Thyrotoxicosis (Thyroid Storm) Overview – Common Symptoms and Treatments

Thyroid storm, or thyrotoxicosis, is an emergency life threatening event caused by a deluge of thyroid hormone dumped into the blood stream. This condition requires immediate medical attention or sudden death can be imminent.  Thyrotoxicosis can manifest in people with untreated hyperthyroidism.  Additionally, it can have an onset due to surgical procedures performed on the thyroid, trauma, as well as viral or bacterial infectious agents.  The patient is admitted to the critical care unit of the hospital for intensive treatment and care.

Symptoms of Thyroid Storm (thyrotoxicosis):

Jaundice

There is a large increase in blood thyroid hormone level, accompanied by fever as high as 106 degrees. This condition can be caused by the following factors that include:

Stopping the required hyperthyroidism medications without consulting with the health care professional
Ingesting too much of thyroid hormone causing an overdose
Radioactive iodine treatments
People with hyperactive thyroid conditions who are suffering from severe infection or illness
Myocardial infarction (heart attack) in a persons with hyperactive thyroid

Anyone who has an overactive thyroid, or hyperthyroidism can develop thyroid storm.  It occurs when the thyroid secretes a large amount of thyroid hormone in a small amount of time.

Anyone with an overactive thyroid who has a fever, rapid heart beat, with confusion, disorientation, should be hospitalized immediately. A medical history will be taken and diagnostic testing will be performed to analyze:

Blood cell count
Electrolytes, such as sodium, potassium, and calcium
Blood sugar level
Thyroid hormone levels will also be tested
Liver function test

Treatments:

The goal of treatment in people suffering with thyroid storm is to decrease thyroid hormone levels, or to inhibit their production and release into the blood.  This is accomplished in the following ways:

The administration of PTU and methimazole in high dosages, will inhibit production of thyroid hormone.
Sodium iodide, potassium iodide, or Lugol’s solution can be administered to prevent the secretion of thyroxin. Beta blockers such as propranolol (Inderal, Inderal LA) to control heart rate, and IV steroids to enhance circulation.

Left untreated, thyroid storm treatment can result in:

Coma
Heart failure
Liver failure
Stroke
Death

Treatment also includes:

Administration of IV fluids and electrolytes
Oxygen via nasal cannula or face mask
Medication and cooling towels to control fever
Vitamins and Antibiotics
Iodine to block secretion of thyroid hormone
Kidney dialysis
Sedatives to induce rest
Propranolol and dexamethasone to block hormone synthesis
Antithyroid drugs and potassium iodine

Pollen Allergy and Food Sensitivity: The Oral Allergy Syndrome and Food Intolerance

Oral allergy syndrome (OAS) is a well-recognized but little known condition. It is characterized by a burning sensation or mouth pain and swelling when you eat specific foods that cross react to pollens to which you are allergic. Interestingly, the specific foods causing this reaction are well established to cross react with certain trees, grass, or weed pollens, house dust mite or latex. There are common groups of foods that cluster with certain nasal allergies. For example, ragweed allergy commonly causes an oral or gut reaction after eating melons or bananas but usually not other foods. Birch tree pollen is commonly associated with reactions to numerous foods as is latex allergy. The explanation for these reactions include similarities in protein structures as well as some chemicals in the foods.

Though this reaction is well documented in the allergy literature it is not commonly recognized or diagnosed by most doctors including some allergy specialists and many stomach specialists. Various allergy web sites include lists of the common foods associated with certain pollens, dust mite or latex. However, a comprehensive list that is easy to read or interpret can be difficult to find. Also, the names of some pollens or the common links between a group of pollens and a group of foods can be confusing.

In its classic form OAS should be easy to recognize. After eating a food associated with a pollen to which you are allergic you experience near immediate burning sensation in your mouth or throat with or without swelling. However, it is commonly recognized that frequently in medicine, symptoms do not occur in the “classic” or typical manner in a specific person. Worded another way doctors are taught “patients don’t read the textbooks”. Therefore, you may experience variations of the reaction such as throat swelling or tightening, burning when swallowing, a lump in the throat or a sense of swallowing difficulty but not make the connection to what you ate or what is happening to you.

