Dislocated Fingers – Prehospital Treatment

I see them all the time in the ER, and they can cause a whole lot of pain. That's not to mention the horror of seeing your finger take a left turn from your other ones. When an injury occurs during sports or out in the backwoods, you often can not just drop what your doing and go to the ER. Many coaches or trainers will attempt to relocate the finger, sometimes fixing it, sometimes not! Here's a basic guide to show you when, and how to attempt a finger relocation:

  1. Just keep in mind that you'll always have to have a doctor look at it.
  2. If the dislocation is absolutely unbearably painful, or if the victim will not let you attempt a relocation, then simply splint it in it's present position, so you can to minimize the discomfort from the affected area bouncing around, and go to the ER.
  3. If you decide to continue ….
    1. Gently squeeze the middle of the bones, NOT THE JOINT, in an attempt to see if the bone is broken. If you hear any crunching, stop, splint, go to ER.
    2. Then feel the joint gently. Try to ascertain wherever the tip of the distal (farther) bone is dislocated above or below the tip of the proximal (closer) bone.
    3. If you're sure that the bones are not broken, the grab the first bone with one hand (the side before the dislocated joint), then grab the distal bone (the bone after the dislocated joint) and pull firmly.
    4. Many times you'll have to apply direct pressure to the joint, with your thumb and forefinger, while pulling. In other words, you'll take the joint between your thumb and index finger and pull the distal bone, while making a squeezing snapping motion.
    5. This maneuver is not for the queasy. It hurts like hell, but it feels better immediately after it's back in. With that in mind, do it FAST AND DELIBERATELY. Do not be wimpy and try not to hurt your buddy, just get it in the first time. If you feel any crunching like broken glass, STOP!
    6. You'll feel it literally flop or pop back into place if you are successful.
    7. Check for full range of motion, or a remaining deformity, then go see a doctor.
    8. Have a beer, you deserve it!

Federal Funding for Cerebral Palsy Research

In March, National Cerebral Palsy Awareness Day came and went as it commonly does, reliably unnoticed by anyone whose lives are not directly affected by it.

For those who may not know, cerebral palsy is an umbrella term referring to a (currently) incurable, non-progressive set of neurological conditions that produce physical and neurological abnormalities during human development. Cerebral palsy is caused by brain damage, either through injury or irregular development during the early stages of life. The damage usually occurs just before, during or shortly after the birthing process. Very few infections are born with detectalbe symptoms. Most will begin exhibiting them over the first few years of life as the child misses developmental milestones.

CP can disrupt a person's ability to walk, talk, stand, sit, swallow or perform other simple movements that healthy people take for granted. Exactly how and to what severity a person is affected can vary. Some patients suffer only minor difficulty with balance and fine motor skills, while others are literally trapped in their own bodies with painfully rigid, useless limbs, unable to speak or swallow.

Cerebral palsy affects more children than multiple sclerosis, cystic fibrosis, spina bifida, hemophilia, fetal alcohol syndrome, muscular dystrophy or childhood leukemia, however, research into the cause and cure of CP receives only a small fraction of government funding compared to these other disorders. These other disorders deserve every penny of research funding that they get, but that does not explain why CP warrants such low consideration. It's certainly not because it's a rare disorder.

One in every 303 US children have cerebral palsy. In the African-American community that number is one in every 270. Shockingly, in 80 percent of CP cases, the exact cause is never accurately determined. Even more shocking, is the fact that the legal community is responsible for determining the specific cause of individual CP cases more often than the medical or scientific community.

And it's not because CP does not cost US taxpayers millions upon millions of dollars in healthcare expenses.

The average lifetime "additional direct cost" for a single person with cerebral palsy is estimated at over $ 1.5 million above and beyond the comparable cost of living for the average, healthy American. Federal and state Governments, in the form of Medicaid and other tax-funded social services absorb the lion's share of these expenses. Back in 2000, it was estimated that the total lifetime medical and care expenses by all US citizens currently living with CP would total more than $ 13.5 billion. Those numbers have risen since then.

Additionally, 75 percent of people with cerebral palsy have at least one additional developmental impairment such as blindness or vision impairment, deafness or hearing injury, autism, arthritis, epilepsy and other intellectual disabilities that are direct result of the CP.

Currently, the US Center for Disease Control (CDC) has no funding appropriated for CP research. The National Institute of Health (NIH) provides some funding, but not for innovative, curative research.

