How to Cure or Alleviate Edema, Including Lymphedema

By edema excessive fluid gather in body parts or greater areas of the body, making the areas swollen. Edema is most often a problem in the legs, but can occur anywhere in the body.

There are several types of edema, and many cases are also a mixture of more than one type. By lymphedema the drainage from the tissues through the lymph vessels are impaired or excessive fluid goes through the blood capillary walls into the spaces between the cells in a tissue. Edema can also occur because of poor drainage of blood through the veins towards the heart. Edema can further be caused by general accumulation of excessive fluid in the body.

The causes of edema are many. Edema is a symptom by most inflammations. Insufficient heart action by congestive heart failure can cause edema. A too high intake of salt can cause general water retention in the body. Sitting and standing long in the same position can cause edema in the lower limbs. A still sitting life without exercise can cause or aggravate edema.

Insufficiencies of the liver or kidneys can cause fluid retention and thereby edema. Edema can be a side effect of antihypertensive drugs, steroids surgery and radiation therapy. Further causes of different types of edema are varicose veins, cancer, hypothyroidism, blood clots, constipation and physical injuries.

To treat edema, it is important to treat the underlying disease or injury. However, medical treatment does not always cure the underlying condition or the cause is wholly or partially lifestyle aspects. Therefore various measures targeted directly against the edema are useful. These are presented here:

Pharmacological treatments: The physician will sometimes prescribe diuretics to reduce fluid retention in the body and thereby reduce edema. This treatment should however be used for serious edema or edema persistent when other measures do not work.

Salt intake restriction: Restriction of salt intake is an important measure by edema. People suffering from edema should avoid adding great amount of salt into their food or consume preprocessed food with a salty taste.

Diet: A good general diet that alleviates chronic inflammation is useful. Elastic bandaging: You can use elastic gloves, stockings, tapes or bandages around the swollen body parts during the day or during physical activities after instruction from a professional person.

Sequential gradient pump therapy: One can get special electric pumps connected to inflatable garments. The pump inflates and deflates the garments repeatedly. Each garment has several rooms that are inflated sequentially in the direction of the natural fluid stream towards the heart. Many times in the selling process we are so focused on what we need to accomplish that we forget about the wants and needs of our customer, the doctor. As our time with the doctor has diminished in the past few years to about 80 seconds per call we tend to focus even more acutely on our agenda rather than what our customer needs from us. During this article, I would like to concentrate on five things you need to know about your physician customer.

Doctors are Problem Solvers

Think about what doctors do every day. They walk into an exam room and ask the patient, “What’s the trouble?” or “What brings you here today?” They are looking for the problem that brought the patient into their office. They begin to ask questions to understand the problem, gather data through exams and diagnostics tests, and ultimately put a treatment plan in place to solve the problem. This is the world the doctor lives in every day of their lives. Dr. Michael Kessler has taught this concept for years in his training classes. I was facilitating a large doctor/rep role-play with one of the Big Pharma companies a couple of years ago. During the break I walked over to one of the doctors and asked what she liked about what the representatives were doing. She replied, “I hate it when they walk in and start asking questions!” I thought, Oh no, probing is a part of every pharma sales model in the world! When the next representative came in to call on this doctor, the representative started with this question. “Doctor, when treating disease X, what side effect is most important for you to avoid?” The doctor stated that GI upset was most important to her. The rep presented how her product’s features and benefits could solve this problem and closed. The doctor agreed to write her product for those patients who were experiencing GI upset. I waited anxiously to hear the doctor’s feedback to a rep who had done exactly what she just told me she hated. I thought, boy is she gonna get it! But she didn’t! In fact the doctor said it was one of the best presentations she had heard that day. After the rep left I asked the doctor to explain to me why she praised a representative who began her call with a question when she had just told me she hated questions. The doctor said, “That’s easy, within the first fifteen seconds she discovered a problem that I was experiencing that her drug could solve.” Since doctors are problem solvers we need to present our products as solutions to problems the doctor may be experiencing!

