If you’re as nearsighted as I am, you’ve probably fantasized about getting rid of your glasses or contacts forever. Many people choose Lasik (laser surgery to reshape the cornea), which takes care of their nearsightedness. Lasik is great if you’re in your 20s or 30s, but it does nothing to address age-related presbyopia-the condition that forces most of us to wear bifocals or trifocals in middle age and beyond.

When I was in my late 30s an ophthalmologist recommended Lasik but added, “Of course, you’ll still need to wear glasses to read or work on the computer.” I guess he wasn’t paying attention when I told him that I make my living as a writer and editor. I spend most of my waking hours reading and working on the computer. Why would I pay thousands of dollars for Lasik surgery if it meant I would still have to wear glasses most of the time?

I have never felt attractive in glasses. When you’re very nearsighted, the lenses make your eyes look smaller. I wore contact lenses from age 18 to 40, but around my late 30s I began to notice that my eyes would turn red every time I put in my contacts. My eye doctor explained that I had “thin tears” that didn’t form a good buffer between the contact lens and the surface of the eye. I decide that bloodshot eyes were even less attractive than Coke-bottle lenses, so I gave up on wearing contacts.

Then a few months before my 49th birthday I interviewed an eye surgeon for an article in a local magazine, and he told me about another option for nearsighted people in middle age and beyond: refractive lensectomy. Simply put, this procedure is cataract surgery before the insurance company will pay for it. Instead of replacing the natural crystalline lens of the eye with a monofocal lens (to correct nearsightedness or farsightedness but not both), the surgeon inserts a multifocal lens. Multifocal lenses (with trade names like ReZOOM, ReSTOR, and Crystalens), provide good vision at a variety of focusing distances.

Of course, there’s a downside. The biggest drawback of refractive lensectomy is the cost: about $4500 per eye, which is 2 to 3 times the cost of Lasik. But after 40 years of wearing corrective lenses (I got my first pair of glasses at age 9), I was eager to get rid of glasses and contact lenses forever. I wanted to rediscover the freedom of life without glasses. I divided the total cost by the number of days in 41 years (because I fully intend to live until I’m 90) and decided 60 cents a day was bargain for a spectacle-free lifestyle. One week before my 49th birthday I had surgery on my left eye, and two weeks later my right eye received its new ReZOOM lens. I was nervous before the first procedure but calmer the second time around because I knew what to expect. Muscle Arthritis – Symptoms and Treatment Explained

Myositis is the medical term for inflammation of muscle tissue and one type of muscle arthritis is dermatomyositis. (Without the rash, the condition is called polymyositis.) In this form, there is soreness and inflammation and pain in the muscles but there is also an accompanying dry rash affecting the skin. This rash is commonly seen on the face and neck, chest and back and the shoulders. Heart muscle may also be affected as well as the lung tissues. It is usually dusky and a purplish red color. This is an autoimmune condition and is treated with anti-inflammatory medications including steroids.

Another disease that can be considered muscle arthritis is called fibromyalgia. Along with muscle pain and aches, fibromyalgia can produce pain in the joints and other connective tissue like tendon and ligaments, as well as fatigue, depression, headache, anxiety, sleep problems and numbness in the extremities. To have this diagnosis, symptoms must last at least three months and at least 11 of the 18 trigger points associated with FM must be tender to the touch.

Treatment for FM has until very recently been simply medications directed at relieving pain and specifics for other symptoms, but now medications specifically for fibromyalgia have reached the market and can be prescribed. About 2% of the population has symptoms of FM.

Another muscle arthritis is the systematic illness called lupus. Usually joints are affected but some lupus patients also have muscle aches and pain associated with the condition. Lupus is treated with corticosteroids and other immune system modifiers. Symptoms of lupus can include a butterfly shaped rash across the face, fevers, fatigue, multiple joint arthritis and many other symptoms. Blood work can help to make the diagnosis. Lupus is also an autoimmune disease and the cause is not known. It is about ten times as common in men as it is in women.