You or your doctor may misinterpret your symptoms. Frequently people just assume it happened because they were having a choking spell on food that was poorly chewed, swallowed too fast, or eaten or drank while too hot or cold. Commonly, it is assumed that an esophageal (swallowing tube) disorder, especially acid reflux with a hiatal hernia is the cause. Acid reflux can cause an esophageal constriction called a stricture or ring that can result in a food sticking sensation, but this is usually associated with heartburn symptoms or food getting stuck which then prompts an upper endoscopy or scope exam. Other times, especially if occurring in an elderly person, a neurological condition like stroke or Parkinson’s disease is blamed. Sometimes doctors decide that your symptoms are due to a nervous reaction or neurosis that historically was called as globus hystericus. The hystericus portion of the term is usually dropped these days to the shorter term globus or globus sensation especially since it is not proven it is due to a psychiatric problem. However, globus may be the diagnosis arrived at if your complaint is that you feel a lump in your throat and an ‘evaluation” appears to turn up nothing even if OAS was not considered or excluded.

An unusual condition that has been more recently recognized in the field of gastroenterology (diseases of the stomach and intestines) that may be related to or a variant of OAS is called eosinophilic esophagitis (EE) or allergic esophagitis. It was first described in the pediatric population but is now known to occur in adults. Classically described in teenage boys and young men who presented with food sticking episodes without heartburn or acid reflux symptoms, it is associated with a strange appearance of the esophagus on endoscopy (lighted scope exam of the upper gastrointestinal tract). What the doctor doing the scope sees is that the esophagus resembles a cat’s esophagus. That is it looks like it has rings (cats have cartilage rings in their esophagus, we do not) and this is referred to as “ringed esophagus” or felinization of the esophagus. On biopsy of such a ringed or felinized appearing esophagus (that is also often narrowed resulting in food sticking) microscopic signs of allergy are noted. The lining shows numerous eosinophils, a reddish pink appearing white blood cell, characteristic of allergic conditions. These eosinophils release chemicals like histamine that trigger swelling, pain, and damage to tissue.

Food allergies are commonly found to present in EE though sometimes the search for a food allergy by traditional skin tests or IgE blood tests is negative. Treatment is avoidance of known food allergens and swallowed nasal steroids sprays that are designed for use in the nose for nasal allergies. Though not specifically proven yet, eosinophilic esophagitis (EE) may be a variant of OAS.

Eosinophilic gastroenteritis and eosinophilic or allergic colitis also exist and can be diagnosed by biopsies of the stomach, small intestine and colon respectively. Allergic colitis is typically seen in infants who have a cow’s milk protein allergy. It presents as colic type abdominal pain, diarrhea, weight loss and bloody diarrhea in an infant on cow’s milk formula or sometimes in breast-fed infants whose mom is drinking a lot of cow’s milk.

Allergic gastroenteritis occurs in any age group presenting typically as abdominal pain, with or without intestinal blockage or perforation; diarrhea; anemia; weight loss; and microscopic bleeding in the intestinal tract also known as occult blood in the stool. Such bleeding is detectable only by special stool chemical tests known as fecal occult blood tests (FOBT) or stool guaiac testing.

At least some people with food intolerance that does not make sense on limited diet diary information, blood tests, biopsies, or allergy testing, may have a form of OAS. In other words, the presence of known pollen or latex allergies may be predisposing to reactions to foods known to cross react with allergies noted in OAS. However, instead of classic oral allergy syndrome symptoms other stomach and intestinal symptoms or even non-gastrointestinal symptoms may result.

Support of this concept can be found in detailed screening of individuals for food intolerance. Those with known pollen or latex allergies, any known food allergies or intolerance including gluten intolerance (celiac disease) and casein intolerance, are asked to complete a series of symptom assessments and severity rating scales followed by a strict elimination diet. This is followed up by re-assessment of response of symptoms while re-introducing foods one at a time while monitoring for recurrence.

Puppy Dog And Kitten Teething

The teething stage in puppies and kittens can be very trying and uncomfortable for both pet and guardian but there are natural solutions that can help. At around 4 months of age, puppies and kittens lose their first set of teeth and the adult teeth begin to emerge from the gums. The teething process and associated discomfort continues on and off for several months.

As with human babies, the puppy teething process is often painful or, at the very least, uncomfortable for your pet. Teething will be evidenced by an increase in chewing and biting on everything imaginable that your puppy can get its mouth on.

Teething discomfort will often make a puppy cry or show irritability as well as bite and chew on objects, including fingers. Appetite loss and diarrhea may also occur, though these symptoms are not common. While a puppy’s teething can be trying to guardians, the teething process helps the permanent teeth to emerge more easily from the gums. The puppy’s chewing helps to relieve the discomfort of the tooth coming through the gums. Easing the pain will reduce destructive teething behavior.

Eliminating the chewing of a teething puppy is not easily done and is probably unnecessary. However, you can minimize the damage rather than prevent biting and chewing altogether. Never hit, kick, slap or punish your dog no matter the circumstances or your level of frustration; this is the quickest way to erode the dog’s trust in you. (Yes, your pet will still love you; even abused dogs love their owners.)