Doctors are scientist and make decisions using unbiased scientific evidence not marketing information. When a doctor is trying to discover what pathology a patient is presenting with during an office visit they gather as much scientific information as they can. Doctors are scientists and need unbiased scientific information to make a decision to use your product. Doctors reported in a focus group that they listen when a sales representative says,”What this means to you…” or “The benefit to your patient is…” I have seen this over and over in person observing doctor/rep role-plays. The reason is simple. Benefits solve problems and doctors are problem solvers! The old adage that features tell and benefits sell is applicable here. Here is an example of how benefits solve problems. Let’s assume that the problem a doctor is struggling with is allergy patients complaining of getting breakthrough symptoms between doses of a BID medication. The feature of longer duration of action does not solve this problem but the benefit of longer allergy symptom relief does solve the problem of breakthrough allergy symptoms. The way you would want to word the features and benefits during your presentation is, “Doctor, Zamfir has a longer duration of action than your current medication.” “What this means to your patient is they will get longer allergy symptom relief alleviating the problem of breakthrough symptoms.” Unfortunately research says that a typical sales representative will present 7-8 features before they mention 1 benefit. Try presenting benefits with features on your next call and watch the doctor look up when you say, “Doctor, what this means to your patient is…..”

Doctors are Taught to Communicate in a Certain Way

Every medical professional has been taught a standard way of communicating medical information. It is called SOAP. Doctors use this process to document clinical information, present patient cases and clinical papers and communicate patient information. Let me give you an example. If you have ever watched any of the medical shows on television such as Grey’s Anatomy you have probably seen an EMT Team handing a patient off to an ER Team and heard a conversation such as this. “I have an adult whit male approximately 46 years of age, BP is 130 over 90, pulse is 82, we suspect possible MI, recommend 10 mg of Epi.” The S stands for Subjective. The subjective information is an adult white male approximately 46 years of age. The O stands for Objective. The Objective information comes from the data they gathered during their work up; BP 130 over 90, pulse 82. The A stands for Assessment and is the medical team’s diagnosis; possible myocardial infarction. Finally the P stands for Plan. In this case the recommended treatment Plan is an injection of 10 mg of epinephrine. Every clinical study is set up this way. The subjective sets up the problem the study is addressing. The objective gives the vital information about the study such as author, journal, date and number of patients in the study. The Assessment reports the results of the study. And the plan gives the author’s recommendations. Two very practical ways to use this healthcare communication tool is to present your product and clinical study information in this format. When presenting your product information set up a problem your product can solve, solve that problem using your products features and benefits, probe to enter into a dialogue with the doctor and close for the doctor to use your product for those patients experiencing that particular problem. When using this format to present a clinical study set up the problem, provide the study information (author, journal, date and study data) report the results and the author’s recommendations. By using this universal language when speaking to all healthcare professionals you will gain credibility and be able to present information in a clear, concise manner. I have trained many people to present a clinical study in 30 seconds using this format. If you were a bilingual sales representative you would never present to a Spanish speaking doctor in English. Now that you understand the doctor’s language, stop speaking your native tongue of sales and begin to speak their language and watch their prescribing behavior change!

Doctors Expect You to Close, So Do It Right

The new hires are afraid to close and the tenured reps don’t want to harm their good relationship by closing hard. In reality, Best Practices Studies reveal that representatives who close consistently are more successful than those who don’t. The fact is doctors expect sales reps to close and actually anticipate it. Over the years through surveys and focus groups doctors have told us what they like and dislike about how we close. Below you will find information we have compiled from physician focus groups on what your physicians want and don’t want from a sales rep during the close.

Don’t Ask for the Next 10 Scripts

We have all been trained and evaluated on asking the doctor for the next ten scripts. So why do doctors hate this close so much? Because it is not practical for them to guarantee you that the next ten patients who walk through their door will be good candidates for your drug. Remember a close is simply asking the doctor to do something. It can be asking them to read a clinical study or a journal article. Be creative! A more practical close would be to ask the doctor to write for your drug for patients experiencing a problem your drug can solve.

Don’t ask for 100% of scripts

Doctors see this close as unrealistic and greedy because no drug will work for 100% of patients. Every drug has certain contraindications, warnings and side effects that prevent it from being used on 100% of patients. A more realistic close would be to let the doctor know which patients would not be good candidates for your drug. For example, “Doctor, I just want to let you know that Zamfir should not be used on patients currently taking beta blockers or MOAs.” The doctor will appreciate your candor and fair balance.

