What is a Jones Fracture?

Metatarsal fractures are referred to as long bone fractures. As a long bone, treatment for these types of fractures varies depending on the location of the fracture and the extent of the damage both to the bone and to the neurovascular supply to that bone.

Treatment of these fractures should be individualized, based on the patients health status, ambulatory needs, and clinical presentation.

Fractures of the fifth metatarsal usually are treated similarly to other metatarsal fractures. Due to the increased weight bearing load of the first and fifth metatarsal bones, extra care should be taken to ensure adequate alignment, compression of the fracture fragment (s) and immobilization to allow proper bone healing.

There are basically four (4) types of 5th metatarsal fractures. Head & Neck fractures, Midshaft, Avulsion, and a unique type of fracture known as a “Jones Fracture”.

A Jones Fracture is a break in the fifth metatarsal that occurs in-between the mid shaft portion of the bone and the posterior or base of the metatarsal. This is not to be confused with an “Avulsion Fracture”, where a small piece of bone breaks off due to the pull of a ligament or tendon. This type of injury usually is caused by “inversion” or rolling in of the ankle.

The Torg classification system is commonly used by physicians to determine proper treatment based on radiographic findings. Due to the high rate of nonunions in these type of fractures (caused by the disruption of the blood supply to the metatarsal fracture site), this classification system allows proper treatment based on the severity of the fracture line.

The Torg system is divided into 3 types of fractures. Type I and II usually are treated conservatively with a non-weightbearing cam walker or cast for 6-8 weeks. This is followed up with gradual weight bearing in a tall or short walking boot. Some Type II fractures may require surgery depending on the patient and activity level (athlete). Type III fractures usually are displaced fractures (separated) and should be treated surgically with fixation devices to hold the two bones together. This is followed up with a non-weight bearing cast or cam walker.

Follow up x-rays are usually taken after the fracture to determine the healing stage of the fracture site.

Whatever treatment options are indicated by your physician, it is important that the patient follow instructions to the letter. Non-unions are very common with this type of fracture and may result in months or years of pain, swelling and disability.

Principle of Working

The action of fluid transfer is achieved by placing an oscillating, nutating, or rotating object in the fluid path so that it measures the exact quantity of the displacing fluid. One of the most popular nutating disc type flow meters in the industry technology is the Positive Displacement. These devices repeatedly entrap the fluid in the system in order to measure its overall flow. The entrapment is typically achieved by using rotating parts that constitute the moving seals between one another and/or the flow meter body. The rotation in the system can either be detected mechanically or by detecting the overall movement of a rotating part. However, it is naturally understood that if more fluid flows through the system, the rotating parts turn faster. Furthermore, a transmitter present in the meter processes the signal to determine the flow of the fluid.

Many PD meters are designed differently, wherein, the rotating parts consists of tight tolerances so that the seals can prevent the fluid from entering the meter before it is measured. A combination of local applications like Vishgarbha-Oil, Maha-Narayan-Oil, Maha-Mash-Oil and Maha-Saindhavadi-Oil can be used both to control the pain and to treat nerve damage. Kumari (Aloe vera), Yashtimadhuk, Manjishtha (Rubia cordifolia), Saariva (Hemidesmus indicus) and Chandan (Santalum album) can also be mixed with Shata-Dhout-Ghrut and used as a soothing ointment.

What can I do?

You do not need to do anything about pits, dimples, or hair tufts other than to bring them to the attention of your paediatrician. The area should be kept clean with regular bathing. No special care is required for a normal pit, dimple, or tuff.

When does my doctor need to be involved?

If the base of the pit is visible when the surrounding skin is gently pulled aside, then the pit is shallow and you should not worry. If the base of the pit is not visible, or if there is a tuft of hair arising only from the pit – rather than a patch of hair generally distributed in the region of the lower back – then your doctor needs to be involved. You also need to contact your doctor if the area has a thick tuft of hair sprouting from a relatively small patch of skin, or if the irregularity is in the form of a skin tag, skin swelling, or an unusual mark.

If a pit or dimple looks red and irritated, swollen, or tender, then it could be infected. Clear or yellowish fluid draining from the pit may be the fluid normally surrounding the brain and spinal cord. White fluid may be pus, signalling an infection. Your doctor should see your baby if he has any of these problems.

Most pits, dimples, and tufts are at the base of the spine, just above (or even tucked within) the crease of the buttocks. When a pit is visible much higher on the back, a doctor should be involved. Your doctor should also evaluate the pit if it is off to one side (not in the midline) or if there are other skin abnormalities in the general area.

Finally, when a child has any other visible defects, especially along the midline of the body, bring them to the attention of your doctor. The midline of the body is the imaginary line that connects the belly button up to the nose, travels up and over the head to the middle of the back of the neck, and then goes down to the crease in the buttocks. This “line” separates right from left.

Structures on either side should be fairly symmetric, and structures along the line should be intact. Some examples of midline defects include cleft palate and hypospadias.

What tests need to be done, and what do the results mean?

Most sacral dimples, pits, or tufts are normal, so testing is usually not necessary. If a test is done, then an ultrasound is typically the first choice. The ultrasound is placed on the baby’s lower back so that a doctor can see whether or not the spinal canal and the skin are connected in any way. If the ultrasound is too difficult to interpret, or if the baby is older than 6 to 12 months, then an MRI may be done.

The ultrasound or MRI can help to show whether or not there is a direct connection between the skin along the lower back and the spinal column below. An MRI can also illustrate the structure of the spinal cord. A normal result shows no connection between the spinal column and any surrounding structures.

What are the treatments?

