up to a week or so for your new doctor to receive the records in his or her office, be sure to include all records dating back to the first time you started experiencing symptoms, including test results since that time.

Step Three: Preparing for Your Visit

Your medical records will not tell the whole story of your health. You will have to fill in the blanks, that's why you should prepare for your first visit by writing down a complete description of your past and current medical conditions and treatments. Microsoft Works has a template that you can use and using a journal to document your symptoms also helps with this process. A symptom journal is a daily record of the symptoms you are experiencing over a period of several weeks or months. You will start with a description of your symptoms followed by the time of day the symptoms occurred, the length of time they rented, anything that may have relieved the symptoms, and any other factors associated with their sunset. If you keep this diary for any length of time you will start to see patterns. These patterns will make it easier for you to either avoid whatever is the trigger or at least prepare yourself for them. I used this method when I was first diagnosed with fibromyalgia and I kept it for over 10 years. I was able to determine my triggers for the fibromyalgia and now I can prepare for them and there are several I absolutely avoid. This method has helped me to manage my fibromyalgia.

If you put all this information together ahead of time it will reduce the likelihood that you will forget important information that will help your new doctor to make a diagnosis or prescribe effective treatments. This is a list of information you will want to collect:

  • your current health conditions.
  • any over-the-counter and prescriptions medicines you take, as well as any vitamins and supplements.
  • any allergies you may have.
  • your health history, including conditions you've had, medicines you have taken, procedures, surgeries and medical tests you're gotten.
  • your family's health history and this includes any conditions that your parents, grandparents, siblings, and children have or had.
  • the names and contact information of other doctors you have seen.

Having all this information on hand can help the doctor by informing him of any symptoms you have that are related to medications, he will be able to make the connection. If you take a lot of medicines and supplements, it might not be a bad idea to bring all the bottles with you. Just put all your medications into a plastic bag and bring with you.

Your new doctor may send out the papers for you to fill out at home before your appointment instead of having you wait until you get to the office, or if it is possible you can fill out the information needed online.

Also, do not forget to bring a list of questions with you that you will want answered in your first visit. A list will prevent you from forgetting to ask something that is important to you. It also helps to eliminate that last minute "Oh by the way can you explain ….." as you walk out the office door.

Here are examples of some questions you may want to ask your doctor.

  • What condition do I have?
  • What are the potential short-term and long-term effects of my condition?
  • Is there anything I can do to prevent further problems or complications from my condition?
  • What are my treatment options? How do they work and what are their potential benefits and drawbacks? Side effects? Cost?
  • What would be the consequences of "waiting it out" and not following through with treatment?
  • Where can I find reliable information about my condition?

Now it is the day of your appointment and here are a few things you will need to do for your first visit:

You will need to get directions if you are not familiar with the location and also parking information if you are arriving in a car. I usually take a dry run to the doctors office to determine where it is, and how long it will take me to get there.

Next you will want to plan to get there at least 15 minutes early. Every doctor's office has a different check-in procedure. The front best staff may want to verify or update your information, including the insurance information. There are always changes that will or can occur between appointments, and it is critical that the staff be up-to-date.

Take your insurance card with you because the staff may want to make a photocopy of it and it will give the co-pay and contact information for requesting prior authorization for medical tests or drugs.

Make sure you get answers to all the questions on the list you made up and any that may have come up during the doctors consultation. At least make sure that you understand everything the doctor is saying to you. Do not hesitate to ask the doctor to spell difficult terms, or write down information that is hard to understand. It is also a good idea to bring someone with you. I always had a hard time trying to relay what the doctor had to say, back to my husband because I was usually in a fibro-fog when I went to see the doctor. I started caring a tape recorder with me on my office visits so I could play it back for my husband and myself.

Toward the end of the initial visit you should have a clear understanding of how and when you will be following up with your doctor. If test results are involved, you should know how you should know how you will get these results once they come in. Before you leave, ask for the name and phone number of the person to contact you should have questions later.

I'm sure that you are thinking that this looks like an awful lot of work, but in the long run your first doctors visit will go much smoother if you prepare for it and you'll walk out of the office with a better understanding of just what is wrong with you.

Air Supply Usage – Monitoring oxygen usage is vital for a safe return to the service. One rule is the rule of thirds. This provides you with a third of your air for the start of your dive, the return, and a supply of air should an emergency arise.

Scuba diving is a thrilling activity. Always remember to be safe before getting into the water to ensure you and your diving partner or diving group come back safely.

PEMF Therapy Beneficial For Treating Ocular Conditions

There are many individuals who are concerned about ocular treatments for conditions such as hyphema, glaucoma, and cornea burns. Most of these conditions require extensive treatment including surgical procedures in order to improve them. There are quite a lot of people who experience these conditions that are concerned about the treatments offered them from their medical provider and could benefit from PEMF therapy.

