Prostate Cancer is a disease caused by the mutation of cells in the prostate that begin to multiply at increasing rates. Prostate Cancer can develop at any age; although it is more common in older men and risks increase drastically with advancing age. In some men, the cancer can develop unnoticed and the men will die of other causes; though when done early, prostate cancer has a 90% cure rate. Signs / Symptoms

Men often do not know that they have prostate cancer until the later stages of the disease, unless they go in for regular screenings. This is due to prostate cancer, in the early stages, is asymptomatic (does not show symptoms). The first manifestations of the disease are often related to bladder obstruction, and are similar to the signs and symptoms seen in patients with BPH (see p. 18). Rectal obstruction can also occur, which causes problems with defecating during a bowel movement.

Screening / Diagnosis

Screening and diagnosis is the same for both BPH and prostate cancer. Procedures include the following:

* Prostate-specific antigen (PSA) blood tests: elevated levels of PSA in the blood may be an indication of cancer

* mental confusion (seen in several cases in which the cancer has spread to the brain)

Transrectal ultrasonography (TRUS): ultrasound examination of the testicles, prostate, and kidneys may rule out other disease or confirm BPH or cancer.


Treatment of prostate cancer varies depending on the stage of cancer, the effects of treatment, age, general health, and life expectancy of the individual. Men who are diagnosed and treated in the early stages of prostate cancer have a 90% chance of being completely cured of the disease.41 Although complete recovery is not as likely during the later stages of cancer, treatment can extend life expectancy, reduce tumor size , and reduce pain. Treatment options include both surgical and non-surgical procedures.

Surgical treatments

* Prostatectomy- the partial or complete removal of the prostate gland.

* Transurethral resection

* Cryotherapy- procedure in which the prostate gland is exposed to freezing temperatures. During the procedure, needles are inserted into the prostate gland through the area between the scrotum and anus. Once the needles are in place the needles produce freezing temperatures which destroy the prostate and all surrounding cancerous tissue. Note: Surgical treatment of prostate cancer can often result in a temporary loss of urinary control and / or sexual dysfunction. Medication can be prescribed by your doctor to help reduce these side effects.

Non-surgical treatments

* Radiation therapy- medical use of ionizing radiation to destroy the DNA of cancerous cells. Radiation is used to destroy or control the multiplication of malignant cells. Radiation is applied to the site of cancer (tumor, lymph nodes, etc.) and its surrounding tissue. This treatment is often used in conjunction with Hormone or Chemotherapy.

* Hormone therapy- Hormone therapy involves the administration of specific hormones, such as steroids, or drugs which inhibit the production or activity of other hormones involved in the development of the cancer. The agents used in hormone therapy are designed to alter gene expression of the cancerous cells, causing cell activity to cease, or by promoting cell death.

* Chemotherapy- Chemotherapy is the use of chemicals and antibiotics to kill cancer cells. Cancer is caused by the uncontrollable division of cells. The chemicals used during chemotherapy are designed to target rapidly-dividing cells, thus targeting the cancer cells and not other healthy cells of the body.

* Immunotherapy- Immunotherapy is a reliably new treatment that stimulates the body's own immune-system to kill off tumors.

How to Stop Your Puppy From Jumping On Everybody

It was not that long ago our Chessie / Golden mix, Sonagh, would go into a state of complete apoplexy with the arrival of guests. She would be completely out of her mind. . . to the point where I could not open the door. Well today, I can report both of my dogs stay in their sit position and are perfectly quiet when anyone arrives at the door.

This article will show you what you need to do to stop your dog from jumping up on everyone who enters your house.

And here's the good news: Your problem is solvable, because dogs are very trainable. That behavior just needs to be modified into acceptable house manners.

So the first thing you want to do, if you do not have one, is get your dog a Gentle Leader. You can find one at your local vet office or pet supply store.

Now here's how the training starts: Before company comes, take your dog for a good, vigilous, half hour walk, making sure they stay in a heel the whole time. You want to be in charge of the walk, not your dog. No sniffing and exploring on this trip. This does two things.

1 – It uses up some of your dog's energy

2 – It establishes your leadership

Next, when company comes, pick a spot you want to establish as your dog's "company spot." This can be 4-5 feet back from the front door. Next, have your dog in their Gentle Leader with the leash clipped to it. Then put them in a sit / stay and step on the leash so your dog can not get their head any higher than a normal sit position.

