What is this Condition?

A throat abscess is a localized accumulation of pus that may occur around the tonsils or in the back of the pharynx. With treatment, the prognosis for a person with throat abscess is good.

What Causes it?

Abscess around the tonsils is a complication of acute tonsillitis, usually after streptococcal or staphylococcal infection. It occurs more often in adolescents and young adults than in children.

Acute abscess in the back of the pharynx is caused by infection in the lymph glands, which may follow an upper respiratory bacterial infection. Because these lymph glands, present at birth, start to become smaller after age 2, this condition most commonly affects infants and children under age 2.

Chronic abscesses in the back of the throat may result from tuberculosis of the cervical spine and may occur at any age.

What are its Symptoms?

The person with an abscess around the tonsils will have severe throat pain, occasional ear pain on the same side as the abscess, and tenderness of the submandibular gland. Difficulty swallowing causes drooling. Tightening of the jaw muscles may occur as a result of swelling and spreading of the infection. Other symptoms include fever, chills, malaise, rancid breath, nausea, muffled speech, and dehydration.

The person with an abscess in the back of the pharynx will have pain, difficulty swallowing, and fever. When the abscess is located in the upper pharynx, the person may develop nasal obstruction. When the abscess is located in the lower pharynx, the person may develop difficult and noisy breathing. Children may experience drooling and muffled crying. A very large abscess may press on the larynx, causing swelling, or may erode into major vessels, causing sudden death from asphyxia or aspiration.

How is it Diagnosed?

The doctor bases the diagnosis on the person’s health history and on a throat exam. A person with an abscess around the tonsils commonly has a history of bacterial pharyngitis. A lab culture may reveal streptococcal or staphylococcal infection.

A person with an abscess in the back of the pharynx commonly has a history of nasopharyngitis or pharyngitis. The doctor will also take X-rays of the larynx and may order culture and sensitivity tests to isolate the causative organism and determine the appropriate antibiotic treatment.

How is it Treated?

If an abscess around the tonsils is caught early, treatment consists of large doses of penicillin or another antibiotic. For late-stage abscess, primary treatment is usually incision and drainage under local anesthesia, followed by antibiotic therapy for 7 to 10 days. Tonsillectomy, scheduled no sooner than 1 month after healing, prevents recurrence but is recommended only after several episodes.

If a person has an acute abscess in the back of the pharynx, the doctor will make an incision to drain the abscess through the pharyngeal wall. In chronic conditions, drainage is performed through an external incision behind the adjacent neck muscles. Postoperative drug therapy includes antibiotics (usually penicillin) and analgesics.

Well, I am going to tell you what worked for me.

About 13 years ago, I had a horrible case of bronchitis. I never believed in natural cures back then so I went to the doctor… and then back to the doctor… and then back to the doctor because it never went away. What’s Good For Bronchitis?

Bronchitis is a pulmonary disease caused by the onset of inflammation in the bronchial tubes, which are the air passages into the lungs. In bronchitis these air passages become inflamed and swollen, a thick mucus is generally produced, and is often accompanied by a wheezing cough. Symptoms may also include a difficulty in breathing, the need to constantly cough out mucus, and a mild to moderate fever. There are two forms of bronchitis, acute and chronic, which are somewhat similar and with similar causes, but run their course in different ways.

Acute bronchitis is characterized by a slight fever that may last for a few days to weeks, and is often accompanied by a cough that may persist for several weeks. Acute bronchitis often occurs after a cold or the flu, as the result of bacterial infection, or from constant irritation of the bronchi by polluted air or chemical fumes in the environment. It may initially affect your nose, sinuses, and throat and then spread to the lungs. For acute bronchitis, symptoms usually resolve within 7 to 10 days, however, a dry, hacking cough can linger for several weeks.

Can You Inherit Obesity from your Parents?

How many times do you hear overweight people say things like, “I’m only this way because I’m big boned” or “both of my parents are big so it’s in my genes” or even “all of the women/men in my family are overweight”.

