This was the age of our parents and grandparents. The World War II Era. The first McDonald’s had just opened toward the end of the decade. Now fast food was just beginning to take off.

Now we are beginning to see obesity in the news, and statisticians are keeping track of the health of our nation. We now have solid data from that bygone age and we can get real facts on how fat we are. Still, “Does fast food cause obesity” is being asked and answered in several different ways. In pre-1940, it is almost impossible to find data on the rate of obesity in America. However, in the 1950’s, we do have some pretty good data to work with.

According to the CDC, Americans had a 33% prevelance of being overweight, and a 9.7% prevelance of obesity. Also, there was no data showing that anyone was “severely obese” (BMI >40).

However, flash forward a decade, and we see obesity on the rise, and the number of fast food chains growing as well. McDonalds was franchising all over California and eventually outside of the state in the fifties. KFC got its start in 1952 in Salt Lake City. Burger King started in Miami in 1954. Pizza Hut opened its first door in 1958 in Wichita. So we begin to see a rise in fast food franchises in the 1950s. Obesity in the 1960s and 1970s

In the 1960s, obesity went up about to about 11.3% of the nation. By the 1970s, obesity was well into the double digits at 14.4%. Also in the 1970s, Americans spent a total of $6 Billion on fast food! As you can see, fast food and obesity have a small correlation at this time.

Obesity in the last two decades

Flash forward to the 1980s when there was a 78% increase in the number of fast food stores during the decade. By the end of the 80s, there were almost 120,000 fast food restaurants in the US.

During that same time, obesity in the US rose to almost one quarter of the population (23%)! The number of overweight individuals surpassed the 50% mark and reached 56% of the population. Things are starting to look glum, aren’t they?

By the end of the 1990s, obesity finally rose to 30%! That is almost one in three people being considered obese. Not to mention the fact that two-thirds of the nation was considered overweight in 2000. At that same time, guess how much Americans spent on fast food? Well, obesity doubled since the 70s, think maybe spending doubled too? Try again! Drum roll please….

Disgusted? Me too! And, in 2006 it was expected to reach $142 Billion

Considering about one in three people are obese and we are spending more on fast food now than any other time in history, it sill begs the question, “Does fast food cause obesity?”

I would have to say that when coupled with our lack of physical activity and the horrendous nutritional value of most fast food, that yes, fast food is contributing to the rising tide of obesity in adults and children alike!

As the pain signal, in chemical form, approaches the post-synaptic cell, the membrane changes and the signal is transferred. If we look at the voltages across the synaptic membrane then, under no pain conditions, the level is about -70 mV. When the pain signal approaches, the membrane potential increases to approximately +30 mV, allowing a sodium flow. This in turn triggers the synaptic vesicle to release the chemical transmitter and so transfer the pain signal across the synaptic gap or cleft. After the transmission, the voltage reduces back to its normal quiescent level until the next pain signal arrives.
The application of pulsed magnetism to painful sites causes the membrane to be lowered to a hyper-polarization level of about -90 mV. When a pain signal is detected, the voltage must now be raised to a relatively higher level in order to fire the synaptic vesicles. Since the average change of potential required to reach the trigger voltage of nearly +30 mV is +100 mV, the required change is too great and only +10 mV is attained. This voltage is generally too low to cause the synaptic vesicle to release the chemical transmitter and hence the pain signal is blocked. The most effective frequencies that have been observed from research in order to cause the above changes to membrane potentials, are a base frequency of around 100Hz and pulse rate settings of between 5 and 25Hz.

Scalp Psoriasis

What is Scalp psoriasis?

As the term suggests, scalp psoriasis is psoriasis involving the scalp. It is common and approximately half of all people with psoriasis have it on their scalp. The reason it
deserves special mention is that it can be particularly difficult to treat and usually requires specifically formulated medicines.

Psoriasis in the scalp forms in the same way as in other parts of the body but the effect of the hair is to trap the scale and stop it being rubbed away as it is, for instance, with psoriasis on the elbow. The result is that the scale can quickly build up causing a thicker plaque which becomes more difficult to treat. This difficulty is compounded by the hair which also acts as a physical barrier obstructing the application of creams and ointments to the affected skin.

The net result can be stubborn thick scaly plaques which require specifically formulated scalp treatments.

What are the symptoms?

