Acute Gastritis

Acute gastritis may produce no symptoms but can be associated with short-lived dyspepsia, lack of appetite, nausea or vomiting. It can occasionally be severe enough to cause gastrointestinal bleeding with melena or hematemesis (see above). The most common cause is ingestion of aspirin or other non-steroidal anti-imflammatory drugs (NSAIDs). It can also occur during the early stages of infection with the bacteria, Helicobacter pylori “HP.”…..

Chronic Gastritis

Chronic gastritis is extremely common and usually results from infection with HP. In the USA and other developed countries, infection with HP becomes more common with increasing age and up to 40-50% of fifty year old people are infected. In underdeveloped countries the infection is much more common and in some areas up to 90% of the population is infected by adulthood….

In the vast majority of cases stomach biopsies show mild chronic gastritis but usually this produces no symptoms and the patient is unaware of the infection. A small percentage, however, will develop peptic ulcer disease because of the infection (see below). At present, however, there is little evidence that patients with symptoms of indigestion but no abnormality at endoscopy will benefit from treatment of the infection….

Further investigations are usually undertaken by a specialist and treatment of pernicious anemia consists of replacement of vitamin B12 by regular injections. Nothing can be done to restore acid secretion but there is little evidence that this impairs the body’s ability to digest and absorb food. There may be a small increase in the risk of stomach cancer in patients suffering from long-standing pernicious anemia…..

Mitral Valve Prolapse: Truth vs. Fiction

One of the chapters in my recent book, “Courageous Confrontations,” describes my experience with a patient named Emma Jorgenson. Shortly after sitting down in my consultation room, she said, “I hope you can help me, Doctor. Those other doctors keep saying that my symptoms are all in my head.”

“What seems to be bothering you, Emma?”

“Bothering me? My problems aren’t just bothering me, they’re killing me. If you don’t do something to help me, I am going to die. I just know it.”

“What kind of symptoms are you having?”

Heaving a huge sigh, Emma shifted uncomfortably in her chair. “I don’t know where to begin,” she said. “Whenever I try to explain my symptoms to a doctor, he just rolls his eyes, and tells me it’s nothing to worry about.” Emma hunched over, and began to well up.

I reached for the box of tissues on my desk and handed them to Emma. “Let’s figure out how we can help you. How about starting at the beginning?” I said. “What was your first symptom?”

“One night, about three years ago, I woke up with a pounding sensation in my chest. My heart was racing so fast, I thought it was going to jump out of my chest. Then I began to get nauseous and dizzy. I called Dr. Cahill, my family doctor who’s also my gynecologist, and when I went in to see her the next day, she found a tumor in my tummy. She said I needed to have an operation to remove it.”

“What did she find?”

“A cyst on my ovary. It was nothing serious, but after the operation, the pounding and the dizziness became more frequent, so she sent me to a neurologist.”

“Why a neurologist?”

“I’d read an article that said the three most common symptoms of brain tumors were headaches, nausea and dizziness, so I asked her to send me to a specialist. He did a bunch of scans and electrical tests, and said everything was okay. He prescribed a tranquilizer, but I knew that wasn’t going to solve my problem.”

“Did the tranquilizer help?”

“A little at first, but then my symptoms got worse. When the pounding started, in addition to becoming dizzy and nauseous, my hands would begin to tingle and become numb. After a while, the numbness spread to my face. The whole area around my mouth would lose all feeling, except for a wired tingling sensation. I was sure I going to have a stroke. That’s when Dr. Cahill referred me to an ENT doctor.”

“An Ears, Nose and Throat doctor?”

“That’s right. She thought my dizziness might be due to an inner ear problem. He examined me and said he wasn’t sure what was going on, but that I needed an operation to get to the bottom of it. But I was too scared. Besides, I still hadn’t recovered from the ovary surgery, and my wounds weren’t healing right, so my gynecologist said that I could wait before having another operation.”

Emma’s story made me wince. She had unwittingly fallen into the maze of modern medicine. Each specialist viewed her symptoms through the prism of his own specialty, ordered the inevitable battery of tests, and treated her with a pill or a procedure without having a diagnosis. Medications are the fifth leading cause of preventable death in the United States.

