o It is critical that hopitalized patients with suspected or documented TB will be placed in appropriate isolation. This includes a private room with negative pressure and adequate air exchanges. Persons entering the room must wear masks or respirators capable of filtering droplet nucleari.

o Patients should remain in isolation until sputum becomes smear-negative; however, patients ordinarily should not be kept in the hospital for the sole purpose of providing isolation, Special arrangements are necessary for patients who live with children, individuals infected with HIV, patients returning to a closed-group setting (eg, nursing home, correctional facilities, residential facility, homeless Shelter).

Further Outpatient Care:

o Patients diagnosed with active TB should have sputum examined for M tuberculosis weekly until sputum conversion is documented. Monitoring for toxicity includes baseline and periodic liver enzymes, complete blood count, and serum creatinine.

o In addition, patients on pyrazinamide should have baseline or periodic serum uric acid determinations, and patients on long-term ethambutol therapy should have baseline or periodical acuity and red-green color perception testing. The latter can be performed with a standard test such as Inhihara test for color blindness.

Deterrence / Prevention:

o Patients with a clinically significant result on tuberculin skin testing (see other tests) should be given a course of therapy once active infection and disease is ruled out. Guidelines published by the CDC in 2000 now refer to this treatment of latent TB. The recommended regimens are listed below:

o INH daily for 9 months

o INH twice weekly for 9 months (given as DOT)

o INH daily for 6 months (should not be used in patients with fibrotic lesions on chest radiograph, patients with HIV, or children)

o INH twice weekly for 6 months (given as DOT, should not be used in patients with fibrotic lesions on chest radiograph, patients with HIV, or children)

o Rifampin daily for 4 months

o Rifampin plus pyrazinamide daily for 2 months

o Children should be given INH for 9 months. In addition, children younger than 5 years who have closed contact to an active case of TB should be started on INH even of skin testing is negative; preventive therapy can be stopped if repeat skin testing is negative 3 months after last contact with a culture positive source case.
o
Patient exposed to MDR-TB may be given ethambutol plus pyrazinamide for 6 – 12 months or pyrazinamide plus levofloxacin for 6 – 12 months; the index isolate should be susceptible to all drugs used.

o Recommended regimens in patients with HIV infection include pyrazinamide plus rifampin daily for 2 months, rifampin alone daily for 4 months, or 9 months of INH (daily or twice weekly). Patients on antiretroviral therapy may need rifabutin in place of rifampin.

There are still other reasons for chest pain that are not cardiological though it is the same with heart attack. Some of the symptoms heart diseases are acute chest pain and fullness in the chest but this does not affect breathing.

Here are some of the problems that cause pain in the chest.

Broken ribs – this is very common cause of pain in breathing especially when you cough. If you had an accident and you had chest trauma, you might have a fractured ribs. When you press your chest or when you force yourself to carry heaving things, you will have a breathing pain.

Pleurisy – this is the inflammation of cavity lining that surrounds the lungs. This is also called "Pleuritis." One of the main symptoms of pleurisy is acute pain in the chest when coughing and breathing. There are also other symptoms like dry cough, chills and fever, and shortness of breathing. Viral infection is the common cause of pleurisy. Other ailments can also cause this problem. Some of the ailments are autoimmune diseases and pneumonia. There are treatments provided for these kinds of diseases.

Pneumonia – do you know that sharp pain in the chest when you cough or breathe deeply is one of its symptoms? Yes, it is. Antibiotics are one of treatments of pneumonia. Among its other symptoms are sweating, shacking chills, muscle pain, high fever, and shortness of breath. Once a person is infected with pneumonia, this should be taken seriously because it can threaten life. If anyone from your family who experiences pneumonia, call your doctor immediately.

Pisceans often have escapist tendencies, and require alone time to recharge their buttons. Like Aquas, they live in their own world. Unlike Aquas, however, Pisces live in a dream world. They can often be found daydreaming about their hopes and dreams for the future. Pisceans are incredibly compassionate and sensitive people who find the real world a harsh, cold place. This is why they tend to escape to their private dream world quite often; negative energy affects them greatly, so they need a break from life every so often to recharge their batteries.. At the same time, their desire to escape the everyday world can land them in difficult situations.

Pisces is a mutable sign, which indicates they are changeable. Pisceans are chameleon-like, and can adapt to new situations, people, and places very easily. This changeable energy can cause difficulties in their personal relationships.

A Piscean’s vision of the perfect relationship is often unrealistic and based on fantasy. It is difficult for them to find a partner who lives up to their ideals. So, what is a girl to do? How do you deal with a guy like this? The first thing to do is to appeal to his emotions in a very gentle way. If he senses anger or negativity, he might swim away. Appreciate him for who he is, and do not take advantage of his kind nature. Appeal to his spiritual side; individuals who are overly-materialistic turn him off. Patience, tolerance, and acceptance are the key to having a relationship with a Piscean. Be a strong, reliable partner to him. Feel free to take the lead and make him feel taken care of and cared for. Pisceans are also known for their wandering eyes, so learn to handle your jealousy. They also suffer from low self-esteem, so stroke his ego.

Once a Pisces is in love with you, you will become his whole world. While he will still disappear from time to time, he will always swim right back to you.

Natural Remedies for Bronchitis Using Therapeutic Essential Oils

I know from experience that once I begin convulsive coughing, I'm really headed for trouble. It's taken me months to get over bronchitis before, so I take every cough as a signal to act quickly with my essential oils and oil-based supplements.

