Virginia Commonwealth University (VCU) recently conducted and presented a study on pegylated interferon alfa-2a (PEGASYS) and its ability to eradicate the Hepatitis C virus from its patients. In order to establish whether or not Hepatitis C reemerges in patients who have achieved success through treatment with PEGASYS or a combination of PEGASYS and ribavirin, researchers at VCU monitored blood levels of hundreds of HCV infected and HCV/HIV co-infected patients who had already realized treatment success. Of the nearly 1,000 patients whose HCV blood levels were reviewed for an average of four years after achieving sustained virologic response (SVR), more than 99 percent had maintained undetectable levels of the virus.

While these results suggest that SVR may be permanent, many newly hopeful Hepatitis C patients remain unaware of the study’s complete details. Do these results actually signify a cure for HCV has been found?

Does Helicobacter Pylori Infection Cause Psoriasis?

One of my good friends had a classic case of psoriasis. His psoriatic rash covered areas of his face and scalp, torso, arms and legs. His story is an important one because he almost completely overcame his psoriasis simply by eliminating hidden digestive infections, eating a “rotation diet” and thoroughly detoxifying his body.

Psoriasis can be a very nasty inflammatory skin disease. It affects 1-3% of the Caucasian population and may severely compromise the quality of life in those affected.

There can be a huge amount of variation in the severity of the condition from person-to-person. The psoriatic rashes can impact day-to-day activities and social life, which is why it can be so unpleasant.

Many leading authorities actually argue that psoriasis is not actually a skin disease at all. Instead, it reflects that the body is responding to something else – usually something being inside the body that should not be there. In fact, it is now agreed that the disease is actually an inflammatory response.

It has been suggested that H pylori infection may be one of the organisms capable of triggering the inflammatory response that manifests as psoriasis.

Night urination can affect the quality of life. It arises due to a variety of problems, for example, enlargement of the prostate gland, congestive heart failure, kidney failure, etc.

Night urination is defined as waking up once or more during the night in order to urinate. This is a common phenomenon that affects the quality of life of much of the population and may affect life expectancy. The percentage of those who suffer from night urination fluctuates with increased age. Various surveys found that the incidence ranges from 16-40 percent among those aged 45-40 and 50-90 percent among those aged 80 years or over.

It is also considered a sleep disorder because it is the cause for daytime sleepiness and fatigue, depression and decreased mental ability. As a result, there may be increased morbidity, increased risk of accidents and increased mortality rate that are mainly due to falls and fractures.

In a healthy person, there are daily periodic changes in the production of urine by the kidneys. Daytime urine output is greater than at night.

By definition, increased nocturnal urine output means more than 20 percent of the total output of urine a day. According to some researchers, the production of more than 0.9 to 1.3 cubic centimeters per minute of urine at night is considered increased nighttime output. Patients who sleep six hours a night and their nocturnal urine output is 324 cc or more will be considered as suffering from increased nocturnal urine output, because they produce over 0.9 cc per minute. Healthy people are not supposed to get up at night to urinate.

The widespread public assumption is that excessive night urination is caused solely due to an enlargement of the prostate gland. But the problem may have several other significant reasons. One is a defect in the “anti -night urination” hormone production. This defect can be congenital or acquired. In many cases, it is part of the aging process, because the hormone production decreases with age. Even drinking fluids or alcohol before bed can affect the production of this hormone.

Another reason is the excessive secretion of salts and water by the body or due to congestive heart failure. Congestive heart failure means your heart can not pump blood effectively. As a result, fluids remain in the body, concentrated mainly in the space between lower limbs cells. These fluids cause edema. Edema can disappear in the morning, because when lying the fluid flows back into blood vessels, the heart and, finally, to the kidneys.

Kidney failure, sleep apnea syndrome or venous insufficiency also can cause blood drainage problems from the lower extremities. As a result, the cumulative fluid in the space between the cells is as in congestive heart failure. While you are lying down at night, the fluid is dripping to the veins in the legs and reaches the kidneys. That produces more urine. Lack of estrogens, i.e. female hormones, causes urinary system dysfunction and frequency in day and night urination. Sleep disorders and urinary bladder storage disorders also increase the need for urinating at night.

The best-known problem associated with it is an enlarged prostate which brings about an obstruction in the bladder’s output. As a result, defects are created causing the need to urinate frequently day and night. A man with the problems should have a short and simple medical examination, which includes a physical examination, completing a questionnaire about his medical condition (illness, surgery and medications he takes) and, of course, filling in a diary documenting his urination output during a 24 hour period.

