I was as shocked as anyone by the recent sudden death of movie star Heath Ledger and revelations of his struggle with insomnia.

Here was a strong, fit 28-year-old man in the prime of life found dead in his apartment surrounded by sleeping pills and anti-anxiety drugs.

I’ll leave it for the experts to determine exactly what caused Ledger’s tragic, tragic death, but it’s clear the Oscar-nominated star of Brokeback Mountain was under enormous personal and career stress and on his own admission he wasn’t sleeping well.

Consider the past six months of Ledger’s life: He parted company with his Brokeback Mountain co-star Michelle Williams, the mother of his two-year-old daughter Matilda, and said his roles as the Joker in The Dark Knight and as Bob Dylan in I’m Not There had left him “stressed out a little too much”.

In an interview in November with The New York Times, Ledger admitted: “Last week I probably slept an average of two hours a night.

“I couldn’t stop thinking. My body was exhausted, and my mind was still going.”

Just before his death in January, Ledger had flown back to New York from London, where he’d been filming

yet another movie, Terry Gilliam’s fantasy The Imaginarium of Doctor Parnassus.

Police have said there were no illegal drugs in Ledger’s apartment and they’ve been focusing on a possible accidental overdose of the prescription drugs he was taking to calm his anxiety and help him get some badly needed sleep.

On Feb 4, the Los Angeles Times reported that another well known actor, Justin Chambers, 37, (who plays Dr. Alex Karev on “Grey’s Anatomy”) had voluntarily checked himself into UCLA Medical Center recently “to get help with a pre-existing sleeping disorder,” according to his publicist, Danica Smith.

Given the stress and anxiety of performing in movies and TV, I’m not surprised insomnia and sleeping disorders are part of the price of fame. In fact, stress and anxiety is one of the seven triggers of insomnia I’ve identified in my e-book Banish Insomnia, which offers a cure for insomnia without drugs.

How the Pancreas Works

The pancreas has two functions:

a. Exocrine gland – Glands that discharge hormones into ducts

b. Endocrine gland – Glands that secrete hormones into the bloodstream

The pancreatic endocrine function is to release the hormones insulin and glucagon into the bloodstream to regulate glucose levels.

The pancreatic exocrine function releases digestive enzymes into the duodenum (first, connected to the stomach, and smallest part of the small intestine).

The pancreas is located beneath the stomach and the pancreatic head is wrapped around by the duodenum.

Bile, an alkaline fluid that helps digest lipids, is transmitted from the gall bladder to the pancreas through the common bile duct connecting to the pancreas at the pancreatic duct or duct of Wirsung. The pancreatic duct collects this alkaline fluid and digestive juices: trypsin, chymotrypsin, pancreatic lipase, and pancreatic amylase, and empties into the duodenum just prior to the ampulla of Vater. The ampulla of Vater regulates the flow of bile and pancreatic juices into the duodenum.

A smaller duct (duct of Santorini) also empties into the duodenum. Enzymes active in the digestion of carbohydrate, fat, and protein flow through these ducts continuously and at an increased rate when food is present in the small intestine. About 41 fluid ounces of digestive juices are secreted daily. Flow is controlled by the vagus nerve and by the hormones secretin and pancreozymin, produced in the intestinal mucosa.

When food enters the duodenum, secretin and pancreozymin (cholecystokinin/pancreozymin hormone) are released in the bloodstream by the duodenum secretary cells. When these hormones reach the pancreas, the pancreas cells are stimulated to produce and release large amounts of digestive enzymes, which then flow into the intestine. When the enzymes reach the intestine, the duodenum stops producing secretin and pancreozymin hormones.

The pancreas cells that produce digestive enzymes are called acinar cells (Latin acinus – grape), so named because the cells aggregate to form bundles appearing much like a cluster of grapes. The base of the cell has fine tubules (endoplasmic reticulum) that produce the secretary granules. These granules are carried from the tubules to a more complex tubular apparatus (Golgi) that concentrate the secretions. The Golgi encases the enzymes in a thin membrane and sends them to the top of the cell. The enzyme granules pass through the acinar cell wall into a complicated network of ducts that eventually reaches the small intestine.

Located between the clusters of the acinar cell area are scattered patches of another type of secretary tissue called the islands (islets) of Langerhans, named after the 17th century German pathologist Paul Langerhans. The islets are responsible for the secretion of insulin and glucagon, which control the amount of sugar stored in the body. Unlike the acinar tissue, the islets of Langerhans secrete their substances directly into the blood stream.

Insulin is vital in carbohydrate (sugar) metabolism. Insulin causes the body to store glucose as glycogen. Insulin stimulates the body’s cells to remove glucose sugar from the blood stream and utilize it. Diabetes mellitus is caused when there is a shortage of insulin in the bloodstream. Sugar then remains in the blood and glucose is not converted into glycogen stores for the body. Too much insulin reduces the amount of sugar in the blood.

