It is important to make sure that you don’t strain your upper back muscles as you flex your hips and bend your knees. Breathing correctly is also very important.

Metatarsal Fractures in Elite Soccer Players

An England World Cup preparation would not be complete without one of its star players suffering from a metatarsal fracture, only weeks before the start of the tournament! David Beckham was the first victim of the much dreaded injury in 2002, and in 2006, Wayne Rooney topped most of the newspapers with the same injury. This time, it was John Terry’s turn, or so we feared. Thankfully things weren’t as bad as they originally seemed, and John Terry will still be present in South Africa, leading England’s defense.

So what exactly is a broken metatarsal and why do we seem to hear so much about it these days more than we ever have done in the past?

Before we identify why, let us take a closer look at the metatarsal bones first.

The bones between the ankle and the toes are called the metatarsal bones. The first metatarsal is located on the inside of the foot, and connects the ankle with the big toe, whereas the fifth metatarsal bone is located on the outside of the foot and connects the ankle with the little toe.

The two main functions of the feet are to propel and to support. The metatarsals play a major role in these two functions. By acting like a rigid lever to aid propulsion, they manage to increase our efficiency whilst walking or running. Additionally, they act like a flexible structure to aid and support balance to cope with uneven ground.

The second, third and fourth and fifth metatarsal bones are usually injured as a result of a prolonged period of overuse, creating something we call a stress fracture. A stress fracture is an incomplete fracture in the bone. It could be described as a very small sliver or crack in the bone. These types of fractures are also known as “hairline fractures”. These kinds of fractures are common with athletes, soldiers, and ballet dancers.

Another common cause for metatarsal fractures is direct trauma i.e. a direct kick to the sole of the opponent’s boot. This typically affects the first metatarsal bone.

One of the reason why the foot is so susceptible to fractures is that is very few muscles and fat protecting the foot from any impact.

The initial therapy with a broken metatarsal bone is to rest it, absolutely no exercise for 4 to 8 weeks. The recovery time of the injury is determined by the extent of the damage and which of the five metatarsal bones is affected.

Walking boots or shoes with very stiff soles may be advised for the player to wear in order to protect the fracture while it heals. If the fracture was caused by overuse and not by direct trauma, the athlete’s training habits, equipment and running technique should all be investigated and possibly adapted.

Putting a time scale on the recovery of a stress fracture is very difficult as the following examples of recovery time’s show. Michael Owen, broke his fifth metatarsal in 2006, was predicted to return in 6 to 8 weeks, but finally made his return 17 weeks later. Steven Gerrard broke his fifth metatarsal in 2004, again was predicted to return to football 6 to 8 weeks later, but eventually returned after 10 weeks. David Beckham broke his second metatarsal in 2002, was predicted to return 6 weeks later, and stepped back on the pitch after 7. David Nugent broke his fifth metatarsal in 2006, and returned 6 weeks later. He claims that his speedy recovery all came down to drinking lots of milk!

Dr Mark S. Myerson, M.D. a renowned expert in foot and ankle reconstruction at the Mercy Medical centre in Baltimore, Maryland, United States thinks that one the reasons for these fractures in Football players is the fact that many football players are bowed legged and have a chronically unstable or loose ankles. This puts an abnormal amount of strain on especially the fifth metatarsal, increasing the chance of a stress fracture.

Other suggestions explaining why we tend to see more metatarsal fractures these days than we have in the past included: an increase in the number of games played at the top level, an increase in training intensity, or even the training that a lot of teams do on artificial surfaces, which has a higher impact on the body.

Additionally, the pitches seem to be a bit harder these days than what they used to be due to their drainage. Grass pitches are often sand-based to improve drainage. The watering of these pitches increases their speed, but does not improve their hardness.

Players themselves are fitter, faster and stronger than they used to be. Again, this increases the forces that feet have to deal with.

Finally, some specialists argue that today’s boots are lighter and less supportive and protective than they were in the past.

With periodic decompression, pain relief and improvement in mobility was achieved by 71% of the study population without any additional therapy. The positive result was subjective pain relief of 0 or 1 from a scale of 0 to 5.