Scalp psoriasis causes redness and scaliness which may also involve the hairline, the
forehead, behind the ears and the back of the neck. It can range from very mild with
slight fine scaling to very severe crusted thick scaling covering the entire scalp which
can in some cases cause hair loss during the flare, but will normally grow back.
A correct diagnosis of scalp psoriasis is essential in treating the condition as there are
other skin disorders which may look similar such as seborrhoeic dermatitis. The
difference being that scalp psoriasis scales appear fine with a silvery colour, whilst
seborrhoeic dermatitis scales often are yellowish and greasy. One of the most
frustrating symptoms is the constant shower of scale on to your collar and shoulders.

What is the treatment?

Tar is effective but it can stain clothing and jewellery and has a strong smell. As a result, some people dislike using it. The precise instructions for use will depend on the formulation of the product but tar products are usually massaged into the scalp, left in contact for a period of time, and then rinsed off. Clothes and bedding can be protected from staining by wearing a shower cap during the contact period.

Topical Steroids

These scalp products are usually formulated as liquids, gels, oils, foams, sprays or
shampoo. They range from mild to very strong potency. They should not be used for
long periods of time. Ideally, they should be used regularly for a few weeks to bring the psoriasis under control, and then gradually phased out, giving way to maintenance with a coal tar shampoo. Abrupt stopping of steroids can result in a rebound or worsening of psoriasis. It is not advisable to use steroid preparations on your face, other areas of sensitive skin such as under the breast and genitals and around the eyes, unless directed by your doctor. Most topical steroid medications are designed specifically for treating scalp psoriasis. These formulations are usually water and alcohol based which make them easier to wash out after treatment.

You can become resistant to some topical steroids used in the treatment of scalp
psoriasis. If this happens consult your doctor for alternative medications. It should also be noted that it can take several months before such topical steroid medications will work again for skin that has become resistant.

Vitamin D Analogues

Vitamin D analogues are available in water and oil – based scalp formulations. They are usually applied once or twice a day and left in contact with the scalp. They do not smell or stain clothing, and are relatively easy to use. They can be used to bring the scalp psoriasis under control and maintain that control. Such medications should be confined to the scalp region only as it can irritate unaffected skin particularly the face. It may be advisable if you think you are prone to sensitivities to test a small patch before applying it to the entire scalp. Avoid contact with the eyes.

Vitamin A Derivative (topical retinoids)

These can be applied as creams or gels for the treatment of psoriasis and can be used in the treatment of scalp psoriasis too. These medications may be less irritating for the people with dry or sensitive skin. To avoid the drying out of skin too much and to reduce irritation, applying a moisturiser 30 minutes before these are used may help. These medications can be used on the face but should never be applied around the eye region and treated skin should not be covered.

Antimicrobial Treatment

If bacterial or yeast infections are present scalp psoriasis can become worse. A crusting scalp together with scaling and/or lymph nodes in the neck are swollen, this may indicate to your doctor that antimicrobial treatment will be necessary as there is
infection present.

Mild scalp psoriasis can respond well to treatment with anti fungal shampoos that will
help to reduce the yeast infection. Anti fungal shampoos may have to be used once or twice a week thereafter to maintain results.

Other things affecting the scalp

These can include scalp acne, infestations, fungal infections and alopecia. Make sure you get a correct diagnosis before starting any treaments. your doctor or healthcare
provider will provide appropriate advice.

Ultra violet light

Successful outcome for using UV light treatments can be variable on the scalp because the hair blocks UV light from penetrating the scalp. It works best on shaved heads. Natural sunlight may also help again if your head is shaved or very thin. It is therefore not surprising that this is not a first line treatment.

Dithranol

Dithranol creams are effective in scalp psoriasis but like coal tar can be difficult to use.
Dithranol is usually applied to the scaly plaques and left in contact for up to 30 minutes before being rinsed out. It needs to be applied with great care as it can irritate and cause purple staining of blonde or red hair. Dithranol can burn non affected skin. Lipid stabilised Dithranol (Micanol), if used correctly, can reduce staining. It should also be noted that dithranol will stain clothing and baths, showers and wash basins. Extra attention is needed.

Medicated Shampoos

There are many coal tar and non coal tar medicated shampoos for treating scalp
psoriasis at your local chemist. For further advice ask your pharmacist who may be able to give you further guidance. You should also bear in mind that medicated shampoos are designed for the scalp not the hair so the use of a regular shampoo and conditioner after your scalp treatments will reduce the smell of any unpleasant medicated shampoo and leave your hair shiny and manageable.
Scalp psoriasis can get worse if it becomes infected with bacteria or yeasts and sometimes medicated shampoos with antifungal medicines can be useful in reducing the plaques.

