Greek philosophers commenced theorizing about the nature as composed of elements like water, earth, ether etc. This nature or environment has always been a jamboree for the inquisitive souls. This is precisely the reason that man has left no stone unturned to explore it. In talking about nature and environment related issues, the foremost question that comes to my mind is that what is environment? Have you ever pondered over the fact that it is not just the foliage and fauna but also the air you breathe, water you drink, things you touch and perceive and even the food you eat that constitutes your environment! You are encased by environment everywhere and anywhere. This environment has friends as well as foes. In case of opponents, the rational selves inject most of these. For instance factories produce and introduce many toxic compounds in the environment that are hazardous for the entire mankind. 1. The harmful pollutants like carbon monoxide, lead, sulphur dioxide etc constantly pollute the air around us. Carbon monoxide is a poisonous compound, excess inhaling of which can be unexpectedly detrimental. Studies have revealed that CO enters into our bloodstream and causes breathing disorders and even cardiac arrest.

2. Lead is another fatal element that attacks the nervous system and is largely responsible for causing evils like anemia and high blood pressure. Lead can be threatening to life. So it is better to keep a check on the release of lead in your surroundings. Reducing the consumption can do this and judicious use of lead based products and like paints, batteries and radiators etc.

Other pollutants too that enter the air and water we consume and destroy the sanctity of environment (like depletion of the ozone layer) are very dreadful. They can result in usual minor problems like common cold to major ones like asthma, bronchitis and conjunctivitis etc.

In order to avert the infection due to these pollutants or germs it is better to built up a strong immune system. Although much of immunity is genetically inherited yet it can be significantly improved by intaking nutritious and balanced diet and exercising regularly.

3. When we talk about pollution, noise pollution is also quite disturbing and annoying. A surge in the noise pollution level due to the escalating traffic, industries and factories leads to hazards like insomnia and lack of concentration.

4. The sun that enlightens the whole world can even be damaging at times. The excess of sun’s heat and ultraviolet rays that creep into the earth’s atmosphere due to the exhaustion of protective ozone layer, cause many dermdisorders amongst individuals. To avoid these perils as much possible, wear sunscreen lotion, sunglasses and covered clothes while moving out in the sun.

5. Apart from the pollutants like smoke and other toxic gasses, the pesticides that are sprayed in the fields and on the crops have an injurious outcome. Through the vegetables and fruits we eat these pesticides composed of poisonous chemicals like DDT, Methanol, carbon monoxide etc. enter our body and harm it. Therefore it is advised to wash the fruits and vegetables thoroughly prior to eating or cooking.

6. The radiations that are released in the atmosphere due to nuclear reactions, nuclear tests, mobile phones, computers and the like are also a source of danger.

7. The enhancement of carbon dioxide in the atmosphere has resulted in global warming which has turned out to be a massive threat to life on earth. Planting more trees and plants that enrich the air with oxygen can prevent global warming.

Imperialism and Patriarchy in the Poems of John Donne

John Donne was an English poet and satirist who began writing in the sixteenth century. Particularly known for his inventive metaphors, brazen use of language and tonal shifts his works resonate with various cultural themes, patriarchy and imperialism being some of them. He rejected the prevalent notions of Petrarch and firmly believed that consummated love was better than perused one. His poetry is characterized by installation of images that lead the reader to wonder at his expertise of the art. It can be contrasted with the smoothness of Elizabethan poetry and an adaptation of the European baroque and mannerist techniques. He sought inspiration from Ovid, who represented love that was fulfilled and regarded sex to be a religious experience. Donne on the other hand seems to exercise an act of using rhetoric in order to formulate the argumentativeness of his verse comprising jugged rhymes that closely resemble casual speech. Donne basically used his own genius to create his poetry coupled with the notions of the era. Elaine Hobby notes that ”his audience was not a conventional lady but other men subordinated in the legal and economic axiomatic structures and the ideologies of the time which sustained and cemented them.” The overt sense of superiority and dominance can be seen in many of his poems including ‘love’s progress’ where the woman is viewed in terms of her ”central part”, ‘the undertaking’ wherein he establishes that the act of finding a virtuous woman was impossible. ‘The Comparison’ is another poem where he demeans the so called beauty of the lady and in poems such as ‘The Apparition’, ‘The Relic’ that embody the inherent contradictions in the womankind.

