Clinical studies have confirmed that people who lack strong social support systems are likely to die from heart and cerebral vascular diseases as well as cancers. Therefore, to increase the resilience of individuals toward all potential forms of stress, while reducing any genetic and environmental dangers that can lead to depression, there is the need to foster strong social support structures among individuals, and within families and societies.

Family heads must hold regular and compulsory meetings with their family members to discuss pertinent problems of individuals at the family level. Parents must make time for their children. Parents must frequent conversations and visitations with them especially in situations where they are physically isolated from their children due to occupational and educational challenges. They must utilise modern communication technologies to keep in touch with their children while showering the needed love, care and affection they need at all times. Individuals must always widen out and learn to make friends while sustaining those friendships through regular contacts.

Also, social institutions like educational institutions and religious organisations must establish outfits for offering counselling and guidance to depressed persons. They must not sit in wait for these ones but must regularly reach out for them through constant sensitization programmes. All in the society must be each other’s keepers as it has been the culture of many indigenous communities. This will make it easier for each one to offer information support to depressed ones, even convincing them of the need to visit counselling and guidance units when they are burdened with the challenges of life. The collective hands of every society member, evident in the culture of communal living and social compactness of indigenous societies can aid in remedying depression cases and its associated sting of mortality cases, typical amongst them is suicide.

Treating Mental Health and Forensic Populations

No longer is it possible to assess and/or treat a mental health population without also interfacing with forensic issues such as legal infractions, Courts, violence, sexual behavior problems, delinquency, crime, Not Guilty by Reason of Insanity, substance abuse, and others. The training and approaches to the mental health population is different than that for a forensic population. So what is to be done, if a person has both issues? How Are the Populations Different

A Mental Health population is comprised primarily Axis I disorders, such as Bipolar Disorder, Schizophrenia, Major Depression, PTSD, and Anxiety Disorders. Daily functioning is on a continuum. Recovery is quick for some and slow for others and is also on a continuum. Well controlled intermittent, mild to moderate episodes of a mood or anxiety disorder will not necessarily interfere with daily functioning. Someone with severe, chronic Schizophrenia or Mood Disorder requiring periodic hospitalizations and extensive community support, will have impairment in daily functioning. Goals for these folks are often pro-social and involve being an active member of society. A therapist can be fairly sure that the mental health client without forensic issues will be relatively honest in his or her interactions and the therapist can take most of what he/she says at face value. An emphasis on a strengths model works well when no personality disorder is involved.

A forensic population can be defined as having personality disorders, interpersonal difficulties, behavioral problems, multiple problems and life long courses of various levels of dysfunction or difficulty. Again, this population fills the full spectrum of effective daily functioning. However, social functioning is often the most severe impairment. There are issues of trust, appropriate relationships, ego centrism, moral development, honesty, manipulation, and danger to self and others. They often have a negative view of themselves and others, especially authority figures. Moral development is often delayed leaving them at the egocentric stage of development. This means that what serves the self is what matters and empathy for others and the ability to have an honest relationship with another person may not yet have developed. Their goals are often self-serving.

The capacity to understand the importance of the best interest of the group through laws and rules that we voluntarily follow, may not be well understood. Many, if not most, have histories of childhood abuse, neglect, or exposure to domestic violence. The assessment and interventions with this population is necessarily different that those for a people with no Axis II disorder or trait. The people with forensic issues do not always tell the truth because of their lack of trust in relationships. The therapist cannot take what he/she says at face value. The therapist must separate the sincere from the manipulative moves for self-gain. The internal boundaries are such that they need the therapist to put external boundaries into place for them. Information must be checked with other sources of information.

How Assessment Tools Differ

In a mental health population, assessment can quite effectively be done through instruments such as the MMPI-A, BASC, and MACI. These self-report tools are quite sufficient for this population and will elucidate psychological dynamics and mental illness, if present. Self-report is not as much of an issue as it is in the forensic population, where third party verification is more important. However when a youth has multiple problems, both mental health and forensic, a combination of tools is preferred.

