Dispelling Myths About Lymphedema

Lymphedema is the swelling of a human body part caused by an abnormal accumulation of fluid, proteins, and cellular waste in the tissues under the skin. It occurs when there is a problem with the lymphatic system caused by one of the factors below:

-The failure of lymph vessels to develop properly

-Damage to lymph vessels by trauma, surgery, or infection

-Removal or destruction of lymph nodes, usually during treatment of cancer

Most of the advancements made in the understanding and treatment of lymphedema result from research on those who acquired lymphedema as a complication of breast cancer treatment.

Here are some common misconceptions about lymphedema:

Myth 1: It is an incurable condition. While it can be managed, lymphedema is a lifelong condition that will not go away over time. In fact, because it is a chronic progressive condition, even mild cases can eventually escalate and have serious consequences if not properly treated.

The gold standard for lymphedema treatment is complete decongestive therapy, which consists of two stages: a clinical phase where a lymphedema therapist performs manual lymph drainage and bandaging, and a self-care phase where the patient or caregiver performs the daily massage and bandaging. The clinical treatment may only take 4-6 weeks, but managing lymphedema is an ongoing process.

Myth 2: It will only develop within the first five years after surgery and radiation. Although we wish this were true, people can develop lymphedema at any stage in life. I have heard of a cancer survivor who 30 years after her treatment developed lymphedema after reaching up to pull down a garage door. It may be uncommon, but it can happen.

Myth 3: Lymphedema will make one’s arm huge. One-size-fits-all is not true for lymphedema. Like cancer, lymphedema has various stages. Stage I is mild, Stage II moderate and Stage III severe. I’ve seen pictures of a lymphedemous leg grossly misshapen, and I’ve seen cases where I could hardly tell the person had lymphedema.

Myth 4: The sentinel node biopsy that only removes a few lymph nodes stops lymphedema from happening. The procedure, while reducing the incidence of lymphedema, has not eliminated it. Genetic predisposition to any disruption of lymphatic flow may tip the scales.

Myth 5: A person at risk for lymphedema or who has it should not carry heavy objects. This has recently been disproven. In fact, gradual lifting of weights with a compression garment helps with lymphatic flow.

Myth 6: If your limb starts to swell you can wait a while to get therapy. If you notice any swelling in a limb after removal of lymph nodes, even after a sentinel node biopsy, see a surgeon right away. The earlier you catch it, the better. If the doctor does not listen, go to another M.D. who will. And if you are diagnosed, insist that you get a prescription for lymphedema therapy.

To reduce the risk of developing lymphedema, many therapists advise patients to have their arm fitted for a compression sleeve to be donned during plane rides longer than four hours. But newer findings indicate that cabin pressure at high altitudes may not make a difference in swelling. In addition, survivors may wish to wear the sleeve when lifting heavy objects with that arm. One should consult the National Lymphedema Network or Step-Up, Speak Out to get current, reliable doctor-approved advice on risk-reduction practices.

While certain triggers have been found anecdotally to bring on lymphedema (such as long plane rides or repeated puncture wounds), many experts in lymphology now surmise that those who develop lymphedema do so because of a genetic predisposition. Also, some people are born with more lymph nodes than others, providing a more protective effect.

Notwithstanding myriad myths and misconceptions about lymphedema, my main advice is never to despair. You can lead a normal life after lymphedema. Some patients turn the anger they feel toward their surgeon in not informing them of lymphedema into legislative and other action to ensure more research is directed toward this under-served condition. Others start Web sites and blogs to provide needed information to patients at risk and those with lymphedema. Still others go on to live their lives without fanfare. All of these responses are valid.

Food Poisoning in San Carlos Caused By Nestlé Cookie Dough

As a result, one young woman, with many others likely to follow, has filed a lawsuit against Nestlé when she became infected with the bacteria after eating the cookie dough. She was held in the hospital for a week, but thankfully survived the encounter with the potentially deadly bacteria. In addition to this young woman’s case, there have been at least 70 other reported cases of E.coli that have resulted from eating Nestlé’s ready-to-bake cookie dough.

The young woman now filing the lawsuit against Nestlé reports that she ate approximately half of the container of Nestlé raw cookie dough right from the package. After a few days, she started to experience nausea and abdominal cramping. She didn’t link her symptoms to the potential for food poisoning, however, until she began to have bloody diarrhea. The young woman went to the hospital where a biopsy was performed and an infection caused by E.coli was found. Because of the potentially fatal results an E.coli infection can have, it was incredibly fortunate that the woman was able to be treated so quickly. If an E.coli infection goes unchecked, it can often lead to the kidney failure and death of its victim.

Investigators are still in the process of determining how Nestlé’s prepackaged cookie dough was contaminated with the E.coli bacteria. When E.coli infections break out, they can typically be linked to meat products that haven’t been cooked well. This is because E.coli bacteria live within the stomach and intestinal tract of cattle, goats, and several other types of farm animals. How and why the bacteria ended up in Nestlé Tollhouse prepackaged cookie dough remains a mystery that the FDA, along with Nestlé Food Corporation, is attempting to solve.

