Itchy (Prurisy) Ears

One of the more common problems I see in my practice is that of itchy ears. The patient would have, by the time they present, tried all sorts of remedies including the dangerous practice of using ears buds, match sticks and a miscellany of other harmful instruments to rid the ear of the itch.

The causes commonly arise from the external ear, and less commonly from the middle ear.

External ear causes

1. Eczema: There are various types of eczema. It can be allergic dermatitis. These patients may have a history of allergies such as asthma or hay fever. It can be in the form of irritating or contact dermatitis: a reaction to products such as shampoos or oils when these come into contact with the skin of the external canal. Ear piercing may lead to eczema if the patient reacts to certain metals, such as nickel, in the ear ring.

2. Infections: eczema can lead to infections of the external ear. This is referred to as otitis externa, and can be caused by bacterial, fungal or viral agents.

Bacterial Otits externa is typically caused by streptococcus and staphylococci bacteria. Very rarely is pseudomonas responsible. It typically presents with itching, and later pain, associated with a purulent ear discharge. The skin of the ear is reddened and thickened. Sometimes the ear canal will be completely stenosed, and with the ear drum barely visible. Hearing might also be affected.

Treatment: entails the use of antibiotic ear drops. The ear must be kept dry since moisture exacerbates the infection. Analgesics are often necessary.

Fungal otitis externa is less common than the bacterial variety. The commonest fungal agent will be candida albicans, and to a lesser extent aspergillus negri. The symptoms will be similar to bacterial otitis externa. Clinical examination will reveal the presence of fungal spores in the external ear canal.

Treatment: consists of the use of topical anti-fungal agents.

Viral otitis externa: are also uncommon and may follow an upper respiratory tract infection. Bell’s palsy and shingles are often associated with inflammation of the external canal due to blisters forming along the track of the Facial nerve.

3. Impacted wax (cerumen accumulation)

Wax is normally produced by the wax glands that situated in the external canal. Wax or cerumen provides a protective function to the ear.

Because of the outward movement in which the ear sheds its skin, wax normally makes its way to the external opening of the ear where it falls out. But there are some people who are genetically overproducers of wax (producing more wax than the ear can remove), leading to impaction in the external ear canal. This wax may either be hard or soft. The other cause of wax impaction is the use of ear-buds, which tend to push the wax deep into the ear canal. Symptoms of ear impaction include itching, pain, and even deafness.

Treatment

A variety of oils are used to soften the hard wax. Suctioning and irrigation of the ear are also used. Instrumental removal with a curette is also common.

For genetic overproducers recurrences are common.

Middle ear causes

Otitis media is a common cause of itching in ear. The build-up of fluid in the middle ear causes the ear to itch, and this may later change to pain. Also, this can happen with the rupture of the eardrum and the discharge constantly making the external ear wet.

Treatment consists of dealing with the underlying condition.

Summary

“Itchy ears” is a relatively common presenting symptom. Often the diagnosis is easy to make, and the treatment simple. Undiagnosed and poorly treated conditions can however lead to serious complications

The King’s Speech 2010

What Worked?

Public speech probably makes the list for top 10 worst fears of practically everybody. For people with speech impediments, that fear grows teeth. For a king with a speech impediment, that fear turns into a monstrous wall. In the King’s Speech, Director Tom Hooper weaves the story of Bertie/King George VI and his problem of dealing with his stammer. A dialogue-driven period piece about a stuttering King-to-be seems like a film tailor-made for the elite elderly, but the team involved makes it an enjoyable, inspiring ride.

Coated in muted colors, the tint of the film matches the low-self esteem of Bertie, as well as the way he is silenced by his stammer and resides in the shadows of his family. Hooper captures the perspective of Bertie rather well, closing tight on his quivering lips or taking the camera low for an ants-eye view of the colossal audience. Everything about the direction works well, and enhances the mood of the film.

