I received a question from the Ask Experts site yesterday that I feel the need to share, since this is coming up more and more with the advent of mass Pilates DVDs and underqualified instructors.

Question: “Hi, I am a beginner in Pilates, when I try to do the V shape or half way crunch, I get a HORRIBLY painful cramp right below my rib cage on the right side, it stops me cold. I feel like I’ll never get rid of this flabby belly that ruins all of my cute outfits. Have you heard of this?”

First, no Pilates beginner should be doing the teaser (the V shape), which is an intermediate/advanced exercise that requires deeper abdominal recruitment to avoid overusing the hip flexors and external obliques.

Second, there is nothing “Crunch” like in Pilates. We do not tighten & compress the midsection like someone who has buttoned his vest to his pants, but seek to strengthen the muscles functionally without compression by using the pelvic floor and transverse abdominus.

While Pilates mat is the most accessible form of Pilates, the equipment is actually more appropriate for beginners. Joseph Pilates invented the apparatus to assist as well as resist movement, enabling people to exercise who otherwise may not have been able to move properly. It is not true that one must do Pilates mat before taking on the machines.

Then I saw an ad for “Cardiolates,” a concept that is at once fascinating and horrifying. Note that Pilates is anaerobic exercise, like weight training. To attempt to blend Pilates and cardio into one workout regimen is just plain wrong, is contrary to the whole basis of Pilates exercise, and will most likely result in lots of injured people who feel that they cannot do Pilates because it hurt them

I hate this mass production of Pilates because it makes Pilates seem simply like a group of exercises (that look like other exercises) that you can do fast, slow, or otherwise. That’s just not it! Pilates is about how you exercise; it’s about how you move and breathe and get into positions. Pilates is about balance and proper movement, not simply about hauling oneself into a certain position.

Improperly done Pilates can cause– hernias (from bearing down too much) muscle cramps (from improper recruitment) neck & back injuries (from improperly flattening the back and neck causing muscle strain and vertebral compression and from overstretching or over-extending the back and neck–all from improper instruction on how to stabilize the pelvis) rotator cuff injuries (from pushing large range of motion in the shoulder joint without proper ribcage stabilization) osteoporotic spinal fractures (from too much forward flexion and other movements contra-indicated for osteoporosis) and other injuries

Listen up –badly taught and poorly executed Pilates can cause you injuries that will be with you forever, so please please find a certified Pilates teacher to work with and be vocal when things hurt or you are not sure how to do something.

Pinched Nerves – How to Determine If You Have One

Sciatica, everyone has probably heard of it but do you know what it really is? Most might say that it’s a pain you get in your leg but do you know where it comes from? What confuses a lot of people and even doctors is that sciatica is not a diagnosis, I repeat that sciatica is NOT a diagnosis, in fact there are 10 reasons at least as to why you can get sciatica, so if you go to your doctor and they give you that diagnosis and hand you a pill, get them to think again. Sciatica by definition is pain experienced in the distribution of the sciatic nerve, the sciatic nerve consists of 5 nerves exiting your lower lumber (back) area and exiting your sacrum (pelvic area), these nerves join as one and is called the sciatic nerve. This runs down the back of your leg to your ankle and sometimes affects the sole of your foot. The sciatic nerve feels sensations and allows you to move the muscles in your leg. Sciatica is a symptom rather then the cause.

Here is a list of the 10 causes of sciatica pain:

  1. Sacroiliitis – an inflammatory process within the sacroiliac joint (pelvis) can be due to infection, sprain of the joint (misalignment), or an arthritic condition.
  2. Iliolumber Syndrome – inflammation or sprain of the iliolumber ligament, a big ligament that runs across the top of your pelvis used for bending and twisting.
  3. Quadratus Lumborum Syndrome – heard of the QL muscle, this is a muscle in your lower back; it can get tight and painful and refer pain down your leg.
  4. Trochanteric Bursitis – the bursa at the upper side of your leg is inflamed.
  5. Ischialgluteal Bursitis – inflammation of the bursa in your buttock, when you sit it is aggravated.
  6. Posterior Facet Syndrome – facet joints are a part of your vertebra, they can be misaligned or degenerated.
  7. Meralgia Paraesthetica – a nerve entrapped that passes through your hip flexor muscle within your pelvic area, can be due to pregnancy, tumours and infection.
  8. Disc Herniation – also known as a prolapsed disc or bulging disc these days, when you injure a disc in your lower back due to strain or misalignment.
  9. Lateral Canal Stenosis Syndrome – degeneration or arthritis where the nerve passes through the spine causing symptoms.
  10. Myofascial Syndromes – when muscles are tight around the lower back and pelvic area they can cause referring pain.

You can have one of those conditions occurring or you can have 3-4 at once occurring but until you get a proper diagnosis of your condition and the primary cause discovered you will not get better. Sciatica occurs when the function of the nerve is affected, weakness, numbness or pins and needles can be present in the legs with or without lower back pain. Understanding Canine Diabetes In Beagles

Diabetes is a common disease in breeds like Beagles. It is important for every dog owner to know the facts about this disease.

– Diabetes is an endocrine disease. It has two types; diabetes mellitus and diabetes insipidus. This disease occurs when the body produces insufficient insulin, a kind of hormone essential in sugar metabolism.

– Diabetes mellitus is the most common of the two types. Also called “sugar diabetes”, this disease occurs when the body produces less insulin. Diabetes mellitus is further subdivided into two; Type 1 and Type 2 diabetes. Type 1 diabetes occurs when there is insufficient production of insulin in the pancreas. Type 2 diabetes on the other hand occurs when there is an inadequate response by the dog to insulin.

– Diabetes insipidus is a condition wherein the excretion of diluted urine is in large amount and can not be reduced even if the fluid intake is reduced. This happens because of the kidney’s inability to concentrate urine. This type of diabetes is caused by a deficiency of antidiuretic hormone (ADH) known as vaporessin.

– The common symptoms of diabetes are excessive drinking and urination. The dog may also suffer weight loss though there is an increase in appetite. In addition, poor skin and hair coat, liver disease, vomiting, weakness in the rear legs, secondary bacterial infections, dehydration and rapid breathing may also be present in dogs with diabetes.

Short-acting insulins are effective for one-to-four hours, medium-range insulins are effective for four to 24-hours and long-range versions of insulin last from eight to 28 hours. Too low dosage of insulin will not control diabetes and too high dosage can cause hypoglycemia (low blood sugar level). If possible, medication should be given at the same times everyday.

– There are also other measures to keep diabetes in control. The dog’s weight and food intake should be managed well because blood sugar is hard to control in overweight dogs. Instead of giving one full meal a day, experts suggest to feed several smaller ones (at least twice a day). Another way to control obesity is through regular exercise.

Morbid Obesity – Big Questions

The modern world is a complicated and confusing place. Americans seem to have reacted to that confusion by becoming bigger themselves. In the past two years I’ve seen my own weight climb steadily until it reached 190 pounds on my 5’6″ frame – at which point I realized that there was an ISSUE here. My weight was an issue that couldn’t be ignored or rationalized away any longer. I’ve whittled myself down to a more normal 164, but I’m still a big woman, and have a ways to go before I hit a healthy BMI (body mass index) of roughly 150 pounds, which puts me in a healthy weight range for my height.

My ballooning weight is far from abnormal in today’s world. Cable television has filled many time slots with shows on the morbidly obese. Having spent three days in bed recovering from the flu I’ve had ample opportunity to watch shows on the “Half Ton Man…Woman…and Teen” and seen “Big Medicine,” along with a slew of other shows devoted to the morbidly obese.

The primary question that springs to mind is how can people afford to be so heavy? Especially the people who manage to reach 500 plus pounds? Many of these shows feature people who are unemployed and bed ridden. So how do they get the vast quantities of food needed to stuff their over-stuffed bodies?