Forensic evaluation tools rely less on self-report because of the trust issues and because it is not always in the client’s best interest to be completely truthful. Self-report assessment instruments can be used, but third party and official reports should also be used in the evaluation phase of a forensic assessment. Courts are concerned with public safety, therefore, the need for tools that assess future risk of dangerousness to others. Risk of future aggression and sexual behavior problems that have been derived from statistical models (actuarial tools) should be part of the evaluation since clinical assessment of risk of future dangerousness is only a little better than chance. While risk assessments are not perfect, they are better than clinical judgment in this area.

How are Interventions Different?

Major Mental Illnesses, while often chronic, can often be very effectively treated with medication and therapy. At the higher functioning end of the continuum, therapy can be supportive, psychotherapeutic, family, or cognitive behavioral. Therapists are trained to accept what the client presents and start where the client is functioning and how the client sees the world. The clients are usually self-motivated and seek therapy voluntarily. They accept responsibility for their behaviors and for making changes in their lives. Use of a strengths model is often very effective. Many people recover fully and lead quite “normal,” non-disrupted lives. When someone is on the lower end of the continuum, with major disruption in every day functioning (work and family),despite medication and therapy, major supports for housing, jobs, and activities of daily living and medication are needed for a very long time, perhaps a life time. However, their life goals are often still pro-social. Serlf-directed care works well with the mental health population without Axis II diagnoses.

In the area of intervention, different approaches are needed for the forensic population. Some level of social and family dysfunction is generally intergenerational and lifelong. These clients are often Court ordered to an assessment or therapy or they are having significant problems at work or within the family causing others to seek assessment or therapy for them. They do not always accept responsibility for their actions or for changing. There are skill deficits that need to be addressed, such as social skills, anger management, and problem solving. You cannot take what these clients say at face value. Third party information is always needed. This is because you need to trust someone in order to be honest with them and most of these folks have been abused, neglected, or exposed to domestic violence and a suspicious arm’s length treatment of others is a coping strategy that is difficult to give up.

This population often has multiple problems so that Multi-systemic Therapy that approaches many areas that need to be addressed is often effective (treating the whole person). Group work and trauma therapies are also good tools. Self-directed therapy may not be effective because of the need to protect oneself from what may appear to be an unsafe world. Nurturing, setting good boundaries, and structure are essential in this work. Motivational interviewing and stages of change can be very helpful. When clients have issues in the mental health and forensic arenas, both approaches must be used to the extent possible.


Clients in a mental health setting range from the single diagnosis of a major mental Illness to the dual diagnosis of a major mental illness and a personality disorder and/or forensic/legal issue. The approaches to these dissimilar populations is unique when clients are dually diagnosed, both approaches are needed. Assessments and treatment for a mental health population can be self-directed and strengths based.

However, the approach for the forensic population cannot be self-directed because the client’s goals are often antisocial and by definition counter to the best interests of society. The therapist or evaluator cannot accept everything the client says at face value because not being honest is part of the disorder that the therapist is treating. Motivational interviewing seems to blend the views of traditional mental health and forensics in a way that is beneficial for the client and society.

Understanding Messiness and Hoarding

Some people are extremely neat, while others tend to accumulate a bit of clutter. Some people live in filth and don’t seem to notice. Then there’s the hoarder, whose possessions pile up until their home is a fire and health hazard.

What goes on in the minds of very messy people? I think that they can be separated into are two types: those who are disorganized and those who have psychological disorders. The former group includes individuals who have problems with keeping things tidy.

They might have some spacial organization issues and just don’t know how to deal with all the papers and objects that make up their home life. They become overwhelmed by it all, and as they give up in despair, the piles begin to accumulate.

These people know that they have a problem but can’t figure out the solution on their own. What they need is a lot of support and some simple systems to fall back on. Organizational consultants are people who are skilled at finding the proper place to put everything and can help those who suffer from clutter and disorganization to have a tidier, less chaotic home life.