Non-surgical spinal decompression is indicated for individuals who are experiencing chronic debilitating symptoms due to degenerative disease of vertebral joints leading to spinal stenosis, characterized by persistent and chronic lower back pain that affects physical independence, progressive degenerative joint disease marked by appearance of complications like bladder incontinence, bowel incontinence, paresthesia, numbness or sciatica, spinal disc herniation that affect the functioning of nerves, bony outgrowth due to constant rub or friction, posterior facet syndrome or radiculopathy (damage to spinal nerve roots).

According to the research conducted by Steven J Atlas, surgical decompression has slightly better long term prognosis when compared to non-surgical decompression but the rate of short-term complications is higher in surgical patients. Atlas suggested that non-surgical approach should be employed in mild to moderate cases but if symptoms does not resolve, surgical decompression should be attempted.

If you have metallic implants in your vertebrae (as a result of severe injury or surgery) this therapy is not for you. In disorders of bone mineral density, established osteoporosis and chronic vitamin D deficiency or patients who have a positive personal or family history of aneurysmal conditions should not opt for manual spinal decompression.

In some cases when non-surgical decompression does not yield fruitful results, the only option left with most practitioners is to opt for surgical decompression that carries more risk and hazards when compared to a non-surgical variety, like infection, bleeding, iatrogenic damage to major nerves or tissues and others.

The 5 Trigger Points of Sciatica

Sciatica is a symptom, or group of symptoms, caused by any number of conditions which compress the sciatic nerve. The main symptom of sciatica is pain radiating from the lower back or buttocks, down the back of the thigh. Pain may travel as far as the calf muscles. There may also be associated tingling, numbness or weakness.

There are numerous conditions which may cause sciatic pain. These include a lumbar disc herniation (also known as a slipped disc, or herniated disc); spinal canal stenosis; sacroiliac joint dysfunction and piriformis syndrome.

Sciatic pain can also be caused by trigger points (T.P’s) within the gluteal (buttock) muscles. This is sometimes referred to as myofascial pain.

A trigger point is also sometimes known as a muscle knot. They are hypersensitive points within a tight band of muscle which often refer pain throughout the area they are found in. T.P’s can be felt as small, tender lumps within a muscle, which when compressed, often cause pain which may radiate elsewhere and a twitch response in the muscle.

What are the 5 trigger points of Sciatica?

Sciatic pain radiating into the hamstrings can be caused by trigger points found in the buttock muscles.

Sciatica caused by T.P’s develops gradually with no specific point of injury; there may be a cramping or tight sensation in the buttocks or hamstrings and tenderness when pressing these muscles. There is not usually any lower back pain, walking and even running may be pain free and strength is rarely affected.

1, 2 & 3: Gluteus Medius

Gluteus medius is one of the three Gluteal muscles (‘glutes’) which form each buttock. They are responsible for extending, abducting and laterally rotating the hip joint.

Gluteus medius is found just below the Iliac crest. There are three common T.P’s in this muscle which can refer pain into the posterior thigh. They are located at the origin of the muscle, all three in a row, just below the iliac crest.

4: Tensor Fascia Latae

TFL (as it is shortened to) is a small muscle found on the outer hip. It attaches to the front of the iliac crest and descends downwards to converge with the IT band on the outer thigh.

A trigger point can often be found in the superior half of the muscle, which radiates pain inferiorly, down the IT band and lateral hamstrings.

5: Piriformis

The Piriformis muscle is a small muscle found deep within the centre of the buttock. It originates from the Sacrum and crosses horizontally to the head of the Femur (thigh bone).

It is strongly associated with Sciatica as a condition called Piriformis syndrome can also cause sciatic symptoms. However, this is an actual compression of the nerve caused by muscle tension or an anatomical variation where the nerve passes through the Piriformis muscle (rather than behind it). This occurs in around 10% of the population.

This is not the same as a Piriformis trigger point which does not affect the sciatic nerve at all, but radiates the pain into the hamstrings in a similar fashion.

