Pain is Not Always Bad

It doesn’t surprise me when patients come to see me complaining they are in pain. More than 86 million Americans suffer pain and about 80% will experience back pain at least once in their lifetime. Pain is a major reason why people seek medical care. The question is how is it treated and what happens if pain persists?

I tell my patients that pain is not necessarily a bad thing. Pain can be a warning sign that something is physically wrong. Depending upon the individual’s pain threshold, pain can range from mild to severe; occasional or constant. Pain can either be acute or chronic meaning sudden or ongoing. Let me explain.

oAcute pain comes on suddenly and is usually sharp in quality letting you know that your body is threatened in some way. Most of the time acute pain is resolved quickly but can persist up to 6 months. It is important to heed the warning of acute pain to prevent it from becoming worse and to develop a treatment plan to alleviate the cause. Examples of acute pain are surgery, broken bones, dental work, burns and cuts.

oChronic pain on the other hand lasts longer than 6 months and may be severe and remain even after the injury has healed. The physical effects of chronic pain can result in limited mobility, lack of appetite, energy level, and muscle tension. Chronic pain can be accompanied by emotional effects as well such as depression, anger, anxiety, and fear of recurring injury. Common types of chronic pain include headache, low back pain, cancer, arthritis, and nerve damage.

It is possible for people to suffer chronic pain even if they have never experienced an injury or have evidence of body damage. As a result chronic pain is more difficult to treat because there is no visible cause to explain the problem.

Food Hygiene

The term food hygiene has been defined, by Codex Alimentarius (an international organisation for the development of food standards and guidelines) as “all conditions and measures necessary to ensure the safety and suitability of food at all stages of the food chain.”

As will be seen from this definition the term “food hygiene” encompasses a wide range of measures that can be applied from the growth of food products and the raising of food animals, through harvesting and slaughter, processing, delivery, storage and final sale. However, for many, food hygiene will be equated with cleanliness; the cleanliness of food premises and food handlers.

Whilst the cleanliness of those who handle food and their equipment and surroundings is essential for good food hygiene to occur it is only one of the measures necessary if food is to be safely prepared.

Of equal or perhaps even more importance in maintaining good standards of food hygiene is the avoidance of cross-contamination and good temperature control. Cross-contamination is the passing of food poisoning bacteria from contaminated, usually raw food to ready-to-eat food. Such cross-contamination can occur in three ways, direct contact of e.g. raw meat with ready-to-eat food, the drip of raw food juices such as blood onto a ready-to-eat food and the use of unwashed hands or equipment to handle both raw food and ready-to-eat food.

Good temperature control is essential if satisfactory standards of food hygiene are to be achieved. In most investigations that occur following the outbreak of a food-borne disease a failure in temperature control is identified as one of causes of the event. Foods which are suitable media for the growth of pathogenic (disease producing) bacteria – mainly moist high protein foods such as meat, eggs and dairy products – must be kept either cold or hot. They should not be allowed to languish in the danger zone between 5°C – 63°C for any longer than absolutely necessary.

This means that good food hygiene practices will include the correct storage of cold food at 5°C or below and hot food at 63°C or above. Good food hygiene will also require that food is properly cooked and if not used straightaway that it is cooled quickly to 5°C and if reheated later that this is done quickly to a good temperature.

As far as cooking and reheating temperatures are concerned for potentially contaminated foods, like raw meat and poultry, the usual recommendations in food hygiene textbooks is 75°C for 30 seconds.

Besides cleanliness, temperature control and the avoidance of cross contamination food hygiene measures will also include a safe supply of water for food preparation and cleaning, pest control and the proper training of staff in the principles of food hygiene.

The modern approach to food hygiene puts considerable emphasis on food safety management systems, which are designed to provide assurance that the correct food hygiene practices are always in place. The internationally accepted HACCP approach is now incorporated as a mandatory requirement in Food Hygiene Regulations across the globe. HACCP stands for “Hazard Analysis Critical Control Point” and is a system that establishes the stages in the food business that is absolutely essential (critical) for food safety and seeks to control and monitor them. Such critical controls might include thorough cooking of food, avoidance of cross-contamination and adequate refrigeration of perishable items etc.

So food hygiene really is a wide discipline and its implementation requires commitment and investment in time and money by those who carry the responsibility of providing and preparing food for consumption by the public.

Acne Boils – Cystic Acne Treatment

Acne boils are hard, red, painful swellings, usually beginning as pimples or nodules about hair roots. They increase rapidly in size and develop “cores” in their centers. Carbuncles are unusually severe boils, usually with more than one core or head, accompanied by general illness and debility.

Boils and carbuncles are caused by the same kind of germ – Staphylococcus aureus as a rule. To start a boil or carbuncle, the germs must gain entrance to an oil or sweat gland or hair follicle. It has often been noticed, however, that a general low level of resistance, a low metabolic rate, rubbing in of dirt by clothing, or diabetes paves the way for boils or carbuncles.

