Researchers believe that African Americans and African Immigrants are predisposed to developing diabetes. Research suggests that African Americans and recent African immigrants have inherited a “thrifty gene” from their African ancestors.

This gene may have enabled Africans to use food energy more efficiently during cycles of feast and famine. Now, with fewer cycles of feast and famine, this gene may make weight control more difficult for African Americans and African Immigrants. This genetic predisposition, coupled with impaired glucose tolerance, is often associated with the genetic tendency toward high blood pressure. People with impaired glucose tolerance have higher than normal blood glucose levels and are at a higher risk for developing diabetes.

What is Diabetes?

Too often, we doctors stray from accepted standards of care as a matter of expediency, the result of habit, or to satisfy perceived parental expectations. Nowhere is this so apparent as when a pediatrician prescribes a medication that experts agree should never be used by primary care doctors.

A medication that is shunned by pediatricians typically possesses one or more of the following features: it is not safe; it is not effective; its use has been supplanted by newer, better choices; pediatricians do not have the expertise required to prescribe the drug. Using these criteria, let us examine 7 medications that your pediatrician should never prescribe under any circumstance.

Promethazine:

Pediatricians commonly use this medication for the treatment of nausea and vomiting that often accompany viral intestinal infections. Unfortunately, it doesn’t work very well and it frequently produces drowsiness, dizziness and confusion: symptoms we’d like to avoid in a child whose mental status must be monitored as an indicator of dehydration. The intra-venous use of this medication has resulted in tragic events requiring the amputation of limbs due to the caustic nature of the drug, and is now prohibited in many hospitals. Fortunately, we have a much more effective medication at our disposal with a far better safety profile; which makes one wonder why some doctors still cling to Promethazine.

Cough suppressants:

The common cold is the most common ailment in childhood. It is natural to want to provide some relief to a child whose cough is keeping her from sleeping. Unfortunately, the cough medications we have at our disposal have not been shown to be effective in children and may cause unacceptable side-effects. Over the counter cough medicines almost universally include the active ingredient, Dextromethorphan (DM), whereas prescription versions may employ Codeine. Both these drugs are derivatives of opiates. Respiratory depression and behavioral problems have been observed in children taking these products, and in infants, overdoses have resulted in deaths.

In October 2007, the American Academy of Pediatrics proposed to the Food and Drug Administration that the following warning be included on the labeling of cough medications:

“This product has been shown to be ineffective in the treatment of cough and cold in children under six years of age. Serious adverse reactions, including but not limited to death have been reported with the use, misuse and abuse of this product.” Inexplicably, many pediatricians continue to routinely prescribe these medications even to infants.

Dexamethasone eye drops:

This is an example of a medication that can be extremely efficacious, but which pediatricians should not prescribe because they lack the requisite expertise to use it safely. Dexamethasone is a steroid that is utilized for its anti-inflammatory properties. These eye drops are a powerful tool for a variety of conditions but require a careful eye exam by an ophthalmologist prior to their use. If applied in the midst of some eye infections, Dexamethasone-containing drops can exacerbate the infection and lead to irreparable eye injury. This medication is best left to the eye specialists.

Nystatin with Triamcinolone Combination Cream:

Sometimes the sum of the parts is less than the individual parts. Nystatin is a useful topical antifungal, routinely used to treat diaper rashes that have an overgrowth of yeast. Triamcinolone is a potent steroid cream that is effective in treating a variety of inflammatory conditions of the skin, including eczema. The problem arises when these two medications are combined. Doctors, wanting to add some anti-inflammatory effect in the treatment of a yeast rash, or uncertain as to whether the rash is the result of a fungal infection or due to plain inflammation, mistakenly instruct parents to use this product in the diaper area. The potency of all steroid creams is multiplied when applied to skin that is occluded by dressings, plastic wraps, or diapers. When applied under cover of a diaper, Triamcinolone can lead to ulcerations of the skin, which often intensify as a well-meaning parent continues to apply ever more cream in a frantic but futile attempt to alleviate the worsening rash. Only thin applications of far weaker steroid creams might be applied to this extremely delicate area of the body, and only after the careful consideration of potential risks and benefits.