The most common trigger point found within the Piriformis muscle is in the muscle belly, slightly towards the insertional tendon. This trigger point will refer pain into the outer buttock and down the back of the thigh.

Trigger Point Therapy

There has been lots of research undertaken into the best treatment of trigger points. Acupuncture (or dry needling) and ischemic pressure have been shown to be the most effective.

Acupuncture is the insertion of a needle into the trigger point. The small needle is inserted into the trigger point and left in place for a few minutes. During this time, the muscle gradually relaxes and lengthens. 3-4 sessions of dry needling are usually required to eliminate the trigger point.

Ischemic pressure is the application of direct pressure to the trigger point, with enough pressure to temporarily cut off blood flow to the TP (ischemic = no blood!). The aim of this technique is to stimulate receptors in the muscle which regulate its tension, to reduce the muscle tone. This pressure also provides an analgesic (pain relief) effect due to the release of pain mediating substances. Once the pressure is released, a sudden influx of blood to the point helps to flush out any waste products and bring the required nutrients for healing.

Following either technique, it is recommended that the muscles treated are stretched thoroughly to reset the muscle spindle length.

Slipped Disc, Pinched Nerve, and Back Pain

Vertebrae are separated by inter-vertebral discs which act as small shock absorbers and facilitate movement of the spine. These discs consist of a very strong fibrous sac filled with an elastic jelly and very firmly welded to the vertebrae and cannot come loose. They can be compressed, twisted, rotated, bent and move obliquely, facilitating movement of the spine.

The vertebrae and discs alone are floppy. The spine’s movement is stabilized by facet joints between each of the vertebrae. Facet joints limit movement making it extremely difficult for the vertebrae to become displaced.

In the centre of each vertebra is a hole, these all line up to form the spinal canal, which creates a well protected path for the spinal cord. An arch below the facet joints forms openings between the vertebrae for the nerve root to pass through. From there the nerve branches out into the body.

Ligaments are made up of long bands of fibrous tissue and are extremely strong. These run up and down the spine and between the small joints, encapsulating the entire spine, holding it firmly together giving it strength and support.

When suddenly stressed as in an accident, sports injury or a fall, the discs get compressed and the facet joints – which take most of the impact – get aggravated causing inflammation.

Inflammation is one of the body’s emergency repair mechanisms, increasing blood flow to the area, bringing in extra nutrients and oxygen. When inflamed, the facet joint swells into the spinal cavity pushing the nerve root up against the vertebral body, pinching it to a greater or lesser degree. This is where the pain comes from and not from a ‘slipped disc’. As we can see from the way spine is supported, it is physically impossible for a disc to slip but the term is still used by many.

The above example is a case of the back being damaged. However – in the case of non-specific lower back pain, why is it that, although there is great value in treatment, one needs to keep returning for the same treatment? Why, for example, does the back keep ‘going out’?

Well… consider this. If you’re suffering from back ache, you don’t really think of it in terms of ‘the back’ as a whole, you narrow it down to something like; the spine is out, the discs ‘have slipped’, muscles have cramped, kidneys etc. So why stop there? If you keep narrowing it down you eventually arrive at cells, tissues and energy itself.

All of the body parts listed above are made up of cells and/or tissue and require energy to operate.

Energy can not flow fluidly over damaged or diseased cells and tissue and therefore the muscles, ligaments, discs, bones, organs, et al, can’t function efficiently. Cleaning up the cells and tissue allows the energy to flow fluidly so the muscles, bones, et al, can operate efficiently. Even though so many psychologists are sometimes quick to make a diagnosis of ADHD when they are presented with a child that is overactive, kids can be overactive and not be ADHD. Children will appear to be ADHD at some stages of their life especially when they are overly excited and not sure what to do with their energies. They act this way when they are nervous and dealing with an onslaught of emotions that they are unaccustomed to and are not yet sure just how to channel. The reasons for ADHD in kids range from it being genetic, being related to biological changes, as well as a change in the structure of their brain, among other factors. Apple Cider Vinegar