The core of an acne boil consists of innumerable bacteria interspersed with white blood cells. It tends to soften and form thick liquid pus around it, which escapes through a break in the skin. The pus, containing live germs, may spread the infection and cause other boils if it comes in contact with unprotected skin. Warmth and moisture hasten the formation and breakdown of the core, and help to keep the skin soft so that the pus may more easily break through. If the skin is kept wet continually, however, it may become soft enough to encourage the spread of germs through it. Dressings wet with strong solutions of salt or other chemicals stimulate drainage of the pus from the tissues into the dressings. Following adequate drainage, a boil subsides quickly.

Never squeeze or pick at a boil. The collection of germs in the forming core may be broken up and spread into surrounding tissues. The germs may even spread into the bloodstream, causing septicemia or “blood poisoning.” The most dangerous spot in the body for a boil to be located is the area marked out by the bridge of the nose, the corners of the mouth, and the outer corners of the eyes. This includes the inside of the nostrils. Many cases fatal septicemia or meningitis has resulted from improper interference with boils or pimples in this area.

What to Do

1. Immediately upon the appearance of a pimple which appears severe enough to develop into a boil – if it is not deep-seated ad has a small yellow spot in the center – dip the point of a needle into tincture of iodine or carbolic acid and open the pimple by thrusting the needle sideways through the yellow spot and lifting the needle. Do not press or squeeze. Wipe off the small amount of pus with a bit of sterile gauze or absorbent cotton. Apply 2 percent tincture of iodine to and around the opened pimple at once. This may abort the boil.

2. If the pimple is deep-seated, or if it does not have a definite yellow center, do not attempt to open it, but paint it and the surrounding skin twice a day with 2 percent tincture of iodine. Let the solution dry on the skin and apply no dressing of any kind for one hour. This will help to protect the surrounding skin from infection.

3. After the skin has been left dry for an hour, apply dressings of several layers of gauze kept wet with a warm saturated solution of Epsom salts on a repeat schedule of two hours on and one hour off. The dressings may be covered with waxed paper, oiled silk, or plastic to prevent their drying out. It is helpful to keep them warm by covering with a hot-water bottle with a layer or two of Turkish toweling between dressing and bottle.

4. The pain of the boil will be considerably relieved if the treatment outline in (2) and (3) is followed, and the boil will probably come to a head and break within a few days. If instrumental opening becomes necessary, it is better to have a physician do it.

5. Penicillin injections and sulfadiazine by mouth are recommended for a carbuncle or a severe boil. The use of these requires the supervision of a physician.

6. It is wise to keep a wide area of skin surrounding the boil disinfected by frequent application of rubbing alcohol or mild tincture of iodine, as described in (2), to prevent the germs in the pus from getting a foothold in the skin and starting new boils.

7. A “run of boils” should always lead to consultation with a physician. If diabetes is a factor, it must be treated. It may be advisable to build up resistance to the special strain of staphylococcus germ causing the boils by giving a course of injections of “autogenous vaccine,” which the physician can have made. Frequent changes of clothing, alcohol sponging, and frequent baths are also important.

Foot Ulcer – Types and Treatment

A skin ulcer is where an area of skin has broken down and you can see the underlying tissue. Most skin ulcers occur on the lower legs or feet. The skin normally heals quickly if it is cut. However, in some people with diabetes the skin on the feet does not heal so well and is prone to develop an ulcer. This can be even after a mild injury such as stepping on a small stone in your bare feet.

Types of Foot Ulcer

Venous ulcers are located below the knee and are primarily found on the inner part of the leg, just above the ankle.

The borders of a venous ulcer are usually irregularly shaped and the surrounding skin is often discolored and swollen. It may even feel warm or hot.
The skin may appear shiny and tight, depending on the amount of edema (swelling). The skin may also have brown or purple discoloration about the lower leg, known as “stasis skin changes.”

Neurotrophic (diabetic)

Neurotrophic ulcers are usually located at increased pressure points on the bottom of the feet. However, neurotrophic ulcers related to trauma can occur anywhere on the foot. They occur primarily in people with diabetes although they can affect anyone who has an impaired sensation of the feet.

Diabetes can cause damage to the nerve and vascular supply in the feet and legs. Patients with neuropathy have reduced or no sensation and, therefore, might be unaware of any trauma to their feet caused by ill-fitting footwear or an object in their shoes, such as a piece of glass, a stone or a drawing pin.
Continued walking on an injured foot will damage it further and minor lesions can become more serious. A simple examination of the feet, feeling inside the shoes before wearing them and not walking barefoot can help to prevent minor injuries. Many of the patients attending the diabetes clinic are older and some cannot see, feel or reach their feet, which means that they are often unaware of any injuries

Diabetic foot ulcers are caused by neuropathic (nerve) and vascular (blood vessel) complications of the disease.

