When your amygdala receives this data it accepts that you are in a safe environment and resets your anxiety level back down to a normal, appropriate level. Ideally you need to get back to an anxiety preset, determined at birth, the bench mark for your anxiety level through your life.

Home Health Coding – Complicated Surgical Wounds

If a patient’s surgical wound may be complicated, but the coding need not be. Heed these three simple steps to ensure the accurately reporting all the care your agency provides.

1. Know when it is complicated:

A complication is a problem that takes place during the healing process of the initial surgical procedure which impacts healing negatively, says Judy Adams, RN, BSN, HCS-D, COS-C, president and CEO of Adams Home Care Consulting in Chapel Hill, N.C. For instance, an infected surgical wound is thought of as complicated.

Tip: According to Adam, there is no time limit to coding a surgical wound as complicated. Even if the complication takes place sometime after surgery, you can still code for the wound as complicated.

2. Get to the right code

See under “complication” in the alphabetic index of your coding manual to start your search, says Adams. After this, verify the code in the tabular list, and follow the directions for any additional codes. Adam points out, there’re many complication codes for wounds of all types in chapter 17 at 996.xx (Complications peculiar to certain specified procedures), 997.6x (Amputation stump complication), 998.3x (Disruption of wound), and 998.5x (Postoperative infection) for postoperative wound complications. Moreover, look to 998.83 (Non-healing surgical wound) and 997.xx (Complications affecting specified body systems, not elsewhere classified).

3. Do not turn to aftercare:

Although your agency is providing care post-surgery, you should not use an aftercare V code when the surgical wound is complicated. In this case, the complication code trumps the aftercare V code in this case. As per coding guidelines, “The aftercare V code shouldn’t be used if treatment is directed at a present, acute disease or injury. The diagnosis code should be used in these cases.”

But you may once in a while find an exception to this rule. For instance, if your agency is caring for an infected surgical wound which requires IV care, you would make use of the right complication codes to report the wound, but you could still list V58.81 (Fitting and adjustment of vascular catheter) and V58.62 (Long-term (current use of antibiotics).

Another instance: Adams says that instructional notes at 996.4x (Mechanical complication of internal orthopedic device, implant, and graft) and 996.66 (Infection and inflammatory reaction due to internal joint prosthesis) counsels coders to “use additional code to identify the prosthetic joint with mechanical complication or infection (V43.60-V43.69).”

A patient of yours has come home after a coronary artery bypass graft (CABG) for coronary atherosclerosis (CAD) of his native artery. He needs constant observation and assessment of the surgical incision on his leg with three areas of incisional separation but with no signs of infection. He’s a bit weak but does not have chest pain anymore. The patient has diabetes with fasting blood sugar of 109 and diabetic peripheral angiopathy. He also has benign prostatic hyperplasia with urinary retention and frequent urinary tract infections and will be planned for a transurethral resection of the prostrate pretty soon. Your orders are meant for skilled nursing and physical therapy coding.

The Influence of the Duodenum on Health

This is the third article in a five part series devoted to digestion where we have been following a nutritionally poor meal, typical of America, from the mouth to the esophagus to the stomach. We have seen that the food enters the mouth where enzymes are added via sublingual glands. The meal then moves through the esophagus where it is warmed, and then to the stomach where pre-digestion takes place depending on the nutritional value of the meal (does the meal have enzymes to digest protein, fats and carbohydrates).   Then stomach acid and a strong protein digesting enzyme are added to complete the stomachs processing of this meal.   Now we will follow this nutritionally poor meal from the stomach into the duodenum.

The Function of the Duodenum

The chyme (digested food from stomach the consistency of a milk shake) is slowly transported from the pylorus (a valve at the end portion of the stomach) through the pyloric sphincter into the first part of the small intestine called the duodenum where further digestion occurs.  

The inside wall of the duodenum has a smooth surface like the skin on your face.   It is at this point that the acidic chyme must be alkalized so the assimilation of this nutrient filled liquid is as complete as possible.    In addition, this alkalization is very important so that the acid from the stomach is neutralized and will not burn the walls of the small intestine. 