“Why did she send you to me?”

“I told her I didn’t think I had an inner ear problem, and that it had to be some kind of a heart condition. After all, how could an inner ear problem cause chest pains and shortness of breath?”

“Chest pain and shortness of breath? You didn’t say anything about that.”

I explained that there are several causes for chest discomfort, and each has a telltale set of characteristics. For example, in patients with pleurisy, an inflammation of the lining of the lungs, pain occurs with deep breaths. With an inflammation of the sac around the heart, called pericarditis, the pain increases when a patient lies down, and improves when they sit up and lean forward. In patients with blocked coronary arteries, the discomfort occurs during physical activity, like walking or climbing stairs. A bulging or tear in the aorta, the main artery in the body, also has characteristic features.

All these possibilities and more needed to be carefully explored by delving into the nuances of Emma’s chest pain, as well as her palpitations and shortness of breath. Emma’s description of her chest pains did not conform to any of the common causes of chest discomfort, but it was important not to overlook other serious possibilities. In patients with pleuritis a rubbing sound can be heard with a stethoscope over the lungs during a deep inhalation. Pericarditis sounds like sandpaper being rubbed in synchrony with the heartbeat. Cardiac birth defects, diseases of the heart muscle, and valve abnormalities all provide telltale murmurs and other characteristic clues on the physical exam.

Despite Emma’s rapid pulse, her blood pressure was normal and her lungs sounded clear. On the cardiac exam, her heart impulse was normal, but when I placed the stethoscope under her left breast, the diagnosis immediately became obvious.

When Emma’s heart contracted, a series of loud clicking sounds were audible. My patient had mitral valve prolapse.

The heart is divided into two sides, each having four chambers. The upper two are called atria, and the lower two are the ventricles. The right atrium and ventricle receives oxygen-depleted blood from the body and sends it to the lungs where its oxygen content is replenished. The left atrium then receives the rejuvenated blood, passing it along to the left ventricle. The powerful left ventricular chamber then pumps its contents back to the body.

The atria and ventricles are separated from each other by valves. The tricuspid valve is located on the right side of the heart, while the mitral valve resides on the left. When the mitral valve opens, blood exits the left atrium, travels through the valve, and enters the left ventricle. As the left ventricle begins to contract, the valve closes, preventing blood from moving backwards into the left atrium.

The mitral valve consists of two leaflets, or flaps, each in the shape of a parachute. Normally, both leaflets close in unison, but in patients with mitral valve prolapse, either the valve leaflets are too large, the chords that attach them to the heart are too long, or the connective tissue in the structure are more elastic than normal. In any case, one or both of them balloons, or flops—prolapsing into the left atrium. The characteristic click heard when listening to the heart is caused by the sound of the valve leaflet prolapsing into the atrial chamber, much like a parachute in the wind.

Mitral valve prolapse is a relatively common condition, occurring in two and a half to five percent of people in the United States. It is particularly prevalent in pre-menopausal women between the ages of fourteen and forty. There has been a considerable amount of speculation about how the valve abnormalities occur, but recent research has shown that there is a genetic predisposition for the syndrome. Between twenty and fifty percent of the relatives of mitral valve prolapse patients also have the syndrome.

Echocardiograms are a valuable means of evaluating patients with suspected mitral valve prolapse. The test confirms the diagnosis by demonstrating the prolapsing valve leaflets. In addition, the presence and severity of any blood leaking backwards across the valve from the ventricle to the atrium can also be detected. In Emma’s case, I did not hear the telltale murmur suggesting the presence of a leak.

Patients with mitral valve prolapse often have symptoms that mimic serious illnesses like heart attacks and cardiac rhythm abnormalities, but in the vast majority of women, the condition is neither dangerous nor life threatening. Most of the close relatives of patients with mitral valve prolapse who demonstrate a floppy valve on echocardiography are completely free of symptoms.

The reason for the chest pains, palpitations, or shortness of breath that occur in some patients with mitral valve prolapse has never been understood. For want of a more scientific explanation, it has been hypothesized that, their nervous systems are programmed to respond excessively to stress. For unknown reasons, they are triggered to react to unthreatening circumstances as though they were dangerous. This imbalance is called dysautonomia.