Why is coughing such a serious matter? Two reasons: 1) it can do serious and permanent damage to my lungs, bronchioles and vocal chords; and 2) it produces a lot of chronic inflammation, irritation, and free radicals through my body, which take their toll on my health over an extended period of time.

Not only can bronchitis escalate into something more serious like pneumonia, but it can leave me weak, exhausted, and vulnerable for more serious degenerative illness over time. Bronchitis is not a temporary thing that ends when the coughing stops and the antibiotic prescription runs out. It must be healed promptly, deeply and completely. For me that will take at least 10 days after all symptoms have ended.

Here's what I do when I have a cold that begins to escalate into coughing.

I continue the steam treatments I started at the first sign of a cold, both in hot therapeutic baths each evening using essential oils and Epsom salts and also steamy showers with an essential oil diffuser in the same room for about 20 minutes each morning before I leave for work.

If I begin to cough at work, I've found that a convenient mug of warm aromatic water will quickly calm my coughing. I fill a thermos of hot tap water and put in a drop of Spearmint or Basil oil. These calming anti-inflammatory oils are soothing and also flavorful.

When I've had bronchitis, it's been common for me to start a convulsive coughing spell shortly after I lie down for the night. When I was a child my mother would slather on Vicks VapoRub and put a cold wash cloth over my thymus area (just below where my collar bones join). Now I use diluted Peppermint oil on the area along with the cold washcloth and cover it with a dry towel. It stops the coughing by cooling the inflated bronchioles that tickle and trigger the coughing. If I find that the cloth heats up and the coughing starts up again before I can go to sleep, I will definitely wave the damp washcloth in the air to cool it and apply it again.

These are by no means the only things I do when I catch a cold. I take a large quantity of Vitamin D3 for 3 days. I up my intake of Krill Oil to control inflammation. I ingest a lot of essential oil capsules over a period of almost a month. I have oils in all my drinking water and many recipes. I use a cold-air essential oil diffuser at least twice a day. I do what's called the "lick trick" with essential oils to help with sore throat infections, and I keep my sinuses clear at all costs. For me, the sinuses are where the infection starts, and if I can get that cleared up quickly and keep it clear through any other symptoms, I can prevent ear infections, sinus headaches, and a lot of other symptoms typically associated with a cold.

I also apply a lot of oils topically, especially after my shower and bath while my skin is moist and the oils are most easily absorbed.

Dr. Daniel Penoel, who trained me in the medical uses of essential oils taught me that I needed to keep up all my aromatic habits for at least 10 days after I feel completely well. That's hard to do when I'm feeling great and back to normal. I also know from personal experience that if I do not heal completely, the symptoms can return with a vengeance.

Avoiding Shoulder Dislocation and Separation Using Prolotherapy

The shoulder is made of a complex matrix of bone and soft tissue that allows it an extreme range of motion. But there is a price the shoulder pays for that range of motion. Greater risk for chronic injury.

The shoulder is held together by soft tissue stabilizers lead by the ligaments that connect bone to bone. Over the course of time, especially in sports that involve heavy shoulder to shoulder contact such as hockey, lacrosse, football, wrestling and basketball, the ligaments stretch out and become "lax." When the ligaments become lax, the risk for dislocation and separation becomes greater.

In a February 2011 study published in the British Journal of Sports Medicine, researchers took a look at rugby players with measurable shoulder laxity to gauge the risk of shoulder dislocation. What they found was that 50% of those athletes tested were at significant risk.

How does dislocation event?

It is estimated that 95% of shoulder dislocations occur when the athlete suffices a blow to the shoulder that forces the shoulder joint "back" or down or when they fall to the ground with their arms stretched over their heads.

Strengthening the ligaments as preventative measure:

The key to avoiding shoulder surgery or devastating shoulder separation requiring surgery is to strengthen the shoulder capsule. This can be accomplished by working the strong shoulder muscle group and / or by treating the weakened shoulder ligaments with a simple injection technique called Prolotherapy.

To understand the importance of the need of strong ligaments to hold the shoulder together, the athlete needs to understand that the severity of shoulder separation injury is measured by the degree of injury to the ligaments.

The acromioclavicular (AC) and the coracoclavicular (CC) ligaments hold the shoulder together at the point where the collarbone (clavicle) and the end (acromion) of the shoulder blade (scapula) meet.

  • In type I level separation the AC is partially torn, the CC is not.
  • In type II separation the AC is completely torn, the CC is partially or not torn
  • In type III separation both ligaments are completely torn.

Obviously the more significant the drinking, the longer the athlete is out of their sport.

Untreated shoulder instability can lead to an alteration of an athlete's game to "protect" the sore shoulders or worse chronic shoulder separation that can keep them away from their sport for significant amounts of time.

Prolotherapy and the shoulder ligaments:

Prolotherapy is an injection therapy consisting primarily of dextrose which accelerates the body's own inflammatory response to repair ligaments.

By isolating damaged ligaments, and injecting these spots, the liagment can be healed and strengthened through controlled inflammation. This is done through the stimulation of a new collagen matrix. New collagen makes ligaments stronger, thicker, and restores them to their normal strong, taut state. To any athlete stronger is always the best option.

Prolotherapy can work fast, a treatment once a week over a 4 – 6 week period is usually all that is needed to see results. Side-effects are also limited and in many cases the athlete can continue their activities during treatments.