The duodenum, an eight to nine inch long chamber is where more chemicals of digestion are added.    The Ampule of Papilla is located in the duodenum wall.    This is an opening where bile from the gallbladder, and enzymes and bicarbonate from the pancreas enter the duodenum.    They combine with the chyme to begin the final part of digestion in the small intestines.    Enzymes are also excreted through the walls of the duodenum to assist with this digestion. 

The Average American Meal and what happens at the Duodenum

Your body is aware of what you have eaten (there are sensors in the duodenum) and it calls on your liver, gallbladder, pancreas and other avenues of digestive chemicals to assist in the breakdown and assimilation of nutrients and protection of the digestive tract.   Your body expects you to bring in live food rich in enzymes (uncooked foods such as produce) to assist in this digestive process. 

Here at the duodenum is where the body has a chance to recover from any mistakes made at your mouth.   But without enough raw food to supply the enzymes and alkaline that was needed to create a balanced meal no recovery will take place.   A meal totally unprepared will continue through the duodenum to the small intestine where impossible assimilation is undertaken.   From this point on the damage to your body takes place over and over bringing disease.

If you do not bring in enough live enzymes with the food you have eaten you may expect to have acid indigestion (undigested food), acid reflux, ulcers and constipation all caused by food improperly prepared at your mouth (no live enzymes).    As this improperly prepared food enters the duodenum, your digestive awareness will call on the pancreas and gallbladder to do the job you did not do.   Your pancreas and gallbladder will then strain to prepare this acidic chyme for release into the small intestine.  Allow this to happen enough times and you will get sick.

Constantly releasing acidic unprepared chyme from the duodenum to the small intestine, can lead to such diseases as Crohn’s disease, inflammatory bowel disease and leaky gut.  These in turn can lead to diabetes, heart disease, cancer, and autoimmune diseases such as rheumatoid arthritis, muscular dystrophy, etc.

Colon Cancer Stages – Colon Cancer Survival Rate

When someone has experienced having inflammatory bowel disease, there is a huge possibility that the person will have an increased risk percentage of having cancer of the colon. While cancer of the colon is one of the easiest cancer types to be prevented from occurring, colon cancer stages are very decisive to identify because they are most often diagnosed when the symptoms are already inconvenient and bothersome. Colon cancer survival rate is lower when one is diagnosed in the later stage.

Colon cancer stages are divided into four unique stages. Each of these stages varies in colon cancer survival rate. The earliest stage of cancer of the colon is called stage 0. During this stage, the malignant cancer cells are just in the lining or mucosa of the rectum and confined in the polyp. Polypectomy is done in order to remove the cancer cells so that they would not progress on the later stages.

The Stage 1 cancer of the colon occurs when the polyp that contains the cancer cells have evolved and became a tumor that extends to the colon or rectum walls. During this stage, the advisable treatment would have to be the removal of the section of the colon that is considered to be infected by the malignant cancer cells. The five – year colon cancer survival rate for this stage is at 95% chance of cure.

Stage 2 colorectal cancer happens when the cancer tumor has spread and has affected the tissues surrounding the colon. Resection surgery is given to the patient in order to treat them of their current ordeal. Survival rates during this stage are very unfortunate; the statistics is just 60 percent chance of cure.

Stage 3 cancer of the colon is when the malignant cells have spread outside the tissues or the colon and have infected the lymph nodes surrounding the colon as well. The Treatments given to patients at this stage are aggressive in order to cope up with spread of the malignant cancer cells. Aside from the surgical operation, chemotherapy and radiation therapy may be given to the patient in order to extend their life and be cured. Rate of survival during this stage drops down to 35 percent.

Stage 4 cancer of the colon occurs when the cancer cells have expanded and infected already the different systems of the body. Aside from the surgical resection and chemotherapy, radiation treatment and surgery is also done to the other infected parts of the body. Cancer of the colon during this stage drops down in a very poor 3 percent survival rate.