If you know even one family that deals with severe cerebral palsy, then you should not require any further convicting about the compassionate nature regarding a need for federally funded research. If you do not, then the costs to you and other American citizens should be enough to get your attention.

In this time of budget sequestration, repeated natural disasters, debt and economy woes, it may not seem like the most opportune time to lobby the US government for money specifically targeted for CP research, but that could not be more wrong. Do not just take my word for it. Do your own research. A federal fund commitment for the cure, prevention and causation of cerebral palsy will allow scientists to develop effective strategies for the prevention and treatment of CP that will, in the end, more than pay for itself.

The Paralysis of Fear and How to Deal With It

Picture this scene which I sure you have experienced at least a few times in your life:

It is a beautiful sunny day, just perfect temperature. Everything in your world could not be better. In fact all things seem perfect. You even find yourself singing your favorite song.

Then out of the blue a flash thought crosses your mind. Something you were very concerned about the other week, and you had put out out your mind, comes darting back. You had no answer for this thing at the time, and could not facing thinking about it any more, so you choose to forget about it. Now it returns to haunt you, and immediately a sinking feeling hits your stomach. A flood of thoughts come rushing in to overpower your mind, bringing a very negative shadow of fears and doubts.

Now you are feeling terrible, your body starts to tense up, you can not even think of the words of the song you were singing before, and you are not even sure if the sun is still shining.

What happened? Nothing in your circumstances changed.

A tiny flashing thought changed the whole day for you. How come? When you put that concern out of your mind you kindly transferred it from your conscious mind, to your subconscious mind. Along with the thought, the emotions you felt at the time were stored in the same "file folder". This folder in your mind, was simply waiting to jump out and haunt you at any given moment to ruin your day, and that is what it did.

Facing Our Fears.

I am sure you are familiar with Job of the Bible Job was the one who went through some severe trials and ended up with double what he once had when it was all over. In the midst of his tribulations, he made an interesting declaration: That which I feared has come upon me. "(Job 3:25) Wow.

Unfortunately there is living proof of this truth within my own family, I had an uncle, my father's only brother who I vaguely remember (I was 3 years old when I saw saw him). His one fear in life was he would get sick with polio which at the time was an epidemic. He died of polio at the age of 21.

So fear is a dangerous thing to allow staying in our lives .

Molly Gordon describes fear as Fantasy Expectations Appearing Real. Michael Pritchard quotes fear as being "that little darkroom where negatives are developed".

Negative fantasies incubated in the darkroom of our conscious and subconscious minds create fears.

So how in the world can we uproot these fears from our lives?

First thing to do is to take a few moments in a quiet place, with a blank sheet of paper and a pen / pencil. Write down all the things you fear. Write fast, and note down everything that comes to mind, whether you think you fear it or not. When you write fast, it helps your mind not get a chance to "challenge" with the thought: "no I really do not fear that".

Now read them back one at a time and face them. There are times when you can do something to avoid the "worst case scenario", and if this is the case then do it! (If you read the Bible you will know that James tells us that faith without works is dead, in other words, real faith, or the law of attraction has to ACTION to be complete), Either will work without ACTION.

The situations that are out of your control will take faith to eliminate. However there is SOMETHING you can do. You can make sure your thoughts are free from fears.

Fear is an emotion, which results from a series of fantasy thoughts which dramatize a negative exit of a situation. You can certainly conquer these thoughts with the power of your will.

This power of the will (or decisions) is first applied to the thought patterns. The decision to change the thought patterns must be made BEFORE any high pressure incident occurs. This will ensure we do not get "jumped" by sudden thoughts that bring fear and doubt that change our emotions before we realize what has happened.

One of the very best ways to achieve this change of mindset is to form a habit of taking time every day to meditate on something positive. Read a chapter of the Bible, a devotional book or a motivational book.

Think about what you have read.

Ask yourself: "How can I apply what I have read to my day". "If something difficult happens today, how will I handle it?"

You will find your day goes a lot better after spending some quiet time at the beginning, with some positive reflection and even some planning for the day.

Let's step out of the world of fantasy expectations and start creating a positive reality.

While we enjoy using 4×4 ATVs to plow snow and tackle mud bogs, we also love to let loose and just ride for fun. And there's nothing like carving the dunes or dicing through the trees on a lighter weight sport quad or the freedom they provide.

