Heart failure: The inability of the heart to effectively and efficiently pump out blood to the rest parts of the body as a result of a weakness in the contractility of the myocardium leads to its failure. The failure of the heart brings about a further short supply in oxygen and nutrients supply to the organs.

4. Stroke: This is bleeding into the brain as a result of a rupture in the blood vessels of the brain. When stroke or cerebro-vascular accident occurs in a part of the cerebrum, the side of the body opposite the affected part becomes partially or totally paralyzed, leading to difficulty in movement, slurred and incoherent speech, etc.

As mentioned in the previous articles, we know that approximately 60 million Americans have heart disease. It is caused by uncontrolled diets high in saturated and trans fat resulting in arteries being clogged up by bad cholesterol LDL and the inner lining of blood vessels being damaged impeding the transportation of oxygen and nutrition to the heart. There are many opinions about pros and cons of dairy products in our diet. Would it also be the cause of cholesterol building up in the arteries, high blood pressure and heart diseases? In this article, we will discuss dairy products: friend or foe?

Diary products which contain lactose are products made from milk including cheese, yogurt, and butter that have been part of the human diet for years. o It is critical that hopitalized patients with suspected or documented TB be placed in appropriate isolation. This includes a private room with negative pressure and adequate air exchanges. Persons entering the room must wear masks or respirators capable of filtering droplet nuclei.

o Patients should remain in isolation until sputum becomes smear-negative; however, patients ordinarily should not be kept in the hospital for the sole purpose of providing isolation, Special arrangements are necessary for patients who live with children, individuals infected with HIV, patients returning to a closed-group setting (eg, nursing home, correctional facilities, residential facility, homeless shelter).

Further Outpatient Care:

o Patients diagnosed with active TB should have sputum examined for M tuberculosis weekly until sputum conversion is documented. Monitoring for toxicity includes baseline and periodic liver enzymes, complete blood count, and serum creatinine.

o In addition, patients on pyrazinamide should have baseline or periodic serum uric acid determinations, and patients on long-term ethambutol therapy should have baseline or periodic visual acuity and red-green color perception testing. The latter can be performed with a standard test such as Inhihara test for color blindness.

Deterrence / Prevention:

o Patients with a clinically significant result on tuberculin skin testing (see other tests) should be given a course of therapy once active infection and disease is ruled out. Guidelines published by the CDC in 2000 now refer to this as treatment of latent TB. The recommended regimens are listed below:

o INH daily for 9 months

o INH twice weekly for 9 months (given as DOT)

o INH daily for 6 months (should not be used in patients with fibrotic lesions on chest radiograph, patients with HIV, or children)

o INH twice weekly for 6 months (given as DOT, should not be used in patients with fibrotic lesions on chest radiograph, patients with HIV, or children)

o Rifampin daily for 4 months

o Rifampin plus pyrazinamide daily for 2 months

o Children should be given INH for 9 months. In addition, children younger than 5 years who have closed contact to an active case of TB should be started on INH even of skin testing is negative; preventive therapy can be stopped if repeat skin testing is negative 3 months after last contact with a culture positive source case.
o
Patient exposed to MDR-TB may be given ethambutol plus pyrazinamide for 6 – 12 months or pyrazinamide plus levofloxacin for 6 – 12 months; the index isolate should be susceptible to all drugs used.

o Recommended regimens in patients with HIV infection include pyrazinamide plus rifampin daily for 2 months, rifampin alone daily for 4 months, or 9 months of INH (daily or twice weekly). Patients on antiretroviral therapy may need rifabutin in place of rifampin.

Pleural Plaques Vs Pleural Effusions

Our lungs are made up of delicate tissue that is incredibly important to keep our body supplied with the oxygen necessary to carry out our daily activities. We have millions of tiny air sacs, called alveoli, that take oxygen from the air and transfer it to our bloodstream. When things go wrong with our lungs, it can result in serious problems because a decrease in oxygen can permanently affect our brains and bodies.

There are two lung problems that both affect the lining of the lungs, called the pleurae. The parietal pleura covers the inside wall of the thoracic cavity, including the ribs and diaphragm. The visceral pleura is responsible for protecting the lungs themselves. The area between the two pleurae is called the pleural space. The two disorders that harm this area are called pleural plaques and pleural effusions. While they both refer to problems in and around the pleural space, they do have some differences.

First, it is important to realize that while pleural plaques can occur on their own, effusions are more likely to be symptoms of another underlying disease, such as lung cancer, mesothelioma, or congestive heart failure. On the other hand, plaques can form as a result of asbestos forming nodules in the lung tissue. Thus, while the two can both be tied back to asbestos exposure, it is not always a guaranteed cause of these lung problems.

Next, the two conditions affect the lungs in different ways. Asbestos can cause plaques in the lungs because when inhaled, the fibers become lodged in the tissue without breaking down. This causes the lungs to scar, affecting the parietal pleura. These scars can calcify and harden. While this may not cause any direct symptoms, it can often signal that someone is at risk for developing mesothelioma.

Pleural effusion, on the other hand, primarily affects the pleural space. Due to congestive heart failure, lung cancer, or sometimes asbestos exposure, this space becomes filled up with a naturally produced fluid. Although your body needs this fluid to keep its membranes from rubbing against one another and creating friction and irritation, it typically drains away by the lymphatic system. However, other health issues can make it difficult to drain away the lubricating fluid. When it builds up in the pleural space, it prevents the lungs from expanding, which makes you unable to take deep breaths.

