First: You need to get from somewhere that brilliant idea that will definitely make all of those kids buzz around like bees at honey. So is there a hidden treasure from where you can pop up something for your three pages? Well maybe you could just consider this places: real life (I bet you knew that), other stories you liked in the past, or maybe one that was just as ugly as… The space is for you to fill in. And do you know that the best place to look for ideas is your imagination? Everybody has one, so just brainstorm for a bit. And after that a little bit more.

Second: After you’re done with step one settle the basics of your tale. Find that sympathetic character who definitely faces a problem. The problem is a must have and with that the character needs to be able to fix his problem at the proper time. Keep in mind to write down this steps.

Sperm is produced in the seminiferous tubules and is transferred to the epididymis. This is a tube coiled inside of the scrotum. Azoospermia can be caused by the sperm not being produced or not being delivered due to an epididymal obstruction. Treatment of Varicella-Zoster Infection

Initial infection with the varicella-zoster virus results in a widespread vesicular eruption known as varicella or chickenpox. Reactivation of the same virus results in a more localized infection known as herpes zoster or “shingles.”

Varicella clinical evidence of infection begins with a prodrome of fever and malaise 10-14 days after exposure to the virus. Shortly thereafter, vesicles begin to develop on the central portions of the trunk and face. These vesicles are 3-5 mm in diameter and are surrounded by a thin ring of erythema. The vesicles are randomly distributed, and clustering does not occur. As individual vesicles resolve, new ones appear in a centrifugal pattern, such that vesicles develop on the arms and legs 2-3 days after the trunk lesions have first appeared. A few vesicles may be found in the mouth, but mucosal involvement is not prominent. Vesicles are also almost always present in the scalp. Pruritus is often severe in children, but this is less prominent in adults. As the individual vesicles age, the fluid within them becomes cloudy, and the roof begins to sag, causing an umbilicated appearance. By the end of the first week the early vesicles have become crusted; eventually, all of the lesions heal with little or no scarring. Diagnosis is based on the epidemiology and clinical appearance of the disease. Confirmation of a clinical diagnosis may be obtained through Tzanck smears or by viral culture .

Herpes Zoster

Viral reactivation ( reinfection) in an individual previously exposed to the varicella-zoster virus results in the development of herpes zoster (shingles). In this disease, small vesicles 3-5 mm in diameter occur in clustered patterns. Each cluster consists of 5-20 vesicles; numerous clusters are found in a linear distribution along a single dermatome. The eruption occurs unilaterally with little or no crossover at the midline.

Any dermatome can be involved, but in children and young adults the thoracic dermatomes are most commonly infected. In older adults the first branch of the trigeminal nerve (scalp and upper face) is frequently involved. Individual vesicles have a narrow ring of erythema around them. Where the clustering of vesicles is tight, the erythema becomes confluent, and the vesicles appear to arise from an underlying erythematous base.

The fluid within the vesicles is initially clear, although a small amount of hemorrhage may be noted in some of the blisters. Vesicular fluid becomes somewhat cloudy as the vesicles age. Because of blister roof fragility the vesicles often rupture, resulting in clusters of irregularly shaped erosions.

In adults, pain generally precedes the appearance of the vesicles (sometimes by as much as a week or more) and may continue, long after the vesicles have resolved, as postherpetic neuralgia. In childhood zoster, pain is not usually a significant factor.

New crops of vesicles continue to occur along the dermatome for several days, but by the end of the first week, new vesicles have stopped forming, and resolution of the earliest vesicles has begun.

Atypical Clinical Manifestations

Disseminated herpes zoster can occur in individuals who have depressed cell-mediated immune responsiveness. In such patients, lesions begin along a dermatome, but shortly thereafter, vesicles also begin to appear in other widely disseminated areas. The scattered lesions occurring outside of the dermatome are identical in appearance with those seen in chickenpox. Disseminated zoster is particularly likely to occur in individuals with advanced Hodgkin’s disease.

Patients with facial zoster sometimes develop an accompanying eye infection. This seems particularly likely to occur if the tip of the nose (representing nasociliary nerve infection) is involved.

