Blood in the stool:

Blood in the stool or gastrointestinal tract may take various forms or appearances depending on where it is coming from and how brisk the bleeding. There are common medical terms and definitions for blood in the gastrointestinal tract or stool that may not be familiar to the lay public but can be helpful to know if you or a family member experience gastrointestinal bleeding.

Hematemesis is vomiting blood:

Red hematemesis is vomiting red blood. Coffee ground’s hematemesis is vomiting blood altered by stomach juices.

Blood in the stomach is quite nauseating usually resulting in vomiting. Vomiting blood is called hematemesis. It can be bright red if occurring briefly after the blood enters the stomach or when the bleeding is brisk. Stomach acid and digestive juices alter any blood that remains in the stomach. If the altered blood is vomited, it usually appears like old coffee grounds, hence the term “coffee grounds like” hematemesis. Blood may originate from the stomach, be swallowed, or regurgitated from the duodenum, the first part of the small intestine just after the stomach.

Melena and blood as a very potent laxative:

If blood in the stomach is not vomited completely the altered blood passes into the intestine where it usually triggers diarrhea. However, the diarrhea is usually black, tarry and foul smelling. This is termed melena or melenic stool. Though many doctors mistakenly refer to such stool as melanotic that is an incorrect term. Melanotic refers to containing melanin, the dark skin pigment present in moles and the malignant skin cancer, melanoma. Melena or melenic stools indicate an upper gastrointestinal tract bleed or source of blood because it has been altered by digestive juices only present in the stomach and upper small intestine. Such bleeding usually originates from a site reachable by an upper scope though occasionally the site is beyond the reach of such a gastroscope.

Lower gastrointestinal hemorrhage and rectal bleeding:

Red blood passed rectally usually indicates the source of bleeding is in the lower colon or rectum, or lower GI bleed, except in massive hemorrhage from the upper tract. Bright red blood on toilet paper or dripping in the toilet bowl is usually from the anus or rectum, most commonly from hemorrhoids or an anal fissure, though can occur with rectal cancer.

Bloody diarrhea and colitis:

Bloody diarrhea, often with mucus, is typical of colitis. Colitis is inflammation of the colon or large intestine from any of a number of causes that may include infection, poor blood flow to the intestine (ischemia) and the chronic inflammatory bowel diseases ulcerative colitis and Crohn’s disease.

Occult blood in the stool:

It takes about 50-100 ml blood to turn the stool black or melenic. Less than 2 cups of bleeding is therefore not usually visibly detectable and is termed occult blood in the stool. Various chemical tests for occult blood in the stool exist that can be done on evacuated stool or stool obtained by a gloved finger exam by a doctor. Occult blood in the stool is concerning for cancer and necessitates an evaluation for the cause. Ulcers, acid reflux, Celiac disease, polyps, colitis and Crohn’s disease, hemorrhoids and aspirin type medication injury to the gastrointestinal tract are all common non-cancer causes of occult blood in the stool.

Bleeding in the GI tract should not be ignored.

Now that you understand these basic terms and definitions it will be easier for you to recognize GI bleeding in yourself or a family member and communicate more clearly to your doctor about your symptoms. Oral Thrush Caused by Yeast Overgrowth

When you get a yeast infection in the mouth, it is called Thrush, characterized by redness and white bumps and can be painful. The Tailgate Party and Liability

We’re only a few weeks away from the fall football season. Millions of rabid football fans will throw food and beverages into their favorite vehicle and head for the football stadium parking lots. When they arrive, the barbecue grills will come out, tables and chairs will be set up, team flags and banners will go up, and the ice chests will be filled with great beverages.

Now, why would I spoil that festive party atmosphere with an article about liability?

That’s simple, my friends. I want to warn you about some very serious issues that could not only spoil your tailgate party, but drastically affect your financial future.

I’ll be talking about tailgate parties thrown by both individuals and businesses because some of the potential problems are the same.

