However, many people may confuse about PUD, gastritis and erosions related to the gastrointestinal tract. PUD is differentiated from gastritis and erosions by its ulcerative extend deeper into the muscularis mucosa of the stomach.

There are 3 common causes of PUD:

Helicobacter pylori (HP) -associated ulcers
It has been proved by evidence that HP infection is causally linked to majority of PUD cases. It is known that the presence of this organism (HP) can expose the patients to the risk of getting ulcers although itself do not colonize or cause infection to the duodenal tissue. However, its formation of ammonia using urase enzyme is and can increase the pH of the stomach leading to increased gastrin secretion that can stimulate the secretion of gastric acid while inhibiting the "feedback" mechanism in the body to neutralize the pH. As a result, the continuous acid secret leads to metaplasia of gastric tissue and allow the bacteria to colonize and causing ulcer.

Nonsteroidal anti-inflammatory drug (NSAID)-induced ulcers
This type of ulcer cause gastric mucosal damage through 2 mechanisms:
Direct irritation of gastric epithelium
Systemic inhibitory of cycloxgenase-1 (COX-1) enzyme.
Both have maintained in decreased synthesis of prostaglandin which is an ulcerative cytoprotective agent in the body.

Stress-related mucosal damage (Stress ulcers)
Clinically, PUD patients are adversely affected by stressful life events. However, there is no evidence on its cause-and-effect relationship by any of the controlled studies yet. Neverheless, it is believed that the development of stress ulcer is because of 3 factors:
Presence of excess gastric acid and pepsin
Reduction of mucosal blood flow leading to accumulation of acidic environment causing ulcer
Reduction of cytoprotective agents causing increased mucosal permeability towards acid.

Most PUD occurs in the presence of acid and pepsin when HP, NSAIDs or other factors disrupt the normal mucosal defense and healing mechanisms. Among those factors, acid is the independent contributor of PUD.

Typically, increased acid secretion has been observed in patient with duodenal ulcer (DU) which results in HP infection. Conversely, gastric ulcer (GU) patient will have a normal or reduced rate of acid secretion.

Sign and symptoms:
Abdominal pain, fullness or cramping
Nocturnal pain which may wake you up from sleep particularly between 12am and 3am
Pain lasting for 1-3 hours after meals and is usually relieved by food (DU) whereas for GU patients, food can precipitate pain for them.
Nausea, vomiting and anorexia (common in GU)
Black or tarry stool (GIT bleeding due to compliance of PUD)

Do remember that pain does not always correlates with ulcer. In addition, the severity of symptoms can vary from patient to patient. It can also be seasonal, occurring more frequently in the spring or fall.

Here, it is advisable to consult with your doctor to confirm your diagnosis. This can prevent the development of complication of PUD such as GIT bleeding and perforation due to delay of treatment.

Chest Pain – Is it Angina, Heart Attack or Non-Serious?

Almost everyone suffers from some sort of physiological chest pain at one time or another, and the important thing to note is that there are many causes of this condition.

Chest pain can be defined as a pain or discomfort which occurs anywhere along the front edge of the torso between the upper abdomen and the lower part of the neck. This is a disturbing condition because of the immediate suspicion in the mind of the sufferer that they are, or are about to suffer a myocardial infarction (heart attack).

It is advisable to consult a medical professional if any of the following symptoms are experienced:

  • Sudden crushing, squeezing, tightening, or pressure in the chest
  • Pain radiating to the jaw, left arm, or between the shoulder blades
  • Nausea, dizziness, sweating, a racing heart, or shortness of breath
  • Existing Angina condition, which is suddenly intensified, brought on by lighter activity, last longer than usual, or occurs whilst resting
  • Sudden sharp chest pain with shortness of breath, especially after a long trip, a stretch of bed rest (for example, following an operation), or other lack of movement that can lead to a blood clot in the leg. As previously mentioned, the physiological sources of chest pain can be many and diverse, and include the following;
  • Heart
  • Lungs
  • Esophagus
  • Muscles
  • Ribs
  • Tendons
  • Nerves
  • However, most chest pain sufferers tend to focus on the big question in their minds – that there are just two different classifications – those that relate to the heart (cardiac) and those that do not (non-cardiac).

    Cardiac causes of Chest Pain

    Heart attack

    This is caused by a blood clot that prevails usual flow of blood flow to the heart muscle. This can cause a feeling of pressure, fullness or a crushing pain in the chest lasting more than a few minutes. The pain may radiate to other areas such as the back, neck, jaw, shoulders and arms, especially the left arm. Other symptoms may include shortness of breath, sweating, dizziness and nausea. All, some or none of these may accompany your chest pain.


    Fatty deposits can build up in the treaties that carry blood to the heart.     Other Cardiac Causes

    There are some other complex medical problems, which can cause chest pain.

    Pericarditis – inflammation of the sac surrounding your heart a short-lived condition often related to a viral infection. Pericarditis causes sharp, piercing and centralized chest pain. You may also have a fever and feel sick.

    Aortic dissection – a rare, life-threatening cause of chest pain where inner layers of the main artery leading from the heart (aorta) separate. This causes blood to flow between them, causing a sudden tear chest and back pain. It is usually caused by a physical trauma to the chest cavity, but can also result from uncontrolled high blood pressure.

    Coronary spasm, also known as Prinzmetal's angina, can cause varying degrees of chest discomfort. In coronary spasm, coronary arteries – arteries that supply blood to the heart – go into spasm, temporarily closing down blood flow to the heart.

