Kidney Cancer Causes, Symptoms, Risks Factors and More

Kidney cancer or renal carcinoma usually occurs in older people and accounts for about 2 to 3% of cancers in adults, affecting about twice as many men as women. In adults, the most common type of kidney tumor is renal cell carcinoma, which begins in the cells that line the small tubes within your kidneys. Kidney cancer rarely strikes children and young adults; the exceptions are a pediatric kidney cancer called Wilms tumor and some forms of hereditary kidney cancer syndromes, such as von Hippel-Lindau disease.

Causes of Kidney Cancer
The causes are not known, however external factors, such as smoking and obesity, have been related to a higher incidence of kidney cancer and changing environmental factors as well as population aging has seen an increase in the presentation of this form of cancer.

Signs and Symptoms
Kidney cancer symptoms are often overlooked because tumours are usually slow growing and not suspected until the patient begins to experience symptoms such as blood in the urine, pain, tiredness and a palpable mass. Since back pain is common among people over 40 years of age, such pain is often ignored and the presence of kidney cancer can go undetected. Kidney cancer may also cause high blood pressure.

Risk Factors
The risk of developing kidney cancer is four times higher if a close relative has had kidney cancer. Being on dialysis for many years is a risk factor for kidney cancer.

People who have had bladder cancer are more likely to develop kidney cancer, and vice versa. About three per cent of kidney cancer patients have inherited a damaged gene that will make it likely the cancer will also be found in their second kidney.

Not smoking is the most effective way to prevent kidney cancer and it is estimated that the elimination of smoking would reduce the rate of renal pelvis cancer by one-half and the rate of renal cell carcinoma by one-third.

Other factors that may decrease the risk of developing kidney cancer include: maintaining a normal body weight, a diet that is high in fruits and vegetables, especially in bananas and root vegetables such as carrots, maintaining normal blood pressure and limited exposure to environmental toxins.

Diagnosing Kidney Cancer
Cancer of the kidney is most commonly detected with either computed tomography (CT) scan, ultrasound or magnetic resonance imaging (MRI). Cystoscopy can rule out associated bladder cancer. Kidney cancer cells may also break away from the original tumor and spread (or metastasize) to other parts of the body such as the lymph nodes, bones or lungs, with about one third of cases showing metastasis at the time of diagnosis.

Types of Kidney Cancer
Almost 85% of this tumor are renal cell carcinomas. A less common type of kidney is Papillary carcinoma. Other rare kidney cancers include: Renal sarcoma, Collecting Duct carcinoma, Medullary and Chromophobe carcinomas.

Radical nephrectomy with or without the removal of lymph nodes offers the only cure but treatment of kidney cancer may include: surgery, arterial embolization, radiation therapy, biological therapy or chemotherapy depends upon the stage of the disease and the patient’s overall health.

Nephrectomy or removal of the entire organ including the adrenal gland, adjacent lymph nodes and surrounding normal tissue has been the norm, but recent research shows that removal of just the tumor, produces similar survival rates and offers less chance of subsequent renal failure in selected cases.

Scientists have also isolated the gene responsible for VHL disease, and this discovery offers exciting future possibilities for improved diagnosis and treatment of some kidney cancers. Various combinations of interleukin-2, interferon, and other biologic agents and even vaccines developed from cells removed from the kidney cancer are also being investigated.

Survival Rates
With prompt and appropriate treatment, the kidney cancer mortality rate is fairly low, unfortunately kidney cancer has a tendency to spread early, especially to the lungs, sometimes before symptoms develop. The five year survival rate is around 90-95% for tumors less than 4 cm. For larger tumors confined to the kidney without venous invasion, survival is still relatively good at 80-85%. If it has metastasized to the lymph nodes, the 5-year survival is around 5 % to 15 %. If it has spread metastatically to other organs, the 5-year survival rate is less than 5 %.

An important factor for those with this form of cancer and for that matter with all cancers is that assertive patients who actively work to overcome cancer often increase the odds of survival, live longer, and enjoy life more.

Mixed Anxiety Depressive Disorder – An Overview

Mixed anxiety depressive disorder is a relatively new term in the mental health community. It is defined as a combination of some of the depression symptoms and some of the anxiety symptoms without either one of them being the ultimate diagnosis. In fact, this disorder is only diagnosed when there are no other diagnoses to give.

Pneumonia and Its Causes, Risk Factors and Diagnosis

There are are mechanisms to protect your lungs from infection at each stage of the respiratory stage, but because you are very much exposed to viruses and bacteria sometimes these mechanisms fail in protect your respiratory system and you develop pneumonia. The infectious agents are attacked by white blood cells and all the process determins inflammation and difficult breathing.

Varius types of pneumonia include: Community-acquired pneumonia that is aquired at school, work or the gym which may be mistaken with the acute eosinophilic pneumonia developed by soldiers. Nosocomial pneumonia which occurs in hospitalized people, especially in those who are on a mechanical ventilator, are in the intensive care unit or have a compromised immune system. This type is severe especially in people with chronic obstructive pulmonary diseases or HIV/AIDS.

