Fractures of the humerus are common and make up about 5% of all fractures, with 80% of them either undisplaced or just minimally displaced. More common in people suffering from osteoporosis, it is common to have a forearm fracture on the same side. Damage to the nerves or circulatory system is possible from these fractures but not often seen. Common areas of fracture are the neck of humerus at the top of the arm(fractured shoulder) and the mid shaft of the arm bone.

A fall onto the outstretched hand, onto the elbow or onto the shoulder itself is the most common cause of a fractured arm. Since many of the arm muscles insert onto the humeral head, when the injury occurs the muscular action involved can displace the fragments and complicate the management. 65 years old is the peak incidence for this kind of fractured humerus and if younger patients suffer this fracture the likely cause will involve high forces such as traffic accidents or sports injury.

If the fracture occurred without significant force then a pathological cause such as cancer must be suspected. On physio examination pain will occur on movement of the shoulder or the elbow, there may be extensive bruising and swelling, the arm may appear short if the fracture is displaced in shaft fractures and there is very restricted shoulder movement. Radial nerve damage is rare in upper humeral fractures but more common in fractures of the shaft, leading to “wrist drop”, weakness of the wrist and finger extensors and some thumb movements.

Shoulder Fracture Management

Initial management is to restrict the patient’s movement and give them enough painkillers to make them comfortable. Upper humeral fractures can be managed conservatively if not displaced but if the greater tuberosity is fractured then an injury to the rotator cuff must be considered, more common in older people, injuries with high forces involved and where there is a lot of displacement. The typical treatment is a collar and cuff sling, allowing the elbow to hang in mid air and keep the humerus in line. Shaft fractures may be managed by humeral bracing.

Fractures with three or four parts plus displacement often need surgical treatment, with open reduction surgical fixation (ORIF) more often required in younger patients. In older people the humeral head may be replaced as the fracture may not heal or give an acceptable pain or movement result. Shaft fractures usually heal without surgery (plating or nailing) and are managed in a functional brace.  Complications include frozen shoulder, avascular necrosis of the humeral head in multi-part fractures and nerve injury in shaft fractures. Six to eight weeks is typical healing time with older people often suffering a permanent reduction in shoulder movement.

Physiotherapy Management of Shoulder Fractures

Initially the physio assesses the arm, asking the patient about their pain level as this varies greatly, examining the swelling and bruising of the arm. The physiotherapist then checks the available range of movement of the shoulder, elbow, forearm and hand. Any muscle weakness and sensory loss is noted as this may denote nerve damage. If not operated on, a sling is continued with and if the fracture is not too painful or severe, early exercises are started by the physiotherapist. Pendular exercises, with the patient bending over at the waist, are important in the early stages as they allow movement of the shoulder joint without much force.

Once the three week stage is reached the fracture will have begun to heal and auto-assisted exercises can be started, where the patient helps the affected arm with the uninjured one. This is progressed to unassisted exercises, moving the arm up above the head, behind the back and behind the neck. As the six week point approaches, when the bone has effectively healed, the physiotherapist will start the patient doing more forceful exercises involving gentle stretches at the end of range to improve mobility. Mobilisation techniques to the shoulder joint and strengthening exercises using Theraband are progressed to increase muscle power and joint range.

Fasting is a scientific method of curing disease, by causing the patient to abstain for a longer or shorter period of time, from all solid and liquid food, with the singular exception of water.

Fasting is as old as the human race. It is an entirely natural method of rejuvenating the body, and is one of more powerful of all remedial measures. It is a therapeutic measure, a curative agent and is quite different from starvation, which deprives the body tissues of nutrients. The effete, poisonous material with the body is the real cause of disease. Regardless of its outward appearance or showing, the various methods of its elimination constitute the various so called “diseases”.

Hygienic fasting is a voluntary act concerned with improvement of the quality of faster’s life. Abstinence is a process instinctive in all animals not spoiled by domesticity, essential to survival when the organism is overloaded with un-discharged toxins. A toxic overload accumulates when; more food is habitually eaten than is required for the body’s need. Food of poor quality imposes a stain on the digestive and assimilative functions of the body, enervating habits reduce the eliminative detoxifying capacities of the liver, kidneys, lungs & skin and excretory efficiency of the bowel, bladder and uterus. To fast is to rest the ‘body physiologically so that energy normally expended in dealing with food is diverted to healing purposes.

It is a fact, however, that the number of red blood corpuscles increases during the early days of a fast, though there may be a slight decrease towards the end, it is protracted, perfectly safe in the therapeutic fast.

The brain and nervous system are profoundly affected by a fast, and it has been frequently observed that the processes of thinking becomes clearer and more facilitated as it progresses, so that long continued thought is possible to a degree which would previously have insured brain fatigue.

