Menopause is an uncomfortable process that each woman must go through at some point in her life. In particularly bad cases doctors may prescribe hormone therapy, but they usually encourage women to treat each symptom individually as it comes.

Hot flashes and night sweats are the worst side effects of the hormonal changes a woman undertakes when moving past her age of fertility. The reason that hot flashes come about is that very drop in estrogen and progesterone. The hormones are what tell our brain whether it is hot or cold. When the hypothalamus gets mixed messages about how to react to temperature, the body oscillates between feeling “hot” and “cold.” The blood vessels expand and contract, increasing the blood flow and causing that first facial flush that triggers the hot flash. Believe it or not, body temperature can rise three or four degrees in just a few minutes!

The blood then moves to the chest and the remainder of the body, causing the woman to sweat profoundly for a few minutes and then stop. Many women have chills following their hot flashes. A hot flash that happens during the night is a night sweat. Many menopausal women wake up cold and covered in sweat, as they can do less to manage the hot flash in their sleep.

During the day, dealing with hot flashes is much easier. Wearing layers can help to some extent, and washing your face with cool water can also bring down your temperature. To deal with night sweats you should wear light pajamas and either use light, thin sheets or layer your blankets in the winder. Have a bottle of ice water and towel or cold compress at your bedside to help you cool down when you awake. And have a fresh pair of pajamas to change in to in the case that you sweat through yours.

All About Bronchitis

What is Bronchitis?

Bronchitis is a term that means inflammation of the lining of the trachea and bronchi which are the tubes leading from the throat to the lungs. The mucus lining of the trachea, the bronchi and the lungs becomes reddened and swollen due to the inflammation. This in turn causes increased mucus production leading to cough and sputum, shortness of breath and tightness in the chest. Depending on the severity of symptoms, it may be diagnosed as either Acute Bronchitis or Chronic Bronchitis.

What is Acute Bronchitis?

Acute Bronchitis is seen in both children and adults. A child with acute bronchitis may initially exhibit signs of common cold such as a runny nose, sore throat and mild fever. Dry cough develops a few days later which turns moist in a day or two. There may be slight wheezing too. Acute bronchitis in children may not need medical treatment and generally goes away on its own in 10 days to a few weeks. Adults may experience similar symptoms along with thick yellow or greenish mucus and pain when they cough, body pains and chills.

What is Chronic Bronchitis?

Chronic Bronchitis, unlike acute bronchitis, is a condition where bronchitis goes on long-term, often for months and even years. Symptoms like coughing and mucus production can return each year and can last longer each time they reappear. Chronic bronchitis does not go away on its own and is a serious respiratory disease that needs professional medical attention and care.

Infectious bronchitis, another term used for acute bronchitis, is predominantly inflammation of the airways. But when it progresses to become chronic, the predominant change is that the irritated and swollen lining of the bronchi start leaking mucus into the lungs. The small hair like protrusions in the lungs called the Cilia that move the mucus out of the respiratory tract now fail to move the excessive mucus. As a result, the mucus settles down and worsens the infection.

Smoking and Increased Risk of Bronchitis

Among adults, smoking is the most common cause of chronic bronchitis. Children with common cold and cough who live around a smoker are more likely to develop acute bronchitis. For a smoker, a bout of acute bronchitis can be much harder to recover from when compared to a non-smoker in good health. The likelihood of it progressing into chronic bronchitis is also much higher. Chronic bronchitis causes the mucus membranes of the trachea and the lungs to remain swollen and irritated, and over time, cause the cilia, small structures in the lung that move out mucus, to be permanently damaged. Further, the constant presence of mucus in the lungs may result in scarring, distortion and irreversible damage to the airways in the lungs.

Prolonged, untreated chronic bronchitis can become serious in people who smoke and elderly people leading to other diseases such as COPD (chronic obstructive pulmonary disease) and emphysema among others. Those with chronic illnesses of the heart or lung disease also become vulnerable to other viral and bacterial infections.

Other Causes of Bronchitis

Almost 90% of cases of acute bronchitis are triggered by viral infection whereas a mere 10% are triggered by bacterial infections. Chronic bronchitis is usually caused by long-term inhalation of irritants, such as cigarette smoke and chemical pollutants. People who live or work in polluted environments and are exposed to chemical pollutants such as miners are more likely to develop both forms of bronchitis.

Facts About Ski Trips: Common Skiing Injuries

Changes in modern ski equipment, improvements in slope design, and maintenance helped reduced the incidence of serious injuries. However, we must take note that there are still a significant number of ski injuries that can possibly occur during your ski trips.

According to the American College of Sports Medicine (ACSM), studies have showed that major of injuries that occur to skiers are sprains, fractures, lacerations and dislocations. At present, injury to the ligaments of the knee is a more common injury that occurs to skiers.