Praise them for sitting nicely and give them a special treat you use ONLY when guests are coming. Try some dried liver bits – disgusting to us but candy for dogs. Now have someone else answer the door while you continue to manage your dog. Keep them focused on the trips, their sit / stay and your praise.

You may even want to develop a command for this activity. If you tell your dog "Company!" they will eventually associate their "company spot" and the desired behavior with this command.

It's just that simple.

Now I'll be honest, you will need to practice this quite a bit before you can expect a calm dog when people come into the house. So make people coming and going from your home a regular thing rather than a special event.

I think one of the other factors is how your guests react to your dog. You can help coach them so they can change their behavior. This will help with your dog's behavior, because their anxiety over the bouncing, jumping dog is being delivered to your dog and she is, in turn, reacting to it.

Otitis Media – What? Why? And How?

The ear, in the world of medicine, is not just the ear that we commonly refer to. The ear is quite a complex structure and consist of 3 parts: the outer ear, the middle ear and the inner ear. Otitis media, as the name suggests, refers to infection in the middle ear.

How does otitis media occur? The middle ear, which is the space behind the eardrum, is normally filled with air. A tube usually connects the middle ear to the throat. This tube is called auditory tube or the Eustachian tube (Eustachian was a chap who discovered the tube hundreds of years ago). The function of the tube is to normalize or equalize the pressure between the middle ear and the atmosphere. When blocked, pressure in the middle ear decreases, causing mucus (or fluid) to fill up the space. The mucus may then become contaminated by bacteria and viruses, thus allowing infection to occur.

This is the basic mechanism of middle ear infection. There are many reasons for the Eustachian tube to become blocked. One of the commonest being an upper respiratory infection or common cold, which causes the Eustachian tube to swell up and block. This is particularly common in children as their Eustachian tubes are smaller and not yet fully developed. The other less common cause being a tumor growing around the opening of the Eustachian tube in the throat, causing the tube to obstruct. This is particularly common in the middle age or the elderly population, where tumor of the throat is prevalent. Having said that, very often, an ear infection occurs 'out of the blue' for no apparent reason.

What are the symptoms of a middle ear infection? Earache is the most common complaints. It is often described as a deep ache or pressure, sometimes bad enough to disrupt sleep. Dulled hearing and deafness are also common symptoms; they usually usually last for several days. Fever is also common, but not very 'specific' as other infections can also cause fever. Young children with middle ear infections may also vomit and tend to look very ill. Babies can not point to their pain but if they are irritable, feel hot and crying all the time, a middle ear infection should always be suspected. Discharge is a common symptom too. As the infection progresses, pressure begins to build up in the middle ear. When it is too high, the eardrum 'pops', allowing its content (usually pus) to flow out from the middle ear.

Are all earaches caused by middle ear infection? Absolutely not! Lots of other diseases can cause earaches as well. For example, people with toothache can sometimes have earaches, this is due to pain being 'referred' to the middle ear. Other causes of earache due to referred pain include sinusitis (inflammation of the sinus), sore throat, tongue ulcers, thyroid problems, outer ear infections, so on and so forth.

What is the treatment for a middle ear infection? The aim of treatment is to kill the bugs responsible, to control the pain and to control the fever (to bring the high temperature down to normal). Antibiotics are commonly given to kill the germs responsible, the commonest being Amoxicillin. Pain killers (or analgesics) are given to control the pain, panadol is usually sufficient but some people may need stronger pain killers. Lastly, panadol can also do the job in lowering the high temperature as well. The course of the disease is usually between 7 to 14 days.

Childhood Hearing Loss

Preschool development of language and other skills is large dependent on the child having normal hearing. Conductive hearing loss and sensori- neural deafness are the two types of hearing loss that can occur in children.

"Nerve deafness" also known as sensori-neural deafness is caused by the inner ear not processing the sound stimuli it receives properly. Children with this type of sensori-neural hearing impairment often have to wear a hearing aid to improve their hearing. This type of hearing injury does not occur very often.

The other type of deafness ie conductive hearing loss is much more common. It is different from sensori-neural hearing injury in that there is no problem with the functioning of the inner ear but the construction of sound from the environment to the inner ear is impaired due to the presence of fluid in the middle ear. This condition is also known as "glue ear". Ideally, the middle ear should be air-filled for sound construction. The presence of fluid impairs the functioning of the middle ear causing an intermittent hearing impirement which is often seasonal in nature.