So now the question is, if other members of your family are overweight and have always been, does it mean that you might have inherited the “fat” gene and are predisposed to being overweight yourself? Is there anything that can be done about it, or are you doomed to be overweight for the rest of your life?

There are some facts that must be considered before you assign blame for your weight problem to your family tree. Genetics do decide what the shape of you body will be as well as determining your height and possible risks to your health. If there is a history of obesity in your family, you may have a higher potential for it, but it does not mean that you are doomed to a life of plumpness.

The fact is, even though you may have inherited a higher concentration of fat cells from your family gene pool, it is the bad habits that you inherit from them that will contribute more to your weight than your genes.

Here are the facts about what makes you overweight that you must know to help you break the family tradition of living with that extra padding.

For openers, the food choices that you make play a very important role in whether or not you will become overweight. Not knowing what a proper portion size is during mealtime will also hurt your weight. If as a kid growing up, your parents always went back for seconds during meals, you probably will have inherited that habit as well. If your parents always ate high calorie snacks at night while watching TV, well, you get the picture.

Another downfall to losing the weight gain battle is physical inactivity. While just being physically inactive does not add extra fat by itself, most overweight people are inactive and that means you will burn less calories throughout the day. When an inactive person eats even a normal amount of food daily which is usually still more than they need, they are more likely to store the extra calories as fat.

Your age plays a part in your weight also. When people age, they typically become less active. Also, after age 30 or so, an inactive person will lose around 1/2 to 1 pound of muscle tissue per year. This makes your metabolism slow down being that muscle is active tissue and the more you have, the higher your metabolism, the less you have, the lower your metabolism. This will account for weight gain.

Most overweight people are not that way because of the genes they inherited, but because of the bad habits they inherited as well as ones they developed on their own. According to the American Medical Association, less than 2% of all obesity can be directly attributed to any kind of metabolic disorder or hormone imbalance.

The bottom line is all of the excuses in the world for being overweight won’t take one pound of fat off of you. The whole thing comes down to modifying your lifestyle to one that is conducive to proper weight control. That means eating less than you’ve been used to eating and being more physically active than you’re used to being.

The Mysterious TMJ (D)

Mention "TMJ" to people, and almost to a person everyone either knows someone who has this disorder, or they have it themselves. Despite the millions of people who suffer from TMJD (Temporo Mandibular Jaw Disorder) there is mostly just educated speculation as to the possible causes of this disorder. Even less is certain about its cure.

The temporo mandibular joints are small ball-and-socket hinge like structures on each side of your head that essentially connect your lower jaw to your skull. The lower jaw has rounded ends (condyles) that smoothly move in and out of this socket when one eats, speaks, yawns, etc. The surfaces of these bones are covered with cartilage, so bone is not grinding on bone, and the surfaces are also separated by a small disk which acts as a shock absorber. Sixty-eight pairs of muscles in your face and jaw area work to help stabilize this joint so that it moves smoothly to ensure speaking, swallowing, chewing, breathing, and even keeping your head on your shoulders! For the person with TMJD, however, any or all of these movements can become extremely painful as the condyles become dislocated or the cartilage excessively worn or overly stretched.

Researches now divide the disorders of the temporomandibular joint into three categories: one is myofascial pain, and it is the most common. This pain is characterized by discomfort or pain in the muscles that control jaw function and the neck and shoulder muscles. Then there is of the internal derangement of the joint, which means the jaw is dislocated, the disc is displaced, or the condyle is injured. The last category is degenerative joint disease, such as arthritis in the jaw joint. (See reference below.)

This much science knows, but the certainty seems to stop here. The causes of TMJD seem to be as variable as the people who have it. Actual numbers of people experiencing TMJ disorders fluctuate widely, ranging from five to fifteen percent of the population of the United States to more than thirty-five percent of people. Women are more likely to develop this condition than men … and no, excessive talking is not to blame, although talking can cause pain if one has this disorder!