Colin Firth playing Bertie/George VI is achingly genuine, as we constantly see him bursting with frustration because of his verbal shackles. Despite his harrowing speech impediment, his warm-heart beams, especially when he interacts with Lionel Logue, played amazingly by Geoffrey Rush. We feel for Bertie, as he is trapped by his lack of voice and we feel his determination as he is climbing out of a dark hole. We even see the envy in his eyes when he watches footage of Hitler rousing up a crowd through his oratory talent. Firth gives a thoroughly magnificent performance, and it’s equally matched by Rush’s performance as Lionel Logue. Logue is an eccentric, brash and rather clever speech therapist. He becomes a trigger for Bertie’s confidence, and guides him as a friend and a teacher. The chemistry of both is the heart of the film. Helena Bonham Carter also gives a touching performance as Queen Elizabeth, feeling Bertie’s pain and standing by him to see thing through it.

This is a character driven piece, and not once does the film get swept up in the major historical moments; rather, it keeps the story intimate to Bertie’s problem and realistic triumph over it. The film is about a man trying to be the best version of himself that he can possibly be, and the start of that is getting over his speech impediment. If he can successfully deliver a speech to his people, he can lead them through the horrors of WWII.

Potential Drawbacks:

Despite all the praise this film has received, it does not leave a lasting impression. There’s nothing special or memorable enough about the film that will preserve it as a true classic. What helps the film may also hinder it: sometimes it is too intimate. For example, the speech King George VI finally gives is not very engaging because the film audience is not concentrating on the words being said, only how they’re being said.

For those who do not enjoy dialogue driven films, this film is probably not for you.

Venetian Architecture and Interior Design

The City of Venice, Italy

Venetian architecture comes from Venice, a city in northeastern Italy which is made up of 118 small islands separated by canals and all linked by a series of bridges. Venice was named for the ancient Veneti people who inhabited this region in the 10th century BC. The city itself was founded between the 5th and 6th century by wealthy inhabitants from the mainland who were fleeing the barbarian invasions.

History of Venetian Design

Venice was the birthplace of unique architectural styles due to the necessity of building homes above the many canals. The buildings were set on closely spaced wooden piles made from the trunks of alder trees. Venetian architecture was influenced by the Gothic style along with Byzantine and Ottoman influences. Some of the more famous Venetian architects were Baldassarre Longhena, who designed many churches in the Baroque style during the 17th century and Carlo Scarpa, a 20th century architect who created many designs of landscapes, gardens and buildings not only in Italy but throughout the world.

Venetian Architecture

Venetian architecture is lighter in structure and more graceful than the heavier buildings in other European cities. Every inch of land was considered valuable so architects never added any more weight or size than was necessary to support the building. The most famous design element of Venetian design is the Gothic lancet arch, where the top of the arch is tall and pointed, like a lance. Another design used in the 14th and 15th centuries is a central hall, called a portego, which is a long passageway usually opened with a loggia with Gothic arches.

Colors That Support the Style

Venetian colors were mostly rich colors like dark reds, muted yellows and bright blues. Many colors were muted in tone and during the Renaissance, they were gradually softened to duplicate natural colors. Of course, you cannot talk about Venetian style without mentioning Venetian plaster. The plaster was mixed with marble dust and applied in thin, multiple layers, then polished to a smooth surface. This gives the illusion of depth and texture. Venetian plaster can also be left unpolished which leaves a matte finish that is rough and stone-like.

Venetian Design Elements

The Venetian style was also used to create beautiful interior designs. In fact, some of the best examples of rich, extravagant Rococo designs were found in Venice. Draperies and curtains were made from materials like damask, velvet and silk. Other elements found in Venetian homes were girandole mirrors, colorful chandeliers using Murano glass and precious stones, polished terrazzo flooring, arches with broken pediments over windows and doorways, porcelain figurines and oriental rugs.

Examples of Venetian Architecture

In order to explore the world of Venetian architecture, we will look at some historical examples.