Fasting is not a “cure all” in the sense that it will cure every sort of disease known acute and chronic, functional and organic and it is useless to pretend that it will. None of its advocates has ever asserted that it does so. At the same time, it is probable that fasting has a wider range of curative influence than any single measure known to us.

Dr. Rabagliati, an eminent cancer specialist, conducted very strongly in favor of fasting for the early stages of cancer, and this view has been supported by a number of physicians since then. And even in later stages of cancer, this method offers more hope for reduction of the growth and the prolonging of life than any method known.

Dr Haig expressed it “fasting act like a dose of alkali”. If there is acidity in the system, fasting will remove it, and restore the chemical balance of the system.

Short facts for a day or two are very beneficial, both spiritually and physically. Our devout Hindus fast in certain days of the year for short duration not more than two days. I remember during my school days, Ekadashi and Dwadashi fasts, told to me by my classmates of their fathers, mothers, grand parents. Make sure you drink plenty of water and take the deep breathing exercises. To obtain from rich food, and to eat but very little plain food, even for days or weeks, would be very beneficial. The weak, the lean, and the under nourished people should not under take any fast at all. It is better if one resort to fasting under medical supervision, so that no untoward incident may happen. We have cases of some people who were cured of their rheumatism, as they fasted two days in every week, and in a short time of rheumatism gone.

The fasting cure is based upon the fundamental assumption, previously referred to, that mist diseases are very largely curative processes, and what we regard as the disease is, as a rule a mere complex of symptoms or outward manifestations of then underlying cause, which cause is in reality the thing to be removed, that which produces the diseases state. Once we succeed in removing the cause, the symptoms disappear of themselves, without having been repressed or suppressed as they too often under allopathic medical treatment. All nature cure methods are based upon the fundamental truth that nature is the prime curative agent and that any methods of cure should attempt merely to assist nature instead of blocking her efforts. The beautiful idea of hunger cure is that wherever a meal is omitted, the body purifies itself from its disease, and it becomes apparent in the subsequent amendment both as regards bodily feelings and strength.

During a fast the energy which was previously utilized in the digestion of food material is now set at liberty, and may be used to cure the body. Secondly during a fast, useless and dead matter is always first eliminated, leaving the healthy tissues to function normally.

As Dr Dewey puts it “take away food from sick man’s stomach and you begun, not to starve the sick man, but the disease.

There are several medical problems that may arise with fasting, if it carried longer than necessary, shot fasting is not more than 3 days at a time. Yearly a person like this way can fast at least 30 days in a staggering way. Longer fasts may have the following complications such as kidney stones, low blood pressure, irregular heart beat, gouty arthritis, headaches or light headedness, abdominal pain or nausea, decreased urine output, and severe cramps, fasting is also used to treat obesity and certain convulsive disorders.

The length of the fast must be gauge very largely but the condition of the patient and by general conditions. There are certain diseased conditions which seem to call especially for the long fast. Many fast for these conditions have been from thirty to fifty days in duration, though all depends upon nature’s indication for the breaking of the fast, a normal true hunger. Those cases ordinarily requiring the long fast for the most speedily recovery is the deep rooted diseases condition, such as Bright’s disease, diabetes, rheumatism gaut, syphilis arteriosclerosis, and asthma in heavy subjects. Other conditions that have responded better to the long fast than to any other method f conducting the fast are obesity, apoplexy and the paralysis resulting there from, liver congestion, and abscesses, appendicitis both acute and chronic, and peritonitis. Cancer and other forms of malignant growths, as well as benign tumors also respond more fully to the long fast than to the short fast. Typhoid fever, among acute disease, call for the long fast. But if great emaciation and weakness have resulted from the disease, the duration of the fast will be materially shortened. Any of the conditions mentioned in the short fast and the partial fast may in an individual case call for long fast also. There are, on the other hand, many things which might be said in favor of a series of shorter fasts. If the patient is run down and debilitated, by a prolonged illness, if he suffers from tuberculosis or some other wasting diseases; when the body is but slightly indisposed, in these and in many other cases a series of shorter facts would be advised.

Right mental attitude when fasting is important. It is patient believes in the fasting cure, and understands that he is deriving benefits from it, he will not, as a rule experience many of the unpleasant symptoms which invariably manifest themselves when fasting has been undertaken under compulsion. The patient should realize that every minute of the fast is ridding his body just to that extent of the causes of his disease, and that while the period of fasting may not be unpleasant, it is nevertheless actually curing him, and incidentally may be prolonged his life for many years.

As Joseph Goodman an authority on fasting has clearly stated that fasting does not cure a disease. It is often the first step in the direction the body healing itself.