Discussing ‘The Elegy: To the Mistress Going to Bed’, the male speaker assumes the role of a teacher and the entire process of disrobing the female becomes an act of usurpation. When he states “to teach thee, I am naked first”, he places the woman in the position of vulnerability and seeks to exploit her both physically and psychologically. He is not a philosopher of love per se but he seeks to explore a new language of love that can contain the opulence of his feelings. Anthony Low avers that he views the female body as an object and lays it bare to be explored and exploited. In most of his poems, though profusely about females there is hardly a female voice to be heard. He silences the woman by his poetically overloaded lines that subvert courtly elegance but nonetheless are engorged with lyrical beauty. George Parfitt views this as being knowledge of love for the male wherein the process the male lover discovers new truths and the meaning of life itself. The entire journey of the undressing of the beloved caters to the sensual and sexual nuances of Donne’s poetry and his obsession with the body of the female. F.R Leavis and T.S Eliot term him as an ”anti-romantic”. Even in his poem ”the flea” he seems to belittle the social considerations of the mistress and almost forces her to grant him immediate pleasure. ‘The Sun Rising’ and ‘The Good Morrow’ are his efforts to create a utopian world for themselves but this world encompasses a ”loquacious, predatory male on one hand and a wholly silent subject on the other” argues Catharine Belsey. The commodification of woman, the act of consciously not praising the virtues in her and the act of wanting to possess her completely are symbolic of Donne’s work. The misogynistic and desperate overtones linger on, juxtaposing them with the complete subordination of the inferior sex, women. The male voice can be analyzed as monarchial, exclusive and oncological that builds a series of imperatives and reiterates them for producing a wholesome effect of the master-slave relationship. Thus the patriarchal heritage features in most of his works and he can be contrasted in this regard with Andrew Marvell who in his poems ”to the coy mistress” combines the persuasion of love with the carpe diem philosophy. The material energies that seem to be the focus of Donne’s body of work do not follow the traditional elegiac pattern but invite the reader to question the prevailing ”politics of love” and examine the power-equation in the hierarchy of the gender roles and the naturalness associated with them. Since love as a moral virtue does not entail its holy connotations for this poet who seems to be interested in pursuing carnal desires and the potency of the male genitalia that raises him to a pedestal and from where he controls both his passion and that of his beloved, so the subjugation seems holistic in terms of spiritual, carnal, emotional and psychological nuances of life.

Imperialism was a factor that was prevalent in England at the time of Donne’s writing. There were energetic and optimistic beginnings of expansion of kingdoms and the manifestation of authority over the oppressed colonies. We can see in his poetry a frequent use of images of cartography, navigation and the like. This can also be attributed to his intrinsic sense of interest in these disciplines. The process of colonization was linked to the process of new discoveries and explorations in terms of topography. The geographical symbolism which categorizes Donne’s work also sustains the process of controlling by force. The territorial advancements of the empire also entail the working of a rule by domination and subordination that is the very essentialist theory of the concept itself. It precludes the exploitation and possession of local resources and includes socially boycotting the subjects of the colonized area.