Finally, there is polymyalgia rheumatica. The symptoms of polymyalgia rheumatica are moderate to severe muscle pain and the location is almost always in or near the neck, shoulders and hips and can come on very suddenly. In virtually every case, a person with polymyalgia rheumatica will have an abnormally high red blood cell sedimentation rate. This disease is often associated with a serious vascular affliction called polyarteritis or sometimes giant cell arteritis in which blood vessels become inflamed. However, prompt treatment usually controls both conditions or polymyalgia rheumatica alone.

A corticosteroid like prednisone is usually given and will produce a remission which can last some time. If the condition recurs, the drug is restarted and can control symptoms. Nonsteroidal anti-inflammatory medications are also used. The disease may also go away on its own but this can take a year or more.

One last form of muscle arthritis is a side effect of drugs given to lower cholesterol called lipid reducing drugs or statins. While not common, this condition called rhabdomyolysis can have serious implications including kidney and heart damage. Persons taking statins need to report any unusual muscle aches and pains to their physicians at once.

While muscle arthritis can be painful and frightening, most forms of it do respond well to treatment. The most important thing is to report muscle pains to a physician, learn the cause, and to then use appropriate treatments for whichever condition is to blame.

Similarities of Gestalt, Existential and Client Centred Therapy

Therapy and Coaching are experiences that help people fulfill their full potential. There are an infinite number of varieties. Each one has its claims to be the most powerful and probably as many detractors arguing against it. Many of the theories and practices are powerful for certain situations. None meets the needs of every situation. In this article I am interested in drawing out some of the similarities across the three therapies of Gestalt (Perls), Existential (Spinelli) and Client Centred (Rogers).

The first similarity is the relationship between the client and the therapist. In each of these therapies the client and the therapist form a system. In this system both client and therapist co create a therapy world. The therapist is giving herself fully to the relationship and will be impacted by the joint experience. The therapist will share the experience she is having with the client. The intent of the therapist or coach is to be a support to the client as she seeks to understand her world more clearly. By understanding the world more clearly the client is able to resolve the conflicts she is confronting and becomes ready to change and grow. The Gestalt therapist or coach is noticing experiences in her own being, and that of the client, during the encounter with the client and is using this information to follow the energy of both the client and the therapist or coach. The existential therapist or coach follows a similar approach and relies on phenomenological experience to inform the co created system. The Client Centred therapist or coach is committing herself fully to providing a helping relationship to the client. The approaches used with these systems is distinct from other forms of coaching and therapy where the therapist sees himself as separate from the client and an expert with the tools to cure, advise or direct the client. The client is seen as a separate entity that has therapy or coaching needs and the therapist or coach has the tools to show the client what she needs to do to change and grow.

The second similarity is the power of description in moving the system of client and therapist forward in a positive way. The Gestalt approach encourages the client to describe her world through use of the distinctions of cycle of experience (attention through awareness through energy through contact through resolution and rest), boundaries (between organism and environment and between what we will do and will not do), resistance (introjection, projection, retroflection, confluence, deflection and desensitisation), grounding (paradoxical theory of change), unfinished business and completion. The Existentialist approach works joint descriptive processes on the three aspects of being which are relatedness, uncertainty and anxiety. Relatedness concerns how we relate to each other through the way we see the world through our own eyes, the eyes of others and the environment. Uncertainty is the necessary uncertainty that arises from never being able to have a complete and full view of the world. We can only have our own perspective which will include what we think others see and what else might be out there. Finally anxiety arises out of the relatedness and uncertainty we experience in the world. The Client Centred approach supports the client describing how her world is. The therapist role is to keep playing back to the client, possibly in her own words, what the client is saying. In each of the approaches the client is clarifying her description of the world and the possibility for change and growth arises as the client becomes clearer.

It is clear that for each of these approaches to be successful the personal qualities of the therapist or coach are critically important. These methodologies require mastery in their application and success only comes with the right combination of personal qualities and delivery. It is my view that someone who is successful as a Client Centred coach or therapist would be equally successful in the realm of relationship to client and descriptive support if they were to use Gestalt or Existential approaches. I believe the same to be true of successful Gestalt and Existential therapists.