Some Useful Tips

Scalp psoriasis may progress down onto the forehead, neck and around the ears. These areas can be treated with the same products you use for your scalp only if indicated to use on such areas. Be careful about your face. Strong steroids should not be used on the face and dithranol can stain and burn facial skin. Avoid getting medication in the eyes.

A lot of product treatments will contain salicylic acid, known as a keratolytic. This
ingredient aims to loosen psoriasis scales so they can be washed away more easily. This ingredient will be contained in both OTC and prescription products mostly found in shampoos and soaps. It should be noted that treatment with high concentrations of this ingredient can cause irritation and used over large areas of skin the body may absorb it leading to the weakening of hair shafts, causing them to break and leading to temporary hair loss. Hair should return to normal after stopping the treatment. Such products may be easier to apply at night and the head covered with a shower cap to avoid messiness and more convenient as they can be time consuming at the beginning of the day before going to work.

Softening and loosening thick scale makes it easier for topical medications to penetrate plaques and clear them. Soaking the scalp in warm water can help loosen scales which can then be removed using a comb. But be gentle – do not break the skin. Round or fine tooth combs or brushes are generally used to remove psoriasis scales. Comb the scalp gently with a light circular motion holding the comb flat against the scalp. Loosen the scale gently then you can shampoo to flush the scale from the scalp and out of the hair. A hairdryer can be used to blow additional scales away from the scalp and the hair. Do not remove scales too fiercely as this can break the skin leading to infection. The Koebner response can occur to damaged psoriasis skin, this can also occur if you scratch or scrape your scalp roughly. If any treatments you use aggravate your psoriasis and scalp always consult your doctor. Care should be taken when removing the scales and applying topical medications so as to avoid the Koebner response.

Never use a shower cap or other occlusion method when using prescription scalp
medications unless specified by your doctor.

Scalp Itch – As mentioned refraining from scratching and picking of psoriatic scales will
reduce infection and the Koebner response. Over the counter medicated shampoos can help in alleviating itching. Please speak to your pharmacist about this.

Combination Medications – The treatment of psoriasis should be tailor made to each
person. A doctor may try various combinations of medications before finding what works for you. Good communication between you and your doctor and good compliance will ensure optimum results for you. Always be aware that when medications are combined they may activate adverse reactions. By being aware of this and consulting your doctor if you are unsure will reduce the likelihood of further discomforts.

Another alternative is to soften and loosen the scales is to use oils, lotions, creams or
ointments applied to a damp scalp providing they do not cause you irritation. To add to their effectiveness a hot towel placed around the head to act as an occlusion will also help loosen and soften the scales. The heating of olive oil and application to the scalp and wrapping your head in a towel for a long period of time may also help or to speed the process up applying the olive oil and then sitting under a hairdryer. Be careful not to burn the scalp with too much intensive heat.

If you are prone or suffer from dry hair conditioners and cream rinses may help to
minimise and moisturise the hair and scalp. There is no evidence to suggest the use of hair dyes, hair sprays or perms will affect your scalp, however please advise your hairdresser before embarking on any treatments so that they can apply patch tests to see if any of their products will irritate your scalp or psoriasis lesions. They may be able to use gentler products. If you have the need to use head lice shampoo preparations it is unlikely you will have a reaction but they can be irritating and great care should be taken not to get the solution into open cuts or lesions or extensively scratched skin.

Hair Matters

It can be highly embarrassing for you finding or going to a new hairdresser or barber. If they are reputable and understanding they should have a knowledge of conditions such as scalp psoriasis and therefore will be able to best advise you on styles, colourings and hair products. It is always worth making general enquiries of friends and relatives about local hairdressers or contacting the Hairdressers’ Council for further advice that may be useful. Some hairdressers are also happy to visit your home if you would prefer not to go to a salon.

When applying scalp products part the hair and hold it in place while you drip the oil or lotion directly onto your scalp. Repeat this process until the whole of the affected area has been treated.

Conditioners and cream rinses to help combat dry hair can be used after you have
completed using your medication.

Hair dyes and sprays do not usually irritate the scalp in scalp psoriasis. However, it may be sensible to test a cosmetic product on a small area of your scalp before committing to it.