The storage organs such as the stomach and the gall bladder are permitted to empty and so cleanse themselves of obstructive impurities and infective of putrefactive organisms. The physiological chemistry and secretions are normalized. The disease and abnormal accumulations are broken down. As a result of an initial fast and subsequent care in nutrition and other aspects daily living there is usually, a rejuvenation of cells and tissues, a revitalization of bodily energies, improved digestion, absorption and assimilation of food stuffs, normalized weight, increased breathing capacity, and improve clarity of thinking and strengthening of the mind. During fasting drugs should never be taken.

Please break your fast with care, single items should be consumed at first, gradually increasing during the following days. A good rule to follow is, take many days returning to a full diet as has spent in actually fasting. Do not eat in the future if the body is fevered, infected, when in serious injury, or n any acute illness, the body appreciates this.

Understanding the Herpes Virus at the Cell Level

The word "herpes" conjures up different images for different people. Some see herpes as an ugly and annoying cold sore or blister that periodically appears on or around the lips. Others see herpes as a feared sexually transmitted disease that, once you have contracted, you have to suffer with for the rest of your life. Of course both of these images do represent the term "herpes" quite accurately for many people throughout the world, but medically speaking herpes is also appropriately applied to some lesser known afflictions. These conditions include herpes inside the mouth (herpes gingivostomatitis), herpes of the throat (herpes pharyngitis), herpes of the eye (herpes keratitis), herpes of the brain (herpes encephalitis), herpes transmitted to newborn infants (neonatal herpes), chickenpox (Varicella-zoster), mono (mononucleosis) and shingles (herpes-zoster). All of these conditions are caused by one of the two closely related herpes viruses known as herpes simplex virus type 1 and herpes simplex virus type 2.

To truly understand the herpes virus it is helpful to first gain some knowledge about viruses in general. Viruses are the smallest known microbes, or infectious agents, that medical science has discovered to date. Most viruses consist of a nuclear acid surrounded by a protein coat known as a capsid; This nucleic acid-protein complex is referred to as nucleocapsid. In more complex viruses, such as the herpes virus, the nucleocapsid is surrounded by a membrane-like structure containing carbohydrates, lipids and proteins. This membrane-like structure is referred to as an envelope. Each virus contains one of two large complex chemicals that contain the viruses genetic code which serves as a blueprint for making more viruses. This complex chemical code is either RNA (ribonucleic acid) or DNA (deozyribonucleic acid).

Unlike bacteria and more complex organisms, viruses do not carry all the equipment necessary to reproduce themselves. In order to multiply, a virus must enter a living cell, remove the cells protein coat and then use its RNA or DNA to redirect the cells synthesizing mechanism to make more copies of the virus. This process of making new viruses can actually destroy or injure the living or 'host' cell. If enough living host cells are injured or destroyed it results in a viral illness such as influenza (the flu), viral diarrhea or genital herpes. There are hundreds of known viruses and probably thousands of others not yet discovered. Each virus has adapted to infect a particular type of cell in a specific living organism which explains why there are so many types of viruses. Because they have become so specialized some viruses can only infect certain types of cells, for instance, liver cells or muscle cells or brain cells while leaving other cells alone. In general, this usually means that viruses which infect cells in one type of animal, say a dog, can not be passed on to another type of animal, say a cat. The term "herpesvirus" refers to any member of the herpes simplex type 1 and herpes simplex type 2 virus family. The nucleocapsid of a herpes virus is surrounded by an envelope with spike-like structures projecting from the surface and contains DNA. So far, scientist have identified over 115 different herpesviruses and have found more than 50 different animal species that can be infected with some type of herpes virus. Humans appear to be a natural reservoir for at least 8 different types of the herpes viruses that normally spread from human to human and generally do not cause disease in other animals.

Coming in contact with the virus, usually through sexual interference, is the first in a complex series of events that result in contracting genital herpes. Projecting from the outer surface of the herpes virus are protein-carbohydrates structures called glycoproteins. Glycoproteins allow the virus to attach initially to proteoglycans, which are complex chemical structures present on the surface of living cells. After attaching to the host cell, glycopoteins then interact with the cells surface structures to trigger changes in the cell membranes cytoskeletal structure. These changes allow the viral envelope to fuse with the cell plasma membrane, essentially merging to form a changed cell. When this fusion occurs, the nucleocapsid of the herpes virus enters into the cytoplasm of the host cell, attaching tiny skeleton-like structures known as microtubules and microfilaments which form an internal transport network used to move materials within the cell. Using this structure the DNA of the herpes virus enters the nucleus of the host cell where it makes copies of the viral DNA which are released from the cell and spread to and infect other surrounding cells. This process of generating new virus particles kills the infected host cell.

