I would like to first touch upon one of Gout related cases. This is amusing yet sad to know that there was an individual who thought he suffered from a sprained ankle for over a year while actually he suffered from a Gout attack. There was another individual who was diagnosed with gout and later re- diagnosed with cellulitis (the common skin infection caused by bacteria). However, finally on a re- examination the doctor claimed that it was indeed gout and nothing else.

Following can be helpful in differentiating between Ankle Gout and Cellulitis:

Ankle Gout: The pain in the ankle feels as if the ankle is broken or twisted. Ankle gout is also confused with pseudo gout and arthritis pain. The main cause of ankle gout is the presence of high levels of uric acid in the body.

Cellulitis: In this condition the skin turns reddish and swollen and is accompanied by fever also The main cause of cellulitis is a bacterial infection on the upper surface of the skin as well as in the underlying tissues.

The first test that should be taken for gout is a joint aspiration test which is also known as the arthrocentesis. The difficult part of getting this test done is that you will have to search and look around for a medical facility that has a polarizing microscope which is required for doing this test.

You can get gout checked by any doctor but right diagnosis for gout requires a rheumatologist- doctor who specializes in arthritis and related diseases. The rheumatologist should have a polarizing microscope and the experience of being able to spot the gout crystals which are usually in the shape of needles. The main challenge lies in being able to collect enough of the synovial fluid out of the joint that is being affected to test this

Furthermore some apparent symptoms of Ankle Gout are:

  • The pain in the ankle is severe.
  • Gout looks swollen, reddish purple in color and may give the shiny look which is actually sore and tender.
  • During a gout attack the inflammation will reach its maximum in 12 to 24 hours.
  • Usually single joint attack occurs; if it happens to be in two or more joints then it is not likely to be a gout attack.
  • Easiest way to figure out if it gout attack or not is to get the level of uric acid in your body checked. If the level of uric acid is 7.0 mg/dL or more in men and 6.0 mg/dL in women then the chances of it being a gout attack are more.

Ways to Control Social Phobias

The person then starts to correlate the sensation of relief they feel in these moments with their memories and will continually dwell in the past to combat their anxieties.

But, the cycle only solidifies when this happens. By living in the past, the individual is actively combating the present and the future, and causing themselves even more pain as they attempt to hide from their actual emotions. While they seek to relive what they have felt in the past so as to experience their temporary relief, the individual is submitting themselves to even greater sensations of fear in the present and future, a time when they have no control over their emotions – they do not know what will come next.

Reality starts to become more of an illusion – one populated with emotions triggered by memories of the past. They live a life that was once their own, but is now no more than an echo; a shadow of who they are now. When the two begin to coincide, the sense of self dissipates and they start to suffer for their unwillingness to face their life in the present. It is this experience, this anxiety, that plagues so many people in North America and that could be holding you back right now.

When My kid Got a Black Eye

My poor little son had a black eye the other day. I have no idea how he did it, normally he would come to me if he hurt himself and usually have some tears as he is only three years old. However, this time I just noticed a scratch and bruise under his eye and he said he didn’t know how he did it. It was really swollen so I popped a gel pack on it for a few minutes as I had it in the fridge and I thought that the coolness might reduce the swelling. He wasn’t too happy at having it there for long though as it meant that he had to stand still when he wanted to run about and play, I was trying to think of something that I could put on it so that the swelling would go down or it would heal a bit while he was running round.

I knew that Colpac would be the best but I was not sure how I could possibly use it because all I had at home was a knee wrap which would hardly do! In the end I took him to the pharmacist and they told me that it was not too serious and it wouldn’t matter if I did nothing as there was no danger of the swelling being too damaging. I guess I over reacted and I tend to with the health of my children. I think that it is really important that they stay well and healthy and that I do everything that I can to look after them because they do not know when they need to see a doctor as they have no idea how serious their medical condition is and what consequences there might be if it isn’t treated. I suppose these are things that you learn – if one child has chicken pox then you know how to treat it when the other child gets it and so you do not worry so much and do not bother to consult the doctor.

They are the experts anyway, it can be dangerous to self diagnose and decide whether your condition is serious or not, if you feel unwell and you do not know why or what is wrong with you then you should seek help. Some people worry about visiting the doctor too much but I say better safe than sorry and I think there is nothing wrong with loving and protecting my children.

Sleeping Disorder – Insomnia

Insomnia is a sleeping disorder characterized by the inability to fall asleep and/or the inability to remain asleep for a reasonable amount of time. Insomniacs have been known to complain about being unable to close their eyes or “rest their mind” for more than a few minutes at a time. Both organic and non-organic insomnia constitute a sleep disorder.

According to the U.S. Department of Health and Human Services, approximately 60 million Americans suffer from insomnia each year. Insomnia tends to increase with age and affects about 40 percent of women and 30 percent of men.

At least three types of insomnia exist: transient, acute, and chronic.

1. Transient insomnia lasts from one night to a few weeks. Most people occasionally suffer from transient insomnia due to such causes as jet lag or short-term anxiety. If this form of insomnia continues to occur from time to time, the insomnia is classified as intermittent.