After putting my stethoscope in the pocket of my lab coat, I patted Emma gently on the shoulder. “I have wonderful news! Your symptoms are being caused by a benign condition called mitral valve prolapse.” I assumed that Emma would be relieved. Instead, the diagnosis increased her anxiety to the point where she became a shut-in.

While the symptoms of mitral valve prolapse are divers and can be frighteningly severe, it is important to emphasize that for the overwhelming majority of individuals with the condition, it is neither dangerous nor life-threatening. Studies have found that increase levels of circulating adrenalin like substances account for the symptoms of chest pains, palpitations, shortness of breath, anxiety and panic attacks that plague people with the problem.

The outlook for the great majority of people with mitral prolapse is excellent. My experience has been that those with debilitating symptoms gradually do feel better over time. They come to realize that their symptoms will not result in a heart attack or sudden death, and the symptoms themselves then become less incapacitating. As one of my patients said, “I just put the pains in my handbag and went about my business.”

For most patients, the only treatment necessary is the use of prophylactic antibiotics before various types of surgery and dental work, but this is used only in those who have a leaking valve. Medication, particularly a group of drugs called “beta blockers” can be useful to control debilitating symptoms. Exercise, a healthy diet and relaxation techniques have all been useful in controlling symptoms.

And as for Emma, she had a transformative experience that changed her life. No longer immobilized by fear, she developed a remarkable new equanimity and a new appreciation of life.

It also gave her a new life purpose. Emma became a patient care counselor and an invaluable member of our health care team, using her experience as a resource to counsel our fear-ridden patients.

How to Cure or Alleviate Edema, Including Lymphedema

By edema excessive fluid gather in body parts or greater areas of the body, making the areas swollen. Edema is most often a problem in the legs, but can occur anywhere in the body.

There are several types of edema, and many cases are also a mixture of more than one type. By lymphedema the drainage from the tissues through the lymph vessels are impaired or excessive fluid goes through the blood capillary walls into the spaces between the cells in a tissue. Edema can also occur because of poor drainage of blood through the veins towards the heart. Edema can further be caused by general accumulation of excessive fluid in the body.

The causes of edema are many. Edema is a symptom by most inflammations. Insufficient heart action by congestive heart failure can cause edema. A too high intake of salt can cause general water retention in the body. Sitting and standing long in the same position can cause edema in the lower limbs. A still sitting life without exercise can cause or aggravate edema.

Insufficiencies of the liver or kidneys can cause fluid retention and thereby edema. Edema can be a side effect of antihypertensive drugs, steroids surgery and radiation therapy. Further causes of different types of edema are varicose veins, cancer, hypothyroidism, blood clots, constipation and physical injuries.

To treat edema, it is important to treat the underlying disease or injury. However, medical treatment does not always cure the underlying condition or the cause is wholly or partially lifestyle aspects. Therefore various measures targeted directly against the edema are useful. These are presented here:

Pharmacological treatments: The physician will sometimes prescribe diuretics to reduce fluid retention in the body and thereby reduce edema. This treatment should however be used for serious edema or edema persistent when other measures do not work.

Salt intake restriction: Restriction of salt intake is an important measure by edema. People suffering from edema should avoid adding great amount of salt into their food or consume preprocessed food with a salty taste.

Diet: A good general diet that alleviates chronic inflammation is useful. The diet should especially contain enough sources of omega-3-fatty acids and monounsaturated fatty acids, like: Fat fish, seafood, flax oil, olive oil, almonds, rape oil and canola oil. Also vegetables and fruit in a natural shape are useful.

Avoiding constipation: Constipation should be treated, since a stomach engorged by excessive content can cause impairment of fluid circulation.

Variation of corporeal position: You should avoid sitting or standing still a long time during the day. When sitting during work or rest, you should change position with regular intervals.

Exercise: Every day you should have some exercise adjusted to your health condition. This could be walking, cycling, swimming, running or any other sport activity where you use greater parts of your body. Doing regular stretching exercises or yoga is useful. By severe edema special therapeutic exercises should be prescribed by a professional.

High position of limb: When you are sleeping or resting the swollen body parts can be placed in an elevated position by means of pillows, pads or by resting in an adjustable bed or chair.