In Donne’s poem ‘ A Hymn to God, My God in My Sickness’, he uses a variety of metaphors like ‘the pacific sea’, ‘Jerusalem’, images of maps, ‘south-sea discovery’ to describe the process of his last voyage that is from earth to heaven. One perspective views this as being transcendental, but more than that it features as a function of the process of colonization that cannot be subdued in effect. He uses puns, ironies, conceits and dislocations in order to explain the shifting meanings in this text. For example, he puns on the word ‘straits’ as passages that divide oceans to passages that redefine desire. In his poem ‘Elegy: to my mistress going to bed’ he metaphorically addresses his mistress as ‘America, a new found land’. Anthony Easthope defines this as a process of ”de-sexualization” of the beloved linking her to the most crude objects or ideas: such as theology, religion or cartography. This also does not undercut the underpinning of the power equation of the oppressor and the oppressed. The woman as the colonial subject is defined in terms of the ”other” whose difference is both biological and sexual and social. The body of the woman becomes the land to be acquired and seized and thus incorporates geographical dimensions. In ‘The Good Morrow’, heroic quests and commandments over sea-facing expeditions help illustrate this further. Also in poems like ‘Canonization’ and ‘The Sun Rising’, imperialism is hinted upon. In the former, Donne establishes his supremacy at a dual level- at the level of making love and at the level of instructing people around him so as not to intrude in this act. Catharine Belsey states that the objective of the male is to ‘possess’ land and ‘possess’ the woman as well. Thus the constant use of the word ‘possess’ heightens the claim that the woman stands as a passive subject, placed textually to glorify the male speaker’s hegemony. The latter poem speaks of a ‘bed’ to be the center of the universe, encapsulating the entire world of the lovers concentrating the sun’s beams wholly upon them. Thus the breadth of the marital bed, the poet enlists the spectrum of his tyranny over both the women, conquering her literally and also challenging the sun to disempower itself. K.W Grandsen sees these explicit and recurrent references to discovery as a ”sexual intercourse to adventure” where Donne uses and dilutes his fantasies to control and subordinate. John Dryden criticizes him of playing with the minds of the fairer sex. Thus Donne uses the twin processes of imperialism and patriarchy in order to reinforce the stasis in his mind to subjugate the weaker individual-be it the woman or the colonial slave. He breaks from employing the traditional poetic instrument of being slave to the mistress and causes a complete inversion of this style. He subverts the existing model and attempts to carve a niche for himself. Though his poetry can be called ‘un-sentimental’ but it nevertheless stirs up the reader’s emotions. He plays on words giving subtle clues to his readers to gain an in-depth glance of his psyche.

Joan Benet avers that ”Donne has tasted every fruit in love’s orchard”. What Donne attempts to do is not to negate the existing patterns of love but to reinvent them so that they are tailor-made to envisage his theories on it. His range of subjects include religion, theology, divinity and his range of tones include the flamboyant as well as the somber. He cannot be shunned of writing about only the instincts of the flesh or only the domination of one particular sex in this aspect. Coleridge praises him stating that he has wonder-exciting vigor, boundless stores of capricious memory, penetrating thought that he uses at will and wit which he exercises at the most unexpected time!

Narasimha, there are many differences. Lord Narasimha is identified in His different forms such as Lakshmi-Narasimha, Yoga-Narasimha,Bhoga-Narasimha, Chintamni-Narasimha etc. For an effective and easy remedy, it is advised to worship the deity with companion; such as Shiva-Parvathi,Lakshmi-Narasimha and SitaRama.

The deities with their wives beside are easy to please and the worshipper is twice blessed by both the deities. But for worshipping of the planet Ketu,the deity of Rudra alone is suggested. For worshipping of Rahu,the deity of DurgaMata alone is suggested. Like wise, the identification of the suitable deity becomes more important, for an effective and immediate remedy. It is said that God is everywhere and in everyone. But the remedies are not.

Sun – Lord Shiva or Vishnu , on a Sunday

Moon – Lord Shiva on a Monday evening.

Mars – Lord Muruga, Lord Hanuman or Lord Lakshmi Narasimha on a Tuesday.

Mercury – Lord Srirama on a Wednesday .

Jupiter – Lord Ganesha on a Thursday.

Selecting a temple : For worshipping a deity, one must select a very old temple. There are two types of temples. First one type is the temples which exist from ancient times and still getting the daily poojas and archanas etc,. The Second type of temples are that those constructed recently, for the purpose of convenience, or commercial purposes, and getting the daily poojas and archanas regularly and sometimes with all the pomp and show. Avoid the Second types. Any temple to be constructed, there are procedures with systems and methods. The Vastu, the installation, The energizing or Prana Pratishtapana of the deity strictly according to Veda and Agamas,are very important.Then only the deity gets the power to bless the

devotees. In the olden days the temples were being constructed, as per the vastu, or according to the Sthalapurana, or Mahime, mostly in remote areas or on mountains. There are reasons behind the selection of the place of construction of a temple. The place, the installation procedure, the energizing procedure all these play an important role in the worshipping of a deity. Secondly while selecting of a temple and the deity, usually the deity selected may give multiple benefits ,because of the naturally associated other deities. For example, if you are worshipping Goddess Lakshmi, then if one selects Sita Mata as the deity representing Venus, then while worshipping the deity of Lakshmi,The other deities, Lord Vishnu as the deity of Rama, representing planet Mercury , Lord Hanuman, representing planet Mars,or Saturn also bestow their blessings to the worshipper. This is different from the temple where a cluster of deities are installed for mere commercial purposes.

