2. Main purification phase

The next morning, after ablutions and a hot water bath, the patient would be asked to sit in a relaxed manner in an armchair. Milk or sugar cane juice is commonly used for this purpose. The patient drinks as much milk as he can. Then a paste of madana phala, along with yashtimadhu powder vacha, rock salt and honey mixed with milk would be given. Within a short time, the patient feels nauseous and starts vomiting. All impure accumulated kapha will come out. Normally eight bouts are seen. Vomiting is stopped when the pitta or yellow colouration is seen, which is considered the correct symptom. If the bouts are less, vomiting should be by irritating the uvula by touching with the figers or a quill. Then allow the patient to wash his face and fingers with hot water.

Then dhoomapana is done. The smoke should be inhaled through one nostril and exhaled through the mouth which is repeated thrice.

This is followed by paschat-karma. These are mainly food restrictions, called samsarjana kriya. Just after vamana, if the patient is hungry, thin gruel is given. After ascertaining the digestive power, thick gruel, porridge, porridge with meat soup are given. The entire treatment takes 15 days.

However, vamanam is not advisable for pregnant women, the aged , the weak and those suffering from cardiac problems.


Virechanam in panchakarma is a purgation treatment using herbal medicines. It expels impurities from mid zones of the body like the liver, gallbladder and the intestinal tract. Pitta and pitta dominant diseases are cured by this pancha karma treatment. In virechanam, the vitiated doshas are made to pass through the anus. Poorvakarma as done in vamana is also done in virechanam. Though several drugs are mentioned for virechana, trivrit is considered the best and several trivrit combinations are used. It is to be noted that the medicine is given only when the last meal has been digested. Avipathy churna, another medicine, mixed with hot water is given after 9 a.m. No food is given till virechana is over. In proper virechana, the patient passes urine, stool, pitta, medicine, kapha and vata in that order. About 20 purges may be seen . However, it all depends on the patient’s health.

Another mild form of virechana is also used an ayurvedic therapy. It is commonly used to treat various kinds of ailments in which poorvakarma are not necessary.

Kashaya vasti Diseases like hemiplegia and those due to vata are treated well with vasti. However, acute diarrheas, chest lesions, piles, hiccups, are not treated. For kashaya vasti, honey, rock salt, sneham, paste of medicines are required and is mixed one by one in the above order. An emulsion is made by churning all and warming in a water bath. This should be taken on an empty stomach at about 10 a.m. Application is the same as snehavasti. When the bowels are clear, the patient is allowed to take a hot water bath. This should be done after two or three snehavastis. Sneham, paste of medicines and kashayam noted above are selected as per disease and stage.


Nasyam in ayurveda refers to the instillation of medicated oil through the nose. It is the best procedure for the treatment of sirorogas i.e., sodhana of urdhvanga.Nasyam helps to discard the mucous toxins from head and neck channels.

In Pancha karma therapy, nasyam treatment involves application of warm oil to the shoulder and neck of the patient with the patient lying supine and tips his head back. Lightly warmed oil is dropped into each nostril. The patient should inhale deeply. The usual dose is 8 drops but this varies according to the disease and its nature. The timings are morning in kapha diseases, noon in pitta diseases and evening in vata diseases. After the nasya, the shoulder, face, palm and sole of limbs should be massaged briskly for few minutes. Dhoomapana and gargling with hot water should be done afterwards. Nasya should not be done continuously for more than seven days.

3. The Post purification phase

Monophobia is not a particularly common condition. There are a lot of people who think that they have it but most really don’t. The main reason for the confusion is that there is a difference between having monophobia and just not liking being alone. Most people fall into the latter category and while they may have an issue that they need to deal with it is not monophobia. There are some very definite symptoms that you need to look for to determine if you have monophobia or not.

A lot of people don’t like to be alone, this is actually quite normal. Some people are in fact so uncomfortable with being alone that they will go to great lengths to avoid it. This does not however mean that they suffer from monophobia. Monophobia is an irrational fear of being alone that goes far beyond just being uncomfortable being alone. It is important to make sure that you understand the difference so that you can recognize whether or not you have a problem.