Lung Cancer FAQs – Can Tumors Lie Dormant For Years?

New research information indicates; scientists, may have discovered that lung cancer tumors are somehow able to lay dormant in the body for many years before becoming active. That is to say – before a sleeping tumor actually becomes a noticeable life-threatening disease (no previous symptoms of its existence).

By using a 3-dimensional cell culture system, scientists have been able to identify a mechanism that allows dormant, metastatic tumor cells (cells that have the ability to spread to other regions of the body) to start growing again, even after many years of inactivity (the dormancy of a tumor becomes proliferative metastatic growth [the rapid growth production of new cells]).

This new growth is believed to happen in a regulated way, where signaling from the surrounding micro-environment may lead to changes in the skeletal infrastructure of dormant tumor cells. However, any tumor size growth due to changes from dormant to proliferative metastatic growth may also be restricted by the availability of an adequate blood supply (something that is needed to provide oxygen and sufficient nutrients for cancerous cells to grow).

Because of this new discovery, future treatments may one day have the ability to inhibit (stop) any such changes through "targeted mechanism therapies." But, although there seems to be this possibility, it would be appropriate to mention that many disseminated tumor cells (cells widely dispersed in a tissue, organ, or the entire body) would usually not survive for very long periods of time. Training with Diabetes: Dawn Phenomenon & Glycogen Metabolism of Swimming

I’m working with an athlete who has recently been diagnosed as a diabetic and is training for an Ironman. He is also new to triathlons and all the training and logistics that come with it.

Last week I got an email from him about two problems with his diabetes management:

1. High glucose levels on his early bike sessions
2. Hypoglycemia during swim workouts done at lunch time

1) Hyperglycemia early in the morning is very common in diabetics. Also known as the Dawn Phenomenon, it is the result of decreased insulin sensitivity and changes in the levels of various hormones in the early morning hours (GH, Cortisol, Glucagon), which can lead to the overproduction of glucose by the liver and to the underutilization of glucose by peripheral tissues. 

Since the bike workouts are only about one hour, there is no need to have a big breakfast before these. So he was instructed to get a big breakfast after those workouts, taking a couple of extra ultra-fast insulin with it, and check blood glucose levels 2 hours after the meal to make sure it was all absorbed.

2) Hypoglycemia during swim workouts done at lunch time

For the second issue, there is also a link between the situations that are causing the hypoglycemia during the swim.

First of all, swimming brings with it a higher metabolism rate and a higher number of muscles involved in the work compared with running or biking. When you swim you are using many small muscles, which make the glycogen consumption a bit higher than during biking or running, thus making swimming the discipline that causes a bigger drop in blood glucose levels for the first hour of training.

Another reason is that on some days this athlete is training in the morning (when the dawn phenomenon occurs) as well. So by the time he gets in the pool, not only is his metabolism higher but his insulin sensitivity is high too.

I remember that back in my training days it didn’t matter how high my blood glucose levels were before a workout – if I didn’t eat anything before the session I would end up with a hypo.

The advice for my athlete was:

1. If blood glucose level is >180: Take a bottle of maltodextrin/sports drink (with 200+ calories) to the pool and drink it halfway through the session.
2. If blood glucose level is <180: Eat a small snack before training AND take a bottle of sports drinks to the pool, drinking it halfway through the session.

Connecting the two problems:

It is very unlikely that you will get an HIPO by training on an empty stomach in the morning, unless of course the workout is longer than 1 hour or you woke up with an extremely low blood glucose level. Swimming is the discipline that has a stronger impact on your blood glucose levels if you were to do them all at a similar intensity. 

One way to manage those two problems is by working on the training schedule. This athlete has only access to a pool at lunch time and has a group bike to train with in the morning. But if you can swim in the morning and run/bike at lunch/evening, that would keep your blood glucose levels steadier.

Plus, swimming in the morning is always best for those without a swimming background since you get to train on a fresher body, struggle less to float in the water, therefore getting more quality out of the sessions.

Mesothelioma: Description, Diagnosis and Treatment Prognosis

Mesothelioma is a tumor of the flat cells that cover the surface of the lungs, adjacent wind pipes and the thoracic cavity. Mesothelioma is an acronym of mesothelium (flat or cellophane covering) and oma (Latin name for growth or tumor). It is a rare type of lung tumor and tends to be associated with occupational hazard. It is an affliction that involves a multi-disciplinary approach and institutions: the physicians, occupational and preventive medicine, surgeons, occupational safety, worker’s compensation, attorneys, occupational therapists and labor unions.

Risk Factors and Evolution of Tumor:

Long term inhalation of asbestos fibers is associated with certain occupations like ship-building, insulation, construction and demolition. The worst culprits are blue and white asbestos. Cigarette smoking may double the risk. The disease is subtle and may take several years if not decades to produce symptoms. At onset, the diseases may not produce symptoms. A construction worker may only notice symptoms long after retirement. Appearance of symptoms may prompt the victim to seek medical help. The symptoms and signs may include shortness of breath, chest pain, fibrosis, pleural effusion, calcified plaques and bronchial cancer. The cancer may eat up the lungs, wind-pipes, ribs and adjacent muscles.