One group classified as ELF 2, which the therapy was for thirty minutes, two times a day at a 2mT intensity. Another group, the ELF 5 group, which also received 30 minute treatment twice a day at a 5mT intensity. Medical therapy (MT) and controls concluded the categories. The study, was a 14 day trial, where examination and digital photos of the corneas were taken initially, and also on days 2, 7, and 14. Upon completion of the study, the rabbits were euthanized and histopathology was to to evaluate the affected eyes. The clinical and histopathologic results were compared in all four groups. The results showing that low frequency pulsed electromagnetic fields treatment provided a significant less defect than those in the MT group, as well as, a lesser loss of keratocyte. Thus proving that this noninvasive treatment for alkaline burns on corneas is a remarkably effective method for therapeutic and healing purposes for eye conditions.

Another evaluation was performed using rabbit models who had well-defined hyphema for evaluating the effects of PEMF therapy. The test included sixteen eyes, broken into two groups, being treated in postoperative conditions with 10mT and 20mT on 4 separate days for sixty minutes per day. The study also included a control group with hyphema alone and a group without hyphema but PEMF irradiation. The resorption rate was recorded each day and histopathologic evaluation was also performed in this study. The 10mT group showed reduced endothelial cell damage, as well as, a reduction in fibrotic clots. Although the resorption time was not markedly different in any group, there was proof that PEMF eye therapy has a positive influence on serious side effects and when used as an adjunctive treatment is effective in ocular trauma and inflammation.

PEMF eye therapy is a proven, safe, noninvasive and effective treatment for ocular conditions. Those individuals who want an alternative to conventional treatments have an alternative.

Personal Growth – Never Stop Learning and Growing

As a recruiter, I come into contact with candidates and job orders that teach me something new every day. The lesson I learned last week is that I should never stop learning, but rather soak up every piece of information wherever in print, audio, on television, online, bathroom wall, wherever I can glean new information, I had better absorb it. I now realize that I never want to get to the point in life where I am stagnant and no longer moving forward.

I met a candidate a few weeks ago, who I thought would have been fabulous for a particular position, but the manager felt he might be too advanced in his career. The reason that he thought he might be too advanced is because he was older and the stigma that older candidates face is that they are too old to learn new technology. It was at that point that I realized that I better keep new information flowing through my gray matter, or else I might grow stagnant myself. This was a wakeup call for me, my ah-ha moment, if you will.

You see, there was a time, not so long ago, I honestly thought that one day I would be perfect in my thinking. I know it sounds arrogant, but let me explain what I mean by "perfect". I thought that I would get to the point in my emotional maturity where every word that came out of my mouth would be filled with sage wisdom, every thought that came to my mind would be straight from God and every action that I took would be perfectly orchestrated and the exact same action that God Himself would have taken! I'm not sure where that mind came from, but suffice it to say, it was a mindset that would put me into a mental tailspin every time that I said something off color, not in line with the Word (ugly words included) Egypt responded completely opposite of the way I should have responded. When these "failures in action" occurred, it would set me back to square one, almost like seeing "failure-student will remain in 3rd grade" on a report card. Then I would have to start all over in an attempt to relearn what I already knew. I had not even thought about my career and progressing in new technology or processes, until last week when I realized that I would be learning and improving until the day I die.

Vitamin B Deficiencies Show in Your Tongue

No person is deficient in any one B vitamin without being deficient in all of them! That’s because the B vitamins occur together in food as part of a complex. This is true of all vitamins and minerals. They work together.

There are, however, as many degrees and variations of B-vitamin deficiencies as there are different individuals. Formerly the disease beriberi was thought to be caused by a deficiency of vitamin B1 (thiamine) and pellagra by lack of the B vitamin, niacin (B3).

When human volunteers stayed on diets lacking vitamin B1 (thiamine) or niacin (vitamin B3), however, neither beriberi nor pellagra has been produced. These diseases actually result from multiple deficiencies of all the B vitamins, the lack of vitamin B1 or niacin being only more prominent.

Even schizophrenia is caused by a vitamin B3 (niacin) deficiency but it too won’t be the only B vitamin deficiency – just the main one. Even The list goes on and on.

Your Tongue

The good news is that you can tell how adequate your intake of B vitamins has been by looking at your tongue. It should be moderate in size, an even pink in color, and smooth around the edges without coating or indentations showing where it has rested against your teeth. The taste buds should be uniformly small and cover the entire surface and edges.

If you can find a healthy child, you may see what the normal tongue should look like.

Vitamin B Deficiency Indications

Many changes take place in your tongue when you’re short of B vitamins. The first change appears to be enlargement of the buds at the front and sides of your tongue. Later these buds become small or even disappear, making the tip and sides smooth, whereas the buds farther back will progressively enlarge. These buds have a flat appearance like button mushrooms.

As the deficiencies of these vitamins become more severe, clumps of taste buds fuse and grow together, pulling apart from other clumps and thus forming grooves or fissures. The first groove usually forms down the center of the tongue.

Groovy Tongue

In a severe B-vitamin deficiency, your tongue may be so cut by grooves and fissures that it looks like a relief map of the Grand Canyon and surrounding territory or a flank steak run through a tenderizing machine.