The best solution is to give your pet some appropriate objects for chewing.

Firm and cold objects are best: some options include a frozen rubber teething toy, ring or a small wet towel that has been wrung out and put in the freezer for a while.

Purchase good things to chew on such as Kong toys or bully sticks. Kongs are made of tough rubber and can be filled with delectable treats such as peanut butter or cheese. Positively reinforce chewing on things your puppy is allowed to have with praise and petting. Negatively reinforce its chewing on inappropriate items by removing the item, saying “No”, and exchanging the object with a proper chew toy while offering positive approval. Do not allow your pet to chew on an old pair of socks or old leather shoe since this will register to your puppy that such items are there for its pleasure to be teething on and destroy.

It is important to use the puppy’s teething period as a training time, to teach your puppy about bite inhibition and limiting mouthing behavior for proper socialization. Dogs must learn to inhibit their bite before they are 4 months old. If at all possible, allow your puppy to socialize with other puppies and already socialized dogs. Puppies need to roll, tumble and play with each other. When they play, they bite each other everywhere. If the teething puppy gets too rough, it will find out quickly because of how the other dogs and puppies react. This happens naturally and is something we humans cannot accomplish nearly as well. The associated lessons are learned from trial and error through the puppy’s own experience. This is where pups learn to inhibit their biting and to control themselves. In packs of dogs, the mother or other dogs would teach this to the pups, but in a home situation, it is up to you.

Obviously, some household objects may prove a danger to your pet if chewed on, such as cables and cords. Keep all electrical wiring out of your teething puppy’s reach, coiled up, packed away or taped with heavy duty insulating tape. Also ensure that household chemicals are stored out of reach as you would for any baby.

Crate training will have the added benefit of protecting your puppy. Crating your puppy will keep it away from the deadly things he’d love to teeth on and chew to pieces when you’re not looking. Make the crate a safe and happy place for your dog by providing stimulating toys while it’s in the crate. The crate has the added benefit of aiding in potty training.

Teething is a stage all puppies and kittens must go through and it will pass fairly soon. However relieving your puppy’s pain or discomfort during the teething stage should be a priority, as well as protecting your possessions from the chewing compulsion.

What Is the History of Heart Disease?

The History of Heart Disease is traced to many factors and Coronary Artery Diseases such as Atherosclerosis and Angina Pectoris. These two conditions specifically focus on these two conditions. It includes the definition of Coronary Arteries and its functions. These circumstances also include variety of signs and symptoms a person may exhibit during the course of the disease; the risk factors of having the condition; types of angina pectoris; and gerontologic concerns about these disorders.

The coronary arteries are the lifelines of every heart in a human body. These arteries supply blood which contains oxygen and nutrients. It consists of two main arteries which are termed as the left and right coronary arteries. The major arteries subdivide into supplementary or minor arteries that provide blood to the different structures of the heart. There would be an ischemia or blood deprivation in the heart muscles because of this occlusion.

Majority of the artery occlusion are associated with coronary atherosclerosis. Atherosclerosis is the unusual collection of fibrous membranes and lipid materials or fatty substances within the linings of arterial blood vessels. This collection of materials can create an occlusion that tapers down the size of the coronary arteries’ lumen. The decrease in size of the artery will result to the reduction of blood supply to the heart. Atherosclerosis can start from a simple atheroma then becomes a thrombus. This thrombus can dislodge from the artery wall and travels within the bloodstream to other parts of the body. Atherosclerosis can originate from the arteries of other parts of the body that may become an embolus which can get to the heart’s arteries.

The existence of an obstruction within the artery will give rise to certain manifestations. The Signs of Heart Disease can come about such as Angina Pectoris. This condition is regarded as a syndrome, an indication of an impeding heart attack. Angina Pectoris is characterized by chest pressure or pain. The quality of pain is described as crushing pain within the anterior portion of the chest. This syndrome may come along with other manifestations like lack of sensation in the arms and legs; arm, shoulder and leg pain; tachypnea and tachycardia.

These symptoms take place when the patient is experiencing emotional anxiety and physical force which can result to the increase in oxygen demand of the body.   • Hold your right hand up and curl your index and middle fingers towards your palm. Place your thumb next to your right nostril. Close the left nostril by pressing gently against it with your ring finger and inhale through the right nostril. The breath should be slow, steady and full.
  • Now close the right nostril by pressing gently against it with your thumb, and open your left nostril by relaxing your ring finger and exhale fully with a slow and steady breath.
  • Inhale though the left nostril, close it, and then exhale through the right nostril.