If the herpes virus remains in the skins cells where initial viral attachment occur, chances are that a healthy immune system could possibly control the infection and rid the body of the virus. The herpes virus however has found a way to hide from the immune system by hibernating in nerve cells. For unknown reasons the herpes virus does not start the replicating process in a select number of nerve cells, instead hibernating and establishing a latent infection. When the herpes virus is hybernating it is in an inactive state and can not be detected by the immune system. This inactive state is referred to as latent infection or simply latency. Latency does not cause illness but unfortunately the latent virus can reactivate and produce more virus which, in turn, causes recurrent herpes.

How reactivation occurs is unknown but it can happen for no apparent reason or be triggered by trauma, stress, or exposure to ultraviolet radiation such as too much sunlight. Whatever the trigger, after reactivation, the herpes virus is transported from the nerve cell body to the nerve endings where it is released into the skin to replicate in the skin cells. This replication may sometimes cause full blown herpes sores outbreaks, but other times may result in "shedding" the virus which causes no recognizable symptoms. However, either way, whether this reactivation is symptomatic or asyptomatic, the person is contagious during this reaction period and it is possible to pass on the virus to a partner. For this reason, experts recommend that people with genital herpes use condoms even when there is no obvious symptoms of an active herpes breakout.

There was a big push to go with electronic medical records. Our government placed deadlines on doctors and hospitals as to when this must be in place. They even offered incentive money to get this done. It all sounded good, doctors and hospitals would be able to see your medical records, with this information they would be able to treat your condition better.

Well like most things that the government forces upon us, no matter how well intentioned, it was not thought out very well. I will use myself as an example, over the past few years I have gone to a few doctors and to some hospitals for different tests. During the check in or check out process I was asked if I want to get login information to view my records. On all occasions I said yes. I then logged in, created an account and sure enough my records for that visit were there. One thing that I did notice, the medical record history was not correct, it needed to get corrected and cleaned up.

The problem is that it seemed that every doctor and hospital had different medical software. So I have about three or four different logins to different medical portals. Yes, I was able to see my information from that particular medical provider, but NO they were not connected. So if I logged into doctor A I could see those records, but I could see no records from doctor B.

So the failure, as it now stands, is the ability of the different medical software’s to communicate with each other. While most medical providers are using electronic medical records it appears that nothing much has changed, except the change from paper to electronic records. Going to electronic records could be the beginning of a good thing. It would facilitate integrating all of the different medical software latter. The government should go back and set a date that all medical software should be integrated, then back off and let business take care of it.

I recently got labs done before a doctor’s appointment, I went online and looked at the results. When I went to see me doctor I was better able to ask questions. This is the way medical care should be, a collaboration between you and your medical provider. So there are many good things about electronic medical records it just needs to be fully integrated to work properly.

Wilson Hammer 6 Racket Review

The Wilson H6 has been one of Wilson’s most popular tennis rackets, from anyone just beginning to a few years into play to people who are very experienced. Every club is familiar with the Wilson H6, it has become one of the most popular rackets out there fitting a variety of playing styles. As I was shopping around for a tennis racket for my own personal use, I quickly found that the H6 had nothing but positive feedback.

I have played tennis for eight years now and throughout my career I have always loved playing with the Wilson Hammer (H6). I think it is a great racket to hit with and I own both the mid size and oversize. In match playing I use my mid size racket because I feel that I have more control and it’s a manageable weight for hitting consistently. When I use the Wilson H6 in practice, coaching kids, and matches, I have found that when doing ground strokes it offers decent pop. This means that when the ball is flying at you, you don’t have to kill the ball when you return it to the opponent. The racket gives that extra little push so you can still get the shot, speed, and spin you want. Stronger and more advanced players might take this into advantage because they will be playing longer points out and when moving side to side on the court the racket becomes your friend to step in with that extra little push for a great put away shot.

I usually don’t hit a lot of my shots as backhand slices but when I do, I found that the Wilson H6 will get the job done effectively. Any racket takes time getting used to but once I got the handle on how to slice a hard shot verses a soft shot, I was putting down some very effective shots. When you have the chance to smash the ball from a lob or a great angle put away shot, the mid plus racket gives off amazing topspin.

When I first started playing tennis my serves were my weakness. I used to get frustrated with myself because I felt that something wasn’t going right when I tried to serve. After thousands of serves and serve returns I found that little adjustments with the Wilson H6 can make a big difference in your serving. Being that my serve is one of my weaknesses in the game, this racket is great for a slower swing while serving and giving off the spin and placement you still want. I also love the grip of the racket. It is very comfortable to handle and doesn’t slide out of your hand, even well into play.

Overall I would say that this racket is by far my favorite and I will continue playing with it throughout my career. It is a very good racket to start out with and as I improved my game the racket supplemented me perfectly. The better I got from lessons and playing matches I could see the benefits of have the Wilson H6. The popularity of this racket is not just hype. Overall it is an extremely solid racket that will put good spin on the ball and also give you that extra power for the big smash point. The Wilson Hammer H6 is a solid and effective racket that I would recommend to all!