Here are four sport quads between 330 and 400cc that are capable of providing a great joyride or day escape. Each packs more punch than the small entry-level models and is less intimidating than the larger displacement speedsters and race-specific quads.

Honda TRX400X

This sport quad was the talk of the industry a decade ago, but that was a different time and 450cc sport quads did not exist! Despite its age, the 400X (the artist formerly known as 400EX) has proven itself to be fun, reliable and affordable! Its 2005 upgrades included a sportier look and reverse to make it more convenient. Then in '08, it received a sleeker fender package and new seat.

Powered by an air-cooled single-cylinder 397cc four-stroke, the 400X does not quite pack the hit or zip as Suzuki's Z400, but it's darn close and every bit as enjoyable. While the 400X could benefit from the addition of electronic fuel injection, the 38mm piston-valve carburetor has proven to be reliable and provides a smooth throttle pull.

Its narrower 45.5-inch width and 32.5-inch seat height make runs through heavily forested trails quick and controllable. Aggressive cornering and big jumps can overmatch the front preload-adjustable Showa shocks. However, while we'd love to see better front shocks, complete with piggyback reservoirs, we understand this machine's affordability would diminish. The fully adjustable rear shock offers 9.1 inches of travel and does an admirable job of soaking up hard hits.

At 408 pounds wet, the steel-framed 400X is considered "heavy" for motocross racing, but it's capable of tackling milder tracks in stock form. Aluminum wheels and an aluminum swingarm reduce the weight somewhat. The 20-inch rear Ohtsu tires are decent at supplying traction and work well in muddier and sandy conditions, but could be stronger.

The 400X has one of the industry's best chain adjustment systems. Triple disc brakes are standard and provide ample stopping power, yet are not as instant as some sport quads we've ridden. The newer seat is less plush but
is also more refined – with a slender front section and wider rear edges – for sport riders. Since its '05 redesign, the 400X has continued to rise in price (up $ 500).

With the current state of the economy, model saturation and sport quad sales figures continuing to decline, we wonder if the 400X will remain in the lineup or follow the 250R into Honda heaven.

Polaris Trail Blazer 330

Polaris categorizes the Trail Blazer 330 as an entry-level ATV and we agree. Although, its weight, displacement and overall size may be too much for some first-time riders, its automatic transmission, power, full floorboards and single-lever braking make it easier to ride than the other three quads in this group. Plus, a host of 2010 upgrades have improved this machine.

Updates were made to the plastic, seat, lighting, suspension, chassis, disc brakes, master cylinder and fuel gauge. Visually, it's easiest to see the changes to the modern plastic, taller seat and the brighter front headlight, which was borrowed from the Outlaw and has 28 percent more power. Owners should also notice the more convenient remote fuel gauge even if they fail to realize the fuel tank's capacity greater .75 gallons.

It's also easy to spot the Trail Blazer's new stance. New floorboards are fashionable and functional and contribute to its improved ergonomics. The seat is more contoured and longer and its height increased an inch, up to 35 inches, due to thicker seat foam and a new suspension. It could have the most comfortable seat in the sport quad market. The ground clearance declined from 5.5 inches to 4.75 because Polaris added a beefier eccentric protection skid plate, which it calls "much improved."

On paper, the 330 is also three inches taller. That's partly due to the incorporation of the half-inch taller Sportsman handlebars that are designed to better accept a windshield and other accessories.

Beneath the plastic, the suspension and chassis also went under the knife. Sportsman front struts (castings), with separate front spindles for mounting the wheels, replace the old front-end setup. Also, the shocks have new springs to improve the ride. Polaris engineers made the chassis stronger, improved the engine mounting and alignment and increased the skid protection. Braking action was altered by using the Sportsman calipers, larger brake discs and new master cylinder, which upped the bleeding capacity. The machine is also an inch wider, but retains the same A-arms.

The 329cc four-stroke engine does OK for lighter riders, but feet sluggish for heavy riders, especially when the quad's 492-pound dry weight is factored in. However, the Trail Blazer better accommodates larger riders due to its overall dimensions. But then again, Polaris did not design this ATV to break any speed records or contend for honors on an MX track. It was built for convenience, ease of use and cruising terrain and other moderately challenging trails. The Trail Blazer has always produced stable cornering and can be fun to wheelie if the Carlisle tires find enough Traction.