    Non Cardiac Causes

    There are many conditions that are unrelated to the heart, which can be the cause of chest pain. These include:

  • Asthma
  • Pneumonia
  • Anxiety
  • Rapid breathing
  • Heartburn
  • Pleurisy

  • Collapsed lung

    Edema can be progressive and start to show many other symptoms. Other symptoms that may be experienced are high blood pressure, headaches, increased urination and puffy eyes. Often sufferers of edema may also notice weight gain. This is hard as the mobility is affected by the swelling of the lower legs.

    Preventing edema is always the best measure, but seeking relief is also a good alternative. Since you are dealing with swelling, elevating the legs is always important. When you elevate the legs above the level of the heart, this puts minimal pressure on the back of the knees and thighs.

    You may also try leg wedges to elevate the feet while sleeping. Make sure that your feet fit well in your shoes and there are even supports socks. When you are trying to choose some shoes, just make sure you try them in the late afternoon. If you try them in the morning and your feet swell then they will not fit well in the afternoon.

    Compression massages have been used to help reduce the swelling associated with edema. A compression massage uses inflatable airbags to provide a firm pressure. This helps to relieve tension to reduce the stiffness.

    I know a nurse who shared with me that she never will forget the first time that she saw a panic attack. She was a young nurse – training in the emergency room, when a lady came in the middle of a panic attack episode. Her whole wheelchair was shaking violently. She could not speak. She was really traumatized. Not all episodes are this bad, of course; and fortunately there are panic attack treatments to assist people before they get to that point.

    Obviously, if a person is in real danger, the fight or flight mechanism which started in the adrenalin, with the release of adrenalin, will have many of the same symptoms as a panic attack. This is normal, expected and necessary to help a person out of danger. Panic attacks are seen as a problem when there is no known source for the symptoms, when they are unexpected and / or uncontrolled, and repeated for no apparent reason.

    What causes a panic attack?

    Although the exact causes of these attacks are unclear, the tendency to have anxiety runs in families. Some researchers state that a balance of chemicals in the brain stimulates its development. There also appears to be a connection with major life transitions such as graduation from college and entering the workplace, getting married, and having a baby. Severe stress, such as the death of a loved one, divorce, or job loss can also trigger a panic attack. A panic attack can be caused by a known source of stress and fear or be unexpected.

    Panic attacks can also be caused by medical conditions and other physical causes. The list below is not comprehensive. If you're suffering from symptoms of panic, it's important to see a doctor to rule out the following (or other) possibilities:

    • Illegal drugs such as cocaine and marijuana

    • Chronic stress

    • Major life stressors – getting married

    • Major physical / life stressors – having a baby

    • Recent surgery or anesthesia

    What are the symptoms of a panic attack ?

    The symptoms include a sudden intensified feeling of anxiety, accompanied with alarming physical symptoms. In addition, there may be other mental symptoms such as a feeling of loss of control or dying, or detachment from yourself or reality. The symptoms often escalate within 10 minutes and most of them will be gone within 30 minutes. Some symptoms disappear completely only after an hour. It is best to consult with your physicist to rule out a heart attack. A panic attack can be caused by a known source of stress and fear or be unexpected. The cause of panic disorder is not exactly known, but it is assumed that of balance of chemicals in the brain does stimulate its development.

    For instance, there are common signs associated with a full-blown panic and anxiety attack. It could be one or a combination of the different predominant symptoms such as hyperventilation of shortness of breath. You may consequentially numbness or some sort of tingling sensations, racing heart, heart pounding or heart palpitations, chest pains and discomfort. Other related physiological manifestations are excessive and uncontrollable sweating, nausea, upset stomach, trembling or shaking, lightheadedness, fainting or dizziness, hot and cold flashes and feeling like you are choking, having a blank mind among others.

    There are many symptoms that accompany a full-blown panic attack. These include having an accelerated (racing or irregulara) heart beat, hyperventilation or shortness of breath – or fast smothered breathing, chest pains and discomfort, light headedness, heavy sweating, choking sensation, nausea, upset stomach, trembling or shaking, fainting or dizziness , hot and cold flashes, numbness, and tingling sensations.

    What types of panic attack treatments are available?

    As mentioned before these attacks can be treated so that they do not recur. There are many available approaches to treatment for this condition. However, they do not all work for everyone. The best suggestion is to try various modalities of panic attack treatments and find out which ones work for you. It certainly is dependent also on the reason for the attacks – and your response to them.

During exposure therapy treatment, a therapist helps the patient remember a disturbing thought, traumatic situation, or feared object. The therapist also helps the patient deal with the unseen emotions or physical symptoms that may arise from this exposure. Through confronting the situations and thoughts that cause stress, patients are often able to learn coping skills, eventually reducing or even eliminating symptoms.

    Prescription Medication : These can be helpful – but should be used cautiously – and not long-term. They all have side effects – which should be reviewed. To find out a complete list of the medications available to treat this condition, see your physician.

    1. Antidepressants are sometimes used in the treatment of panic attacks and panic disorder. However, it takes several weeks before they begin to work, so you have to take them continuously, not just during a panic attack.

    2. Benzodiazepines are anti-anxiety drugs that act very quickly (usually within 30 minutes to an hour). Taking them during a panic attack provides rapid relief of symptoms. However, benzodiazepines are highly addictive and have serious withdrawal symptoms, so they should be used with caution.

    Natural cures (natural panic attack treatments):

    1. Change your diet – by eating a healthy diet, avoiding stimulants such as coffee and nicotine, avoid alcohol, and drink plenty of water.