A few decades ago, Cefaclor was one of the only oral preparations of a family of antibiotics known as the Cephalosporins, which are commonly used to treat a variety of childhood infections. This particular medication, however, has a much higher rate of allergic reactions compared to other drugs in its class, including a particularly severe complication known as Serum Sickness-Like Reaction, in which children develop rash, fever, swollen, painful joints, and other troubling symptoms. Furthermore, many bacteria have become resistant to this drug. In a large study published in 2003, out of 19 antibiotics tested, Cefaclor was the least likely to kill the most common bacterium implicated in ear infections, sinus infections and pneumonia. Academic medical centers have stopped using this medication some 20 years ago, but some doctors in the community just can’t break the habit.

Albuterol Oral Syrup:

Albuterol by inhalation, either in an aerosol or inhaler form, remains the most important rescue medicine for the treatment of asthma attacks. When the medication is inhaled, it travels directly to receptors located on the walls of the respiratory airways, signaling muscle fibers to relax, thereby reducing bronchial constriction and improving the flow of air in and out of the lungs. The inhaled route maximizes the amount of medication delivered to its intended target and mitigates common side-effects, such as jitteriness and an accelerated heart rate, that are produced when the drug enters the bloodstream. When the oral formulation is used, the medication must first be absorbed from the intestinal tract into the circulation, by which it then travels throughout the body, with just a fraction of the ingested dose eventually finding its way onto the receptors of the airways. This is a highly ineffective delivery system that magnifies the side-effects while minimizing effectiveness. Albuterol oral syrup is often used not for asthma, but as a type of ersatz cough medicine; a practice that is a relic of the past.

Anti-Diarrhea Compounds:

Diarrhea is a common occurrence in childhood, most often the result of a viral gastroenteritis; what is commonly referred to as the “stomach flu”. At times, it can be the result of bacterial dysentery. The key to the treatment of this usually self-limited but bothersome condition is to provide hydration and nutrition. Guidelines from the American Academy of Pediatrics and the Center for Disease Control and Prevention discourage the use of anti-diarrhea compounds due to their lack of efficacy and the potential for serious side-effects, including severe cramping, and temporary paralysis of the gut, which can lead to a concentration of bacteria and their toxins in the setting of dysentery. Unfortunately, many pediatricians are either unaware of, or choose to ignore these recommendations.

At its best, the practice of pediatrics incorporates scientific evidence, critical thinking, best practices, and accepted standards of care, while embracing humility and a deep compassion; ever cognizant of each child’s unique constitution and the cultural values of his family. Too often, we doctors fall far short of this objective, particularly when we are rushed, when we don’t take the time to listen patiently or examine carefully, or when we fall into the ruts of habit, not questioning what we think we know.

Sir William Osler, a renowned 19th century physician who practiced at a time where many of the treatments were ineffective and fraught with hazards, wrote, “One of the first duties of the physician is to educate the masses not to take medicine”: an admonition that retains a clear ring of truth even in the 21st century. All medications have potential side-effects and, particularly in children, a wise philosophy is to use the fewest medications with the most favorable safety profile for the least amount of time.

Many childhood illnesses are self-resolving conditions that doctors should manage by providing comfort to the child and by counseling parents, rather than blithely dispensing prescriptions designed to subdue symptoms without a conscientious attempt at identifying the true nature of the underlying condition. We doctors must do better. The best parents can do is to become informed consumers of health-care, and never hesitate to question their doctor.

Cough Remedy Tips – Quick Relief When You Need It

To treat cough, its underlying cause must be identified and correct medication must be administered. But this will take some time. When you are in a hurry and you have to give a speech or go to a job interview, all you want is a quick relief from cough. Although the results are temporary at best, quick reliefs should save you from dire situations. So to help you, here is a list of some quick cough remedies:

Saline Solution

Cough is often the result of bacterial infection on the throat. The presence of bacteria results in inflammation which in turn leads to itching. Your body will attempt to relieve the itch by coughing. Saline solution helps in this situation because salt has been known for years for its germicidal properties. For this to work more effectively, you have to gargle with saline solution for several seconds. You should also avoid swallowing and spitting for a while. This will expose your inflamed throat to the saline solution longer.

