With a grade 1 sprain, there is only slight stretching of the fibers with minimal damage to the ligament. With a grade 2 sprain, there is partial tearing of the ligament and laxity (looseness) of the ankle joint. An ankle sprain is a grade 3 when there is a complete tear of the ligament.

How is it treated?

If you have a sprained ankle, proper treatment and care is necessary for it to heal correctly. Most ankle sprains only need rest in order to heal while others require casting, a special splint or boot, or even surgery.

For a grade 1 ankle sprain, you should use RICE (rest, ice, compression and elevation):

REST – Do not walk on your ankle for a specified amount of time. Use crutches to get around.

ICE – Apply ice wrapped in a soft cloth to the ankle. Do this for 20 minutes, four or five times a day to reduce swelling and ease pain.

COMPRESSION – Use an ace-wrap type of dressing to immobilize the ankle and give it support.

ELEVATE – Raise your ankle above the level of your heart. This is especially useful during the first 48 hours after the injury.

If your doctor tells you your sprain is a grade 2, the RICE method should be observed. Also, this level of injury takes more healing time. In addition, the orthopedic specialist may use a device to splint or immobilize your ankle while it heals. Grade 3 sprains are often associated with long-term instability. While surgery is rarely necessary, a short leg cast or brace could be required for around three weeks. To prevent chronic ankle problems, the doctor may order physical therapy, ultrasound, or electrical stimulation.

A Grade 3 sprain can be associated with permanent instability. A short leg cast or a cast-brace may be used for two weeks to three weeks. If your ankle does not heal with conventional nonsurgical treatment and persistent instability exists, the orthopedic specialist may recommend surgery.

Reconstructive surgery involves repair of the torn ligament and grafting with other ligaments. Arthroscopic surgery is done when the doctor needs to look inside the joint for loose bone fragments or pieces of cartilage.

What Are the Best Treatments for Whiplash?

When it comes to whiplash, there are multiple treatment options available depending on where the injury occurred and the resultant symptoms. With some perseverance, dedication, and a positive attitude, treatment results will be optimized.

It is very common that whiplash victims don’t experience the pain right away. It may take a few days to a few weeks for the discomfort to show itself. This can be from adrenaline causing you to not feel any pain.

If you were transferred to the emergency room after the accident, the workup will be to rule out something very acutely serious, such as a fracture or neurologic injury. Once those are ruled out, then the typical diagnosis comes down to a Cervical Strain/Sprain. This can mean a bunch of different things.

Acutely, treatment should consist of some common things such as:

1) Ice, not heat – the objective is to decrease inflammation, and ice may constrict blood vessels and limit the swelling and inflammation. Hence pain may be decreased as well.

Pain medication – Can be very effective in the acute setting for pain relief. It is not recommended for chronic intake.

Muscle relaxers – these medications include Valium, Skelaxin, Soma, Flexeril, etc and act to counter muscle spasm.

NSAID’s – anti-inflammatory medications like ibuprofen and naproxen may help with whiplash pain.

Additional medications that may be considered include neuropathic medications like Lyrica or Neurontin, sleep aids like Ambien or Restoril, and anti-depressant medications such as Paxil or Prozac.

Sixty percent of chronic whiplash pain has been shown to be coming from the facet joint capsule. Pain Doctors have had good results utilizing cervial epidural steroid injections (ESI), trigger point injections, and radifrequency ablation (RFA). RFA gives much longer pain relief typically than an ESI. The medial branches, which are the tiny nerve endings supplying the joint capsule with sensation (and pain) usually regenerate between 270-400 days, so patients may achieve over one year of pain relief.

Whiplash injuries respond typically best to an active and comprehensive therapeutic intervention. The “wait and see” approach may end up in a missed window of opportunity for making great strides in pain relief. Strong evidence = Stretching/Strengthening along with Spinal manipulation

Moderate Evidence = Stretching/Strengthening, IV steroids for whiplash, and epidural injections. Low level laser therapy, electrotherapy (TENS), Intermittent traction (like spinal decompression), and acupuncture.

Limited Evidence = Magnetic stimulation, Chinese massage, trigger point injections, and orthopaedic pillow.

Biliary Dyskinesia – To Cut or Not to Cut?

Many people, including children, may experience attacks of pain at the right upper quadrant. This is the kind of abdominal pain that sometimes radiates to the upper back or shoulder blades. Usually it occurs after eating fatty or fried foods, heavy meals and is accompanied with nausea/vomiting and/or bloating, followed by loosening of stools.

Typically, even when you have all this symptoms, the tests results may come in normal, which would leave the doctors thinking that the disorder you contain is called biliary dyskinesia. “Biliary” means bile, “dys” means abnormal, and “kinesia” means motion, movement. Abnormal moving of the bile can occur in the gallbladder, in the bile ducts inside and outside the liver, and also in the valve between bile duct and duodenum; the first part of the small intestine. This valve is named sphincter of Oddi. In the medical literature doctors consider biliary dyskinesia and type 3 sphincter of Oddi dysfunction, as similar conditions.

If tests reveal no structural abnormalities, there are no severe inflammations or gallbladder stones biliary dyskinesia is often misdiagnosed with IBS, functional dyspepsia, stomach flu, reflux, psychosomatic diseases, etc. When patients are given the medication, it modifies or covers the symptoms. More specific tests may show low ejection fraction that means low gallbladder function; <35% EF is considered abnormal. When the detailed tests reveal that patients have a low ejection fraction, they are referred to the surgeons’ for gallbladder removal.