Fungal infections

Almost all fungal infections are caused by a type of parasitic fungi known as dermophytes that infect the skin, hair or nails. Fungi are microscopic organisms consisting of cells such as mould, mildews and yeast. They are saprophytes that absorb nutrients from the skin.   • Jock itch looks like a red ring-shaped rash that appears around the groin area. It can be extremely itchy and painful.
  • Athlete’s foot appears between the toes, making the skin scaly and sore. The first sign is a rash. Athlete’s foot should be treated quickly because its makes the skin more prone to bacterial infection.
  • Nail ringworm causes the infected nail(s) to look thick and dull. Toenails are more susceptible to this infection than fingernails. The infected part of the nail may break off from the rest of the nail.
  • Scalp ringworm causes an itchy scalp and hair loss in the infected parts.

How to Treat Fungal Infections?

Proper diagnosis is very important for detecting fungal infections. This may be done by scraping a tiny piece of skin from the affected area to examine under a microscope. Latex Balloon Allergies

Latex allergies are becoming more of a problem and here are some balloon tips and ideas:

The allergies are caused by the powder in and outside of latex balloons.  This powder picks up the latex molecules and then spreads it around a room where it may be inhaled and possibly cause an individual who has a latex allergy to have a severe reaction.

Instead of using the plain rubber balloons for decorations, use the mylar (foil type) or plastic balloons or vinyl balls for table centerpiece, stage and room decorations.   Mylar and plastic balloons are made without latex, last weeks longer than regular rubber balloons and can be reused until the self-sealing valve fails.

To deflate a mylar or plastic balloon for recycling, remove any ribbons from the balloon; insert a common drinking straw into the valve until the air starts to come out.  Gently squeeze the balloon and let the air out till flat, remove the straw, fold the flat balloon and store till next use.  The mylar, plastic, polyurethane and vinyl balloons can be reused until the valve fails. 

The 4″ sized mylars in round, heart and star shape work well as water balloons, fill with a turkey baster and water, hand tie off like a regular balloon.  

The latex balloons are made of rubber; they will stretch from small to large size when they are blown up.  The latex balloon substitutes (latex balloon alternative) will not stretch and are simply flat or filled.

The mylar and plastic balloons will float with sizes 18″ and greater and take 0.5 cu ft of helium each to fill.  Never let any person inhale the gas.

Wash the mylar balloons with warm soap and water and they become clear!

Mylar balloons will conduct electricity.  Plastic balloons in 18” size, round and heart shapes will not conduct electricity.   Please use non conductive curling ribbon and never a metallic ribbon on any balloon.  Some schools and hospitals have started switching to non-latex balloon substitutes for latex sports balls, decorations, science experiments and crafts.  Mylar balloons can be washed with warm water and dish soap to remove the coloring.  End result will be a plastic clear balloon.  Mylar and plastic balloons are not biodegradable and are harmful to the environment.  Please do not release the balloons into the air!

Small vinyl balloons (less than 4 to 4.5 feet in size) will not float, however there are large polyurethane inflatable balloons in different shapes that can be used for outdoor events and do not contain latex.  The most common shapes in jumbo inflatable’s are hot air, blimp and round shapes.  Custom shapes can be made of just about anything.  They are refillable, come with a patch kit, last outdoors for about a year and can be custom imprinted.

Vinyl spherical shaped balloons are an excellent mold for paper Mache’ and plaster projects or even balloon toss.  They are just like beach ball material.  Sizes range from 5″ to 96″.  Vinyl balloons will not float at all.

For a great substitute for latex water balloons.  At your local supermarket, purchase a box of regular sandwich bags, not the zip lock, not the fold lock top, just cheap regular plastic bags.  Fill the corner of the bag with a fist full of water, spin the bag, tie off like a regular balloon, cut off the excess plastic.  They work great, readily available, cheap and fun.

THE PHYSIOLOGICAL CHANGES THAT PRODUCE THE SYMPTOMS

By depression there is a decreased amount of neurotransmitters in parts of the central nervous system, mainly deficiency of serotonin, but also to some extend of noradrenalin, acetylcholine, dopamine or gamma-amino-butyric acid (GABA), or the nerve cells do not react properly by stimulation from neurotransmitters. A neurotransmitter is a signal substance that transmits the nerve signal through the junctions between two nerve cells.

Serotonin and noradrenalin cause nerve cells to send impulses along to other nerve cells, and thus increase the activity in the brain. Deficiency of these substances causes slowness in parts of the brain, and that again causes the depressive symptoms.

The role of GABA is the opposite, namely to slow down some nerve impulses, mainly those causing anxiety and panic response. Lack of GABA causes higher anxiety and easier panic response. Yet, lack of this transmitter also seems to cause depressive symptoms. This is because a too high activity in some brain processes may slow down other processes.

There are many causes and subtypes of depression with different physiological mechanisms involved.