Risk factors
Persons aged 65 and older, very young children, persons with immune deficiency diseases as HIV/AIDS and chronic illnesses, removed spleen, impaired immune system. People who require mechanical ventilation are particularly at risk because the breathing tube, patients hospitalized in an intensive care unit, exposion to certain chemicals or pollutants, toxic fumes. Exposion to fungus as: Coccidioidomycosis, found in the US, Southern California and the desert Southwest or histoplasmosis moslty found in Ohio and Mississippi River valleys.

When to seek medical advice if you experience shortness of breath, chest pain that fluctuates, cough, an unexplained fever with chills and sweats. If you have heart failure or lung ailments, pneumonia can prove fatal in as little as 24 hours.

Screening and diagnosis

Canine Fears and Phobias

I would like to share some of my experiences with dogs who are fearful or phobic. First of all there is a difference between fears and phobias. Fears can be worked with and possibly get better but phobias can not be cured they can only be managed.

Phobias often cause dogs to react with irrational behaviors. Canine fears and phobias vein out to feed so many more underlying behavioral issues. Most of the time fears stems from lack of socialization as puppies. Phobias develop from some sort of trauma when the puppy was very young and then the fear becomes a learned behavior and finally the fear turns in to a phobia. Traumatic experiences for an adult dog can also instill phobias.

Most common fears are; fears of men, other dogs, loud noises, strange objects, children or it can be anything they were not exposed to as a puppy.

The most common phobias are thunder, fire crackers, car travel and gun shots. The uncommon types of Phobias are other dogs, odors example: veterinary hospitals; then phobias of certain people such as impaired individual’s example: wheel chairs, crutches and prosthetic limbs.

The best treatment for these types of behaviors is desensitization. The process is to start applying a very weak stimulus; then gradually increasing the strength. For example a phobia or fear of gun shots would be to desensitize them to the sound by starting the sound off at a distance and gradually approach the dog. You can even play a tape recording of gunshots at a low volume and then graduate to increased loudness. You can find tapes of thunder and other sounds to help desensitize your dog to their fears or phobias.

Another way of desensitizing is counter conditioning. The process for counter conditioning is to ask a dog to do an obedience command for example; a down stay then start the stimuli very weak and reward the dog with food for maintaining the down stay. Only reward the dog for the desired behavior. If a dog is doing one behavior then they can not do the undesirable behavior at the same time. Increase the stimuli slowly and do consistent exercises to help conquer the fear.

Fears usually develop from lack of socialization as a puppy. If a dog is not exposed to different sounds, environments and people it is easy for them to have a fear of the unknown. After your dog is immunized you should do an exercise.

Three called “777”. The exercise is slowly exposing your puppy to a desired object or person. Everything is paired with positive motivation. Whether it is food or toys or any other thing that is a positive motivator for your dog.

You have to introduce your dog to 7 size humans, 7 different surfaces, 7 different environments and 7 sounds at different levels of loudness.

The first “7” is seven different size humans for example; have a small child just walk pass the puppy at a distance and toss some food at the puppy and walk on. Then again repeat. By the time the child is close enough to meet the puppy it will have paired a child with good things. Pairing the child with food makes the child more desirable.

The second “7” is try to get your puppy on 7 different surfaces. If your puppy is not socialized to sand then at age two when you visit the beach and it steps on this hot sinking surface it may think it is on a different planet and then might exhibit unwanted behavior. If your dog has never gone to the bathroom on anything but grass then if you are in the desert you will have a problem. Besides having a dog get used to different surfaces I also make them eliminate on different surfaces [except concrete or any flooring]. The reason for that is a dog should be able to eliminate on bark, pine straw, sand, mud etc. so when you’re traveling your dog will eliminate on whatever surface is around. This will keep you from having to travel with pine straw or a square of grass. Some dogs will imprint on one type of surface and only eliminate on that surface.

The third set of “7” is different environments. I take puppies [once fully emmunized] to warehouses, dog parks, soccer games and other environments. If a dog is well-socialized to change and different environments then in the future when they experience these environment it becomes no big deal. When I take my dog or puppy to a new environment I do it slowly. When the dog is comfortable then I will move forward in to the environment.

I pair new places with good treats and then once they are fully comfortable in the environment, I fade the food out and always keep the praise coming.

The fourth “7” is sound desensitization. Puppies and dogs who live inside all the time might build a fear to normal everyday sounds. For example; traffic noises, horns, roller bladers, skate boards etc… To start the process of sound desensitization, I drop a book 2 inches off the floor and toss my dog/puppy a treat. Then I raise the book a little more and repeat till I am dropping the book from 5 feet high. By then the puppy will associate the sound with a reward. This exercise is done slowly and consistently. I raise the level of sounds and add in different sounds. I start with dull noises like books. Then I go to tin sounding noises like dropping cans. I will graduate to a baking sheet which can get quite loud when it is dropped from 5 feet in the air. By the time you get these objects to 5 feet in the air your dog sound be getting socialized to sounds and understand that loud noises produces positive rewards.