Glucagon has the opposite effects of insulin. Glucagon is also a hormone used for carbohydrate metabolism. Glucagon is released into the bloodstream when the glucose levels are low (hypoglycemia) causing the liver to convert glycogen to glucose in the bloodstream Glucagon releases stored sugar and increase the blood sugar level. Glucagon acts as a control mechanism when the body produces too much insulin.

Weakness in the legs occurs as a result of a decrease or loss of strength in any of the muscles or muscle groups involved in the movement of the legs. The most common causes of leg weakness are due to spinal cord/nerve compression or injury in the region of the lower back and include traumatic spinal cord injuries, scoliosis, intervertebral disc prolapse, nerve impingement, tumours, infections, and so on. Skeletal muscle movement is controlled by nerves originating from the brain but these nerves are not continuous all the way to the muscles.

There is, normally, a relay in the spinal cord, where the nerves from the brain communicate with the nerves that actually reach the muscles. For the leg muscles, this nerve relay occurs in the lower section of the spinal cord in the lower back, so problems affecting the spinal cord and nerves in the region of the lower back can cause weakness in the leg muscles. These nerves, however, do not only control movement but also carry fibres that control sensation, urination, defecation and erection (in males), so it is not unusual for leg weakness to be associated with problems with these other functions.

Other causes of muscle weakness include stroke, meningitis, polio, diabetic neuropathy, multiple sclerosis, polymyositis, dermatomyositis, amyotrophic lateral sclerosis, electrolyte imbalance, metabolic disorders, nutritional deficiencies, autoimmune disorders, malignancies, toxins (e.g. Botulism), poisoning (e.g. organophosphates), connective tissue disorders, myasthenia gravis, thyrotoxicosis, drugs and so on (the list is endless really). Most of these other possible causes, however, will cause a more generalized muscle weakness, rather than weakness restricted to the legs alone. Stroke, however, tends to cause muscle weakness on one half of the body, rather than affecting both legs, for instance it could cause weakness on the right arm and leg with weakness on the left side of the face or weakness on the left arm and leg with weakness on the right side of the face.

Treatment of leg weakness, obviously, depends on the cause, but may include medications, surgery, physiotherapy and other specific and supportive therapies. However, please note that nerves, unlike most tissues in the body, have a very low capacity to heal. So if the leg weakness is due to nerve damage and depending on the extent of the damage, the individual may not be able to regain full strength in the legs. I’m not, in any way, implying that the individual doesn’t need treatment by this please, quite the contrary. The aim of treatment may be to halt or slow down the progress of whatever is causing the problem, and also to prevent complications that may arise from inactivity and disuse of the legs. On a more optimistic note, however, in some cases where the weakness is caused by spinal cord or nerve compression, surgery aimed at relieving the compression coupled with physiotherapy may restore full strength; but note that surgery has its own risks as well, including the possibility of causing complete paralysis.

Colon Cancer Polyps

Often, colon cancer begins as a small cluster of cells known as “colon polyps.” While benign at first, some of these polyps may become cancerous over time. There are many important risk factors for cancer of the colon that makes someone a good candidate for colon cancer screening. Approximately 5% of all colon cancer is caused by a genetic syndrome passed through the familial line. These syndromes include FAP (familial adenomatous polyposis) and Lynch syndrome (hereditary nonpolyposis colorectal cancer). About 95% of the people diagnosed with either syndrome will develop colon polyps that lead to cancer. The good news is that both of these syndromes are detectable through genetic testing. The idea that someone without these syndromes will develop the cancer because an aunt, grandmother, sibling or parent has is still debated. Some say the family may have all been exposed to the same environmental conditions or unhealthy lifestyle.

There are many screening procedures for colon cancer. The most basic is a stool blood test (or a fecal occult blood test), which allows a patient to take a kit home, create a sample and return that sample to the lab, where a doctor will examine the results under a microscope. Similarly, a stool DNA test will send the sample kit off to the laboratory for more in-depth analysis, which may be able to indicate DNA mutations or tumors that indicate the presence of cancer. A flexible sigmoidoscopy takes just a few moments, and involves a flexible, slender, lighted tube being inserted into the last two feet of the rectum and colon to see if any colon polyps are present. A barium enema uses a contrast dye and x-rays to evaluate the lining of the bowels. A colonoscopy is similar to the sigmoidoscopy, but the instrument allows the doctor the ability to search the entire colon and rectum for polyps, rather than just the lower portion. Lastly, a virtual colonoscopy uses a computerized tomography machine to take images of the colon, which is a less invasive than a conventional colonoscopy screening.