Recommendations are given to patients to change their lifestyles, such as raising the legs while lying down or using elastic stockings for patients suffering from edema. These measures improve the vein drainage and reduce edema. Another recommendation is to avoid drinking a lot-especially alcohol, tea and coffee-before going to bed. If the patient suffers from sleep apnea syndrome, using a positive pressure mask improves sleep quality and reduces night urination.

Other means of dealing with the problem are the treatment of diseases such as congestive heart failure, diabetes, situations of an overactive bladder or blockage of the bladder by an enlarged prostate.

What is systole?

Systole is when the heart is contracting.

What is diastole?

In the typical lub-dub of a heart beat, lub is systole and dub is diastole.

Heart Sounds:

S1: start of systole; caused by the tricuspid and mitral valves closing.

S2: diastole; aortic and pulmonic valves close

S3: inrushing blood from the atria causes oscillation of the blood back and forth in the walls of the ventricles. Occurs in the middle of diastole because initially there is not enough blood in the ventricles to cause the reverberation.

Pathologic causes:

-rapid ventricular filling:

-mitral regurgitation (which means atria will be overfilled in systole and can thus send more blood to ventricles during diastole and thus cause rapid ventricular filling)

-elevated left atrial and left ventricular filling pressures (from a stiff and dilated left ventricle)

-ventricular septal defect (hole allows rapid filling from other side)

-poor left ventricular function

-post-MI (dead tissue is stiff and does not relax so filling is quick)

-dilated cardiomyopathy (thin and stiff walls)

S4: atria contracting forcefully to overcome stiff or hypertrophic ventricles. (stiff from post-MI fibrosis, hypetrophy from aortic stenosis, hypertension, overloading).

Occurs immediately before S1. Sometimes heard in healthy children and trained athletes.

If pathologic, will be best heard at apex.

The consequence was six new influenza with pneumonia codes for the 2011 ICD-9 update:

  • 488.01 – Influenza owing to identified avian influenza virus with pneumonia
  • 488.02 – Influenza owing to identified avian influenza virus with other respiratory manifestations
  • 488.09 – Influenza owing to identified avian influenza virus with other manifestations
  • 488.11 – Influenza owing to identified novel H1N1 influenza virus with pneumonia
  • 488.12 – Influenza owing to identified novel H1N1 influenza virus with other respiratory manifestations
  • 488.19 – Influenza owing to identified novel H1N1 influenza virus with other manifestations
  • Disorder of fluency

    After this, there are some changes to the fluency disorder ICD-9 codes. New code titles will help distinguish childhood onset fluency disorder, adult onset fluency disorder, and – here is where it picks up interest for home care – fluency disorder following brain lesion or disease.

    Probably, you will remember that (Late effects of cerebrovascular disease, fluency disorder) was implemented on October 1 last year. The plan for this year is to alter the inclusion term “Stuttering” at 438.14 to read “Stuttering due to late effect of cerebrovascular accident”.

    Other such changes include revising 307.0 to read “Stuttering; Adult onset fluency disorder” and adding up new code 315.35 (Childhood onset fluency disorder).

    Cystostomy Complications

    Right now, you have specific codes for compilations of all sorts of artificial stomas; however not for a cystostomy. If you are trying to code for an infected cystostomy, you have got 997.5 (Urinary complications) or 996.39 (Mechanical complication of other genitourinary device, implant, and graft); however these codes do not specify that the compilation is with the cystostomy or that there’s an infection. For the coming year, one can look forward to four new codes that’ll rectify this problem.

    Postoperative Aspiration Pneumonia

    There’s confusion when it comes to coding aspiration pneumonia resulting from a procedure code 997.39 (Other respiratory complications), includes aspiration pneumonia complicating a procedure; however there’s a use additional code note at the start of the category to find out the complication.

    This left coders with a sticky situation. Should they pair 507.0 (Pneumonitis due to inhalation of food or vomitus) with code 997.39? Or should they go for just 997.39 since aspiration pneumonia is an inclusion term.

    The proposal for the next year is add a new code and to change the instructional notes at categories 507 and 997 to help coders in choosing the right codes for this complication.