The main difference between monophobia and a fear of being alone is that people who suffer from monophobia actually feel unsafe when they are alone. Most people who are not comfortable with being on their own feel this way mainly because they have never had to get used to it. In these cases it is more a feeling of being lonely than of being afraid to be alone.

In the case of monophobia there is an actual fear involved and an irrational one at that. These fears almost always go back to childhood, usually because something bad happened when you were alone. People with this phobia actually feel like they are not safe when they are alone and they will panic if they are placed in this situation. This is very different from simply feeling lonely.

The other big difference between monophobia and a simple fear of being alone is that there will be physical symptoms in the case of monophobia. These symptoms usually occur because there is a panic attack. The result will be rapid breathing, heavy sweating and a fast heartbeat. In fact one of the reasons that people with monophobia are so afraid to be by themselves is that they expect that they will have a panic attack. If you have no physical symptoms than there is a pretty good chance that you do not in fact have monophobia.

Which Anxiety Are We Treating?

Anxiety with ADHD can be difficult to assess and quite refractory to treat. Often whatever medications are tried, just don’t work effectively, and many can significantly contribute to emotional deterioration with impulsivity and depression. The most frequently overlooked anxiety is ‘cognitive anxiety’ and the reason it is overlooked is simple: descriptively, on the surface, it looks like OCD – obsessive compulsive disorder. Obsessions can be a part, can be a symptom, but let’s look more deeply at the cause.

And don’t be surprised if your doctor doesn’t agree with these next remarks, they are not yet in the standard diagnostic text.

7 Tips To Recognize Cognitive Anxiety:

  1. Look first for Worry, for ‘Unmanageable Cognitive Abundance.’ Anxiety presents in our offices when it becomes unmanageable, when life becomes frozen with indecision. Thinking too much can result in three repetitive outcomes discussed below.
  2. Remember Cognitive Anxiety is Subtle, and if you don’t look you won’t see it. Many have so much cognitive anxiety they simply don’t recognize it. If you ask the simple question: “Are you anxious?” the most frequent answer is negative. Word the question like this: “Do you worry, do you think to much and too often about small things and find yourself thinking so much you can’t get things done – that you feel overwhelmed?” This form is the most frequently missed form of counterproductive thinking.
  3. OCD is Not the Only Cognitive Diagnosis: Don’t ask the question only looking for OCD, – yes, OCD can be associated with cognitive abundance. But OCD can present as a symptom for a ‘spectrum of disorders,’ not only ADHD. Use the history to tease out the ADHD details over time. ADHD most often has an academic history except with the Decisive Form below.
  4. A – The Stuck Form: One form of ADHD with cognitive anxiety becomes stuck on the details, and presents as overwhelmed and indecisive. They think so much, about so many details, they simply can’t decide. They need a life partner to decide the next moves. These folks start falling apart in the 5th, 6th, or 7th grades, most often the 6th because the work load changes.
  5. B- The Decisive Form: This form of ADHD is compulsively decisive, a hard worker, shows no ‘inattention,’ but rather runs around fixing and making decisions to preclude thinking to much. They often decide quickly, often do it well, but come in because they too are overwhelmed with micromanagement and picky details thinking they are the only one who can ‘do it right.’ These individuals often do well all the way through school, become professionals, and have no idea what is driving their ‘compulsive’ work driven motor. They can’t stand trash in their mind.
  6. C- The Combo Form; Some individuals with pervasive cognitive anxiety wax and wane between these latter two subtypes. At work they can be strong and decisive as they understand the context and have mastered that game. At home they may have trouble with family, with errands and lack of structure. The spouse often wonders why they are so effective over there, but so ineffective here.
  7. Cognitive Anxiety Treatment Considerations: If the cognitive anxiety is primarily ADHD in origin, it should be carefully treated with stimulant medications. As noted above, watch for SSRI reactions, and drug-drug interactions outlined in detail on my website below if the diagnosis includes depressive elements. SSRIs aggravate ADHD when not combined with a stimulant strategy.

Depression is the word that nobody would want to hear. Millions across the world get affected by various forms every year. One of the types is known as acute depression. You may not notice all the symptoms in sufferers as the symptoms vary person to person. If you notice majority of these symptoms in an individual, do ask him or her to seek medical attention. Supination – What Exactly is it and How Can I Fix It?