Suzuki QuadSport Z400

Although the Z400 is the most expensive 400-class sport quad, it's the most comfortable and loaded with features. Electronic fuel injection was added last year giving the Suzuki improved throttle control and stronger, more efficient acceleration. There's nothing wrong with a quicker 400, right? For anyone not looking to compete in a motocross race, this is the Suzuki sport quad for you. Even if you do want to race it, the removable headlight, suspension and stronger steel-alloy chassis should appease you.

The resilient 398cc single-cylinder four-stroke has the most thrilling power and is the lone liquid-cooled mill in this quartet. It fires electronically and is outfitted with a near bulletproof five-speed transmission with reverse. I've ridden the Z400 with a group of 450 machines and, despite underpowered, its capabilities, speed, controllable powerband and comfort levels all made me forget about its power disadvantage.

At 46.9 inches, the Z400 is the widest of these four ATVs and has the best stock suspension. Despite its advantageous width, the four-stroke remains a stable stalwart in the dense woods and can carve through trees like a love-hungry 10-point buck. A 31.9-inch seat height helps it rail corners.

Fully adjustable piggyback shocks supply excellent suspension travel figures and superb tuning capabilities. The linkage-type rear end, with it's lightweight aluminum swingarm and single shock, offers 9.1 inches of wheel travel. Up front, the fully independent A-arm suspension offers 8.5 inches of travel and tracks very well – point and shoot! In addition, this machine feels lighter on the trail than its 425-pound curb weight would suggest.

Ergonomically, the Z can appeal to riders of various sizes and shapes. The Z400's signature T-shaped seat may be the best platform in the business and is definitely the most copied. The larger 46mm foot pegs aid in supporting
boots for aggressive racers and weekend warriors alike. The 20-inch rear tires are great for casual woods riders and admirably supply a good balance between straight-line traction and sliding.

For those who enjoy a custom look, Suzuki offers a limited edition Z400 with special graphics and black wheels, for an additional $ 200. The Z400 is in a similar predicament as the Honda 400X, seeing huge sales figures early on in
its history and market saturation more recently. However, the Z400 is Suzuki's trail machine not the QuadRacer 450, which targets MX racers. Plus, the addition of EFI gives it a technological edge over the other guys.

Yamaha Raptor 350

The Raptor 350 dates back to 1987 and the Warrior nameplate, yet it's still living off a 2004 redesign and its new name. And the fact that it shares a lot of parts with its big-bore brothers, the Raptor 700R and YFZ450, and has a unique package also help. Yamaha has confirmed the 350 Raptor will return to its lineup for 2010.

The hearty, two-valve, 348cc, air-cooled four-stroke is just enough muscle to put a smile on your face, but can ever leave you wanting more. Reverse gear has also been a strong selling point for the Raptor, even though it has the trickiest reverse lever to operate in this group. The Raptor's six-speed transmission is also unique to the industry.

Surprisingly, at 396 pounds (wet), the Raptor is the lightest quad in this group. Even so, it can feel heavy and slightly underpowered when the tracks require all-out speed. On the forest trails, however, the 350 is better than average partly due to its thin 43.1-inch width.

That skinny design and a dated suspension can also make it more challenging to control for inexperienced riders. And the five-way preload-adjustable shocks can only do so much to slow body roll. Some of the handling quirks are also due to the Raptor's tires; especially the rear treads which have a more rounded profile and are often slide happy. The hydraulic discs do the job and I've always enjoyed the flip-type parking brake.

Unfortunately the Raptor's size may be its largest detractor. It's the shortest (length-and height-wise) and the narrowest machine in this group and has the shortest wheelbase. This ergonomic configuration can make some riders taller than 6 feet feel cramped.

Pain & Injury Management – Understanding Your Symptoms

Many people struggle to understand their flaws or know who to turn to for help. The purpose of this article is to help you differentiate between various types of injuries and the different types of treatments available.