Several studies have shown that the most common injury to the knee is the damage to the medial collateral ligament (MCL). Physical therapist Jennifer Lewis of Athletes’ Performance in Phoenix, Arizona explains that “MCL injuries occur when a sudden force or twisting motion is applied to the outside of the knee forcing the knee inward, most often when the foot is planted on the ground.” Usually, all degrees of MCL sprains can be managed with bracing and limited range of movement.

Another type of knee injury is the damage to the anterior cruciate ligament (ACL). ACSM explains that the injury occurs when the lower leg is suddenly twisted away from the upper leg as in “catching the edge.” Usually, this injury is triggered by a backward fall as the lower leg moves forward. This type of injury often requires surgical repair and extensive rehabilitation.

According to ACSM, 30 to 40 percent of all injuries are caused by the injuries to the upper extremities. The most vulnerable part of the upper body is the thumb. Injury to the ulnar collateral ligament of the thumb occurs when a skier falls on an outstretched arm that is still gripping the pole. The thumb is suddenly pulled outward, injuring this joint. Sprains of the thumb are graded as first degree, second degree or third degree depending on how severe the damage to the ligament is. It may just be a thumb, but when an injury to a thumb is not attended to properly, it can result to long-term disability.

Another injury in the upper extremities is the dislocation of the shoulder. After rehabilitating the dislocated shoulder, protection of the joint to avoid recurrence is important. Despite rehabilitation programs design to strengthen the shoulder, dislocation is still possible. Surgical repairs are often necessary to repair dislocated joints to restore its functional state.

They also recommend that injuries should be properly evaluated and treated by appropriate medical personnel. With the enhanced equipment used in skiing, the risk of getting serious injuries on ski trips is minimal. However, let’s not forget that certain injuries can still happen if we do not observe proper safety measures.

Types and Treatment of Shoulder Ligament Injuries

Ligaments hold one bone to another bone and limit the amount of motion available in the joint. This prevents excess movement or motion in directions not intended for a particular joint. The tighter the ligaments are, the less motion available. Because the shoulder is a highly mobile joint, the ligaments must be loose to allow motion in all directions. This creates some inherent instability in the joint, and an avenue to potential injury.

Anterior Ligament Sprain/Dislocation

This injury usually occurs when we raise the arm overhead or out to the side and apply a force, such as with retrieving a heavy object from an overhead shelf. If the load is too great, then the ligaments in the front of the shoulder become overloaded and can tear, causing a sprain. If the ligaments tear enough, then the bones can separate and cause a dislocated shoulder. Dislocations should get immediate medical attention in the emergency room. Treatment involves applying traction to the joint, which allows the shoulder to return to its normal position and then a period of immobilization to allow the ligaments to heal.

Unfortunately, the ligaments do not heal quite as tightly as they once were. The shoulder becomes increasingly unstable, disposing it to another dislocation. Conservative treatment following a dislocation involves strengthening of the rotator cuff, a group of four muscles that provide additional stability to the shoulder. As you move your arm through space, the rotator cuff sucks the head of the arm bone, medically termed the humerus, into the socket. After a dislocation, it is even more important to keep the rotator cuff working properly. You may be referred to a physical therapist, who can provide instruction in the proper exercises.

Should the shoulder continue to dislocate, surgical intervention may be required. This entails tightening the shoulder capsule by “pulling up the slack” in the loose ligaments and stitching them back in place. This surgery is highly successful at stopping future dislocations, but there is a period of immobilization and rehabilitation for several weeks following the surgery.

Shoulder Separation

This is a common traumatic injury caused by falling on an outstretched hand. With this injury, the force of landing on the hand is transmitted through the shoulder, causing a tear in the ligament that holds the collarbone in place. The result is a dislocation of the joint where the collarbone meets the shoulder blade, located at the bony area on top of the shoulder. This joint is called your acromioclavicular, or AC joint, and because this is the pivot point where the shoulder blade rotates, an injury here can cause significant loss of function in the arm, especially in the overhead range of motion. Minor sprains in the ligament may heal, but active individuals who have difficulty or pain with arm use may need to undergo surgical correction, as conservative treatment usually will not restore proper mechanics and movement patterns. Surgery involves harvesting a tendon from elsewhere in the body and using is as a replacement for the broken ligament. This effectively anchors the collarbone back in its correct position.

Frozen Shoulder

Medically termed adhesive capsulitis, this condition may be a result of injury to the shoulder, but just as often occurs without any type of trauma. It is an inflammation of the shoulder joint capsule, which contains the shoulder ligaments. As the capsule becomes inflamed and thickened, the shoulder becomes painful and loses significant mobility. Frozen shoulder may persist from several months to a year or longer and usually follows a predictable pattern of presentation that includes three stages.

1. The acute stage is marked by sharp pain in the shoulder throughout the range of motion, but especially with reaching overhead and out to the side. Shoulder mobility becomes limited.