The next step would be to be cautious about the Acrchaks in the temple in performing pooja. There is a tendency that the Archaks put fear in the minds of the devotees, and try to extract money. One need not be afraid of the Archaks, who are mostly not well versed in method and pronouncement or chanting of Mantras. But one need not worry, but will have to take the assistance of such Archaks only. Only one can ask them to adhere to minimum of procedure. The Sankalpa, Archana,Pooja by special but simple ways are to be told to archaks well before. Now I will give examples of worshipping some of the deities ,through a simple and methodical means of worship.

An example of worshipping the deity Lakshmi or Parvathi :If there is a problem of Acne or Pimples recurring on the face, leading to deformation of cheeks, then it must be Shukraditya Yoga according to Phaladeepika. The remedies to the problems relating to marriage, the problems relating to removal of Daridrya, are all likewise obtained through the worship of Goddess Lakshmi or Parvathi with their companion deity besides. Like wise each problem is related to planets and the presiding deity, one way or the other.Here in the problem area of SukradithyaYoga, the planets to be appeased are Sun and Venus. For Sun, the deity is Shiva and for Venus the deity is Parvathi.

Srilalitha, Trpurasundari,Meenakshi ,call Her by any name. “What is in a name”, who said this ? But there is everything in name and deity. One cannot substitute a different deity of same god , for a particular pooja. One must take little strain and should not resort to convenience of the worshipper. For example Parvathi is also recognised as Shakti Devatha , such as Chandi,Durga, Chamundi, Kali etc. Those deities are to be worshipped for a different purposes. For our present purposes, the planet Venus is to pleased. The beautiful, the pleasant, the all powerful, not only possessing all the wealth but also finds pleasure in the exhibition and decoration , with all the pomp and glory , all these are the nature and qualities of the Goddess Parvathi or Lakshmi. In the incarnation of such amorous,benevolent, gracious, georgeous and yet with all these, generous in granting boons, the qualities of the deity represent the planet Venus. Worship her. Because She is easy to please. Worship Her,starting the pooja on a Shuklapaksha First Friday, then continue for five Fridays. A simple pooja and archana is enough. But only specificity is to décore Her with Lotus flowers. This part of the offering Lotus flowers is very important in the whole of the pooja worship procedure. While worshipping either Lakshmi or Parvathi, one is not worshipping the female deity alone. Goddess Lakshmi is found along with Lord Narasimha or Lord Srirama etc., Goddess Parvathi, is found along with Lord Shiva or Lord Sundaresha, with His many names and incarnations. While worshipping a deity of female god, you worship Her Husband The Great Lord also. Likewise, the planet Rahu is worshipped though Goddess Durga or Amba .The all in all, the all powerful deity of Durga is the primordial deity of Shakthi in Her most ferocious form killing the demons and destroying the Asuri-shakthi. She re-istalls the Dharma in this Earth, reposing the confidence of the Satvic devotees. She is liberation.

She is Mukthi. She is worshipped during evening or night. Likewise Lord Shiva is worshipped during night for propitiation of the planet Saturn. Thus the timings, the offerings to be made to a

particular deity, the procedure etc, are all different for different planets and their deities. The following are the procedures in the worship of a deity.

  • Sankalpam : Making a vow before the deity that the person is undertaking a pooja pronouncing his Gotra & Pravara and praying for blessings of the Lord for pooja without any obstacles, by the grace of Lord Vighneshwara.

  • Archana : Offerring of flowers or kunkumam pronouncing the different names of the deity 100 times for Shatanamarchana or 1000 times, if it is Sahasranamarchana.