Patients with varicella or disseminated zoster occasionally develop systemic infection, most usually in the form of pneumonia or encephalitis. Motor nerve infection with accompanying paralysis is even less commonly seen.

Diagnosis of varicella and herpes zoster can almost always be made on the basis of history and physical examination. However, it should be noted that herpes simplex infection can sometimes cause a dermatomal vesicular eruption. In fact, about 10% of zosteriform eruptions of the groin and legs appear to be due to HSV infection. Laboratory confirmation is possible with Tzanck smears and viral cultures .

Course and Prognosis

Varicella (chickenpox) infections resolve spontaneously within 7-14 days. Contagion is unlikely after the first week. Healing generally takes place without scarring, but a few small pits may be left at the site of secondarily infected vesicles. after resolution of varicella, inactive viral DNA remains indefinitely in a latent form within the sensory ganglia. Subsequent reactivation of the virus results in the appearance of herpes zoster. Reinfection from an external source is believed by some to also cause the appearance of zoster, but this must be a very rare event.

The lesions of herpes zoster resolve spontaneously in 10-14 days in children but may remain active for 14-21 days in adults. Scarring following resolution of zoster rarely occurs in children but is occasionally found in adults. Neuralgia neither precedes nor follows zoster in children. Pain before, during, and after infection in older adults can, however, be extraordinarily severe.

Most patients will experience only a single episode of zoster, but approximately 5% of patients will develop one or more recurrences.


The varicella-zoster virus, Herpesvirus varicellae, is a DNA virus of the herpesvirus group. Initial exposure to this virus occurs via droplet spread and results in a viremia with subsequent skin and mucous membrane infection. Viral replication takes place in the nuclei of epithelial cells and continues until the process is halted by development of cell-mediated immunologic response. The antibody formation that occurs seems to be important in preventing internal, systemic dissemination of the disease but apparently plays little or no role in the control of skin lesions. Interferon and other T-cell-mediated responses, on the other hand, are responsible for controlling the severity and spread of the infection within the skin. When the lesions of varicella heal, viral DNA remains in the sensory ganglia indefinitely in an inactive, latent phase.

The virus may be reactivated under a variety of circumstances. Reactivation occurs in the form of zoster. The trigger that sets off reactivation is not known, although depression of cell-mediated immune responsiveness and the occurrence of trauma to a latently infected ganglia or nerve root may play some role. Nerve destruction is greater in zoster than in herpes simplex, and this is presumably the reason for the clinical problem of preherpetic and postherpetic neuralgia.


Acyclovir has recently been approved for the treatment of zoster, and several studies have demonstrated that varicella can be effectively treated also. Certainly, immunosuppressed patients with either zoster or varicella should be treated with acyclovir, but the desirability of treatment in normal, healthy individuals is currently controversial. The treatment of varicella in children does not interfere with the development of a standard antibody response. For this reason, treated children are unlikely to be at greater than normal risk for the development of varicella-zoster complications later in life. It is also apparent that even very early use of acyclovir in zoster has little or no effect on the likelihood that postherpetic neuralgia will occur. The dose of acyclovir for zoster is 800 mg (an 800-mg capsule is now available) 5 times/day for 7-10 days. Types of Diabetes

Before we start discussion about type of  diabetes  we must know what exactly is  diabetes ?

 Diabetes  is a disorder of metabolism–the digestion system of our body for growth and energy. Almost every food we eat broken down to glucose, the form or sugar which is the fuel for our body.

After digestion, glucose passes into the bloodstream, where it is used by cells for growth and energy. For glucose to get into cells, insulin must be present. Insulin is a hormone produced by the pancreas, a large gland behind the stomach.

When we eat, the pancreas automatically produces the right amount of insulin to move glucose from blood into our cells. For the people having  diabetes  this is the place of disorder, there pancreas either produces little or no insulin, or the cells do not respond appropriately to the insulin that is produced.

Type 1  Diabetes  (previously known as insulin-dependent  diabetes )

Type 1  diabetes  is an autoimmune disease. An autoimmune disease results when the body’s system for fighting infection stops in a part of body. In  diabetes , the immune system attacks the insulin-producing beta cells in the pancreas and destroys them. The pancreas then produces little or no insulin. A person who has type 1  diabetes  must take insulin daily to live.