Chronic versions Acute Injuries

Chronic Injuries can be defined as persistent injuries with an insidious sunset. The injury develops over time and is a result of a range of factors which include our postural habits, the physical stresses we place on our bodies, nutrition, hydration and stress. The human body is not designed to remain in the same post for several hours a day. Doing so will create imbalances in muscle function and altered posture, which in turn increase stress on joints and other tissues, leading to pain and dysfunction. Examples include chronic musculo skeletal pain, tendonitis, bursitis and Repetitive Strain Injuries (RSI). We often fail to seek treatment until the odd twinge or recurrence injury becomes regular fixtures in our lives. The longer the injury exists, the longer it will take to resolve, so taking action early can save a lot of pain, frustration and money.

Acute Injuries arise out physical trauma, they include fractures, dislocations, open wounds etc. The coverage of these injuries in this article will be limited as they all require the same treatment protocol, being first aid, and depending on the injury may require medical attention followed by physiotherapy.

Symptoms indicate which tissue is involved, it can be one or all of those listed, in varying degrees of severity, and more than one tissue may be involved.

Nerve impingement:

Nerve impingement refer to pressure placed on a nerve or nerve root by surrounding tissue, which may be bone, disc, muscle or fascia. Hernias and tumors can also increase pressure on nerves. Symptoms include numbness, tingling, pins and needles, weakness, and sometimes a degree of loss of control of an affected limb. It is for example, impingement of the sciatic nerve that leads to a temporary numbness in the buttocks, legs and feet, caused by muscles tightening around the nerve. This is often caused by sitting in an unnatural position for some time. Stretching the gluteal muscles and sports / remedial massage can help prevent and treat these symptoms, however, if the symptoms persist after the activity which caused them has ceased, or they recur frequently, then medical advice should be thought. Your doctor may then recommend that you see an Osteopath, Chiropractor or Physiotherapist.

Numbness in peripheral areas such as fingers or toes, or around the mouth can also be indicative of a viral infection or a metabolic disorder and should be discussed with your doctor.

Nerve impingement is often neglected as the symptoms are not sufficient enough to prompt action, yet it can be a precursor to nerve infection, a very painful and serious condition.

Nerve infection

An impeding nerve can become inflamed. The inflammation further increases pressure on the affected nerve and causes its dysfunction. This is symptomized by sudden sharp shooting or stabbing pain, which follows the nerve pathway, therefore it tends to radiate from an epicentre to another part of the body eg along a limb. It can cause muscle spasms and reduced function in the affected limb. In both nerve impinging and infection, the symptoms can occur with or without movement but are often worsened by specific positions. Sciatica for example, is inflammation of the sciatic nerve and causes pain down the side and back of the leg, the inner thigh and into the foot. Viral infections such as Shingles also cause nerve inflammation.
Nerve infection can be excruciating. You should seek medical advice as soon as possible as permanent nerve damage can result if the cause is untreated.

Muscle spasms and strains

In the absence of an impact or accident, very few musculo skeletal injuries are "sudden" They occur as a result of long term muscle tension, often thought about by remaining in one position for several hours a day ie sitting at a computer. A wry neck (torticollis) is an excellent example, where a normal neck movement can result in agonising pain and restricted movement.

When muscles remain in a state of prolonged contraction, blood flow through the tissue is restricted, thus reducing the delivery of oxygen and nutrients to the muscle and impeding the removal of metabolic wastes. Surrounding connective tissues and the nerves which run through the muscle are also affected; adhesions form in the myofascia – the slippery gel like connective tissue that is integral to the muscle tissue. The fascia not only provides the tensile strength of the muscle, but also acts as a lubricating surface to facilitate the smooth glide of muscles and other tissues over each other. Testimonies in the fascia account for many of the postural changes which occur in our bodies, and a significant amount of pain in musculo skeletal injury. Pain can refer through the fascia, resulting in a variety of symptoms from sharp jabbing pain on movement to burning and tingling. The latter is a result of nerve sensitization caused by the fascia and or muscle impinging on the nerve endings, and a build up of metabolic wastes in the tissue. Trig points form in the muscle, areas of hyper reflexivity in which dysfunctional nerve function will cause muscles to spasm, and shorten, thus reducing the normal range of motion. Any attempt to lengthen the muscle or achieve normal range of motion will evoke a "stretch reflex" which leads to increased spasm and pain. In this state the muscle is highly susceptible to injury through sudden movements or loads, and stretching should be avoided.