  • Flowers : Specific flowers are to be offered. Mostly the lotus flowers as in the case of Goddess Lakhmi or Parvathi. Pushpamala of Jasmine, Crysenthium, Dasavala etc., Like wise Bilvapatra and Tulasi are also offered to Lord Shiva or Lord Vishnu respectively.

  • Naivedyam: The suitable prasadam to be prepared and offered.

For example :- Curd and chopped rice, wheat flour and joggery laddus,for Mercury or Kodanda Rama. Kadubu or Obbattu, Vadehara are also offered to all the Vishnu Devatas, including Anjaneya. For Vighneshwara and Lord Shiva , different prasadams are offered. The archak of the deity is to be consulted for offering of Prasadam who is well-versed in this following the local customs.

Starting of Pooja : The Moon is the Karaka for giving strength to all the planets. When the moon is waxing, She is more powerful, and provides the necessary strength and grace to the planet and also to the pooja of the worshipper. So any pooja or for that matter any good work must be started on, the first Monday, First Wednesday or First Friday etc., depending upon the deity to be worshipped, immediately after the New Moon or Amavasya. It is called Shukla-Paksha.

No questions asked : The meeting of the minds of the astrologer and the seeker of the help are very important. The seeker when asking a question and the remedies, the astrologer tells some remedy. The person asks for slight amendment of procedure , like location,substitution of temple, or even delegation of some other person to perform pooja, on behalf of the same family members etc,. This way of convenient or half-hearted worship , dilutes the remedy. Whatever the Previous Karmic impact and resultant problem manifestation troubling the person , who seeks advice, at that moment, the astrologer by intuition tells some remedy propitiation, which performed will definitely yield good results. It is the will power, determination and the frame of mind of the worshipper , which are going to yield the results and fruits of worship, which are all have a definite Karmic ills removal moment. Respect that auspicious moment. Respect the deity you are going to worship. Inculcate and imbibe the qualities of the planet and its deity within you. For example, the planet Jupiter can destroy our sins. He can make the other planets to behave by His aspects. Jupiter stands for Dharma. The History of HMO Plans


Health Maintenance Organization Plans – HMO Plans for short – are a type of managed care program. The idea behind managed care programs is that maintaining good health will be achieved by preventing disease and providing quality care. By maintaining good health, it is believed that escalating health care costs can be controlled.

When HMO Plans were first introduced, members paid a fixed, prepaid monthly premium in exchange for health care from a contracted network of providers. The contracted network of providers includes hospitals, clinics and health care providers that have signed a contract with the HMO. In this sense, HMOs are the most restrictive form of managed care plans because they restrict the procedures, providers and benefits by requiring that the members use these providers and no others.


HMOs were intended to take health care in a new direction. They were designed by the government to do away with individual health insurance plans and to make affordable health insurance available to everyone. At that time employers were purchasing individual health insurance plans for their employees ~ a costly expense that many were starting to forego.

The Health Maintenance Organization (HMO) Act was approved by President Nixon in 1973. The managed health care plans were subsidized by the government and the new HMO-type systems began to grow, typically organized by businesses and community groups eager to make health care available to their workers and members at costs they could better afford. This subsidy created deals from the insurance companies to lure these businesses to buy these new discounted low cost health plans for their employees instead of the costly individual health plans.

Feeling the power of the government behind them and the frantic desire of employers to enroll their employees in these new HMO Plans, insurance companies began to apply pressure to doctors to join an HMO. Doctors were told that if they didn’t join, the insurance company would find doctors who would join and they would effectively take all their patients away.  Thus, doctors ended up joining an HMO so they would not lose their patients and subsequently their entire practice. 

As time went on, the Insurance companies added more and more rules each time the doctor’s contract was renewed. The popularity of the HMO Plans meant that the majority of their patients had HMO plans so they accepted the new conditions.  New terms included seeing more patients, more stringent confidentiality agreements, and more services requiring pre-approvals.

Up until the 1980’s most members agreed that HMO’s were a great health plan. However, by the end of that decade, faced with mounting numbers of denied claims, members began to sour on the HMO Plans.

What led to the increase of denied claims? It wasn’t a result of the claims themselves; it was a result of bad investments by the insurance companies.