Problem Number One: is your legal liability at the football venue. You are parked in a public parking lot. You may be parked in a private lot. But either way, someone else owns the ground on which you sit. So, they could be found legally liable in case a guest gets injured on their premises. The problem arises when your participation is included. If you are taking up space with your truck, camper, motor coach or tent, along with tables, chairs, grilles, and such, you are adding to the number of things that could cause a personal injury. Chairs collapse, people trip on things, grilles blow up…and the personal injuries would not have occurred but for your tailgate party.

Problem Number Two: is injury to others from tainted food. Many times, tailgate parties go on for many hours. Food sits out on tables waiting for guests to load up their plates. But if someone gets food poisoning from food they ate at your tailgate party, you could be found liable for their injury, medical costs, loss of income from missing work, or even death.

To minimize your tainted food risk, be careful to keep your hot foods hot and your cold foods cold. Use covered dishes and throwaway plates and utensils. And refrain from serving foods that have a reputation for going bad, like potato salad or seafood.

Problem Number Three: is your legal liability for serving alcoholic beverages. If a guest gets injured at the tailgate party, or in the football stadium, or while driving home, or injures others because he is determined to be drunk, you could be found legally liable for furnishing the alcoholic beverages to him.

If you feel you must furnish alcoholic beverages, consider a drink voucher system to limit the number of drinks served. Or, serve alcohol for only a short period.

Serve plenty of non-alcoholic beverages: water, sodas, juices, coffee, tea.

Arrange transportation for intoxicated guests if they are leaving the tailgate party with some of your booze in their belly. Call a cab, use designated drivers, or get them home in your vehicle.

Don’t sell alcoholic beverages at the tailgate party. No cash bar, no cash contribution jar displayed on the table. If an individual host sold booze at the tailgate party, he would have likely violated state liquor laws.

So, does the individual tailgate party host have any liability coverage for the party?

NO YOU DO NOT!! If you have a Homeowners Insurance Policy, you have no liability coverage built in for off-premises activities like a tailgate party. In Section II, Exclusions, E. Coverage E, 4. “Insured’s Premises Not An Insured Location: “Bodily injury or property damage arising out of a premises rented to an insured.” It could be argued that if you paid for parking, you rented the space you occupy in the parking lot. But the exclusion still stands.

An individual throwing a tailgate party should buy a one-day event liability insurance policy to protect their assets.

Does a business throwing a tailgate party have any liability coverage for the party?

YOU MAY HAVE COVERAGE depending upon the liability policy you carry. In the Commercial General Liability Coverage Form CG0001, the “coverage territory” is defined as the United States of America. That protects you at off-site locations.

Businesses – Remember that even if you carry adequate insurance — called “host liquor liability insurance coverage” – it will not apply, and will not protect your business if alcoholic beverages are being sold at your party. You should purchase additional liquor liability insurance coverage before the party.

A business throwing a tailgate party should consider hiring a separate bartender or caterer to serve alcohol. He will know better when to say ‘No’ to a guest who’s had one too many. The hired bartender should have his own liability insurance and provide you a copy of his insurance certificate BEFORE the party. Instruct the bartender/caterer to notify an appropriate event manager if he finds someone who’s had too much to drink.

Even top athletes will initially find it difficult to exercise at higher altitudes. They will feel the lack of sufficient oxygen with each labored breath, say experts, because their oxygen needs have not changed even though the altitude has changed, and the air has become thinner.

But the body is an amazingly adaptive machine that, in time, becomes acclimated to exercising at altitude by producing some physiological changes.

Physiological Adaptations to Higher Altitudes

Because oxygen is thinner at higher altitudes, one must breathe faster in order to obtain the same amount of oxygen as experienced at sea level. This increased breathing rate is necessary to supply sufficient oxygen to the blood stream, which then sends it to the oxygen-dependent muscles. The heartbeat simultaneously increases in an effort to transport oxygen throughout the body, via the bloodstream.

Studies have shown that in just a few days, however, the lungs become extremely efficient at breathing this thinner air. Several studies, for instance, have shown that the volume of air that the lungs are able to accommodate eventually doubled in those exercising at altitude from 12,000 to 14,000 feet.