So, the question remains… to remove or not to remove the gallbladder in case of biliary dyskinesia?

The excess build up of pressure in the bile ducts or irritations are thought to be accountable for these typical biliary dyskinesia symptoms. Unfortunately, there are many other reasons.

On one hand, gallbladder, liver, bile ducts, sphincter of Oddi, stomach, and duodenum work together as a team. Nervous system and digestive hormones (blood messengers) conduct their interrelated work. Very often, it can be a setback with proper regulation of the digestive organs.

On the other hand, bile is extremely complicated solution that consist of water, minerals, bicarbonate, cholesterol, lecithin, and bile salts. It also has highly aggressive bile pigments, and bile acids, various fat-soluble toxic substances. Changes of biochemistry of the bile make it very aggressive and irritating fluid. In turn, it can cause spasms and irritation of the gallbladder wall.

A number of parasites, which reside in the gallbladder and bile ducts, are difficult to spot. For example, Giardia lamblia can be responsible for some cases of the biliary dyskinesia and type 3 sphincter of Oddi dysfunction, in especially in children.

It appears that hepatitis, fatty liver, Candida-yeast overgrowth, parasites, congestion, inflammation, infection, high acidity levels, and poor eating habits can cause the bile to be thick, acidic, and aggressive. Such things make it difficult for the bile to move through the ducts. Toxic substances in the bile, parasites, irritation, and food allergy can restrain the proper motion of the gallbladder, bile ducts, and sphincter of Oddi.

Whole body acidity can change the bile pH. Because of precipitation of the extremely aggressive bile acids, acidic bile irritates gallbladder, bile ducts, sphincter of Oddi and duodenum causing spasmodic contractions with pains. Aggressive, acidic bile irritates small intestine and even stomach, causing nausea and vomiting. I explained this in more detail, in my EBook and articles.

By putting in account scientific research, clinical evidence and common sense, the treatment in case of biliary dyskinesia should be started with the non-invasive approach. Some methods of the complementary or alternative medicine can be particularly useful in this situation. Let’s focus on some of them.

First, what we need to do is to make bile more liquid and less congested. It can be done by drinking plenty of water, fresh vegetable blends or juices, eating vegetable soups, drinking Karlovy Vary healing mineral water, herbal teas, etc. Avoiding dehydration is mandatory.

Second, it is necessary to promote liver to produce more bile. Some herbs such as barberry, rosehips, fennel, corn silk, peppermint have choleretic actions, which lead to producing more bile. Opposing, animal fats, trans fats, combination of fats and sugars, alcohol, unhealthy eating habits such as eating on the go, irregular meals, overeating leads to gallbladder congestion and, in case of biliary dyskinesia, needs to be avoided.

Fifth, this is virtually unknown even for health professional. The bile’s acidity and toxicity are primary factors for biliary dyskinesia. 3 Ways to Slow Down Bleeding Caused by Menstrual Cycle

The function of the gallbladder in the digestive process is great. Another huge source of estrogen are xenoestrogens (also spelled zenoestrogens) which are substances that mimic the natural hormone. They are found in pesticides, herbicides, some chemical solvents, petroleum and exhaust fumes. Then, there are the plastics, everything from PVC pipes to food wraps. Avoid heating food on or covered by plastics. Avoid hot beverages in plastic. Keep plastic water bottles cool to reduce the risk of plastics leaching into the contents.

Complex Ovarian Cyst – Are They Harmful Or Not?

The differences between cysts can be dramatic such as a complex ovarian cyst which are less common and more bothersome. This type has both the solid and liquid components.

There are three types of these kinds of cysts; these are-

(1) Dermoid cysts when there are tissues of the teeth, skin and hair in the ovary. They usually cause no symptoms and they are often small and undetected. When at some point they grow and become large it can cause a rupture and bleeding which is a medical emergency

Pain is the most subjective symptom of this kind of cyst. The woman experiences pain during and after menstruation, pain during intercourse and even pain upon voiding and bowel movements

(3) and another type that affects the ovarian tissue is the Cystadenomas.

A complex Ovarian cyst often causes pain and the signs and symptoms can obviously be observed as the size increases and these symptoms are usually the same as those of endometriosis and ectopic pregnancy. The pain usually radiates from the pelvis to the buttocks and thighs. To diagnose or to rule out these cysts, the physician will perform an ultrasound, a manual pelvic examination and transvaginal ultrasound.

Blood test such as CA 125 is done to test for cancer and endometrioma. CA 125 (Cancer antigen 125) is a tumor marker. These are found in a large amount of cancer cells in the body. In relation to an ovarian cyst, CA 125 has greater concentration than any other cells. The normal value for CA 125 is 35 U/ml. What does the results mean? With this type of cyst, if the result of CA 125 is elevated this means that there is tumor recurrence while on the other hand, if the result is decreased it means that the therapy which includes chemotherapy has been effective.

If the cyst is found to be the complex type, it must be further evaluated by the physician. To be able to know if the cyst is not cancerous, the physician will order a series of tests. They would take into consideration the patient’s age and symptoms before making their final diagnosis.