Don’t Be Pushy

I know you are saying that it is the pushy reps that get the business. We asked doctors in a focus group what they meant by pushy. Here is the example they gave us of a pushy representative’s close. Reps were selling a diabetes drug during a doctor/rep role-play. They presented a study on how drug B was more efficacious than drug A and closed for the doctor to switch patients from drug A to drug B. When we were debriefing this scenario after the role-play all the doctors began to laugh out loud. They said we would never switch patients who have been controlled for ten years based on one clinical study. They said a less pushy close would be to ask them to “consider” drug B for patients on drug A experiencing a problem.

Summarize What We Have Discussed Before Closing

In a focus group conducted in Canada by Novartis Ophthalmics after doctor/rep role-plays doctors were asked what they noticed about the really good representatives. One thing they reported was that the really good representatives would summarize what they had discussed prior to closing. The doctors said this helped remind them of why they should use your drug. A good way to summarize is by saying, “Doctor based on the fact that Zamfir’s longer duration of action provides 24 hour allergy symptom relief alleviating the problem of breakthrough symptoms would you write for Zamfir for those patients experiencing breakthrough symptoms?”

Always Give Dosing Information

A GP or FP has hundreds of drugs to remember, yet every day we ask doctors to write prescriptions for our drugs with out ever telling them how to write the prescription. A recent study published in Pharmaceutical Representative Magazine reported that patients complain that doctors don’t give them enough information on how to take their medication. Could this be because we don’t give them the information they need to pass along to their patients? At they end of every call we should remind the doctor of the dosing information for our product. “Doctor, I just want to remind you that the dosing for Zamfir is one tablet BID.”

Summary

Let’s begin to look at our sales call from the point of view of our customer. After all, the call is about them not us. With only 80 seconds to make an impact we need to focus on what our customer needs and wants rather than what we think they want. By presenting our products as solutions to problems, using scientific information presented in the SOAP format, presenting benefits with features and closing appropriately we will be CUSTOMER focused rather than SALES focused!

Heart Disease – What Is The Difference Between Organic Heart Disease And Degenerative Heart Disease?

Although Heart Disease is the main cause of death in the Western World it is amazing how little the general public actually know about it.

For example very few people realize that there isn’t just one type of Heart Disease. In fact there are at least ten different types and these fall into two distinct categories – Organic and Degenerative.

The major difference between Organic and Degenerative Heart Disease is their causes.

Organic refers to a situation where the organ (the heart) is damaged by a specific event. This can also be referred to as “acute”, which simply means that it happened suddenly or over a short period of time. Degenerative Heart Disease (sometimes referred to as “chronic”) is caused by gradual deterioration over a long period of time.

There are two types of Organic H.D. – Congenital and Rheumatic .

Defects that occur at birth are classed as Congenital Heart Disease. These may affect the heart itself : it may not have developed normally during pregnancy, the wall of the heart may be damaged (hole in the heart), or the blood vessels may be underdeveloped. These defects may be hereditary or more likely have been caused by external factors such as drugs or infection during pregnancy. They are normally diagnosed at birth or in early childhood but it is not uncommon for the symptoms to occur for the first time in adulthood..

Rheumatic Heart Disease can be the result of a bout of rheumatic fever. Occurrences have decreased considerably due to the use of antibiotics to treat rheumatic fever.

There are at least eight specific diseases, which fall into the category of Degenerative Heart Disease. The common factors within this category are that the disease has progressed gradually and that there is no specific event that has caused it.

The vast majority of people who are diagnosed with Heart Disease have some form of degenerative heart disease. This is the form of disease that is the target of the awareness campaigns and is the type that we can help to prevent by our lifestyles choices.

How Shoko Tendo’s Tattoos Transformed Her Life And Inspired Mine

Simply Magical. It is not everyday that a book inspires you with so much courage and strength to face the difficulties in your life.