When deficiencies are still more severe, your taste buds literally disappear. First the tip and edges become smooth and shiny; then the buds disappear progressively from front to back. This extreme condition is found most often in elderly persons whose diets have been inadequate for years; they complain that their foods have little flavor. In some cases such tongues are intensely sore. In other cases, persons having extremely abnormal tongues are surprised to find that they differ from normal.

Beefy Tongue

The size of your tongue also indicates deficiencies of these vitamins. It may be large, beefy, and full of water (edematous). Often such a tongue shows scallops around the edges where it has rested against the teeth. The beefy tongue is called that because it has the appearance of beef and is usually an intense deep red. On the other hand, it may become too small, or atrophied.

Purple Tongue

Other tongues may have a purplish or magenta cast, and still others may be a brilliant red. Often the tongue shows a combination of colors with perhaps a red tip and a magenta center. The color and texture vary depending upon which B-vitamin lack is most prominent.

For example, a magenta tongue (the color seen most often) indicates that a deficiency of vitamin B2 predominates over the other B-vitamin deficiencies. A beefy tongue is thought to show that pantothenic acid (vitamin B5) is particularly under-supplied. When deficiencies of vitamin B12 and folic acid are most prominent, the tongue becomes strawberry red and smooth at the tip and sides; it is often shiny and not coated.

Fiery Red Tongue

If the deficiency is predominantly the B vitamin, niacin (vitamin B3), your tongue may be fiery red at the tip and may appear to be either too small or too large and so coated that it is fuzzy with debris. The heavy coating is caused by the growth of undesirable bacteria; it usually indicates much putrefaction in the intestine. Since valuable bacteria in the intestine produce B vitamins, such coating probably never occurs if bacteria growth is normal.

Stomach Acid and Digestion

Studies indicate that 60 – 100% of the persons showing severe tongue changes are unable to produce sufficient amounts of hydrochloric acid in their stomachs. This leads to the use of antacids – the worst thing you can do because the problem is actually caused by too little acid.

The output of digestive enzymes is far below normal. In such cases, digestion is so faulty that unless tablets of hydrochloric acid and digestive enzymes are taken temporarily, much gas, flatulence, digestive disturbances, and discomfort may be experienced.

In fact, if your digestion is so faulty that you have intestinal gas after you add foods rich in the B vitamins to your diet, you can be sure you have been deficient in these vitamins. The best thing to do is continue and take enzymes till the deficiency has been corrected. The worst thing to do is stop because then the problem of the deficiencies is still there.

Water Soluble

Most of the B vitamins dissolve in water and for this reason cannot be stored in the body. Just as a sponge can be slightly moist or dripping wet, however, so can the cells hold little or much of each B vitamin, depending on the amount offered. To maintain ideal health, the offering of B vitamins should be sufficient for each cell to take all it can use to advantage. Any B vitamins not needed are excreted in the urine.

B Vitamins Work Together

It appears that all B vitamins work together. This co-operation is called the synergistic action of the B vitamins. The taking of one or more B vitamins increases the need for the others not supplied, probably because any one B vitamin alone can increase the activity of each body cell. The group in its entirety can be obtained only from such foods as liver, yeast, and wheat germ. (Please note that due to a high phosphorus – also an important mineral – content in Brewer’s Yeast it is important to balance it with calcium.)

To discuss the deficiencies of the B vitamins separately is as unrealistic as to believe in men from Mars. Such deficiencies exist only in an experimental laboratory. A deficiency of one, however, often predominates over others. If the first symptoms of deficiency are recognized, they can serve as a warning that unless your nutrition is improved, great deviations from health can be expected.

(*Beriberi is a disease of the nervous system caused by a person not getting enough thiamine (vitamin B1) in the diet. Thiamine is needed to break down food such as glucose. It is also found on the membranes of neurons. Symptoms of beriberi include severe lethargy and tiredness. Beriberi may also cause problems that affect the cardiovascular system, the nervous system, muscles, and gastrointestinal systems. It is often found in people with a history of drinking too much alcohol or who don’t eat healthy. There are two kinds of beriberi, wet and dry. Wet beriberi mainly affects the cardiovascular system. Dry beriberi affects the nervous system

**Pellagra is a disease caused by a lack of the vitamin niacin (vitamin B3). Symptoms include inflamed skin, diarrhea, dementia, and sores in the mouth… Primary pellagra is due to a diet that does not contain enough niacin and tryptophan.)

Is Trichotillomania a Reason of Your Hair Loss?

Some people may do this consciously and are getting a good feeling while pulling their hair off, while some may be pulling their hair unconsciously without even realizing it. After pulling the strand some may inspect it or even swallow it. The symptoms usually are:-

shame and even denying what they are doing

experiencing relief when the hair is pulled

How common is Trichotillomania?

Trichotillomania is a pretty rare condition in which only about 1 percent of the population may have it. This condition is usually found in females outnumbering males in a ratio of 3 to 1. There is a problem in finding the frequency of the people who have Trichotillomania. This is because people tend to deny that they have the disorder and do not seek help from professionals. Trichotillomania usually begins around the age of 11 to 13 however, there is a possibility that children of age 1 to 2 also have Trichotillomania and pull their hair.