What Is Supination?

Supination in the foot occurs normally in the gait cycle when the foot returns to a stable position before push off of. Excessive supination is associated with high arches and is a tendency for the foot to roll outward…or not to move inward (pronate) enough during the gait cycle. As your heel lifts from the ground and the weight moves to the ball of foot area and toes to push the body forward, a natural amount of supination occurs. Over-supination may cause you to appear bow-legged.

The natural movement that is the opposite of supination is called pronation. This occurs when the outer edge of your heel hits the ground and the foot rolls inward. If overpronation occurs during standing, an individual may appear knock-kneed. Pronation and supination are natural movements in your gait cycle, however, with overpronation or over-supination occur, foot problems often follow.

Overpronation is more common than over-supination, but either problem needs addressed. Too much supination causes an issue because the foot is less able to provide shock absorption. It also causes extra rotational force on the feet, shin, knees and thighs, adding further stress on muscles, tendons and ligaments.

What Problems Can Excessive Supination Cause?

Supination places extra strain on both muscles and tendons that stabilize the ankle. This can lead to a number of foot problems which may include:

How Can I Tell if I Over Supinate?

To see if supination could be a problem for you, look at your feet while you are standing to see if there is a high arch at the inside of the foot that doesn’t move toward the floor much when you stand or walk. This is one clue. Also, look at your shoes. If your shoes are worn mostly on the outside of the sole, especially around the ball of foot area, you may have excess supination when you walk.

Another method is to examine a wet footprint. If the area between the heel and ball of foot shows only a thin strip at the outside part (little toe side) of your foot and you see no print at all in the arch area, supination may be occurring. A normal footprint should have a strip that is approximately half the width of the foot. Pronated feet often fill the footprint all the way through the arch.

Can I Prevent or Treat Supinated Feet?

Excessive supination (or pronation) is a biomechanical and foot alignment issue. A pedorthist or podiatrist may complete an assessment and gait analysis to determine the specific alignment and biomechanical problems and prescribe foot orthotics (arch supports) to help you. Foot orthotic insoles work to distribute your weight more evenly and to correct or accommodate abnormal foot posture. They are usually the best option for treatment of supinated feet.

Your choice of footwear is also important when supination is a problem. You should search for shoes that match your foot type. A general rule of thumb is more stability for pronators and more cushion for supinators, but reasonable shoe stability is still important. Make sure they are the most appropriate size and are not too tight or too loose. Try on shoes in the later half of the day because your feet swell a bit as the day goes on and will be slightly larger. Also try both feet, because one will be a bit bigger than the other and you should fit to the larger one. Wear the socks you plan to wear when using your shoes. Feet with over supination often have a high instep, so laces are important for the ability to adjust this area. Running shoes tend to be best for supinated feet to give a good combination of cushion and stability.

These are just a few measures that may be necessary to decrease your risk of injury due to supination. One of the best ways to deal with supination problems is by using arch supports or foot orthotics designed for high arches or supination. These redistribute pressure away from the pressure points of supinated feet and provide better shock absorption. Custom foot orthotics are an option, but expensive. The best over the counter foot orthotics providers offer specifics for different conditions and different areas of pain.

Living Alone with Hemiparesis

Safety would be the primary concern of course when people with hemiparesis live alone or are alone for extended amounts of time. Of course your first references are your doctor, physical therapist and occupational therapists especially. This being stated, I can offer insights on some important items to consider in practical aspects of living alone after a traumatic brain injury or a stroke that result in a severe muscle weakness along the left or right side of the body and is sometimes exhibited in slight nerve damage as with me. Perhaps instead of living by them selves, it would be better to consider us living independently. At this writing, I will have reached almost thirty years of living with hemiparesis. Most of this time I was living alone and working as an engineer and writer, author.

For us, the detrimental effects of hemiparesis are still prevalent even after initial rehabilitation and limited recovery. The effects can change with extreme variation.