Type 2  Diabetes  (previously known as non-insulin dependent  diabetes )

The most common form of  diabetes  is type 2  diabetes . Nearly 90 to 95 percent of people with  diabetes  have type 2. This form of  diabetes  is strongly genetic. About 80 percent of people with type 2  diabetes  are overweight.

Type 2  diabetes  is increasingly being diagnosed in children and adolescents. However, type 2  diabetes  in youth are not in common.

When type 2  diabetes  is diagnosed, the pancreas is usually producing enough insulin, but for unknown reasons, the body cannot use the insulin effectively, a condition called insulin resistance. After several years, insulin production decreases. The result is the same as for type 1  diabetes –glucose builds up in the blood and the body cannot make efficient use of its main source of fuel.

Gestational  Diabetes : (Gdm)

Gestational  diabetes  develops only during pregnancy. Like type 2  diabetes , it occurs more often in African Americans, American Indians, Hispanic Americans, and among women with a family history of  diabetes . Women who have had gestational  diabetes  have a 20 to 50 percent chance of developing type 2  diabetes  within 5 to 10 years.

Obesity and Self Esteem

Why does obesity exist (when not caused by medical problems)? This question has been asked by many in the medical and psychiatric profession worldwide.

The reason obesity exists is in all honesty very simple. If it is not caused by a medical condition, it is because folk have eating disorders such as eating too much, not eating enough, eating in an unhealthy fashion, bingeing or stuffing themselves. Comfort eating, eating high-calorie foods, eating too much fast food – there is an abundance of cheap food available. Then there is the mind of the person. The mind is a great controller of an individual’s emotions and how they see themselves, and it can create low self-esteem.

The advancement of technology has produced televisions, game consoles and the computer. These all entail sitting positions, so if the person works in an office he or she is sitting down, and when they get home they are sitting down. What exercise did they get during the day? Just to walk to the toilet or the canteen at break time. Oh they ate where? Ah yes, in a fast food outlet. No wonder there is an obesity epidemic. The world community is geared up to be obese, and it all began in the 40s. Since then mankind has poisoned the ground, the rivers, the lakes, the sky, the very air we breathe.

Each piece of food we eat has come into contact with some kind of food enhancer or preservative, and none of this is good for the human body. When you’re sitting in the comfort of your home watching all these adverts for food from all the high street fast food chains try to remember they have known of this poisoning since the 40s and they did not tell you. Their food is truly poison to your insides. Just look at the rise in cancer. When did it begin to become so prevalent? Yes in the late 40s, and now one in three people will experience some form of cancer in their life due to the neglect of governments who should have been looking after their people’s welfare.

It is time to sort out this childhood and adult obesity problem, and begin a program of education for parents, schools and any organization dealing with obesity. There is no time to lose. It is very important to note that the loss to production caused by obesity last year worldwide was $300 billion. Add to that the cost of health services, and the figure is a mind boggling $575 billion.

It is time to avoid all processed foods as they are too high in calories and preservatives. Do not allow your children to eat in any fast food outlet.

Take responsibility for your children’s health and take the same responsibility for your own health; lead by example. I believe that the majority of obese people can be helped to lead healthy lifestyles by proper diet plans and an exercise routine which begins easily and gets more effective as time goes by. It is very true to say that if you’re obese there is only one way to stay healthy and that’s to have a mind-changing attitude, and if you have a God in your life then use that God to assist you. Also, it is a fact that once you have become obese, fat or overweight, you need a daily program of healthy living to keep off the weight, and if you relax on your program the weight will go back on. So be aware of this and don’t let it happen to you. You’re worth more. Be good to yourself and help yourself to gain a healthier lifestyle, starting now.

So You Want To Be An Endurance Rider

Since I began endurance riding about 10 years ago I have been asked many questions. Many of these questions had to do with which rides do I like best, how to do something and in general things you could find in a book or by trial and error. Recently, I had the fortunate experience of introducing someone new to the sport of endurance riding and was asked the toughest question yet, “Why?”

I began to realize that putting this into words would take some time to collect my thoughts. Sure we all know “why” we love it, but when you have to put it into words, how can we do it justice?