Anxiety -A Lifelong Challenge

There are many people that suffer from one form or another of anxiety. According to the National Institute of Mental Health, a recognized authority on various mental health issues, about 17 to 20% of the American population may suffer some type of anxiety disorder. Given the current state of the country, society and the world at large, this is not surprising.

GAD, or generalized anxiety disorder, is like being in a chronic or continued state of worry and anxiety. This can be a specific as a mother's concern for her child who walks to school everyday. Or as general as being constantly worried about anything or everything you need to do during the day. Understand were not talking about general concern about safety crossing the street.

In the example above of the mother being worried about her child, someone with an anxiety disorder may not even be able to consider allowing their child to walk to school. The idea of ​​their son or daughter walking along a public sidewalk and possibly even crossing a street is just too terrifying to bear. This type of behavior can also have long-term consequences that can negatively impact the child.

Understand that there are varying degrees of almost any mental illness. While most people suffering from generalized anxiety disorder are able to continue to function, some are paralyzed by the fear of making any decision.

I have a relative who has refused to seek treatment for years and has continued to spiral down into a morass of mental disorders. While many in the family considered her mental state and decision making ability simply "quirky", in hindsight, there were many signs of mental illness. These unfortunately were ignored because although there were both mental and physical symptoms, she was still able to function and continue on in her daily life.

While her family members continued to dismiss her many symptoms as personality traits, she developed ever more serious symptoms. As years went by the family was required to modify their personal interactions to placate the situation and keep her in a somewhat safe place. This in turn allowed the spiral to continue and the symptoms to worsen. While at first this family member suffered with GAD, over the years her condition worsened to where she now shows signs of depression, obsessive-compulsive characteristics, paranoia and panic.

She is now almost unable to make any decision at all. And lives in a constant state of anxiety and worry about the possibility of making a mistake or wrong decision. In attempting to work through a decision-making scenario, she will consistently develop situations where all answers are potentially wrong and therefore put herself in a state of constant and continued worry.

She appears to be developing signs of paranoia and now demonstrates a lack of trust toward everyone. Her days are filled with emotional anxiety while picking and pulling at her hair and eyebrows. Fitful sleep comes only from emotional exhaustion.

The moral of the story is that everyone should understand the basics symptoms of the most common mental disorders. Although only a qualified professional can accurately diagnose mental illness, we should be able to recognize the symptoms if only to encourage people we know to get help. What is Insomnia – 5 Tips to Help With Insomnia

How to Treat Ingrown Toe Nails – Onychocryptosis

Ingrown toe nails are a very painful condition where the nail edge of your nail penetrates the nail fold. It is a commonly presented problem in the Podiatry clinic in which it can be treated conservatively without the administration of Local Anaesthetic or surgically with the administration of local anaesthetic. If you have this condition it is important that you seek the advice of your local Podiatrist who can treat your ingrown toe nails to prevent further pain and infection. There are two primary causes of ingrown toe nails. The nail can splinter off and can penetrate the nail sulcus (onychocryptosis) which is often accompanied by hypergranulation tissue. This type of nail is generally thin and broad, and if cut poorly, grows into the nail fold, it is a condition which is more prevalent among young adult men. Involuted nails which have increased transverse curvature and can grow down the side into the nail fold can also be a big problem. This type of ingrown nail is less likely to become infected but painful keratosis may develop in the nail fold. It is a difficult nail to manage at home and a spike of nail can easily penetrate in to the skin if cut poorly. The condition can be congenital or caused by poor fitting shoes in older women. These nails can easily become thickened (onychauxis or onychogryphosis) which can cause further complications. Advice on cutting your nails

-It is always advisable to cut your nails straight across to allow the corners to protrude. This may not be straight forward with a severely involuted (curved) nail, so seek the advice of a Podiatrist.

Treatment for Ingrown Toe Nails

The Podiatrist will treat your ingrown toe nail without the administration of LA if this can be done painlessly at the initial appointment. Antibiotics may be advised if infection is present, but do not forget, antibiotics alone will not cure your condition. The corner or spike of nail can easily be removed by a Podiatrist, the toe is then dressed with an antiseptic dressing. If the condition keeps re-occurring it may be advisable to consider minor surgery for your ingrown nail or you may decide to seek regular Podiatry treatment for your condition.

Nail surgery can be done very simply and effectively at the Podiatry Clinic. A full patient history is always undertaken, written advice is given out and a signed consent form is necessary before treatment can commence.

The most common surgical procedure is PNA (partial nail avulsion) in which the portion of nail that is causing the problem is removed. It may be necessary on occasions to do a TNA (total nail avulsion) to remove the whole nail. This is done with the administration of Local Anaesthetic to make it a totally painless nail procedure. The toe is then dressed and you will be seen within a few days to re-dress the area. It may also be necessary to phenolize the toe nail to prevent nail re-growth.

A written after care advice sheet is given for you to follow at home to prevent any problems occurring. Generally this will involve keeping the foot elevated and rested for a few days to allow healing. Ensuring that there is no pressure on the area, open shoes are a good option.