Before you get to the stage where you are experiencing the symptoms above, you will receive plenty of clues that all is not well. Over a period of time you will experience dull muscular aches, stiffness and muscle fatigue that is likely to be worse in the morning due to the build up of toxins overnight. Pain tends to dissipate as activity increases. These are signs that you need take action! At this stage applying heat to the area and remedial massage can restore normal function and prevent injury. If you choose to leave it until you are unable to move without significant pain then you will need a longer course of intensive treatment. Regular massage and preventive exercises such as Yoga and Pilates can increase your performance and prevent injury.

Care should be taken if you have an injury – do not forget most muscle injuries are overstretch injuries so seek advise from a sports massage therapist or physio before taking up new activities.

Acute muscle injury

If redness, swelling, turgidity or bruising are present as well as loss of or reduced function, massage and heat are contraferenced and can cause further damage. It is illegally these will occur without some form of trauma, so the principles of acute injury management apply: first aid and see either a doctor or physiotherapist.

Tendon injuries (strains)

Tendons are the inelastic ends of the muscle which attach muscle to bone. A torn tendon will usually result in spasm of the muscle with corresponding pain and reduced function of the muscle and joint. The symptoms usually occur at the time of the trauma, and as with any acute injury will worsen as the inflammation set in. The tendon may be tender to touch, and there may be some swelling. It is important to see a doctor or physio therapist for early treatment as you risk a permanent reduction or loss of function without judicial treatment.


One of the problems arising out of tendon injuries, long after the pain has gone, is a perceived fatigue. You may have badly sprained an ankle and found thereafter, that you roll the ankle for no good reason ie on perfectly flat ground. The likely cause of this is damage to the proprioceptors in the tendon which relay spatial information relating to the limit, to the brain, when these receptors are damaged, the brain does not know where your foot is in space, so that you may be putting your foot down on its side rather than the sole, since you keep rolling your ankle. Special exercises are required to re-program the proprioceptive cells to prevent the injury recur over and over again. A physiotherapist will be able to provide this information.

Tendonitis is inflammation of the tendon which usually arises out of excess friction of the tendon due to poor biomechanical function. This in turn may cause impinging of the tendon as is common in rotator cuff injuries of the shoulder. Pain and stiffness occurs on movement and there is usually restricted range of motion due to the pain. The tendon can feel tender or bruised to touch, and as with most inflammatory conditions, pain is worse after rest. This injury requires immediate attention, it will not self resolve. Stop any activity which exacerbates the pain and see a physiotherapist. They may refer you to a sports massage therapist for adjunctive treatment. Acupuncture can also be effective in treating tendonitis, but you should have the problem diagnosed by a doctor or physiotherapist.

Tenosynovitis is inflammation of the tendon sheath, occurring with or without tendonitis. The symptoms are similar to those of tendonitis, though the pain can be over a greater area. Again, seek immediate treatment from a physiotherapist. Massage is not usually appropriate. Acupuncture may be of benefit.


Bursa are fluid filled sacs which lie between tendons and bone and protect the tendon from excess friction between the tendon and bone. They are found around most joints. Bursitis is inflammation of the bursa due either to an impact or excess friction from a tendon. Pain occurs when pressure is placed on the bursa, from lying or leaning on it, or on movement as the tendon rubs on it. If the tendon and muscle are tight enough to result in bursitis, then the underlying cause is almost certainly a significant biomechanical discrepancy, and it is likely that you are experiencing muscular and joint pain in other areas. The treatment may well require a multi disciplinary approach involving a combination of physiotherapy or osteopathy, massage and possibly a podiatrist if the lower limb is affected. Rehabilitation is usually a lengthy process as the body has to go through a number of biomechanical changes for healing to occurrence. This problem will not self resolve.

Ligament and Joint Injury (sprain)

Ligaments attach bone to bone and provide stability of the joints. If the joint is overstretched ligament and cartilage may be damaged, and the joint may be misaligned. Sprains are painful on movement or weight bearing on the joint, and because of inflammation are likely to cause discomfort at rest. Often it is difficult to find a comfortable position. The pain is sharp and usually localized to the joint and corresponding muscles. The joint will feel weak and unstable. See a physiotherapist as you will need specific exercises to restructure the area as part of the treatment protocol. Ligament injuries are slow to heal and the resultant joint instability can give rise to other compensatory problems, for example a weak knee can lead to hip, back and neck problems. Massage as an adjunct to physiotherapy or any of the above modalities will hasten your recovery and help in preventing the development of secondary problems. If the joint can not be realigned through soft tissue therapy, then manipulation may be suggested. This should only be attempted by a fully qualified osteopath, chiropractor or physiotherapist. Do not let anyone else manipulate your joints, ever! It could lead to a stroke or permanent disability!

