I know that you are sorting a lot of things out and may have conflicting feelings. But I was glad to hear you hear you say that it’s not over. Because it’s not over for me either. Maybe if we are patient with one another and move slowly, we will begin to see come progress and things will continue to improve and become more clear. In the meantime, I hope that we can continue to see each other with the kids and present a united front for them.”

In this way, you aren’t putting any pressure on him and you are reassuring him that he’s not the only one with conflicting emotions. Preventing Kidney Stones After Kidney Stone Surgery

It is a well-documented fact that even after surgical removal of kidney stones, up to 50% of patients may have recurrence within 5 years.Therefore, some preventive measures must be taken in order to avoid kidney stones in the future.
In the longer term, a daily urine output of at least 2 liters is advisable in all patients with stone disease. Fluid intake should therefore be about 3 liters per day—more if the climate or the patient’s occupation causes much sweating. Suitable measures should be implied to correct any known cause of stone formation. Preparations containing vitamin D must be avoided, even milk intake has to be reduced.
Idiopathic hypercalciuria can be helped by reduction of sodium intake (low salt intake) and by the use of a thiazide diuretic. Reduction of calcium intake is not recommended unless it is very high, as it may lead to a negative overall calcium balance and reduction of bone mass which ultimately makes bones weaker, also reduction of calcium intake causes increased oxalate absorption and excretion.
Citrate excretion can be increased by daily administration of potassium nitrate or potassium bicarbonate. Alternatively, lemon juice may be a very good dietary source. Hypokalemia should be prevented as it leads to a reduction in citrate excretion.
In patients with recurrent oxalate stones, foods and some liquids rich in this salt, such as rhubarb, spinach, tomatoes, strawberries, chocolate and tea, should be avoided. Persons who have passed several uric acid or urate stones benefit from allopurinol, 100-300 mg daily, depending on renal function. Allopurinol also has a place in treating calcium oxalate stone disease, since urates may contribute by acting as a nidus (nest) for stone formation.
Phosphate-containing calculi are formed only in alkaline urine; hence acidifying the urine by giving ammonium chloride may be effective. In contrast, cystine and urate stones may be prevented or sometimes dissolved, by giving sufficient sodium bicarbonate to make the urine persistently alkaline, and ensuring a high urine output of 2-4 liters/day. When these measures fail or are unacceptable to the patient, treatment with D-penicillamine, a chelating agent, in a dose of 1-1.5 gram daily may be tried, although it is frequently associated with significant side effects.
In case of struvite stones, which are actually magnesium-ammonium-phosphate stones and quite common in women with recurrent urinary tract infections, protease inhibitors are advised. Frequently, struvite stones are large staghorn calculi, and urine pH is high. These stones are formed due to infection with urease producing organisms such as Proteus mirabilis, Pseudomonas, Klebsiella, Staphylococcus and Mycoplasma but not E. coli. Therefore, struvite stones are also called as Infection stones.

1, High levels of hCG (human chorionic gonadotropin). Levels of this pregnancy hormone rise quickly during the early stages of pregnancy and may trigger the part of the brain that controls nausea and vomiting.

2, Increased estrogen levels. This hormone also affects the part of the brain that controls nausea and vomiting.

3, Helicobacter pylori. A research study in Obstetrics &Gynecology reported that 90% of pregnant women with hyperemesis gravidarum are also infected with this bacterium, which may sometimes cause stomach ulcers.

4, Thyroid imbalances

5, Vitamin B deficiency

6, Gastrointestinal changes. During your entire pregnancy the digestive organs displace to make room for the growing fetus. This may result in acid reflux and the stomach emptying more slowly, which can cause nausea and vomiting.

7, Psychological factors. Stress can often make you physically ill. The anxiety that can accompany a pregnancy may trigger acute morning sickness.

8, High-fat diet. Recent research shows that women with a high-fat diet are at a much greater risk for developing hyperemesis gravidarum. Their risk increases five times for every additional 15 grams of saturated fat (such as a quarter-pound cheeseburger) they eat each day.
People who are at the risk of HG

Those with the following characteristics are at a higher risk of HG:
1, being obese

2, being of young maternal age

3, carrying twins

4, having previous hyperemesis gravidarum
How to recognize HG
The symptoms of HG typically begin around the sixth week of pregnancy. Usually, symptoms disappear around the 20th week, however, in about 5% of women, symptoms can continue throughout the third trimester.

The most common symptoms of HG are:

1, Persistent, excessive vomiting or the urge to vomit. Vomiting is considered excessive if it occurs more than three or four times per day and prevents you from keeping down any food or fluids for a period of 24 hours.

2, uncontrollable vomiting

3, vomiting blood or bile

4, vomiting after eating or drinking

5, vomiting not related to eating

6, severe weight loss (at least 5% of pre-pregnancy weight) over a 1-2 week period.