My family has asked me this very same question. Some of them think I am crazy (and the others know I am). They just don’t understand why, why I enjoy spending endless hours in the saddle, sometimes not in the greatest weather conditions just so I can test my (and my horse’s) endurance. I think of riding my horse on top of mountains formed in the last ice age with a view stretching for miles. How the clouds slowly approach against the backdrop of blue while the trees sing when a gentle breeze blows through them. Listening to the distant call of a bird gliding along like a surfer rides a wave. Watching the sun rise, and set. How when you ride under the stars at night on a 100 you feel a peace that can’t be explained.

As an endurance rider I love my horses, and I love the outdoors. If you gave me a choice of going to a concrete theme park or to go hit the trail with my horse, the trail would win hands down. If you give me a choice of staying at a 5 star hotel or sleeping in my trailer, my trailer would win hands down. I also truly love taking care of my horses, feeding them, cleaning up after them, and tending to their every need. I love watching them turn into the incredible athletes that they have become.

My curiosity makes me need and want to see what is around the next bend or up the next hill, and it doesn’t matter how many times I do the same ride every time it is different. The same things won’t be around the same bend each and every time; sometimes the bend is even in a different spot. I like the knowledge that what matters is the experiences I take home with me.

Then there is always the adventure that keeps me coming back for more. The simple knowledge that many of these great places can only be seen by a few. I enjoy the challenge of getting to a certain place with my horse. It is worth every step, bug bite and any other discomfort along the way just to know that they will soon be forgotten, with the first aah or wow of the view or the satisfaction of completing the ride. I love the flowers and the trees that surround me. I love the way being out there makes me feel, refreshed relaxed and carefree. I love the smell of the woods, the dirt under my fingernails, and the scent of horse sweat while enjoying the rhythm of his hoof beats.

Why do you want to be an endurance rider? For me it was not an easy question to answer, and I couldn’t answer it with only one phrase, such as I love riding my horse. It goes beyond words; it’s the feelings and the enjoyment of spending time with my horse, the great outdoors and the competition.

Endurance riding is a sport that has something for everyone. You can ride alone, with your friends or family. You can start with a horse you already own or search for the ideal prospect. Most riders participate for completion rather than placing, for them riding 50 or 100 miles on a sound horse is satisfaction enough. The greatest reward of all is the bond you will form with your horse and the great feeling of accomplishment you will feel when you work together as a team.

Diaper Rash And the Way I Handled It

I thought I was the perfect mother. Changing diapers after every nap and before every feeding and often between. Making sure to clean thoroughly with wonderfully scented baby wipes each time. The baby’s bottom was beautfiul. Notice I used the past tense. And this was baby number six! As foster parents we had cared for our share of baby bottoms. Then it hit. Diaper  rash . It came so quickly. And I had no idea how to cope.

I received advice from every corner of the globe. And the funny thing is even though everyone had their own idea, they were all good and they all worked. Hmmmm….

My mom praised browned flour.

My friend swore by Eucerine.

My aunt said only A & D worked for her babies.

But yet another foster mom swore by Butt Paste!

And to top it off they were handing out samples of yet another diaper  rash  cream at my local store!

What did I do?

After panicking and calling the advice nurse I analyzed the situation and applied my attack plan.

The  rash  comes about because tender baby skin is being exposed to irritants. Sometimes it’s an allergic reaction to a new food or the wipes themselves. Sometimes the brand of diaper is the culprit. Many reasons can bring on diaper  rash  and many solutions can clear it up.

Now I keep a variety of things on hand and here is my personal attack plan for hands on healing.

1. Mix up solution of distilled water, 2-3 drops of *tea tree oil and a dash of castille soap. Soak papertowels in this for diaper wipes.

2. Change baby diaper very often and try to give a good airing if time permits.

3. Slather on zinc diaper ointment and then whatever heavy duty moisturizer I have on hand (vaseline, Eucerine) over this. If it was bedtime add a diaper liner for good measure.

Some other tips I use:

Keep Clotrimazole on hand for yeast  rashes .

Keep a handy chart of the different types of diaper  rashes 

Call the doctor is improvement isn’t seen in 24 hours.

With these methods I don’t fear the dreaded diaper  rash  any more.