During the real estate boom, the insurance companies thought it would be a good idea to invest in real estate deals.  Unfortunately, when the savings and loan industry crashed along with real estate values, insurance companies began to lose money. These losses resulted in their coming up short to cover the claims of their HMO members. 

Thus began the practice to deny the claims of the HMO Plan members. The insurance companies denied claims on the basis that they were too expensive or medically un-necessary. At that time, members and their doctors did not fight these denials and because the insurance company got away with the denied claim process so well, they have continued to do this as part of their operating procedure.

However, a new concept has sprung up in recent year ~ HMO Law.

There are now lawyers and law firms dedicated to bringing cases against HMO’s. These claims include wrongful death, bad faith and medical malpractice. This means that an HMO can be sued when a person dies as a result of the HMO denying coverage for necessary medical treatment; for the denial of valid claims; and for medical malpractice on the part of one of its physicians.

Additionally, individual states are tightening up their laws governing HMO Plans.

How to Eliminate Candida Yeast Infections From Your Baby Safely

It is hardly surprising that many people do experience Candida yeast at some point in their lives. Ayuvedic practitioners regard constipation as a cause for yeast infection in babies and toddlers. The haritaki herb is used to get rid of Candida yeast. It is also recommended that ghee (clarified butter) is given to the child in tiny amounts to lighten digestion and get the bowels and tract moving.

An old home remedy is simply to soak a piece of soft clean cloth or cotton wool in glycerin. Wrap it around your finger and gently cleanse the inside of the baby’s mouth and tongue to rid it of the yeast infection. This can be done daily until the infection is gone. It is recommended it be done every now and then to make certain the mouth stays clear of a yeast infection.

It has been noted that babies and toddlers who drink from a bottle are more prone to outbreaks of Candida yeast infections in the mouth. One theory is that it happens when a bottle is not properly sterilized. In breastfed babies it is possible for the mother to contract a yeast infection on her breasts and nipples. This can be passed on to her baby. It is just as important for a mother to keep a high standard of hygiene and to make sure her breasts and nipples are kept clean. Drying off properly after showering is essential in keeping yeast infection at bay.

Using uncomplicated and natural ingredients and easy practices of personal hygiene is far healthier than subjecting mother and baby to either over the counter products loaded with harmful chemicals or prescription medication.

Removing Tonsil Stones With Highly Effective Proven Remedies

Avoid Insulin Resistance And Manage Diabetes – Naturally

Insulin resistance is when the cell (particularly liver, muscle and fat cells, with the liver losing sensitivity first, then the muscle and then the fat cells) loses its responsiveness on the insulin receptor site. Your body adds more and more insulin to store fat. Overtime, the pancreas give up leading to type II diabetes.

In type II diabetes, your body isn’t making enough insulin and/or the cells are resistant to insulin causing too much sugar to remain in the blood.

How to Lose Weight

On a recent post, Deb, a reader to the God, Diet, Fitness and Weight Loss blog, said her doctor advised her a good place to start to drop some necessary pounds and improve your health is to avoid the 3 S’s. That is great advise if you need to lose weight, improve your health and live a long active life. The 3 S’s all help you add pounds while wrecking havoc on your immune system causing you to be more susceptible to numerous diseases including heart disease, cancer, diabetes ( the 3 deadliest diseases).

Avoid Salt

Our body needs salt. The problem is we take too much salt. The recommended intake of salt is between 500 and 2400 milligrams. Optimally we should consume between 1500 and 2000 milligrams daily. Unfortunately most Americans consume 5 times the amount necessary. This excess sodium causes heart problems, high blood pressure, loss of calcium, increased chances for certain cancers, malfunctioning kidneys, and a long list of health problems not to mention increased weight gain and even obesity. Being obese and overweight in and of itself adds another long list of potential health problems. Therefore if you avoid excess sodium intake you improve your health and reduce the risks of being overweight. Note that reducing the consumption of excess sodium only reduces the risk of being overweight it does not burn fat after you may have already gain too much weight. You must exercise and build muscle to burn fat.