Amazingly, many suffering from a TMJ ailment seem to know the exact cause of his / her disorder. Some develop this condition due to facial or head injuries because of blows to the head, jaw or face, or from whiplash A great many sufferers indicate that they developed TMJ pain after prolonged dental procedures. Could it be your bite? Maybe your spine is misaligned. Do you need orthotics in your shoes? Perhaps there is a genetic connection. NIDCR is conducting a study to identify biological or genetic factors that may contribute to this disorder. Another recent study suggests there is a link to serotonin levels. While it would seem very reasonable to blame grinding and clenching of teeth for TMJD, ironically many people who grind and clench never experience TMJ pain. Arthritis can certainly be a cause, as can simply having tight jaw muscles due to stress.

Overuse of the jaw is another possible cause. Excessive gum chewing, nail biting and gnawing on items like plastic straws can overtax the muscles of this joint.

You may be experiencing TMJ problems if your jaw clicks, pops, catches, or locks when you open your mouth, Frequent headaches, neck-aches and ear pain and ear infections are other indicators of TMJ joint disorder. When TMJ affects the ear, one may then experience dizziness, lightheadedness, ringing in the ears, and a fullness or pressure on the ear drum. (Perhaps you do not need those antihistamines after all.)

Pain from the jaw can also radiate down your shoulders into your arms. Your teeth may not line up when you close your mouth, and you may not be able to open your mouth fully. In fact, the old song about the leg bone being connected to the hip bone, being connected to the back bone is an entirely appropriate analogy of TMJ pain. You may experience aches and pains throughout your body as your jaw struggles to align itself.

Fortunately, most people suffering from TMJ disorder will recover without medical intervention. Others will recover but will continue to have periodic bouts of the condition. Sadly, there are those for whom the pain of this disorder will become an indelible black mark on the fabric of their lives. Those with osteoarthritis or rheumatoid arthritis in the jaw joint may not experience total recovery either. Whether your case of TMJD is fleeting or permanent, experts agree that one of the most important things you can do is to ease the stress on the jaw joint, and this can be done through dietary changes. This does not mean one must subsist on oatmeal, soup, or pureed foods for six months to a year, and that is how long (or longer) it may take for your jaw to fully recover. It does mean, however, that a diet that is easy to chew will be most helpful for your condition.

Other possible courses of action for improvement involve physical therapy, chiropractic work, applying heat or ice packs, massage therapy, hypnotism, and avoiding extreme jaw movements (such as wide yawning, singing, gum chewing, screaming, etc.) Learning stress reduction techniques is another possible course of action, as well as short-term use of pain relievers. Mouth guards are widely prescribed by many dentists, but one must remember that mouth guards are not a cure for TMJ disorder. Mouth guards will help prevent damage to one's teeth if one is grinding and clenching excessively, especially at night. Mouth guards, however, do not address the fundamental issue of WHY one is clenching and grinding. For some people, wearing a foreign object in one's mouth can cause an increase in clenching and grinding and resulting pain.

Be cautious with this disorder. More medical treatment is not necessarily better. Some dental experts urge that TMJD treatment be conservative and reversible. "Conservative treatments do not invade the tissues of the face, jaw or joint. Reversible treatments do not cause permanent, or irreversible, changes in the structure or position of the jaw or teeth." (Ibid). Always seek a second opinion if you are advised to have orthodontics to change the bite, or crowns, bridge work or grinding the teeth down to balance the bite. "Scientists have learned that certain irreversible treatments, such as surgical replacement of jaw joints with artificial implants, may cause severe pain and permanent jaw damage. Some of these devices may fail to function properly or may break apart in the jaw over time." (Ibid).

TMJ disorder can be an extremely painful condition that can affect every moment of your life, not just when you're eating. Sleep can be disturbed, work can be compromised, activities are affected (whether they be sports, exercise, singing, or playing wind instruments) participation in group outings can be affected, and your dental health as well as your overall health and well being can suffer.