When even this doesn’t help, it’s because the person’s problems are more severe – perhaps they suffer from ADD – attention-deficit disorder – and simply can’t cope with trying to keep all their stuff organized. These individuals need a lot more support, perhaps even medication, in order to manage all their papers and possessions.

A more severe form of messiness pertains to those people who don’t clean. We’ve all seen them on the reality shows about dirty homes. These are the people who never change their bed sheets or the kitchen sponge; who rarely if ever empty the cat litter, dust, sweep, mop or even scrub a surface. Their kitchen and bathroom are petri dishes growing pestilence and plagues, and yet they persist in their ways.

Chronic non-cleaners are living in an unpleasant, smelly and unhealthy environment, but don’t seem overly upset by this, which is in itself, a sign of a serious problem. Many of these individuals have a mental disorder which allows them to create mess and then live in it without concern. They may be able to function adequately in other areas of their life, but their psychological problems are demonstrated by the literal dirty secret of their filthy home.

A milder form of this problem is those individuals who let their dishes pile up in their sink over a week, don’t do their laundry for a month, sweep their floors only occasionally and rarely if ever dust. They wouldn’t qualify for the TV shows but the level of mess and dirt in their homes is unacceptable to a normally neat and tidy person.

These folks suffer from low self-esteem, passivity and inertia. They are overwhelmed by life and feel helpless about having any control over things. Basically, they have given up on themselves and their messiness is just one sign of the problem. They could benefit from supportive psychotherapy.

Finally, there are the hoarders. These people have an extreme disorder. Their overwhelming anxiety and internal chaos is expressed through the need to accumulate as much stuff as possible and the inability to throw anything away, be it old clothes, wrapping papers, newspapers or even their garbage.

When I was in pre-med, I ended up sharing a house with a 27 year-old woman, let’s call her Jenny, who had a form of this problem. She was, on the surface, an attractive, well-groomed young woman from a nice, middle-class family. It was only in living with her that her problem was revealed. The first clue was that she locked her bedroom door and hid the key.

The one time I did get to see her room, I was shocked. There was so much stuff heaped up on the floor that I had to wade through it all to get to the other side of the room. She’d invited me in only because she was in a panic: she’d lost something in the two-foot-high piles and needed my help in finding it.

Every week Jenny would go grocery shopping, and would come home with enough food to feed a family of six. She was one small person, and yet she’d buy a dozen grapefruit, ten pounds of potatoes, two quarts of milk and three loaves of bread for her own consumption. Each night she’d cook herself a big dinner, and then dutifully put the left-overs into a plastic container which she never looked at again.

I’d go through the fridge and pantry each week, throwing out squishy grapefruit, rancid left-overs (container and all), potatoes with long green sprouts, curdled milk and moldy bread. I wondered at the time if she just missed her family, but I realized later on that she just had to accumulate things. This was further demonstrated by her compulsive shopping; the evidence of which lay piled on her bedroom floor in the form of bags, scarves, belts, sweaters, jewelery and assorted shopping bags.

Jenny had filled the room across the hall with the overflow from her bedroom. One day I came home to find her sitting in the hallway, surrounded by bags and boxes and piles of stuff. She’d emptied out the room, hoping to sort through years worth of possessions and throw out as much as she could. She sat there, paralyzed, for several hours and eventually gave up and put it all back into the spare room.

At the time, I thought that she was just odd. She was kind of uptight and had some strange habits, like piling all the cutlery into the utensils drawer without sorting the various forks, etc. into each slot of the tray. I didn’t realize that her problem had a name. It’s actually a form of OCD, Obsessive-Compulsive Disorder. There are many manifestations, and compulsive hoarding is a particularly challenging one.

Jenny began dating Harold, and chose to hide her problem from him. On the few occasions he came by to visit, they stayed in the living room. Over the course of their entire relationship, Harold never once got to see her bedroom. I wondered at the time what it must be like to be intimate with someone and keep such a big secret from them.