A child with a connection between a sacral pit, dimple, or tuft and the underlying spinal cord requires surgery. Eventually this connection can limit spinal cord growth, damaging the nerves of the cord. In some cases, it can also lead to infection of the spinal cord or the fluid surrounding the spinal cord (meningitis).

If the skin around the pit is infected, then antibiotics are usually necessary. These must be taken by mouth; they cannot be applied to the site of the pit. If there is a large amount of fluid or pus, then drainage may be required. If the spinal cord or the cord’s surrounding fluid becomes infected, then intravenous antibiotics will likely be needed.

What are the possible complications?

When the area looks red or irritated, it could be infected. Infection may just involve the skin, or it may involve the deeper tissues, including muscles, nerves, the fluid around the spinal cord, and the brain.

The area of the dimple – or a neighbouring area, for that matter – can become filled with fluid. At this stage it is called a pilonidal cyst and often requires drainage.

If the sacral pit or tuff reaches down to the spinal cord and involves those nerves, then bowel and bladder function can become impaired. The most common manifestation is urinary tract infection caused by inability to effectively drain the bladder due to poorly functioning nerves. The resulting stagnant urine in the bladder breeds infection. If nerves that go to the bowel are involved, then chronic constipation may also result.

Untreated Strep Throat Can Lead To Serious Complications

While sore throat is an ailment that clears on itself within a few days, strep throat often requires medical treatment with antibiotics. Physicians claim that in the absence of an appropriate medical treatment, strep throat can rapidly aggravate and lead to complications such as rheumatic fever, cardiovascular diseases or kidney affections. Before penicillin was discovered, infection with streptococcus bacteria could even cause life-threatening diseases. However, nowadays strep throat can be easily overcome with the means of specific antibiotic treatments.

It is very important to understand that sore throat and strep throat are not the same thing. Sore throat is caused by infection with viral agents and its generated symptoms are mild. By contrast, strep throat is caused by infection with group A streptococcus bacteria and its generated symptoms are usually intense. Strep throat is very contagious and it can be easily acquired in the flu seasons. Unlike in the case of sore throat, the symptoms of strep throat have a rapid onset and they are considerably more intense.

Sometimes, clinical examinations alone can distinguish between sore throat and strep throat. By measuring patients’ body temperature and by examining the aspect of the tonsils, doctors are usually able to tell whether the infection is caused by viruses or bacteria. However, strep throat can be correctly diagnosed only with the means of laboratory tests. In order to determine the nature of the infection, doctors take a sample of mucus for analysis. If the illness is indeed caused by group A streptococcus bacteria, doctors usually recommend an appropriate medical treatment with antibiotics.

There is a school of thought that our emotions get under our skin. And there is no doubt that skin conditions are a very physical phenomenon.

Sometimes, removing just some of the causes results in a significant reduction or complete cessation of the symptoms. Sometimes the same cause has more or less effect than usual. This is because it is the combination of causes that usually results in symptoms. The more you experience that irritates your skin, the more likely it is that your skin will be irritated. Similarly, the less factors you are exposed to that irritate your skin, the less likely it is that your skin will be irritated.

It used to be though that just a genetic element is involved. But what if you can overcome this legacy? What if there is more to this than genes?

To start with, we all know the old saying that you are what you eat. After all, your skin is literally made up of things that you have eaten.

There are foods that are often found to be culprits. It is generally accepted that the two most frequent baddies that people find are wheat and gluten. Gluten is the stuff that holds bread together, a bit like glue or wallpaper paste. And maybe it is not such a good idea to eat gluey stuff on a daily basis. And dairy foods are often not what you would find in nature. Some people cut out dairy and substitute, for example, with soya, then get a reaction to soya milk. If you have been tested and find that dairy is bad for you, then of course, follow the instructions given to you when tested. If not, and you find that cutting down on dairy is a help, that’s great. And maybe you can have dairy on some days and not others. Really, variety is the key. If your body is likely to develop a reaction to something, eating the same food almost daily is not a good idea. After all, in nature, we cannot eat the same food all the time; there are seasonal changes. And our bodies are designed to work with that. So it is a good idea to vary what you eat from day to day.

Another culprit in the modern diet is sugar. Too much sugar invites organisms that attack your body and reduce your immunity. One of the most common bugs that overgrows iin the body is Candida, which you may or may not have heard of. Candida thrives on sugar, upsets the intestines, and so weakens your whole body, including your skin. The modern diet has too much sugar in any case, and that has repercussions on your immunity in many other ways. And a healthier immune system means a healthier skin.

Now that we have discussed the baddies, what about the goodies in your diet? Well, it is generally accepted that having sufficient vitamins, minerals, and essential oils is good for you. Are you having enough? A visit to your health food shop or a nutritionist should point you in the right direction. It is also generally accepted that we should eat enough fiber to have regular bowel movements. Old stools in your colon result in toxins for your skin. And of course we all get told to drink so many glasses of water every day.

You also are how you feel. It is becoming more widely accepted these days that our emotions influence our physical health. So you can ask yourself what may be causing your skin to get upset. What irritates you? What gets under your skin?

Time and time again, I find that there is an emotional component to skin conditions. It may be big or it may be small, but eliminating it is the easiest part for most people. So just ask yourself: “What irritates me?” Common emotional irritants are rejection, shame, guilt, fear, or unexpressed anger. Find a technique that works for you for releasing these emotions. I particularly like EFT and Reiki, but you can use many others, including hypnotherapy, meditation, Yoga, NLP, shiatsu, and acupunture, to name a few. Whatever works for you is fine.