Avoid Sugar

Not just table sugar but all forms of processed sugars and substitutes are bad for you.  Like salt sugar is necessary for our bodies to function properly. Not processed simple sugars, but complex carbohydrates that our bodies convert to the energy necessary to do anything. Our consumption of sugar or substitutes is far worse than the intake of salt. Sugar is in all processed foods. The number one additive in all processed foods sugar is also the most addictive drug we could take. You have craved something sweet right. This is not because because it is good to or for you. It is because your body craves sugar like a drug or alcohol. This addiction causes cancer, diabetes, heart disease, obesity and more deadly diseases. So to be healthy simply limit the sugar you consume. Sugar is the number one cause of death by eating. This slowly deteriorates your your body down to a cellular level, while it wipes out your immune system and adds unusable fat. Obesity kills. Limit all forms of sugar and lose weight while you improve your health. Do not forget to exercise.

Avoid Seconds

This is simple. Do not overeat. Large portions only add excess that become indigestible causing you to become overweight. You do not lose weight simply by eating less. This is a news flash to many people. But you do stop gaining by eating less. You will also infinitely improve your health. One thing is inevitably is you will consume less salt and sugar by avoiding seconds. Healthy Weight Loss includes eating healthy and exercising regularly. What Other Diseases "Masquerade" as Rheumatoid Arthritis? Part 2 – The Infectious Group

While rheumatoid arthritis (RA) is the most common form of inflammatory arthritis, the diagnosis is not always easy to make. The reason is that there are more than 100 different kinds of arthritis. Most of them involve inflammation. When a patient goes to a rheumatologist to get a diagnosis, there is a process of elimination in order to arrive at the proper diagnosis. This process of elimination is called “differential diagnosis.”

Differential diagnosis can be a difficult undertaking because so many forms of arthritis, particularly inflammatory forms of arthritis look alike. Generally it is helpful to divide the differential diagnosis of rheumatoid arthritis into two groups. The first group are the non-infectious diseases to consider and the second group are the infection-related conditions.

In part 1 of this article, I discussed the non-infectious causes of arthritis that need to be considered when assessing a patient with possible rheumatoid arthritis. In this article I will discuss those types of arthritis that are directly or indirectly due to infections.

Many infections can present with arthritis due to either direct inoculation of a joint (either from the outside or from a bloodstream infection) or due to autoimmune reactions. In many instances, infections lead to acute single joint arthritis; however, in some cases, chronic single or multiple joint arthritis can be present.

Missed infections can lead to significant complications; therefore, it is important to have a high index of suspicion for infection in any patient presenting with acute or chronic arthritis.

Here are some examples:

Gonococcal arthritis is an infection due to the organism that causes gonorrhea (N. gonorrhea). It usually affects a single joint (in 90% to 95% of cases). Symptoms include:

o Joint pain that migrates (jumps around) for 1 to 4 days;

o Pain in the hands/wrists due to inflammation of tendons;

o Sometimes a single joint can be inflamed;

o Fevers;

o Skin rash;

o Burning on urination;

o Lower abdominal pain.

The diagnosis of gonorrhea is made by taking the history and by culture or DNA polymerase chain reaction (PCR) analysis of areas of possible infection, including the throat, genitals, and anus. Since the organism that causes gonorrhea is difficult to grow, it can often be missed on culture. Gonococcal arthritis can usually be distinguished from rheumatoid arthritis (RA) by clinical presentation, blood tests, and cultures.

Lyme disease is a bacterial infection due to the spirochete Borrelia burgdorferi. It presents with a skin rash, swollen joints and flu-like symptoms, caused by the bite of an infected tick. Symptoms may include:

o A skin rash, often resembling a bulls-eye; the rash may be more widespread, though;

o Fever;

o Headache;

o Muscle pain;

o Stiff neck;

o Numbness and tingling

o Bell’s palsy

o Swelling of knees and other large joints.

The diagnosis of Lyme disease is typically made by blood tests. Standardization of Lyme tests has improved greatly in the last few years. If chronic single joint arthritis develops, joint fluid analysis or joint tissue biopsy may be necessary for diagnosis. Lyme arthritis can usually be distinguished from RA by clinical presentation and blood testing.

