What Is a Microkeratome and Why Is It Used in Laser Eye Surgery?

If you have looked into having Laser Eye Surgery (Lasik) you have probably heard of an instrument called a microkeratome. Have you wonder what exactly the microkeratome is and what role it plays in Lasik? These are excellent questions and many people never fully comprehend the answers.

To understand completely, it is helpful to look back at the early genesis of Laser eye surgery. In the beginning, these surgeries were called PRK (photorefractive keratectomy) and they did not involve a microkeratome. An excimer laser was used to directly treat and reshape the outer surface of the cornea in order to change the optics of the eye.

Optically PRK is quite effective but there is a major drawback. When PRK is performed, there is a large bare covered area on the front of the cornea which is like a huge scratch on your eye. If you have ever had a scratch on your cornea, you know that it can be excruciatingly painful and can take some time to heal.

In the case of PRK, it usually takes about 3 days for the outer surface epithelium of the cornea to close and it takes some time thereafter for the swilling and healing to stabilize. Typically, a bandage contact lens is placed on the eye for 3 days to protect it and to increase comfort.

Lasik was developed by combining PRK with an older technology, the microkeratome, from another kind of eye surgery called ALK. The purpose of combining these techniques was to reduce the recovery time and increase comfort compared to PRK.

The microkeratome is able to cut a thin layer of tissue off of the surface of the cornea. This layer can be hinged and carefully lifted and folded back out of the way. Imagine it like a wall calendar where the previous month can be folded back to expose the next page underneath. Once this flap of tissue is out of the way, the excimer laser can optically treat and reshape the underlying cornea. Once finished, the flap can be replaced to cover the treated cornea.

The flap takes the place of the bandage contact lens of PRK and acts as a natural tissue bandage, if you will. It allows the surface of the cornea to remain practically intact with no large scratch on the surface. Vision returns quickly and often patients can see 20/20 by the next day. Pain is also usually minimal and limited to the early hours after the procedure. This flap is delicate at first and patients are advised not to rub their eyes in the early days following Lasik, but over time it heals and seals down so that patients have no functional limitations.

The early microkeratomes actually used physical razor blades and high speed oscillating mechanisms to work. In the last few years, they have evolved so that no blades or moving gears are needed. A special laser called the femtosecond laser can create a flap with far greater precision and accuracy than the older instruments.

These laser microkeratomes also have a much lower incidence of flap related problems like free cut flaps, button-holes in the flaps, or metal fragments / oils dragged benefit the flaps. Because of their design, the surgeon also has much better control because he can actually see the flap being created and even adjust its size and shape on a computer screen as needed to customize it for the eye of any individual.

In summary, the use of the microkeratome in Laser eye surgery has greatly influenced the procedure allowing greater patient comfort and faster recovery compared to PRK. The evolution of the femtosecond laser microkeratome has taken the procedure light years ahead with greater safety and an incredible level of precise control that allows us the flexibility to customize Lasik flaps in ways that were inconceivable before.

The food is taken into the mouth where it is chewed, mixed with saliva which is secreted by glands in the mouth. After mastication a portion of food is collected on the tongue. It is then swallowed and the opening to the larynx is closed off by a small flap of tissue called the epiglottis. This flap prevents the food from entering into the respiratory tract. The food passes from the esophagus into the stomach, contractions of the muscular wall of the stomach ensure further mixing and mechanical disintegration of the food, which at the same time is mixed with the gastric juice secreted by the stomach.

The stomach has a mucous membrane lining and is protected from the gastric juice which contains enzymes and hydrochloric acid. The food is broken down into smaller particles for easier absorption into the bloodstream and the rest of the food passes into the small intestine and duodenum through the pyloric sphincter valve, a muscular ring at the end of the stomach and intestine.

In the small intestine, rhythmic movements of the walls churn up the food and mix it with the digestive juices, which include pancreatic and bile juices (The bile is produced by the liver and stored by the gall bladder) further digestion then takes place as the result of the action of enzymes present in these juices.

Absorption of the digestive nutrients takes place mainly through the mucus membrane of the small intestine. The rhythmic movements of the intestine force the unabsorbed food, and waste matter excreted from the blood into the large intestine, here the water is absorbed by the walls of the intestine and partially dehydrated material passes to the rectum from which it is excreted from the body through the anus as faeces.

Acid Reflux Diet Not As Effective When There Is Mercury Toxicity From Silver Fillings

Although the nutrient-dense, acid reflux diet is a major step to healing and building optimal health, removing amalgam fillings (silver fillings) is extremely important as they are a major source of heavy metal toxicity.

What most people do not realize is that the largest portion of metal used for fillings is mercury–at least 54% mercury! The mercury is mixed with other metals (copper, silver, tin and zinc) that actually increase its toxicity through galvanization–an electrical current produced from two or more dissimilar metals, thereby increasing the rate of mercury corrosion from 10-20 times! So while the teeth are protected from further decay, the resulting heavy metal toxicity may be devastating to health and be a serious block to overcoming acid reflux.

Mercury is extremely toxic in very minute amounts, and has been linked to many neurological diseases such as Alzheimer’s and Multiple Sclerosis. Heavy metals migrate to fatty tissues like the tissues of the brain and the myelin sheath that protects nerve cells. Mercury is a “cytotoxin.” It is poisonous to all living cells and can bind with any molecule called a “sulfhydrl”–which is found in most proteins–the building blocks for all tissues. As a result–mercury can interfere with virtually any process or organ in the body.

Hal Huggins, DDS, MS, a leading biological dentist who has been a pioneer in informing the public about mercury toxicity, writes in his book called, It’s All in Your Head, The Link Between Mercury Amalgams and Illness that “Mercury kills cells by interfering with their ability to exchange oxygen, nutrients and waste products through the cell membrane. Inside the cell, mercury destroys our genetic code, DNA, leaving us without the ability to reproduce that cell ever again. Immunologically, mercury embeds itself in a cell membrane, giving the cell the appearance of being “non-self,” which is the trigger for the immune system to destroy that specific cell. With mercury in your cell membranes, the immune system will start destroying your own tissues, thus the term autoimmune disease. Examples of these are diabetes, multiple sclerosis, scleroderma and lupus.”

Dr. Michael Ziff, DDS, who with his father, Sam Ziff, took a series of critical studies and reports examining the adverse effects of mercury and combined them in a single symptom list that analyzed 1569 patients who had eliminated their mercury-containing dental amalgams (see the book, The Mercury in Your Mouth–The Truth about “Silver” Dental Fillings, Quicksilver Press, p. 8-9), stated that, “You wouldn’t take a leaky thermometer, put it in your mouth, and leave it there 24 hours a day, 365 days a year. Yet that’s exactly what happens when an amalgam filling is installed in your mouth.”

A summary of how mercury interferes with basic metabolic processes is eloquently described by Alfred Zamm, MD, FACP in this statement from his research: Mercury poisoning is impaired oxidation. It’s like having an invisible cord around your neck that’s strangling you, but you can’t feel the cord is there. [The strangulation] is biochemical, but the principal is the same: mercury reduced the amount of oxygen you get. The body keeps adjusting, but with every adjustment it gets sicker and sicker. And ultimately you will die from this.

Most people are unaware that as mercury toxicity deprives the body of oxygen, the body in turn initiates a support of Candida overgrowth as a protective mechanism.