Doge’s Palace – This structure was originally built in 810 in the Venetian Gothic style, was rebuilt and partially reconstructed a number of times. It is now a museum and is a landmark of Venice.

Ca’ d’Ora (golden house) – Also known as Palazzo Santa Sofia, it got its original name from the gilt and polychrome decorations on the outside walls. The architects were a father and son team – Giovanni and Bartolomeo Bon, who also, by the way, worked on the Doge’s Palace.

My dream is to travel and learn about culture, history and design. I hope some day to visit the romantic city of Venice, Italy; to see the things I write about, experience the culture and the warmth of its people. The Telephone Revolution That Changed the World

In 1876, Bell was the first to receive the U.S. patent award. There were many other inventions that Bell invented. He invented the optical telecommunications, hydrofoils and aeronautics. In 1888, Alexander became one of the founders of the National Geographic Society.

In the 1870s, Alexander Graham Bell created the first device that could transmit speech electronically; it was called the first telegraph. However, Bell had some competition on the invention of the telegraph, (Elisha Gray) that ended up in a legal battle for the patent rights. The telegraph and telephone are both wire-based electrical systems that resulted in an improvement of a better telegraph. When Alexander experimented on electrical signals, the first telegraph was born and it continued on the next 30 years.

When inventing the telegraph, Bell used a musical or harmonic approach as the best choice, that way several notes could travel simultaneously along the same wire if the notes or signals were on the same pitch. By 1874, Alexander’s research improved a great deal that he informed his father-in-law, Gardiner Greene Hubbard, about creating multiple telegraphs. In March of 1875, Alexander Graham Bell met with the director of the Smithsonian Institute, Joseph Henry, who heard Bell’s conversation about the telephone and Joseph Henry approved of the idea. In 1875, Alexander Graham Bell and Thomas Watson got to work on using a device that would transmit speech electronically, which was about to become a reality. Different tones varied the strength of an electrical current in a wire. In order to succeed, they needed a transmitter with a membrane that would be capable of different electrical currents along with a receiver that could reproduce audible frequencies.

On June 2, 1875, Alexander Graham Bell was experimenting on his “harmonic telegraph” and discovered that he could hear sound over the wire which sounded like a twanging clock spring and then he called his partner, Thomas Watson, in the other room of the hotel that they were staying at telling Watson to come over. On March 10, 1876, Alexander Graham Bell created the first telephone that ended the multiple telegraphs. After that, the telephone became a huge success changing the world on how we, as people, communicate electronically through the air waves.

Alexander Graham Bell not only invented the telephone and telegraph, he also invented the photo phone and phonograph. He was a teacher, teaching the hearing impaired. He also founded AT&T, The National Geographic Society, Bell Labs, Bell System, and Aerial Experiment Association. Alexander Graham Bell went to the University Edinburgh, Scotland in (1864 – 1865) and the University London, London England (1868 – 1870).

On August 2, 1922, Alexander Graham Bell died from pernicious anemia at the age of 75 years old.

Four Tips on How to "Behave" on Your Job Interview

You’ve done it. You’ve made it through the resume jungle. And you’ve finally secured an interview with a real live person. Knowing you only have 60 minutes, if you’re lucky, how do you create an impression in the hiring manager’s mind that you’re the right person for the job? How do you answer questions truthfully to represent the real you? And how do you do all this when the hiring manager is asking really lame questions? It easy-if you know how to “behave.”

“Behave” is a reference to the interviewing technique called behavioral interviewing. For years, behavioral interviewing techniques have helped hiring authorities discover the best candidates for their jobs. Now, using the knowledge of behavior interviewing, you can put your best foot forward during the job interview process. But first, a little background on behavioral interviewing.