The book which I am referring to is Yakuza Moon, which was written to reflect the life story of the author Shoko Tendo. Born in 1968 into a family headed by a yakuza or Japanese gangster Dad, she underwent an arduous upbringing after falling into bad company at a tender age. Descended from medieval gamblers and outlaws, the yakuza were long portrayed as latter-day samurai, bound by traditions of honor and duty while living extravagant lives. Dad was the gang leader linked to the Yamaguchi-gumi, the largest yakuza group, led a “classic” yakuza life replete with Italian suits and imported fast cars.

To be fair, Dad never really spoke about his yakuza business while at home and brought Tendo up to have impeccable manners.

But surrounded by bad influences, Tendo flunked her early school education, morphed into a yanki or teenage gangster and lost her virginity to her first boyfriend when she was barely in junior high school. And as if life wasn’t tough enough, she became addicted to thinner-sniffing and “speed” or marijuana, and ended up in a reformative centre after getting caught in a gang fight.

Eight months later, she was released only to learn that Dad fell seriously ill with tuberculosis. Her family was in total shambles cowering from daily visits by rowdy debtors amid mounting debts. In a state of numbness and denial, she continued her regular fix of drugs, mixing with bad company, suffered from multiple rape incidents and engaged in casual sordid sex in “love hotels”.

In short, she had become a total rebel drifting aimlessly from day to day, and – get this – she was barely eighteen then. It was the kind of growing pains that would move any reader to tears.

Life for Tendo then took on a different perspective at age twenty-one when she was bundled into a tattoo shop by an old acquaintance. Though she was exposed to irezumi or full-body tattoos throughout her life, it was the first time she had felt so deeply about skin art. Transfixed and true to her character, she impulsively decided to get herself tattooed from the base of her neck down to the tips of her toes, with a design centered on a geisha with a dagger in her mouth. And on each of her arms was a tattoo of dragons.

Strangely, since getting the tattoos, her attitude took on a complete change and a whole new meaning. Such was the emotional power and influence of tattoos that they can turn a person ravaged by a rough upbringing around overnight. Literally, empowerment came when she had herself tattooed.

Then on her twenty-second birthday got married to a yakuza member who had to chop off his left pinky in order to prepare for his marriage by leave his gang. However, the joyous occasion had a bitter-sweet aftertaste for her as she had been raped by her possessive and mentally unstable ex-boyfriend the day before.

Shortly later, she was forced to abort her first child as she and her husband was simply too poor to afford raising one. Reality was extremely harsh on the both of them.

But the cruelties of life did not quite end there. Two days after she received her first paycheck as a married woman, her mother suffered a massive stroke and died shortly thereafter, without recovering. Depressed by blow after blow in life, she caved in and attempted suicide using sleeping pills. Luckily, she was rescued by hospital staff only to receive news that her Dad was dying from stomach cancer. God help her!

The loss of both parents finally brought Tendo to her senses. Determined to leave behind her seedy past, she plunged her life into work, marked by the opening of her first saving account ever at thirty years old. In fact, she eventually managed to save enough to buy her parents proper dignified burial plots.

Now in her late thirties, Tendo is completely free from her turbulent past and leading a blissful life with her young daughter, something which Tendo had resigned to not having for the longest time. Her life story has taught us to cherish and treasured fully our loved ones. Compared to what she went through, most of the problems we faced are simply too insignificant. Remember: No conflict is too much to be resolved – swiftly. Unfortunately, the tattoos on her will remain – a stark and vivid reminder of her days of being wild. I wish her the best.

Hemangiosarcoma – A (Usually) Silent and Deadly Canine Cancer

November is Pet Cancer Awareness Month. Among the most deadly of canine cancers is hemangiosarcoma, or cancer of the blood vessels. Hemangiosarcoma can either present as skin cancer, which can be successfully treated if caught early enough, or as cancer of the internal organs, particularly the spleen or the heart. The prognosis for splenic or cardiac hemangiosarcoma is extremely poor, even with aggressive treatment, as frequently the first sign of any problem is when the tumor ruptures and causes massive internal bleeding. An additional complication arises from the fact that since it is a blood vessel cancer, the cancer cells have usually spread to other areas of the body by the time of diagnosis. As a result, the median survival time for internal tumors post-diagnosis is measured in weeks or months, even with surgery and chemotherapy. Hemangiosarcoma can occur in any breed, but there is an identified pre-disposition in German Shepherd dogs, labrador retrievers and golden retrievers. Within my own circle of pet parent friends, in the past year we have lost a Siberian husky, an Australian shepherd, a golden retriever and my own miniature poodle, Tiny, to hemangiosarcoma.