Medicated Menthol Candy

Medicated menthol candies such as Strepsils can easily be bought as an over-the-counter drug. Although not really designed to cure some underlying causes of cough, they can be used as quick relief. Regular menthol candies with extra strong menthol flavor can also provide temporary relief. In some conditions such as sore throat however, the sugars in regular candies can aggravate the situation.

Regular Mouthwash

Mouthwash can also be used to provide relief from sudden episodes of cough. The menthol content of most mouthwash products is good for an inflamed throat. Also, mouthwash products commonly contain antibacterial ingredients. This is good for disinfecting bacterial buildup in the throat area. A quick mouthwash gargle will provide instant relief from cough in most cases.

Lemon Drops

Fresh lemon drops straight to the throat is also a good way to provide quick relief from cough. The natural ingredients found in lemon juice can disinfect bacterial buildup on the throat. Also, the rich vitamin C content of lemon juice can remedy throat inflammation. Of course, you can also slowly sip down a glass of lemon juice for a more enjoyable way of treating cough. Home Remedies for Cold Sores That Actually Work

Cold sores are a type of facial lesion that is found either on the lips or else on the skin in the area immediately adjacent to the mouth. The herpes simplex virus causes cold sores. After a first breakout often during childhood, the virus may lay dormant for years, but it can come active at anytime. Triggers include fever, infection, stress, sun or windy weather. Cold sores are contagious so take care not to spread them by contact.

Home Remedies for Cold sores

Use Echinacea and goldenseal extract mixture. Echinacea is an immune system booster that can also prevent a cold sore outbreak. Take one dropper-full a day for no more than 3 days. Also put a few drops on a cotton ball and place on the affected area for a few minutes several times a day.

Lemon balm contains a high concentration of polyphenols and appears to minimize herpes outbreaks. It can be applied topically in cream form. Or brew as a tea lemon balm can be taken internally as it has antiviral qualities.

Dab on some lip balm with sun protection. If you have had an outbreak of cold sores in the past, you should wear a lip balm with a sun protection factor (SPF) of 30 at all times, but especially when you are outdoors in the sun, to prevent a cold sore, says Dr. Kakita. You can find lip balms with high SPFs in sporting goods stores and drugstores.

Avoid the foods that are acidic or very salty such as citrus fruit and potato chips as they can both aggravate cold sores as well as being painful on the lesion and make sure you eat red meat and dairy products which are high in lysine.

Because sun can cause cold sores, it is advisable to use a sunscreen, particularly a high protection cream designed specifically for the lips.

Creams containing lysine, or based on lemon balm or tea tree oil help moisturize the infected area as well as provide a protective layer and fight the virus.

A lot of the better home remedied for cold sores typically include taking vitamins and other supplements, as well as changing your overall diet. You can also use ice packs or try applying different juices or powders directly to the sore.

Lowering Blood Sugar – Naturally

Chances are you know someone, probably more than one person, who is affected by some form of diabetes. Or, perhaps, you are one of the 57 million Americans who are pre-diabetic right now. But did you know there’s an herb for that? Actually, there are several.

The National Diabetes Statistics, 2011estimates that diabetes affects 25.8 million people of all ages. There are 18.8 million diagnosed and 7 million undiagnosed, or 8.3 percent of the United States population. But what is more alarming than these statistics is the growing number of early onset diabetes in younger populations.

Further, they report that diabetes is the leading cause of kidney failure, non traumatic lower-limb amputations, and new cases of blindness among adults in the U.S. It is also a major cause of heart disease and stroke and it is the seventh leading cause of death in the U.S.

The medical community follows standard medical practices and pharmaceutical treatments, including insulin to treat diabetes in the United States today. But there is a growing movement, as this disease becomes so much more intimate to us all, that is taking another look at possible natural alternatives that can help us control, prevent and possibly reverse the ravaging affects of diabetes.

The first two and the most obvious additions to a treatment plan are exercise and diet. A combination of an aerobic and a strength training program will do wonders to lower stress levels, decrease belly fat and stimulate your metabolism. And as we all know (we just have trouble following it) a diet high in fiber and low in fat and bad carbohydrates is our best prevention against blood sugar issues.

But beyond that, there are herbal extracts and tinctures that you can easily incorporate into your daily routine that could greatly enhance your quality life and heal damaged cells that have been affected by dangerous sugar levels.