TYPES OF DEPRESSION

Depression is often divided into subtypes according to exhibited symptoms.

1. Mono-polar depression and dysthymic disorder  

By mono-polar depression there are pure depressive symptoms. Mild cases of mono-polar disorder that do not affect a person’s ability to work and to participate in social activities are often called dysthymic disorder.

2. Bipolar disorder (manic-depressive disease) and cyclothymic disorder

In this condition there are periods with symptoms of depression – the depressive phase, alternating with periods of elevated mood level with increased mental and physical activity – the manic phase. In the manic phase, the affected person also sleeps poorly and has concentration difficulties. A mild form of this disease is called cyclothymic disorder.

3. Manic disorder

This condition is characterized by abnormally elevated mood, by unrealistic optimism, by lack of sleep and by hyperactive behaviour. Many psychiatrists think that this disorder is simply the same disease as bipolar disorder where the depressive face has not yet occurred.

4. Depression with mainly physical symptoms

Sometimes the physical symptoms of depression are alone or dominant, as for example: Digestive problems, constipation, difficulties with urination, slow response to sensorial stimuli or slow physical reactions.

CAUSES OF DEPRESSION

Two or more factors can have an effect simultaneously to cause depression. Depression can be an independent disease, or a part of other disease. Depression is also divided into different subtypes according to cause.

1. Reactive depression

This disease is simply a result from psychological stress, physical struggle or mental straining without proper rest or sleep over a long time period. The straining will simply wear out the nervous system or deplete the organism from nutrient necessary for the nervous system to work properly.

2. Endogenous depression

When there has not been any period of stress, straining or lack of rest that can explain the condition, the condition is often called endogenous depression. Inheritance is thought to be a part of the cause.

3. Depression by physical disease

Depression or depressive symptoms may be a symptom of physical disease. This is perhaps the most common cause of depression.

Diseases often associated with depression are: Heart disease, Parkinson’s disease, stroke, hypertension or Cushing’s syndrome.

Mononucleosis or flu may trigger depression that continues after the infection has gone.

By lack of thyroid hormones, hypothyroidism, the metabolism in the whole body is slowed down, including the production of neurotransmitters in the brain. Therefore depression is an important symptom of hypothyroidism.

4. Depressive symptoms as a consequence of unsound lifestyle

A general unsound lifestyle with too less exercise, too high consunsume of alcohol, coffee or tea, too less of important nutrient and too much of sugar and fat may give depressive symptoms, as well as physical problems.

5. Postnatal depression

6. Seasonal affective disorder

Depression can occur in cold and dark periods of the year and go away in warm and light periods. Light stimulates brain activity, and lack of light is a causative factor. 

TREATMENT OF DEPRESSION

Serious or prolonged depression is often treated with anti-depressive medication. Medicines used against depression generally increase the level of neurotransmitters like serotonin in the central nervous system, or they mimic the neurotransmitters.

The medications mostly used today increase the serotonin concentration by decreasing the removal of serotonin from the space around nerve cells. Examples of this medication type are: Fluoxetine (Prozac), fluvoxamine (Luvox), paroxetine (Paxil), escitalopram (Lexapro, Celexa), sentraline (zoloft).

By bipolar disorder in the manic face, heavy tranquilizers (neuroleptica) are used to stop the manic symptoms. By bipolar disorder, lithium salts are sometimes used to stabilize the condition, and prevent new outbreak of depressive or manic faces.

Psychotherapy is sometimes used by depression, usually in combination with medication.

Sometimes serious depression is treated by applying electric shock through the head, electroconvulsive therapy. The shock induces epileptic eruption of nerve signals through the brain and this gives cramps throughout the body. The cramps are alleviated or stopped by applying anaesthesia before the electroshock. This form of treatment is controversial, since it can cause memory loss and is suspected of causing brain damage. The possibility of brain damage is however denied by most psychiatrists.

Adjustment of lifestyle should always be considered by depression or depressive symptoms. Lifestyle measures can sometimes be enough to cure depressive symptoms before a serious depression develop. Lifestyle adjustments can be:

– To slow down a stressful life with too much work or activities.

– Enough rest and sleep.

– A good diet with enough of necessary nutrients.

– Some physical exercise.

– Meditation.

– Supplement of vitamins, minerals, antioxidants, lecithin, amino acids and essential fatty acids.

– Stimulants like coffee or tea may help against depressive feelings in moderate amount. However, if you are a heavy user of these stimulants, you should cut down on your consumption.

There exist nutritional products in the marked to help against depressive symptoms. What is a Phobia?