Types of Mesothelioma: The growth may be fibrosed and localized or it may be diffused and malignant spreading to adjacent tissues.

Fibrous Mesothelioma:

Presents as chest discomfort, and all sorts of breathing problems. As with pleural-affected tumors, joint pain may be noticed in the hands, wrists, ankles and knees. Examination of the hands may reveal finger clubbing.

Chest x-ray is important in the clinical diagnosis and may reveal localized growth as soft-tissue mass. There may be air-fluid level showing effusion into the lung cavity.

The prognosis with surgical resection for this type is good even with evidence of spread. However, some may progress to full-blown malignancy.

Malignant Mesothelioma:

The symptoms of malignant mesothelioma appear much later and seem to peak at 55. The symptoms are diffuse and tend to develop in a couple of decades after exposure to asbestos. Cigarette smoke tends to worsen the onset of the symptoms like breathlessness, and chest pain.

Chest x-ray may reveal connective tissue complications like clumping of the pleural tissues (pleurisy), pleural effusion, lung collapse, basal fibrosis, pulmonary hypertension and lupus erythematosis. Other autoimmune diseases and bronchial cancer may develop.

Sputum test is non-contributory and open pleural biopsy may be necessary to characterize the type of tumor before treatment.

At this stage, surgery is hopeless. Palliative radiotherapy or anticancer chemotherapy may be attempted but the prognosis is hopeless.

Prevention–better than cure:

The best treatment of mesothelioma is preventive measures and health advice to the at- risk professions. These include wearing face masks and finding alternatives to asbestos and quitting smoking. Change of jobs may be necessary.

Depending on your age and the condition of your hip, total hip replacement surgery (total hip arthroplasty) may offer the best solution to your problem. Hip replacement hardware (ball and socket prosthesis) can last up to 20 years. And it can often be replaced if the first prosthesis wears out (though second surgeries are not always as successful as the initial hip surgery). If you are fairly young, your doctor may suggest delaying surgery. If so, medications or lifestyle changes may help control your symptoms until the time is right for joint replacement. Losing weight can take a tremendous amount of stress off your hip and knee joints.

Surgical Procedure

Most hip replacement surgeries involve the traditional approach to the hip joint (lateral/posterior or posterior approach). With the common lateral approach, an incision (8-12 inches long) is made down the outside of your hip. The ball at the upper end of your femur (thigh bone) is cut off and replaced with a ball/metal stem component. The stem is placed into the marrow cavity of the thigh bone and the ball portion at the top fits into your new ceramic or plastic socket that is implanted into your pelvis (the cartilage from your hip socket is removed or smoothed).

The Birmingham Hip Resurfacing System (BHR) is relatively new (introduced in 1997). Long term studies are still in progress, but so far results have been favorable. The BHR method eliminates the need to cut off the ball at the top of the femur. Instead, the ball is left intact and merely resurfaced (reshaped) to allow an anatomically shaped metal sphere to be attached. The socket is replaced with a metal implant. The procedure is bone-saving and results in a stronger hip joint than the traditional total hip replacement. The chance of hip dislocation is greatly reduced. For younger and active people, this may be the procedure of choice.

Minimally invasive hip replacement surgery involves an anterior approach to the hip joint. An incision is made along the front of the hip, which allows a more direct route to the hip joint. The major hip muscles are not in this area, so there is minimal tissue/muscle damage during surgery. This usually results in a shorter hospital stay, a quicker recovery/rehab period, and less pain. The only downside to this method is there are few surgeons performing the anterior approach surgery. It requires a special surgery table (which few hospitals have) and special surgical training.

Total Hip Precautions

After traditional total hip replacement surgery, patients are required to follow strict precautions that restrict certain movement/positions of your new hip. This is to prevent your new hip from dislocating. They are commonly called total hip precautions, or total hip protocol, and include the following:

  • Do not cross your legs
  • Do not point your toes inward (no pigeon toes)
  • Do not bend your new hip more than 90 degrees

Your doctor may also restrict the amount of weight you can put on your new surgery leg while walking. This is called your weight-bearing status. This should be strictly followed so you do not damage your new hip while it’s healing.

Surgery Complications

All surgeries carry risk. Hip surgery complications include, but are not limited to, infection, DVT (deep vein thrombosis- blood clot), dislocation of your new hip, leg length discrepancy, vascular damage, nerve damage, just to name a few.

Surgery Recovery

Recovery after hip surgery depends on the amount of cartilage damage, your age, and your fitness level prior to surgery. Exercise before your surgery and your recovery will be easier. Approximately 4-6 months after surgery you should be fully healed, but you must take an active part in your recovery by doing daily exercises and taking daily walks.