7, increased heart rate

8, shivering

9, extreme fatigue

10 increased salivation

11 dehydration

12 nutritional deficiencies

13 Lightheadedness or fainting.

14 Infrequent urination.

15 Pale skin.

16 Jaundice.
How HG can affect you and your baby
HG is a serious illness that can pose potential health complications. If left untreated, hyperemesis gravidarum can lead to continued weight loss and malnutrition, which may become life threatening. The illness also causes extremely poor quality of life. If vomiting continues, there is a risk of developing liver or stomach complications. The disease is also associated with an increased risk of gallbladder disease.

Though serious, hyperemesis gravidarum does not appear to pose any severe complications for your baby. If treated immediately, most babies are born completely healthy. However, if treatment is not pursued, there is an increased risk of going into preterm labor or having a baby with a low birth weight. Certain drug treatments given to mothers with hyperemesis gravidarum have been known to cause defects in baby.
Treatment for HG
Treatment for HG is to reduce nausea and vomiting,replace fluids and electrolytes and improve nutrition and weight gain.

It is important to contact your health care provider if you are experiencing severe nausea and vomiting. If properly treated, there should be no serious complications for you or your baby. Your health care provider can tell you whether your case is mild or severe. If it’s mild, you should change your diet by eating more protein and complex carbohydrates, such as nuts, cheese and crackers, and milk. It’s best to eat these foods in small portions, several times throughout the day. You should also avoid eating fatty foods, drink plenty of water, and get as much rest as possible. (For more suggestions, see our morning sickness article). Your doctor may also recommend taking antacids and an antiemetic (anti-vomiting) medication.
Medications your doctor may prescribe for severe cases include:
1, Antihistamines, which help ease nausea and motion sickness.

2, Vitamin B6, which helps ease nausea (if you’re unable to take it orally your doctor can give you an injection).

3, Phenothiazine, which helps ease nausea and vomiting.

4, Metoclopramide, which helps increase the rate that the stomach moves food into the intestines.

5, Antacids, which can absorb stomach acid and help prevent acid reflux.

6, Corticosteroids (such as Medrol)
Severe cases of hyperemesis gravidarum require hospitalization. Once there, you may receive intravenous fluids, glucose, electrolytes, and, occasionally, vitamins and other nutritional supplements. Your vitamin levels may also be monitored since women suffering from hyperemesis gravidarum are often deficient in thiamine, riboflavin, vitamin B6, vitamin A, and retinol-binding proteins. Remember, pregnant women need to maintain a much higher level of calories, protein, iron, and folate than nonpregnant women. Your health care provider will talk to you about the sufficient levels and how to maintain them.
Anti-nausea drugs and sedatives may be given, and you will be encouraged to rest. After receiving intravenous (IV) fluids for 24 to 48 hours, you may be ready to eat a clear liquid diet and then move on to eating several small meals a day. You will be monitored by your health care provider after you leave the hospital, and be readmitted if problems continue or recur.
Some women opt to use natural treatment methods to handle side effects of the illness. Acupuncture, acupressure, and hypnosis have proven to be effective treatments, and produce no side effects in baby.

Hyperemesis is the second leading cause of hospitalization in pregnancy, second only to preterm labor. Additionally, many women experience significant psychosocial morbidity, occasionally interfering with assumption of the maternal role and rarely leading to termination of the pregnancy. Women with hyperemesis gravidarum are often encouraged to work with a counselor since emotional problems may not only contribute to this condition, but may result from it as well.

All About Vaccinations In The Immigration Medical Examination

Most green card applicants find that the Immigration Medical Examination can be one of the most excruciating part of the entire journey of immigration. But with the right guidance one can go through it without much of a problem.

Vaccinations are an important part of the Immigration Medical Examination.


An amendment to the Immigration and Nationality Act in September 1996 requires that any person who is in the process of seeking an immigrant visa to show evidence that he has received vaccinations against diseases that are preventable by vaccines.


The Secretary of Health and Human Services is the body that regulates the Immigration Medical Examination process.


As recommended by the U S Advisory Committee on Immunization Practices green card applicants are required to take vaccines. The immigrant’s vaccine history, medical history and age will determine which of the vaccines need to be administered.

Payment for the vaccinations depends upon the panel physician or Civil Surgeon. Some Civil Surgeons include the cost of some standard vaccinations in the total cost of the Immigration Medical Examination there are some doctors who charge for every extra vaccination in addition to the cost of the Immigration Medical Examination. It is advised that the applicant for the Exam checks up with the Civil Surgeon before he or she shows up for the appointment.