While pleural problems may not be too dangerous on their own, they can signify the presence of another, more serious problem, or indicate that you may be at risk for lung cancer or mesothelioma. Is Your Dog Suffering From Luxating Patella?

Luxating Patella, also known as trick knee or a floating patella, is a condition in which the patella (the kneecap) moves or become dislocated from its normal location. This is common condition seen in the smaller breed of dogs. It can also occur in cats, but has been linked with dogs more often than cats. Luxating Patella can sometimes be caused as result of trauma, but more often than not it is either caused from genetics or a developmental defect.

Certain breeds of dogs have been shown to have a predisposition for having Luxating Patella. The miniature and toy breed of dogs seem to be the most common. Some of these include the Toy Poodle, Maltese, Jack Russell Terriers, Yorkshire Terriers, Pomeranians and Boston Terriers. Pekingese, Chihuahuas and Papillion's also seem to have a genetic predisposition for this condition. Luxating Patella has also been seen larger breed dogs such as the Labrador retriever. The condition usually makes it self-known between the ages of 4 to 6 months.

Symptoms of this condition include lameness and pain at the location. Some pets will also show a limping like action, and the pet's hind legs will also be stiff. The pet owner will generally notice a skip in the pets step and sometimes the pet will be running or walking on three legs. It is crucial at this time to get your pet to the vet so they can diagnosis the Luxating Patella. Diagnosis is made through the vet palpitating the knee and a series of X-rays. If the pet has been diagnosed with Luxating Patella, the treatment will depend on which grade of Luxating Patella the pet has.

There are four grades used in the diagnostic of Luxating Patella. The higher the number of the grade is the more the serious condition is. Grade One is where the patella can be manually be luxated (dislocated) when the leg is fully extended. When the leg is returned to normal position the patella moves as well. There is no stiffness at this grade, or very minimal. Grade two is the next step up and this is where as with Grade One the patella can be manually dislocated or can now also spontaneously dislocate itself when there is flexion of the joint. The patella remains dislocated until either the animal extends the joint in the opposite direction or is manually pushed back into place. In Grade Three the patella remains dislocated most of the times but can be manually repositioned. The final grade with In, the Grade Four Luxating patella is permanently Dislocated and can not be pushed back into place. Some muscles may also start to become displaced at this stage.

Grades 2- 4 will require the pet to undergo surgery if the pet is having any difficulty walking. Depending on the grade level will depend on upon the exact details of the surgery. The surgery generally involves deepening the groove that the patella sits on. This procedure is called osteotomy. The goal of osteotomy is to lessen the risk of the patella becoming displaced. Some surgery's will also require that additional bone is removed. When the bone is removed it forms a deep groove that the patella can rest safely in and hopefully not be dislocated again. Each vet's office may differ in the actual prices of this procedure, so it is advised to speak with your vet in reference to pricing and or any payment plans.

The prognosis of recovery is very high, it is about a 90 percent success rate. After about 10- 14 days after you pet has had the surgery they should be walking with their toes touching the ground. In about 2- 3 months after the surgery you should see no signs of a limp. This means your pet is on the way to recovery and about to embark on a long and hopefully healthy life.

Cerebral Palsy – Life As an Infant – One Mom's Experience

It is difficulty to diagnose a baby with cerebral palsy. Many times a child will be one or two years of age before an official diagnosis can be obtained. Of course, the earlier the diagnosis the better. Physical and Occupational therapy can begin even as babies – all the better for your child.

If your child had complications at childbirth, medical equipment or additional training may be required to take your child home.

My son had difficulty breathing at birth and we brought home a heart monitor that was on 24 hours a day. Michael was tube fed the first two weeks because he did not have the natural instinct to suck. It came gradually but feeding could take hours for just a couple of ounces. The hospital also required that we had infant CPR training. Grandparents were required to take the training as well.

My son Michael had a bad case of colic. He cried constantly. Several tests were ran, but nothing ever helped. We had to tough it through.

The only comfort Michael had was his swing. He loved his swing! I put the swing next to the bed each night for him to sleep. Every 20 minutes he cried when it stopped and I would rewind it – I wish the battery powered swing had been invented at that time!

It was difficult to change diapers. The legs were very tight and they "scissored". This means the legs are very stiff and crisscross each other in a scissoring fashion. Scissoring would also occur when lifting him in the air at the waist. At times, his whole body would stiffen and he would throw himself backwards. He especially liked to hold his neck in an upward fashion and seemed to always be looking at the sky or ceiling.

Children with cerebral palsy do not progress as other children do and will show delays. Common concerns or signs a parent will begin to notice will be when their child is not sitting, crawling, or reaching at the appropriate age. It will be especially noticeable when placing the infant along with other infants of the same age. Whenever I questioned my son's progression, I looked at my friends and their babies as a guide.

The first insight into my son's health issues actually came from our public school system, not the doctors. Look for health care professionals, organizations, and school systems who have experience with disabilities. We look to them even now. These professionals work with individuals every day with a variety of disabilities and they can recognize early signs. They can give guidance and many times will be able to explain and prepare you for the next stage. They are not doctors and they can not give you medical advice, but many times they can share their experiences with you.

Remember, persistence pays off. It's okay to get a second or third opinion. The sooner your child receives assistance, the better.