Massage: By minor edema yourself, your spouse or a friend can massage the affected body part to alleviate the edema. The massage should be done in the direction of natural flow of blood and lymph that is towards the heart. You should however avoid massaging distinctly injured or distinctly inflamed body parts. By severe edema a professional massager should be engaged to perform the massage type called “manual lymphatic drainage”.

Elastic bandaging: You can use elastic gloves, stockings, tapes or bandages around the swollen body parts during the day or during physical activities after instruction from a professional person.

Inflatable garments: There are inflatable garments to be found that can be used around the engorged body parts some times of the day. Also these should be used after professional instruction.

Sequential gradient pump therapy: One can get special electric pumps connected to inflatable garments. The pump inflates and deflates the garments repeatedly. Each garment has several rooms that are inflated sequentially in the direction of the natural fluid stream towards the heart. In this way the fluid is dynamically pumped the right way out of the engorged body part. This type of therapy is used in professional settings or at home after instruction from a professional.

Skin care: The skin on a body part affected by severe edema should attain special care to avoid and treat sores. Proper cleaning, daily use of moisturing creams, proper rinsing of skin injuries, antiseptic creams by injuries.

Complete decongestive therapy: This is a complete set of edema treatment techniques done by a professional person combined with self-care instructed by the professional. The techniques used are: Manual lymphatic drainage, compression bandaging, therapeutic exercise and skin care.

Herbal and natural products: Many herbs and products composed of a blending of these herbs are used against edema. Such herbs are: Dandelion, Gingko biloba, horse chestnut, buchu, goldenrod, corn silk, horsetail, juniper, butchers broom, cleaver and ginger. Some people have experienced a good effect from using apple cider vinegar.

The flavonide rich product pycnognenol extracted from the bark of the French maritime pine has shown promising effects against edema according to scientific studies. The flavonides hydroxyethylrutosides also seem to have good effect against edema. Other natural substances that may help to treat edema are the flavonids diosmin and hesperidin and vitamin C.

Oil pulling: The procedure called “oil pulling” may help for edema. It consists of rinsing the mouth with one tablespoon of natural oil for 15-20 minutes and then spitting the oil out. It is done in the morning before eating. Sesame or sunflower oils are usually recommended for this use.

Heart Problems Associated With An Anxiety Attack

Anxiety or panic attacks can heart problems which is anything related to the cardiovascular system of your body. The leading causes of heart attacks or other cardiovascular issues among North Americans today is anxiety attack heart problems which is not surprising given the nature of a panic attack. It is easy to measure the effects a panic or anxiety attack has on the heart and blood flow which is why it is more noticeably dangerous than the effects on the other systems of the body.

A person in the mood of the moment can be paralyzed during the sudden onset of a panic attack when the fear or other terror causes frightening symptoms. To a large degree an anxiety attack resembles the notion many people report of being “frozen by fear”. The great difficulty of a panic attack causes symptoms that the patients discuss as being shaking, trembling, chills, numbness in extremities, heart palpitations and trouble breathing.

Since heart problems are closely associated with stress it is common to have anxiety attack heart problems. Because of a close relationship in bodily functions and systems any heart disease or cardiac condition that causes a drop in blood pressure can accompany anxiety. In fact this causes a sudden decrease in the amount of blood being pumped by the heart which is a decrease in cardiac output. A condition called mitral valve prolapse or MVP is also closely related to anxiety, but it is not the cause.

The “fight or flight” response is a common human response generated by panic attacks. Early human types either fled danger or took it on if they could and it is said our current response evolved from this. Because of the energy the body needs to perform either task there are a lot of biological processes involved in the fight or flight response. Typically when the body experiences this reaction the heart races, the blood quickens, the eyes function differently, senses are heightened and other parts of the body begin to react in other ways such as muscle tightening.

If the heart is being told to work too fast by the part of the brain that is in charge of the “fight or flight” mechanism, then anxiety attack heart difficulties can arise. Since it’s easier on the systems involved, at this time the body often decides simply to shut down. As a result the virtual gamut of both beating faster and beating slower is run by the heart as it attempts to calm the body down and slow blood flow one moment and then speeding the body up and creating more blood flow next.