Gurmar, also called Gymnema Sylvestre, Cinnamon and Blueberry Leaf are three herbs whose properties have shown great promise as a natural way to control and possibly eliminate blood sugar issues.

The herb, Gurmar has been used in Ayurvedic medicine for over 2000 years and is an indigenous plant in the tropical forests of India. It has been used to treat diabetes, digestion and urinary tract infections, as well as obesity and hypoglycemia. Known as the “Sugar Destroyer” this herb boosts the amount of insulin your body produces, reduces excessively high blood sugar levels, and allows your body to process sugar more efficiently.

It is interesting to note that Gumar extract is without side effects, unless you are on pharmaceutical medications that perform a similar function and exerts its blood sugar-lowering effects only in cases of diabetes. When given to healthy volunteers, Gumar extract, does not produce any blood sugar lowering or hypoglycemic effects.

As with any new substances you may introduce into your body, anyone with diabetes should use Gumar only under the supervision of a physician and should never be used in place of insulin. Gumar can enhance the blood glucose lowering effects of insulin and hypoglycemic drugs and, consequently, blood glucose levels. Because of this, your blood glucose levels should always be monitored closely by your health care provider.

The second herb is Cinnamon. There is a growing interest in the use of Cinnamon and the potential benefits it may offer in the treatment of diabetes. Recent U.S.D.A. reports have demonstrated that extracts of Cinnamon may increase insulin activity, enhance the breakdown of glucose and improve blood sugar concentrations.

Type- 2 diabetes represents 90 to 95 percent of all diagnosed cases of diabetes and is the most chronically diagnosed form of diabetes. Daily Cinnamon consumption may actually lead to a dramatic reduction in blood sugar, cholesterol and triglycerides by as much as twenty percent in Type-2 diabetic patients who are not taking insulin. When you lower your blood sugar level and improve your insulin resistance weight loss will often follow. So, much like Gumar, Cinnamon can become a part of your weight loss program as well.

There are 4 types of diabetes. Two types are fully recognized and two are still being researched to fully understand the anomalies of each type.

Type 1 diabetes or juvenile-onset diabetes develops when the body’s immune system destroys the pancreatic beta cells. These are the only cells in the body that make the hormone insulin that regulates blood glucose. If these cells are destroyed, the body can no longer deliver glucose to the cells through the bloodstream. The bloodstream fills with sugar and begins to damage the body.

Type- 2 diabetes, or once called, adult-onset diabetes, usually begins as insulin resistance when the cells are not using insulin properly. As the need for insulin rises, the pancreas gradually loses its ability to produce insulin.

Type 3 diabetes is brain based and has been linked to Alzheimer’s. This can type of diabetes can lead to a disruption brain synapses that make up memory and cognition. The brain becomes insulin resistance, the brain becomes inflamed and cognitive and memory blockages can occur.

Type 4 diabetes is hypoglycemia, or low blood glucose. When this type of diabetes occurs it is recognized by elevated insulin levels that lead to chronic persistent and often progressive pain, as well as a loss of sensation. As the symptoms worsen, patients develop digestive, cardiac, sexual and other severe complications. It can be and often is progressive.

For type-3 and type-4 diabetes, Blueberry Leaves extract may be helpful because of their anti-inflammatory properties. This herb has been shown to lower blood sugar when consumed on a daily basis. Blueberry Leaf can also stop damage to the blood vessels, which is sometimes associated with the symptoms of these types of diabetes. But more importantly, Blueberry Leaves can reduce inflammation and lower triglyceride levels in the blood.

The recent discovery that Blueberry Leaf extract reduces blood sugar levels by 26% and reduces plasma triglyceride by 39% indicates its usefulness in the treatment of diabetes. The herb contains a compound called Myrtillin which acts like insulin, though it is not as strong as insulin. And, it is far less toxic. A single dose of Blueberry Leaf can show continuous results for several weeks.

Blueberry and Blueberry Leaves are considered safe and have no documented drug interactions. However, coumarins in Blueberry may increase blood thinning activity and inhibit platelet activity. Psoriasis: Low Temperatures Vs. Ultra Low Temperatures

Nowadays psoriasis still remains a riddle both to experienced dermatologists and to the psoriasis sufferers.