Nerve damage due to diabetes causes altered or complete loss of feeling in the foot and/or leg. This is known as peripheral neuropathy. Pressure from shoes, cuts, bruises, or any injury to the foot may go unnoticed. The loss of protective sensation stops the patient from being warned that the skin is being injured and may result in skin loss, blisters and ulcers.

Treatment options for all ulcers may include:

Antibiotics, if an infection is present
Anti-platelet or anti-clotting medications to prevent a blood clot
Topical wound care therapies
Compression garments

As a rule, the better the control of your diabetes, the less likely you are to develop complications such as foot ulcers. Also, where appropriate, treatment of high blood pressure, high cholesterol level, and reducing any other risk factors will reduce your risk of diabetes complications. In particular, you are strongly advised to stop smoking if you smoke.

Our feet keep us balanced and carry our weight. In a lifetime they walk as much as going five times around the earth. We should look after our feet before any of these problems creep in. Always keep cooking materials clean because these come in first contact with the food we eat. Dry these well after washing to prevent moisture. Wet surfaces are good environments for germs and other bacteria to grow.

Using Gaviscon For Acid Reflux? – Here's What You Should Know

“Is Gaviscon really safe for my Acid Reflux?”

I asked the same thing to myself when I first heard of the drug from a friend of mine. Her description of it was a liquid antacid that tastes good (I don’t know about you, to me… sort of) and is quite effective in alleviating the symptoms of acid reflux, GERD, or heartburn.

Sounds good to me, I thought to myself. But I’m not the person who easily jumps into newer medication, especially the ones that you have to take on a regular basis. If you are like me, I normally do research and get the facts straightened out and just don’t trust any doctor’s advice. I even have my own take on other meds such as the purple pill (Nexium), Maalox, and other antacids (even TUMS).

So I guess it’s a natural reaction for me to ask if Gaviscon is really safe for my acid reflux. Does it actually do what it claims to do? Or are there any better alternatives?

Here’s what I found that I believe every individual suffering from the above digestive problems should know. This is quite a bit long article but the information found here is priceless:

First of all, Gaviscon is an antacid that helps treat acid indigestion, heartburn, peptic ulcers, hiatal hernias, as well as reflux esophagitis. And it is NOT advisable to take Gaviscon for your GERD, if you are both old and suffering from Alzheimer’s.

Gaviscon might also change the effects on some other medication that you are taking – a hard list includes Nizoral, Mandelamine, Rifamate and Tetracycline antibiotics. If you ask me, even if these other meds are non-prescribed or consist of vitamins or food supplements, you still need your wise doctor’s advice.

There shouldn’t be any side effects or complications if you follow the above precautions well, save that the only things you should remind yourself of when taking this drug are that it might cause dizziness and therefore, you should not drive or operate heavy machinery after taking it. One time after dinner and eating the wrong kind of food (spicy pasta), I had no choice but to take it. I did feel a bit dizzy but still managed to drive. I believe the effect varies from person to person.

For knowledge’s sake, the common side effects include “dizziness, drowsiness, nausea, vomiting, dry mouth and diarrhea”, of which if any of these occur while taking Gaviscon, you are advised to consult your doctor immediately.

I’m sure you’re also asking if there is such a thing as Gaviscon overdose. The answer to that is definitely, just like all drugs do. That’s why you should only use the recommended dosage and if you think you had an overdose, contact your doctor immediately. Overdose symptoms include “drowsiness, dizziness, stomach problems, weakness, fatigue, nausea, vomiting and loss of consciousness”. Never ever take a double dose at a time… even if the packaging is pleasing to the eye and it tastes good to you, just like it does to me.

In the course of my battle with acid reflux and GERD, I’ve found that Gaviscon, along with other usual prescriptive medications, should normally be taken for emergency purposes only. If you want to have long-lasting relief and even cure your condition, then conventional methods are not going to be of any help to you at all. At best you can take them for a week, but this is frankly nothing more but a “band-aid” approach – focusing on masking the symptoms, instead of centering on the root cause.

Just having to read and remember all the side effects, precautions, warnings on overdosage, and important things to remember about Gaviscon, and you’re obviously intelligent enough to know that if you’re not careful, you can easily create another disease or health problem in your body because of this drug. That’s one of the things pharmaceutical companies don’t want folks like you and me to know about… even though they are already plain in sight.

If you want an honest recommendation, take Gaviscon for your acid reflux if you have taken all the necessary precautions and if you need to. Take it consistently for a week at the most and then, for emergency purposes only. But find safer and natural ways to treat your condition. You’ll be glad you did.