Behavioral Interviewing

Behavioral interviewing is based on the premise that the best predictor of future behavior is past behavior. Managers ask questions based on the needs of the job and try to elicit specific examples of previous similar experience in order to project how the interviewee will perform in their position. Questions typically begin with phrases such as, “Tell me about a time when you…” or “Give me an example of how you…”

While the behavioral interviewing technique is extremely valuable, not all hiring managers know of or use the technique. But the good news is, whether you’re interviewing with a manager using behavioral interviewing techniques or a manager who is using less productive interview questions, you will be well advised to follow the points below.

These points are not about getting the job at any cost. That would be a disservice to you and to the employer. You absolutely need to be your authentic self when you interview. But, there is a way to paint a picture of yourself using past behaviors that will more faithfully portray who you are and how you work and create an image in the hiring managers mind about what it would look like to have you on their team.

Tip #1 – Use specific examples

Whether you’re asked a behavioral interviewing question or asked a non-behavioral interviewing, answer the question using specific examples.

o Give example of the projects you completed. If the project had a name, tell the interviewer the name of the project and give further explanation if the project name doesn’t adequately explain what the project was about.

o Provide details about what you actually did, especially if the project was a group assignment. Avoid using the word “we” so often that the hiring manager wonders if “you” actually did anything yourself on the project or merely coasted along on the coat tails of the other group members.

o Talk about what happened on the project-the good, the bad and the ugly. When things go well, life is great for all of us. But the reality is, life doesn’t always work out as we want and employers need to know how you handle circumstances when things go wrong. They want to know how you’ve resolved problems in the past so that they can ascertain if your style will work for them in the future.

o Describe the results. Successful managers are results oriented managers. They want to hear about the end results and what you learned. When you use examples, even if you’re not asked for them, you illustrate in concrete terms what you’ve done and how you’ve done it. The image you create helps that hiring manager envision you in the role he or she is trying to fill. That hiring manager is going to feel better about you as a candidate because you’ve given specific examples. They may not be able to put their finger on why, but they are going to like you better than another candidate who only gave general, generic answers.

Tip # 2 – Be honest

Simple advice? Yes. You will never go wrong being honest. Don’t answer the way you think the hiring manager wants you to answer. Answer with your experience and factors that are important to you. If how you solve problems and how you handle issues when things go wrong doesn’t work in the hiring manager’s company culture, you’re both better off knowing that up front and not taking that job. Trust me, there will be another company that is a good cultural fit for you. You want to hold out for finding that right fit. The right fit with the right company will lead to a better, more productive work experience for you and your employer.

Tip #3 – Prepare

Go through your career and make a list of your work history and the successes you’ve had. Rack your brain and come up with various examples. Remember the problems you had and how you overcame those obstacles. Look for examples of what you did well, of what you were proud of. Look specifically for examples that will appeal to the hiring manager in the industry that you are applying for. Invest time in remembering what you did and how you did it. The time invested will be well worth the effort when you present yourself competently and professional to that hiring manager.

Tip # 4 – Practice, practice, practice

Interviewing is a skill like any other skill. The more you do it, the more comfortable you’ll be in your own skin. Practice with people in your network. If you are attending support groups for job seekers, practice with someone else who is looking for a job. (Be sure to give them a copy of this article before you start.) Practice with friend or colleague. Ask a family member to help. Find a family friend who hires people and ask them to practice with you. Imagining what you will say is very different from actually saying it out loud. You need to hear questions asked and practice answering them aloud. Better to stumble over your words in a practice scenario and rethink how you want to say them than to stammer through the experience in front of the hiring manager.

In an ideal world, employees, colleagues, and bosses would coexist with the most difficult and challenging personalities. However, we know that the ideal world can elude the most well intended among us. Researchers, articulating the importance of personality and its influence on how people communicate, stress that every individual develops a primacy in one of four major behavioral functions: Controller, Analyzer, Supporter, or Promoter. Individuals tend to get along better with those whose communication style is similar to their own. Conversely, we have the most difficult time communicating and relating with someone who is in the exact opposite quadrant as ours. If you know your own communication style and the style of the other person, you are much better equipped to communicate in a manner that seems more open and trustworthy to the other person.