It’s believed that at the base of psoriasis, there lays a genetic predisposition. Some lucky person may live for her/his entire life without ever developing psoriasis. In the less lucky persons under the influence of a provoking factor, they will develop psoriasis.

What may serve as a provoking factor for psoriasis? It may be a stress, an intellectual over-strain, an infectious disease, bad diet etc…

Hypothermia may also become a provoking factor for psoriasis.

Hypothermia – the Low Temperatures.

What is hypothermia? Hypothermia is a prolonged lowering of the internal body temperature.

Hypothermia is a very dangerous condition, which may occur when a person is being subject to low air or water temperatures for a prolonged period of time. Hypothermia does not always occur in the winter. It may also occur in the spring, autumn, or even the summer. For example the body temperature of a child who has been swimming in cool water for a long time, may lower without him/her even noticing that.

The body temperature of a human is maintained through the so-called homeostasis — the mechanism which insures the stable internal environmental of our bodies. With hypothermia, the homeostatic systems will eventually fail to produce high enough heat levels in the body.

Hypothermia is very dangerous because it can lead to incapacitation, frost bite and even death in the worst scenario.

Even though hypothermia is not the most common trigger of psoriasis, it was known to trigger psoriasis in some cases.

Hypothermia is a threat for example for the survivors of a sinking ship. People in this case are subject to the cold waters until they are rescued. Water absorbs heat very quickly and therefore the body temperature of a person staying in cold water will start to drop very quickly.

In this case, hypothermia would also be connected with stress induced by fear. Such combination would especially be capable of provoking psoriasis in a genetically predisposed person.

Cryotherapy – the Ultra Low Temperatures.

Surprisingly, the ultra low temperatures are reported to be capable of treating psoriasis. Ultra low temperatures are used in the so-called Cryotherapy. Cryotherapy has practically nothing in common with hypothermia.

What is Cryotherapy? Cryotherapy is a treatment with the use of ultra low temperatures. Such low temperatures, as they use in Cryotherapy, cannot be achieved without special equipment. The usual temperature of the gas used in the Cryotherapy composes -150° Celsius.

The contact of the body surface with the ultra low temperatures with Cryotherapy composes only several minutes.

The skin temperature at the time of the Cryotherapy procedure lowers down to -2° Celsius. This provokes a potent stress on the skin and in the whole body. This type of stress is probably one of the only ones, capable of influencing psoriasis beneficially.

The irritating action of the Cryotherapy causes the mobilization of various impaired body functions, i.e. it may help to repair the skin affected by psoriasis. Cryotherapy is especially known to be effective for the treatment of the scalp psoriasis.

Explaining the "Hungerstat" – Read This Before Considering a Gastric Bypass

When I have an obese patient in their 300 pound state, many times they come to me for advice and to clear them for gastric bypass surgery. Some insurers require these patients to undergo 6 months worth of diet and psychological counseling before undergoing the surgery, some just require that they have a primary care doctor like me to look after them around the perioperative period (the time surrounding the surgery) because of all of the complications involved with a risky surgery. I am usually the person who says whether it is safe to move forward with surgery.

However with my obese patients I hate giving this clearance because I have seen so many of my patients end up with complications… including death. I had one patient die from sepsis after the surgery. One patient ended up going the opposite direction from obesity and became bulemic. One patient constantly gets transfusions due to iron deficiency anemia and yet another has b12 deficiency because the part of the stomach that absorbs vitamin b12 is blocked from absorbing now. And I have countless patients who lost weight… only to gain it back. For every successful patient after the gastric bypass, I have 8-9 other patients who have had major complications that will go on for the rest of their lives because of the permanent change made to their stomach.

But I am having less and less patients experiencing these issues, yet STILL having the weight loss similar to the gastric bypass. The reason is because of my new understanding of the “Hungerstat,” the thermostat for hunger in our bodies. I give my patients an alternative weight loss method that is free and does not require surgery and most of them have avoided bypass with this system of weight loss. Let me explain the hungerstat before I get to this diet.

The hungerstat is in all of us and is controllable. Our satiety levels can be forcefully changed by surgery and gastric bypass. That’s a given or else we wouldn’t give our most unhealthy patients such a dangerous option for treatment. However Alwin Lewis, M.D., M.P.H. and I have been using our bodies’ natural hungerstat control system to divert our patients away from gastric bypass to allow them to lose weight at the same rate WITHOUT SURGERY.