I moved out at the end of the year, and never saw Jenny again. We got along OK as room-mates but her problem made it impossible for us to be close. I thought about her from time to time, and once I was a psychiatry resident, realized how instructive it had be face-to-face with someone who was described in my textbooks. There’s nothing like seeing it first-hand to recognize how troubled these people are. And it turns out that hers was a mild case. More severe sufferers can’t contain the clutter, and their lives are taken over by the problem.

It’s clear that except for those who are organizationally impaired, individuals who live with extreme messiness or hoarding are actually exhibiting signs of a significant mental disorder. Unless these problems are recognized for what they are and are dealt with by skilled mental health professionals, the people who live in extremely disorganized, cluttered or dirty environments will stand no chance of making any meaningful changes toward cleanliness and order.

The Treatment And Cure

For type 1 diabetes:

Daily injections of insulin are necessary. One to four daily injections are required to control blood glucose levels. Long acting and rapid acting insulin preparations are available; a combination of the two kinds is often prescribed. A strict diet and schedule of meals are necessary to control blood glucose levels. Your doctor may recommend a diet low in fat, salt, and cholestrol, and May advice you to see a nutritionist for dietary planning. Because both exercise and insulin lower glucose levels. Exercise and insulin injections must be timed so that they do not combine to cause a dangerous drop in blood sugar (hypoglycemia). Strict adherence to the timetable of injections, meals, and exercise is necessary for proper management of the disease.

For type 2 diabetes:

A low diet in fat and other calories, in addition to regular exercise, is necessary to control weight. Oral hypoglycemic drugs, such as tolbutamide, chlorpropamide, tolazamide, acetohexamide, glyburide, glipizide, glimepiride, repaglinide, or meglitonide may be prescribed to increase insulin production by the pancreas, if exercise and diet do not lower glucose levels sufficiently. Other oral agents can reduce insulin resistance (metformin, pioglitazone, and rosiglitazone) or slow the absorption of sugars from the intestine (acarbose and miglitol). Insulin injections may be necessary in more severe cases of type 2 diabetes or, or if a patient with type 2 diabetes contracts an additional illness.

For both types of diabetes:

Blood test to measure glucose levels should be performed as your doctor recommends, one to four times a day. Your doctor will recommend a blood-monitoring device to use at home. Careful attention must be paid to the risk factors for atherosclerosis because of its increased occurrence with diabetes. Those suffering from diabetes should not smoke, should reduce dietary saturated fat, cholesterol, and salt, and should take any medications prescribed for high blood pressure or high cholesterol levels. People with diabetes should drink generous amounts of water when stricken with another illness, such as the flu to replace lost fluid and prevent diabetic coma. When ill, people with type 1 diabetes should test their urine for ketones every four to six hours. People with diabetes should practice good foot care and check their feet every day. Nerve damage from diabetes mellitus reduces sensation in the feet, and small foot problems may turn into major infections. Laser photocoagulation to prevent the rupture of tiny blood vessels in the eye may help to prevent or treat diabetic retinopathy. Most patient with diabetes need an eye examination by an ophthalmologist at least once a year to detect the earliest manifestations of retinopathy. Dialysis, artificial blood filtering process, may be necessary to treat kidney failure. In advanced cases a kidney transplant may be advised. Amitriptyline, desipramine, or nortriptyline, medications usually used to treat depression, or the anti seizure medication gabapentin may be prescribed to relieve the pain in the limbs. Excellent control of blood glucose levels delays or prevents late complications affecting the eyes, kidneys, and nerves. Kidney damage can be slowed by controlling blood pressure and using ACE inhibitors. The American diabetes association can provide information about support groups in your area.

The more protected we are as an individual the more we can pursue what is written in the second sentence of the declaration of independence. This would cause chaos and oppression for those that may not fall into the correct class or ethnicity. These rights allow you to become whomever you want without fear or reproach. These rights have made the United States a beacon to the rest of the world.