If the applicant has already been vaccinated before for certain diseases and wants to avoid being re-vaccinated then the applicant needs to bring in a personal vaccination record or copy of the medical record with entries that have been made by a physician or appropriate medical personnel to the Immigration Medical Examination.

Before the applicant appears for the Exam the applicant should ask the family doctor for a copy of the immunization record.


It is not required for visa applicants to have received all doses of every vaccine before they leave their country for the United States. What is important is that they should have received at least one dose of the recommended vaccine and as many as possible before they leave the country. Once they arrive in the United States they can finish the series.

What To Do If You Have IBS And Diarrhoea?

One symptom of bowel dysfunction is diarrhoea. Diarrhoea is when food passes through and out of the bowel very quickly.

When does diarrhoea occur? Diarrhoea is usually a reaction to food poisoning. You have eaten food which is full of bugs; the body’s defence mechanism is engaged and the food is expelled from the body as fast as possible.

Diarrhoea may also occur when you are tense or scared. Ancient man was born with a flight or fight response. What this means is that when ancient man faced dangers that could kill him, the body produced adrenaline which either helped man run away or have the courage to face the danger. Humans today still have this defence response. These days it is stress, pace of life and the like that causes us to produce adrenaline rather than facing Woolly Mammoths. The effect of adrenaline on the body is to speed up bodily functions including the gut; remember exam time or your driving test? You may also notice that your breathing is quicker, that you can’t relax as easily as you used to or that you’re more rushed to fit everything in. If your bowel is reacting to stress then you need more than a change in diet. You would need to relax more. This could mean taking up yoga, meditation, hypnosis or a more physical activity such as running.

If your stool is frequent and loose then it might be an idea to take a break from food and just drink fluids [not tea or coffee] for a day. It is thought that over cooking white or brown rice until it is a pulp is also easy on the digestion. Rice is a very mild food on the bowel, which is why you will find that on any exclusion diet, rice is still allowed. Linseed is also useful in reducing diarrhoea as it acts as sponge by absorbing excess fluid in the bowel.

Turning to medicines should be a last resort. Medicines have their place in the treatment of illnesses but an eastern philosophy is useful to adopt here which is cure the cause not the symptom. If you are suffering diarrhoea, then your body is telling you that it is using diarrhoea to heal itself. Taking medicines now, like Imodium, may block the healing process. In addition it is possible that by taking a drug that prevents diarrhoea you end up with constipation.

If you suffer alternating bouts of constipation and diarrhoea then seek a cure for the constipation first as the diarrhoea maybe a by product of constipation. The lining of a constipated bowel maybe contaminated with toxins such that the body uses watery stools to flush the toxins away.

If your bowel symptoms persist, you must see your medical doctor. Do not self diagnose as your pain may be a sign something more dangerous.

It’s funny, I found this out completely by accident. When a panic attack or that pretzel anxiety would hit I would strike back with a tall glass of cool water. The dizziness and tension in my neck would usually go away within minutes.

Exercise Isn’t For Jocks

I was never athletic. I was one of the girls who did the minimal workouts in high school. When my Doctor suggested that it might help my anxiety I had to test it out. She was right! What should I do if I have the symptoms?

Most people will have multiple symptoms.  If you experience these symptoms, contact your doctor and have a thyroid test done.  A key step is to determine if too high or low a Thyroid Stimulating Hormone (TSH) level or a lack of T3 hormone is preventing you from feeling well.  You may lack the ability to convert T4 to active T3.  So, be sure that they not only test the TSH but also the T3 and T4 levels.

So, your first step for you is knowing your TSH (thyroid stimulating hormone) level, and other key thyroid levels such as T4 (Levothyroxine) and T3 (Liothyronine).  These levels allow you to help gauge where your doctor is in terms of treatment, and give you a common point of discussion.  More doctors are beginning to believe that a TSH of around 1 – 2 (in the low end of the normal range) is optimal for most people to feel well and avoid having hypothyroid or hyperthyroid symptoms.  Some people feel terrible at 4-5, and at .2, but pretty good at 1 – 2.


For some people, you can still have hypothyroidism at a cellular level with the symptoms although your TSH level may be normal.  This may be due to conversion problems, inadequate T3 hormone, or other factors.


They will probably prescribe medication.  Many times they will prescribe Synthroid, Levoxyl, Levothyroid, and Unithroid medications which only deals with the T4.

Request your doctor to provide you with a prescription that is a combination of T3 AND T4


Request that your doctor has the pharmacist to make up a T3/T4 combo as per the doctor’s specs.

What can I do to help myself?

There are different things that you can do to help yourself.  I have experienced issues with my Levoxyl not working.  Although my TSH levels were normal and fluctuated, I still had a major amount of the systems which the doctor failed to related to the hypothyroid and I just didn’t feel well.