Conductive deafness is often inconsistent. Sometimes the child with this condition hears well and sometimes the build-up of fluid eg. when the child has an ear infection causing the hearing threshold to be significantly reduced. This type of hearing impairment is also seasonal as children are more likely to have colds and upper respiratory tract infections in the winter months.

It is worth pointing out that fluid can also occur in the middle ear, causing conductive deafness, in the absence of an accompaniment ear infection. Sometimes there may be a build-up of middle ear fluid in the absence of an accompaniment ear infection.

The position of the eustachian tube in the preschool child is a significant factor in the development of aductive hearing impairment. This tube connects the ear with the throat to allow an equilibrium of air pressure in the ear and to allow any fluid present to drain away easily. However, it does not work very well in youngger children because it is nearly horizontal. As the child grows, the eustachian tube has a more vertical position between the ear and the throat, resulting in better drain of middle ear fluid in the older child than in a pre-schooler.

Conductive deafness is common in preschoolers because of the position of the eustachian tube in this age group.

There is a surgical procedure to correct conductive hearing loss if it is chronic in the young child. "Grommets" can be inserted surgically via the eardrum allowing drainage of middle ear fluid but most children with conductive hearing impairment do not need this procedure. Conductive hearing loss often resolves by itself in children who present with it and the simple surgical procedure of grommet insertion or myringotomy as it is known is an option for those children in whom the deafness is not resolving itself.

It is a particular type which is done to test for breast cancer in women. The image that is produced is known as mammogram. Tumors that cannot be felt by hands are shown by these images. It is an operation to remove tumor from the body. It is one of the most used and oldest forms of treatment. It can also be used for different purpose like diagnosing cancer, location of cancer and the extent to which it has spread. It can be used alone or in conjunction with other techniques like chemotherapy and radiation therapy.

In some cases, a surgeon is not able to remove the whole tumor as it may cause damage to the body. In these cases, debulking is done. As much of the tumor is removed from the body and the remaining tumor is removed with the help of other methods.

3. Radiation Therapy

Cancer cells are destroyed with the help of high energy x rays and other particles. The main aim of this treatment is to slow the growth of cancer cells and kill them without harming the nearby healthy tissue. More than half the people suffering from cancer receive radiation therapy at least once.

There are two ways of providing it. One is by external beam radiation in which radiation is delivered from outside the machine. It can treat large areas of the body too. Other method is internal radiation therapy.

Achievement Towards Elimination Of Leprosy Rate In India


Leprosy is a chronic granulomatous infection. It is a major health problem in India which has 1/3 of total leprosy patients in the world. It is more prevalent among the lowest socio-economic strata. World Health Assembly in 1991 passed a resolution to eliminate leprosy as a public health problem by the year 2000. Elimination of leprosy as a public health problem is defined as a priority rate of less than one case per 10 000 persons; The target was achieved on time. The widespread use of MDT by WHO has reduced the disease burden dramatically; Over the past 20 years, more than 14 million leprosy patients have been cured about 4 million since 2000.




Leprosy was recognized in the ancient civilizations of China, Egypt and India. The first known written mention of leprosy is dated 600 BC. The word leper comes from a Greek word meaning scaly. In India, leprosy is known since ancient times as kustha roga and attributed to punishment or curse from God. Leprosy is a chronic disease caused by a bacillus, Mycobacterium leprae; M. leprae multiplies very slowly and the incubation period of the disease is about five years.


Leprosy bacilli mainly affect the skin and peripheral nerves; if untreated, there can be progressive and permanent damage to the skin, nerves, limbs and eyes. Injuries to these insensitive parts may lead to disfigurement, the main consequence of this disease which generates fear and stigma.

Thus early detection and prompt treatment of leprosy with prescribed multi drug therapy (MDT) not only cures leprosy but also interrupts its transmissions to others. It is transmitted via droplets, from the nose and mouth, during close and frequent contacts with untreated cases.
This chronic infectious disease usually affects the skin and peripheral nerves but has a wide range of possible clinical manifestations. Patients are classified as having paucibacillary (few bacillus) or multibacillary Hansen's disease:

"Paucibacillary (PB) Hansen's disease is milder and characterized by one or more hypopigmented skin macules.