Effects When Tired or Awoken

When the body is tired, effects can be worse and certainly more dangerous when living alone. An example is if awoken in the night, maybe by thirst, a call, or other concern. Even a healthy person could possibly stumble on sleepy legs or with eyes half open, but for those with left or right hemiparesis, the negative effects are magnified. A stumble can turn into a fall and possibly a head injury since one arm has poor strength, does not react quickly and is not likely to stop a fall. If a foot drags slightly during the day with shoes, then it can be so much worse at night even so unsafe that the person will roll over the toes so as to bruise them and fall forward on the weak side. Lessons learned here emphasize the need for uncluttered floors and hallways. A stray shoe on the floor or even newspaper can prove disastrous. Either could throw off the balance suddenly for a waking person especially in the dark.

This brings up a suggestion for low level lighting in the home or apartment. Economical battery operated motion detector lights are available at many stores now or a search online. Even if a cane is not needed during the day; it would be wise to have a cane near the bed, even a quad cane for late night awakenings or an early morning rising. In the morning, there are other issues as tightened up leg muscles that need stretching before safely moving.

Shower and Bathroom Safety

Morning stretching is important, especially as we who are injured age but more importantly, shower safety needs to be addressed. There is much assistive devices available and occupational therapists can communicate effectively the needs here, however I can give you my thoughts. These are that it can be quite scary at times, especially if you have had an accident, falling in the shower, tub or bathroom before. You will need to be able to have something within reach on your good side to grab onto, basically at all times. Please note that if you can’t use the screwed in support rails and use the bars with suction cups, you really need at least two bars together. Redundant support as these are known t slip and when you have water or soap in your eyes and reach out with the bar slipping, it can be devastating. I would recommend having a bar on the entry or exit and on the opposite side. This way if a person with hemiparesis turns around there is always a bar on the good side.

But it does not stop there. Bathroom floors are usually smooth surfaces and with one foot having no or minimal grab, then any water on a smooth tile or marble surface is like ice and can easily cause a fall when drying. Rubber backed rugs are most often used but can bunch up under feet. A work around solution for these throw rugs is rubber strips with adhesive on both sides. The local hardware store will have some rug grippers or search the web


This is an area again where an occupational therapist assures you are prepared with any assistive devices for reaching around the weak side or fastening in back and collar. However, I can emphasize that so much more time needs to be available, just in case. Even though, there are assistive devices and there is ample training in doing this usually available before even leaving the hospital.

Food Preparation, Cooking Kitchen Safety

One’s ability to prepare meals will vary. I understand it’s good to be independent but in the kitchen it just may be smarter to minimize preparation with knives or stovetop. Even the oven can cause burns and fire easily. Having a cardboard pizza box fall onto a hot broiler iron can start a fire that is not too easy for someone with a disability to put out quickly. Sure there are now small convenient fire extinguishers available but some often need two good hands to operate correctly and then even finding it quickly can be an issue for trauma, brain injury or stroke victims.

Fortunately, there has been an improvement In microwave dinners. This certainly would not be your first option in the past. However, I have been at this for almost thirty years now and I really have seen a great variety come on the market and costs go down. Good, healthy dinners can be cooked in one minute. I still bake chicken and pizze plus a few others but safety really needs to bethe forst priority. Losing balance and reaching out to find as hot frying pan to lean on can happen and I have personal experience, it’s not a quick heal having a burn in the palm of your good hand. Agsin here as in the bathroom, a rug with rubber backing and rug grabbers should be required.

Mind Body Health Needs

Concentration and Focus are the key in most everything that the person with hemiparesis might have to overcome. That is why ongoing brain training is so important.

Human interaction, relationships contribute greatly to complex usage of our brain and even helping to stay young. Well living alone minimizes this benefit ad over time can have an adverse effect. Sure, some will say the upward potential may be limited but start with the objective of not losing ability. Without regular exercise, mind or body can atrophy to borrow a term used to describe not using muscles.

There are free and paid sources on the web. There are now brain improvement magazines at the local supermarket and brain games are popular and effective, most even for one hand use.