2. Acute insomnia is the inability to consistently sleep well for a period of between three weeks to six months.

3. Chronic insomnia is regarded as the most serious; persists almost nightly for at least a month.

A common misperception is that the amount of sleep a person requires decreases as he or she ages. The ability to sleep for long periods, rather than the need for sleep, appears to be lost as people get older. Some elderly insomniacs toss and turn in bed and occasionally fall off the bed at night, diminishing the amount of sleep they receive. An overactive mind or physical pain may also be causes. Finding the underlying cause of insomnia is usually necessary to cure it. Insomnia can be common after the loss of a loved one, even years or decades after the death, if they have not gone through the grieving process.

Ankle Injuries and Treatment Options

Spring is here and when the weather warms up, that means more outdoor activities to do. Whether that means hiking your favorite trail, running just for fun, or playing with your kids at the park, this sudden increase in exercise leaves your unconditioned ankles at risk. You may have rolled your ankles multiple times before, but this time your ankle is really bothering you and you begin to wonder what is causing this to happen.

What actually happens to your ankle?

When you roll your ankle in the most common fashion, the tibia moves laterally over your talus, which is inverting, while your calcaneous moves medially, and your cuboid moves inferiorly. The severity of the injury can fall into a few different categories: strains, sprains, tears, or fractures.

Sprain or Strain: Is there a difference?

Simply described, strains are injuries of the muscle and fascia. In a mild strain only a few muscle fibers are injured, the fascia is intact, and little function is lost. A severe strain is very painful and accompanied by discoloration caused by blood vessel rupture. The fascia and many muscle fibers tear, and muscle function may be lost completely. Sprains result from overstretching or tearing of the connective tissues, ligament, and/or tendons. In severe sprains the tendons can tear or pull away from the bone causing fracture. These two can occur simultaneously, as ankle injuries are notoriously complex.

Treatment Options for Ankle Injuries

Treatment plans for ankle injuries depend largely on the severity of the injury. The options for treatment range from conservative to invasive. By following proper treatment, most ankle sprains heal within two to six weeks, but may take longer depending on injury severity and patient compliance. You may need to use crutches to assist you during walking. Consult your health care provider to reduce the risk of re-injury or setbacks.

Swelling and pain are the body’s way of making you take a time out and rest so that you don’t injure yourself further. Applying ice In 20 minute intervals four to five times a day in the beginning stages is a good way to reduce swelling and control pain. Caution should be used when icing due to the potential of tissue damage, when icing for longer periods of time than directed.

Compression therapy is great for stabilizing the area and used to help your body to push the Inflammation from the affected area. Using elastic wraps, air casts, or splints not only help to reduce the edema but also protect the ankle by immobilizing it.

Elevation of the injured part helps to decrease the edema in the ankle due to pooling of blood as a result of gravity. Keeping the ankle elevated on a few pillows, ideally above heart level will decrease swelling and pain of your injury.

RICE Therapy: RICE is an acronym for Rest, Ice, Compression, and Elevation. This can be used to address the initial early signs of inflammation, edema, and swelling, which are present in most traumatic injuries.

Contrast Therapy: aka Hot/Cold Inversion Therapy is to speed up the process of reducing inflammation, by using the pumping action of vasoconstriction and dilation. This helps to break up fibrin plugs that formed from tissue damage, so that local lymphatic channels can reopen to reduce the swelling to the injured area.

This is performed by using two tubs, new trash cans work beautifully, with water filled to 12 to 14 inches of water in each tub. One will be an ice bath with cold water and one-third ice, and the other will be warm water about 100-degree Fahrenheit. You should then place the effected ankle in the Ice bath for one minute and then into the warm bath for one minute, alternate back and forth for 11 minutes being certain to finish with the ice bath. If at any time you feel nauseous, dizzy or experience chest pains stop and consult a health care professional.

Passive therapies: Passive therapies are those which the patient has no active part in and just has to relax while the therapy is performed. Therapies such as muscle stimulation, ultrasound, and cold laser have been proven effective to help with reducing painful symptoms, decreasing local swelling, retarding muscle wasting, and stimulating healing.

Ankle Supports: To provide temporary support and stability to the injured ankle, supports can be used as a short-term solution. They can prevent motions that may cause re-injury to the vulnerable joint. Bracing may include the use of air splints, lace-up ankle braces, elastic wraps, taping, or removable cast boots. Long-term use of braces is not advised because it de-conditions the musculature surrounding the joint, weakens ligaments, and can cause bone density loss.i

Chiropractic Adjustments: Chiropractic care helps to restore function to joints of the foot and lower extremity that are “locked up”, fixated, and not moving properly. Muscles, tendons, and ligaments depend upon proper joint movement to function at optimal levels. Normalizing joint function helps speed the healing of soft tissues and reduces the de-conditioning effect of being immobilized.

Rehabilitation: Mobilization of the ankle should be performed as soon as tolerated to avoid long-term stiffness and swelling of the joint. A physical therapy program consisting of range of motion exercises, stretching, muscle strengthening, and movement pattern retraining can be implemented for this purpose.

Another component of this rehabilitation process Includes retraining the ankle joint to protect against abnormal movements. Proprioception Is the body’s ability to sense movement and position of body parts. Nerve endings In the ankle joint may be Injured during an ankle sprain, making this process more difficult. Retraining faulty movement and stabilization firing patterns will help train the body to protect itself from abnormal motion that can cause re-Injury. This Is accomplished by using physical therapy devices such as a wobble board and unstable surfaces to retrain the ankle for uneven terrain.

Footwear: Shoes worn after ankle injury should have greater support around the heel and may extend above the ankle joint to provide additional stability. Trained personnel at your local athletic store can direct you to the shoes that would best support your vulnerable ankle.