Remember the old expression, "patience is a virtue, possess it if you can …" Most of us are sadly lacking in this virtue and want a quick fix, but patience, time, a softer diet, and stress management will go a long way in easing the pain of this disorder for the majority of people who suffer from it.

Hip Dysplasia and Your Dog

Your pet's health is important to you. After all, they are part of your family! Sadly, dogs of all ages are subject to hip dysplasia; however, in most cases, the symptoms do not begin to show until the middle or later years in a dog's life. This disease primarily affects large and giant bred dogs, such as German Shepherds, Labrador Retrievers, Rottweilers, Great Danes, Golden Retrievers, Saint Bernards, although it can occur in medium-sized breeds as well.

Hip dysplasia is caused by a laxity of the muscles, connective tissue, and ligaments that normally support the joint. Most dysplastic dogs are born with normal hips, but due to their genetic make-up (and possibly other contributing factors) the soft tissues that surround the joint develop abnormally causing the subluxation (a partial dislocation of bones that leaves them misaligned but still in some contact with each other.) this "dislocation" and the subsequent "reshaping" of the hip leads to the symptoms associated with this disease, which may or may affect both the right and / or left hip.

Symptoms of hip dysplasia in your pet can include:

o Walking / running with an altered gait;
o Resisting movements requiring full extension of their rear legs;
o "Bunny Hopping" gait;
o Showing stiffness, pain in their rear legs after exercise, or first thing in the morning;
o Difficulty climbing stairs;
o Limping;
o Unwillingness to participate in normal daily activities;

Many pet parents attribute these changes to normal aging, but once treatment has begun, they are pleasantly surprised to see a more normal, pain-free gait return. While researchers agree that hip dysplasia is a genetic disease, they also believe obesity can increase the severity of the disease in genetically-susceptible animals. Lack of consistent, muscle-building exercise can be another risk factor.

Veterinarians typically diagnosis canine hip dysplasia by combining:

1. Clinical signs of arthritis and pain;
2. Complete physical exam;
3. X-rays

If your pet is showing outward signs of arthritis, there are usually easily-recognized changes in the joint (visible on x-rays.) Additionally, your veterinarian may be able to feel looseness in the joint, or may detect pain when they fully- extend your pet's rear leg (s).

Roughly one half of all dogs evaluated by their vets have no physical signs of the disease, but are being evaluated for their "hip health", as they are intended to be used for breeding. Breeders want to ensure their animals are at a "low risk" for transmitting the disease to their offspring. There are two different testing methods that can be performed: the traditional is OFA (Orthopedic Foundation for Animals) testing; the other, relatively newer, technique is the PennHip method.

Once diagnosed, canine hip dysplasia can be treated surgically; your veterinarian will determine which procedure, listed below, is best for your pet, based on his or her age, body size, and severity of the hip joint's deterioration:

1. Triple Pelvic Osteotomy (TPO)
2. Juvenile Pubic Symphysiodesis
3. Total Hip Replacement
4. Femoral Head and Neck Excision

Check with your veterinary pet insurance provider regarding their policy with the above-mentioned surgeries, as they can be costly. Medical management of hip dysplasia and osteoarthritis has improved tremendously in recent years, thanks to innovative drug therapies. However, because hip dysplasia is primarily an inherited condition; there are no preventative products on the market today. Through proper diet, exercise, supplements, anti-inflammatories, and pain relievers, pet parents can decrease the progression of degenerative joint disease, but the looseness in the joint will not change significantly. Below are suggestions from experts in the medical management of the disease:

1. Weight Management: Helping your pet maintain their recommended weight is perhaps the most critical part of this equation; surgical procedures and medical therapies have far better outcomes if you pet is not overweight; more than 50% of the pets in the US are overweight, making it reasonable to assume that many of the dogs with hip dysplasia / osteoarthritis are also overweight. If your pet is overweight, ask your veterinarian for recommendations regarding a lower calorie dog food and an exercise program;