Phobia comes from the Greek word for “fear”. It is a strong, persistent fear of situations, objects, activities, or persons. Broken down by age and gender, the study found that phobias were the most common mental illness among women in all age groups and the second most common illness among men older than 25.

Phobias affect people of all ages, from all walks of life, and in every part of the country. The American Psychiatric Institute for Research and Education (APIRE) has reported that in any given year, 7.8% of American adults have phobias. They are the most common psychiatric illness among women of all ages and are the second most common illness among men older than 25.

Phobias are among several anxiety disorders, which also include panic disorder, post-traumatic stress disorder, and obsessive-compulsive disorder and generalized anxiety disorder. Such unrealistic or excessive fear of objects or situations is a psychological disorder that can makes life miserable for years.

Phobias should not be confused with fear. Fear is much less dramatic than a phobia. According to the dictionary, the word “fear” denotes a painful feeling of impending danger, evil; trouble etc…the feeling or condition of being afraid. While fear is a consequence of a phobia, it is not the definition of it. It is simply a symptom.

Phobia is also used in a non-medical sense for aversions of all sorts. These terms are usually constructed with suffix -phobia. A number of these terms describe negative attitudes or prejudices toward the named subject.

Some examples include homophobia (fear or dislike of homosexuals), xenophobia (fear or dislike of strangers), and even Christianophobia (fear or dislike of Christians).

It is possible for an individual to develop a phobia over virtually anything. The name of a phobia generally contains a Greek word for what the patient fears plus the suffix -phobia as we have said. Creating these terms can become a type of word game, however, few of these terms are found in medical literature.

Still, many people wear their phobic badges around their family and friends. They may be claustrophobic (afraid of closed in spaces), agoraphobic (afraid of wide open spaces or crowds), hydrophobic (afraid of water), or genophobic (afraid of sex). The list goes on and on.

It is not exaggerated to say that majority of people have an irrational fear of certain things or situations, without necessarily fulfilling the diagnostic criteria for a phobia (irrational fear may grow into a phobia when it starts interfering with the person’s usual activities). The most common simple (specific) phobias are those of the insects (arachnophobia in your case), mice, reptiles and other animals. These phobias usually begin in childhood and often disappear without treatment.

While technically we can call anything a phobia, the actual definition – you will remember – involves having an irrational fear of something that can cause no physical harm or trauma. For example, to have a fear of cotton balls (yes, some people do) would technically be classified as a true phobia since cotton balls cannot cause harm.

However, fearing lions or a hurricane does not classically qualify as a phobia because encounters with either do carry a possibility of harm or death. So what about the fear of spiders?

Technically called arachnophobia, the fear of spiders is real, but is it irrational? Perhaps. After all, we, as humans are slightly bigger than our arachnid neighbors, so can they cause us harm? Yes, but only if we are unaware of their presence. Well, unless it’s the spider in that episode of “Gilligan’s Island” that was the size of a hippopotamus.

At any rate, even though some spiders are poisonous, we still should not have fear of them. We can exterminate them and be on with our lives. For the arachnophobe, yes, that is easier said than done, but it is truth.

So, yes, fear of spiders does really qualify as a phobia.

When encountered with the phobic stimuli, people are overcome with a strong and immediate anxiety reaction (sweating, palpitations, paleness, breathing difficulty). Gradually, mere anticipation or imagery of the phobic stimulus comes to provoke anxiety symptoms and can lead to avoidance behavior and/or significant modification of one’s lifestyle.

With sleep apnea, you may think you have gotten your full eight hours of sleep, yet be groggy and tired the next morning; or you may wake frequently in the middle of the night for no apparent reason.

Ingrown Toenails – How to Treat & Prevent Them

An ingrown toenail is a curved shaped nail which grows into the skin at the edges. Mild curvature of the nail may not result in much pain, but severe curvature can cause redness, swelling, considerable pain and infection. The medical term for an ingrown nail is onychocryptosis. “Onych/o” means nail, “crypt” means hidden and “osis” means disease or condition. The roots of the word describe a hidden nail condition, which is what happens in severe ingrown nail infections. The nail curves in and grows into the skin edges.

The skin becomes inflamed and as the swollen skin covers the nail, the nail edges become hidden. The nail corners can break through the skin, causing an opening in the skin and potential for infection. The medical term for an infected ingrown nail is paronychia. “Par/a” has many meanings, but is defined as alongside of, beside or near, “onych/o” means nail and “emia” means presence of an infectious agent and is shortened to “ia” in paronychia.