2. During the frozen stage, acute pain starts to subside, but mobility of the shoulder continues to be limited.

3. The final phase is the thawing phase, where the joint mobility of the shoulder begins to improve, and functional use returns.

Although frozen shoulder can afflict a wide range of people, there are certain risk factors that may dispose someone to getting this condition including diabetes, cardiac disease, and hypo or hyperthyroid issues. Treatment is usually non surgical, as the condition usually gets better with time. During the acute and frozen phases, anti-inflammatory medications may be prescribed, as well as a steroid injection directly into the joint capsule, which significantly limits the degree of inflammation. Physical therapy may be ordered in order to learn some gentle mobility exercises, followed by more aggressive stretching and mobilization of the shoulder as the condition progresses into the thawing phase.

Symptoms of colles’ fracture include wrist pain and swelling and it will be difficult for a person with this fracture to hold or lift heavy objects with the hand. This article will take a look at the medical symptoms of colles’ fracture of the wrist and explains how it is treated.

Colles’ Fracture Symptoms And Causes

Symptoms include:

  • Pain
  • Swelling
  • Tenderness and bruising on the thumb-side of the wrist
  • The hand may appear to be turned or displaced outward in some cases

Colles’ fracture is generally caused by trauma to the wrist often involving a fall on an outstretched hand or a direct trauma to the wrist. This condition can happen when a person attempts to break a fall during sports such as rollerblading, skateboarding, or other high-speed activities. In older adults any fall on an outstretched hand could result in this fracture.

In small children, bones are soft and tend to result in an incomplete fracture on one side of the bone, this may be termed a torus, buckle or greenstick fracture. In elderly, the bones are brittle and a complete colles’ fracture is more common.

Colles’ Fracture Diagnosis And Treatment

Diagnosis involves the observation of the aforementioned symptoms following an injury or fall and the condition should be evaluated by a physician. The evaluation may include a physical and x-ray examination.

First aid or emergency care should include immobilization of the wrist in a normal resting position. The application of cold packs or ice is recommended to control inflammation. If the wrist is displaced, do not attempt to move or straighten it. The person should be taken to a medical care facility.

The fracture may need to be reduced in order to align the bone ends to allow proper healing. If small fragments of bone are involved, surgery and the use of pins, screws, plates, wires, etc., may be necessary. Following reduction, the fracture site is immobilized with a cast or splint.

If you have a teen or young adult who has fallen on an outstretched hand during a sporting event and now has difficulty holding or grasping a heavy object with the involved hand. Or the person in question is an elderly person who tried to break a fall with hands extended and is now experiencing pain and swelling in the wrist, a colles’ fracture should be suspected and an appointment for medical care should be scheduled.

I Broke My Wrist!

OK, I didn’t break my wrist, but I treat many people who have sustained such an injury. Due to the common nature of the injury, I have prepared answers to the common questions that arise in such situations.

What is a Broken Wrist?

Though there are 10 bones that make up the wrist, and each bone has the potential to fracture (medical term for broken bone), by far the most common broken bone in the wrist is a fracture of the end of the radius bone.

Typically, a patient sustains a broken wrist from one of two scenarios; one is that they were in an accident, or fell with such force on their hand that the otherwise normal wrist crushed under significant forces. The second scenario involves one with less than ideal bone density who falls on an outstretched hand in a way that causes the bone to break. The difference between the two is the difference in the quality of the bone and the amount of force required to break the bone. The distinction between the two mechanisms is significant as the patient with higher quality bone often has multiple injuries due to the significant force of the injury, and in the patient with poorer bone quality, a thorough bone quality investigation should be considered.

How are Fractures to the end of the Radius evaluated?

X-rays and a physical exam can usually give the information needed to evaluate a distal radius fracture. Occasionally, further imaging such as a CT or MRI may be considered, but this is rare. In addition to the bone, there is certainly injury to the surrounding soft structures such as tendons, ligaments, muscles and nerves which will affect your outcome.

How is my broken wrist going to be treated? Do I need surgery?

Many factors go into deciding the best treatment for your wrist fracture, but typically I choose the treatment regimen which will allow the best recovery potential, both in the short term and in the long term. This usually initially involves an attempt at manipulating the fracture into the most ideal healing position and holding that position with a splint or cast. If the wrist fracture falls into an unacceptable position for healing despite maximal non-operative care, then surgical intervention is considered. With experience, training and careful review of current research, I can often predict which fractures can be treated without surgery and which ones will need further surgical stabilization.

How long is the recovery from my broken wrist?

Most patients, regardless of the type of break, have very good long term results. Most fractures treated without surgery will require 6 weeks in a cast, and another 6 weeks to regain most of the strength that was lost. Distal Radius fractures which require surgical stabilization often only need 2 weeks of splinting, followed by 4 weeks of exercise to regain range of motion and another 6 weeks to regain strength. Most patients are able to do most activities by three months after the injury, but individual circumstances certainly vary.