Other physiological adaptations that the body makes to higher altitudes include:

– A higher heart rate required to move the disease amount of oxygen throughout the body. Within a few days, however, the increased blood that is pumped through the heart decreases, as does the heart rate. In a very short time, both the blood volume pumped by the heart and the heart rate decrease to the levels of those that were present at sea level.

– An increased number of red blood cells, which enables greater oxygen transportation by the blood stream. (One study showed that as subjects became accustomed to the higher altitude of 13000 feet, their blood/oxygen levels increased by 19 percent while the blood volumes processed by the heart decreased by 9 percent.)

– Chemical enzyme changes. Of particular interest to an athlete is the performance of the muscles, performance that is significantly affected by chemical enzymes, which supply necessary energy to the muscles. As the body becomes accustomed to higher altitudes, these enzymes adapt to the lower oxygen rate, per experts, and perform much as they did at sea level.

What these physiological changes mean for those regularly exercising at altitude, according to New York Times sports reporter William Stockton, “is a steady improvement in [exercise] performance. (The) heart rate begins to decline [for the same amount of effort] and breathing becomes less difficult.”

Altitude Dangers

The athletic-enhancing benefits of exercising at altitude are so well known that many coaches recommend this type of training to their athletes. There are, however, certain dangers that can accompanyy such training.

One of the chief dangers is Acute Mountain Sickness, AMS (also called altitude sickness.) AMS is the body’s effort to acclimate to the reduced oxygen levels at higher altitudes. As previously mentioned, during the acclimatization process one’s breathing rate increases in an attempt to obtain additional oxygen. In some individuals, this process can lead to hyperventilation symptoms such as shortness of breath, dizziness, nausea, and headache. These symptoms can be extremely distressing and even debilitating.

Although most cases of AMS are minor, with a small percentage of those afflicted, a potentially life-threatening condition called high-altitude pulmonary edema (HAPE) may develop, which is caused by the leakage of fluids from the blood vessels into the lungs. The first symptom of HAPE is usually an extreme shortness of breath that occurs simultaneously with ascension to a higher altitude. Shortly thereafter, the heart rate and the breathing increase and a cough develops that sometimes contains bloody sputum. By the time that these symptoms appear, quick medical attention is necessary to prevent the sufferer from lapsing into a coma and dying.

Still another form of AMS, and one that is even more dangerous, is called high-altitude cerebral edema (HACE). Although the exact mechanism whereby this disorder develops is not completely understood, it usually begins with the classic symptoms of AMS and then progresses into neurological symptoms such as speech impairment, loss of coordination, alternated states of consciousness, and hallucinations which eventually lead to death if medical attention is not immediately administered. Although HACE is very rare, it is a high-altitude risk that cannot be ignored.

Tips for Exercising at Altitude

To minimize such potentially dangerous medical conditions, and to manage any unpleasant physical symptoms caused by ascending to higher altitudes, experts recommend these tips:

– Make the ascension slowly. If possible, allow your body a full day to acclimate to the altitude change of moving from 4,000 to 5,000 feet and a similar time period to acclimate to the change from 8,000 to 12,000 feet.

– Watch your training schedule. Make sure that you train slowly to allow your body time to adjust to the higher altitude.

– Stay hydrated. Higher altitudes can lead to a greater risk of dehydration if sufficient amounts of water are not consumed. Experts recommend that drinking a minimum of 8 ounces of water for every 20 minutes spent exercising at altitude.

– Listen to your body. If the intensity of your exercise program is causing your breathing to become excessively labored, reduce the intensity until your breathing eases. (Because of this acclimation process, you will likely have to adjust your exercise program many times until full acclimatization is achieved.)

Exercising at altitude is a productive training method that one should attempt only when fully apprised of and prepared for the attendant risks. If an athlete adequately prepares, and is cognizant of the potential deadlier risks, and thus adjusts his or her training schedule accordingly, the athletic benefits of exercising at altitude can be enjoyed without incurring the dangerous side effects.