The most common area for an ingrown toenail is at the great toenail. Ingrown toenails are common in children who play sports such as soccer or football, in adults who wear certain types of shoes or in individuals with fungal toenails. Many cases can be self-treated, but severe cases may require a procedure and/or antibiotics. Ingrown toenails can be a very serious problem for diabetics or individuals with poor circulation and at the first sign they should seek treatment immediately.

Causes: Some individuals have a genetic predisposition to developing ingrown toenails and the problem can start early in life. More common causes of ingrown toenails include inappropriate shoefit, trauma and improper nail cutting technique. Shoes which are too small can place pressure on the toes and cause the nail edges to curve in. This is more common with rigid, narrow shoes, such as dress shoes or soccer cleats. Shoes which are too large can also cause ingrown toenails.

This is more common with running shoes or hiking boots, when the individual is going up and down hills. The foot slides back and forth in the shoe and the repetitive trauma at the toes increases the pressure on the nail, increasing the chance for an ingrown toenail. Socks which are too tight can also cause ingrown toenails and this tends to be more common with thin dress socks and stockings. This has also been reported to occur with the use of the Strassburg Sock, a soft night splint worn while sleeping and used to treat plantar fasciitis.

Blunt injury or trauma can cause ingrown toenails. Catching the toe on a table leg or bed post can initially cause injury to the toenail. Dropping something on the toe, even of moderate weight, can cause inflammation to the skin surrounding the nail and as the nail grows, it grows into the inflamed skin, worsening the condition. Sports, such as soccer, increase the chance of ingrown toenails. This is more common at a younger age when children are learning the sport and more likely to kick the ball with their toe.

Nails should be cut straight across and cutting the nails too short, cutting down the sides of the nails or excessively rounding the edges can all cause ingrown toenails. Individuals with hyperhidrosis, excessive foot sweating, are at increased risk for ingrown toenails because the excess moisture weakens the skin and increases the chance for bacterial infection. Individuals with a fungal infection of the toenail (toenail fungus/onychomycosis) are at higher risk for developing ingrown nails. The fungus affects the nail bed and distorts the shape of the nail.

Signs and Symptoms: The early signs of an ingrown nail are tenderness along the nail edge, with or without swelling or redness. Many times individuals report dull, generalized pain at their toe when walking or in shoes. In some cases individuals only feel this pain at night, when the bed sheets and covers place pressure on the toe. This pain is reproduced when palpating the nail border. The skin edge may be hard or swollen. As the condition worsens, the skin edge becomes red, the area swells, becomes tender and very painful. There may be oozing, bleeding or pus and the drainage may show up on the socks. Moderate redness and swelling are signs of inflammation, but a significant amount of redness and swelling, pus or red streaks indicate infection.

Treatment: Treatment should be initiated at the first signs of an ingrown toenail to avoid worsening of the condition and possible infection. Soak the foot in warm water and epsom salts for 15-20 minutes three times a day. Wear open toed, or loose fitting shoes, avoid tight socks and decrease activity. The redness and tenderness should decrease within 2-3 days of foot soaking, but the soaking should be continued until all signs of inflammation have diminished. Don’t cut the nail on the edges or down the sides, this could further worsen the problem. If the redness worsens, if there is pus, excess drainage or streaking, see your healthcare provider immediately. If you are diabetic or have poor circulation, do not self-treat the condition.

If there are signs of infection, your doctor may prescribe antibiotics. In many cases, antibiotics can be avoided if the ingrown nail is addressed. A wedge resection of the nail can be performed. This involves your doctor using a specialized nail cutter to take out the wedge of nail growing into the skin. The procedure does not require anesthesia and works best when only the corner of the nail is growing into the skin. Soaks are recommended for 5-7 days following the wedge resection.

The second option is a nail avulsion. A nail avulsion is a partial or complete removal of the toenail. The toe is numbed with local anesthetic and the nail is freed from the surrounding skin and removed at the base. It is most common to remove only the section of nail which is causing the problem. The nail will always grow back in and depending on which toenail is removed, the nail can take up to 8 months to regrow. Warm water soaks with epsom salt twice a day for 5-7 days are recommended and typically a few days after the procedure, the toe is pain free. If the ingrown nail was due to trauma, poor shoe fit or poor cutting technique, when the nail regrows, it should grow in correctly and there should be no further problems or complications.