2. I Don’t Talk Like You Do

The next time you read a newspaper, examine the difference between the headlines and fine print. You’ll quickly discover that headlines are big, bold, and concise while the fine print is smaller, more detailed and extensive. The headlines and fine print in a newspaper offer a great example of how most men and women communicate. Men communicate in headlines, while women communicate in fine print. Men will say: MY DAY WAS GOOD, THE DINNER WAS DELICIOUS, YOUR DRESS IS NICE. Women will go into great detail about the intricacies of the day, dinner, and dress. Neither the man nor the woman is communicating incorrectly, they’re just communicating differently. The argument erupts when the woman interprets the man’s lack of elaboration as a sign that he’s uninterested in her day, dinner, or dress. This scenario is tragic yet common and it causes men to shut down in exasperation. Noted neuropsychiatrist Dr. Louann Brizendine is an expert on the male and female brain (I will refer to her research again in this article). I am going to disclose a secret about most men that will shock most women. Ready? MEN ARE PREOCCUPIED WITH SEX! Ladies, are you shocked? Hopefully, every woman reading this article is chuckling at the sheer obviousness of that revelation. Women know that men are fixated with sex, but they don’t know to what extent. Not even a full minute goes by before men are delving into sexually-charged thinking. Men are forced to subdue this shocking reality because if most women knew how often men contemplated intercourse, they would think all men are perverted sex-addicts. Men tend to view sex as an existential need while women tend to view it as a periodic treat. When a woman hears the words, ‘I love you,’ her heart skips a beat. She is automatically translated into a sugarplum and licorice world filled with gushy hearts and hyper-colored rainbows. She floats on cloud nine and never even considers the authenticity of the uttered words. Yes, some men use those meaningful words strategically. They know that in so many cases those words are the keys to unlocking female vulnerability. On the other end of the spectrum, there are men that NEVER say I love you. It’s not that they don’t feel the love; they just don’t effectively express it. Maybe they were raised in environments that exploited any signs of weakness. Maybe no one ever communicated love to them so they have a hard time communicating it to anyone else. In situations like these, women should focus on what a man does rather than what he says. Men communicate love in different ways. Fidelity is the loudest way that men say I love you. If you read point #3, you understand that men are constantly thinking about sex. Sometimes their thoughts lead them to find an outlet in illicit areas. The fact that your man has learned to ignore and/or suppress this urge for you is proof that he’s head over heels. 6. The Thing That I’m MOST Afraid of is Failure

When two guys meet, they normally engage in casual banter for only moments until they ask the size-up question- What do you do for a living? This loaded question is designed to covertly establish male dominance among the conversing men. Like a guerilla beating his chest in the jungle, the man with the more prestigious career feels validated when he discovers that he’s the higher income-earner. Sadly, the other man quietly feels inferior and unaccomplished. Although most men would publicly deny this scenario, we all secretly know it’s true. Nothing hurts worse than when a man examines the totality of his life and realizes that he’s grossly underachieved. For men, feeling like a failure is a worse sentence than death. Unfortunately ladies, there are times when you contribute to this feeling. How do you make your man feel like a failure? If any of the following statements sound familiar to you, then you’d better be careful and learn how to approach your man more productively:

• “Why can’t you spend more time with the kids?”

• “Are you ever going to come home on time?”

• “Were you born this way or did you develop into being such a jerk?”

• “Why can’t you simply understand what I’m trying to say to you?”

Are you getting the picture of what not to say? I’m sure you’re wondering what the letter “P” stands for. I promise I will explain after this anecdote. Do you remember what it was like when you learned how to drive? Depending on who taught you, your experience was either highly instructional or highly stressful. A good driver’s training teacher knows that they need to watchfully guide their pupil and avoid yelling profusely when he/she makes a mistake. The bad instructor anxiously looms over their pupil and berates them when they make a false move. The difference between the two instructors boils down to one word…PATIENCE! The good instructor has the patience to gently correct their protégé’s foibles while the bad instructor creates an environment that is not conducive to successful learning and development. The highway of life is replete with potholes and filled with detours at every turn. The ability to carefully maneuver through each pitfall is a skillset that men are honing on a daily basis. Sometimes, men make wrong turns and display the fact that they’ve still got a lot to work on. As the compassionate instructor, your job is to show patience while we blossom into effective road warriors. Give us time to drop the habits. Give us time to develop new ways of thinking. Give us time to evolve into the kings that we know we are. Your patience makes our transition easier and more enjoyable.