To sum up, If pain persists or continues to worsen beyond 3 days, if you have an injury that recurs every few weeks or months, if you experience any symptoms mentioned above on a regular basis, and they move around your body, seek advice

If in doubt see your doctor for advice on what to go to for treatment. They will normally recommend physiotherapy or massage, other options include chiropractic and osteopathic therapy.

Detecting Flat Head Syndrome or Plagiocephaly

There is a condition that causes a baby's head to become misshapen or have a flat spot and is known as plagiocephaly. The most common type is situational plagiocephaly and happens when a baby's head develops a flat spot because of pressure on that area. Babies' heads are soft when they are born so are vulnerable to this condition.

When babies are born and spend time in a position that puts pressure on one part of their skull, this is when the condition typically develops and is known as positional plagiocephaly.

The condition has increased dramatically since the early 1990s because this is when parents were told that putting babies on their backs to sleep would decrease the risk of Sudden Infant Death Syndrome. This advice has saved thousands of babies' lives.

Sometimes when movement in the uterus is restricted positional plagiocephaly can develop with the baby's head but is rarer and is usually because the mother has a multiple pregnancy or it can happen with breech babies who get wedged under their mother's ribs.

There is a birth defect known as craniosynostosis and is a type of plagiocephaly. It is when the joints between the bones of the baby's skull close too early. With this condition, surgery is needed to help the brain grow properly.

Detecting Plagiocephaly

With vaginal births it is common for babies to have misshapen heads because of the pressure to them when they pass through the birth canal. In most cases their heads naturally correct themselves to a normally rounded shape by six weeks. However, if there is a flat spot on your baby's head after six weeks of age then it is probably positional plagiocephaly.

Plagiocephaly is most common in babies who are premature, have weak muscle tone, have very large heads or are known as good sleepers.

Torticollis is a condition that can cause plagiocephaly. It is common in premature babies and is when they sleep with their head tipped to one side, while the chin is turned to the other and is caused by a tight or shortened muscle on one side of their neck.

If your baby develops a flat spot on their head

No one's head is completely symmetrical. In most cases slight flat spots will round themselves by the time the baby is crawling around and sitting up at six months of age but if you do notice your baby's head beginning to flatten it is advisable that you speak with your doctor straight away.

The younger the baby is the softer the skull is so treatment is best started sooner rather than later. As the baby grows the skull becomes harder and less pliable.

Treatment for positional plagiocephaly

Treatment is decided once your doctor has assessed the baby's age and the amount of the condition. If the condition is not severe and the baby is young enough then simple re-positional therapy may be administered. In more serious cases then cranial orthotic therapy may be determined and this means using a helmet to change the shape of the baby's head.

By contrast, St Paul's Cathedral is the fourth London cathedral to occupy its site. It was designed by the court architect Sir Christopher Wren and the construction was finished in 1710. A £ 40 million restoration scheme has been in place for some time now – and the new facilities will be a great way to celebrate the 300th anniversary of the cathedral in 2010!

For the moment, though, visitors can enjoy a program of regular worship and sermons. This is the Church of London, witness to famous events such as the funeral of Admiral Lord Nelson, the laying to rest of the Duke of Wellington and the funeral of Sir Winston Churchill have taken place across the centuries.

You may even see a service of remembrance or sometimes a royal commemoration or a magnificent wedding when you visit! Of course most visitors come to see the supreme dome of St Paul's Cathedral, one of the largest cathedral domes ever built, and the one which symbolises London's defiance in the face of Second World War fire bombs. It's an astounding 111.3 meters high.

St Paul's is built to the standard plan of a cross design, with the dome and the famous Whispering Gallery, high above the nave, at the dead center of the transept. Decorated with astounding mosaics, intricately carved stonework and amazing arches, St Paul's is a masterpiece of cathedral architecture.

You can enjoy a recorded audio tours at your leisure as you stroll around the building; they come in eight languages ​​and last for about 45 minutes. If you wish to know more about the building, you can book a one hour Triborium Tour which takes in the library, the nave, the Trophy Room – featuring Wren's plans – and the famous geometric staircase.