How do we do this? We use our bodies’ natural system for losing weight and maintaining health. When we were hunter-gatherers before the ice age, when famines or winters came along and food was scarce to hunt or gather, our bodies would get hungry… for 3 days. After 3 days, the hunger goes away because the body goes into catabolic mode: a mode that allows it to burn fat. When you eat 10 or less bites of food per day, this mode is automatically turned on after 3 days.

So to be specific, if you eat five bites twice a day, after 3 days of hunger, and tiredness, your body turns off the feeling of hunger and is satisfied with minimal food! The body becomes satisfied because it starts burning fat to keep you alive. It’s how we were designed. Your hunger thermostat, or hungerstat, tells the body to not feel hungry anymore.

Let’s look at kids in those poorer countries, in those commercials asking you to sponsor them for a few bucks a day. When you see them, they are sleeping. Even though they are malnourished, believe it or not, they are not feeling hunger. They look terrible, don’t get me wrong, to be in that situation. But nature was nice enough to design us to not feel that hunger after 3 days so we don’t have to suffer. Those kids are using their fat reserves to survive and their hungerstats are fully turned off.

So the amazing thing is if you eat five bites twice a day only, in 3 days you will feel absolutely no hunger. You will feel just like you had a gastric bypass.

However here is the second amazing thing. After 14 days of easily taking in only 5 bites twice a day, the stomach naturally shrinks down. In other words, after 14 days of eating only 5 bites twice a day, the stomach shrinks to a point where eating more than 5 bites makes you feel drastically full. Your thermostat for hunger goes down to a point where it feels satisfied with smaller amounts of food. Your portion sizes, from that point forward, are naturally controlled as if your stomach had gone through a gastric bypass. My patients that are on this diet end up having portion control for the rest of their lives because they have naturally controlled their hungerstat so they no longer feel the need to go to the buffet with a full plate for 3 visits, they no longer overeat, and they no longer have the capacity in their stomachs to overeat (unless they blow it all and eat huge amounts of food and overstretch their newly-shrunken stomach’s capacity).

If you’ve ever seen a friend after surgery, many a time their appetites just won’t allow them to eat more than a few bites off their plates. Hospitals constantly have people with full plates on the table even though the surgeon has given the person the OK to eat as much as they want. But many people just can’t get their appetites back up after a few days of not eating due to abdominal surgery or abdominal conditions. These people are having the same exact effects on their hungerstat. So the question is… is it really the gastric bypass helping people lose weight, or the post-surgical period creating a lower hungerstat?

So with a drastic portion reduction and only eating 5 bites twice a day, in 3 days you feel no hunger, and in 14 days you feel like you are satisfied after only 5 bites. Sounds like a person who has had a gastric bypass doesn’t it? But instead, you are using your body’s natural hungerstat to make the same exact changes.

After a gastric bypass, you end up eating 3 bites 3 times a day. When my patients come to me for pre-operative counseling, I put them on Dr. Alwin Lewis’s “Five bite diet” and they lose the same amount of weight, have the same effects on their hunger, and avoid the surgery altogether. While on this diet, they lose 1 pound a day. And best of all, the diet costs NOTHING. You save from the surgical costs, and the costs to your body as you avoid life-threatening surgery.

At first you won’t see any marks or rashes but when they appear, you can easily confuse them with other skin conditions like eczema, etc.

The most frustrating part is the extreme itching especially at night or after taking a hot shower. This is a result of your skin being irritated from their eggs and waste matter. Many people find it really frustrating to get a good amount of sleep because of the intense itching.

Permethrin is one of the most commonly prescribed treatments for scabies. It’s a synthetic chemical that has a very wide range of insecticidal activity. It works by attacking certain nerve endings of the scabies mites. This causes complex reactions to different organs which leads to the destruction of the nervous system and to the death of the scabies mites.

Your first irritation is due to your skin reacting to the mites and the waste they leave behind rather than just the burrowing areas. It’s believed that new bumps after Permethrin appear because the chemical aggravated the original rash and it’s really hard to tell the different irritations when your skin is in this state of infestation.