8. Your Friends Drive Me Crazy

Quantaneka, LaTremanisha, and all of your other girlfriends with four to five-syllables in their name need to go! Of course I’m only kidding but your man might secretly feel this way. If you are in a committed relationship yet most of your girlfriends are single, chances are your man does feel this way. Because your friends are not with anyone and probably lonely, they attach themselves to you like a leech and presumptuously try to include themselves in all of your affairs. When you get into a spat with your man, it’s your single girlfriend that says, “Girl, if I was you I would leave him alone. Let me list the things that your girlfriends do that drive men up the wall:

• Engages in open conversations with you about other men in front of your man’s face

• Ask to go on a double-date with you and forces your man to spend hours with some guy he doesn’t even know

• Contradicts everything your man says and tells you to ignore all of it

• Tries to offer relationship advice to you even though she’s been single for ages

Honestly, I could go on for days. A man can respect his woman’s friends if they respect him, but the moment she crosses the line he takes a defensive posture and retreats into battle mode. It’s hard to have a peaceful relationship when your man and your friend are more like Martin and Pam than they are like Gina and Tommy.

9. The Secret Behind “Guy Friends”

I have been waiting the entire article to reach this point so excuse me while I go in! In point #8 I talked about trifling girlfriends. Now, I will shift gears and tell you how men really feel about their girlfriend’s so-called “guy friends”. The 21st Century woman is not afraid to admit that she has more “guy friends” than girlfriends. She explains that she gravitates more to men as friends because they offer a lot less drama. Although there is some validity to this claim, they are naïve if they think the men that they befriend do not have ulterior motives. It’s the oldest trick in the book that nearly every guy has played. Meet a woman, and if she’s not interested romantically just stick around as a “friend”. Over time, she will see how caring and attentive the guy is. She will confide in him when she goes through relationship issues. She will tell him her deepest secrets and eventually, she’ll become putty in his hands. While the woman is totally oblivious, like a cheetah stalking its prey, her “guy friend” moves in for the kill. Men are acutely aware of this façade so they distrust another man that tries to play the “friend” role. Men know that all he’s waiting for is an opportunity. Unfortunately, women are quick to label men as insecure, possessive and/or overbearing when they voice their concern about the friendship. Actually, a man can have total and complete trust in his woman and still disapprove of the friendship because he knows that the “guy friend” is an opportunistic charlatan. Frustrations are aroused even more when the man feels like the woman is naively denying this reality. In this homie-lover-friend age of unattached intimacy, conniving men are all too willing to pose as the confidant while plotting as the con-artist. Ladies, trust your man’s instincts and know that in the constant chess match called dating, game recognizes game.

10. There’s A Reason Why I Act Differently Around The Fellas

Male bravado is a powerful thing. When men get together there is palpable electricity that enlivens any room. This fact was clearly evident when the church I previously worked for held a Super bowl party. In previous years, the church excluded women from attending the annual party. It was a men-only event that drew hundreds of guys out of their house and into the church fellowship hall. This particular year, the church decided to make the party co-ed. They issued an announcement inviting women hoping that their inclusive olive branch would draw more people. Unfortunately, that year’s party was a horrible failure. Men refused to go because they felt betrayed. For most of them, the Super bowl party was the one day in the year that they could hoot and holler without being called juvenile. It was their only annual opportunity to embellish on their high school football stories without being called ostentatious. It was their day and the church took it from them. Women often complain that when men get around their friends they become cocky and aloof. Actually, I would add another word to that description- relaxed. When a man is hanging out with his friends, it’s his opportunity to be genuinely and unashamedly male. There is no better example of black male camaraderie than in Greek fraternities. They stand as a shining example that when black men are on one accord, they have no problem stepping in harmony.