What are the signs and symptoms to watch for? For skin-based tumors, any unusual growth on the skin should be evaluated by your veterinarian and biopsied if there is any suspicion of cancer. It is a good idea to check your pet’s skin frequently, particularly as he ages, for any abnormal lumps or bumps. Many are benign, but only your veterinarian and a pathologist can identify cancerous skin growths.

For internal organ cancer, the signs can be much more subtle, and sometimes non-existent. In the cardiac form of hemangiosarcoma, you might notice weakness, weight loss, loss of appetite, difficulty breathing or difficulty recovering from any kind of exertion. These can all be signs of simple aging, other heart or lung problems, or tumor growth. Again, a visit to your veterinarian is in order for possible x-rays, ultrasound, CT or other diagnostic scans to determine the cause of the problem. If not diagnosed, the heart tumor will eventually rupture and cause massive internal hemorrhage.

In the splenic form of hemangiosarcoma, unless the tumor is extremely large and can be felt on abdominal exam, the first warning sign might be total collapse when the tumor ruptures. In Tiny’s case, he exhibited greater than usual “old man weakness” one evening at home, and could not stand up. He was seventeen years old at the time, and had a bulging abdomen to begin with due to loss of muscle tone associated with aging. I quickly rushed him to the veterinary emergency clinic (he was never one to have his emergency situations during regular veterinary clinic hours), where the doctor quickly tapped his abdomen and withdrew bloody fluid. She talked with me about her suspicions that a splenic tumor had ruptured, and recommended an ultrasound to confirm her diagnosis. Ultrasound did show a very large spleen as well as some suspicious spots on the liver. We discussed the two options: surgery to remove the spleen and the suspect portions of his liver or euthanasia. Given his age and all of the potential complications, we made the difficult decision to say goodbye.

But, when they brought Tiny into the room for that final procedure, he had miraculously recovered from his collapse, was very excited to see us, and started asking us to play with him. The veterinarian suspected that the internal bleeding had stopped and that he had re-transfused himself. After more discussion of the other alternatives and based on the fact that he seemed to be telling us that he wasn’t ready to go just yet, we brought him home and scheduled a specialist visit early the next morning.

Tiny had a splenectomy and partial liver lobectomy, and came through the surgery with flying colors, especially given his age. We opted for a shortened and low-dose course of chemotherapy, and for the remainder of his life he took several mild medications such as doxycycline and Deramaxx to help keep the cancer at bay. He also received acupuncture and Chinese herbal formulations in addition to Western medicine. In spite of the six months or less that most hemangiosarcoma patients survive, Tiny lived another two and a half years until the cancer spread to his brain and his mouth. When he began to have difficulty eating and started having seizures, it was time to help him cross the “Rainbow Bridge.” His outcome and length of survival with good quality of life was unusually positive, but he was a fighter with a strong will to live.

Survival in hemangiosarcoma is largely a function of how early it is caught and whether it is a surface/skin lesion instead of an internal tumor. Treatment options may be limited, especially if a tumor ruptures, and diagnostics, surgery and chemotherapy can be expensive. You know your dog better than anyone else, and are in the best position to make informed decisions (with the help of your veterinarian) as to the best course of action if this deadly cancer strikes your dog.

Complications With Walking Pneumonia

Walking pneumonias, also referred to as mycoplasma pneumonia or atypical pneumonia, results due to an infection by a pathogen known as Mycoplasma pneumoniae. This form of pneumonia is mild and not as severe as other types of pneumonia. The symptoms develop and surface slowly over a period of one to three weeks. Often the symptoms in this disorder can be mistaken for those related to flu or severe cold.

As per MedlinePlus, most people affected by this infection are children or those below 40 years. It is often spread amongst people who are in close proximity. Some of the symptoms include persistent cough, sore throat, and fever or chest pain.