    You will see the following new and modified codes:

  • 507 – Pneumonitis due to solids and liquids
  • Add Excludes: postprocedural pneumonitis (997.32)

  • Postprocedural aspiration pneumonia

    These codes were found in the Federal Register as part of the proposed Hospital Inpatient PPS rule; however we will not know the final list of new and changed codes for a few months to come. So stay tuned to these home health ICD-9 codes.

    Chest Pain – Is it Angina, Heart Attack or Non-Serious?

    Almost everyone suffers from some sort of physiological chest pain at one time or another, and the important thing to note is that there are many causes of this condition.

  • Heart
  • Lungs
  • Esophagus
  • Muscles
  • Ribs
  • Tendons
  • Nerves
  • However, most chest pain sufferers tend to focus on the big question in their minds – that there are just two different classifications – those that relate to the heart (cardiac) and those that do not (non-cardiac).

    Cardiac causes of Chest Pain

    Heart attack

    This is caused by a blood clot that prevents usual flow of blood flow to the heart muscle. This can cause a feeling of pressure, fullness or a crushing pain in the chest lasting more than a few minutes. The pain may radiate to other areas such as the back, neck, jaw, shoulders and arms, especially the left arm. Other symptoms may include shortness of breath, sweating, dizziness and nausea. All, some or none of these may accompany your chest pain.

    Angina

    Fatty deposits can build up in the arteries that carry blood to the heart. This can reduce their width, and this in turn can cause a restriction of the blood flow to the heart, which is usually more noticeable after physical activity or exertion. This type of restricted blood flow to the heart can cause regular, recurrent episodes of chest pain, which is termed Angina Pectoris, or Angina, is often described as a pressure or tightness in the chest. It’s usually brought on by physical or emotional stress. The pain usually goes away within minutes after you stop the stressful activity.

    Other Cardiac Causes

    There are some other complex medical problems, which can cause chest pain.

    Pericarditis – inflammation of the sac surrounding your heart a short-lived condition often related to a viral infection. Pericarditis causes sharp, piercing and centralized chest pain. You may also have a fever and feel sick.

    Aortic dissection – a rare, life-threatening cause of chest pain where inner layers of the main artery leading from the heart (aorta) separate. This causes blood to flow between them, causing a sudden tearing chest and back pain. It is usually caused by a physical trauma to the chest cavity, but can also result from uncontrolled high blood pressure.

    Coronary spasm, also known as Prinzmetal’s angina, can cause varying degrees of chest discomfort. In coronary spasm, coronary arteries – arteries that supply blood to the heart – go into spasm, temporarily closing down blood flow to the heart.

    Non Cardiac Causes

    There are many conditions that are unrelated to the heart, which can be the cause of chest pain. These include:

  • Asthma
  • Pneumonia
  • Anxiety
  • Rapid breathing
  • Heartburn
  • Pleurisy
  • Rib or muscular injuries
  • Collapsed lung
  • Swallowing disorders

    4. Heat up feet in a bucket of hot water or just take a hot bath.
    5. Dry the feet
    6. Wring out the cold cotton socks.
    7. Immediately put the ice cold socks onto the feet and cover with the wool socks.
    8. Lay down for the night and go to sleep – or keep them on for the day while around the house.

    I recommended this to a friend of mine with nightly migraine headaches. She did this prior to bed and her migraines did not appear.

    It sounds like it will be a shock to the system but it is fine as the feet are hot. It actually feels nice.

    Topical essential oils

    These are wondrous yet can be problematic if not careful. They should not be in reach of children as they are deadly. Yes deadly. They are super concentrated oils and if swallowed or touched directly onto a little one’s skin, serious burns or other harms can be done. Keep in a locked cabinet or way up high.

    Visit your favorite health store to figure out which ones you like the best. I am a fan of eucalyptus, rosemary and cedar. One must dilute the essential oil in a base of olive oil before applying.

    Apply to your temples, on your maxillary and frontal sinsuses and you may want to put some between your upper lip and nose. Do not put in your nose directly as they will burn your mucous membranes. Keep away from your eyes also.

    Limit dairy products

    Dairy products and sugar are major mucous forming foods and will only increase head congestion. Limit the amount eaten during this time and you will likely recover faster.

    Third Eye Acupressure Point

    According to acupressure.com, this point will help: “Directly between the eyebrows, in the indentation where the bridge of the nose meets the center of your forehead.” Simply place pressure at this point using the method shown to the right.