Scabies itching tends to feel a lot worse because the first application of Permethrin kills the mites that you didn’t know you had and hasn’t started to itch yet. Permethrin treats you in the first few days then suddenly the itch becomes so intense. You may think that you’ve been infected again but that’s not really the case.

Permethrin is one of the most common scabies treatments but unfortunately it also makes you more itchy and leaves you a stinging and burning feeling in your skin. Like I’ve mentioned, it’s a chemical and it’s made from a synthetic form of Pyrethrum which is a common ingredient found in insecticides!

Fungus Infection of the Scalp (tinea Capitis)

There are several types of fungal infections which can infect the scalp. One of these is the ringworm commonly referred to as Tinea capitis. Ringworm infection of the scalp is extremely common world wide. It is predominantly an infection of toddlers and young children. The infection is quite obvious because it presents with constant itching, redden scalp and patches of hair loss. Despite the alarming symptoms, ringworm of the scalp is not life threatening. The most difficult problem is the treatment. When it is not adequately treated, it can be easily transmitted to friends and family.

Symptoms: Most children who have a ring worm infection will have some symptoms, but on rare occasions they will not have any. Constant and intense itching around the scalp is the most obvious sign. Other features of the infection include loss of hair in circular patches. Often the skin is red and tender. The hair loss is quite significant. Even the hair that remains can be easily pulled out without any tension. In almost all cases, ringworm infections of the scalp tend to increase in size if it is not treated. A small patch will generally infect other areas of the scalp. Besides infection of other areas of the scalp, ringworms can easily be transmitted to others: person to person transmission is quite common. Other modes of transmission include sharing personal care items, bed linen pillows and combs. Sometimes pets acquire the infection and can transmit it to humans.

Risk factors

Ringworm is most common in children who attend day care and kindergarten. Outbreaks of ringworm infections are quite common in North America and difficult to control. Even inanimate objects like pencils, door handles, chairs and tables can transmit this fungus. The risk of ringworm infection is also increased if:

– one has poor hygiene

– lives in overcrowded conditions

– has problems with excessive sweating

– shares personal care items in a large family

Once a ringworm infection of the scalp is suspected, it is important to seek help from a health care professional. This fungus is quite hardy and is easily transmitted to others in the home.

Diagnosis

The diagnosis of tinea capitis is relatively simple. In most cases the dermatologist will be able to make a diagnosis based on the history and physical examination. If there is any doubt about the diagnosis, scrapings of the skin or hair can be examine underneath a microscope. In rare cases, the hair and skin are sent for culture.

Fungus of the scalp is not serious but can have social and health implications. If not treated, the fungus can causes a severe infection known as kerion. Kerion is basically collection of pus underneath the scalp. When the fungus has reached the stage of a kerion, most people are left with permanent hair loss and even scars.

Treatment

Once the diagnosis of tinea capitis is made, treatment of scalp infection includes a variety anti fungal medications. The most common medications are Griseofulvin and Terbinafine.

Griseofulvin is available as liquid and tablet whereas terbinafine is an oral capsule. These medications have to be taken for at least 4-6 weeks. Topical medications are not useful in the treatment of tinea capitis because they are not able to penetrate deep into the scalp and enter hair follicles. There is no shampoo or gel that is useful in the treatment of tinea capitis.

The medications work slowly and changes are gradual. In most cases, the pills must be taken for several weeks to notice a change in the condition of the scalp.

Prevention

Tinea capitis is best prevented, but prevention of this infection is quite difficult. The fungus is very easily transmitted. The majority of people acquire the fungus long before they have any symptoms. The only way to prevent the fungus is with the following approach:

Maintain decent hygiene. Clean or shampoos the scalp regularly. Regularly wash your hands and teach your children the same principles.

Do not share personal care items, especially in school or in any type of social gathering.

Teach your child to keep his/her personal locker/desk clean at school. After any outdoor activity, the clothes should be changed and personal items should not be shared.

Examine your child’s scalp every now and then so that you can identify the infection at an early stage.

If you have pets, examine them and if they appear sick, take them to a veterinarian. If your pet is sick, do not touch the pet with bare hands.

If you suspect that your pet or child has an infection, buy selenium sulfide shampoo and use it regularly. If the infection appears to be getting worse, go to your health care worker or veterinarian.