For individuals with recurrent ingrown toenails, a permanent nail procedure may be necessary. This procedure is called a matrixectomy and involves destroying or removing the root of the nail. A chemical matrixectomy is the most common procedure and this can be done in the office. The procedure is exactly the same as a nail avulsion, but a chemical (usually phenol or sodium hydroxide) is placed on the nail root to destroy it and prevent the nail, or side of the nail, from growing back. In some cases, surgical removal of the nail root may be necessary.

Prevention: The best treatment is prevention and this starts with proper care of your feet and toenails. Cut your nails straight across. Do not leave sharp edges at the corners, but do not excessively round the edges. Cut your nails regularly so they don’t become too long and hit the front of your shoes, but don’t cut your nails too short either. Make sure your shoes fit properly and avoid shoes which are too tight or too narrow. Avoid socks which are too tight and constricting. If you are diabetic or have poor circulation, see your podiatrist regularly.

Toenail Fungus–Cure It Or Live With It?

Nail fungus is most often  found in the toenails and is a nuisance infection which you can choose to ignore or to cure as you see fit. The fungus has no very serious health effects and as a consequence it is quite safe to simply ignore it if you can stand  having thick ugly and yellow looking toenails. The affected nails will be very thick and tough and can sometimes come completely off, after a time, to bare a new toenail which has grown below. This is the result of a fungus infection which is in place right under the nails and as a result is difficult to reach in order to treat it. Some prescription drugs are available through your doctor and these will cure it if you want to  try this. The manufacturers of these drugs do list certain side effects, which can occur in some users, so you should ask your doctor about this before being prescribed the drugs. If you would rather opt for something else there are a few alternative home treatments, which have been found to work fairly effectively.

Vicks Vaporub chest rub, the old standby cold medication can kill the toenail fungus. The only disadvantage is that  this treatment can take a long time. You will probably need to apply the Vicks Vaporub to your toenails once or twice per day for a number of months to get good results. The Vicks preparation smells of eucalyptus oil but you will not notice it since it is under your socks and shoes. This  treatment works  because the medication has some ingredients which are known  to be fungus fighters, although it is made for use  as a cold treatment.

One more remedy, which seems to work, is tea tree oil, which can be purchased at health food stores. It is necessary to dilute the tea tree oil before using it on  the nails, as it has been found in some cases, to cause  some skin irritation if used without dilution. A good recipe to use is to mix about 1/4 cup of cider vinegar in half a cup of boiled water and add roughly 20 drops of tea tree oil. If you want this to smell good you can just add some nicely scented essential oil such as clove oil or lavender oil. You should keep the mixture in a dark glass bottle, as strong light can cause decomposition. Shake  the mixture thoroughly before applying it to the nails and swab the  fungus infected toenails twice or thrice every day. This procedure also may take quite some time to do get the desired results, so you should  be ready to do this for a some months if needed.

Toe nail fungus also responds to application of zinc compounds. The old well known zinc ointment, which is still to be found in many drug stores, should be used on the  fungus infected toenails twice or thrice daily and should aid in curing the fungus infection. This treatment also necessitates application for a number of months to get results and seems not to work as well as tea tree oil treatment, for some cases. Consequently we would use the tea tree oil or Vicks Vaporub as our preferred treatment. What it boils down to is that you can suffer a little in silence with your rather ugly fungal  toenails or you may do something to get rid of the problem if you desire to.

What Are Sprains?

Sprains and strains are often lumped together and are among the most common of sports injuries. A sprain is an injury to a ligament, the thick, fibrous tissue that connects bones together. A strain is an injury to a muscle or tendon. Sprains and strains can both cause tremendous pain. Sprains most often occur in the back or the hamstring muscle, which is located in the back of the thigh. The people most at risk for sprains and strains are soccer, football and hockey players as well as boxers and wrestlers. Hand and arms sprains can happen in gymnastics, tennis, rowing, golf, and other sports that require extensive gripping. Elbow sprains sometimes occur in individuals who participate in racquet sports, throwing, and contact sports. These conditions can develop in anyone who performs strenuous or repetitive movements on a frequent basis however, not just athletes.

What Are the Symptoms of a Sprain?

Swelling, and bruising of the affected joint are the typical symptoms of a sprain. A significant ligament tear (Grade 3) causes an inability to use the affected joint and could lead to joint instability. Less serious injuries (Grade 1) may only cause pain with movement. A person with a mild sprain usually experiences minimal pain, swelling, and little or no loss of functional ability. Victims of a moderate sprain may have more difficulty putting weight on the affected joint and can experience some loss of function. Sometimes people feel a pop or tear when the sprain happens, alerting them to their condition.

What causes a Sprain?