2. Exercise: Exercise is equally important in losing and / or maintaining your pet's appropriate weight. That exercise should provides good range of motion and muscle building, but minimize "wear and tear" on your pet's joints. Leash walks, swimming, and slow jogging are ideal low-impact exercises. Remember, it is important to exercise daily; regular exercise in shorter sessions is always better than long work-outs on weekends. Again, your veterinarian can recommend an exercise program appropriate for your pet fi you have questions;

3. Warmth and good sleeping areas: Keeping your pet warm may help him / her be more comfortable; you may consider keeping your home's temperature a bit warmer. Also, an orthopedic foam bed helps many dogs with arthritis; those with dome-shaped, orthopedic foam distribute weight evenly, reducing joint pressure; (They make getting in and out easier for your pet, too!) Remember to keep the bed in a warm spot away from drafts;

4. Massage and physical therapy: Your veterinarian can demonstrate how to perform physical therapy and massage on your pet to help relax stiff muscles and promote a good range of motion in their joints. Begin by petting the area and work up to gently kneading the muscles around the joint with your fingertips using small, circular motions; gradually work your way out to the surrounding muscles. (Moist heat may also be beneficial);

5. Making daily activities less painful: Going up and down stairs is often difficult for our arthritic pets; it can make going outside to use the bathroom difficult. Many pet parents invest in ramps, especially on stairs leading to their yard, making it easier for their pets. Ramps can also make car travel easier for arthritic dogs.

By following these recommendations, you can minimize your pet's discomfort from and the progression of hip dysplasia.

Causes of Hiatal Hernias

A hiatal hernia is an anatomical abnormality that is caused when a part of a person's stomach sticks through the diaphragm and up into the person's chest. The esophagus (also known as the food tube) will usually pass down the chest and will cross the diaphragm before it enters the abdomen through a hole in the diaphragm. This hole is called the esophageal hiatus.

Those who have a hiatal hernia have an opening of the esophageal hiatus that is larger then what is normal. Because of this a part of the stomach will get passed it and lodge in the chest. Most hiatal hernias will affect adults – but it can affect a few infants.

Symptoms Of Hiatal Hernias

Many hiatal hernias are the sliding type and do not have any symptoms associated with it. However, the larger the hernia the harsher the symptoms can be and the more likely someone will experience them. When sliding hiatal hernias do cause symptoms they are more often affiliated with gastroesophageal reflux disease (GERD) or complications related to it.

These complications will occur from the formation of the hernia when it has interfered with the lower esophageal sphincter. This sphincter helps to prevent acid from refluxing from the stomach into the esophagus. Patients with GERS have a higher risk of suffering from a hiatal hernia then those who are not diagnosed with GERD.

That is why many doctors believe that hiatal hernias are often associated with GERD. Yet, it has not been discovered whether or not these hernias alone can cause GERD because it can occur without it. Symptoms of hiatal hernias that are associated with GERD include; nausea, heartburn, and regurgitation.

If this is in the first week after the injury, then recovery chances are good and if after approximately 6 months there is not return to movement than the disability is more likely to be permanent.

According to the Merck Medical Manual, "During a typical year, there are about 11,000 spinal cord injuries in the US. Nearly 48% occur in motor vehicle collisions, and 23% result from falls. The remainder is attributed to violence (14%) , sports (9%), and work-related accidents. About 80% of patients are male. " Looking at these statistics it would seem that a spinal injury is likely to involve other people and in the case of a motor vehicle collision, violence, and work-related accidents, there may be someone at fault due to negligence. Spinal injuries are expensive. There is the initial hospitalization, medical and possibly surgery to consider. After that there is the cost of rehabilitation so that the victim can gain back as much of former mobility as possible. At the same time, the victim is unable to work and may not ever be able to go back to the same level of functioning without help that must be paid for. The victim and their family needs to find help to overcome these burdens with community resources and excellent legal help to take action to recover any compensation that they and their family deserve.