Common fallout of mycoplsama pneumonia is infection of middle ear. Such infections are more common during early childhood and usually affect children in the age group of 6 to 24 months. The chances of children getting affected are more than adults.

While the infection is mild, walking pneumonia has the potential to cause several complications. Some of those complications are highlighted below.

In a middle ear infection, the lining in this section gets inflamed and fluid begins to accumulate behind the eardrum. Some symptoms of this condition are high fever, imbalance, irritability, dizziness, insomnia, and ear pain, drainage in ear, vomiting, appetite loss, temporary loss of hearing, and/or diarrhea. The treatment includes administration of over-the-counter drugs, antibiotics, and warm compress and drainage tubes. If left untreated, the infection can cause rupture of the ear-drum or cause loss of hearing.

People suffering from walking pneumonia are more susceptible to getting hemolytic anemia. In this condition, the bone marrow does not produce sufficient red blood cells to replace the prematurely damaged ones in the blood. Hemolytic anemia can result due to protein abnormality, variation in the protein level in blood, clots in blood, and side effects of medicines, infection or abnormal response of the immune system. Some related symptoms include dark coloration of urine, fatigue, and enlargement of spleen, chills, acceleration of heart rate, fever, and pale coloration of skin, jaundice or difficulty in breathing. The treatment includes ingestion of iron, folic acid, transfusion of blood or administration of corticosteroids.

Another fallout of this condition is rash of the skin. People, who are more allergic and sensitive to antibiotics, are at a greater risk of getting allergic reactions when treated with antibiotics. In this condition, rashes develop which can be red, flat spots; or raised small bumps Amoxicillin or penicillin administration can also trigger this condition. Discontinuation of antibiotics can reduce rashes.

Walking pneumonia can also cause severe pneumonia due to weak immunity or poor response to any treatment.

Tips for Chest Congestion

Symptoms of chest congestion are easy to identify. Main symptoms are chest tightness and chest pain. Symptoms can be listed as under;

  • Chest pain
  • Coughing
  • Runny nose
  • Wheezing
  • Discomfort
  • Loud noises while breathing
  • Blood in cough
  • Shortness of breath
  • Labored breathing
  • Dizziness

This can easily be cured by home and herbal remedies. Home remedies are nothing but have anti inflammatory effects thus making the breathing easy. Here are some of the remedies for getting quick relief from chest congestion.

Drink plenty of water. This will help by flushing the toxin out of body and also softens the mucus in the chest so one can easily cough it out.

Food that contains anti oxidants and is yellow in colour also helps to eliminate the chest congestion. These may include mangoes, lemon, papaya and spinach.

Massage of olive oil for a few minute to chest is very effective in providing relief from chest congestion. Massage the oil until it is properly absorbed in the skin. This will increase its efficacy.

Another effective remedy is to boil garlic cloves and then strain it. Drink the water twice daily, this will help soothe the throat and chest. Herbal tea is also known as a good decongestant and you can choose from ginger, fennel, salvia, eucalyptus and green tea.

Chicken soup is also very effective in relieving sinus and chest congestion. This actually helps by working as an anti-inflammatory agent, making mucus thin and allows easy productive cough.

Thick mucus in the chest can also be thinned out by proper gargling with half of a teaspoon of salt mixed with one glass of water.

Inhalation of steam is the best chest congestion treatment. Steam inhalation in addition to oil massage is very effective, Take hot water in bowl and mix some drops of any essential (clary sage, eucalyptus, pine and rosemary. Eucalyptus oil is very effective in chest congestion treatment. Now incline the face over the bowl in such a way that steam directly enters your nostril. Cover your head with a large towel and prevent your eyes from the steam as it may harm you. It can soften and break down the sputum and loosens up the phlegm thus making it easy to be cough up giving immediate relief from chest congestion. Steam inhalation should be for about 4 to 7 minutes.

Chest congestion remedies can also be relieved by using a warm or cold water humidifier while you are sleeping. This will certainly provide relief from symptoms.

Spicy food also acts as decongestant. Food that contains chili, ginger and garlic paste helps in decongestion. Spicy food helps in getting rid of unwanted bacteria. Spicy food thinned put the mucous and helps in easy productive cough.