Acute rheumatic fever (ARF) is an inflammatory disease that may develop after an infection with Streptococcus, the bacteria that causes strep throat and scarlet fever. The disease can affect the heart, joints, skin, and brain. Symptoms include:

o Fever;

o Arthritis (mainly affecting the knees, elbows, ankles, and wrists);

o Skin rash and skin nodules;

o A peculiar movement disorder, called Sydenham’s chorea;

o Epistaxis (nosebleeds);

o Heart problems;

o Abdominal pain;

ARF is diagnosed by history, physical exam, and blood testing for antibodies against streptococcus. ARF and RA can have similar clinical features including arthritis and nodules. But, ARF can usually be distinguished from RA. For instance, rash and migratory arthritis (arthritis that moves from joint to joint) are unusual in RA. Blood tests are also useful for making the distinction.

Bacterial endocarditis (BE) happesn when bacteria from the skin, mouth or intestines enter the bloodstream and infect the heart valves and heart lining. Symptoms include fever, chills, and other flu-like symptoms as well as unexplained weight loss and weakness. Diagnosis is made by blood cultures and ultrasound imaging of heart valves. Rheumatoid factor can be elevated in endocarditis, so it is not useful for distinguishing BE from RA.

Arthritis may be a symptom of many viral illnesses. The duration is usually short. Clinical features in adults include:

Joint symptoms occur in up to 60%. Joint pains are more common than true joint inflammation. The joint pains usually don’t last long. They are symmetric, and affect small joints of the hands, wrists, knees, and ankle joints. Morning stiffness and swelling can be present. A rash may be present

The most common cause of viral arthritis is probably Parvovirus B19.

Diagnosis of viral arthritis is usually made by blood testing.

RF testing is not helpful in distinguishing between hepatitis C infection and RA because RF levels can be elevated in patients with hepatitis C. However, in these situations, testing for anti-cyclic citrullinated peptide (anti-CCP) can be useful since these antibodies are not significantly elevated in hepatitis C infections.

What Are the Symptoms of Skin Cancer?

The symptoms of skin cancer are the easiest symptoms to detect. This is because cancer of the skin usually manifests itself in abnormal skin growths that are in plain sight. Although these may sometimes look like a simple sore or a pimple, when it fails to heal over time, this can immediately alert the patient that something is wrong. Also, most cancerous skin growths often ooze some fluid, bleed, and crust or scab over, which are more telltale signs that it is more than just a simple growth. These, however, are usually painless, which is why some people still fail to report their case, leading to delayed treatments.

If you see a new growth on the skin that looks abnormal or does not seem to heal or go away, these should be reported to a doctor. Although many skin growths such as moles are nothing to worry about, it is still best to seek the advice of a medical professional. When a growth is cancerous, however, you can usually tell over time as they often change in size or color.

Symptoms of the Most Dangerous Type of Skin Cancer

The most dangerous type of skin cancer is malignant melanoma. This usually forms on the trunk or on the legs. Although these don’t get constantly exposed to the sun, these areas usually get rare but very intense exposure to the sun, such as when a person spends an excessive amount of time at the beach without applying skin protection products. However, melanomas can also form from an existing mole or freckle, so it is sometimes difficult to identify as well. Keep in mind that when a freckle-like spot on the skin is multi-colored and combines shades of brown, black, red, white, and blue, then it could be a melanoma, not a freckle, which is just light to dark brown in color. Melanomas also have an irregular or uneven border, while a freckle usually have clear-cut borders.

Diagnosing Skin Cancer

Experienced doctors who are trained in diagnosing skin cancer can often recognize its symptoms with one look. To get an idea of the full extent of the disease, however, a complete examination of the entire skin surface will still be needed. Once some symptoms are spotted, a specialist will do a biopsy to determine whether cancer cells are indeed present in the skin cells. This is the surest way to diagnose whether the disease is present or not and to determine its specific type.

In a biopsy, a doctor will take cells or tissues from your skin and analyze them under a microscope. There are two types of biopsy: excisional and incisional. Excisional, which obtains a wider margin of tissue around the affected area, is more effective in diagnosing malignant melanoma. Incisional biopsy removes only a small sample of the growth. This is more common and can usually identify other forms of the disease.