The obvious answer is that for every morbidly obese person there is one or more enablers who are willing to purchase food, in most cases prepare it, and feed their loved ones to death. It’s obvious from these programs that most of these people abhor their hefty hindquarters. It’s also apparent that the mothers (yep it usually seems to be the mamas) live in fear of their pudgy progeny dying. And that is a very real possibility. The morbidly obese are more prone to heart attacks and diabetes along with any number of health concerns. TLC’s “Half Ton Woman” died from obesity related complications on cable TV, which is not your average end to daytime programming. Being fat literally killed her.

So what’s the deal? If I can hit 190 and think – wow – this is unacceptable. I’m one chubby monkey and have to do something NOW, why can’t everyone?

To put this question into perspective an 800 pound person’s life expectancy is zero. They could literally keel over dead at any moment. As far as I know even heroine junky alcoholic crack heads have a better life expectancy than that.

The striking thing about many of the people on the cable programs is that they generally have one or more obese parent and so the child’s weight gain may at first be considered less concerning than if the child were born into a family where people maintained a healthy weight.

My own father was obese for long periods of time. My own weight inched higher in conjunction with the weight of my sisters. We were skinny together…and grew increasingly chunky together. It’s much less disconcerting eating fries and burgers when everyone else is eating that way as well.

One recent article also suggested that obesity and poverty go hand in hand. The poor do have less access to high quality or organic food, but while recent studies have found that a calorie is a calorie is a calorie. Six hundred calories from McDonald’s – and six hundred calories from a well balanced meal still equals the same amount of weight. Of course a healthy meal has a myriad of other benefits, but as far as weight gain a calorie is a calorie.

The average American woman can maintain her current weight eating 2,000 calories a day. The average American man can eat 2,500 without gaining weight. McDonald’s may be the obvious choice in a poor neighborhood, but that doesn’t mean it needs to make you fat. A cheeseburger, soda, and a small fry at McDonald’s is 550 calories. A breakfast of three eggs, two slices of bacon, and toast is roughly 500. A dinner of spaghetti with meat sauce is roughly 280 calories per serving – have two and that’s still only 560 calories. This leaves 390 calories for a glass of milk, a soda, or a reasonable snack. Eating less food, even if it’s unhealthy food, may not do much for your cholesterol, but it will save a person from a whole slew of other health problems. Figure out what a portion of food really is – eat crap if you must – just don’t supersize it.

Big box stores like Walmart do offer fruits and vegetables – and even the small corner markets have canned fruits and vegetables if fresh aren’t available. Frozen fruits and vegetables are a great alternative since they don’t usually add sodium and will last months if necessary. Being poor doesn’t mean being stupid – and it doesn’t mean being either.

Given the high cost of obesity we should be looking at how to deal with it as a society. Obesity clearly negatively impacts the health of our children and in many cases it can impact their academic progress. In schools obesity should be treated with the seriousness given to any other addictions students may have. And frankly – perhaps enabling your minor child to become obese should be looked at like any other form of child abuse – necessitating state intervention and counseling for both parent and child.

HMO’s and Employers and the government should be allowed to intervene if an employee or a HMO participant tops 300 pounds – offering counseling, nutritional interventions, and surgery if necessary before weight gets to the point where it’s endangering the health of the patient – and most likely the health of patients’ family, as well. The morbidly obese cost us all money in disability payments and raised health care costs. Obesity, like crime, is everyone’s problem and our society should have better tools for dealing with it.

I wish my peer group had come to me somewhere around 170 pounds and said, “Look – this is not a road you want to travel. Here are some options, let me help.”

The good news is that healthy eating is just as contagious as bad habits. When I began to change my eating habits my obese father followed suit. Healthier and happier he was also able to cut his blood pressure medication in half. My sisters have slimmed down and now fall within a normal weight range. My sister’s family are watching their habits more closely in order to impact the slight chunkiness of my 11 year old niece, before her weight gets out of hand. Now when I meet my family members they are likely to suggest going for a walk or hike – instead of meeting over a meal. If we start making changes, instead of excuses, there is a much healthier light at the end of our big fat tunnel.