"Multibacillary (MB) Hansen's disease is associated with symmetric skin lesions, nodules, plaques, thickened dermis, and frequent involvment of the nasal mucosa resulting in nasal congestion and epistaxis.


Leprosy may be transmitted via aerosols containing M.leprae (droplet infection). Numerous studies indicate that it is tranmitted from person-to-person by close contact between an infectious patient and a healthy but suspect person. This contact may be direct (eg, skin-to-skin) or indirect (eg, contact with soil, and fomites such as contaminated clothes and linen). It may also be transmitted via breast milk from lepromatous mothers, by insect vectors, or by tattooing needles.


Leprosy is a curable disease and treatment provided in the early stages averts disability; with minimal training, leprosy can be easily diagnosed on clinical signs alone; A World Health Organization (WHO) Study Group recommended multidrug therapy (MDT) in 1981. MDT consistants of three drugs: dapsone, rifampicin and clofazimine.

This drug combination kills the pathogen and cures the patient; MDT is safe, effective and easily administrated under field conditions. MDT has been highly successful both in MB and PB. By april'94 a total of 5.6 million cases world over were cured by MDT. The estimated cases of leprosy fell from 10-12 million to 2.7 million.

MDT is available in convenient monthly calendar blister packs to all patients; Since 1995, WHO provides free MDT for all patients in the world, initially through the drug fund provided by the Nippon Foundation and since 2000, through the MDT donation provided by Novartis and the Novartis Foundation for Sustainable Development.


PB patients treated with MDT are cured within six months; MB patients treated with MDT are cured within 12 months; Patients are no longer infectious to others after the first dose of MDT. In other words, transmission of leprosy is interrupted; There are virtually no relapses, ie recurrences of the disease after treatment is completed; No resistance of the bacillus to MDT has been detected; WHO estimates that early detection and treatment with MDT has preceded about four million people from being disabled. This suggests great cost-effectiveness of MDT as a health intervention, considering the economic and social loss averted.


Leprosy was considered till 1980 as a perennial problem with no solution in sight, the introduction of multi-drug therapy (MDT) in the last twenty years has resulted in massive progress towards conquering the disease. The leadership provided by WHO together with the strong commitment of leprosy endemic countries and the support of Non-Govermental Organizations (NGO) and donor agencies have greatly contributed to the reduction of the global burden of leprosy by nearly 95% and the elimination of the disease as a public health problem in over 120 countries. The latest available information on the leprosy situation indicates that the bulk of the problem continues to be in Asia which with about 198,000 cases contributes to a share of 60% of the global burden, while the contribution of Africa and Americas is only 19% and 15 % respectively. In the Americas the only country yet to reach leprosy elimination is Brazil with a prevalence rate of 1.77 per 10,000. According to the latest available information, intensive efforts are still needed to reach the leprosy elimination target in five countries: Brazil, India, Madagascar, Mozambique, and Nepal.


The prevalence rate in India, which traditionally accounted for the highest burden of leprosy, globally and regionally, has declined from 5.9 per 10,000 in 1996 to 0.8 per 10,000 population in 2006. The new case detection has declined from a peak of 89 / 100,000 in 1999 to 14.27 / 100,000 in the year 2006.

While the member states should receive primary credit for the above achievements, WHO technical support and financial assistance to some critical activities has been a strong contributor factor, along with assistance from partner agencies like World Bank, The Nippon and Sasakawa Foundations of Japan and the International Federation of Anti-Leprosy Associations (ILEP).
Another major contributing factor has been the free supply of leprosy drugs to all endemic countries from 1995 to 2000 by The Nippon Foundation and from 2000 onwards by the Novartis Foundation for Sustainable Development. Novartis has assured free supply of leprosy drugs until 2010.


The goal is to achieve elimination of leprosy as a public health problem everywhere in the world including India. Elimination of leprosy aims at reducing the disease burden to very low levels so that after reaching such low levels the disease will slowly disappear over a period of time. This very low level has been defined by WHO as a level of prevalence of less than one case per 10,000 population. On 30th January, 2006, the Ministry of Health, Government of India formally announced that India achieved the elimination target (leprosyvalence was 0.8 per 10,000).
The strategy and implementation of leprosy elimination in India is proving to be a huge success story. However there are still some challenges to be addressed specifically in the area of ​​rehabilitation of leprosy affected persons.