Geographic Area Specific Issues

Depending on your locality, power outages can occur during natural events mostly as snow or ice storms, hurricanes, tornadoes. When living on the eastern coast, I lost power even when their was a high windstorm. Natural events as this can have an impact on people living with hemiparesis just as they do for others with a disability, elderly or families in general. However preparedness is the key here and living alone increases importance of having battery lighting, food, water, cell phone, radio, all available and easily attainable in possibly in the dark.

The power loss may be worse in cold weather for this condition as detrimental effects can disable some people with this extreme muscle weakness and possibly nerve damage. This varies of course depending on person or injury but in general arms and legs can start shaking uncontrollably and a serious limitation to mobility. Poor effects are also exhibit in sudden changes of weather. Each person may experience this differently but a check with your physician is best.

To assist in times as these, warm clothing, even a safe generator where it can be installed in a home as a backup are suggestions with heat packs available at sporting good stores or camping supply shop.


Some of the techniques and tips outlined here may seem obvious to some or simple but they are not always thought of ahead of time before they are used. This is evident in support forums online, knowing individuals and my own thirty years of living with hemiparesis. There are of course more but in general, consideration that movements can be slow at times as can thinking. I Have Blood In My Poop!

You probably don’t want to yell “I have blood in my poop!” when you’re standing in a crowd. However, if you turn around one day and see bloody stools in the toilet, it’s certainly something you might be screaming inside your head. Seeing blood is bad enough, seeing your own blood is worse. When you start seeing blood in your poop, that’s gone too far. If you’ve just exclaimed to yourself, “I have blood in my poop!”, what could be the problem? What should you do?

There are actually quite a few reasons why you might have blood in your poop. Some of them could be serious. Most of them are not. The first thing you need to ask yourself is, “How do I know it’s blood?” You may not have realized this before, but there are actually several foods that can make your poop look bloody. Tomatoes and beets can turn your stools red. That makes sense now that you think about it, doesn’t it? Other possibilities include blueberries, licorice, iron supplements, Pepto-Bismol–all of these can turn your stools black, which looks a lot like blood in your poop. So the first thing you need to eliminate as a cause is your diet. Have you eaten any of these foods or taken any of these substances in the last 18 hours?

If food is not the cause of your strange-looking stools, then you probably should accept the likelihood that you actually do have blood in your poop. By itself, this certainly isn’t the end of the world. But here’s the thing–you need to call up your doctor and tell him, “I have blood in my poop!” Or, you might want to wait until you’re back in one of those private exam room. It’s your decision. But don’t delay making an appointment to see your doctor. Most causes of blood in your poop are minor. But it’s important for you to know for sure.

The very first thing that your doctor will want to know is what color your stools were. The obvious answer is “red,” but not all bloody poop is red. The actual color depends on where the problem is. The farther up your digestive tract the bleeding is occurring, the darker the blood will be. You see, as blood passes through your intestines, bacteria works to digest it like food. The chemical changes that bacteria cause darkens the color of the blood. The longer it stays inside your body, the darker it gets.

So what does this all mean? Well, bright red blood in your stool most likely was added just before exiting your body. The most common causes of bright red blood in your poop are hemorrhoids and anal fissures. Hemorrhoids can begin to bleed when you strain during a bowel movement or even when you wipe a little too roughly. Anal fissures are also quite sensitive to straining and may cause you to have bloody poop. If this is the cause of your problem, the doctor will probably encourage you to drink more fluids and deal with any possible causes of constipation. Another remedy that will provide great results is a colon cleanse. Natural colon cleaners help to remove old waste matter that may be contributing to your constipation.

Darker blood in your stools means that the cause is more likely in your stomach or small intestine. One of the most common causes of blood from your stomach is a bleeding ulcer. Even over-the-counter medications like aspirin or ibuprofen can lead to ulcers that cause no pain but that bleed into your digestive tract. Polyps are not that dangerous by themselves, but there is a possibility that polyps could develop into colon cancer. This type of cancer is much more likely in people over age 50. If that describes you, then you should not hesitate to get screened for colon cancer. Although colon cancer is one of the major cancer killers, it’s also very easy to prevent. By discovering and removing polyps before they turn cancerous, colon cancer can be completely prevented. Whenever you notice anything that might indicate digestive tract bleeding, call your doctor immediately and tell him, “I have blood in my poop!”
